Pigmented villonodular synovitis is an uncommon benign neoplastic proliferation associated with the synovium, bursa, or tendon sheaths; most commonly occurring in the third to fourth decade of life. It is rare in children and may be painful or painless. Magnetic resonance imaging is the diagnostic study of choice. In this report, the radiologic, ultrasound, and magnetic resonance imaging findings of pigmented villonodular synovitis of the flexor hallucis longus in a 12-year-old girl are discussed. We briefly review the surgical findings as well. To our knowledge, this is the first case report that simultaneously synthesizes the imaging findings of 3 diagnostic imaging modalities for optimal visualization and is the youngest reported case of pigmented villonodular synovitis of the flexor hallucis longus tendon.

The benefits of large-scale genetic studies for healthcare of the populations studied are well documented, but these genetic studies have traditionally ignored people from some parts of the world, such as South Asia. Here we describe whole genome sequence (WGS) data from 4806 individuals recruited from the healthcare delivery systems of Pakistan, India and Bangladesh, combined with WGS from 927 individuals from isolated South Asian populations. We characterize population structure in South Asia and describe a genotyping array (SARGAM) and imputation reference panel that are optimized for South Asian genomes. We find evidence for high rates of reproductive isolation, endogamy and consanguinity that vary across the subcontinent and that lead to levels of rare homozygotes that reach 100 times that seen in outbred populations. Founder effects increase the power to associate functional variants with disease processes and make South Asia a uniquely powerful place for population-scale genetic studies.

Background Haematological abnormalities following renal transplantation are frequently observed and have a significant effect on survival and graft outcomes. The pattern of haematological abnormalities varies globally. Few studies have been conducted in Asian countries. We aimed to evaluate the patterns of haematological abnormalities in post-transplant recipients in our center during the first year after post-renal transplant and the association of post-transplant anemia with graft function. Methods This single-center retrospective study was conducted on renal transplantation recipients between 2014 and 2019. The study included all patients who received kidney transplants from live/cadaveric donors and had follow-up data collected up to 12 months after the transplant. The outcome studied was the prevalence of haematological abnormalities and the association between post-transplant anemia (PTA) and graft function in post-transplant recipients. Results A total of 106 renal transplant recipients were included in the study. The prevalence of PTA was 98% in the first week, 75% at one month, 35% at three months, 32% at six months, and 27% at 12 months. The other cytopenia cases were leukopenia (43.4%), thrombocytopenia (33.2%), and pancytopenia (15.1%). Post-transplant erythrocytosis was observed in 17.9% of patients. 18 patients with severe PTA in the first week of transplant had significant allograft dysfunction (p=0.04). Patients with and without PTA had similar graft functions at six and 12 months (p=0.50). Conclusions Haematological abnormalities are common in renal transplant recipients. PTA is highly prevalent during the first week and improves over time. Other haematological abnormalities observed were leukopenia, thrombocytopenia, pancytopenia, and post-transplant erythrocytosis. Leucopenia was primarily drug-induced, and thrombocytopenia and pancytopenia were frequently caused by infections in our cohort. Additionally, severe PTA was significantly associated with graft dysfunction in the first week post-transplant, whereas similar graft function was observed at 6 and 12 months post-transplant, irrespective of the presence or absence of PTA.

Introduction Patients with chronic kidney disease (CKD) are at increased risk of developing tuberculosis (TB). These patients may also be at higher risk of developing antitubercular treatment (ATT)-associated adverse drug reactions (ADRs). Although dose modification has been recommended, data regarding the impact of impaired kidney function on ATT-associated ADRs is sparse. We studied the incidence and profile of ATT-associated ADRs in patients with CKD and compared them with those with normal kidney function.Methodology This retrospective study analyzed all patients initiated on ATT from January 2016 to August 2019. Patients were grouped into CKD and normal kidney function based on their eGFR. Data on ATT-associated ADRs were collected from medical records. Predictors of ADRs were assessed using univariable and multivariable logistic regression. Additionally, Propensity score matching and analysis were done for CKD and normal kidney function in 1:3 ratio.Results Of 1815 patients on ATT, 75 (4.1%) had CKD. ADRs were more frequent [36/75 (48.0%) vs. 239/1740 (13.7%), p ≤ 0.0001] and more severe [15/46 (32.6%) vs. 43/283 (15.1%), p = 0.010] in CKD than those with normal kidney function. The most common ADRs were hepatobiliary [23/75 (30.6%) vs. 156/1740 (8.9%), p ≤ 0.0001], neuropsychiatric [8/75(10.6%) vs. 21/1740(1.2%), p ≤ 0.0001], renal [4/75(5.3%) vs. 8/1740(0.4%), p = 0.001], and gastrointestinal [5/75(6.6%) vs. 34/1740 (1.9%), p = 0.020]. CKD was an independent predictor for ADRs (OR −4.96, 95% CI: 2.79–8.82; p ≤ 0.0001). The matched cohort showed similar results.Conclusion ATT-associated ADRs were more common and severe in patients with CKD, despite drug dose modifications. Optimal dosing of ATT in CKD needs to be further evaluated.

The generation of waste is significantly influenced by the increase in population and industrialization, thereby compelling the increased demand for waste management and resource recovery. This paper investigates the potential opportunities presented by the utilization of microwave-assisted pyrolysis for processing plastic and organic waste materials, with a particular focus on industrial hemp leaves, hurds, and root materials, and other feedstocks. Drawing from a range of published studies, it is suggested that microwave-assisted pyrolysis has the potential to achieve energy neutrality or even energy generation, if all byproducts are used. Depending on factors such as recoverable volumes and the associated recovery costs of commercially significant chemicals like vinegars and bio-oils, the microwave-assisted pyrolysis of industrial hemp leaves, hurds, and root materials may prove to provide high return of the yield and profits. Additionally, this paper explores the production of other valuable byproducts such as syngas and biochar from alternative feedstocks, particularly when data related to hemp processing is not readily available.

Purpose Rapid and effective revascularization is very important in the treatment of acute ischemic stroke (AIS). Endovascular treatment is a promising modality in the management of AIS in young patients. Methods We evaluated the clinical and imaging records in 14 patients younger than 18 years presenting within 6 hours of AIS. They received endovascular therapy (ET) either by mechanical thrombectomy, thromboaspiration, or both (Solumbra) between July 2017 and June 2021. The National Institute of Health Stroke Scale (NIHSS) score was calculated on admission and before the discharge of all patients. The 90-day modified Rankin Scale (mRS) score on disability-free outcome was also evaluated. Results The mean preprocedure NIHSS score was 10.78 ± 2.11 that improved to 4.5 ± 1.88 after the procedure. Thrombolysis in cerebral infarction (TICI) grade 2b and 3 blood flow could be established in 12 (85.72%) patients. One patient had TICI 2a flow and one patient had recurrent occlusion despite repeated recanalization (TICI grade 0). The disability-free outcome, mRS score at 90 days was 0 to 1 in 12 (85.72%) patients, mRS score 2 in one (7.14%) patient, and mRS score 3 in one patient (7.14%). We did not have any major complication related to the procedure. Conclusion ET provides high rates of arterial recanalization and favorable disability-free outcomes in young patients with AIS.

As data processing volume increases, the limitations of traditional computers and the need for more efficient computing methods become evident. Neuromorphic computing mimics the brain's low‐power and high‐speed computations, making it crucial in the era of big data and artificial intelligence. One significant development in this field is the memristor, a device that exhibits neuromorphic tendencies. The performance of memristive devices and circuits relies on the materials used, with graphene being a promising candidate due to its unique properties. Researchers are investigating graphene‐based memristors for large‐scale, sustainable fabrication. Herein, progress in the development of graphene‐based memristive neuromorphic devices and circuits is highlighted. Graphene and its common fabrication methods are discussed. The fabrication and production of graphene‐based memristive devices are reviewed and comparisons are provided among graphene‐ and nongraphene‐based memristive devices. Next, a detailed synthesis of the devices utilizing graphene‐based memristors is provided to implement the basic building blocks of neuromorphic architectures, that is, synapses, and neurons. This is followed by reviewing studies building graphene memristive spiking neural networks (SNNs). Finally, insights on the prospects of graphene‐based neuromorphic memristive systems including their device‐ and network‐level challenges and opportunities are given.

Hyperuricemia is a risk factor for the progression of chronic kidney disease (CKD). We compared febuxostat versus allopurinol in the progression of CKD and hyperuricemia in 101 patients with Stage 3–4 CKD treated with febuxostat or allopurinol for at least 6 months for hyperuricemia (>7 mg/dL) between January 2012 and December 2016. Baseline characteristics, serum uric acid (SUA), serum creatinine, and estimated glomerular filtration rate (eGFR) at entry and 6 months were compared. The primary outcome was the decline in eGFR and the secondary outcomes were reductions in SUA and adverse events. Fifty-four were in the febuxostat group and 47 were in the allopurinol group. The baseline characteristics were comparable except for age. The mean dose of febuxostat and allopurinol was 43.70 ± 14.5 mg and 108.51 ± 40 mg, respectively. After 6 months, the median rate of decline in eGFR was 1.2 mL/min/1.73 m2 (IQR: 1.2, 5.5) in the febuxostat group and 3.1 mL/min/1.73 m2 (0.6, 6.2) in the allopurinol group, but this was not statistically significant (P = 0.136). The mean reduction in SUA was significantly better (P = 0.004) in the febuxostat group (3.9 ± 1.7 mg/dL) compared with the allopurinol group (2.1 ± 1.0 mg/dL). Both drugs had no serious adverse events. Febuxostat was better at reducing hyperuricemia than allopurinol, but there was no significant difference in the progression of CKD. Large randomized trials and long-term follow-up are necessary to see whether febuxostat has a favorable effect on the progression of CKD.

AbstractPopulation-scale genetic studies can identify drug targets and allow disease risk to be predicted with resulting benefit for management of individual health risks and system-wide allocation of health care delivery resources. Although population-scale projects are underway in many parts of the world, genetic variation between population groups means that additional projects are warranted. South Asia has a population whose genetics is the least characterized of any of the world’s major populations. Here we describe GenomeAsia studies that characterize population structure in South Asia and that create tools for economical and accurate genotyping at population-scale. Prior work on population structure characterized isolated population groups, the relevance of which to large-scale studies of disease genetics is unclear. For our studies we used whole genome sequence information from 4,807 individuals recruited in the health care delivery systems of Pakistan, India and Bangladesh to ensure relevance to population-scale studies of disease genetics. We combined this with WGS data from 927 individuals from isolated South Asian population groups, and developed a custom SNP array (called SARGAM) that is optimized for future human genetic studies in South Asia. Implant-associated infections (IAI) cause significant health issues and healthcare costs. In this research, we deposited graphene (Gr) on a medical-grade cobalt-chromium (CoCr) alloy surface by radiofrequency plasma-enhanced chemical vapor deposition (RF-PECVD) using Origanum vulgare as a precursor material. The deposition of Gr on the CoCr was confirmed using Raman spectroscopy and X-Ray photoelectron spectroscopy (XPS) and scanning electron microscopy (SEM). The biocompatibility and antibacterial properties of CoCr-Gr were investigated. CoCr-Gr was biocompatible and promoted cell adhesion and spreading of RAW 267.4 macrophage cells. CoCr-Gr were antibacterial against Staphylococcus aureus and Pseudomonas aeruginosa and inhibited P. aeruginosa attachment. The results indicate that CoCr-Gr could be used as a potential antibacterial coating material for implantable devices.

Introduction: In hemodialysis (HD) patients, catheter-related bloodstream infections (CRBSI) cause significant morbidity and mortality. Objectives: We analyzed dialysis CRBSI incidence, associated factors, and the spectrum of the causative organisms. Patients and Methods: Patients aged ≥18 years either on maintenance HD or with acute kidney injury having CRBSI (NKF-KDOQI criteria) were included in this prospective observational study and patients with other infections were excluded. Blood, catheter tip culture, and antibiogram were analyzed. All patients were initially treated with antibiotics covering both gram-positive and gram-negative pathogens. Abstract Atmospheric pressure microwave plasma has the lead in synthesizing freestanding and scalable graphene within seconds without the need for high vacuum and temperature. However, the process is limited in utilizing chemical sources for synthesis, such as methane and ethanol. Herein, the usage of an extract of a sustainable precursor, that is, Melaleuca alternifolia, commonly known as tea tree, is for the first time reported to synthesize graphene nanosheets in atmospheric pressure microwave plasma. The synthesis is carried out in a single step at a remarkably low microwave power of 200 W. The morphology, structure, and electrochemical properties of graphene are studied using state‐of‐the‐art characterization techniques such as Raman spectroscopy, X‐ray diffraction, transmission electron microscopy (TEM) and electrochemical impedance spectroscopy. The TEM images reveal the presence of a combination of nanostructures such as nano‐horns, nano‐rods, or nano‐onions consisting of multi‐layer graphitic architectures. An excellent sensing capability of as‐synthesized graphene is demonstrated through the detection of diuron herbicide. A commendable linear range of 20 µm to 1 mm and a limit of detection of 5 µm of diuron is recorded.

The underlying pathophysiology of metabolic inflexibility is a common thread linking insulin resistance to cardiometabolic-based chronic disease (CMBCD), including dysglycemia, hypertension, and dyslipidemia progressing to downstream ASCVD events. The complex CMBCD paradigm in the LMIC population within the socio-economic and cultural context highlights considerable heterogeneity of disease predisposition, clinical patterns, and socio-medical needs. This review intends to summarize the current knowledge of CMBCD. We describe recently established or emerging trends for managing risk factors, assessment tools for evaluating ASCVD risk, and various pharmacological and non-pharmacological measures particularly relevant for LMICs. A CMBCD model positions insulin resistance and β-cell dysfunction at the summit of the disease spectrum may improve outcomes at a lower cost in LMICs. Despite identifying multiple pathophysiologic disturbances constituting CMBCD, a large percentage of the patient at risk for ASCVD remains undefined. Targeting dysglycemia, dyslipidemia, and hypertension using antihypertensive, statins, anti-glycemic, and antiplatelet agents has reduced the incidence of ASCVD. Thus, primordial prevention targeting pathophysiological changes that cause abnormalities in adiposity and primary prevention by detecting and managing risk factors remains the foundation for CMBCD management. In this multi-dose phase-2 study, we evaluated the safety and efficacy of TRC150094 in diabetic subjects with dyslipidemia receiving standard of care.Methods: A randomized, multicenter, double-blind, placebo-controlled, parallel-group, Phase 2 study was conducted in 225 subjects from July 2013 to August 2015. The key inclusion criteria were body mass index of 23– 35 kg/m2, age between 30 and 65 years, fasting glucose of ≥ 126 or glycated hemoglobin (HbA1c) of ≥ 6.4% stabilized on treatment with ≤ 2 oral hypoglycemic agents, apolipoprotein-B (apo-B) ≥ 100 mg/dL, serum triglyceride (TG) ≥ 150 mg/dL, systolic blood pressure (SBP) ≥ 130 mmHg, and diastolic blood pressure (DBP) ≥ 85 mmHg with/without antihypertensive treatment. The subjects were randomly assigned to one of three TRC150094 doses (25, 50, or 75 mg) or placebo for 24 weeks. The outcomes assessed included fasting plasma glucose (FPG), insulin, mean arterial blood pressure (MAP), and apoB. In addition, safety and tolerability were assessed.Results: A reduction for dose up to 50 mg was noted for FPG in the range of 13.9 to 21.7 mg/dL (p < 0.05 for TRC150094 25 and 50 mg), fasting insulin reduction in the range 2.7 to 6.0 mU/L (all doses, p > 0.05), and improved HOMA-IR (− 2.0 to − 2.5) (all doses, p > 0.05) compared to placebo after 24 weeks of treatment. Furthermore, a significant reduction in MAP in the range 3.1 to 4.2 mmHg (p < 0.05 for TRC150094 25 and 75 mg) was noted. In addition, TRC150094 treatment was weight neutral, had a favorable effect on lowering atherogenic lipid fractions, including non-HDL cholesterol (− 6.8 mg/dL at 50 mg dose). Adverse events were mild to moderate in nature and not dose-related. One adverse event not related to treatment led to the discontinuation of the study. Overall, TRC150094 was safe and well tolerated for up to 24 weeks.Conclusion: In this study, TRC150094 treatment in the dose range of 25 to 50 mg showed improvement in various components of CMBCD, ie, dysglycemia, dyslipidemia, and hypertension.Trial Registration: This study was registered in the Clinical Trial Registry of India. The significant quantities of food waste that require disposal have a high environmental impact, and the depletion of non-renewable fuel sources has heightened the need to investigate sustainable and efficient methods of biomass conversion into energy. This research focuses on utilising pumpkin peel as a feedstock for energy recovery through microwave pyrolysis under different operating conditions. The study demonstrated that a higher biochar yield (11 wt%) was achieved at 0.9 kW. However, results revealed that superior quality biochar was obtained at 1.2 kW, characterized by high carbon content (70.33%), low oxygen content (23%), and significant pore formation in the carbon surface area. Optimal operating conditions, such as 1.2 kW, resulted in superior quality biochar and higher bio-oil generation. The pumpkin peel demonstrated the potential for CO2 (carbon dioxide) sequestration, with a rate of 14.29 g CO2 eq/kg. The research findings contribute to the exploration of sustainable solutions for biomass conversion and emphasize the importance of utilizing food waste for energy production while mitigating environmental impacts.

The synthesis of free-standing graphene in a microwave plasma at ambient conditions is currently of great interest. The past works have relied on the usage of higher microwave powers to synthesize free-standing graphene which is not only costly but also an obstacle to the industrialization of the process. The aim of this work was to bring down the cost of the process by synthesizing graphene at a significantly lower microwave power, i.e. 250 W. The formation of graphene was confirmed through Raman spectrum and scanning electron microscopy, where the Raman spectrum showed the signature 2D peak of graphene, and the vertical orientation of the graphene was observed in the microscopic images. The application of graphene in oil-water separation is demonstrated based on its hydrophobic and oleophilic properties. The as-synthesized pristine graphene coated on a melamine sponge showed a mass absorption capacity (57 g/g) comparable to that of functionalized or composite graphene.

Introduction: In transplantation, accurate estimation of the donor glomerular filtration rate (GFR) is crucial. While various creatinine-based equations are in use, none are validated in Indians. Objectives: This study was conducted to judge the accuracy of creatinine-based GFR estimation equations and urinary creatinine clearance. Patients and Methods: A single-centre, observational and retrospective study at a tertiary care hospital. Adult voluntary donors GFR measured (mGFR) by technetium-99m diethylenetriaminepentaacetic acid (Tc-99m DTPA) were included. The primary outcome was the performance of estimated GFR (eGFR) by "Cockcroft-Gault’s formula corrected for body surface area (CG-BSA) formula", "modification of diet in renal disease (MDRD) 4 and 6 variable equation" and "Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation"; secondary outcome was the performance of "24-hour urinary creatinine clearance (Cr Cl)". Results: 102 kidney donors were analysed with mean age of 45.89 ± 9.98 years and 85.3% females. Mean ± SD mGFR by Tc-99m DTPA was 82.11 ± 14.32 mL/min/1.73 m2 . Mean ± SD eGFR by "CGBSA" was 99.68 ± 23.71 mL/min/1.73 m2 , by "MDRD-4 variable equation" was 98.25 ± 28.61 mL/ min/1.73 m2 , by "MDRD-6 variable equation" was 93.66 ± 19.44 ml/min/1.73 m2 and by "CKD-EPI" was 111.14 ± 31.61 mL/min/1.73 m2 . The lowest bias (2.3), highest precision (16.23), and accuracy (97.1%) were with "MDRD-6 variable equation"; "24-hour urinary Cr Cl" highly overestimated GFR (158.27 mL/min/1.73 m2 ) with the highest bias, lowest precision, and accuracy. Conclusion: The "MDRD-6 variable equation" was the most precise and accurate of the equations, whereas "24-hour urinary Cr Cl" was the least dependable. This study highlights the need for a correction factor or a new GFR estimation equation and not to consider urinary Cr Cl to assess donor GFR.

The adverse effects of oxalic acid (OA) on human health linked with its excessive consumption necessitates an improved sensor. Here, we demonstrate an electrochemical sensor for oxalic acid detection based on silver nanoparticles (Ag-Nps) and nitrogen-doped graphene oxide (N-GO) nanocomposite. N-GO, which was synthesized using atmospheric pressure microwave plasma has been first time employed for electrochemical application. The nanocomposite formation was confirmed through scanning electron microscopy and EDS elemental analysis. The nanocomposite-based sensor showed a higher current response, good selectivity and stability which can be attributed to the synergistic-effect of Ag-Nps and N-GO. Amperometric responses were proportional to the concentration of OA between 10 and 300 μM, and the detection limit was 2 μM.
