Abstract

36歳, 主婦.主訴は排尿困難.入院時, 外尿道口6時の方向から腟前壁にかけて隆起する表面平滑で充実性の腫瘤を認めた.骨盤部MRIにて外尿道口から膀胱頸部にかけて紡錘形の腫瘤を認めた.この腫瘤はT1強調画像で筋肉と同信号, T2強調画像でも筋肉とほぼ同程度の信号を呈しており, 内部に一部点状の高信号領域を認めた.周囲とは境界明瞭で, ガドリニウム造影にて腫瘤全体が均一に造影された.傍尿道平滑筋腫と診断し, 脊椎麻酔下に載石位にて手術を施行した.腫瘍が尿道内腔と連続していないことを確認した上で, 腫瘤茎部を可能な限り切除した.摘出標本は重量20g, 弾性硬の充実性腫瘤であった.病理検査でleiomyomaの診断であった.術後は順調に経過し, 術前にあった排尿困難は消失した, We experienced a case of paraurethral leiomyoma in a 36-year-old woman. She had been complaining of dysuria. She had noticed the presence of a mass in anterior vagina. That mass was about 3 x 3 cm, floating moderately firm with a smooth face sited in a paraurethral region. Magnetic resonance imaging showed medium-signal intensity on T1-weighted images and a homogeneous low signal on T2-weighted images. We diagnosed it as a paraurethral leiomyoma and enucleated it by surgery. At histological examination the tumor resulted in being a leiomyoma. The diagnosis and the treatment of female paraurethral leiomyoma are discussed.

16. A mechanism of antigen-induced goblet cell degranulation in the nasal epithelium of sensitized rats

Abstract

Background: We have produced hypertrophic and metaplastic changes of goblet cells in nasal epithelium of ovalbumin (OVA)-sensitized rats by intranasal challenge with OVA. A variety of allergic mediators and inflammatory cells are capable of stimulating goblet cell degranulation (epithelial mucus secretion); however, little is known about the mechanism by which antigen induces mucus hypersecretion. Objective: Our aim was to explain the mechanism of goblet cell degranulation in allergic inflammation. Methods: Antigen-induced goblet cell degranulation was evaluated by the transient decrease of epithelial mucosubstance 1 to 6 hours after intranasal challenge with OVA in nasal epithelium of OVA-sensitized rats. The effects of the H1-antagonist (d -chlorpheniramine malate), H2-antagonist (cimetidine), atropine, indomethacin, cysteinyl leukotriene (cysLT) antagonist (ONO1078), and antirat PMN antiserum on OVA-induced goblet cell degranulation were examined. Results: Goblet cell secretion 1 hour after OVA challenge was significantly inhibited by H1-antagonist, atropine, and cysLT antagonist, whereas the secretion 6 hours after the challenge was significantly inhibited by cysLT antagonist and antirat PMN antiserum. Circulating PMN cells and mucosal infiltrating eosinophils were completely abolished by antirat PMN antiserum. Conclusions: These results indicate the different mechanisms of goblet cell secretion between early-phase (1 hour after OVA challenge) and late-phase (6 hours after the challenge) reactions. Histamine stimulates early-phase secretion through the H1-receptor of cholinergic nerve terminals, and infiltrating inflammatory cells (eosinophils and/or neutrophils) play a role in late-phase secretion. CysLTs (leukotrienes C4, D4, and E4) are important for both early-phase and late-phase secretion.

Abstract

BackgroundChronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an immune-mediated peripheral neuropathy that is currently classified into several clinical subtypes, which are presumed to have different pathogenic mechanisms. Recently, studies identified a subgroup of patients with CIDP who were positive for IgG4 autoantibodies against paranodal proteins, such as neurofascin-155 and contactin-1, who respond poorly to first-line therapies for typical CIDP, including intravenous immunoglobulin therapy. ObjectiveThis study aims to evaluate the efficacy and safety of intravenous rituximab according to IgG4 autoantibody status in patients with refractory CIDP. MethodsThe Evaluation of the Efficacy and Safety of Rituximab in Refractory CIDP Patients with IgG4 Autoantibodies in the Exploratory Clinical (RECIPE) trial consists of 2 cohorts: a multicenter, placebo-controlled, randomized study cohort of 15 patients with IgG4 autoantibody-positive CIDP (rituximab:placebo = 2:1) and an open-label trial cohort of 10 patients with antibody-negative CIDP. The primary endpoint is improvement in functional outcome assessed using the adjusted Inflammatory Neuropathy Cause and Treatment Disability Scale score at 26, 38, or 52 weeks after the start of treatment with rituximab in patients with CIDP and anti-paranodal protein antibodies. Secondary outcome measures include grip strength, manual muscle testing sum scores, results of nerve conduction studies, and other functional scales. ResultsWe plan to enroll 25 cases for the full analysis set. Recruitment is ongoing, with 14 patients enrolled as of January 2020. Enrollment will close in September 2020, and the study is planned to end in December 2021. ConclusionsThis randomized controlled trial will determine if rituximab is safe and effective in patients with anti-paranodal antibodies. An open-label study will provide additional data on the effects of rituximab in patients with antibody-negative CIDP. The results of the RECIPE trial are expected to provide evidence for the positioning of rituximab as a pathogenesis-based therapeutic for refractory CIDP.
