Treatment of breast lesions has evolved toward the use of less-invasive or minimally invasive techniques. Minimally invasive treatments destroy focal groups of cells without surgery; hence, less anesthesia is required, better cosmetic outcomes are achieved because of minimal (if any) scarring, and recovery times are shorter. These techniques include cryoablation, radiofrequency ablation, microwave ablation, high-intensity focused US, laser therapy, vacuum-assisted excision, and irreversible electroporation. Each modality involves the use of different mechanisms and requires specific considerations for application. To date, only cryoablation and vacuum-assisted excision have received U.S. Food and Drug Administration approval for treatment of fibroadenomas and have been implemented as part of the treatment algorithm by the American Society of Breast Surgeons. Several clinical studies on this topic have been performed on outcomes in patients with breast cancer who were treated with these techniques. The results are promising, with more data for radiofrequency ablation and cryoablation available than for other minimally invasive methods for treatment of early-stage breast cancer. Clinical decisions should be made on a case-by-case basis, according to the availability of the technique. MRI is the most effective imaging modality for postprocedural follow-up, with the pattern of enhancement differentiating residual or recurrent disease from postprocedural changes. ©RSNA, 2021.

Telehealth has not previously been widely implemented as a result of regulatory and reimbursement concerns; however, in the current national emergency of the COVID-19 pandemic, the Centers for Medicare and Medicaid Services has relaxed many of its rules, allowing increased adoption of telehealth services, improving the safety and access of outpatient health care. A complete understanding of the regulatory requirements, technologic options, and billing processes of telehealth is required to initiate a successful clinic. A model is presented here based on a single institution's experience with implementing telehealth in the outpatient interventional radiology clinic.

Purpose To state the Society of Interventional Radiology's position on the use of image-guided thermal ablation for the treatment of early stage non-small cell lung cancer, recurrent lung cancer, and metastatic disease to the lung. Materials and Methods A multidisciplinary writing group, with expertise in treating lung cancer, conducted a comprehensive literature search to identify studies on the topic of interest. Recommendations were drafted and graded according to the updated SIR evidence grading system. A modified Delphi technique was used to achieve consensus agreement on the recommendation statements. Results A total of 63 studies, including existing systematic reviews and meta-analysis, retrospective cohort studies, and single-arm trials were identified. The expert writing group developed and agreed on 7 recommendations on the use of image-guided thermal ablation in the lung. Conclusion SIR considers image-guided thermal ablation to be an acceptable treatment option for patients with inoperable Stage I NSCLC, those with recurrent NSCLC, as well as patients with metastatic lung disease.

To evaluate clarity and usefulness of MRI reporting of uterine fibroids using a structured disease-specific template vs. narrative reporting for planning of fibroid treatment by gynaecologists and interventional radiologists. This is a HIPAA-compliant, IRB-approved study with waiver of informed consent. A structured reporting template for fibroid MRIs was developed in collaboration between gynaecologists, interventional and diagnostic radiologists. The study population included 29 consecutive women who underwent myomectomy for fibroids and pelvic MRI prior to implementation of structured reporting, and 42 consecutive women with MRI after implementation of structured reporting. Subjective evaluation (on a scale of 1-10, 0 not helpful; 10 extremely helpful) and objective evaluation for the presence of 19 key features were performed. Purpose To compare adrenal radiofrequency (RF) ablation with adrenalectomy in treating unilateral aldosterone-producing adenoma (APA). Materials and Methods Between April 2008 and September 2013, 44 patients with adrenal venous sampling–confirmed (lateralization index ≥ 4) unilateral APA underwent adrenal RF ablation (12/44 [27%]) or adrenalectomy (32/44 [73%]). Outcomes of adrenal RF ablation (patient age, 51 y ± 11; 4/12 men) were compared with adrenalectomy (patient age, 50 y ± 11; 19/32 men). Blood pressure (145/94 mm Hg ± 19/13 vs 144/89 mm Hg ± 10/8, P = .92), number of antihypertensives (3.0 ± 1.3 vs 2.7 ± 0.89, P = .38), and serum potassium (3.2 mEq/L ± 0.6 vs 3.5 mEq/L ± 0.6, P = .65) of patients were similar before treatment. Results RF ablation and adrenalectomy resulted in normokalemia (RF ablation, 4.2 mEq/L ± 0.1, P = .0004; adrenalectomy, 4.3 mEq/L ± 0.6, P P = .02/ P = .001; adrenalectomy, 128/85 mm Hg ± 13/12, P P = .07) in all patients. Proportions of RF ablation and adrenalectomy patients cured of hypertension (2/12 [17%] vs 12/32 [38%], P = .28) or requiring fewer antihypertensives (7/12 [58%] vs 13/32 [40%], P = .29) were similar. RF ablation patients had a shorter length of stay (0.6 d ± 0.8 [range, 0–2 d] vs 1.7 d ± 1.4 [range, 0–7 d]; P = .01) and less intraoperative blood loss (1.2 mL ± 3 vs 40 mL ±85; P = .01). Procedural complications occurred in 5/32 (15%) adrenalectomy patients (2 major, 3 minor) and in 0/12 RF ablation patients. Conclusions RF ablation to treat APA can achieve similar clinical outcomes as adrenalectomy and results in shorter hospital stays. Larger, prospective trials are needed to validate these results.

To determine how hypnosis and empathic attention during percutaneous tumor treatments affect pain, anxiety, drug use, and adverse events.For their tumor embolization or radiofrequency ablation, 201 patients were randomized to receive standard care, empathic attention with defined behaviors displayed by an additional provider, or self-hypnotic relaxation including the defined empathic attention behaviors. All had local anesthesia and access to intravenous medication. Main outcome measures were pain and anxiety assessed every 15 minutes by patient self-report, medication use (with 50 mug fentanyl or 1 mg midazolam counted as one unit), and adverse events, defined as occurrences requiring extra medical attention, including systolic blood pressure fluctuations (or =50 mm Hg change to180 mm Hg or105 mm Hg), vasovagal episodes, cardiac events, and respiratory impairment.Patients treated with hypnosis experienced significantly less pain and anxiety than those in the standard care and empathy groups at several time intervals and received significantly fewer median drug units (mean, 2.0; interquartile range [IQR], 1-4) than patients in the standard (mean, 3.0; IQR, 1.5-5.0; P = .0147) and empathy groups (mean, 3.50; IQR, 2.0-5.9; P = .0026). Thirty-one of 65 patients (48%) in the empathy group had adverse events, which was significantly more than in the hypnosis group (eight of 66; 12%; P = .0001) and standard care group (18 of 70; 26%; P = .0118).Procedural hypnosis including empathic attention reduces pain, anxiety, and medication use. Conversely, empathic approaches without hypnosis that provide an external focus of attention and do not enhance patients' self-coping can result in more adverse events. These findings should have major implications in the education of procedural personnel.

AIM: To determine utility of transplant liver biopsy in evaluating efficacy of percutaneous transluminal angioplasty (PTA) for hepatic venous obstruction (HVOO). METHODS: Adult liver transplant patients treated with PTA for HVOO (2003-2013) at a single institution were reviewed for pre/post-PTA imaging findings, manometry (gradient with right atrium), presence of HVOO on pre-PTA and post-PTA early and late biopsy (EB and LB, 60 d after PTA), and clinical outcome, defined as good (no clinical issues, non-HVOO-related death) or poor (surgical correction, recurrent HVOO, or HVOO-related death). RESULTS: Fifteen patients meeting inclusion criteria underwent 21 PTA, 658 ± 1293 d after transplant. In procedures with pre-PTA biopsy (n = 19), no difference was seen between pre-PTA gradient in 13/19 procedures with HVOO on biopsy and 6/19 procedures without HVOO (8 ± 2.4 mmHg vs 6.8 ± 4.3 mmHg; P = 0.35). Post-PTA, 10/21 livers had EB (29 ± 21 d) and 9/21 livers had LB (153 ± 81 d). On clinical follow-up (392 ± 773 d), HVOO on LB resulted in poor outcomes and absence of HVOO on LB resulted good outcomes. Patients with HVOO on EB (3/7 good, 4/7 poor) and no HVOO on EB (2/3 good, 1/3 poor) had mixed outcomes. CONCLUSION: Negative liver biopsy greater than 60 d after PTA accurately identifies patients with good clinical outcomes.

Lanreotide and Octreotide are cyclic octapeptide analogues of somatostatin that were labeled with the radioisotope Technetium-99m for use in diagnostic nuclear medicine. The peptides were processed in a tartrate/phthalate buffer solution containing reducing agent. The purpose of this investigation was to optimize direct labeling by varying some parameters, and to evaluate radiochemical stability and biodistribution in animals. The marked peptides were obtained with high labeling efficiency and no need for subsequent purification. Best radiolabeling results corresponded to a molar ratio of SnCl2.H2O/peptide of 4.5. 99mTc-peptides were radiochemically stable for 6 hours. 99mTc-octreotide was relatively more susceptible to cysteine challenge than 99mTc-lanreotide. 99mTc-peptides were mainly distributed in the gastrointestinal tract but 99mTc-lanreotide showed a greater uptake by the liver than 99mTc-octreotide. Results indicated that the products can be obtained with high radiochemical yield, in a simple routine appropriate for further studies to assess their efficacy in radiodiagnosis. Os peptídeos são preparados em solução tampão ftalato/tartarato contendo um agente redutor. O objetivo deste estudo foi a otimização da marcação direta variando alguns parâmetros e a avaliação da estabilidade radioquímica e biodistribuição em animais. Os peptídeos marcados foram obtidos com alta eficiência de marcação e sem a necessidade de etapa de purificação no final do processo. Os melhores resultados de radiomarcação corresponderam á razão molar de SnCl2.2H2O/peptídeo de 4,5. Os peptídeos-99mTc mostraram-se radioquimicamente estáveis por 6 horas. Octreotídeo-99mTc mostrou-se relativamente mais suscetível frente à cisteína do que o lanreotídeo-99mTc. Os peptídeos-99mTc foram principalmente distribuídos no trato gastrointestinal, porém o lanreotídeo-99mTc mostrou maior captação pelo fígado do que o octreotídeo-99mTc. Os resultados indicaram que os produtos podem ser obtidos com alto rendimento radioquímico e com procedimento simples, permitindo estudos posteriores para avaliação de sua eficácia em radiodiagnóstico.

The onset of hepatic decompensation in cirrhosis heralds an accelerated downhill course with poor outcome. The sole predictor of this decompensation in cirrhosis is increased hepatic vein to portal vein gradient hepatic venous pressure gradient (HVPG). Surrogate markers of liver function or hepatic reserve appear to be less relevant. The hepatic sinusoids become less elastic and more rigid as liver fibrosis and cirrhosis progress. We propose that the Hagen-Poiseuille's law, which applies to rigid, but not elastic vessels, determines the pressure-flow characteristics in the sinusoids. In the rigid cirrhotic liver, HVPG rises dramatically with any change in net surface area or radius, r(4) of the vasculature that follows surgical resection. This review relates liver stiffness to the risk of decompensation in patients with cirrhosis. The liver has a unique dual blood supply comprising a low pressure portal vein and high pressure hepatic artery. We compare the complexity of autoregulation in the normal elastic liver with that in the rigid cirrhotic liver. Therapeutic modalities to reduce portal pressure may reduce the risk of hepatic decompensation and improve outcomes in cirrhosis.

We report a life-threatening subcapsular hepatic hemorrhage after VATS, successfully treated with Gelfoam embolization of the right hepatic artery. The postprocedure course was complicated by infarction of the right hepatic lobe.

CONTEXT AND OBJECTIVE: Recent studies have shown noncontrast computed tomography (NCT) to be more effective than ultrasound (US) for imaging acute ureterolithiasis. However, to our knowledge, there are few studies directly comparing these techniques in an emergency teaching hospital setting. The objectives of this study were to compare the diagnostic accuracy of US and NCT performed by senior radiology residents for diagnosing acute ureterolithiasis; and to assess interobserver agreement on tomography interpretations by residents and experienced abdominal radiologists. DESIGN AND SETTING: Prospective study of 52 consecutive patients, who underwent both US and NCT within an interval of eight hours, at Hospital São Paulo. METHODS: US scans were performed by senior residents and read by experienced radiologists. NCT scan images were read by senior residents, and subsequently by three abdominal radiologists. The interobserver variability was assessed using the kappa statistic. RESULTS: Ureteral calculi were found in 40 out of 52 patients (77%). US presented sensitivity of 22% and specificity of 100%. When collecting system dilatation was associated, US demonstrated 73% sensitivity, 82% specificity. The interobserver agreement in NCT analysis was very high with regard to identification of calculi, collecting system dilatation and stranding of perinephric fat. CONCLUSIONS: US has limited value for identifying ureteral calculi in comparison with NCT, even when collecting system dilatation is present. Residents and abdominal radiologists demonstrated excellent agreement rates for ureteral calculi, identification of collecting system dilatation and stranding of perinephric fat on NCT.

OBJETIVO: Avaliar a reprodutibilidade da tomografia computadorizada sem contraste na avaliação da litíase ureteral e os sinais secundários de obstrução do sistema coletor em pacientes com cólica renal aguda. MATERIAIS E MÉTODOS: Estudo prospectivo de 52 pacientes com diagnóstico clínico de cólica renal aguda submetidos a exame de tomografia computadorizada de abdome sem contraste. Os exames foram realizados com técnica helicoidal e posteriormente analisados por três observadores independentes, com a concordância interobservador avaliada pelo método estatístico kappa (kapa). Foram analisados os parâmetros: a) presença, localização e mensuração dos cálculos ureterais; b) dilatação do sistema coletor intra-renal; c) heterogeneidade da gordura perirrenal; d) dilatação ureteral; e) edema da parede ureteral (sinal do halo). RESULTADOS: Foram encontrados 40 cálculos ureterais na tomografia computadorizada (77%). A concordância interobservador para a identificação do cálculo ureteral e da dilatação ureteral foi quase perfeita (kapa = 0,89 e kapa = 0,87, respectivamente), substancial para dilatação do sistema coletor intra-renal (kapa = 0,77) e moderada para heterogeneidade da gordura perirrenal e para edema da parede ureteral (kapa = 0,55 e kapa = 0,56, respectivamente). CONCLUSÃO: A tomografia computadorizada de abdome sem contraste apresenta elevada reprodutibilidade na avaliação da litíase ureteral e dos sinais secundários de obstrução do sistema coletor. OBJECTIVE: To evaluate the interobserver agreement on non-contrast computed tomography interpretation by a group of experienced abdominal radiologists, for the study of urolithiasis in patients presenting acute flank pain. MATERIALS AND METHODS: Prospective study of 52 patients submitted to non-contrast enhanced helical computed tomography. The images were subsequently analyzed by three independent observers, with the interobserver agreement assessed by means of the kappa (kappa) statistical method. The following parameters were analyzed: a) presence, localization and measurement of ureteral calculi; b) intrarenal calyceal system dilatation; c) perirenal fat heterogeneity; d) ureteral dilatation; e) ureteral wall edema (halo sign). RESULTS: Ureteral calculi were found in 40 of 52 patients (77%). The interobserver agreement was almost perfect as regards identification of ureteral calculi (kappa = 0.89) and ureteral dilatation (kappa = 0.87), substantial for calyceal system dilatation (kappa = 0.77), and moderate for perirenal fat heterogeneity (kappa = 0.55) and ureteral wall edema (kappa = 0.56). CONCLUSION: Non-contrast-enhanced abdominal computed tomography presents high reproducibility in the evaluation of urolithiasis and secondary signs of the calyceal system obstruction.

Medical procedures in outpatient settings have limited options of managing pain and anxiety pharmacologically. We therefore assessed whether this can be achieved by adjunct self-hypnotic relaxation in a common and particularly anxiety provoking procedure. Two hundred and thirty-six women referred for large core needle breast biopsy to an urban tertiary university-affiliated medical center were prospectively randomized to receive standard care (n=76), structured empathic attention (n=82), or self-hypnotic relaxation (n=78) during their procedures. Patients' self-ratings at 1 min-intervals of pain and anxiety on 0-10 verbal analog scales with 0=no pain/anxiety at all, 10=worst pain/anxiety possible, were compared in an ordinal logistic regression model. Rationale and Objectives To evaluate the relationship between intramammary arterial calcifications detected by screening mammography and concomitant coronary artery disease, detected by coronary angiography. Materials and Methods We retrospectively evaluated screening mammography and coronary angiography results of 131 consecutive patients, ages 42–81 years. Risk factors for coronary artery disease (hypertension, diabetes mellitus, dyslipidemia, smoking, and obesity) were obtained from medical records to identify possible simultaneous risk factors for coronary artery disease. Results Eighty-five women had coronary artery disease (41 with intramammary arterial calcifications and 44 without such calcifications) and 46 had normal coronary angiography (11 had intramammary arterial calcifications and 35 did not). A strong association between intramammary arterial calcifications and coronary artery disease was seen, with an OR of 2.96 and P = .006 in the raw model and OR of 4.6 in the adjusted one. The evaluated risk factors did not present as confounding variables. Most patients had only two risk factors or less. The most prevalent risk factor for coronary artery disease was hypertension, with significantly higher prevalence in the coronary artery disease group ( P Conclusion A strong association exists between intramammary arterial calcifications and coronary artery disease. Therefore a mammographic finding of intramammary arterial calcification should receive more of the radiologists’ attention and its presence should never be omitted from the final report.

O principal sinal diagnóstico de ureterolitíase na tomografia computadorizada é a visualização direta do cálculo no interior do ureter. Todavia, a sua caracterização pode ser prejudicada devido a suas pequenas dimensões, variação da respiração entre a aquisição dos cortes tomográficos, escassez de gordura retroperitoneal ou eliminação recente do cálculo. Neste contexto, foram descritos diversos sinais secundários de obstrução ureteral, observados na tomografia computadorizada, que podem auxiliar no diagnóstico de casos duvidosos, além de quantificar o grau de obstrução urinária. The most important diagnostic sign of ureterolithiasis is the direct visualization of the stone within the ureter. However, in some patients with ureterolithiasis stone visualization may be impaired due to stone small size or low attenuation, respiratory artifacts between data acquisitions, low amount of retroperitoneal fat or recent elimination of the stone. In this context, we describe many secondary computed tomography signs of ureteral obstruction that may useful in the diagnosis of inconclusive cases, and also quantify the degree of urinary obstruction.

Background: Made of lightweight titanium and a glass cube, Google Glass (GG) has to ability to stream computer content directly into a wearer’s eyesight, producing “augmented reality.” The device also boasts a front facing HD camera. When coupled with wireless networking, the device is capable of both intaking video from external sources and also broadcasting a live point of view. The device is currently being used by other medical specialties to access clinical data on the fly, consult in real time, and record surgeries for training. The applications to IR are potentially extremely useful. An augmented reality will allow more seamless integration of the visual information used in IR when placed directly into the physician line-of-site. This will both improve ergonomics and allowing visualization of the patient 100% of the time.The high definition camera has applications for training and education. The video feed can be outputted live to an external workstation, providing an attending physician with a point of view of a trainee’s progress during a procedure. Alternatively, the camera can be used to record common procedures including TIPS, venous access, and angiography. Clinical Findings/Procedure Details

Splenic injury is a rare complication following colonoscopy with fewer than 100 reported cases worldwide to date. We describe a case of splenic laceration presenting 5 days following diagnostic colonoscopy. Although hemodynamically stable, active contrast extravasation on contrast-enhanced multidetector computed tomography predicted likely failure of conservative management. Splenic artery angiography confirmed active extravasation from the lower splenic pole and the patient was successfully treated with super selective coil embolization of a lower pole splenic artery branch. This is the eighth reported case of endovascular treatment of splenic injury following colonoscopy. To our knowledge, however, superselective splenic artery embolization has not been previously reported to treat this rare endoscopic complication.

OBJETIVO: Determinar a freqüência de identificação das estruturas músculo-ligamentares do assoalho pélvico por meio de exames de ressonância magnética e avaliar o índice de concordância entre os observadores. MÉTODO: Estudo prospectivo em 20 voluntárias assintomáticas, idade de 20 a 80 anos (média de 50 anos). Realizados exames de ressonância magnética pélvica (1,5 T) nas seqüências turbo spin-eco ponderadas em T1 e T2 nos planos axial e sagital. Os exames foram avaliados por dois observadores independentes, que procuraram identificar os músculos levantador do ânus (músculos coccígeo, pubococcígeo, iliococcígeo e puborretal), obturador interno e compressor da uretra e os ligamentos pubovesical e pubouretral. Os resultados foram comparados com base na freqüência de identificação das estruturas anatômicas e na concordância entre os observadores (índice kappa -- kapa). RESULTADOS: A freqüência de identificação das estruturas variou de 50% a 100%, sendo pouco inferior para os ligamentos. A concordância interobservador na identificação das estruturas foi a seguinte: músculos levantador do ânus e obturador interno (kapa=1), pubococcígeo (kapa=0,62), iliococcígeo (kapa=0,86), puborretal (kapa=0,27), coccígeo (kapa=0) e compressor da uretra (kapa=1), e ligamentos pubovesical (kapa=0,50) e pubouretral (kapa=0,58). CONCLUSÃO: A ressonância magnética de pelve permite identificar as principais estruturas músculo-ligamentares do assoalho pélvico na grande maioria dos indivíduos, com boa concordância interobservador.

Many years ago, we published the first report in the literature of a patient having infected pancreatic necrosis who survived following strictly medical therapy, namely antibiotics. 1 That paper was harshly criticized, perhaps rightly so, since conventional wisdom recommended—and to some extent still advises—open necrosectomy and drainage for nearly 100% of such patients. 2 It should however be noted that, in that case, administering only antibiotics was neither a simple option, nor an unauthorized experiment, for the very good reason that the patient himself irrevocably refused any surgical intervention on religious grounds. Even at that time, experienced surgeons in international congresses would privately concede that every once in a while they had come across this situation: nonoperated patients who had survived on antibiotics only, but publication of such results was generally regarded as an anathema. Fifteen years and several articles later, infected pancreatic necrosis has not become a trivial disease – and probably never will – nor has conservative therapy become mainstream. However, advances in interventional radiology and other forms of minimally invasive management have greatly benefited patients with deep-seated infected foci in general, including those associated with severe pancreatitis. 3,4 Of course, minimally invasive management is not the gold standard for infected pancreatic necrosis, and open surgical debridement of contaminated foci still produces the most reliable results, with present reported mortalities of 20% to 25%. 2,5 However, the use of minimally invasive
