1. Volatile organic compound and microbiome profiling in patients with colorectal cancer, their spouses and first degree relatives

Abstract

Colorectal cancer (CRC) is the one of the commonest causes of cancer and cancer related death worldwide. Its aetiology is linked to a number of reversible and irreversible genetic and environmental factors, including age, sex, genetics, smoking and diet. There has been a drive in recent years for non-invasive biomarkers for many malignant and non-malignant diseases across multiple medical specialties. One of the areas of interest is the detection of volatile organic compounds (VOCs) in various bodily substances by means such as mass spectrometry and electronic noses. CRC patients have been shown to be distinguishable from healthy controls using urinary VOC detection in several studies, including two published by the research group at UHCW and the University of Warwick. There has also been much interest in recent years into the role that the intestinal microbiome plays in health and disease in humans. The aim of this thesis was to characterise the urinary VOC and stool microbiome profiles of CRC patients, their spouses and first degree relatives. The aim being to determine whether the urinary VOC profiles could be distinguished using this technology and to try and better understand the underlying mechanism which lead to CRC carcinogenesis. The first degree relatives and spouses were selected as “common gene pool” and “shared environment” control groups respectively. This work was done using an LC-FAIMS-MS hybrid machine to detect urinary VOCs and 16s RNA sequencing using an Illumina Miseq platform. Comparisons were also made between pre-treatment and post-treatment CRC samples to try and determine if there was any change in either VOC or microbiome profiles after CRC treatment. The urinary VOC profiles of CRC subjects could be distinguished from both sets of healthy controls using a 5-fold cross validation and sparse logistics regression and Random Forrest statistical classifiers, achieving sensitivities of 63-69%, specificities of 64-69% and AUC 0.71-0.72. No statistically significant differences could be found in the urinary VOC profiles of pre-operative and post operative samples. Microbiome analysis revealed over 1300 operational taxonomic units (OTUs), with a similarity of >93% between the CRC samples and the control groups, with significantly different bacterial abundances identified in only 82 OTUs (6.2%), mainly Clostridiales bacteria. Pre-treatment and post-treatment sample analysis revealed differences of 17 (3%) and 22 (4%) OTUs at 3 and 6 months respectively, again principally clostridiales. This thesis provides further data on the microbiome composition in CRC. It also provides further proof of the utility of urinary VOCs, for the first time here using LC-FAIMS-MS technology, a variant of the previously utilised FAIMS technology, as a non-invasive biomarker for CRC.

Abstract

The Daya Bay experiment measures sin^2 2{\theta}_13 using functionally identical antineutrino detectors located at distances of 300 to 2000 meters from the Daya Bay nuclear power complex. Each detector consists of three nested fluid volumes surrounded by photomultiplier tubes. These volumes are coupled to overflow tanks on top of the detector to allow for thermal expansion of the liquid. Antineutrinos are detected through the inverse beta decay reaction on the proton-rich scintillator target. A precise and continuous measurement of the detector's central target mass is achieved by monitoring the the fluid level in the overflow tanks with cameras and ultrasonic and capacitive sensors. In addition, the monitoring system records detector temperature and levelness at multiple positions. This monitoring information allows the precise determination of the detectors' effective number of target protons during data taking. Abstract

Two indicating systems for measuring the dose of erythmal UV light have been developed. The first system is of a viologen salt, preferably benzyl viologen chloride, dissolved in poly(vinyl alcohol). This viologen dosimeter responds clearly to different wavelengths and intensities of UV light, although the colour change is hampered by the presence of oxygen, and completely reversed in the absence of UV light. Attempts are made to enhance the rate of the reaction of the viologens with UV light by changing the halide of the salt or adding electron donating species. There is no positive effect noted with the addition of these materials, but changing the halide does alter the UV spectrum of the viologen species, resulting in a slower reaction and thus providing a way of retarding the dosimeter if required. This dosimeter shows a demonstrable reaction to both artificial and natural sources of erythemal light. The second UV dosimeter is based on the tetrazolium salts and is shown to behave in a similar way to the viologen dosimeter, with the neotetrazolium chloride salt as a preferred material for use in the dosimeter. This tetrazolium system is shown to respond clearly to different wavelengths and intensities of light, and shows a demonstrable response to erythemal levels of UV. A solvatochromic indicator is developed, using Reichardt's dye adsorbed onto fumed silica to identify and measure the concentration of various solvent vapours. The fumed silica films are shown to be faster in response than polymer systems and have been shown to distinguish between different solvent vapours, but do not do so in a way that correlates with Reichardt's dye in the pure solvent. These films are shown to be more reliable in identifying alcohol vapours in a predictable manner. A review is also carried out on existing methods of assessing the performance of photocatalytic films, comparing them with a new method developed within the Mills group.

19. Base-induced cyclisations of ortho-substituted nitro-aromatics

Abstract

In Chapter I some of the chemical, biological and physical properties of purine analogues, particularly benzimidazole N-oxides, are briefly discussed. In Chapter II, the preparations of 4-, 5-, 6-, and 7-amino-1H-benzimidazole 3-oxides are described. The methods employed involve base-induced cyclisation of suitably protected aminonitrophenyl glycine derivatives (esters or nitriles) followed by hydrolysis of the ester or nitrile and decarboxylation. In Chapter III, attempts are made to prepare imidazopyridine N-oxides, an area of N-oxide chemistry little explored. Although few derivatives of this class of compound are synthesised, the preparation of the parent 3H-imidazo[4 ,5-b]pyridine 1-oxide is accomplished. In Chapter IV, the possibility of preparing 1-methylbenzimidazole 3-oxides by the methods used in Chapters II and III is investigated, but unexpected results are obtained e.g. cyclisation of O-nitropheny1-sarcosine esters gives 1-hydroxy-4-methylquinoxaline-2, 3-diones. These reactions have instigated an investigation into the general mechanism for the base-induced cyclisations discussed in this thesis.

Abstract

The simulation process, milestones and inputs are unknowns to first-time users of the SES. The Simulator Planning Guide aids in establishing expectations for both NASA and non-NASA facility customers. The potential audience for this guide includes both internal and commercial spaceflight hardware/software developers. It is intended to assist their engineering personnel in simulation planning and execution. Material covered includes a roadmap of the simulation process, roles and responsibilities of facility and user, major milestones, facility capabilities, and inputs required by the facility. Samples of deliverables, facility interfaces, and inputs necessary to define scope, cost, and schedule are included as an appendix to the guide.

22. Jane Jacobs The Ethicist: Systems of Survival and Jacobs' Moral Philosophy

A Thesis Submitted to the Faculty of Graduate Studies and Research In Partial Fulfillment of the Requirements for the Degree of Master of Arts in Social & Political Thought, University of Regina. iv, 150 p. This work intends to show that Jacobs’ moral philosophy makes a strong case for objective moral knowledge. She posits that there are two moral syndromes that are intended to guide working life morality. Roughly speaking, the commercial syndrome guides commerce and the guardian syndrome governs politics and other occupations associated with territorial management. These syndromes are composed of several interconnected precepts and Jacobs argues that these precepts from one syndrome should not be employed with precepts from the other. Should we fail to observe this rule, we will trigger what she calls the Law of Intractable Systemic Corruption (LISC). This law states that every time a working environment mixes precepts together the result is a monstrous moral hybrid syndrome that produces intractable systemic corruption. While this work is too small to prove such a grandiose claim, by diving into Jacobs’ example of the Latin American debt crisis we can see how her system of analysis makes it clear what moral knowledge the actors involved mistakenly ignored and how their doing so caused the crisis. Even though one successful example does not prove the LISC correct it does show that, in at least some instances, it is objectively better to behave morally than to behave immorally. Jacobs hopes to employ this objective knowledge, through increased moral education, to support the use of fear and enforcement that are intended to keep societies from collapsing from corruption within. However, Thomas Hobbes, whose political philosophy also relies on fear, has a different perspective on human nature than Jacobs. Jacobs assumes that people generally want to behave morally and that many moral mistakes can ultimately be attributed to a lack of understanding instead of intentional ii selfishness. Hobbes uses the idea that human beings prioritize their self-interest as the basis of his conception of human nature. He argues that if this is the case then people can only be motivated by fear to adhere to the prosocial behaviour necessary for societies to succeed. A set of experiments conducted by C. Daniel Batson et al. on moral hypocrisy seem to create a sort of Ring of Gyges scenario in the lab meaning that participants can pursue their self-interest without with the fear of consequences. At first glance, the results of these experiments provide a justification for Hobbes’ view of human nature. However, through reference to the work of George W. Watson et al. and Maureen Sie, we can show that Batson’s experiments failed to take into account reporting bias and context. Through further experiments Watson et al. show that we still have good reason to believe that people value morality and can be motivated by it. If Hobbes’ pessimistic view is not justified by the empirical evidence then it seems fair to say that Jacobs’ contributions toward objective moral knowledge are a helpful step toward supporting her proposed addition to fear in moral education. Abstract

Despite evidence supporting the effectiveness of best practices of infection prevention and management, many surgeons worldwide fail to implement them. Evidence-based practices tend to be underused in routine practice. Surgeons with knowledge in surgical infections should provide feedback to prescribers and integrate best practices among surgeons and implement changes within their team. Identifying a local opinion leader to serve as a champion within the surgical department may be important. The “surgeon champion” can integrate best clinical practices of infection prevention and management, drive behavior change in their colleagues, and interact with both infection control teams in promoting antimicrobial stewardship.

Abstract

Intra-abdominal infections (IAIs) are common surgical emergencies and have been reported as major contributors to non-trauma deaths in the emergency departments worldwide. The cornerstones of effective treatment of IAIs are early recognition, adequate source control, and appropriate antimicrobial therapy. Prompt resuscitation of patients with ongoing sepsis is of utmost important. In hospitals worldwide, non-acceptance of, or lack of access to, accessible evidence-based practices and guidelines result in overall poorer outcome of patients suffering IAIs. The aim of this paper is to promote global standards of care in IAIs and update the 2013 WSES guidelines for management of intra-abdominal infections.

The Global Alliance for Infections in Surgery appreciates the great effort of the task force who derived and validated the Sepsis-3 definitions and considers the new definitions an important step forward in the evolution of our understanding of sepsis. Nevertheless, more than a year after their publication, we have a few concerns regarding the use of the Sepsis-3 definitions.

Abstract

Gastric cancer (GC) is the fourth commonest cancer worldwide, with the second highest mortality rate. Its poor mortality is linked to delayed presentation. There is a drive towards non-invasive biomarker screening and monitoring of many different types of cancer, although with limited success so far. We aimed to determine if any genes from a 32-gene panel could be used to determine GC prognosis. We carried out a retrospective study on the expression of 32 genes, selected for their proven or potential links to GC, on historic formalin fixed paraffin-embedded (FFPE) GC specimens from our unit. Gene expression was measured using quantitative nuclease protection assays (qNPA) technology. Following statistical analysis of the results, immunohistochemical staining for eight genes, both discriminating and non-discriminating, was conducted in seven age and sex matched non-metastatic: metastatic GC pairings. The stained samples were reviewed by two blinded consultant histopathologists. Multivariate Cox analysis of the gene expression data revealed metastatic status, age, sex and five genes appeared to influence GC survival. Genes negatively influencing survival included and (relative risks 2.20, 3.73 and 7.53 respectively). Genes conveying survival benefit included and (relative risks 0.10 and 0.24 respectively). Immunohistochemical staining of seven age and sex matched non-metastatic: metastatic pairs revealed no association between gene expression and protein expression. Our study found several genes whose expression may affect GC prognosis. However, immunohistochemical analysis revealed no association between gene expression and protein expression. It remains to be determined whether gene expression or protein expression are reliable means of assessing GC prognosis.

Abstract

Intra-abdominal infections (IAI) are an important cause of morbidity and are frequently associated with poor prognosis, particularly in high-risk patients. The cornerstones in the management of complicated IAIs are timely effective source control with appropriate antimicrobial therapy. Empiric antimicrobial therapy is important in the management of intra-abdominal infections and must be broad enough to cover all likely organisms because inappropriate initial antimicrobial therapy is associated with poor patient outcomes and the development of bacterial resistance. The overuse of antimicrobials is widely accepted as a major driver of some emerging infections (such as C. difficile), the selection of resistant pathogens in individual patients, and for the continued development of antimicrobial resistance globally. The growing emergence of multi-drug resistant organisms and the limited development of new agents available to counteract them have caused an impending crisis with alarming implications, especially with regards to Gram-negative bacteria. An international task force from 79 different countries has joined this project by sharing a document on the rational use of antimicrobials for patients with IAIs. The project has been termed AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections). The authors hope that AGORA, involving many of the world's leading experts, can actively raise awareness in health workers and can improve prescribing behavior in treating IAIs.

Abstract

The open abdomen (OA) procedure is a significant surgical advance, as part of damage control techniques in severe abdominal trauma. Its application can be adapted to the advantage of patients with severe abdominal sepsis, however its precise role in these patients is still not clear. In severe abdominal sepsis the OA may allow early identification and draining of any residual infection, control any persistent source of infection, and remove more effectively infected or cytokine-loaded peritoneal fluid, preventing abdominal compartment syndrome and deferring definitive intervention and anastomosis until the patient is appropriately resuscitated and hemodynamically stable and thus better able to heal. However, the OA may require multiple returns to the operating room and may be associated with significant complications, including enteroatmospheric fistulas, loss of abdominal wall domain and large hernias. Surgeons should be aware of the pathophysiology of severe intra-abdominal sepsis and always keep in mind the option of using open abdomen to be able to use it in the right patient at the right time.

Abstract

Background: Antimicrobial Stewardship Programs (ASPs) have been promoted to optimize antimicrobial usage and patient outcomes, and to reduce the emergence of antimicrobial-resistant organisms. However, the best strategies for an ASP are not definitively established and are likely to vary based on local culture, policy, and routine clinical practice, and probably limited resources in middle-income countries. The aim of this study is to evaluate structures and resources of antimicrobial stewardship teams (ASTs) in surgical departments from different regions of the world. Methods: A cross-sectional web-based survey was conducted in 2016 on 173 physicians who participated in the AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections) project and on 658 international experts in the fields of ASPs, infection control, and infections in surgery. Results: The response rate was 19.4%. One hundred fifty-six (98.7%) participants stated their hospital had a multidisciplinary AST. The median number of physicians working inside the team was five [interquartile range 4-6]. An infectious disease specialist, a microbiologist and an infection control specialist were, respectively, present in 80.1, 76.3, and 67.9% of the ASTs. A surgeon was a component in 59.0% of cases and was significantly more likely to be present in university hospitals (89.5%, p < 0.05) compared to community teaching (83.3%) and community hospitals (66.7%). Protocols for pre-operative prophylaxis and for antimicrobial treatment of surgical infections were respectively implemented in 96.2 and 82.3% of the hospitals. The majority of the surgical departments implemented both persuasive and restrictive interventions (72.8%). The most common types of interventions in surgical departments were dissemination of educational materials (62.5%), expert approval (61.0%), audit and feedback (55.1%), educational outreach (53.7%), and compulsory order forms (51.5%). Abstract

Background: Antimicrobial Stewardship Programs (ASPs) have been promoted to optimize antimicrobial usage and patient outcomes, and to reduce the emergence of antimicrobial-resistant organisms. However, the best strategies for an ASP are not definitively established and are likely to vary based on local culture, policy, and routine clinical practice, and probably limited resources in middle-income countries. The aim of this study is to evaluate structures and resources of antimicrobial stewardship teams (ASTs) in surgical departments from different regions of the world. Methods: A cross-sectional web-based survey was conducted in 2016 on 173 physicians who participated in the AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections) project and on 658 international experts in the fields of ASPs, infection control, and infections in surgery. Results: The response rate was 19.4%. One hundred fifty-six (98.7%) participants stated their hospital had a multidisciplinary AST. The median number of physicians working inside the team was five [interquartile range 4-6]. An infectious disease specialist, a microbiologist and an infection control specialist were, respectively, present in 80.1, 76.3, and 67.9% of the ASTs. A surgeon was a component in 59.0% of cases and was significantly more likely to be present in university hospitals (89.5%, p < 0.05) compared to community teaching (83.3%) and community hospitals (66.7%). Protocols for pre-operative prophylaxis and for antimicrobial treatment of surgical infections were respectively implemented in 96.2 and 82.3% of the hospitals. The majority of the surgical departments implemented both persuasive and restrictive interventions (72.8%). The most common types of interventions in surgical departments were dissemination of educational materials (62.5%), expert approval (61.0%), audit and feedback (55.1%), educational outreach (53.7%), and compulsory order forms (51.5%). Abstract

This declaration, signed by an interdisciplinary task force of 234 experts from 83 different countries with different backgrounds, highlights the threat posed by antimicrobial resistance and the need for appropriate use of antibiotic agents and antifungal agents in hospitals worldwide especially focusing on surgical infections. As such, it is our intent to raise awareness among healthcare workers and improve antimicrobial prescribing. To facilitate its dissemination, the declaration was translated in different languages.

Abstract

Acute left sided colonic diverticulitis is one of the most common clinical conditions encountered by surgeons in acute setting. A World Society of Emergency Surgery (WSES) Consensus Conference on acute diverticulitis was held during the 3rd World Congress of the WSES in Jerusalem, Israel, on July 7th, 2015. During this consensus conference the guidelines for the management of acute left sided colonic diverticulitis in the emergency setting were presented and discussed. This document represents the executive summary of the final guidelines approved by the consensus conference.
