Background: The protective role of vitamin D in the occurrence of breast cancer is nowadays a controversial matter. Based on conflicting results of the studies in this field and also considering the prevalence of vitamin D deficiency in Iranian women, this work was conducted to evaluate the association between vitamin D and breast cancer. Method: This matched case-control study was conducted on 70 newly diagnosed breast cancer patients and 70 controls with the same age, menopause status, and time of blood sampling in Zanjan. Information regarding demographic, reproductive, history of diseases, medication, use of dairy products, and sunlight exposure was collected using a questionnaire. The serum level of vitamin D was measured with ELISA method. The data were analyzed utilizing chi-square test, independent t-test, and odds ratios using conditional logistic regression model. Results: The mean level of vitamin D was 39.04 and 63.34 ng/ml in the cases and controls, respectively (P=0.046). The proportion of the cases in the highest quartile of vitamin D was significantly smaller than that in the controls compared with the lowest quartile (Ptrend=0.028). Using conditional logistic regression model, an inverse and independent association was observed between vitamin D and breast cancer after controlling main confounders. The risk of breast cancer was independently associated with body mass index and low income. Conclusion: In this study, an inverse association was confirmed between vitamin D and breast cancer. Prospective intervention studies should be performed to explore its role in the prevention of breast cancer. [ABSTRACT FROM AUTHOR]

Introduction: Semaphorin 3A (Sema 3A) is a secreted protein, which plays an integral part in developing the nervous system. It has collapse activity on the growth cone of dorsal root ganglia. After the development of the nervous system, Sema 3A expression decreases. Neuropilin 1 is a membrane receptor of Sema 3A. When semaphorin binds to neuropilin 1, the recruitment of oligodendrocyte precursor cells to the demyelinated site decreases. In Multiple Sclerosis (MS), Sema 3A expression increases and inhibits oligodendrocyte precursor cell differentiation. Therefore, the remyelination of axons gets impaired. We hypothesized that the function of Sema 3A could be inhibited by neutralizing its binding to membrane NRP1. Methods: we cloned a soluble form of mouse Neuropilin 1 (msNRP1) in a lentiviral vector and expressed the recombinant protein in HEK293T cells. Then, the conditioned medium of the transduced cells was used to evaluate the effects of the msNRP1 on the inhibition of Sema 3A-induced growth cone collapse activity. Dorsal root ganglion explants of timed pregnant (E13) mice were prepared. Then, the growth cone collapse activity of Sema 3A was assessed in the presence and absence of msNRP1-containing conditioned media of transduced and non-transduced HEK293T cells. Comparisons between groups were performed by 1-way ANOVA and post hoc Tukey tests. Results: msNRP1 was successfully cloned and transduced in HEK293T cells. The supernatant of transduced cells was concentrated and evaluated for the production of msNRP1. ELISA results indicated that transduced cells secreted msNRP1. Growth cone collapse assay showed that Sema 3A activity was significantly reduced in the presence of the conditioned medium of msNRP1-transduced HEK293T cells. Conversely, a conditioned medium of non-transduced HEK293T cells could not effectively prevent Sema 3A growth cone collapse activity. Conclusion: Our results indicated that msNRP1 was successfully produced in HEK293T cells. The secreted msNRP1 effectively prevented Sema 3A collapse activity. Therefore, msNRP1 can increase remyelination in MS lesions, although more studies using animal models are required. [ABSTRACT FROM AUTHOR]

The asthma treatment and control might be associated with significant burden on family and community' thus exploring other therapeutic plans could be desirable. The aim of this study was to investigate the effect of salt space on clinical findings and peak expiratory flow rate among children with asthma. In this randomized crossover trial, 34 patients aged 6-14 years old with mild to moderate asthma were selected and randomly divided into two groups. The first group went through a period of salt therapy by staying in the salt room for one hour, three times a week for 3 consecutive weeks and then was under observation for three weeks. This process was reversed for the second group (three weeks under observation followed by salt therapy). The wash-out period was one week. During the study, the morning and evening peak expiratory flow (PEF), the frequency of coughing, wheezing, dyspnea and use of rescue medications were measured. Salt therapy had a significant effect on raising the morning and evening PEF in the second week in both groups (p=0.028 and p=0.032, respectively). However, there was no significant effect on PEF variabilities' cough' wheezing, dyspnea, and the frequency of rescue medication (p>0.05). No side effect was observed during salt therapy. This study showed the significant effect of salt therapy on PEF rate of the patients in the second week. However, further studies with different frequency and time of salt therapy on respiratory disorders are recommended. [ABSTRACT FROM AUTHOR]

Background: Women's satisfaction with childbirth is an important measure of the quality of maternity care services. This study aims to address factors related to women's childbirth satisfaction in physiological and routine childbirth groups. Materials and Methods: This descriptive-analytical study was conducted among 340 women in physiological and routine childbirth groups in 2012. Women were selected through convenience sampling method in the routine group and by census in the physiological group. Data were collected using a 5-part questionnaire composed of demographic and obstetrics details, Mackey's Childbirth Satisfaction Rating Scale (CSRS), satisfied with birth setting, Labor Agentry Scale (LAS), and Visual Analogue Scale (VAS), which was completed by interview 24 hours after childbirth. Data were analyzed using the Statistical Package for the Social Sciences version 18 software using Pearson correlation test, independent t-test, analysis of variance, and linear, multivariate regression model at the significant level of P < 0.05. Results: In both the physiological and routine childbirth groups, satisfaction was found related to the severity of pain (P < 0.05), self-control (P < 0.0001), and birth setting satisfaction (P < 0.0001). In the physiological group, satisfaction was significantly related to previous knowledge of childbirth (P = 0.024), attitude toward the recent pregnancy (P = 0.007), and perceived severity of pain (P = 0.016). However, in the routine group, satisfaction was related only to intentional pregnancy (P = 0.002). In neither group, satisfaction was related to demographic characteristics, maternal parity, and participation in pregnancy and childbirth classes or maternal feelings toward the onset of childbirth (P > 0.05). Conclusions: Improved physical structure and setting of birth room, nonmedical pain relief, mothers' involvement in the process of labor, and sense of being in control are associated with mothers' satisfaction. Objective(s): Diabetic nephropathy is an important long-term complication of diabetes mellitus which appears to be partially mediated by an increase in secretion of transforming growth factor-β (TGF-β). Fibromodulin, the small leucine-rich proteoglycan, has been proposed to be the potent TGFβ1 modulator. In this study, the therapeutic effects of recombinant adenoviral vectors expressing fibromodulin on TGF-β1 expression on diabetic nephropathy were assessed. Materials and Methods: Forty-eight Sprague-Dawley rats were divided into 4 groups: STZ-induced diabetic rats (diabetic-control), fibromodulin adenovirus vector treated STZ rats (Ad- fibromodulin), and Ad-lacZ-treated STZ rats (Ad-lacZ), and vehicle control (PBS-control). At 10 weeks after STZ treatment, we measured urinary albumin excretion (UAE), urine creatinine was measured by Jaffe method.We also measured kidney TGF-β1 levels by reverse transcription polymerase chain reaction and Real-time PCR. Results: Urine albumin to creatinine ratio or UAE level were listed in four groups. UAE difference between healthy and diabetic rats in all three groups were significant (P⩽0.005) and between the control group and treated groups were not significant. Our results indicated that TGF-β1gene expression in diabetic rats were increased and difference between normal group and diabetic group were significant (P⩽0.001). Fibromodulin gene transfection mediated by a recombinant adenovirus decreased TGF-β1 level in STZinduced diabetic rats and TGF-β1 mRNA in diabetic kidney were reduced 2 weeks after Ad-fibromodulin injection. Conclusion: Intraperitoneal injection of adenoviral vectors expressing fibromodulin reduced TGF-β1 level in diabetic rat models. Introduction: Urinary tract infection (UTI) is common in children. UTIs are important in view of the morbidity and risk of scarring. Several factors have been reported to be responsible for progression to scarring. The aim of this study was to determine the incidence of scar and its related factors. Materials and Methods: In this study, 26 males and 77 females (3 months - 12 years) with first pyelonephritis were evaluated. All patients underwent ultrasound, cystourethrography, and Dimercaptosuccinic acid scan. A follow-up scan was performed 6 months later. Age, gender, organism, presence, and grade of vesicoureteral reflux (VUR), delay in treatment, total white blood cell counts (WBC), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) levels on admission were recorded. Logistic regression analysis was used to evaluate the association between the variables and scar. Results: Of 103 patients, 47.6% had VUR. Scar was detected in 38.8%. There were significant associations between delay in treatment (p=0.0001), grade of VUR (p=0.03) and elevated ESR (p= 0.006), CRP (p=0.002) and WBC (p=o.oo5) with scar. No association was established with age, sex, VUR, and organism. On multivariate analysis, delay in treatment was independently associated with scar. Conclusions: We found that the grade of VUR, delay in treatment, and increased ESR, CRP and WBC were important factors related to scar. [ABSTRACT FROM AUTHOR]
