- Abstract
Simple Summary The location of the primary tumor in the right colon, left colon, or rectum affects the efficacy of biological drugs used in the treatment of metastatic colorectal cancer, but how? We examined how the primary tumor location affects disease characteristics, treatability, quality of life, and outcome in a real-life study population of 1080 Finnish patients in the RAXO study. The primary tumor location correlates with the location of metastases, the frequency of gene mutations, how often metastases can be operated upon, long-term survival after curative surgery or palliative chemotherapy, and the quality of life during the disease trajectory. The primary tumor location is a helpful surrogate for clinicians working with metastatic colorectal cancer patients in estimating the clinical course of the disease. This study cannot identify the reasons for the associations, i.e., whether it is the primary location per se, the different mutations, or other reasons.Abstract The primary tumor location (PTL) is associated with the phenotype, metastatic sites, mutations, and outcomes of metastatic colorectal cancer (mCRC) patients, but this has mostly been studied according to sidedness (right vs. left sided). We studied right colon vs. left colon vs. rectal PTL in a real-life study population (n = 1080). Health-related quality of life (HRQoL) was assessed multi-cross-sectionally with QLQ-C30, QLQ-CR29, EQ-5D, and 15D. A chi-square, Kaplan-Meier, and Cox regression were used to compare the groups. The PTL was in the right colon in 310 patients (29%), the left colon in 396 patients (37%), and the rectum in 375 patients (35%). The PTL was associated with distinct differences in metastatic sites during the disease trajectory. The resectability, conversion, and resection rates were lowest in the right colon, followed by the rectum, and were highest in the left colon. - Abstract
Older adults are underrepresented in metastatic colorectal cancer (mCRC) studies and thus may not receive optimal treatment, especially not metastasectomies. The prospective Finnish real-life RAXO-study included 1086 any organ mCRC patients. We assessed repeated centralized resectability, overall survival (OS), and quality of life (QoL) using 15D and EORTC QLQ-C30/CR29. Older adults (>75 years; n = 181, 17%) had worse ECOG performance status than adults (<75 years, n = 905, 83%), and their metastases were less likely upfront resectable. The local hospitals underestimated resectability in 48% of older adults and in 34% of adults compared with the centralized multidisciplinary team (MDT) evaluation (p < 0.001). The older adults compared with adults were less likely to undergo curative-intent R0/1-resection (19% vs. 32%), but when resection was achieved, OS was not significantly different (HR 1.54 [CI 95% 0.9-2.6]; 5-year OS-rate 58% vs. 67%). 'Systemic therapy only' patients had no age-related survival differences. QoL was similar in older adults and adults during curative treatment phase (15D 0.882-0.959/0.872-0.907 [scale 0-1]; GHS 62-94/68-79 [scale 0-100], respectively). Complete curative-intent resection of mCRC leads to excellent survival and QoL even in older adults. Older adults with mCRC should be actively evaluated by a specialized MDT and offered surgical or local ablative treatment whenever possible.

- Abstract
Metastatic colorectal cancer is the second most common cause of cancer death. Long-term survival and cure can be achieved after intensive treatments, including metastasectomy, i.e., the removal of all metastases. We wanted to clarify whether a patient health-related quality of life (HRQoL) was reduced by treatments that aimed to maximise metastasectomy rates, and whether HRQoL of treated patients is comparable to the general population. In a cross-sectional study of 444 patients (1751 questionnaires) in the RAXO-study population, we show that HRQoL of intensively treated patients, sometimes with multiple and multisite metastasectomies-usually combined with systemic therapy-remains at a high level during and after curative treatment and when compared with the general population. Good HRQoL was also seen during non-curative treatment from first- to later-lines, with an impaired HRQoL only at end-of-life. Thus, we should aim at maximising metastasectomies since they give long-term survival and sometimes cure with a high HRQoL. Metastasectomy and/or local ablative therapy in metastatic colorectal cancer (mCRC) patients often provide long-term survival. Health-related quality of life (HRQoL) data in curatively treated mCRC are limited. In the RAXO-study that evaluated repeated resectability, a multi-cross-sectional HRQoL substudy with 15D, EQ-5D-3L, QLQ-C30, and QLQ-CR29 questionnaires was conducted. Mean values of patients in different treatment groups were compared with age- and gender-standardized general Finnish populations. The questionnaire completion rate was 444/477 patients (93%, 1751 questionnaires). Mean HRQoL was 0.89-0.91 with the 15D, 0.85-0.87 with the EQ-5D, 68-80 with the EQ-5D-VAS, and 68-79 for global health status during curative treatment phases, with improvements in the remission phase (disease-free >18 months). - Abstract
Background KRAS mutations, present in over 40% of metastatic colorectal cancer (mCRC), are negative predictive factors for anti-EGFR therapy. Mutations in KRAS-G12C have a cysteine residue for which drugs have been developed. Published data on this specific mutation are conflicting; thus, we studied the frequency and clinical characteristics in a real-world and population-based setting. Methods Patients from three Nordic population-based cohorts and the real-life RAXO-study were combined. RAS and BRAF tests were performed in routine healthcare, except for one cohort. The dataset consisted of 2,559 patients, of which 1,871 could be accurately classified as KRAS, NRAS, and BRAF-V600E. Demographics, treatments, and outcomes were compared using logistic regression. Overall survival (OS) was estimated with Kaplan-Meier, and differences were compared using Cox regression, adjusted for baseline factors. Results The KRAS-G12C frequency was 2%-4% of all tested in the seven cohorts (mean 3%) and 4%-8% of KRAS mutated tumors in the cohorts (mean 7%). Metastasectomies and ablations were performed more often (38% vs. 28%, p = 0.040), and bevacizumab was added more often (any line 74% vs. 59%, p = 0.007) for patients with KRAS-G12C- vs. other KRAS-mutated tumors, whereas chemotherapy was given to similar proportions. OS did not differ according to KRAS mutation, neither overall (adjusted hazard ratio (HR) 1.03; 95% CI 0.74-1.42, reference KRAS-G12C) nor within treatment groups defined as "systemic chemotherapy, alone or with biologics", "metastasectomy and/or ablations", or "best supportive care", RAS and BRAF wild-type tumors (n = 548) differed similarly to KRAS-G12C, as to other KRAS- or NRAS-mutated (n = 66) tumors. Background Lower limb or trunk melanoma often presents with femoral and pelvic sentinel lymph nodes (SLNs). The benefits of harvesting pelvic lymph nodes remain controversial. In this retrospective study, the frequency and predictors of pelvic SLNs (PSLNs), and the impact of PSLNs on survival and staging was investigated. Methods Altogether 285 patients with cutaneous melanoma located in the lower limb or trunk underwent sentinel lymph node biopsy of the inguinal/iliac lymph node basin at Helsinki University Hospital from 2009–2013. Patient characteristics, detailed pathology reports and follow-up data were retrieved from hospital files. Subgroups of patients categorized by presence of PSLNs were compared for outcome parameters including progression-free survival, melanoma-specific survival and groin recurrence. Results Superficial femoral/inguinal SLNs were present in all patients and 199 (69.8 per cent) also had PSLNs removed. Median number of SLNs per patient was five and median number of PSLNs was two. Sixty-three patients (22.1 per cent) had metastases in their SLNs and seven (2.5 per cent) had metastases in PSLNs. A single patient had metastases solely in PSLNs, while superficial SLNs remained negative. Harvesting PSLNs or the number of PSLNs retrieved had no impact on progression-free survival or overall survival. The removal of PSLNs did not affect the risk of postoperative seroma or lymphoedema. The only predictor of positive PSLNs was radioactivity count equal to or more than that of the hottest superficial SLNs. Conclusion Pelvic SLNs have minimal clinical impact on the outcome of melanoma patients especially in cases with negative superficial femoral/inguinal SLNs. Removal of PSLNs should be considered when they are the most radioactive nodes or equal to the hottest superficial femoral/inguinal SLNs in lymphoscintigraphy or during surgery. Preliminary results were presented in part at the International Sentinel Node Society Biennial Meeting, Tokyo, Japan, 11–13 October 2018., There is no consensus regarding the management of sentinel lymph nodes located in the pelvis in melanoma patients. In this study, the outcomes of 285 melanoma patients who underwent sentinel lymph node biopsy of the inguinal/iliac lymph node basin were analysed. Pelvic sentinel lymph nodes had minimal impact on survival and staging of melanoma patients, and removal of pelvic sentinel lymph nodes may be considered when they are the most radioactive nodes in lymphoscintigraphy or during surgery.

Side-streams of wood product industries such as wood chips, planer shavings, saw dust, sanding dust, off-cut pieces, bark and liquid and gaseous wastes considerably contribute to cascading, sustainability and carbon balance as well as profitability of saw mill, plywood and further processing industries in northern Europe. They are important raw materials for pulp and paper, wood panel and bioenergy industries, and provide much basis for novel value-added products. Industrial ecosystems based on side-streams may be integrated with pulp and paper mills, chipboard or fibreboard mills or heating and power plants, concentrated to SME industry parks or decentralized between feedstock terminals, wood processing mills and biorefinery plants of different size. Availability and flows of raw materials and semi-finished products, enterprise network and collaboration, storage and transportation logistics and scaling of production essentially affect the optimal build-up of industrial ecosystems and value-add. The objective of this paper is to summarize results from our recent state-of-art and future perspective studies in Finland regarding industrial ecosystems based on sidestreams of wood product industries. We stress here value and supply chains, scaling production, business and ownership, and present examples on well-functioning industrial ecosystems and enterprise networks in Finland. Finland has excellent potential for becoming an international pioneer of the bioeconomy. Plentiful renewable resources and profound competence in their exploitation create a foundation for the development of new solutions that enable the transition to a low-carbon and resource-efficient economy. The realisation of this goal is promoted by the ongoing digitalisation process, which will make natural resource data available for the use of a wider group of operators and enable new, networked operating models. VTT Technical Research Centre of Finland Ltd and Natural Resources Institute Finland have drawn up this roadmap, which points the way towards a knowledge-intensive, competitive and collaborative bioeconomy making use of the possibilities of digitalisation.We identified three transition paths to achieve a digitalised bioeconomy and presented sample cases connected to those paths. The transition paths are: 1) Smart biomass flow management, aimed at creating more value from raw materials through the implementation of digital solutions for the optimal use of raw materials, smart logistics and flexible production control systems. 2) Data-driven bioeconomy, compiling development needs related to the generation, analysis and refinement of natural resource data that promotes the bioeconomy. Digital solutions can increase transparency and support predictive decision-making. 3) Networked and collaborative natural resource economy, comprising the development of business models and digital platforms to enable the networked operations of bioeconomy actors.Suggested measures for the various key parties were formulated in the course of roadmap work. Digitalisation requires fast action, agility and adaptability. In this operating environment, it is vital to adopt a culture of experimentation. Research should be steered towards the development of new concepts and anticipating the consequences of business models related to them. It is vital to identify customer needs and see services as an integral part of products. Authorities must adopt a predictive approach that supports the creation of new solutions. - Abstract
Suomella on erinomaiset mahdollisuudet nousta biotalouden kansainväliseksi edelläkävijäksi. Runsaat uusiutuvat luonnonvarat ja pitkälle kehittynyt osaaminen niiden kestävässä hyödyntämisessä tarjoavat perustan kehittää uusia ratkaisuja, joiden avulla voidaan siirtyä vähähiiliseen ja resurssitehokkaaseen talouteen. Tavoitteen toteuttamista edistää käynnissä oleva digitalisaatio, joka tuo luonnonvaroja koskevan tiedon entistä laajemman toimijajoukon hyödynnettäväksi ja mahdollistaa uudenlaiset verkottuneet toimintamallit. Teknologian tutkimuskeskus VTT Oy ja Luonnonvarakeskus ovat yhteistyössä laatineet tiekartan, joka viitoittaa polkuja kohti digitalisaation mahdollisuuksia hyödyntävää, tietointensiivistä, kilpailukykyistä ja yhteistoiminnallista biotaloutta. Työssä tunnistettiin kolme muutospolkua digitalisoidun biotalouden saavuttamiseksi ja kuvattiin niihin kytkeytyviä case-esimerkkejä. Muutospolut ovat: 1) Älykäs biomassavirtojen hallinta, jonka tavoitteena on tuottaa enemmän arvoa raaka-aineesta toteuttamalla digitaalisia ratkaisuja raaka-aineen optimaaliseen käyttöön, älykkääseen logistiikkaan ja joustaviin tuotannonohjausjärjestelmiin. 2) Datavetoinen biotalous, joka kokoaa biotaloutta edistävän luonnonvaratiedon tuottamiseen, analysointiin ja muokkaamiseen liittyviä kehitystarpeita. Digitaalisilla ratkaisuilla voidaan lisätä läpinäkyvyyttä ja tukea ennakoivaa päätöksentekoa. 3) Verkottunut ja yhteistoiminnallinen luonnonvaratalous, joka käsittää digitaalisten alustojen ja liiketoimintamallien kehittämisen biotalouden toimijoiden verkostoituneen toiminnan mahdollistamiseksi. Tiekarttatyön tuloksena muotoiltiin toimintasuosituksia eri avaintoimijoille. Digitalisaatio vaatii nopeutta, ketteryyttä ja sopeutumiskykyä. Tällaisessa toimintaympäristössä kokeilukulttuurin omaksuminen on tärkeää. Tutkimusta tulee suunnata uusien konseptien kehittämiseen ja niihin liittyvien liiketoimintamallien seurausten ennakointiin. Keskeistä on tunnistaa asiakastarpeita ja ymmärtää palvelut olennaiseksi osaksi tuotteita. Viranomaistoiminnassa on omaksuttava ennakoiva asenne, joka tukee uusien ratkaisujen syntymistä. Innovatiivisten hankintojen edistäminen on yksi keino toteuttaa muutosta.
