Summary Background & aims COVID-19 patients present a high hospitalization rate with a high mortality risk for those requiring intensive care. When these patients have other comorbid conditions and older age, the risk for severe disease and poor outcomes after ICU admission are increased. The present work aims to describe the preliminary results of the ongoing NUTRICOVID study about the nutritional and functional status and the quality of life of adult COVID-19 survivors after ICU discharge, emphasizing the in-hospital and discharge situation of this population. Methods A multicenter, ambispective, observational cohort study was conducted in 16 public hospitals of the Community of Madrid with COVID-19 survivors who were admitted to the ICU during the first outbreak. Preliminary results of this study include data retrospectively collected. Malnutrition and sarcopenia were screened at discharge using MUST and SARC-F; the use of healthcare resources was measured as the length of hospital stay and requirement of respiratory support and tracheostomy during hospitalization; other study variables were the need for medical nutritional treatment; and patients’ functional status (Barthel index) and health-related quality of life (EQ-5D-5L). Results A total of 176 patients were included in this preliminary analysis. Most patients were male and older than 60 years, who suffered an average (SD) weight loss of 16.6% (8.3%) during the hospital stay, with a median length of stay of 53 (27–89.5) days and a median ICU stay of 24.5 (11–43.5) days. At discharge, 83.5% and 86.9% of the patients were at risk of malnutrition and sarcopenia, respectively, but only 38% were prescribed medical nutrition therapy. In addition, more than 70% of patients had significant impairment of their mobility and to conduct their usual activities at hospital discharge. Conclusions This preliminary analysis evidences the high nutritional and functional impairment of COVID-19 survivors at hospital discharge and highlights the need for guidelines and systematic protocols, together with appropriate rehabilitation programs, to optimize the nutritional management of these patients after discharge.

Liver cirrhosis is a progressive and chronic disease of the liver, of diverse etiology, which is frequently associated with glucose intolerance and in some cases concurs with type 2 diabetes (DM2). DM2 is associated with adverse outcomes in patients with cirrhosis, including a higher rate of hospitalizations, a higher prevalence of hepatocellular carcinoma, and an increased risk of mortality. Malnutrition is another frequent complication of cirrhosis, the prevalence of which increases with the degree of liver dysfunction, worsening the prognosis. La DM2 se asocia con resultados adversos en pacientes con cirrosis, incluyendo una mayor tasa de ingresos hospitalarios, una mayor prevalencia de carcinoma hepatocelular y un mayor riesgo de mortalidad. La desnutrición es otra complicación frecuente en la cirrosis, cuya prevalencia aumenta con el grado de disfunción hepática, empeorando el pronóstico. Diabetes is a frequent comorbidity in cancer patients, since they share common risk factors. In cancer, the concurrence of cachexia represents a poor prognostic factor, which is aggravated by poor nutritional status. Clinically, cancer cachexia manifests as a significant reduction in body weight, accompanied by changes in body composition and alterations in the balance of the biological system, and causes progressive dysfunction. This article describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with cancer cachexia.La diabetes es una comorbilidad frecuente en pacientes con cáncer, ya que comparten factores de riesgo comunes. En la enfermedad oncológica, la presencia de caquexia tumoral representa un factor de mal pronóstico, que se ve agravado por un estado nutricional deficiente. Clínicamente, la caquexia se manifiesta como una reducción significativa del peso corporal, acompañado de cambios en la composición corporal y alteraciones en el equilibrio del sistema biológico, y causa una disfunción progresiva. People with diabetes are at high risk of requiring surgical intervention throughout their lives, and of perioperative complications in case of poor metabolic control. Hospitalization represents a stressful event that, together with other factors associated with diagnostic and therapeutic procedures, leads to a deterioration in the nutritional status of the patients. Hydroxytyrosol (HT) and punicalagin (PC) exert cardioprotective and antiatherosclerotic effects. This study evaluated the effect of an oral supplement containing HT and PC (SAx) on dyslipidemia in an adult population. A randomized, double-blind, controlled, crossover trial was conducted over a 20-week period. SAx significantly reduced the plasma levels of triglycerides (TG) in subjects with hypertriglyceridemia (≥150 mg/dL) (from 200.67 ± 51.38 to 155.33 ± 42.44 mg/dL; p < 0.05), while no such effects were observed in these subjects after the placebo. SAx also significantly decreased the plasma levels of low-density lipoprotein cholesterol (LDL-C) in subjects with high plasma levels of LDL-C (≥160 mg/dL) (from 179.13 ± 16.18 to 162.93 ± 27.05 mg/dL; p < 0.01), while no such positive effect was observed with the placebo. In addition, the placebo significantly reduced the plasma levels of high-density lipoprotein cholesterol (HDL-C) in the total population (from 64.49 ± 12.65 to 62.55 ± 11.57 mg/dL; p < 0.05), while SAx significantly increased the plasma levels of HDL-C in subjects with low plasma levels of HDL-C (

Type 2 diabetes mellitus is a highly prevalent disease worldwide, generating an increasing clinical and economic burden due to its micro and macrovascular complications. Frequently, people with diabetes are hospitalized for various pathologies. These patients generally have higher risk of complications, prolonged hospitalizations and mortality. An additional factor that worsens the prognosis in these patients is the concurrence of malnutrition, especially in elderly people. All this makes the management of these patients challlenging and requires a specific nutritional approach, whose purpose is to cover the nutritional requirements while always maintaining glycemic control. The purpose of this work is to provide, based on the evidence available in the literature and clinical experience, consensus recommendations by eighteen experts in Endocrinology and Nutrition on the nutritional approach of hospitalized patients with diabetes/hyperglycemia and compare the optimal management based on these recommendations with bedside usual care according to a panel of Spanish doctors surveyed about their daily clinical practice. This first article of the Supplement describes the methodology of the study and the results obtained regarding common issues for all pathologies.

Heart failure (HF) is one of the leading causes of morbidity and mortality among older people, making it a major public health problem. Cardiovascular diseases in general, and HF in particular, are common comorbidities in people with type 2 diabetes (DM2). The concurrence of DM2 and HF is associated with more severe clinical symptoms and signs, and poorer quality of life and prognosis. Furthermore, due to the hypercatabolic state and nutrient absorption disorders, malnutrition is present in many HF cases. This document describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with HF.

Sarcopenia is a multidimensional condition that negatively affects muscle mass, muscle strength, and physical performance. The prevalence of sarcopenia in people with diabetes is much higher than that of the general population, especially in individuals with poor nutritional status. Both sarcopenia and malnutrition are conditions amenable to intervention to improve clinical prognosis. This document describes the results of the expert consensus and the responses of the panelists on the nutritional management in routine clinical practice of patients with diabetes/hyperglycemia hospitalized (non-critically ill) with sarcopenia.

Surgical stress predisposes patients to have immune dysfunction and an increased risk of infection. Malnourished surgical patients have higher postoperative morbidity and mortality rates, higher readmission rates, and higher hospital costs. The use of an immunomodulatory formula is associated in the ESPEN guidelines with a reduction in wound healing problems, suture failure, and infectious and global complications. Several authors have suggested that, since most clinical trials evaluating the efficacy of immunonutrition have been carried out in a traditional perioperative setting, it would be interesting to investigate its efficacy in a more controlled setting, such as in the ERAS (Enhanced Recovery after Surgery) protocol. The objective of this work was: a) to define the role that immunonutrition should play in ERAS protocols based on the best scientific evidence available; b) to analyze the difficulties that continue to exist in real-life clinical practice to screen the nutritional risk of patients; c) to make a proposal of algorithms adapted to the characteristics of our environment regarding the screening, assessment, and nutritional treatment of surgical patients in fast-track surgery.El estrés quirúrgico predispone a los pacientes a la disfunción inmune y a un mayor riesgo de infección. Los pacientes quirúrgicos desnutridos presentan una mayor morbimortalidad posoperatoria, mayores tasas de reingreso y costes hospitalarios más elevados. En las guías de la ESPEN se asocia el uso de una fórmula inmunomoduladora a una reducción significativa de los problemas de la cicatrización de heridas, de los fallos de la sutura y de las complicaciones infecciosas y globales. Varios autores han sugerido que, dado que la mayoría de los ensayos clínicos que evalúan la eficacia de la inmunonutrición se han realizado en un entorno perioperatorio tradicional, sería interesante investigar su eficacia en un entorno más controlado, como en el protocolo ERAS (Enhanced Recovery after Surgery). El objetivo de este trabajo es: a) definir el papel que debe jugar la inmunonutrición en los protocolos ERAS sobre la base de la mejor evidencia científica; b) analizar las dificultades que siguen existiendo en la práctica clínica real para realizar el cribado del riesgo nutricional del paciente; c) proponer unos algoritmos adaptados a las características de nuestro entorno sobre el cribado, la valoración y el tratamiento nutricional del paciente quirúrgico en modalidad fast-track.

Aim: to present the results of the Spanish home enteral nutrition (HEN) registry of the NADYA-SENPE group for the years 2018 and 2019. Material and methods: from January 1, 2018 to December 31, 2019 the home enteral nutrition registry was recorded, and afterwards a further descriptive and analytical analysis was done. Results: in 2018, 4756 active patients were registered and the prevalence was 101.79 patients per one million inhabitants; in 2019 there were 4633 patients with a prevalence of 98.51 patients per one million inhabitants. They originated in 46 hospitals: 51.3 % were male, and median age was 71.0 years in both periods. The most frequent diagnosis was a neurological disorder that presents with aphagia or severe dysphagia - 58.7 % and 58.2 %, respectively. The main cause of episode termination was death. A total of 116 pediatric patients were registered in 2018 and 115 in 2019. Females represented 57.8 % and 59.1 %, respectively, in each of the periods. Median age at the beginning of HEN was 5 and 7 months. The most commonly recordered diagnostic group (42.2 % and 42.6 %) was included within the other pathologies group, followed by neurological disorders that present with aphagia or severe dysphagia in 41.4 % and 41.7 % of children. The route of administration was gastrostomy in 46.6 % and 46.1 %, respectively, in each of the periods. Conclusions: the NED registry of the NADYA-SENPE group continues to operate uninterruptedly since its inception. The number of registered patients and the number of participating hospitals remained stable in the last biennium analyzed.Objetivo: exponer los resultados del registro de nutrición enteral domiciliaria (NED) de los años 2018 y 2019 del Grupo NADYA-SENPE. Material y métodos: se recopilaron los pacientes introducidos en el registro desde el 1 de enero al 31 de diciembre de 2018 y en las mismas fechas para 2019, procediendo al análisis descriptivo y analítico de los datos. Resultados: en el año 2018 se registraron 4756 pacientes activos con una tasa de prevalencia de 101,79 pacientes/millón de habitantes; en 2019 fueron 4633 con una tasa de prevalencia de 98,51 pacientes/millón de habitantes. Procedían de 46 hospitales. Fueron el 51,3 % los varones registrados y la edad mediana fue de 71,0 años en ambos periodos. El diagnóstico más frecuente fue el de enfermedad neurológica que cursa con afagia o disfagia severa (58,7 % y 58,2 %), respectivamente. La causa principal de finalización de los episodios fue el fallecimiento. Los pacientes pediátricos registrados fueron 116 en 2018 y 115 en 2019. Las niñas representaron el 57,8 % y 59,1 %, respectivamente, en cada uno de los periodos. La edad mediana de inicio de la NED fue de 5 y 7 meses. El grupo diagnóstico más registrado (42,2 % y 42,6 %) se englobó dentro del grupo de otras patologías, seguido de la enfermedad neurológica que cursa con afagia o disfagia severa de los niños (41,4 % y 41,7 %). Se alimentaban a través de gastrostomía el 46,6 % y 46,1 %, respectivamente, en cada uno de los periodos. Conclusiones: el registro de NED del grupo NADYA-SENPE sigue operativo de forma ininterrumpida desde sus inicios. El número de pacientes registrados y el de hospitales participantes permanece estable en el último bienio analizado.

Introduction: excess weight represents a public health problem due to its associated risk factors. A sedentary lifestyle, an inadequate diet or a decrease in the feeling of satiety are some of the causes. Objetives: to evaluate the satiating properties of the consumption of a functional Iberian broth enriched with phospho-fructooligosaccharides (FOS) in healthy people through the plasma concentration of hormones involved in appetite. Material and method: acute, crossover, randomized, double-blind and controlled nutritional clinical trial carried out in 18 randomized participants in two treatment sequences (functional broth (CF), composed of 5.6 g POS/100 g and control broth (CC), with 0.4 g of maltodextrin/100 g) with 14 days of washing in between. Satiety-related parameters (glucose, insulin, leptin, ghrelin, GLP-1, PYY) and visual analog scales (VAS) were measured. Results: the percentage of body fat decreased in those who took the CF (-0.15 ± 0.32 vs 0.09 ± 0.52) (p0.05). Leptin concentration was higher with CF (p0.001), which was shown at time points -30 (p0.001), 0 (p0.001), 30 (p = 0.026) and 120 (p = 0.049) when compared to CC. The areas under the curve (AUC) for GLP-1 (p = 0.0033) and PYY (p = 0.022) were higher for CF as compared to CC. Conclusions: consumption of an Iberian broth enriched with POS improves the plasma concentration of hormones involved in the control of satiety, and reduces the amount of body fat. This result could have beneficial effects for the prevention and treatment of overweight.Introducción: el exceso de peso representa un problema de salud pública debido a los factores de riesgo asociados. El sedentarismo, una alimentación inadecuada o una disminución de la sensación de saciedad son algunas de sus causas. Objetivos: evaluar las propiedades saciantes del consumo de un caldo ibérico funcional enriquecido con fosfofructooligosacáridos (FOS) en personas sanas a través de la concentración plasmática de las hormonas involucradas en el apetito. Material y métodos: ensayo clínico nutricional, agudo, cruzado, aleatorizado, doble ciego y controlado, llevado a cabo en 18 participantes aleatorizados en dos secuencias de tratamiento (caldo funcional (CF) compuesto de 5,6 g de FOS/100 g y caldo de control (CC), con 0,4 g de maltodextrina/100 g) con 14 días de lavado entre ellas. Se midieron parámetros relacionados con la saciedad (glucosa, insulina, leptina, ghrelina, GLP-1, PYY) y escalas analógicas visuales (EAV). Resultados: el porcentaje de grasa corporal disminuyó en los que tomaron el CF (-0,15 ± 0,32 vs. 0,09 ± 0,52) (p0,05). La concentración de leptina fue superior con el CF (p0,001), mostrándose dicho aumento en los tiempos -30 (p0,001), 0 (p0,001), 30 (p = 0,026) y 120 (p = 0,049) con respecto al CC. Las áreas bajo la curva (AUC) de GLP-1 (p = 0,0033) y PYY (p = 0,022) fueron superiores con el CF en comparación con el CC. Conclusión: el consumo de un caldo ibérico enriquecido con FOS mejora la concentración plasmática de las hormonas involucradas en el control de la saciedad y reduce la cantidad de grasa corporal. Dichos resultados podrían tener efectos beneficiosos para la prevención y el tratamiento del exceso de peso corporal.

Introduction: breast cancer is the most common invasive cancer among women in developed countries. At diagnosis, approximately 70 % of women are overweight, and the additional weight gain that can result from the ensuing treatments has been associated with cancer recurrence and progression. Objectives: the main objective was to compare the effect of only a nutritional intervention (CG) with a nutrition education program (nutritional intervention, nutrition education, and physical activity) (IG) for 1 year. Methods: a total of 65 women with breast cancer who had been evaluated at the Clinical Nutrition Department, La Paz University Hospital, Madrid, Spain were recruited into 2 groups: a control group (CG) and an intervention group (IG). Results: the IG showed a significant reduction in body weight (-1.87 ± 3.41 vs. 1.48 ± 2.01 kg, p0.05), BMI (-0.61 ± 1.40 vs. 0.65 ± 0.88 kg/m2, p0.05), total cholesterol (-32.92 ± 38.45 vs. -3.23 ± 39.73 mg/dl, p0.05), and low-density lipoprotein cholesterol (-35.29 ± 27.50 vs. 6.33 ± 40.70 mg/dl, p0.05). Both groups were shown to be more conscious of the importance of physical activity, with increased consumption of grains, fruits, oily fish, and dairy. Conclusions: dietary interventions and physical activity were shown to be important to achieving several physical and physiological benefits that could reduce some risk factors associated with breast cancer recurrence and progression.Introducción: el cáncer de mama es el cáncer invasivo más común entre las mujeres de los países desarrollados. En el momento del diagnóstico, aproximadamente el 70 % de las mujeres tienen sobrepeso, y el aumento de peso adicional que puede resultar de los tratamientos subsiguientes se ha asociado con la recurrencia y progresión de la enfermedad. Objetivos: el objetivo principal del estudio fue comparar el efecto de solo una intervención nutricional (GC) con un programa integral de educación nutricional (intervención y educación nutricional y actividad física) (IG) durante 1 año. Métodos: un total de 65 mujeres con cáncer de mama previamente evaluadas en la Unidad de Nutrición Clínica y Dietética del Hospital Universitario La Paz, Madrid, España, fueron reclutadas y divididas en 2 grupos: grupo de control (GC) y grupo de intervención (GI). Resultados: el GI mostró una reducción significativa del peso corporal (-1,87 ± 3,41 vs. 1,48 ± 2,01 kg, p0,05), IMC (-0,61 ± 1,40 vs. 0,65 ± 0,88 kg/m2, p0,05), colesterol total (-32,92 ± 38,45 vs. -3,23 ± 39,73 mg/dl, p0,05) y colesterol unido a lipoproteínas de baja densidad (LDL) (-35,29 ± 27,50 vs. 6,33 ± 40,70 mg/dl, p0,05). Al finalizar el estudio, ambos grupos fueron más conscientes de la importancia de la actividad física y demostraron consumir una cantidad más elevada de cereales, frutas, pescado azul y lácteos. Conclusiones: las intervenciones dietéticas junto con la práctica de actividad física son importantes para lograr beneficios físicos y fisiológicos que podrían reducir algunos factores de riesgo asociados con la recurrencia y progresión del cáncer de mama.

Aim: to communicate the home parenteral nutrition (HPN) data obtained from the HPN registry of the NADYA-SENPE group (www.nadya-senpe.com) for the year 2019. Material and methods: a descriptive analysis of the data collected from adult and pediatric patients with HPN in the NADYA-SENPE group registry from January 1 to December 31, 2019. Results: a total of 283 patients (51.9 % women), 31 children, and 252 adults from 47 Spanish hospitals were registered, which represents a prevalence rate of 6.01 patients per million inhabitants for year 2019. The most frequent diagnosis in adults was "palliative oncological" and "others" (21.0 %). In children, it was Hirschsprung's disease together with necrotizing enterocolitis, alterations in intestinal motility, and chronic intestinal pseudo-obstruction, with 4 cases each (12.9 %). The first reason for the indication was short-bowel syndrome in both children (51.6 %) and adults (37.3 %). The most used type of catheter was tunnelled both in children (75.9 %) and in adults (40.8 %). Sixty-eight episodes ended, all in adults, and the most frequent cause was death (54.4 %); 38.2 % were switched to oral. Conclusions: the number of collaborating centers and professionals in the NADYA registry is increasing. The main indications and reasons for HPN termination remain stable.Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadya-senpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El diagnóstico más frecuente en los adultos fue “oncológico paliativo” y “otros” (21,0 %). En los niños fue la enfermedad de Hirschsprung junto a la enterocolitis necrotizante, las alteraciones de la motilidad intestinal y la pseudoobstrucción intestinal crónica, con 4 casos cada uno (12,9 %). El primer motivo de indicación fue el síndrome del intestino corto tanto en los niños (51,6 %) como en los adultos (37,3 %). El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD.

Osteoporosis is a multifactorial disease characterized by the loss of bone mass and deterioration of the internal structure of the bone, increasing the risk of fractures, and is becoming an economic and social problem. The main treatment is pharmacological, however, the population demands other therapies, such as foods with nutrients beneficial to bone health. Seventy-eight healthy menopausal women at risk of osteoporosis or untreated osteopenia were recruited for a randomized, parallel, double-blind clinical trial with two intervention groups: one group consumed a serving a day of the experimental enriched product (experimental group (EG)) and the other group (control group (CG)) consumed the same product without enrichment. The main objective was to compare the effect of consuming a dairy preparation to reconstitute, similar to yogurt when prepared, enriched in calcium, vitamin D, vitamin K, vitamin C, zinc, magnesium, L-leucine and probiotic (Lactobacillus plantarum 3547) on bone metabolism markers for 24 weeks. The EG showed a significantly increased bone mass compared to the CG (0.01 &plusmn, 0.03 vs. &minus, 0.01 &plusmn, 0.03 kg, p &lt, 0.05). In addition, the EG maintained their bone mineral density (BMD) compared to the CG, whose BMD significantly decreased at the end of the study. For biochemical markers, the EG significantly increased the serum levels of the N-terminal propeptide of type I collagen (P1NP) bone formation marker (13.19 &plusmn, 25.17 vs. &minus, 4.21 &plusmn, 15.62 ng/mL, 0.05), and decreased the carbo-terminal telopeptide of type I collagen (CTx) bone resorption marker compared to the CG (&minus, 0.05 &plusmn, 0.19 vs. 0.04 &plusmn, 0.14 ng/mL, 0.05). On the other hand, the EG exhibited a significantly decreased systolic and diastolic blood pressure compared to the start of the study. Finally, the EG significantly increased their dietary calcium and vitamin D intake compared to the CG. In conclusion, the regular consumption of a dairy product to reconstitute enriched with bioactive nutrients improves bone health markers in menopausal women at risk of osteoporosis without pharmacological treatment.

Obesity is a global public health problem. Objective: To evaluate the effect of the regular consumption of the product Lipigo&reg, on body weight and rebound effect on overweight/obese subjects undergoing a comprehensive weight loss program. Methods: A randomized, parallel, double-blind, placebo-controlled clinical trial was conducted with male and female subjects presenting a BMI 25&ndash, 39.9 kg/m2. All subjects underwent a comprehensive weight loss program (WLP) for 12 weeks, which included an individualized hypocaloric diet, physical activity recommendations, nutritional education seminars, and three times a day consumption of the product Lipigo&reg, or Placebo. After-WLP, subjects continued the treatment for 9 months to assess rebound effect. Body weight (BW), BMI, and body composition were measured at the beginning and the end of the WLP, and in the follow-up. Results: A total of 120 subjects (85% women) 49.0 &plusmn, 9.5 years old and with a BW of 81.57 &plusmn, 13.26 kg (BMI 31.19 &plusmn, 3.44 kg/m2) were randomized and 73 subjects finished the study. At the end of the WLP, there was a tendency toward reduced BW (p = 0.093), BMI (p = 0.063), and WC (p = 0.059) in the treated group. However, subjects with obesity type 1 (OB1) from the treated group significantly reduced body weight (&minus, 5.27 &plusmn, 2.75 vs. &minus, 3.08 &plusmn, 1.73 kg, p = 0.017) and BMI (&minus, 1.99 &plusmn, 1.08 vs. &minus, 1.09 &plusmn, 0.55 kg/m2, p = 0.01) compared with placebo. They also presented a minor rebound effect after 9 months with product consumption (&minus, 4.19 &plusmn, 3.61 vs. &minus, 1.44 &plusmn, 2.51 kg, p = 0.026), minor BMI (&minus, 1.61 &plusmn, 1.43 vs. &minus, 0.52 &plusmn, 0.96 kg/m2, p = 0.025) and tended to have less fat-mass (&minus, 3.44 &plusmn, 2.46 vs. &minus, 3.29 kg, p = 0.080) compared with placebo. Conclusions: The regular consumption of the product Lipigo&reg, promotes the reduction of body weight and reduces the rebound effect of obese people after 52 weeks (12 months), mainly in obesity type 1, who undergo a comprehensive weight loss program.

espanolIntroduccion: la desnutricion relacionada con la enfermedad (DRE) es un tipo especifico de desnutricion producida por la respuesta inflamatoria de la enfermedad de base. Su prevalencia en todo el mundo varia del 30 al 50 %, siendo similar en los hospitales espanoles. La DRE no se reconoce comunmente, se diagnostica inadecuadamente y generalmente no se trata. Se asocia a un mayor riesgo de morbilidad, mortalidad y costes. Las sociedades nutricionales recomiendan que el cribado se realice en las primeras 24 a 48 horas posteriores al ingreso para la deteccion temprana de la desnutricion. Ninguna herramienta de cribado se acepta universalmente. Objetivos: evaluar la validez predictiva (estancia hospitalaria, complicaciones, reingresos y mortalidad) del metodo CONUT en comparacion con la VGS y el VEN. Material y metodo: estudio retrospectivo incluido en un prospectivo observacional de 365 pacientes hospitalizados de julio a diciembre de 2012. Resultados: los servicios de ingreso mas frecuentes fueron Medicina Interna y Oncologia (30,7 % y 29,3 %). Deteccion de los riesgos moderado y severo de desnutricion: CONUT, 42,2 y 12,1 %; VSG, 25,8 % y 10,1 %; VEN, 13,7 y 14,5 %. La desnutricion evaluada utilizando el metodo CONUT se relaciono significativamente con las complicaciones (p = 0,036), los reingresos (p = 0,041) y la mortalidad (p = 0,007). Las curvas ROC de la mortalidad, para todos los metodos evaluados, mostraron que CONUT es la mejor herramienta. Conclusiones: CONUT es una herramienta de deteccion automatica que se puede utilizar como primer paso en el diagnostico del riesgo de desnutricion. CONUT ofrece la ventaja de ser un factor pronostico de complicaciones, reingreso y mortalidad. EnglishIntroduction: disease-related malnutrition (DRM) is a specific type of malnutrition caused by the inflammatory response to the underlying disease. Its prevalence worldwide varies from 30 % to 50 %, being similar in Spanish hospitals. DRE is not commonly recognized but is usually misdiagnosed and generally not treated. It is associated with an increased risk of morbidity, mortality, and costs. Nutritional societies recommend that screening be performed within the first 24 to 48 hours after admission for the early detection of malnutrition. No screening tool is universally accepted. Objectives: to evaluate the predictive validity (hospital stay, complications, readmissions and mortality) of the CONUT method as compared to SGA and NSA. Material and method: a retrospective study included in a prospective observational study of 365 hospitalized patients from July to December 2012. Results: the most frequent admission services were Internal Medicine and Oncology (30.7 % and 29.3 %). Moderate and severe risk of malnutrition: CONUT, 42.2 % and 12.1 %, SGA 25.8 % and 10.1 %, and NSA 13.7 % and 14.5 %. Malnutrition evaluated using the CONUT method was significantly related to complications (p = 0.036), readmissions (p = 0.041) and mortality (p = 0.007). The ROC curves for mortality, for all the methods evaluated, showed that CONUT is the best tool. Conclusions: CONUT is an automatic detection tool that can be used as a first step in the diagnosis risk of malnutrition. CONUT offers the advantage of being a prognostic factor for complications, readmission, and mortality.

CF-COVID19-Spain Registry Group., [Background] Given the high incidence of confirmed infection by SARS-CoV-2 and mortality by COVID-19 in the Spanish population, its impact was analysed among persons with Cystic Fibrosis (CF) as a group at risk of a worse evolution. The possible causes of the incidence observed in them are explained and how CF Units have faced this health challenge is detailed., [Methods] Retrospective descriptive observational study, for which a Spanish CF Patients with Confirmed COVID-19 Registry is created, requesting information on number of people affected between 8 March–16 May 2020 and their clinical-demographic characteristics from the CF Units participating in the European Cystic Fibrosis Society Patient Registry (ECFSPR). The accumulated incidence is calculated, compared with that of the general population. Additionally, a survey (CF-COVID19-Spain) is carried out on prevention of SARS-CoV-2 infection, workings of CF Units and possible reasons for the incidence observed., [Results] COVID-19 was diagnosed in eight CF patients, one of whom had received a lung transplant. The accumulated incidence was 32/10000 in CF patients and 49/10000 in the general population. General death rate was 5.85/10000 while no CF patients included in the ECFSPR died. The characteristics of those affected and the results of the survey are described., [Conclusions] Despite being considered a disease at high risk of severe COVID-19, the low incidence and mortality in CF patients in Spain contrasts with the figures for the general population. The possible factors that would explain such findings are discussed, with the help of the results of the CF-COVID19-Spain survey.

Aim: To communicate home parenteral nutrition (HPN) data obtained from the HPN registry of the NADYA-SENPE group (www.nadya-senpe. com) for the year 2018. Material and methods: Descriptive analysis of the data collected from adult and pediatric patients with HPN in the NADYA-SENPE group registry from January 1st, 2018 to December 31st, 2018. Results: There were 278 patients from 45 Spanish hospitals (54.7% women), 23 children and 255 adults, which represent a prevalence rate of 5.95 patients/million inhabitants/year 2018. The most frequent diagnosis in adults was “palliative cancer” (22.0%), followed by “others”. In children it was Hirschsprung’s disease together with necrotizing enterocolitis, with four cases (17.4%). The first indication was short bowel syndrome in both children (60.9%) and adults (35.7%). The most frequently used type of catheter was tunneled in both children (81.0%) and adults (41.1%). Ending 75 episodes, the most frequent cause was death (52.0%) and change to oral feeding (33.3%). Conclusions: The number of centers and collaborating professionals in the registry of patients receiving HPN remains stable, as well as the main indications and reasons for termination of HPN., Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadyasenpe. com) del año 2018. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE del 1 de enero al 31 de diciembre de 2018. Resultados: se registraron 278 pacientes (54,7% mujeres), 23 niños y 255 adultos, procedentes de 45 hospitales españoles, lo que representa una tasa de prevalencia de 5,95 pacientes/millón de habitantes/año 2018. El diagnóstico más frecuente en adultos fue “oncológico paliativo” (22,0%), seguido de “otros”. En niños fue la enfermedad de Hirschsprung junto con la enterocolitis necrotizante, con cuatro casos (17,4%). El primer motivo de indicación fue síndrome de intestino corto tanto en niños (60,9%) como en adultos (35,7%). El tipo de catéter más utilizado fue el tunelizado tanto en niños (81,0%) como en adultos (41,1%). Finalizaron 75 episodios, la causa más frecuente fue el fallecimiento (52,0%) y el paso a vía oral (33,3%). Conclusiones: el número de centros y profesionales colaboradores en el registro de pacientes que reciben NPD se mantiene estable, así como las principales indicaciones y los motivos de finalización de la NPD.

Hydroxytyrosol (HT) and Punicalagin (PC) exert cardioprotective and anti-atherosclerotic effects. This study evaluates the effect of oral supplementation with HT and PC (SAx) on early atherosclerosis markers in middle-aged, seemingly healthy adults. A randomized, double-blinded, placebo-controlled, crossover trial was performed for 20 weeks. There were two treatment sequences (Placebo/SAx, n = 41, SAx/Placebo, n = 43) for which the intervention periods (Placebo and SAx) were 8 weeks long, followed by a 4-week wash out period. The supplement was composed of 9.9 mg of HT and 195 mg of PC, and the placebo was composed of maltodextrin. SAx increased endothelial function (Flow-mediated dilatation [FMD]: 2.36%, p &lt, 0.001) in the endothelial dysfunction subgroup compared to the placebo (2.36 &plusmn, 3.9 vs. 0.76 &plusmn, 3.5%, p &lt, 0.05). SAx also reduced oxLDL by &minus, 28.74 ng/mL (p &lt, 0.05) in subjects with higher levels of oxLDL, which was an improvement compared with the placebo (&minus, 28.74 &plusmn, 40.2 vs. 25.64 &plusmn, 93.8 ng/mL, p &lt, 0.001). The prehypertension and hypertension subgroups exhibited decreased systolic (&minus, 15.75 &plusmn, 9.9 mmHg, 0.001) and diastolic (&minus, 6.36 &plusmn, 8.7 mmHg, 0.001) blood pressure after SAx consumption. Moreover, the systolic prehypertension and hypertension subgroups presented significant differences in systolic blood pressure compared to the placebo (&minus, 9.9 vs. &minus, 2.67 &plusmn, 12.0 mmHg, p &lt, 0.05). In conclusion, the supplement exerted anti-atherosclerotic effects by improving endothelial function, blood pressure, and levels of circulating oxLDL, especially for persons in whom these parameters were altered.

Individualized parenteral nutrition is the most specialized type of nutritional support in the hospital setting. The composition and prescribing patterns for parenteral nutrition have evolved due to new emerging scientific evidence. In the last few years, there has been a tendency to increase the nitrogen and lipid content and decrease the carbohydrate content. To assess the prescribing pattern in a tertiary referral hospital in Spain, the nutritional composition of individualized parenteral nutrition was evaluated retrospectively from January to December of 2016. A total of 3029 parenteral nutrition units were analysed, corresponding to 257 hospitalized adult patients. Medical specialists in General Surgery and Haematology were the most common petitioners. The three most frequently prescribed parenteral nutrition formulae contained 13.4 (28.8%), 15.7 (19.54%) and 17.9 (17.79%) g of nitrogen. The quantity of carbohydrates and lipids showed a mean non-protein calories-to-nitrogen ratio of approximately 78:1 and a carbohydrate-to-lipid ratio that was near 50:50 in most cases. These results suggest a trend towards the administration of parenteral nutrition with a high content of nitrogen and smaller proportion of the non-protein components.

Over the last years, there has been an increase in adverse food reactions, probably associated with life style changes in the past decades. An adverse food reaction is any clinically abnormal response that can be attributed to ingestion, contact or inhalation of a food, its derivatives or an additive contained in it. They can be classified as food allergy or intolerance. Food allergies are usually immune-mediated, associated with IgE. Adverse reactions to food have a large clinical and social repercussion, which can be fatal in some cases and impair the quality of life of patients. This implies directly the services of collective catering and food manufacturing, which is why a legislature and regulations were implemented for its correct management. The most allergenic foods change according to the age group; being the egg the most frequent in children under 5 years, and fresh fruits in the older than 5 years. The most frequent clinical manifestations are cutaneous-mucous type. Food intolerances may be due to a pharmacological, metabolic, mixed or idiosyncratic mechanism. Clinical manifestations are usually dose dependent. The most common and known food intolerance is lactose, which is a metabolic type. The main treatment of both types of adverse reaction to foods is avoidance of the causal food of the reaction.En estos últimos años se ha evidenciado un aumento de las reacciones adversas a alimentos, probablemente asociadas a los cambios en el estilo de vida producidos en las últimas décadas. Una reacción adversa a alimentos es cualquier respuesta clínicamente anormal que puede atribuirse a la ingestión, contacto o inhalación de un alimento, de sus derivados o de uno de sus aditivos.Pueden clasificarse en alergia o intolerancia alimentaria. Las alergias alimentarias suelen tener un componente inmunológico generalmente asociado a inmunoglobulina E (IgE). Las reacciones adversas a alimentos tienen una gran repercusión tanto clínica como social y perjudican la calidad de vida de los pacientes y, en algunos casos, resultan fatales, de ahí que se implique directamente a los servicios de restauración colectiva y de manufacturación de alimentos para su correcto manejo.Los alimentos más alérgenos cambian según el grupo etario. La alergia al huevo es la más frecuente en menores de 5 años y a las frutas frescas, en los mayores de 5 años. Las manifestaciones clínicas más frecuentes son de tipo cutáneo-mucosas. Las intolerancias alimentarias pueden deberse a un mecanismo farmacológico, metabólico, mixto o idiosincrático, pero no inmunológico. Las manifestaciones clínicas suelen ser dosis dependientes, mientras que las alérgicas son dosis independientes.La intolerancia alimentaria más frecuente y conocida es la intolerancia a la lactosa, que es de tipo metabólica. El tratamiento principal para ambos tipos de reacciones adversas a alimentos consiste en evitar el alimento que causa de la reacción.

Resumen Introducción: la desnutrición relacionada con la enfermedad tiene una elevada prevalencia, con consecuencias clínicas potencialmente graves para el paciente y de alto impacto económico para el sistema sanitario. Objetivos: realizar una revisión de la literatura sobre los costes económicos de la desnutrición asociada a la enfermedad, analizar sus complicaciones y evaluar la utilidad de la nutrición enteral u oral bajo la perspectiva del análisis de costes. Métodos: se llevó a cabo una revisión de la literatura hasta junio de 2016 sobre los costes económicos de la desnutrición y el análisis de costes del tratamiento nutricional, priorizando las revisiones sistemáticas, los metaanálisis y los ensayos clínicos aleatorizados. Resultados: se identificaron 31 publicaciones, 15 sobre los costes de la desnutrición relacionada con la enfermedad y 16 de análisis de costes del tratamiento. La desnutrición relacionada con la enfermedad incrementa los costes sanitarios por una mayor estancia hospitalaria, mayor incidencia de complicaciones infecciosas y no infecciosas, mayor necesidad de tratamientos, incremento de los reingresos, estancias más prolongadas en unidades de cuidados intensivos y/o la necesidad de derivación al alta a centros de continuación de cuidados. Las publicaciones evaluadas sobre el tratamiento mediante suplementos nutricionales orales sugieren que los suplementos nutricionales orales son coste-efectivos y coste-útiles tanto en pacientes ambulatorios como en pacientes hospitalizados. Conclusiones: la desnutrición relacionada con la enfermedad produce un incremento de los costes sanitarios que podría minimizarse, entre otras formas, mediante un diagnóstico y tratamiento precoz de la misma, para lo cual los suplementos nutricionales orales constituyen una herramienta coste-eficaz y coste-útil.

Eating disorders (ED) are characterized by persistent changes in eating habits that negatively affect a person's health and psychosocial abilities. They are considered psychiatric disorders, highly variable in their presentation and severity, with a huge impact on nutrition, which conditions various therapeutic approaches within a key multidisciplinary context. A group of experts in nutrition, we decided to set up a task force adscribed to the "Sociedad Española de Nutrición Parenteral y Enteral" (SENPE), which has stated as one of its goals the development of a consensus document to generate a protocol based on the best scientific evidence and professional experience available in order to improve health care in this field.

Bulimia nervosa and binge eating disorder are unique nosological entities. Both show a large variability related to its presentation and severity which involves different therapeutic approaches and the need to individualize the treatment, thus it is indispensable a multidisciplinary approach. Patients with bulimia nervosa may suffer from malnutrition and deficiency states or even excess weight, while in binge eating disorders, it is common overweight or obesity, which determine other comorbidities. Many of the symptoms and complications are associated with compensatory behaviors. There are many therapeutic tools available for the treatment of these patients. The nutritional approach contemplates the individualized dietary advice which guarantees an adequate nutritional state and nutritional education. Its objective is to facilitate the voluntary adoption of eating behaviors that promote health and allow the long-term modification of eating habits and the cessation of purgatory and bingeing behaviors. Psychological support is a first-line treatment and it must address the frequent disorder of eating behavior and psychiatric comorbidities. Psychotropic drugs are effective and widely used although these drugs are not essential. The management is carried out mainly at an outpatient level, being the day hospital useful in selected patients. Hospitalization should be reserved to correct serious somatic or psychiatric complications or as a measure to contain non-treatable conflict situations. Most of the guidelines' recommendations are based on expert consensus, with little evidence which evaluates clinical results and cost-effectiveness.

Anorexia nervosa is the most common psychiatric disease among young women and it is assumed to be of multifactorial origin. Diagnostic criteria have recently been modified; therefore amenorrhea has ceased to be a part of them. This disease shows a large variability in its presentation and severity which conditions different therapeutic approaches and the need to individualize the treatment, thus it is indispensable a multidisciplinary approach. The goals are to restore nutritional status (through an individualized diet plan based on a healthy consumption pattern), treat complications and comorbidities, nutritional education (based on healthy eating and nutritional patterns), correction of compensatory behaviors and relapse prevention. The treatment will vary according to the patient's clinical situation, and it may be performed in outpatient clinics (when there is clinical stability), in a day hospital or ambulatory clinic (intermediate mode between traditional outpatient treatment and hospitalization) or hospitalization (when there is outpatient management failure or presence of serious medical or psychiatric complications). Artificial nutrition using oral nutritional supplements, enteral nutrition and exceptionally parenteral nutrition may be necessary in certain clinical settings. In severely malnourished patients the refeeding syndrome should be avoided. Anorexia nervosa is associated with numerous medical complications which determines health status, life quality, and is closely related to mortality. There is little clinical evidence to assess the results of different treatments in anorexia nervosa, when most of the recommendations are being based on expert consensus.

Disease-related malnutrition increases health care costs in relation to a longer hospital stay, higher incidence of infectious and non-infectious complications, greater need of treatment, increase in readmissions, more prolonged stay in the intensive care unit and/or the need of referral to continuing care centers at discharge. Los trastornos de la conducta alimentaria (TCA) se caracterizan por una alteración persistente de la conducta relacionada con el hecho de alimentarse que impacta negativamente sobre la salud y las capacidades psicosociales de aquellos que los padecen. Se consideran enfermedades psiquiátricas con una gran variabilidad en su presentación y gravedad, con gran repercusión nutricional, lo que condiciona diferentes planteamientos terapéuticos, haciéndose indispensable un enfoque multidisciplinar. Expertos en nutrición hemos decidido crear un grupo de trabajo adscrito a la Sociedad Española de Nutrición Parenteral y Enteral (SENPE), que ha asumido entre sus objetivos el elaborar un documento de consenso que genere un protocolo basado en la mejor evidencia científica posible y en la experiencia profesional, con la finalidad de mejorar la práctica asistencial en este campo.

Fiber definition includes all those carbohydrates which are not digested nor absorbed in the upper gastrointestinal tract allowing them to reach the colon with no previous processing. Traditionally fiber has been classified according to their solubility into soluble and insoluble and different physiological properties have been defined for each type. The physiologic role of the fiber intake has been studied in diabetes, dyslipidemia or obesity. Fiber intake has also demonstrated to be beneficial in the prevention of many neoplastic diseases like colorectal cancer. It´s also known that fiber plays an important role in the faecal excretion of nitrogen.To evaluate the current evidence that fiber intake plays in the management and prevention of several different diseases, being able to determine, if possible, the most recommended fiber type for each clinical condition.A non-systematic review by searching the Medline and Pubmed was made and studies which met the inclusion criteria were identified and selected for analysis.Different fiber types can be useful for the treatment of several gastrointestinal diseases like constipation, diarrhea, irritable bowel syndrome, ulcerative colitis remission or short bowel syndrome. Patients diagnosed with diabetes, obesity, hyperlipidemia, hypertension and other cardiometabolic diseases can get a clinical improvement with soluble fiber intake. Dietary fiber has demonstrated to play a role in the prevention of colorrectal cancer and other neoplastic diseases. Patients with hepatic encephalopathy or chronic kidney disease will also benefit from fermentable fiber intake.Fiber plays an important role in the prevention and treatment of many clinical conditions. Se clasifica en soluble e insoluble, con propiedades fisiológicas distintas. La fibra se ha empleado frecuentemente para tratar y prevenir diversas patologías gastrointestinales. También se ha estudiado su papel en la fisiopatología de enfermedades como la diabetes, la dislipemia, la hipertensión arterial y la obesidad. Además se ha relacionado su consumo con la prevención de ciertos tumores, en especial del cáncer colorrectal, así como con el aumento de la excreción de nitrógeno en heces. Objetivos: analizar la evidencia del papel que la fibra puede tener en el tratamiento y prevención de distintas enfermedades, así como el tipo de fibra más adecuado en cada una. Métodos: revisión no sistemática en Medline y Pubmed, y posterior aplicación de los criterios de inclusión y exclusión. Resultados: diferentes tipos de fibra pueden ser útiles en el tratamiento de enfermedades gastrointestinales, como el estreñimiento, la diarrea, el síndrome de intestino irritable, la colitis ulcerosa en remisión o el síndrome de intestino corto. Los pacientes con diabetes, obesidad, hiperlipidemia, hipertensión y enfermedad cardiovascular también pueden beneficiarse del consumo principalmente de fibra soluble. La fibra alimentaria ha demostrado prevenir el cáncer de colon y otros tumores. En pacientes con encefalopatía hepática o insuficiencia renal, la fibra fermentable ha demostrado beneficios. Conclusiones: la fibra tiene un papel importante en la prevención y tratamiento de múltiples enfermedades; sin embargo, son necesarios más estudios de calidad para poder realizar recomendaciones más específicas.

For the last 20 years, most adult patients following home parenteral nutrition (HPN) under the care of the Clinical Nutrition and Dietetics Dept. of the La Paz University Hospital (Unidad de Nutrición Clínica y Dietética del Hospital Universitario La Paz), Madrid, Spain, have received their nutrition formula via a catering system (Nutriservice) responsible for its preparation and home distribution.To assess the clinical characteristics, quality of life and degree of satisfaction with the care received, of patients undergoing HPN who received their nutrition formulae via the named catering service.The characteristics of the patients who received HPN via this service between 1992 and 2013 were retrospectively collected. Those patients still undergoing treatment completed a quality of life questionnaire, plus a survey of their satisfaction with the catering system and the care provided by our department.Seventy eight patients were examined; 57.7% were men. The mean age of the patients was 53.1±14.3 years. The most common underlying disease was cancer, both in advanced stage (33.8% of all cases) and in stages in which active treatment was being provided (34%). The most common indication for HPN was intestinal obstruction (46%). The median duration of HPN treatment was 96 [1-5334] days. The most common complication was catheter infection (72%). For the analysis of the results, the patients were divided depending on disease type: those with advanced stage cancer, those with cancer under active treatment and with a better prognosis, and those with non-oncological disease. The patients undergoing active oncological treatment believed the quality of their lives to have been improved by the Nutriservice catering system, and rated the care received by our department positively.In our experience, patients with cancer are those who most commonly receive HPN, especially those with advanced disease. Given the seriousness of their conditions, and the complexity surrounding the use of HPN, catering systems appear to offer a means of improving their quality of life.La mayoría de los pacientes adultos con nutrición parenteral domiciliaria (NPD) que dependen de la Unidad de Nutrición Clínica y Dietética del Hospital Universitario La Paz, reciben desde hace más de 20 años la fórmula de nutrición a través de un sistema de catering que elabora y distribuye directamente al domicilio del paciente.Evaluar las características clínicas, calidad de vida y grado de satisfacción con la atención recibida de los pacientes con nutrición parenteral domiciliaria que recibieron las fórmulas de nutrición mediante el sistema de catering: Nutriservice.Se recogen retrospectivamente las características de los pacientes que utilizaron fórmulas de nutrición parenteral en su domicilio a través de Nutriservice, desde el año 1992 hasta la actualidad. Los pacientes en tratamiento activo realizaron un cuestionario de calidad de vida y encuesta de satisfacción sobre el sistema de catering y la atención prestada por nuestra Unidad.Se registraron 78 pacientes, el 57,7% eran hombres y la edad media 53,1±14,3 años. La patología de base más frecuente fue el cáncer, tanto en estadios avanzados (34%) como en tratamiento activo (34%). El motivo principal de indicación fue la obstrucción intestinal (46%).La duración del soporte nutricional tuvo una mediana de 96 días (1-5334). Las complicaciones más frecuentes fueron las infecciones relacionadas con el catéter (72%). Se analizaron los resultados, clasificando a los pacientes según la enfermedad de base: cáncer en estadio avanzado, cáncer en tratamiento activo y mejor pronóstico y patología no oncológica. Los pacientes en tratamiento activo consideraron afectada su calidad de vida y valoraron positivamente el servicio ofrecido por Nutriservice y la atención recibida de la Unidad de Nutrición.En nuestra experiencia, los pacientes oncológicos son los que con mayor frecuencia tienen nutrición parenteral domiciliaria, siendo muy frecuente su indicación en estadios avanzados de la enfermedad. A la gravedad de la patología de base se une la complejidad de la NPD. Los sistemas de catering podrían ser una alternativa eficaz para mejorar la calidad de vida en algunos de estos pacientes.

Anorexia nervosa is an eating disorder that often causes malnutrition and carries high mortality risk. A multidisciplinary and highly experienced team is needed to succeed in nutrition education and avoid the refeeding syndrome. We report the most severe case of malnutrition secondary to anorexia nervosa treated in our unit, a 33-year-old woman with a BMI of 8.8 kg/m2 and high liver aminotranferases who did not experience any complication during the refeeding process despite the extreme gravity of her situation.La anorexia nerviosa es un trastorno de la conducta alimentaria que con frecuencia ocasiona malnutrición y asocia riesgo de mortalidad. Requiere la colaboración de un equipo multidisciplinar con amplia experiencia clínica para obtener óptimos resultados, una exitosa educación nutricional y evitar el síndrome de realimentación. El caso que se presenta constituye la malnutrición más severa secundaria a anorexia nerviosa que hemos tratado en nuestra unidad: una paciente de 33 años con IMC de 8.8 kg/m2 y elevación de enzimas hepáticas, que, a pesar de la situación de extrema gravedad en la que se encontraba, no presentó complicaciones durante el proceso de realimentación, que se detalla.

Advanced Chronic Kidney Disease (ACKD) is associated with high prevalence of malnutrition. The habitual continuous dietary restrictions used in clinical practice. increased the malnutrition risk. The aim of this study was to evaluate the effects of a Nutritional intervention Program (NIP) on renal function and nutritional status in patients with ACKD.93 patients, (53.7% men, 66±17 years) were included in a prospective longitudinal study. The patients recived a NIP during 6 months with mensual visits. At baseline and six months the outcome assessed were: nutritional status by Chang criteria, anthropometric, dietetic and biochemical parameters (albumin, prealbumin, creatinine clearance, serum phosphorus, potassium, total-Cholesterol, LDL, HDL, triglycerides, and PCR).After intervention, caloric intake decreased in nourished patients (1833 ±318 vs. 1571±219 kcal p=.001). and it was constant in malnourished patients. The intake of protein (69,9 ± 16,6 vs 54,9 ± 11 g p0.001), potassium (2938 ± 949 vs 2377 ± 743 mg p0.001) and phosphorus (1180 ± 304 vs 946,6 ± 211 mg p0.001) significantly decreased. 16.5% patients required supplementation. A total of 41.7% of patients were malnourished at baseline (27.8% mild, 10.10% moderate and 3.8% severe), and 16.8% at the end (8% mild, 5% moderate and 3.8% severe) by Chang criteria. At the end of NIP, patients significantly increased creatinine clearance (17,8 ± 5,2 vs 19,4 ± 6,9 ml/min, p0,01), albumin (3,3 ± 0,5 vs. 3,5 ± 0,4 g/dL, p0,05), and decreased serum potassium (4,8 ± 0,6 vs 4,5 ± 0,5 mmol/L, p0,05), total cholesterol (179,8 ± 44,3 vs 170,0 ± 15,1 mg/dL, p0,05), LDL (113,2 ± 37,0 vs 108,3 ± 27,3 mg/dL, p0.01) and tryglicerides (141.9 ± 60.8 vs 129.9 ± 52.7 mg/dL, p0.05).\The study reflected a NIP usefulness in the nutritional status and renal function improvements within an interdisciplinary framework during ACKD consultations.Introducción y Objetivos: La Enfermedad Renal Crónica Avanzada (ERCA) se asocia a una elevada prevalencia de malnutrición. La práctica habitual en estos pacientes va dirigida a reducir la ingesta proteica, recomendación que podría favorecer esta situación. Disease-Related Malnutrition (DRM) is present in a high percentage of patients admitted to hospital and their impact is diverse and of great importance.Nutrition Department of University Hospital La Paz, in collaboration with all members of the Nutrition Committee, we decided to implement some method of screening in our centre which allowed us to detect as many patients with malnutrition risk as possible.Due to the large size of our centre, with about 1,500 beds and the few human resources in our unit, we chose to use the CONUT system (Nutritional Control), a 100% automatic method based on analytical parameters, very easy to use, low cost and whose validity is confirmed, characteristics that fulfilled our needs. The method collects information from databases of Admission Service (affiliation, age, date) and Laboratory (albumin, cholesterol, total lymphocytes) and generates, in the report of analytical results, "alert" information about each patient''s nutritional risk and also nutritional recommendations based on the risk identified. Prior to its implantation several evaluations were performed in order to allow us to better know the extra workload, as it was the main factor that could limit our ability to assume that screening method and also many training activities for doctors and hospital health professionals who were increasingly assuming responsibilities in the nutritional treatment of their patients. This screening system has been working from June of 2010 and it has detected risk of malnutrition in 32% of the evaluated patients. In general, only in those cases where the alert corresponded to a high-risk nutritional situation, which represents approximately 10% of the total, specialized intervention by the Clinical Nutrition Unit was required. Later, as we have detected some possible causes that justify the system does not work properly; the screening system has undergone several changes . In this sense, we have made a retrospective evaluation of 233 patients admitted to the General Surgery plants and documented that those entering for scheduled surgery, having a alarm of malnutrition (50% of cases), its presence increases significantly the hospital stay and mortality.The implementation of this nutritional screening method has led to a change in the "nutritional" culture of our centre respect to DRM in most of our professionals: doctors and nurses and even in the management team, so all of them understand the importance of the process and know about the available tools and knowledge to indicate an adequate and early nutritional support.La malnutrición asociada a la enfermedad (MAE) está presente en un porcentaje muy elevado de los pacientes que ingresan en los hospitales y su repercusión es diversa y de mucha trascendencia. Objetivo. Desde la Unidad de Nutrición del Hospital Universitario la Paz, en colaboración con todos los miembros de la Comisión de Nutrición, nos planteamos implantar algún método de cribado en nuestro centro, que permitiese abarcar al mayor número posible de pacientes. Resultados. Debido al gran tamaño de nuestro centro, con cerca de 1500 camas, unido a una escasez de recursos humanos en nuestra unidad, optamos por utilizar el sistema de cribado CONUT (Control Nutricional) 100 % automático y basado en parámetros analíticos, muy fácil de manejar, de bajo coste y cuya validez está confirmada, adaptándolo a nuestras necesidades. El método recoge información de las bases de datos del servicio de Admisión (filiación, edad, fecha) y del Laboratorio (albúmina, colesterol, linfocitos totales) y genera, en el informe de resultados de los análisis clínicos, la información sobre la alerta de riesgo nutricional de cada paciente, así como las recomendaciones nutricionales a seguir en función del riesgo detectado en cada caso. Para llegar su implantación se llevaron a cabo diferentes evaluaciones que nos permitieron conocer previamente la carga extra de trabajo que podría generar su implantación y nuestra capacidad para asumirlas, así como numerosas actividades formativas encaminadas a que los médicos y profesionales sanitarios del hospital asumieran cada vez más responsabilidades en el proceso del tratamiento nutricional de sus pacientes. Este sistema de cribado funciona desde Junio del año 2010 y detecta riesgo de desnutrición en el 32 % de los pacientes evaluados. En general, sólo en los casos en los que la alerta corresponde a una situación de Alto riesgo nutricional, hecho que se da aproximadamente en el 10 % del total, se requirió la intervención especializada por parte de la Unidad de Nutrición Clínica y Dietética. Posteriormente, conforme hemos ido detectando posibles causas que justificaran que el sistema no funcionase adecuadamente, el sistema de cribado ha sido objeto de diversas modificaciones. En este sentido, se ha realizado una evaluación retrospectiva en 233 pacientes ingresados en las plantas de Cirugía General y se ha documentado que aquellos que ingresan para cirugía programada y presentan una alarma alta o moderada de desnutrición (el 50 % de los casos), la presencia de la misma aumenta significativamente los días de ingreso y la mortalidad. Conclusiones. La implantación del cribado nutricional ha favorecido un cambio en la cultura de nuestro centro con respecto a la malnutrición asociada a la enfermedad y la mayoría de nuestros profesionales, tanto médicos como enfermeras e incluso el equipo directivo, comprenden la importancia del proceso y saben que disponemos de herramientas y de conocimiento suficiente para indicar un soporte nutricional adecuado y precoz.

To identify predictive variables of low adherence to a pilot Lifestyle Modification Program (LMP) for overweight and obesity treatment in primary health care (PC ).Sixty subjects with BMI27 kg/m(2) were recruited. Health professionals directed the program in a group structure and biweekly, based on nutrition education with individualized dietary guidelines, promotion of physical activity and motivational support. A validated questionnaire on lifestyle habits for overweight and obesity subjects was used to identify variables related with program adherence and anthropometric variables were measured before and 6 months after intervention. Low adherence was considered when patients attended to less than 80% of visits.Twenty-seven subjects (45%) presented high adherence to the program. The variables associated to low adherence were related to baseline with IMC ≥ 35 kg/m(2) (p0.05); ex smoker period ≤ 4 months (p0.01); high caloric diet (p0.01) and scarce physical activity (p0.05). At 6 months the subjects who finalized the program presented a significant decrease of weight (86.0 ± 15.6 vs 79.2 ± 13.4 kg; p0,001); fat mass percentage (41,6 ± 4,6 vs 38.8 ± 5,4%; p0.001), blood glucose (108 ± 45.48 vs 94.38 ± 11.97 mg/dl; p0.01). It also improved caloric diet profile, above all decreasing the percentage of fat (39.6 ± 4.8 vs 35.5 ± 5.6%; p0.01).Have recently left smoking, obesity degree two or higher, a high caloric diet and scarce physical activity were basal variables identified as predictive of a low adhesion to a LMP for the treatment of overweight and obesity in primary health care. In the last three decades, the prevalence of obesity in developed countries has reached epidemic proportions, and continues rising. Many factors have influence on the incidence of obesity, and with the decline of physical activity, overeating plays a role in the emergence of this public health problem. Although a clear relationship between fat intake and weight gain has been established, the role of carbohydrates and more specifically from sucrose and the development of obesity is more controversial. Much of this controversy is due to the growing demand for sweetened drinks and caloric increase posed by these in the diet. Despite multiple studies and communications on this subject in recent years, there are still many areas of uncertainty about the role played by diets rich in sugars over the increase in obesity in last years.En las últimas tres décadas, la prevalencia de obesidad en los países desarrollados ha alcanzado dimensiones epidémicas y continua en aumento 1. Existen múltiples factores que influyen en la incidencia de obesidad, y junto con el descenso de actividad física ,el exceso de ingesta, juega un papel preponderante en la aparición de este problema de salud pública 2. Aunque existe una clara relación entre la ingesta de grasas y la ganancia de peso, el papel de los carbohidratos y más concretamente el de la sacarosa en el desarrollo de obesidad es más controvertido. Gran parte de esta controversia se debe a la creciente demanda de bebidas azucaradas y al posible incremento calórico en la dieta asociado a su consumo. A pesar de la publicación de múltiples estudios y comunicaciones a este respecto en los últimos años, siguen existiendo numerosas incógnitas acerca del papel que juegan las dietas ricas en azúcares en el incremento de incidencia y prevalencia de obesidad en los últimos años.

Background & aims: Several authors have reported low folate intake in patients with eating disorders (ED). This vitamin plays an essential role in synthesis reactions for neurotransmitters and structural elements of neurons, and therefore its deficiency has been associated with the presence of different disorders linked to mental function. The aim of this study was to determine the effect of folic acid supplementation on homocysteine levels and the cognitive and depressive status of a group of patients with eating disorders with low folate intake. Subjects/methods: The study was designed as a randomised, prospective clinical trial, which included 24 participants assigned to two treatment groups for six months: supplemented group (SG) (10 mg/day of folic acid [ACFOL®]) and a placebo group (PG). Both groups maintained their medical, dietary and psychological treatment. At baseline and end of the intervention, anthropometric, dietary and biochemical parameters (plasma homocysteine [Hcy], serum and red blood cell folate) were recorded. Cognitive and depressive status questionnaires were administered (Stroop Test, Trail Making Test and Beck Depression Inventory). Results: Twenty-two patients completed the study (SG: 12, PG: 10, mean age: 24.2 ± 8.8 years, BMI 18.9 ± 3.5 kg/m2). The SG significantly increased their serum and red blood cell folate levels and lowered Hcy levels (9.4 ± 2.4 μmol/l vs. 7.5 ± 1.7 μmol/l, P < 0.01). The SG also significantly improved most of their test scores for cognitive and depressive status. The PG showed no significant changes in any of the evaluated variables. Conclusions: The results show that folic acid supplementation may be used as another tool within the comprehensive and multidisciplinary treatment applied to patients with ED, Introducción y objetivo: Diferentes autores han reportado una baja ingesta de ácido fólico en pacientes con Trastornos de la Conducta Alimentaria (TCA). Esta vitamina desempeña un papel esencial en las reacciones de síntesis de neurotransmisores y elementos estructurales de las neuronas y, por lo tanto, su deficiencia se ha asociado con la presencia de diferentes trastornos relacionados con la función mental. El objetivo de este estudio fue determinar el efecto de la suplementación con ácido fólico sobre los niveles de homocisteína y sobre marcadores de función cognitiva y depresión en un grupo de pacientes con TCA con baja ingesta de ácido fólico. Sujetos y métodos: Estudio clínico randomizado y prospectivo en el que se incluyeron 24 pacientes asignados a dos grupos de tratamiento durante un período de 6 meses: grupo suplementado (SG) (10 mg/día de ácido fólico [ACFOL®]) y grupo placebo (PG). Ambos grupos mantuvieron su tratamiento médico, dietético y psicológico. Al inicio del estudio y tras la intervención se evaluaron parámetros antropométricos, dietéticos y bioquímicos (homocisteína plasmática [Hcy], folato sérico y eritrocitario). Como marcadores de función cognitiva y depresión se administraron diferentes cuestionarios (Test de Stroop, Trail Making Test, BDI: Cuestionario de percepción de función cognitiva). Resultados: Completaron el estudio 22 pacientes (SG: 12, PG: 10, edad media: 24,2 ± 8,8 años, IMC 18,9 ± 3,5 kg/m2). El grupo SG incrementó de forma significativa sus niveles de folato sérico y eritrocitario y redujo el de homocisteína (9,4 ± 2,4 μmol/l vs. 7,5 ± 1,7 μmol/l, P < 0,01). Además, el grupo SG también mejoró significativamente las puntuaciones de los test de función cognitiva y depresión. En el grupo PG, en cambio, no se observaron cambios significativos en ninguna de las variables evaluadas. Conclusiones: Los resultados obtenidos demuestran que la suplementación con ácido fólico podría emplearse como una herramienta más dentro del complejo y multidisciplinario tratamiento que requieren estos pacientes
