Abstract

BACKGROUND: Kidney transplant (KT) candidates with HIV face higher mortality on the waitlist compared with candidates without HIV. Because the HIV Organ Policy Equity (HOPE) Act has expanded the donor pool to allow donors with HIV (D + ), it is crucial to understand whether this has impacted transplant rates for this population. METHODS: Using a linkage between the HOPE in Action trial (NCT03500315) and Scientific Registry of Transplant Recipients, we identified 324 candidates listed for D + kidneys (HOPE) compared with 46 025 candidates not listed for D + kidneys (non-HOPE) at the same centers between April 26, 2018, and May 24, 2022. We characterized KT rate, KT type (D + , false-positive [FP; donor with false-positive HIV testing], D - [donor without HIV], living donor [LD]) and quantified the association between HOPE enrollment and KT rate using multivariable Cox regression with center-level clustering; HOPE was a time-varying exposure. Abstract

BACKGROUND: In 2021, four patients who had received solid organ transplants in the USA developed encephalitis beginning 2-6 weeks after transplantation from a common organ donor. We describe an investigation into the cause of encephalitis in these patients. METHODS: From Nov 7, 2021, to Feb 24, 2022, we conducted a public health investigation involving 15 agencies and medical centres in the USA. We tested various specimens (blood, cerebrospinal fluid, intraocular fluid, serum, and tissues) from the organ donor and recipients by serology, RT-PCR, immunohistochemistry, metagenomic next-generation sequencing, and host gene expression, and conducted a traceback of blood transfusions received by the organ donor. FINDINGS: We identified one read from yellow fever virus in cerebrospinal fluid from the recipient of a kidney using metagenomic next-generation sequencing. Recent infection with yellow fever virus was confirmed in all four organ recipients by identification of yellow fever virus RNA consistent with the 17D vaccine strain in brain tissue from one recipient and seroconversion after transplantation in three recipients. Two patients recovered and two patients had no neurological recovery and died. 3 days before organ procurement, the organ donor received a blood transfusion from a donor who had received a yellow fever vaccine 6 days before blood donation. INTERPRETATION: This investigation substantiates the use of metagenomic next-generation sequencing for the broad-based detection of rare or unexpected pathogens. Health-care workers providing vaccinations should inform patients of the need to defer blood donation for at least 2 weeks after receiving a yellow fever vaccine. Despite mitigation strategies and safety interventions, a low risk of transfusion-transmitted infections remains. Abstract

In this work, we propose a novel safe and scalable decentralized solution for multi-agent control in the presence of stochastic disturbances. Safety is mathematically encoded using stochastic control barrier functions and safe controls are computed by solving quadratic programs. Decentralization is achieved by augmenting to each agent's optimization variables, copy variables, for its neighbors. This allows us to decouple the centralized multi-agent optimization problem. However, to ensure safety, neighboring agents must agree on "what is safe for both of us" and this creates a need for consensus. To enable safe consensus solutions, we incorporate an ADMM-based approach. Specifically, we propose a Merged CADMM-OSQP implicit neural network layer, that solves a mini-batch of both, local quadratic programs as well as the overall consensus problem, as a single optimization problem. This layer is embedded within a Deep FBSDEs network architecture at every time step, to facilitate end-to-end differentiable, safe and decentralized stochastic optimal control. The efficacy of the proposed approach is demonstrated on several challenging multi-robot tasks in simulation. By imposing requirements on safety specified by collision avoidance constraints, the safe operation of all agents is ensured during the entire training process. We also demonstrate superior scalability in terms of computational and memory savings as compared to a centralized approach.

Abstract

BackgroundOrgan transplantation from donors with human immunodeficiency virus (HIV) to recipients with HIV (HIV D+/R+) presents risks of donor-derived infections. Understanding clinical, immunologic, and virologic characteristics of HIV-positive donors is critical for safety.MethodsWe performed a prospective study of donors with HIV-positive and HIV false-positive (FP) test results within the HIV Organ Policy Equity (HOPE) Act in Action studies of HIV D+/R+ transplantation (ClinicalTrials.gov NCT02602262, NCT03500315, and NCT03734393). We compared clinical characteristics in HIV-positive versus FP donors. We measured CD4 T cells, HIV viral load (VL), drug resistance mutations (DRMs), coreceptor tropism, and serum antiretroviral therapy (ART) detection, using mass spectrometry in HIV-positive donors.ResultsBetween March 2016 and March 2020, 92 donors (58 HIV positive, 34 FP), representing 98.9% of all US HOPE donors during this period, donated 177 organs (131 kidneys and 46 livers). Each year the number of donors increased. The prevalence of hepatitis B (16% vs 0%), syphilis (16% vs 0%), and cytomegalovirus (CMV; 91% vs 58%) was higher in HIV-positive versus FP donors; the prevalences of hepatitis C viremia were similar (2% vs 6%). Abstract

In this paper, we introduce a novel deep learning based solution to the Powered-Descent Guidance (PDG) problem, grounded in principles of nonlinear Stochastic Optimal Control (SOC) and Feynman-Kac theory. Our algorithm solves the PDG problem by framing it as an $\mathcal{L}^1$ SOC problem for minimum fuel consumption. Additionally, it can handle practically useful control constraints, nonlinear dynamics and enforces state constraints as soft-constraints. This is achieved by building off of recent work on deep Forward-Backward Stochastic Differential Equations (FBSDEs) and differentiable non-convex optimization neural-network layers based on stochastic search. In contrast to previous approaches, our algorithm does not require convexification of the constraints or linearization of the dynamics and is empirically shown to be robust to stochastic disturbances and the initial position of the spacecraft. After training offline, our controller can be activated once the spacecraft is within a pre-specified radius of the landing zone and at a pre-specified altitude i.e., the base of an inverted cone with the tip at the landing zone. We demonstrate empirically that our controller can successfully and safely land all trajectories initialized at the base of this cone while minimizing fuel consumption.

Abstract

This paper introduces a new formulation for stochastic optimal control and stochastic dynamic optimization that ensures safety with respect to state and control constraints. The proposed methodology brings together concepts such as Forward-Backward Stochastic Differential Equations, Stochastic Barrier Functions, Differentiable Convex Optimization and Deep Learning. Using the aforementioned concepts, a Neural Network architecture is designed for safe trajectory optimization in which learning can be performed in an end-to-end fashion. Simulations are performed on three systems to show the efficacy of the proposed methodology.

Abstract

In this work we propose the use of adaptive stochastic search as a building block for general, non-convex optimization operations within deep neural network architectures. Specifically, for an objective function located at some layer in the network and parameterized by some network parameters, we employ adaptive stochastic search to perform optimization over its output. This operation is differentiable and does not obstruct the passing of gradients during backpropagation, thus enabling us to incorporate it as a component in end-to-end learning. We study the proposed optimization module's properties and benchmark it against two existing alternatives on a synthetic energy-based structured prediction task, and further showcase its use in stochastic optimal control applications.

Abstract

Systems involving Partial Differential Equations (PDEs) have recently become more popular among the machine learning community. However prior methods usually treat infinite dimensional problems in finite dimensions with Reduced Order Models. This leads to committing to specific approximation schemes and subsequent derivation of control laws. Additionally, prior work does not consider spatio-temporal descriptions of noise that realistically represent the stochastic nature of physical systems. In this paper we suggest a new reinforcement learning framework that is mostly model-free for Stochastic PDEs with additive spacetime noise, based on variational optimization in infinite dimensions. In addition, our algorithm incorporates sparse representations that allow for efficient learning of feedback policies in high dimensions. Abstract

This paper presents a novel approach to numerically solve stochastic differential games for nonlinear systems. The proposed approach relies on the nonlinear Feynman-Kac theorem that establishes a connection between parabolic deterministic partial differential equations and forward-backward stochastic differential equations. Using this theorem the Hamilton-Jacobi-Isaacs partial differential equation associated with differential games is represented by a system of forward-backward stochastic differential equations. Numerical solution of the aforementioned system of stochastic differential equations is performed using importance sampling and a Long-Short Term Memory recurrent neural network, which is trained in an offline fashion. The resulting algorithm is tested on two example systems in simulation and compared against the standard risk neutral stochastic optimal control formulations.

Abstract

We present a deep recurrent neural network architecture to solve a class of stochastic optimal control problems described by fully nonlinear Hamilton Jacobi Bellmanpartial differential equations. Such PDEs arise when one considers stochastic dynamics characterized by uncertainties that are additive and control multiplicative. Stochastic models with the aforementioned characteristics have been used in computational neuroscience, biology, finance and aerospace systems and provide a more accurate representation of actuation than models with additive uncertainty. Previous literature has established the inadequacy of the linear HJB theory and instead rely on a non-linear Feynman-Kac lemma resulting in a second order forward-backward stochastic differential equations representation. However, the proposed solutions that use this representation suffer from compounding errors and computational complexity leading to lack of scalability. In this paper, we propose a deep learning based algorithm that leverages the second order Forward-Backward SDE representation and LSTM based recurrent neural networks to not only solve such Stochastic Optimal Control problems but also overcome the problems faced by previous approaches and scales well to high dimensional systems. The resulting control algorithm is tested on non-linear systems in robotics and biomechanics to demonstrate feasibility and out-performance against previous methods.

Abstract

Stochastic Spatio-Temporal processes are prevalent across domains ranging from modeling of plasma to the turbulence in fluids to the wave function of quantum systems. This letter studies a measure-theoretic description of such systems by describing them as evolutionary processes on Hilbert spaces, and in doing so, derives a framework for spatio-temporal manipulation from fundamental thermodynamic principles. This approach yields a variational optimization framework for controlling stochastic fields. The resulting scheme is applicable to a wide class of spatio-temporal processes and can be used for optimizing parameterized control policies. 16. Learning Deep Stochastic Optimal Control Policies using Forward-Backward SDEs

Abstract

In this paper we propose a new methodology for decision-making under uncertainty using recent advancements in the areas of nonlinear stochastic optimal control theory, applied mathematics, and machine learning. Grounded on the fundamental relation between certain nonlinear partial differential equations and forward-backward stochastic differential equations, we develop a control framework that is scalable and applicable to general classes of stochastic systems and decision-making problem formulations in robotics and autonomy. The proposed deep neural network architectures for stochastic control consist of recurrent and fully connected layers. The performance and scalability of the aforementioned algorithm are investigated in three non-linear systems in simulation with and without control constraints. We conclude with a discussion on future directions and their implications to robotics.

Abstract

This paper develops a variational inference framework for control of infinite dimensional stochastic systems. We employ a measure theoretic approach which relies on the generalization of Girsanov's theorem, as well as the relation between relative entropy and free energy. The derived control scheme is applicable to a large class of stochastic, infinite dimensional systems, and can be used for trajectory optimization and model predictive control. Our work opens up new research avenues at the intersection of stochastic control, inference and information theory for dynamical systems described by stochastic partial differential equations.

Abstract

In this paper we investigate the use of MPC-inspired neural network policies for sequential decision making. We introduce an extension to the DAgger algorithm for training such policies and show how they have improved training performance and generalization capabilities. We take advantage of this extension to show scalable and efficient training of complex planning policy architectures in continuous state and action spaces. We provide an extensive comparison of neural network policies by considering feed forward policies, recurrent policies, and recurrent policies with planning structure inspired by the Path Integral control framework. Abstract

BACKGROUND: In 2021, four patients who had received solid organ transplants in the USA developed encephalitis beginning 2-6 weeks after transplantation from a common organ donor. We describe an investigation into the cause of encephalitis in these patients. METHODS: From Nov 7, 2021, to Feb 24, 2022, we conducted a public health investigation involving 15 agencies and medical centres in the USA. We tested various specimens (blood, cerebrospinal fluid, intraocular fluid, serum, and tissues) from the organ donor and recipients by serology, RT-PCR, immunohistochemistry, metagenomic next-generation sequencing, and host gene expression, and conducted a traceback of blood transfusions received by the organ donor. FINDINGS: We identified one read from yellow fever virus in cerebrospinal fluid from the recipient of a kidney using metagenomic next-generation sequencing. Recent infection with yellow fever virus was confirmed in all four organ recipients by identification of yellow fever virus RNA consistent with the 17D vaccine strain in brain tissue from one recipient and seroconversion after transplantation in three recipients. Two patients recovered and two patients had no neurological recovery and died. 3 days before organ procurement, the organ donor received a blood transfusion from a donor who had received a yellow fever vaccine 6 days before blood donation. INTERPRETATION: This investigation substantiates the use of metagenomic next-generation sequencing for the broad-based detection of rare or unexpected pathogens. Health-care workers providing vaccinations should inform patients of the need to defer blood donation for at least 2 weeks after receiving a yellow fever vaccine. Despite mitigation strategies and safety interventions, a low risk of transfusion-transmitted infections remains. Abstract

A neuropatia diabética é uma complicação crônica comum do diabetes mellitus que pode levar a danos sensoriais, motores e autonômicos. O teste do monofilamento de Semmes-Weinstein é uma ferramenta amplamente utilizada para avaliação da sensibilidade cutânea em pacientes com neuropatia diabética. Este artigo apresenta uma revisão integrativa da literatura com o objetivo de avaliar a eficácia do teste do monofilamento na avaliação da sensibilidade em pacientes com neuropatia diabética. Foram selecionados oito artigos publicados nos últimos cinco anos nas bases de dados Google Scholar, LILACS e Scielo. Os estudos indicam que o teste do monofilamento é uma ferramenta confiável e eficaz para o diagnóstico precoce e monitoramento da neuropatia diabética, apresentando alta sensibilidade e especificidade. Além disso, a padronização da técnica e o treinamento adequado dos profissionais que aplicam o teste são essenciais para garantir a sua validade e confiabilidade. Os resultados desta revisão integrativa reforçam a importância do teste do monofilamento na avaliação da sensibilidade em pacientes com neuropatia diabética.

Abstract

Introdução: A otite média é uma das infecções mais comuns em crianças e pode levar a complicações graves, incluindo complicações neurológicas. Este estudo teve como objetivo revisar a literatura atual sobre as complicações neurológicas da otite média em crianças.Métodos: Foi realizada uma busca eletrônica nas bases de dados PubMed e Scopus para artigos publicados entre 2010 e 2021. Foram incluídos estudos que relataram as complicações neurológicas da otite média em crianças com idade inferior a 18 anos.Resultados: Foram identificados 20 estudos que atenderam aos critérios de inclusão. As complicações neurológicas mais comuns da otite média em crianças foram meningite, abscesso cerebral e labirintite. A prevalência das complicações neurológicas variou entre os estudos, mas foi mais comum em crianças menores de 2 anos. Os fatores de risco para o desenvolvimento de complicações neurológicas incluíram a gravidade da otite média e a presença de doenças subjacentes.Conclusão: As complicações neurológicas da otite média em crianças podem levar a morbidade significativa e até mesmo mortalidade. É importante que os profissionais de saúde estejam cientes dessas complicações e adotem medidas preventivas para reduzir o risco de ocorrência.

Introdução: Pacientes em surto psiquiátrico apresentam risco iminente de danos a si mesmos e a terceiros, exigindo um manejo adequado e eficiente no atendimento de urgência.Objetivo: O presente estudo teve como objetivo revisar as diretrizes brasileiras para o manejo do surto psiquiátrico em serviços de emergência e discutir a conduta adequada após o surto.Métodos: Foi realizada uma revisão de literatura em bases de dados eletrônicas, utilizando as palavras-chave “surto psicótico”, “emergência psiquiátrica”, “diretrizes”, “tratamento” e “cuidados pós-surtos”.Resultados: As diretrizes brasileiras indicam o uso de antipsicóticos de segunda geração para o manejo agudo do surto, além de medidas de contenção física e cuidados de suporte. Após o surto, são recomendados acompanhamento psiquiátrico, terapia ocupacional e suporte social e familiar.Conclusão: O manejo adequado do surto psiquiátrico é fundamental para prevenir danos e melhorar a qualidade de vida dos pacientes. O tratamento deve ser individualizado e baseado nas diretrizes brasileiras, com atenção especial aos cuidados pós-surto.

Abstract

O acidente isquêmico transitório (AIT) é uma emergência neurológica comum que pode levar a sequelas graves se não tratado. O tratamento agudo inclui a administração de medicamentos para prevenir a recorrência do AIT e o controle de fatores de risco modificáveis, como hipertensão e diabetes. Neste estudo, realizamos uma revisão da literatura para identificar as sequelas neurológicas mais comuns após um AIT, bem como os medicamentos de escolha para prevenir sua recorrência. Concluímos que o tratamento com clopidogrel e aspirina é eficaz na redução de eventos vasculares recorrentes em pacientes com AIT, além de ser seguro e bem tolerado. Outras intervenções, como terapia anti-hipertensiva e anti-diabética, também podem ajudar a reduzir o risco de recorrência do AIT e melhorar o prognóstico dos pacientes.

Objetivo: Este artigo teve como objetivo revisar a literatura científica atual sobre o manejo clínico de pacientes pediátricos portadores de autismo em crise convulsiva na emergência. Métodos: Foi realizada uma revisão integrativa de artigos publicados nos últimos cinco anos nas bases de dados MEDLINE, LILACS e SciELO, utilizando os descritores “autismo”, “convulsões” e “emergência”. Foram selecionados 15 artigos para a análise. Resultados: Os artigos revisados demonstraram a importância de uma abordagem individualizada e adaptada às características do paciente com autismo em crise convulsiva na emergência, incluindo uma equipe multidisciplinar capacitada e treinada para lidar com esse tipo de situação. As diretrizes brasileiras de pediatria enfatizam a necessidade de uma avaliação clínica completa, incluindo o histórico do paciente, sinais vitais e exames complementares, além do uso adequado de medicações antiepilépticas. Conclusão: O manejo clínico de pacientes pediátricos com autismo em crise convulsiva na emergência exige uma abordagem individualizada e multidisciplinar. As diretrizes brasileiras de pediatria fornecem orientações importantes para o atendimento a esses pacientes, incluindo avaliação clínica completa e uso adequado de medicações antiepilépticas.

Abstract

This study evaluates whether 853 municipalities of Minas Gerais have developed one specific strategy of electronic government. We searched for municipalities that have websites for the executive branch, relating this information to a number of secondary data. These data include variables on the Municipal Human Development Index (HDI-M), municipal budgetary revenues, year of founding of municipalities, the mayor’s party affiliation, and a public policy initiative related to the creation of municipal executive branch websites. This analysis was performed to develop descriptive statistics among the variables collected. In addition to exploring descriptive analysis, we attempted to determine statistical models that explain the existence of websites for municipal executives. This allowed us to determine whether municipalities that were established earlier than others, have the highest HDI-M, and have larger budgets are also more likely to have websites. The results indicate that older municipalities, as well as those with higher HDI-M and elevated budgetary revenues, are more likely to have websites. A mayor’s party affiliation indicates that municipalities administered by the Workers’ Party (Partido dos Trabalhadores, or PT, the center-left-wing party) are more likely to have websites than municipalities governed by the Brazilian Social Democratic Party (Partido da Social Democracia Brasileira, or PSDB, the center-right-wing party). Furthermore, municipalities that use the state government program Integra Minas (Connect Minas) are less likely to have their own websites, which is an indication that these locations are using policy resources to communicate electronically with the population. Thus, newly founded municipalities with low HDI-M, with low budgetary revenue, and administered by center-right-wing parties may benefit from public policies that encourage greater transparency in the communication of municipal executives with their population.

47. Tabla Estructural para el Desarrollo de Estrategias de Gamificación - TEDEG: una propuesta de recurso didáctico-pedagógico para soluciones de aprendizaje gamificadas

Abstract

Gamification, a methodology based on the insertion of elements, mechanics, design and/or game narrative in other contexts, presents itself as an active methodology option capable of keeping the student motivated and engaged in their learning process. As few works about gamification assume the practical character of changing the methodological perspective from passive to active, it worries us to know how the teacher can develop a gamification strategy so that motivation and engagement favor the leading role by the students. To answer this question, this article presents the Structural Table for the Development of Gamification Strategies (STDGS) as a didactic-pedagogical resource that allows the teacher to organize and structure a methodological gamification strategy that combines game elements, motivation, engagement and models motivational learning. STDGS presents a structural plan and an organizational plan in order to establish the necessary relationships for a gamified methodological structure that does not focus only on entertainment and effectively favors the learning of students. From this perspective, we understand that it is a valuable resource for teachers who aim to develop gamification strategies, although it can be improved according to the teachers’ needs. La gamificación, metodología basada en la inserción de elementos, mecánicas, diseño y/o narrativa del juego en otros contextos, se presenta como una opción metodológica activa capaz de mantener al estudiante motivado e involucrado en su proceso de aprendizaje. Como pocos trabajos sobre gamificación asumen el carácter práctico de cambiar la perspectiva metodológica de pasiva a activa, nos preocupa saber cómo el docente puede desarrollar una estrategia de gamificación para que la motivación y el compromiso favorezcan el protagonismo de los alumnos. Para responder a esta pregunta, este artículo presenta la Tabla Estructural para el Desarrollo de Estrategias de Gamificación (TEDEG) como un recurso didáctico-pedagógico que permite al docente organizar y estructurar una estrategia metodológica de gamificación que combina elementos de juego, motivación, compromiso y modelos de aprendizaje motivacional. La TEDEG presenta un plan estructural y un plan organizativo con el fin de establecer las relaciones necesarias para una estructura metodológica gamificada que no se centre solo en el entretenimiento y favorezca de forma efectiva el aprendizaje de los alumnos. Desde esta perspectiva, entendemos que es un recurso valioso para los docentes que pretenden desarrollar estrategias de gamificación, aunque puede mejorarse según las necesidades de los docentes. A gamificação, metodologia baseada na inserção de elementos, da mecânica, do design e/ou da narrativa de jogos em outros contextos, se apresenta como opção ativa capaz de manter o aluno motivado e engajado em seu processo de aprendizagem. Como poucos trabalhos sobre gamificação assumem o caráter prático de mudança do viés metodológico passivo para ativo, nos inquieta saber como o professor pode desenvolver uma estratégia de gamificação para que a motivação e o engajamento favoreçam o protagonismo dos alunos. Para responder a essa pergunta, este artigo apresenta a Tabela Estrutural de Desenvolvimento de Estratégias de Gamificação (TEDEG) como recurso didático-pedagógico que permite ao professor organizar e estruturar uma estratégia de gamificação que combine elementos de jogos, motivação, engajamento e modelos motivacionais de aprendizagem. A TEDEG apresenta um plano estrutural e um plano organizacional com intuito de estabelecer as relações necessárias a uma estrutura metodológica gamificada que não foque apenas no entretenimento e favoreça de forma efetiva a aprendizagem dos alunos. Nessa perspectiva, entendemos que ela é um importante recurso para professores que almejam desenvolver estratégias de gamificação, ainda que possa ser aprimorada de acordo com as necessidades do professor.
