BACKGROUND: For the prevention of attacks of hereditary angioedema (HAE), the efficacy and safety of subcutaneous human C1-esterase inhibitor (C1-INH [SC]; HAEGARDA, CSL Behring) was established in the 16-week Clinical Study for Optimal Management of Preventing Angioedema with Low-Volume Subcutaneous C1-Inhibitor Replacement Therapy (COMPACT). OBJECTIVE: To assess the long-term safety, occurrence of angioedema attacks, and use of rescue medication with C1-INH(SC). METHODS: Open-label, randomized, parallel-arm extension of COMPACT across 11 countries. Patients with frequent angioedema attacks, either study treatment-naive or who had completed COMPACT, were randomly assigned (1:1) to 40 IU/ kg or 60 IU/kg C1-INH(SC) twice per week, with conditional uptitration to optimize prophylaxis (ClinicalTrials.gov registration no. NCT02316353). RESULTS: A total of 126 patients with a monthly attack rate of 4.3 in 3 months before entry in COMPACT were enrolled and treated for a mean of 1.5 years; 44 patients (34.9%) had more than 2 years of exposure. Mean steady-state C1-INH functional activity increased to 66.6% with 60 IU/kg. Incidence of adverse events was low and similar in both dose groups (11.3 and 8.5 events per patient-year for 40 IU/kg and 60 IU/kg, respectively). For 40 IU/kg and 60 IU/kg, median annualized attack rates were 1.3 and 1.0, respectively, and median rescue medication use was 0.2 and 0.0 times per year, respectively. Of 23 patients receiving 60 IU/kg for more than 2 years, 19 (83%) were attack-free during months 25 to 30 of treatment. CONCLUSIONS: In patients with frequent HAE attacks, long-term replacement therapy with C1-INH(SC) is safe and exhibits a substantial and sustained prophylactic effect, with the vast majority of patients becoming free from debilitating disease symptoms. Hypotone Infusionslösungen können aufgrund ihrer im Vergleich zum Plasma erniedrigten Konzentration an Natriumionen insbesondere bei einem bestehenden Syndrom der inadäquaten ADH-Sekretion (SIADH) eine Hyponatriämie provozieren oder verstärken. Bis zu 30 Prozent der hospitalisierten Patienten sind von einer Hyponatriämie betroffen. Damit ist vor allem das Risiko des Auftretens schwerer neurologischer Ausfallerscheinungen verbunden. Einige Meldungen schwerwiegender Nebenwirkungen in den letzten Jahren zeigen, dass dieses Risiko einer durch hypotone Infusionslösungen verursachten Hyponatriämie bei hospitalisierten Patienten möglicherweise unterschätzt wird. Neuere Nebenwirkungsmeldungen führten dazu, dass dieses Thema vor Kurzem im europäischen Ausschuss für Risikobewertung im Bereich der Pharmakovigilanz (PRAC) diskutiert wurde.

Modern Web data is highly structured in terms of entities and relations from large knowledge resources, geo-temporal references and social network structures, resulting in a massive multidimensional graph. This graph essentially unifies both the searcher and the information resources that played a fundamentally different role in traditional information retrieval. Graph search-based systems offer major new ways to access relevant information. Graph search affects both query formulation (complex queries about entities and relations building on the searcher’s context) as well as result exploration and discovery (slicing and dicing the information using the graph structure) in a completely novel way. This new graph based approach introduces great opportunities, but also great challenges, in terms of data quality and data integration, user interface design, and privacy.

Erythropoietin, ein Glykoprotein, das die Bildung von Erythrozyten aus Vorläuferzellen des Stammzellkompartiments stimuliert, wird zur Vorbeugung der bei Frühgeborenen häufig auftretenden Anämie angewendet und trägt möglicherweise zur Entwicklung einer Retinopathie bei. Nach der Publikation von Cochrane-Metaanalysen in den Jahren 2012 und 2014, deren Ergebnisse auf ein zusätzliches Risiko der Retinopathie durch die Anwendung von Erythropoietin hinweisen, wurde auf europäischer Ebene ein Signalverfahren durchgeführt, in dem das potenzielle Risiko bewertet wurde. Im Folgenden wird über die Ergebnisse des europäischen Verfahrens berichtet.

The emergence of Web 2.0 and the semantic Web as established technologies is fostering a whole new breed of Web applications and systems. These are often centered around knowledge engineering and context awareness. However, adequate temporal formalisms underlying context awareness are currently scarce. Our focus in this paper is two-fold. We first introduce a new OWL-based temporal formalism - TOWL - for the representation of time, change, and state transitions. Based hereon we present a financial Web-based application centered around the aggregation of stock recommendations and financial data.

The magnetic properties of the mixed compounds CoxNi1 x 3 V2O8 CNVO investigated by magnetization and neutron diffraction measurements are presented. Unlike their parent compounds Ni3V2O8 NVO and Co3V2O8 CVO , only one magnetic phase transition into an antiferromagnetic phase was detected for powder samples with x 0.27, 0.52, and 0.76. The magnetic structures are modulated according to a propagation vector k delta,0,0 with delta being dependent on the composition parameter x. Furthermore, magnetization data of a CVO single crystal is featured, which is qualitatively different from previous publications and exhibits a controversial aspect concerning the behavior of the curve under an applied magnetic field along the b axis
