Rapid rise of population migration is a defining feature of the 21st century due to the impact of climate change, political instability, and socioeconomic downturn. Over the last decade, an increasing number of migrant peoples travel across the Americas to reach the United States seeking asylum or cross the border undocumented in search of economic opportunities. In this journey, migrant people experience violations of their human rights, hunger, illness, violence and have limited access to medical care. In the ‘Divine Comedy’, the Italian poet Dante Alighieri depicts his allegorical pilgrimage across Hell and Purgatory to reach Paradise. More than 700 years after its publication, Dante's poem speaks to the present time and the perilious journey of migrant peoples to reach safehavens. By exploring the depths and heights of the human condition, Dante's struggles resonate with the multiple barriers and the unfathomable experiences faced by migrant peoples in transit across South, Central, and North America to reach the United States. Ensuring the safety of migrant peoples across the Americas and elsewhere, and attending to their health needs during their migratory paths represent modern priorities to reduce social injustices and achieving health equity. Patients with coronavirus disease 2019 (COVID-19) frequently develop acute encephalopathy and encephalitis, but whether these complications are the result from viral-induced cytokine storm syndrome or anti-neural autoimmunity is still unclear. In this study, we aimed to evaluate the diagnostic and prognostic role of CSF and serum biomarkers of inflammation (a wide array of cytokines, antibodies against neural antigens, and IgG oligoclonal bands), and neuroaxonal damage (14-3-3 protein and neurofilament light [NfL]) in patients with acute COVID-19 and associated neurologic manifestations (neuro-COVID). We prospectively included 60 hospitalized neuro-COVID patients, 25 (42%) of them with encephalopathy and 14 (23%) with encephalitis, and followed them for 18 months. We found that, compared to healthy controls (HC), neuro-COVID patients presented elevated levels of IL-18, IL-6, and IL-8 in both serum and CSF. MCP1 was elevated only in CSF, while IL-10, IL-1RA, IP-10, MIG and NfL were increased only in serum. Patients with COVID-associated encephalitis or encephalopathy had distinct serum and CSF cytokine profiles compared with HC, but no differences were found when both clinical groups were compared to each other. Antibodies against neural antigens were negative in both groups. While the levels of neuroaxonal damage markers, 14-3-3 and NfL, and the proinflammatory cytokines IL-18, IL-1RA and IL-8 significantly associated with acute COVID-19 severity, only the levels of 14-3-3 and NfL in CSF significantly correlated with the degree of neurologic disability in the daily activities at 18 months follow-up. Thus, the inflammatory process promoted by SARS-CoV-2 infection might include blood-brain barrier disruption in patients with neurological involvement. In conclusion, the fact that the levels of pro-inflammatory cytokines do not predict the long-term functional outcome suggests that the prognosis is more related to neuronal damage than to the acute neuroinflammatory process.

Este libro presenta los resultados de la investigación sobre Justicia Social Educativa (JSE), desarrollada entre docentes y estudiantes del Programa de Filosofía de la Facultad de Educación, del Departamento de Humanidades y Formación Integral, de la Universidad Santo Tomás. Esta se articula con la investigación Fodein sobre Geopolítica de la JSE: oportunidades para disminuir desigualdades, que se realiza entre investigadores del programa de Filosofía y de Humanidades de esta institución. Su estructura se centra en dos ejes: inicialmente presenta una discusión teórica sobre las comprensiones de JSE con una mirada crítica sobre esta en el caso colombiano y la articulación con la filosofía social. En un segundo eje da cuenta de los resultados de la investigación documental, realizada por parte del equipo investigador, en la cual se revisaron políticas públicas educativas de índole nacional y regionales en las que se indaga sobre las comprensiones relacionadas con los elementos centrales de la JSE. Por último, se pretende instalar la discusión sobre la educación en Colombia y su relación con la JSE en los escenarios de posconflicto.

Objective: To report the clinical, neuroimaging, and antibody associations in patients with autoimmune encephalitis (AE) and thymoma. Methods: A retrospective cohort study of 43 patients was conducted. Antibody determination and immunoprecipitation to characterize novel antigens were performed using reported techniques. Results: Patients' median age was 52 years (range: 23-88 years). Forty (93%) had neuronal surface antibodies: gamma-aminobutyric acid receptor A (GABAAR) (15), amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR) (13), contactin-associated protein-like 2 (CASPR2) (4), leucine-rich, glioma inactivated 1 (LGI1) (3), glycine receptor (GlyR) (3), and unknown antigens (2). Concurrent antibodies against intracellular antigens occurred in 13 (30%; 9 anti-collapsin response mediator protein 5 [CRMP5]) and were more frequent in anti-AMPAR encephalitis (54% vs 20%; p = 0.037). The most common clinical presentation was encephalitis with multiple T2/fluid-attenuated inversion recovery hyperintense lesions in 23 (53%) patients (15 GABAAR, 5 AMPAR, and 1 unknown neuropil antibody), followed by encephalitis with peripheral nerve hyperexcitability in 7 (16%; 4 CASPR2, 2 LGI1, and 1 unknown antibody), limbic encephalitis in 6 (14%; 4 AMPAR, 1 LGI1, and 1 antibody negative), progressive encephalomyelitis with rigidity and myoclonus in 4 (9%; 3 GlyR and 1 AMPAR antibodies), and encephalitis with normal MRI in 3 (7%; AMPAR antibodies). Anti-GABAAR encephalitis was more prevalent in Japanese patients compared with Caucasians and other ethnicities (61% vs 16%; p = 0.003). In anti-AMPAR encephalitis, 3/4 patients with poor and 0/6 with good outcome had concurrent CRMP5 antibodies (p = 0.033). Immunoprecipitation studies identified metabotropic glutamate receptor 3 antibodies that were additionally found in 5 patients (3 with and 2 without encephalitis). Conclusions: AE in patients with thymoma include several clinical-radiologic syndromes that vary according to the associated antibodies. Anti-GABAAR encephalitis was the most frequent AE and occurred more frequently in Japanese patients.

Históricamente en Colombia se evidencia que la presencia del Estado en las partes más alejadas del país, llega principalmente por medio de las Fuerzas Militares FFMM (Sepúlveda, 2018). El Ejército, quien hace parte de las FFMM, es una herramienta fundamental para la construcción del Estado puesto que, a través de ellas, el Estado moderno ejerce el monopolio de la fuerza física y adquiere la protección, expansión territorial, consolidación y legitimación estatal (Barany, 2012). Sin embargo, en Colombia la construcción del Estado fue mediada por una élite económica que no le prestó atención al campo militar (Acosta y Cancelado, 2012), llevando a que frecuentemente el Ejército interviniera políticamente en las decisiones estatales (Leal Buitrago, 2002 y Schultze-Kraft, 2012). Dicha intervención, sirvió de pilar para la autonomía castrense, permitiendo diseñar una política militar estatal que estableció las directrices para enfrentar el conflicto armado que ha imperado en el país durante los últimos 70 años. En este contexto, el país ha estado inmerso en una guerra irregular, donde se ha profundizado la soberanía fragmentada, y el poder del Estado ha sido disputado por actores insurgentes y el Ejército, quien representa al Estado en este caso. En este tipo de guerras, las vanguardias no son claramente definidas y no es clara la distinción entre combatientes y civiles (Kalyvas, 2001). Precisamente, al no ser clara la distinción entre combatientes y civiles, el apoyo de la población civil se convierte en un componente de conflicto, debido a que la población civil representa la base de apoyo de los grupos armados – legales e ilegales -.

RESUMEN: Objetivo de la investigación: comprender los significados en salud desde la práctica de medicina ancestral en la comunidad Carare. Metodología: estudio de naturaleza cualitativa, usando la metodología de la investigación dialógica, que permite favorecer el diálogo, la interacción y la comunicación, para lo cual se realizaron: revisión documental, observación participante, entrevistas semi estructuradas y técnicas propias de la Comunidad como círculos de palabra o mambeaderos. Hallazgos: la práctica de medicina de la Comunidad se ha configurado a partir de la mediación desde la espiritualidad que se da a través de las plantas sagradas yagé, mambe y ambil, lo que ha permitido la resignificación de su práctica de medicina a partir de la confluencia de pensamientos, saberes y prácticas en salud con diferentes coordenadas epistemológicas. Así mismo, su práctica de medicina ancestral propone el rescate del sujeto como activo y protagonista de sus procesos de salud, siendo desde su propia vivencia de sanación que se configura como actor en salud. De esta manera la Comunidad ha otorgado su propia significación a conceptos como salud y enfermedad. ABSTRACT : Research objective: to understand the meaning of health from within the Carare community's practice of ancestral medicine inside. Methodology: a qualitative study using the methodology of the dialogical investigation, which favors dialogue, interaction and communication. The methods applied were a documentary review, participant observation, semi-structured interviews and the Carare community's own techniques such as: the circle of the word or mambeaderos. Findings: the practice of community medicine has been reconfigured based on the practice of mediation from the spirituality given by the sacred yage, mambe and ambil plants. They have allowed the resignification from this practice of medicine from the confluence of thoughts, knowledge and practices in health with different epistemological coordinates. Likewise, this practice of ancestral medicine proposes the rescue of the subject as an active and protagonist of his health process, being from his own experience of healing that configures himself as a main actor in the health process. In this way, the community has given its own meaning to concepts such as health and disease.

La localización de descargas parciales resulta esencial para poder evitar que los daños causados por estas descargas sean tan grandes que puedan llegar a romper o inutilizar los aparatos eléctricos en los que se produzcan. Para ello, se aplica el método de localización por antenas, en el cual se mide la diferencia de tiempos de llegadas de las descargas desde que se producen en la fuente eléctrica hasta que se reciben en las diferentes antenas. Para poder calcular las diferencias de tiempos de llegadas, hay que averiguar en qué momento comienza la descarga, para lo que se usan algoritmos de búsqueda de inicio de un pulso como los basados en los cambios de energía acumulada y curtosis. Debido a que las descargas parciales se transmiten por radiofrecuencia, es importante filtrar la señal para eliminar todo el ruido posible y, así, localizar correctamente el punto en el que ocurre la descarga parcial. The localization of partial discharges is of vital importance so as to prevent the discharges from being so considerable that they are able to break or make the electric devices in which the discharges take place, useless. For this localization, an antenna localization method is used. In this method, the time differences of arrival (TDOA) differences of the discharges are measured from the moment they are produced in the electrical source until the partial discharge arrives at the several antennas. In order to calculate the incoming time differences of the discharges, the moment in which the discharge starts is needed to be found, for what pulse start search algorithms, such as the ones based on the cumulative energy and kurtosis changes, are used. Due to the fact that partial discharges are transmitted through radio-frequency, it is essential to filter the signal in order to eliminate as much noise as possible and, therefore, locating the exact instant where the partial discharge takes place properly. Background Cervical cancer (CC) and genital warts (GW) are a significant public health issue in Venezuela. Our objective was to assess the cost-effectiveness of the two available vaccines, bivalent and quadrivalent, against Human Papillomavirus (HPV) in Venezuelan girls in order to inform decision-makers. Methods A previously published Markov cohort model, informed by the best available evidence, was adapted to the Venezuelan context to evaluate the effects of vaccination on health and healthcare costs from the perspective of the healthcare payer in an 11-year-old girls cohort of 264,489. Costs and quality-adjusted life years (QALYs) were discounted at 5%. Eight scenarios were analyzed to depict the cost-effectiveness under alternative vaccine prices, exchange rates and dosing schemes. Deterministic and probabilistic sensitivity analyses were performed. Results Compared to screening only, the bivalent and quadrivalent vaccines were cost-saving in all scenarios, avoiding 2,310 and 2,143 deaths, 4,781 and 4,431 CCs up to 18,459 GW for the quadrivalent vaccine and gaining 4,486 and 4,395 discounted QALYs respectively. For both vaccines, the main determinants of variations in the incremental costs-effectiveness ratio after running deterministic and probabilistic sensitivity analyses were transition probabilities, vaccine and cancer-treatment costs and HPV 16 and 18 distribution in CC cases. When comparing vaccines, none of them was consistently more cost-effective than the other. In sensitivity analyses, for these comparisons, the main determinants were GW incidence, the level of cross-protection and, for some scenarios, vaccines costs. Conclusions Immunization with the bivalent or quadrivalent HPV vaccines showed to be cost-saving or cost-effective in Venezuela, falling below the threshold of one Gross Domestic Product (GDP) per capita (104,404 VEF) per QALY gained. Deterministic and probabilistic sensitivity analyses confirmed the robustness of these results. eDap Documentos de Arquitectura y Patrimonio es una revista digital que se edita con una periodicidad anual en un doble formato: en www.arquitecturaypatrimonio.com y en soporte físico de dvd con una edición limitada, dirigida a universidades y centros públicos de ámbito nacional e internacional. Fue creada en el año 2008 por un grupo de profesores y alumnos vinculados al Departamento de Proyectos Arquitectónicos de la ETSA de Sevilla y ha editado hasta la fecha un total de nueve números de carácter monográfico, cuyas líneas temáticas atienden a: la historia de la cultura como herencia y valor de actualidad; sobre el proyecto y las prospectivas del futuro; de la transversalidad e intertextualidad en la creación contemporánea; del patrimonio como geografía antropizada, como arqueología del paisaje; sobre el patrimonio en peligro y las demandas sociales, entre otras. It was created in 2008 by a group of teachers and students linked to the Department of Architectural Projects of the Technical High School of Architecture of Seville (Spain), and a total of nine monographic issues have been published to date, whose subjects address the following: the history of culture as legacy and value for the present; the project and the prospective studies of the future; the transversal approach and intertextuality in the contemporary creation; the heritage as geography transformed through anthropization, as archaeology of the landscape; the endangered heritage and the social demands, among others. An impression of the intervention is offered from the project,based on the idea of building on what was built before, from the heritage, barely following the Benjaminian determination to “pull out” the tradition from the “conformism that is about to subjugate it”.
