Abstract

We propose a laser-based method for the preparation of high-energy, highly-polarized electrons, from the ionization of isolated spin-polarized hydrogen (SPH) atoms. The SPH atoms are prepared from the photodissociation of HBr, using two consecutive shaped UV pulses of ps duration. By appropriately timing and focusing the pulses, we can spatially separate the highly polarized SPH from other unwanted photoproducts, which then act as the target for the ion-acceleration lasers. The density of SPH can surpass $\approx$10$^{18}$ cm$^{-3}$, allowing the production of GeV electrons with polarization larger than 90%.

Abstract

BACKGROUND In multimorbid older patients with type 2 diabetes mellitus (T2DM), the intensity of glucose-lowering medication (GLM) should be focused on attaining a suitable level of glycated hemoglobin (HbA1c ) while avoiding side effects. We aimed at identifying patients with overtreatment of T2DM as well as associated risk factors. METHODS In a secondary analysis of a multicenter study of multimorbid older patients, we evaluated HbA1c levels among patients with T2DM. Abstract

Background Although research has been mainly focused on effective treatment for SARS-COV-2 infection, psychosocial aspects of the infection it is vital to be taken into consideration. The aim of the present study is to evaluate the psychosomatic effects and the fear of stigma which patients may face after the end of treatment and discharge from hospital. Subjects and methods This was a non-intervention perspective study conducted in the Department of Infectious Diseases of University General Hospital of Alexandroupolis (Greece). Patients on the discharge day completed questionnaires in which 5 topics were evaluated: pain/discomfort, anxiety/distress, fear/worries, stigma and tolerance of treatment. The questionnaires were derived from similar Quality of Life Tools. The total score of each patient was normalized as percentage. Results Females and younger than 40 years old had more worries and fears on discharge day. Significant factors were days of hospitalization, days of fever and need of oxygen therapy. Patients who hospitalized more than 10 days, particularly in isolation negative pressure rooms, with persistent fever more than 7 days and need of oxygen therapy had more anxiety, worries for their clinical condition and fear of stigma. The majority of patients (80%) were expecting to face moderate to severe problems with family members, friends and colleagues underlying the dimensions of stigma. Conclusions It is crucial to evaluate the psychosocial aspects of this infection and limit the stigma which patients may face returning to their daily routine. Further studies are needed with larger patient series and with the usage of psychometric instruments.

Abstract

Non-alcoholic liver disease (NAFLD) constitutes a global health pandemic. It is estimated that about 25% of the world’s population suffers from NAFLD. In the long-term, a subgroup of the patients can develop inflammation and fibrosis. The end result in some cases is cirrhosis and even liver-related death. The epidemiology and natural history of NAFLD lead to extreme financial costs. To date, there is no approved treatment for NAFLD. Lipotoxicity has been proposed to be one of the main regulators of the implicated molecular pathomechanisms. Research has been focused on the role of cholesterol, free fatty acids and ceramides. Nevertheless, lysophospholipids, such as sphingosine 1-phosphate (S1P), lysophosphatidylcholine (LPC), lysophosphatidic acid (LPA), lysophosphatidylinositol (LPI), lysophosphatidylethanolamine (LPE) have emerged as potential contributors to NAFLD/NASH. Finally, the metabolism of other lysophospholipids, such as lysophosphatidylserine (LPSer), lysophosphatidylglycerol (LPG), and lysocardiolipin (LCL), has come to light in the context of NAFLD. In this review, we try to summarize the current knowledge regarding the potential of lysophospholipid signalling and metabolism as therapeutic targets and biomarkers in NAFLD and/or NASH.

24. Cutaneous Manifestations in Patients with SARS- CoV-2 Infections

Abstract

While SARS-CoV-2 is known to cause pneumonia and acute respiratory distress syndrome (ARDS), many extrapulmonary manifesta- tions of COVID-19 have also been observed. Cutaneous manifestations including erythematous rash, urticaria, and chickenpox-like vesicles have been described in patients with SARS-CoV-2. Six patients, two men and four women, in the age group of 50 to 60 years old, hospitalized with SARS-CoV-2 infection confirmed with real-time polymerase chain reac- tion (real-time PCR) presented cutaneous manifestations. The rash was confluent, spotty, centrifugal, and non-itchy on the head and torso. It was not hemorrhagic, and no crust or blisters were observed. The results of laboratory tests were normal, and the rash disappeared on its own. Sev- eral cases of cutaneous manifestations have been reported in patients with SARS-CoV-2 infection. Further studies are needed in order to assess the skin lesions and determine their association with COVID-19.

Abstract

Background Myasthenia gravis (MG) is a rare autoimmune disorder affecting the neuromuscular junction (NMJ). Here, we investigate the genetic architecture of MG via a genome-wide association study (GWAS) of the largest MG data set analysed to date. Methods We performed GWAS meta-analysis integrating three different data sets (total of 1401 cases and 3508 controls). We carried out human leucocyte antigen (HLA) fine-mapping, gene-based and tissue enrichment analyses and investigated genetic correlation with 13 other autoimmune disorders as well as pleiotropy across MG and correlated disorders. Results We confirmed the previously reported MG association with TNFRSF11A (rs4369774; p=1.09×10-13, OR=1.4). Furthermore, gene-based analysis revealed AGRN as a novel MG susceptibility gene. HLA fine-mapping pointed to two independent MG loci: HLA-DRB1 and HLA-B. MG onset-specific analysis reveals differences in the genetic architecture of early-onset MG (EOMG) versus late-onset MG (LOMG). Furthermore, we find MG to be genetically correlated with type 1 diabetes (T1D), rheumatoid arthritis (RA), late-onset vitiligo and autoimmune thyroid disease (ATD). Cross-disorder meta-analysis reveals multiple risk loci that appear pleiotropic across MG and correlated disorders. Discussion Our gene-based analysis identifies AGRN as a novel MG susceptibility gene, implicating for the first time a locus encoding a protein (agrin) that is directly relevant to NMJ activation. Mutations in AGRN have been found to underlie congenital myasthenic syndrome. Our results are also consistent with previous studies highlighting the role of HLA and TNFRSF11A in MG aetiology and the different risk genes in EOMG versus LOMG. Finally, we uncover the genetic correlation of MG with T1D, RA, ATD and late-onset vitiligo, pointing to shared underlying genetic mechanisms. [ABSTRACT FROM AUTHOR]

Older adults with chronic conditions are at high risk of complications from influenza and pneumococcal infections. Evidence about factors associated with influenza and pneumococcal vaccination among older multimorbid persons in Europe is limited. The aim of this study was to investigate the prevalence and determinants of these vaccinations in this population. Multimorbid patients aged ≥70 years with polypharmacy were enrolled in 4 European centers in Switzerland, Belgium, the Netherlands, and Ireland. Data on vaccinations, demographics, health care contacts, and comorbidities were obtained from self-report, general practitioners and medical records. The association of comorbidities or medical contacts with vaccination status was assessed using multivariable adjusted log-binomial regression models. Among 1956 participants with available influenza vaccination data (median age 79 years, 45% women), 1314 (67%) received an influenza vaccination within the last year. Of 1400 patients with available pneumococcal vaccination data (median age 79 years, 46% women), prevalence of pneumococcal vaccination was 21% (n = 291). The prevalence of vaccination remained low in high-risk populations with chronic respiratory disease (34%) or diabetes (24%), but increased with an increasing number of outpatient medical contacts. Chronic respiratory disease was independently associated with the receipt of both influenza and pneumococcal vaccinations (prevalence ratio [PR] 1.09, 95% confidence interval [CI] 1.03-1.16; and PR 2.03, 95%CI 1.22-3.40, respectively), as was diabetes (PR 1.06, 95%CI 1.03-1.08; PR 1.24, 95%CI 1.16-1.34, respectively). An independent association was found between number of general practitioner visits and higher prevalence of pneumococcal vaccination (p for linear trend <0.001). Abstract

The current severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic warrants an imperative necessity for effective and safe vaccination, to restrain Coronavirus disease 2019 (COVID-19) including transmissibility, morbidity, and mortality. In this regard, intensive medical and biological research leading to the development of an arsenal of vaccines, albeit incomplete preconditioned evaluation, due to emergency. The subsequent scientific gap raises some concerns in the medical community and the general public. More specifically, the accelerated vaccine development downgraded the value of necessary pre-clinical studies to elicit medium- and long-term beneficial or harmful consequences. Previous experience and pathophysiological background of coronaviruses' infections and vaccine technologies, combined with the global vaccines' application, underlined the obligation of a cautious and qualitative approach, to illuminate potential vaccination-related adverse events. Moreover, the high SARS-CoV-2 mutation potential and the already aggregated genetical alterations provoke a rational vagueness and uncertainty concerning vaccines' efficacy against dominant strains and the respective clinical immunity. This review critically summarizes existing evidence and queries regarding SARS-CoV-2 vaccines, to motivate scientists' and clinicians' interest for an optimal, individualized, and holistic management of this unprecedented pandemic.

Abstract

Background: Although research has been mainly focused on effective treatment for SARS-COV-2 infection, psychosocial aspects of the infection it is vital to be taken into consideration. The aim of the present study is to evaluate the psychosomatic effects and the fear of stigma which patients may face after the end of treatment and discharge from hospital. Subjects and methods:This was a non-intervention perspective study conducted in the Department of Infectious Diseases of University General Hospital of Alexandroupolis (Greece). Patients on the discharge day completed questionnaires in which 5 topics were evaluated: pain/discomfort, anxiety/distress, fear/worries, stigma and tolerance of treatment. The questionnaires were derived from similar Quality of Life Tools. The total score of each patient was normalized as percentage. Results: Females and younger than 40 years old had more worries and fears on discharge day. Significant factors were days of hospitalization, days of fever and need of oxygen therapy. Patients who hospitalized more than 10 days, particularly in isolation negative pressure rooms, with persistent fever more than 7 days and need of oxygen therapy had more anxiety, worries for their clinical condition and fear of stigma. The majority of patients (80%) were expecting to face moderate to severe problems with family members, friends and colleagues underlying the dimensions of stigma. Conclusions: It is crucial to evaluate the psychosocial aspects of this infection and limit the stigma which patients may face returning to their daily routine. Further studies are needed with larger patient series and with the usage of psychometric instruments.
