Allergenexposition führt zu einer Induktion von Th2- und Th9-Zellen sowie zu einer Reduktion von regulatorischen T-Zellen, während sich bei allergenspezifischer Immuntherapie eine Suppression von Th2- und Th9-Zellen zeigt. Zudem zeigt sich ein Abfall von IL9 und TGFß bei erfolgter AIT bei gleichzeitig erhöhten Th9 und verringerten Treg-Spiegeln. TGFß und IL9 zeigen sich somit konkordant zu Th9-Zellen proinflammatorisch, während Treg-Zellen TGFß-unabhängig immunmodulatorisch wirken., Allergen exposure induced local and systemic Th2, Th9 cell and reduced Tregs, while therapeutic allergen exposure by AIT suppressed Th2 and Th9 cell frequencies. On cytokine levels IL-9 and TGFß in patients who underwent AIT were decreased along induced Th9 levels and decreased Treg cells. Altogether, these findings support that TGFß and IL9 in allergic airway inflammation act concordantly to Th9-cells, while Treg cells seem to be immunomodulatory and TGFß-independent.

Ziel dieser Arbeit ist eine Identifizierung der Innaten Typ 2 Lymphoid Zellen, in zwei Kompartimenten (lokal in der Lunge vs. peripher im Blut), in Abhängigkeit von saisonalen Einflüssen und Immuntherapie bei Probanden mit allergischer Rhinitis/allergischem Asthma und gesunden Kontrollen. Bei 58 Probanden wurden in zwei Untersuchungszeiträumen (Pollenflug) klinische Daten, Sputum- und Blutproben gesammelt und mittels Durchflusszytometrie aufgearbeitet. We collected clinical data, sputum and blood samples from 58 participants and analyzed them using flow cytometry. The ILC2s, through their production of type 2 cytokines, can initiate and propagate an allergic reaction while also being suppressed through immunotherapy.

Non-steroidal anti-inflammatory drug (NSAID)- exacerbated respiratory disease is a severe inflammatory condition affecting particularly the airways and nasal sinuses. Macrophages as one of the key players in immune regulation yet have been neglected in N-ERD research. Through our experiments we aimed to reveal differences in aMDM (alveolar-like macrophages) and sMac (sputum macrophages) as well as alterations in body fluids of N-ERD, CRSwNP and healthy subjects. Makrophagen und Monozyten spielen eine entscheidende Rolle bei allergischen Entzündungsprozessen im respiratorischen Trakt. In dieser Studie werden die inflammatorischen und regulativen Eigenschaften dieser Zellen analysiert. Zudem werden die Vorteile einer erfolgreichen allergenspezifischen Immuntherapie (AIT) bei Allergikern demonstriert. Die Expression von PD-L1 auf Monozyten sowie der Neopterin-Spiegel im Sputum können als Biomarker für den Erfolg der AIT dienen. Weitere therapeutische Angriffspunkte stellen das Chemokin IP-10, die PD-Liganden sowie das Enzym IDO dar., Macrophages and monocytes play a critical role during allergic inflammatory processes in the respiratory tract. In this study, the inflammatory and regulatory characteristics of these cells are analyzed. The benefits of a successful allergen-specific immunotherapy (AIT) for allergic patients are demonstrated. The expression of PD-L1 on monocytes as well as the level of Neopterin in sputum can be used as biomarkers for treatment success by AIT. This study addressed the contribution of IL-12 to the formation of psoriatic lesions in mice by use of the IMQ-model and studied the impact of high dietary salt intake on skin tissue-resident immune cells and skin inflammation. It could be shown that in the skin IL-12 mediates a tissue-protective, regulatory programme, which limits psoriatic inflammation. The study further demonstrated that excessive salt intake primes the skin-resident immune system towards an inflammatory type 17 response, thus presenting high salt intake as a potential dietary trigger for psoriasis development., Die Arbeit untersucht den Beitrag von IL-12 zur Entstehung psoriatischer Läsionen in der Maus unter Anwendung des IMQ-Modells und analysiert die Wirkung einer salzreichen Diät auf Haut-ansässige Immunzellen und Hautentzündungen. Es zeigte sich, dass IL-12 ein schützendes, regulatorisches Programm in der Haut induziert, welches die psoriatische Entzündung begrenzt. Die Arbeit zeigt außerdem, dass übermäßiger Salzkonsum das Haut-Immunsystem in Richtung einer entzündlichen TH17 Immunantwort polarisiert und präsentiert damit eine hohe Salzaufnahme als möglichen Ernährungsfaktor für die Entwicklung von Psoriasis.

Treatment options for chronic inflammatory airway diseases are limited due to an incomplete understanding of the underlying pathomechanisms. In this thesis, metabolic- and immune cell profiles of patients with NSAID-exacerbated respiratory disease (N-ERD), a severe type 2 inflammatory condition, were characterized by multi-omics approaches. N-ERD macrophages displayed a persistent, pro-inflammatory phenotype, which was associated with local and systemic lipid metabolism alterations. Our study suggests that pro-inflammatory macrophage reprogramming is a pathomechanism of N-ERD., Die Therapiemöglichkeiten für chronisch-entzündliche Atemwegserkrankungen sind aufgrund eines unvollständigen mechanistischen Verständnisses limitiert. In dieser Studie wurden metabolische und immunologische Profile von Patienten mit Samter Trias (engl. N-ERD), einer besonders schweren Atemwegsentzündung des Typs 2, mittels Multi-Omics-Ansätzen charakterisiert. SARS-CoV-2 has evolved to enter the host via the ACE2 receptor which is part of the kinin-kallikrein pathway. This complex pathway is only poorly understood in context of immune regulation but critical to control infection. This study examines SARS-CoV-2-infection and epithelial mechanisms of the kinin-kallikrein-system at the kinin B2 receptor level in SARS-CoV-2-infection that is of direct translational relevance. From acute SARS-CoV-2-positive study participants and -negative controls, transcriptomes of nasal curettages were analyzed. Primary airway epithelial cells (NHBEs) were infected with SARS-CoV-2 and treated with the approved B2R-antagonist icatibant. SARS-CoV-2 RNA RT-qPCR, cytotoxicity assays, plaque assays, and transcriptome analyses were performed. The treatment effect was further studied in a murine airway inflammation model in vivo. Here, we report a broad and strong upregulation of kallikreins and the kinin B2 receptor (B2R) in the nasal mucosa of acutely symptomatic SARS-CoV-2-positive study participants. A B2R-antagonist impeded SARS-CoV-2 replication and spread in NHBEs, as determined in plaque assays on Vero-E6 cells. B2R-antagonism reduced the expression of SARS-CoV-2 entry receptor ACE2, G protein-coupled receptor signaling, and ion transport in vitro and in a murine airway inflammation in vivo model. In summary, this study provides evidence that treatment with B2R-antagonists protects airway epithelial cells from SARS-CoV-2 by inhibiting its replication and spread, through the reduction of ACE2 levels and the interference with several cellular signaling processes. Future clinical studies need to shed light on the airway protection potential of approved B2R-antagonists, like icatibant, in the treatment of early-stage COVID-19.
