BACKGROUND: Hidradenitis suppurativa (HS) is characterized by periodic worsening of both clinical manifestations and symptoms. The aim of this study was to investigate the role of flare outbreak as a possible predictive factor of response to adalimumab. METHODS: One hundred fifteen HS patients in treatment with adalimumab, with moderate-severe HS, 3 abscesses and inflammatory-nodules (ANs) from 5 Italian centers were included in this retrospective analysis. The information about gender, ages at onset/baseline, therapeutic delay, family history, body mass index, smoking, comorbidities, phenotypes, body areas, severity indexes at baseline was collected. Baseline characteristics, total number and timeline of flares were analyzed by regression and survival analysis with Hidradenitis Suppurativa Clinical Response (HiSCR). RESULTS: During the observational period, 80.9% of patients developed flares, detecting 252 flares. Univariate model identified five factors as-sociated with the absence of response: age (P=0.020), comorbidities (P=0.030), genital-perineal involvement (P=0.004), no response at week 12 (P=0.027), and flares outbreak (P=0.010). Joint analysis of recurrent and terminal events showed a positive correlation between flare recurrence and no response (P<0.001). Among the identified variables associated with poor response to the therapy: occurrence of a flare before week 12 was the one with the highest risk of no response (P<0.001). CONCLUSIONS: The analysis of a "dynamic" variable, as flares evaluation together with an appropriate clinical baseline assessment can be a useful approach to predict the middle-long-term response to adalimumab. Le lesioni della mucosa orale in corso di malattia bollosa autoimmune sono tra le più difficili da gestire e tendono a recidivare con maggiore frequenza rispetto alle lesioni cutanee, con notevole impatto sulla qualità di vita. Attualmente il PRP è stato approvato anche per le lesioni aperte cutanee e mucose. Il meccanismo d'azione si basa sull nota capacità riparativa, strutturale e funzionale, e sull sua capacità di stimolare la proliferazione e la rigenerazione tissutale. Si tratta inoltre di una tecnica sicura che finora non ha mostrato alcun effetto collaterale. Attualmente abbiamo utilizzato il gel piastrinico su 10 pazienti affetti da MBA e, nonostante il numero ridotto, è stato possibile effettuare una prima valutazione circa l’efficacia ma anche i limiti di tale trattamento. Tutti i pazienti hanno mostrato un rapido miglioramento della sintomatologia dolorosa con conseguente miglioramento della qualità della vita., Mucosal Lesions in patients with autoimmune bullous diseases are the most difficult to manage and tend to relapse more frequently than skin lesions. Consequentially, these lesions have a very important impact on quality of life of affected patients. PRP has recently been approved for the treatment of dermatological diseases characterized by difficult-to-treat mucosal and cutaneous ulcerated lesions. The mechanism of action is based on the its well-known repairing capacity (structural and functional) and on its ability to stimulate tissue proliferation and regeneration. PRP is very safety and side effects have not been described so far. We have currently used platelet gel on 10 patients with MBA and, despite the small number, it was possible to observe an initial assessment of the effectiveness but also the limits of this treatment. All patients showed a rapid improvement in pain symptoms with a consequent improvement in the quality of life.

Background: Insights into the real-world treatment paradigm and long-term burden of atopic dermatitis (AD) are needed to inform clinical and health policy decisions. Methods: The prospective, observational EUROSTAD study enrolled adults with moderate-to-severe AD starting or switching systemic therapy (51 sites in 10 European countries). We report the baseline characteristics, treatment patterns, and outcomes of these patients using descriptive statistics. Results: A 12-month enrollment period of EUROSTAD was completed and 308 patients were enrolled: average age 37 years, AD duration 25 years, 43% were female. Most patients reported use of systemic therapy (93%) and ≥1 atopic comorbidity (82%). Mean [standard deviation] disease severity/burden measures were high: Investigator’s Global Assessment (3.1 [0.8]), Eczema Area and Severity Index (16.2 [10.9]), Peak Pruritus Numerical Rating Scale (5.5 [2.5]), sleep impairment Visual Analog Scale (49.8 [31.6]) scores, and time lost from work (4.1 [13.7] days/year) or usual activities (16.8 [38.7] days/year). Most patients showed borderline or clinical levels of anxiety (59%) and/or depression (63%) using the Hospital Anxiety and Depression Scale. Conclusions: Adults with moderate-to-severe AD starting/switching systemic treatment enrolled in EUROSTAD have a high burden of longstanding disease despite continuous use of topical drugs, emollients, and systemic therapies.

INTRODUZIONE: Il Lichen Sclerosus è una dermatosi autoimmune, cronico-recidivante che coinvolge tipicamente i genitali. Questa patologia può avere un forte impatto sulla qualità della vita e sull’attività sessuale dei pazienti: molti di questi hanno sintomatologia pruriginosa, bruciore, dispareunia, disturbi della sfera sessuale, spesso persistenti anche dopo le terapie. Ad oggi, la terapia steroidea è la più efficace opzione terapeutica, sebbene non sia scevra da effetti collaterali, soprattutto nelle applicazioni a lungo termine. OBIETTIVI: Indagare l’efficacia e gli effetti a lungo termine della terapia con laser Co2 frazionato nel controllo della sintomatologia del lichen scleroso femminile e maschile, in pazienti che applichino lo steroide 1-2 volte alla settimana, per valutare se questo trattamento possa essere una valida alternativa alla terapia steroidea. METODI: Sono stati arruolati nello studio prospettico pazienti di entrambi i sessi affetti da lichen scleroso. I pazienti sono stati trattati con laser CO2 frazionato ogni 2 mesi, per un totale di 3 sessioni. Prima di ogni trattamento e al follow up (9 mesi dopo l’ultima seduta) sono stati somministrati ai pazienti dei questionari: DLQI,FSFI per le donne e MSHQ per gli uomini, e infine 2 questionari specifici creati per valutare la severità di segni e sintomi della patologia basati sullo score proposto da Günthert nel 2012. RISULTATI: 23 donne e 10 uomini, tutti maggiorenni, sono stati arruolati nello studio. La terapia con laser Co2 frazionato ha migliorato in maniera significatica I punteggi di tutti I questionari, dalla baseline fino al follow up. Nessuno ha mostrato effetti collaterali importanti. CONCLUSIONI: I nostri risultati suggeriscono che il laser Co2 frazionato sia un trattamento efficace e sicuro ed una valida alternativa alla terapia steroidea nei pazienti affetti da lichen scleroso genitale. Maggiori studi sono comunque necessari per confermare questi risultati., BACKGROUND: Lichen Sclerosus is an autoimmune, chronic relapsing dermatosis that typically involves vulvar and penile area. This condition can strongly impact on the quality of life: many patients feel uncomfortable, some have persistent discomfort despite apparently successful treatment. To date, topical steroids are the most effective therapeutic option, although adverse effects are possible, especially in long-term application. OBJECTIVES: To investigate the efficacy and the long-term effect of fractional CO2 laser as a steroid sparing treatment to control the symptoms in women an men with lichen sclerosus who have tapered the application of topical steroids to twice weekly or less. METHODS: Patients with a clinical and histological diagnosis of vulvar and penile lichen sclerosus were prospectively enrolled in the study. The patients were treated with fractional CO2 laser every 2 months, for a total of 3 sessions. Before each treatment and at the 8-month follow up 4 questionnaires were administered to the patients: DLQI, FSFI for women, MSHQ for men, and 2 specific questionnaires created to assess the severity of signs and symptoms, based on the score proposed by Günthert in 2012. RESULTS: A total of 23 adult women and 10 adult men were enrolled in the study. The fractional CO2 laser treatment significantly improved the scores of all scales from baseline to the 8-month follow up. CONCLUSIONS: Our results suggest that fractional CO2 laser is an efficient and safe steroid sparing treatment in patients that have tapered topical steroids to twice weekly or less. However, further investigations are needed to safely generalized our findings.

La psoriasi è una malattia infiammatoria cronica cutanea che, a seconda della gravità, può essere classificata in lieve, moderata e severa. Per le forme severe, sono stati recentemente introdotti i farmaci biologici, che determinano un’ottima clearance delle lesioni cutanee, aumentando quindi la sensazione di guarigione del paziente. Tali farmaci hanno regimi di somministrazione ben stabiiti, tuttavia, alcuni studi hanno proposto schemi di somministrazione dilazionati per migliorare la qualità di vita del paziente psoriasico. Sono stati arruolati soggetti con diagnosi di psoriasi cutanea moderata-grave e/o artropatica in trattamento con uno dei quattro principali farmaci biologico (infliximab, etanercept, adalimumab e ustekinumab), che avessero raggiunto un PASI100, mantenuto per almeno 12 mesi consecutivi, presso 3 centri italiani. Per tali pazienti, è stato allungato il tempo di somministrazione. È stata valutata la percentuale di recidive e l’associazione con ipertensione, diabete mellito, artropatia psoriasica o con l’abitudine al tabagismo. La sensazione di guarigione è misurata con un questionario somministrato a 3 mesi dal baseline. Sono stati analizzati in totale 199 pazienti in terapia con farmaco biologico da gennaio 2005 a giugno 2018. Sono stati confrontati 96 pazienti che hanno effettuato una riduzione della dose con 103 trattati secondo foglietto illustrativo. La differenza nell’incidenza di recidive tra i due gruppi è apparsa essere trascurabile (p-value 0,445). L’ipertensione arteriosa appariva essere correlata a una più alta percentuale di recidive (p-value<0.05). I farmaci biologici determinano un’ottima risposta terapeutica, associata spesso a una completa remissione della malattia. Il nostro studio ha dimostrato come la somministrazione dilazionata dei farmaci biologici mantenga la medesima efficacia della somministrazione standard e trovi anche ampio consenso da parte del paziente stesso. Ulteriori studi su più ampie casistiche per valutare l’ef, Psoriasis is a cutaneous chronic inflammatory disease which can be classified in mild, moderate or severe, according with the skin involvement. For the severe forms, biological therapies have been recently introduced. These therapies often cause a complete clearance of the skin lesions, but they have specific and rigid dose administration regimens. Recently, some studies have proposed a dose tapering to improve the patient's quality of life and reduce the health costs. We considered in this study 199 patients treated with one biological therapy among infliximab, etanercept, adalimumab or ustekinumab, from January 2005 through June 2018. Of them, 96 reached a complete clearance of the lesions (PASI100) maintained for at least 12 months. In this group, we elongated the administration period of the used biologic and we administered a questionnaire about the “healing sensation”. In the other 103 patients, who did not reach the PASI100, we maintained the therapy at the standard dose. We also evaluated the percentage of relapses among the two groups and the association between the incidence of relapses and hypertension, diabetes, psoriatic arthritis or smoking habits. The difference in terms of relapses between the two groups appeared negligible (p-value 0.445) while hypertension has been related to a higher percentage of psoriasis recurrence in both groups. In the group of dose tapering, most patients (61,70%) reported high satisfaction and “healing sensation” after the dose tapering of the biologic. Biological therapies cause often a good therapeutic response or, in most cases, a complete remission of the disease. Our study demonstrated that a dose tapering of biologics maintain good results in terms of efficacy and it is well accepted by the psoriatic patients. More studies are needed to evaluate the long-term efficacy of these new therapeutic regimens of biologics and to study the newly introduced parameter of “healing sensation”.
