5. The Effects of the Pneumonia Lung Microenvironment on MSC Function.

Abstract

Background: Despite promise in preclinical models of acute respiratory distress syndrome (ARDS), mesenchymal stem cells (MSC) have failed to translate to therapeutic benefit in clinical trials. The MSC is a live cell medicine and interacts with the patient's disease state. Here, we explored this interaction, seeking to devise strategies to enhance MSC therapeutic function. Methods: Human bone-marrow-derived MSCs were exposed to lung homogenate from healthy and E. coli-induced ARDS rat models. Apoptosis and functional assays of the MSCs were performed. Results: The ARDS model showed reduced arterial oxygenation, decreased lung compliance and an inflammatory microenvironment compared to controls. MSCs underwent more apoptosis after stimulation by lung homogenate from controls compared to E. coli, which may explain why MSCs persist longer in ARDS subjects after administration. Changes in expression of cell surface markers and cytokines were associated with lung homogenate from different groups. The anti-microbial effects of MSCs did not change with the stimulation. Moreover, the conditioned media from lung-homogenate-stimulated MSCs inhibited T-cell proliferation. Conclusions: These findings suggest that the ARDS microenvironment plays an important role in the MSC's therapeutic mechanism of action, and changes can inform strategies to modulate MSC-based cell therapy for ARDS. 6. The process of adhering to aerosol therapy in adolescents with cystic fibrosis : patient and parent perspectives

Abstract

Advances in medicine and healthcare have resulted in significant improvements for those children and adolescents living with a chronic health condition. However, the treatments prescribed will only work if they are taken. Rates of adherence to treatment in cystic fibrosis are low. This is especially true for adherence to aerosol therapy, a particular concern considering the reliance on aerosol therapy for the treatment of Pseudomonas aeruginosa. The advent of Adaptive Aerosol Delivery™ (AAD) technology has enabled accurate and objective measurement of adherence to aerosol therapy treatment. However, the factors which influence the process of adhering to aerosol therapy remain largely elusive. Therefore, the current study aims to qualitatively explore the process of adhering to aerosol therapy in a clinical sample of adolescents with CF from both a patient and parent perspective. In addition, objective data downloaded from an AAD device (the I-neb™, Respironics, Chichester, UK) will be used to cue reflections on actual events around adherence. 12 participants were interviewed (six parent-child dyads). The participating adolescents were all aged between 11 and 16 years and had a CF diagnosis. There were five mothers and one father who took part. The parent and adolescent data were analysed separately using the Grounded Theory Method. Nine core categories for the parent data and nine core categories for the adolescent data were developed into a coherent framework and represented as a theoretical formulation which described the process of adhering to aerosol therapy from both a parent and adolescent patient perspective. The theoretical formulation highlights the complexity involved and details the numerous interacting biological, psychological, social and environmental influences on adherence to aerosol therapy. These findings are discussed in relation to the existing literature and clinical implications are considered. The results of the current study complement and extend previous research on adherence in CF.

Additional file 1. Animal procedure schematic. Animals under anaesthesia received intratracheally an E.Coli culture dose establishing the lung injury. One hour later and still under anaesthesia, animals were ventilated using a Flexivent ventilator and EVs or vehicle were delivered using an Aerogen nebuliser connected to the ventilator by a specific module. The system was controlled by a computer allowing to control the delivery internal, and nebulising EVs only in the inspiratory phase to the experimental animals. After this procedure, animals were allowed to recover for the following 48 hours. Finally, animals under anaesthesia were cannulated gaining arterial access and mechanically ventilated to collect different samples such as blood and measure physiological parameters before animals were euthanized.

Abstract

Acute Respiratory Distress Syndrome (ARDS) is a complex and debilitating disease of the lungs, which continues to have a high mortality rate and huge disease burden on patients. Incidence is rising, possibly due to greater awareness leading to more diagnoses rather than a change in the underlying rate. It arises from multiple etiologies, though pathogenic infection, termed pneumonia, is the most prevalent and widely studied. The distinct pathophysiology and rapid evolution of ARDS makes it uniquely challenging with regard to therapeutics development and, to date, no medicines are licensed for specific therapy. Antibiotics, ventilation, and other organ support remain intervention standards.

Abstract

The turtle Angolachelys mbaxi Mateus et al., 2009, is an enigmatic turtle from the Late Cretaceous of Angola which in its description it was erected a new clade, the Angolachelonia Mateus et al., 2009. The phylogenetic position of Angolachelys and Angolachelonia has been questioned and reviewed multiple times since their definition. The new specimen of Angolachelys mbaxi, collected from the same locality as the holotype, consists of postcranial material and a skull, which will not be described in this thesis. The postcranial material is described and studied in this thesis, with the objective of reviewing and ascribing the phylogenetic position of Angolachelys and confirming the validity of Angolachelonia as a clade, to confirm if Angolachelys and the Angolachelonia were in fact of the marine ecology, and therefore suggesting the existence of an evolutionary transition to the marine habit by the Angolachelonia. The paleoecology of Angolachelys mbaxi is studied, namely in respect to the adaptations of its feeding habits. Angolachelys and the Angolachelonia were found in multiple phylogenetic positions, the most prevalent being found as the sister taxon of the Chelonioidea. Angolachelys has been defined as a marine turtle, with features indicative of a more pelagic lifestyle A tartaruga Angolachelys mbaxi Mateus et al., 2009, é uma tartaruga enigmática do Cretáceo Superior de Angola que na sua descrição foi erigida um novo clado, a Angolachelonia. A posição filogenética de Angolachelys e Angolachelonia foi questionada e revista várias vezes desde a sua definição. O novo espécime de Angolachelys mbaxi, recolhido na mesma localidade do holótipo, constituído por material pós-craniano é descrito e estudado nesta tese, com o objetivo de rever e atribuir a posição filogenética de Angolachelys e confirmar a validade de Angolachelonia como clado, confirmar se Angolachelys e a Angolachelonia eram de facto da ecologia marinha, sugerindo assim a existência de uma transição evolutiva para o hábito marinho pela Angolachelonia. É estudada a paleoecologia de Angolachelys mbaxi, nomeadamente no que diz respeito às adaptações dos seus hábitos alimentares. Angolachelys e Angolachelonia foram encontrados em várias posições filogenéticas, a mais prevalente sendo encontrada como o táxon irmão de Chelonioidea. Angolachelys foi definida como uma tartaruga marinha, com características indicativas de um estilo de vida mais pelágico.

This article follows up on the authors' 1988 rejoinder to the symposium 'Budgeting for Improved Productivity,' which appeared in the spring 1987 issue of Public Productivity Review. Using data from a national survey of local government budget practitioners conducted in 1996, 10 years after the first article's survey, the authors investigate trends in the use and importance of budgeting tools related to productivity. In particular, this article critically examines whether the 1996 survey data support the prediction made in the earlier article of an ongoing trend toward greater emphasis on productivity-enhancing management practices in local government.

Abstract

Abstract Objectives Sildenafil citrate, a specific phosphodiesterase-5 inhibitor, is increasingly used for pulmonary hypertension in pregnancy. Sildenafil is also emerging as a potential candidate for the treatment of intra-uterine growth retardation and for premature labor. Its effects in the feto-placental circulation are not known. Our objectives were to determine whether phosphodiesterase-5 is present in the human feto-placental circulation, and to characterize the effects and mechanisms of action of sildenafil citrate in this circulation. Study Design Ex vivo human chorionic plate arterial rings were used in all experiments. The presence of phosphodiesterase-5 in the feto-placental circulation was determined by western blotting and immunohistochemical staining. In a subsequent series of pharmacologic studies, the effects of sildenafil citrate in pre-constricted chorionic plate arterial rings were determined. Additional studies examined the role of cGMP and nitric oxide in mediating the effects of sildenafil. Results Phosphodiesterase-5 mRNA and protein was demonstrated in human chorionic plate arteries. Immunohistochemistry demonstrated phosphodiesterase-5 within the arterial muscle layer. Sildenafil citrate produced dose dependent vasodilatation at concentrations at and greater than 10 nM. Both the direct cGMP inhibitor methylene blue and the cGMP-dependent protein kinase inhibitor Rp-8-Br-PET-cGMPS significantly attenuated the vasodilation produced by sildenafil citrate. Inhibition of NO production with L-NAME did not attenuate the vasodilator effects of sildenafil. In contrast, sildenafil citrate significantly enhanced the vasodilation produced by the NO donor sodium nitroprusside. Conclusion Phosphodiesterase-5 is present in the feto-placental circulation. Sildenafil citrate vasodilates the feto-placental circulation via a cGMP dependent mechanism involving increased responsiveness to NO.
