Abstract

BackgroundTwo congenital bleeding diatheses have been identified in Thoroughbred horses: Glanzmann thrombasthenia (GT) and a second, novel diathesis associated with abnormal platelet function in response to collagen and thrombin stimulation.Hypothesis/objectivesPlatelet dysfunction in horses with this second thrombasthenia results from a secretory defect.AnimalsTwo affected and 6 clinically normal horses.MethodsEx vivo study. Washed platelets were examined for (1) expression of the αIIb-β3 integrin; (2) fibrinogen binding capacity in response to ADP and thrombin; (3) secretion of dense and α-granules; (4) activation of the mammalian target of rapamycin (mTOR)-protein kinase B (AKT) signaling pathway; and (5) cellular distribution of phosphatidylinositol-4-phosphate-3-kinase, class 2B (PIK3C2B) and SH2 containing inositol-5'-phosphatase 1 (SHIP1).ResultsPlatelets from affected horses expressed normal amounts of αIIb-β3 integrin and bound fibrinogen normally in response to ADP, but bound 80% less fibrinogen in response to thrombin. α-granules only released 50% as much Factor V as control platelets, but dense granules released their contents normally. Protein kinase B (AKT) phosphorylation was reduced after thrombin activation, but mTOR Complex 2 (mTORC2) and phosphoinositide-dependent kinase 1 (PDK1) signaling were normal. SH2-containing inositol-5'-phosphatase 1 (SHIP1) did not localize to the cytoskeleton of affected platelets and was decreased overall consistent with reduced AKT phosphorylation.Conclusions and clinical significanceDefects in fibrinogen binding, granule secretion, and signal transduction are unique to this thrombasthenia, which we designate as atypical equine thrombasthenia.

Abstract

BackgroundCats with hypertrophic cardiomyopathy (HCM) are at risk for development of systemic thromboembolic disease. However, the relationship between platelet activation state and cardiovascular parameters associated with HCM is not well described.ObjectivesTo characterize platelet activation by flow cytometric evaluation of platelet P-selectin and semiquantitative Western blot analysis of soluble platelet-endothelial cell adhesion molecule-1 (sPECAM-1).AnimalsEight normal healthy cats (controls) owned by staff and students of the School of Veterinary Medicine and 36 cats from the UC Davis Feline HCM Research Laboratory were studied.MethodsPlatelet-rich plasma (PRP) was used for all flow cytometry studies. Platelet surface CD41 and P-selectin expression were evaluated before and after ADP stimulation. sPECAM-1 expression was evaluated by Western blot analysis of platelet-poor plasma that had been stabilized with aprotinin. Standard echocardiographic studies were performed.ResultsResting platelets from cats with severe HCM had increased P-selectin expression compared to controls, and expressed higher surface density of P-selectin reflected by their increased mean fluorescence intensities (MFI). Stimulation with ADP also resulted in significantly increased P-selectin MFI of platelets from cats with severe HCM. Increased P-selectin expression and MFI correlated with the presence of a heart murmur and end-systolic cavity obliteration (ESCO). sPECAM-1 expression from cats with moderate and severe HCM was significantly increased above those of control cats.Conclusions and clinical importanceP-selectin and sPECAM expression may be useful biomarkers indicating increased platelet activation in cats with HCM.

Este artículo es fruto de una experiencia interuniversitaria, internacional y transdisciplinar, llevada a cabo el curso 2021-2022 en la Universidad de Lleida con la colaboración de la Universidad de Zaragoza, en la que participaron también otros colectivos e instituciones, sobre África Joven: arte y diáspora. En ella se llevaron a cabo dos Seminarios, un Seminario teórico-práctico con alumnado del Grado de Educación Social sobre el objeto artístico negroafricano como referencia del pluralismo cultural, y un Seminario Internacional con tres ponencias sobre arte, resiliencia y rehumanización donde se trató de la música, el baile y la oralidad como terapia resiliente en las tradiciones africanas y afrodiaspóricas; además, una iniciativa sobre el Top Manta, que es la marca del proyecto comercial del Sindicato Mantero y, por último, una propuesta práctica para intervenir desde el contexto artístico en colectivos en riesgo, es decir, se realizaron tres propuestas combinadas con una exposición sobre Máscaras negroafricanas titulada "Aprender a ver lo invisible".

7. Cultura organizacional, situaci��n de aula y desarrollo de la Universidad Tecnol��gica Empresarial de Guayaquil-Ecuador

Abstract

La Universidad Tecnol��gica Empresarial de Guayaquil, ha experimentado un acelerado crecimiento en respuesta a demandas del entorno y nuevas tendencias educativas basadas en virtualidad, lo cual ha derivado en la incorporaci��n de profesionales en diferentes campos y diversificaci��n de la matricula estudiantil. En tal contexto, se pretende identificar la cultura organizacional actual y deseada para la Universidad, fundamentada en valores y principios declarados, su relaci��n con la situaci��n ��ulica y desarrollo institucional. Se utiliza la encuesta OCAI y Denison, aplicada de manera aleatoria a 125 miembros de la universidad, junto con entrevistas y grupos focales, bajo criterios espec��ficos para seleccionar a los informantes. Los resultados muestran que el perfil cultural de esta Universidad es una mezcla de los cuatro ���arquetipos��� culturales, con un puntaje mayor para la cultura de mercado; mientras que el perfil deseado es el adhocr��tico, pero sin presentar discrepancia considerable con los valores actuales. La valoraci��n cualitativa sugiere que esta cultura organizacional es asumida por la comunidad en general, concret��ndose en buen clima de convivencia entre docentes y alumnos. Se concluye, que la mezcla del perfil cultural puede haber otorgado flexibilidad ante los cambios del entorno, resultando en una imagen positiva de la trayectoria de desarrollo institucional.

Abstract

Objective To evaluate the responsiveness in terms of correlation of the Hospital Universitario La Princesa Index (HUPI) comparatively to the traditional composite indices used to assess disease activity in rheumatoid arthritis (RA), and to compare the performance of HUPI-based response criteria with that of the EULAR response criteria. Methods Secondary data analysis from the following studies: ACT-RAY (clinical trial), PROAR (early RA cohort) and EMECAR (pre-biologic era long term RA cohort). Responsiveness was evaluated by: 1) comparing change from baseline (Delta) of HUPI with Delta in other scores by calculating correlation coefficients; 2) calculating standardised effect sizes. The accuracy of response by HUPI and by EULAR criteria was analyzed using linear regressions in which the dependent variable was change in global assessment by physician (Delta GDA-Phy). Results Delta HUPI correlation with change in all other indices ranged from 0.387 to 0.791); HUPI's standardized effect size was larger than those from the other indices in each database used. In ACT-RAY, depending on visit, between 65 and 80% of patients were equally classified by HUPI and EULAR response criteria. However, HUPI criteria were slightly more stringent, with higher percentage of patients classified as non-responder, especially at early visits. HUPI response criteria showed a slightly higher accuracy than EULAR response criteria when using Delta GDA-Phy as gold standard. Conclusion HUPI shows good responsiveness in terms of correlation in each studied scenario (clinical trial, early RA cohort, and established RA cohort). 12. Comparison of nutritional status among elderly in hospital and in community environments

Abstract

Introduction: The elderly are considered vulnerable population by the senescence process, which puts them at high risk of morbidity and mortality. Several studies indicate a high prevalence of malnutrition in the elderly in the community and in hospital admissions, so the main purpose of this study is to compare the nutritional status of the elderly persons in both settings, located in Lisbon. Methods: The screening tool Mini Nutritional Assessment (MNA®), body mass index (BMI), arm circumference (AC), arm muscle area (AMA) and C-reactive protein (CRP) were used to assess nutritional status. Based on MNA® questions, food and water intake were evaluated. Results: A total of 99 elderly were evaluated (57.6% in hospital and 42.4% in the community). A higher level of inflammation was observed in hospital, according to CRP levels. It was reported a lower intake in the hospital for the number of meals (15% less), portions of fruit and vegetables (10.5% less), protein intake (33.4% less) and water intake (44.3% less). In the hospital, BMI indicated that 26.7% of the elderly were malnourished whilst in the community it was observed to be 20%. 43.9% had a form of malnutrition in the hospital and 45% in the community when AMA was evaluated. MNA® detected 63.1% of patients were malnourished or at risk of malnutrition in the hospital and 62% in the community. Overall the elderly revealed worse nutritional status is in hospital. The results suggest the need to regularly assess nutritional status allowing to identify and decrease complications associated with malnutrition, regardless of where the elderly are to remain. info:eu-repo/semantics/publishedVersion

13. Nutritional assessment of an elderly population by application of 24-hour recall and the Mini Nutritional Assessment

Abstract

Introduction: Aging is characterized by physiological, social and psychological changes that compromise food intake and nutritional status. To ensure good nutritional status it’s crucial to assess the nutritional status through a validated tool and evaluate the food intake, which is the purpose of this study. Methods: An analytical, observational, transversal, correlational and quantitative study was conducted. The elderly’s assessment and characterization was done by the application of Mini Nutritional Assessment (MNA), arm circumference (AC), laboratory parameters and 24 hours recall. Results: In the study, 42 elderly people were assessed, from which 26 women. By applying the MNA_Long Form® (MNA_LF®) 0 were undernourished, 26 (62%) at risk of desnutrition and 16 (38%) eutrophic. By applying the body mass index (BMI) 2 (5%) undernourished, 6 (15%) at risk of desnutrition, 7 (17%) eutrophic e 26 (63%) overweight. By the AC 19 (45%) exhibit depletion of lean body mass, 20 (48%) good nutritional state and 3 (7%) excess of body fat. The 24 hours recall indicated low intake of macronutrients and of some micronutrients (vitamins A, D, and E, folate, potassium, calcium, and magnesium) and high intake of sodium and phosphorus. The AC correlated with IMC (r = 0,61; p = 0) and MNA_LF (r = 0,45; p = 0,003). The IMC correlated with the presence of dyslipidemia (r = 0,41; p = 0,01), diabetes mellitus (r = 0,45; p = 0,003) and arterial hypertension (r = 0,375; p = 0,02). Key conclusions: The population being studied showed a high prevalence of malnutrition, a similar percentage of risk of desnutrition and overnutrition, and a deficit of food intake, as seen in other Portuguese studies about the elderly in the community. info:eu-repo/semantics/publishedVersion

Abstract

Glycemic and lipidic profiles might be influenced by several factors. The influence of the age group in the most extensively evaluated lipidic and glycemic parameters is more or less well-accepted. To verify this empirical notion, 996 human subjects aged between 21 and 90 years from different localities were characterized according to age. To assess lipid profile, total cholesterol and cholesterol associated with lipoprotein fractions (low-density lipoprotein cholesterol and high-density lipoprotein cholesterol) and triglycerides were determined. Regarding glycemic profile, glucose, and glycated hemoglobin were measured. The majority of the population had values of lipidic parameters fit into the reference values, presenting low or moderate risk for developing cardiovascular disease. Blood glucose was often far above the desirable, but this can be devalued due to the HbA1c values, which were overwhelmingly located in the normal range. The categorization of data in different age groups did not allow defining statistically significant differences. Despite the discriminant linear model was presented, the results indicate that age group did not act as a strong discriminant factor. Somehow unexpectedly, the most significant differences were found among the different localities, which tended to show a similarity according with their latitude. Furthermore, there were no significant correlations in the parameters associated with lipidic profile, but there was a direct correlation between glucose levels and HbA1c (glycemic parameters).

Abstract

Este estudo pretende compreender o fenómeno da indigência educativa nas crianças do circo que frequentam a escola primária. A sua aprendizagem é irregular, interrompida e baseada na reconstrução sucessiva e dolorosa de relações, conduzindo ao abandono escolar precoce. A metodologia de investigação foi dual, qualitativa e quantitativa. A população-alvo é formada pelos empresários de todos os circos portugueses, os seus artistas com filhos na escola primária, essas crianças, e os seus professores, em escolas que acolheram alunos itinerantes, na Regão de Lisboa e Oeste. A solução proposta é um misto de ensino presencial móvel, com reforço de aprendizagem em ambiente virtual, e eventual itinerância escolar voluntária. Isso significa uma adaptação do ano lectivo à temporada de circo, transformando a variável crítica ‘tempo escolar’ num recurso mobilizável para uma educação inclusiva.

Ataulfo de Paiva, 135 sl. 1104, Rio de Janeiro, RJ 22440-901, BrazilTel +55 21 3875-6660Fax + 55 21 2259-3395Email maurogeller@gmail.comPurpose: Urinary antiseptics including methenamine and methylene blue are used in the symptomatic treatment of urinary tract infections (UTIs).Patients and Methods: This was a prospective, double-blind, randomized, double-dummy safety and efficacy study of 2 urinary antiseptic combinations in the symptomatic treatment of recurrent cystitis: methenamine 120mg + methylene blue 20mg (Group A) versus acriflavine 15mg + methenamine 250mg + methylene blue 20mg + Atropa belladonna L. 15mg (Group B). All subjects underwent pretreatment urine culture and antibiotic sensitivity tests prior to 3-day oral treatment with study drug, followed by 3 days of antibiotic therapy (based on urine culture) + study drug treatment. Efficacy was evaluated using the Urinary Tract Infection Symptoms Assessment Questionnaire (UTISA). The primary endpoint was the percentage of patients presenting improvement in cystitis manifestations on the UTISA domain “Urination Regularity” at Visit 2. The primary safety variable was the incidence of treatment-related adverse events.Results: A total of 144 subjects were randomized per group and 272 completed the study. Primary endpoint analysis demonstrates homogeneity between treatment groups, with 69.4% and 72.2% subjects, respectively, showing improvement in the score of the urinary regularity UTISA domain after 3 days of treatment (p= 0.87). At Visit 2, incidence of treatment-related adverse events was higher in Group B (Group A: n= 11, Group B: n= 31, p= 0.0057).Conclusion: Both treatments were effective in reducing UTI symptoms assessed by UTISA questionnaire after 3 days of treatment. Abstract

The objective of this study is to examine and compare the trends of mastitis pathogens in quarter milk samples (n=240,232) submitted for microbiological examination at the Milk Analysis Laboratory (L.I.G.A.L.) at Galicia, Spain from June 2005 to September 2011. Autoregressive IntegratedMoving Average (ARIMA) models and multivariate statistical techniques such as Cluster Analysis were used in order to detect seasonal trends and similarities between the series trends and to classify mastitis pathogens into relatively homogeneous groups. The decrease of bulkmilk somatic cell counts achieved by the mastitis control program, developed in recent years in this region, is the result of the decrease in IMI caused by a limited number of mastitis pathogens. The obtained results reflect a greater complexity in the behavior of mastitis pathogens, unlike the traditional classification into contagious or environmental. Staphylococcus aureus showed a trend similar to Streptococcus dysgalactiae, a mastitis pathogen can behave in both a contagious and an environmentalmanner. Among the traditionally considered environmental mastitis pathogens, Strep. uberis showed a different behavior to Escherichia coli and Klebsiella pneumoniae. Coagulase-negative staphylococci (CNS) species and Streptococcus other than Strep. agalactiae showed differences in the trend model. Time-series analysis and multivariate statistical techniques, such as Cluster Analysis, could be powerful tools to assess the isolation trend of mastitis pathogens because of their ability to cope with stochastic dependence of consecutive data. Furthermore, they could be used to identify the epidemiological behavior of mastitis pathogens using the results of milk samples submitted for routine microbiological examination, by classifying them into relatively homogeneous groups.

Abstract

La Escala de Actitudes Alimentarias (EAT-40) y un cuestionario de Imagen Corporal fueron administrados a un grupo de 220 adolescentes de la provincia de La Coruña. Con este trabajo intentamos examinar la relación entre las actitudes alimentarias de una muestra de adolescentes españoles y una serie de variables determinantes en el estudio de los trastornos alimentarios como son el sexo, el peso, la percepción corporal propia, la satisfacción corporal, así como la preocupación por el peso y la dieta. Los sujetos que se autopercibían obesos, aquellos que se habían sometido a dietas restrictivas en algún momento de sus vidas para adelgazar y concretamente, a dietas frecuentes, obtuvieron puntuaciones superiores en la escala de conductas alimentarias. El hecho de que la población más afectada sea la femenina subraya el importantísimo papel de la cultura occidental y los modelos corporales esbeltos en la estimulación y el mantenimiento de los trastornos alimentarios The Eating Attitude test (EAT) and a self-report questionnaire measuring body image were administrated to 220 adolescents from La Coruña (Spain). The purpose of this study was to examine the relationship between eating attitudes and other variables such as sex, body weight, body self-evaluation, body satisfaction as well as being worried about one’s weight and diet. Those subjects who perceived themselves as obese, those who have been on restrictive diets in order to lose weight anytime during their life and those who were on frequent diets showed higher scores on the EAT. Abstract

AimsTo evaluate the efficacy of methotrexate (MTX) in preventing the recurrence of acute anterior uveitis (AAU).MethodsThis prospective, open, longitudinal study included patients from June 2002 to March 2005 who had either three or more episodes of AAU in the previous year, or a recurrence of AAU within 3 months before starting the trial. We excluded uveitis of infectious origin, masquerade syndromes, and patients with contraindications to MTX. The response criteria were defined as an absence of symptoms and the presence of a normal ophthalmologic examination. The study outcome compared the number of flare-ups of uveitis over an MTX-treated for 1 year to the number of flare-ups of the same group during the previous year without MTX.ResultsA total of 571 patients with uveitis were evaluated during the period of the study, and 10 fulfilled the inclusion criteria. One patient refused the treatment, and nine completed the study. The mean number of recurrences in the pre-MTX year was 3.4 (SD: 0.52), which was significantly reduced to 0.89 (SD: 1.17) in the year of treatment (P=0.011).ConclusionMTX treatment seems to reduce the number of flare-ups in patients with recurrent AAU.Eye (2009) 23, 1130–1133; doi:10.1038/eye.2008.198; published online 8 August 2008 [ABSTRACT FROM AUTHOR]

Abstract

Abstract Background Genetic Hypophosphatemic Rickets (HR) is a group of diseases characterized by renal phosphate wasting with inappropriately low or normal 1,25-dihydroxyvitamin D3 (1,25(OH)2D) serum levels. The most common form of HR is X-linked dominant HR (XLHR) which is caused by inactivating mutations in the PHEX gene. The purpose of this study was to perform genetic diagnosis in a cohort of patients with clinical diagnosis of HR, to perform genotype-phenotype correlations of those patients and to compare our data with other HR cohort studies. Methods Forty three affected individuals from 36 non related families were analyzed. For the genetic analysis, the PHEX gene was sequenced in all of the patients and in 13 cases the study was complemented by mRNA sequencing and Multiple Ligation Probe Assay. For the genotype-phenotype correlation study, the clinical and biochemical phenotype of the patients was compared with the type of mutation, which was grouped into clearly deleterious or likely causative, using the Mann-Whitney and Fisher's exact test. Results Mutations in the PHEX gene were identified in all the patients thus confirming an XLHR. Thirty four different mutations were found distributed throughout the gene with higher density at the 3' end. The majority of the mutations were novel (69.4%), most of them resulted in a truncated PHEX protein (83.3%) and were family specific (88.9%). Tubular reabsorption of phosphate (TRP) and 1,25(OH)2D serum levels were significantly lower in patients carrying clearly deleterious mutations than in patients carrying likely causative ones (61.39 ± 19.76 vs. 80.14 ± 8.80%, p = 0.028 and 40.93 ± 30.73 vs. 78.46 ± 36.27 pg/ml, p = 0.013). Conclusions PHEX gene mutations were found in all the HR cases analyzed, which was in contrast with other cohort studies. Patients with clearly deleterious PHEX mutations had lower TRP and 1,25(OH)2D levels suggesting that the PHEX type of mutation might predict the XLHR phenotype severity.

36. Consensus and contention in beauty judgment.

Abstract

Variance across participants is at the heart of the centuries-old debate about the universality of beauty. Beauty's belonging to the eye of the beholder implies large interindividual variance, while beauty as a universal object property implies the opposite. To characterize the variance at the center of this debate, we selected two quartets with either high- or low-variance images with high typicality and a given mean beauty. The quartets have high or low variance across 50 participants ( group variance) and correspondingly high or low variance across images of a quartet for each participant ( quartet variance). We asked 52 new participants to estimate their own mean and quartet variance. Participants successfully predicted their quartet mean but failed to predict their quartet variance. Though invisible, beauty variance is essential to prediction, both in theory and in practice. However, many psychophysical measurements, including visual crowding, require accurate eye fixation, which is classically achieved by testing only experienced observers who have learned to fixate reliably, or by using a gaze tracker to restrict testing to moments when fixation is accurate. Alas, both approaches are impractical online as online observers tend to be inexperienced, and online gaze tracking, using the built-in webcam, has a low precision (±4 deg). EasyEyes open-source software reliably measures peripheral thresholds online with accurate fixation achieved in a novel way, without gaze tracking. It tells observers to use the cursor to track a moving crosshair. At a random time during successful tracking, a brief target is presented in the periphery. The observer responds by identifying the target. To evaluate EasyEyes fixation accuracy and thresholds, we tested 12 naive observers in three ways in a counterbalanced order: first, in the laboratory, using gaze-contingent stimulus presentation; second, in the laboratory, using EasyEyes while independently monitoring gaze using EyeLink 1000; third, online at home, using EasyEyes. We find that crowding thresholds are consistent and individual differences are conserved. The small root mean square (RMS) fixation error (0.6 deg) during target presentation eliminates the need for gaze tracking. Abstract

What role do the emotions of subject and object play in judging the beauty of images and music? Eighty-one participants rated perceived beauty, liking, perceived happiness, and perceived sadness of 24 songs, 12 art images, and 12 nature photographs. Stimulus presentation was brief (2 seconds) or prolonged (20 seconds). The stimuli were presented in two blocks, and participants took the Positive and Negative Affect Score (PANAS) mood questionnaire before and after each block. They viewed a mood induction video between blocks either to increase their happiness or sadness or to maintain their mood. Using linear mixed-effects models, we found that perceived object happiness predicts an increase in image and song beauty regardless of duration. The effect of perceived object sadness on beauty, however, is stronger for songs than images and stronger for prolonged than brief durations. Subject emotion affects brief song beauty minimally and prolonged song beauty substantially. Whereas past studies of beauty and emotion emphasized sad music, here we analyze both happiness and sadness, both subject and object emotion, and both images and music. We conclude that the interactions between emotion and beauty are different for images and music and are strongly moderated by duration.

40. Beauty isn't special: Comparing the information capacity of beauty and other sensory judgments.

Abstract

Information theory (bits) allows comparing beauty judgment to perceptual judgment on the same absolute scale. In one of the most influential articles in psychology, Miller (1956) found that classifying a stimulus into one of eight or more categories of the attribute transmits roughly 2.6 bits of information. That corresponds to 7 ± 2 categories. This number is both remarkably small and highly conserved across attributes and sensory modalities. This appears to be a signature of one-dimensional perceptual judgment. We wondered whether beauty can break this limit. Beauty judgments matter and play a key role in many of our real-life decisions, large and small. Mutual information is how much information about one variable can be obtained from observing another. We measured the mutual information of 50 participants' beauty ratings of everyday images. The mutual information saturated at 2.3 bits. We also replicated the results using different images. The 2.3 bits conveyed by beauty judgment are close to Miller's 2.6 bits of unidimensional perceptual judgment and far less than the 5 to 14 bits of a multidimensional perceptual judgment. By this measure, beauty judgment acts like a perceptual judgment, such as rating pitch, hue, or loudness.

Abstract

Objectives: The large number of infected patients requiring mechanical ventilation has led to the postponement of scheduled neurosurgical procedures during the first wave of the COVID-19 pandemic. The aims of this study were to investigate the factors that influence the decision to postpone scheduled neurosurgical procedures and to evaluate the effect of the restriction in scheduled surgery adopted to deal with the first outbreak of the COVID-19 pandemic in Spain on the outcome of patients awaiting surgery., Design: This was an observational retrospective study., Settings: A tertiary-level multicentre study of neurosurgery activity between 1 March and 30 June 2020., Participants: A total of 680 patients awaiting any scheduled neurosurgical procedure were enrolled. 470 patients (69.1%) were awaiting surgery because of spine degenerative disease, 86 patients (12.6%) due to functional disorders, 58 patients (8.5%) due to brain or spine tumours, 25 patients (3.7%) due to cerebrospinal fluid (CSF) disorders and 17 patients (2.5%) due to cerebrovascular disease., Primary and Secondary Outcome Measures: The primary outcome was mortality due to any reason and any deterioration of the specific neurosurgical condition. Second, we analysed the rate of confirmed SARS-CoV-2 infection., Results: More than one-quarter of patients experienced clinical or radiological deterioration. The rate of worsening was higher among patients with functional (39.5%) or CSF disorders (40%). Two patients died (0.4%) during the waiting period, both because of a concurrent disease. We performed a multivariate logistic regression analysis to determine independent covariates associated with maintaining the surgical indication. Abstract

Objective: To assess the effect of the first wave of the SARS-CoV-2 pandemic on the outcome of neurosurgical patients in Spain., Settings: The initial flood of COVID-19 patients overwhelmed an unprepared healthcare system. Different measures were taken to deal with this overburden. The effect of these measures on neurosurgical patients, as well as the effect of COVID-19 itself, has not been thoroughly studied., Participants: This was a multicentre, nationwide, observational retrospective study of patients who underwent any neurosurgical operation from March to July 2020., Interventions: An exploratory factorial analysis was performed to select the most relevant variables of the sample., Primary and Secondary Outcome Measures: Univariate and multivariate analyses were performed to identify independent predictors of mortality and postoperative SARS-CoV-2 infection., Results: Sixteen hospitals registered 1677 operated patients. The overall mortality was 6.4%, and 2.9% (44 patients) suffered a perioperative SARS-CoV-2 infection. Of those infections, 24 were diagnosed postoperatively. Age (OR 1.05), perioperative SARS-CoV-2 infection (OR 4.7), community COVID-19 incidence (cases/10 5 people/week) (OR 1.006), postoperative neurological worsening (OR 5.9), postoperative need for airway support (OR 5.38), ASA grade ≥3 (OR 2.5) and preoperative GCS 3-8 (OR 2.82) were independently associated with mortality. For SARS-CoV-2 postoperative infection, screening swab test <72 hours preoperatively (OR 0.76), community COVID-19 incidence (cases/10 5 people/week) (OR 1.011), preoperative cognitive impairment (OR 2.784), postoperative sepsis (OR 3.807) and an absence of postoperative complications (OR 0.188) were independently associated., Conclusions: Perioperative SARS-CoV-2 infection in neurosurgical patients was associated with an increase in mortality by almost fivefold. 45. A 47-Year Comparison of Lower Body Muscular Power in Spanish Boys: A Short Report.

Abstract

Much of the evidence examining temporal trends in fitness among youth has found a decrease in measures of muscular strength and muscular power over recent decades. The aim of this study was to examine trends in lower body muscular power in Spanish boys over 47 years. In 1969 140 boys (10-11 years; body mass index = 19.24, SD = 2.91 kg/m 2 ) and in 2016, 113 boys (10-11 years; body mass index = 19.20, SD = 3.15 kg/m 2 ) were recruited. Lower body power was assessed using the vertical jump (VJ) and standing long jump (SLJ) tests. Significant differences and a large effect size were shown between groups in the SLJ ( p = 0.001; d = 0.94) and the VJ ( p = 0.001; d = 0.66). SLJ data in 1969 were higher (1.52 m, SD = 0.19) when compared to the 2016 data (1.34 m, SD = 0.18). The VJ performance of the 1969 sample was also higher (25.95 cm; SD = 6.58) than the 2016 sample (21.56 cm; SD = 4.72). SLJ and VJ performance of the 2016 group decreased 11.8% and 16.9%, respectively. There were no significant differences between groups in body mass index. The results indicate a secular decline in lower body muscular power in 10-11-year-old Spanish boys with no significant changes in body mass index over the 47-year study period.

Abstract

Introduction: Recombinant human growth hormone (rhGH) is the first biosimilar drug approved by the European Medicines Agency in 2006, using the biosimilar registration process. It was authorised for the treatment of growth hormone deficiency, and growth disorders associated with Turner's syndrome, chronic renal failure, Prader-Willi syndrome, and growth disorders in children/adolescents born small for gestational age, and replacement therapy in adults with pronounced growth hormone deficiency., Materials and Methods: This review is focused on the scientific evidence published about this drug in the last ten years, including the clinical trials on which the approval of the regulatory authority is based, and the most relevant studies evaluating the clinical impact of the drug in clinical practice., Results: The equivalence between biosimilar and original product has been confirmed in the clinical trials published by Romer et al. and López-Siguero et al. Furthermore, studies carried out in real-life conditions confirm its long-term efficacy and safety, as well as the absence of clinical impact by switching treatment from the original to the biosimilar product., Conclusion: The number of patients receiving this medication has continuously increased since its approval. Its equivalence with the original product has been verified. Preliminary data from the post-authorisation PATRO study confirm the efficacy and safety of the biosimilar product in comparison with data from clinical trials. Abstract

This study assessed 24 adult patients, suffering from severe chronic unilateral pain diagnosed as temporomandibular joint (TMJ) disorder (TMD). The full dentate patients had normal occlusion and had never received an occlusal therapy, i.e., were with natural dental evolution/maturation. The following functional and dynamic factors were assessed: (1) chewing function; (2) TMJ remodeling or the condylar path (CP); and (3) lateral jaw motion or lateral guidance (LG). CPs were assessed using conventional axiography, and LG was assessed by K7 jaw tracking. Seventeen (71%) of the 24 (100%) patients consistently showed a habitual chewing side. The mean (standard deviation [SD]) of the CP angles was 47.90 (9.24) degrees. The mean (SD) of the LG angles was 42.95 (11.78) degrees. Data collection emerged from the conception of a new TMD paradigm where the affected side could be the habitual chewing side, the side with flatter lateral jaw motion or the side with an increased CP angle. These data may lead to improved diagnosis, therapy plans and evolution in TMD patients.

Abstract

The presence of a conspicuous and frequent but never-described structure in the skull cavities of sciaenid fish was noted during population studies in an urbanized bay. The ultrastructure closely resembles the cupula of neuromasts, an organ associated with the perception of the environment in teleost fish. The bodies were recorded detached in both preserved and freshly sampled individuals and without associated cilia. Prominent characteristics are acellularity, the elliptic-conic shape composed of stack-like protein lamellas, and a mesh-like appearance in cross section. These acellular lamellar cephalic bodies (ALCBs) were more abundant in larger individuals and showed temporal peaks of abundance independently of the fish size. The conic and lamellar features suggest that the deposition of protein layers follows fish growth, and the bimodality of the size of these structures in individuals indicates temporal peaks of production. These results indicate that these ALCBs are a consequence of the accretion of the cupula of neuromasts at a faster rate than they degrade. Given the novelty of this structure and the increasing records of diseases of marine organisms worldwide, an important question is whether these bodies occur subsequently to some environmental change and whether their accumulation in the skull cavities has consequences to fish health.

Stranding combined with mass-mortality events of sandy-beach organisms is a frequent but little-understood phenomenon, which is generally studied based on discrete episodes. The frequency, magnitude, and possible causes of stranding and mass-mortality events of the trigonal clam Tivela mactroides were assessed based on censuses of stranded individuals, every four days from September 2007 through December 2008, in Caraguatatuba Bay, southeastern Brazil. Stranded clams were classified as dying (closed valves did not open when forced) or dead (closed valves were easily opened). Information on wave parameters and the living intertidal clam population was used to assess possible causes of stranding. This fine-scale monitoring showed that stranding occurred widely along the shore and year-round, with peaks interspersed with periods of low or no mortality. Dead clams showed higher mean density than dying individuals, but a lower mean shell length, attributed to a higher tolerance to desiccation of larger individuals. Wave height had a significant negative relationship to the density of dying individuals, presumed to be due to the accretive nature of low-energy waves: when digging out, clams would be more prone to be carried upward and unable to return; while larger waves, breaking farther from the beach and with a stronger backwash, would prevent stranding in the uppermost areas. This ecological finding highlights the need for refined temporal studies on mortality events, in order to understand them more clearly. Last, the similar size structure of stranded clams and the living population indicated that the stranded individuals are from the intertidal or shallow subtidal zone, and reinforces the ecological and behavioral components of this process, which have important ecological and socioeconomic implications for the management of this population.

Abstract

The use of indirect estimates of ghost-crab populations to assess beach disturbance has several advantages, including non-destructiveness, ease and low cost, although this strategy may add some degree of noise to estimates of population parameters. Resolution of these shortcomings may allow wider use of these populations as an indicator of differences in quality among beaches. This study analyzed to what extent the number of crab burrows may diverge from the number of animals, considering beach morphology, burrow depth and signs of occupation as contributing factors or indicators of a higher or lower occupation rate. We estimated the occupation rate of crabs in burrows on nine low-use beaches, which were previously categorized as dissipative, intermediate or reflexive. Three random 2-m-wide transects were laid perpendicular to the shoreline, where burrows were counted and excavated to search for crabs. The depth and signs of recent activity around the burrows were also recorded. The occupation rate differed on the different beaches, but morphodynamics was not identified as a grouping factor. A considerable number of burrows that lacked signs of recent activity proved to be occupied, and the proportions of these burrows also differed among beaches. Virtually all burrows less than 10 cm deep were unoccupied; the occupation rate tended to increase gradually to a burrow depth of 20-35 cm. Other methods (water, smoke, and traps) were applied to measure the effectiveness of excavating as a method for burrow counts. Traps and excavation proved to be the best methods. These observations illustrate the possible degree of unreliability of comparisons of beaches based on indirect measures. Combining burrow depth assessment with surrounding signs of occupation proved to be a useful tool to minimize biases.
