1. Clinical Efficacy of an Osmotic, Antiviral and Anti-Inflammatory Polymeric Nasal Film to Treat Covid-19 Early-Phase Respiratory Symptoms

When symptoms appear, the disease physiopathology is already established. The immune system tries to control the infection but if the infection persists, it gets tired, fatigued, stressed, and finally, there is burnout. Therefore, an effective treatment should be multi-targeted and should not focus only on one parameter. Minimizing virus concentration, reducing nasal mucosa inflammation, and keeping the nasal surface clean should lessen systemic infection and the probability of developing severe respiratory distress. We evaluated clinical efficacy and safety of an osmotic nasal surface cleaning, virus and cytokine trapping polymeric film, in early-stage Covid-19 positive symptomatic patients.Methods: A randomized, multicentric, observational study was performed to evaluate the efficacy and safety of the osmotic film in 213 patients, randomized in 2 equal arms, and confined for 14-days just after the RT-PCR+ test. Due to ethical reasons, all patients received symptomatic treatments (ST). In addition, the patients in Arm-1 received the test product (ST+TP, 2– 3 sprays, 4– 5 times/day) for 14-days. Leicester Cough Questionnaire, Visual Analogue Scale, and Covid-19-associated symptoms such as fever, pain, taste, smell, and headache were evaluated daily. Home-confined patients were immediately hospitalized in case of aggravation of any life-threatening clinical sign.Results: Among 213 patients, 98 in the ST and 102 in the ST+TP group completed the study. Thirteen patients did not complete the study as 8 in the ST group and 5 in the ST+TP group were hospitalized during the study due to low blood oxygen levels or sudden health deterioration and were excluded from the study. In the remaining control standard treatment group (n=98), clinical signs sharply aggravated on day 1– 2 followed by stabilisation between days 3– 6 and progressive reduction thereafter. In the test product group (n=102), symptoms were stabilized just after the first application and improved progressively. Only 42/102 patients in the test product group presented nasal discharge or irritation due to the osmotic properties of the product.Conclusion: The continuous multi-target approach of detaching and neutralizing virus particles and pro-inflammatory cytokines from the nasal surface to minimize systemic virus exposure is a very logical and efficient approach to avoid Covid-19-induced systemic pathology.Keywords: clinical trial, Covid-19, osmotic filmogen barrier

This triggers a strong cytokine release and inflammation of the nasal mucosa. A multitarget approach of cleaning the nasal mucosa and suppressing chances of nasal and systemic inflammation should minimize severe respiratory consequences. Unfortunately, no such treatments are yet available.Methods: We describe the conception of an osmotic polymeric film using an in vitro nasal mucosa mimicking model, containing polymers to neutralize Covid-19 specific viral S1, RBD proteins and selected proinflammatory cytokines.Results: The filmogen barrier forms a stable and osmotic film on the nasal mucosa. Hypotonic liquid exudation from the nasal surface detaches and drains the inflammatory cytokines and other contaminants towards the film where selected polymers bind and neutralize SARS-CoV-2 spike S1 and RBD protein as well as Covid-19 disease-specific key pro-inflammatory IL-6, TNF-α, IL-10, IL-13, and GM-CSF cytokines.Conclusion: Minimizing the nasal surface concentration of pro-inflammatory cytokines and viruses should help nasal mucosa repair and avoid immune stress. Ravi Shrivastava, Nathalie Cucuat, Monika Rousse, Thomas Weigand, Pedro Neto, Claire Janicot, Christiane Shrivastava VITROBIO Research Institute, Issoire, France Purpose: Incidence of chronic wounds is constantly rising worldwide, but all currently available treatments are intended either to provide symptomatic relief or to assist cicatrization to some extent, but not to directly stimulate cellular growth. Physiologically, chronic wound healing simply requires cell growth to fill the injured cavity. To grow, our cells need to attach onto a cushion, called extracellular matrix (ECM), secreted by the mother cells and composed of multiple proteins. Recent scientific works prove that the concentration of certain matrix metalloproteinases (MMPs) is extremely high in all chronic wounds and, because of their proteolytic nature, some MMPs completely degrade the ECM, hindering cell attachment and cell growth. The aim of this study was to identify, neutralize, and eliminate these MMPs from the wound surface so as to design an effective treatment for chronic wounds. Methods: Acute and chronic models of human epithelial and fibroblast cells were prepared on a defined ECM cushion in vitro and MMPs were added in the culture medium to identify the MMPs causing ECM disintegration for each cell type. ECM-degrading MMPs were then incubated with selected procyanidin-rich plant extracts (PCDs) and cell growth was reanalyzed. Results: It was observed that: 1) multiple MMPs are involved in cellular matrix destruction; 2) ECM-destroying MMPs are specific with respect to cell type; and 3) specific PCDs may bind and neutralize selected MMPs. Conclusion: Topical application of specific plant PCDs to selectively neutralize ECM-destroying MMPs in acute and chronic wounds represents a novel approach for the treatment of superficial and deep skin wounds. Abstract

Rickets is the failure of mineralization of osteoid and newly formed bones in a child skeleton. It is commonly associated with vitamin D deficiency; however, it can be because of a decrease in the serum phosphate levels leading to inadequate mineralization of cartilage and bone, consequent skeletal deformities, and growth retardation. The hypophosphatemic conditions that interfere in bone mineralization comprise many hereditary or acquired diseases. One of the hereditary types of hypophosphatemic rickets is the familial hypophosphatemic rickets. This rare variety was diagnosed in a 9-year-old patient who had come with a chief complaint of a missing tooth. In the present case, radiographic aspects of oral and systemic manifestations of familial hypophosphatemic rickets are highlighted.

Abstract

Background: There is a high failure rate of endodontic treatment in the mandibular incisors due to insufficient knowledge of the morphology. Aim: The aim of this study was to analyze the root canal morphology in mandibular incisor teeth in the North Indian subpopulation to improve the outcome of endodontic treatment. Setting and Design: A cross-sectional study was conducted at the OPD of a tertiary care institute. Materials and Methods: A total of 200 mandibular central incisors and 200 mandibular lateral incisors were selected from available cone-beam computed tomography examinations of 100 patients aged 18–79 years. They were evaluated for root canal system configuration and symmetry between right and left elements in the same individual. Statistical Analysis: Chi-square/Fisher’s exact test was used to find the association between different study variables. Results: For mandibular central incisors, type I Vertucci classification was present in 69% of cases, type II in 3.5%, type III in 18.5%, type IV in 2%, and type V in 7% of cases. For mandibular lateral incisors, type I was present in 70.5% of cases, type II in 4% of cases, type III in 19.5%, type IV in 2%, and type V in 4% of cases. There was no evidence of types VI, VII, and VIII. Symmetry of root canal morphology between left and right teeth was observed in 90% of central incisors and 93% of lateral incisors. Conclusion: Type I Vertucci configuration was the most prevalent; however, the prevalence of other configurations is also evident. Therefore, endodontists should explore the morphology of these teeth carefully while performing root canal treatments.

Abstract

Cefepime/zidebactam is in clinical development for the treatment of carbapenem-resistant Gram-negative infections. MICs of cefepime/zidebactam (1:1) and comparators against Enterobacterales (n = 563), Pseudomonas (n = 172) and Acinetobacter baumannii (n =181) collected from 15 Greek hospitals (2014-2018) were determined by reference broth microdilution method. The isolates exhibited high carbapenem resistance rates [(Enterobacterales (75%), Pseudomonas (75%) and A. baumannii (98.3%)]. Cefepime/zidebactam showed MIC50/90 of 0.5/2 mg/L, against Enterobacterales including metallo-β-lactamases (MBL)-producers. Reduced susceptibility rates to tigecycline (16.8%), colistin (47.4%), ceftazidime/avibactam (59.8%), and imipenem/relebactam (61%) indicated high prevalence of multi-drug resistance among Greek Enterobacterales. Cefepime/zidebactam exhibited MIC50/90 of 8/16 mg/L against Pseudomonas including MBL-producers. The MIC50/90 of ceftazidime/avibactam and imipenem/relebactam were high (≥32 mg/L). Cefepime/zidebactam showed MIC90 of 64 mg/L against A. baumannii which is within its therapeutic scope. Other antibiotics including colistin showed limited activity against A. baumannii. The activity of cefepime/zidebactam against multi-drug-resistant isolates is attributable to zidebactam mediated novel β-lactam-enhancer mechanism. © 2021 Elsevier Inc.

Abstract

Background: Angiogenesis is a necessary step in the growth of any tumour and metastasis. Vascular endothelial growth factor (VEGF) is a major mediator of breast cancer angiogenesis. The expression of VEGF by breast tumours has been previously correlated with a poor prognosis in the pathogenesis of breast cancer. Method: In this cross-sectional descriptive study 54 women were enrolled over the period of 2 years. This study was aimed to determine the correlation between serum and tissue levels of VEGF-A and prognosis in carcinoma of breast cases. It also tried to correlate the aggressiveness of the disease with serum and tissue levels of VEGF-A in the subject population. Results: In benign cases, preoperative serum VEGF-A (sVEGF-A) levels remained within 3000 pg/ml while post-operative sVEGF-A levels never went beyond 2300pg/ml. In the majority of carcinoma breast cases, preoperative sVEGF-A levels remain within 4000 pg/ml while post-operative sVEGF-A levels never went beyond 3000pg/ml. All the benign lesions were negative for tissue VEGF-A (tVEGF-A). While all malignant lesions were positive for tVEGF-A in the immunohistochemistry (IHC) study. Patients with a malignant lesion of higher tumour grades were having high tVEGF-A readings (2+ or more). In this study higher nodal status was associated with higher tVEGF-A reading. Similarly, tVEGF-A was higher in ER/PR/Her2neu negative cases. Conclusions: Determination of VEGF-A can be considered a promising, but investigative and prognostic tool. Large prospective studies are necessary to better define the relationship of prognosis to angiogenesis and VEGF-A.

The characterization of all these novel synthesized compounds 3a1-3d4 were established on the basis of spectral studies (IR, NMR and Mass) and elemental analysis. The anti-microbial activities of these compounds were tested in vitro by agar diffusion cup-plate method against two Gram-negative Escherichia coli, Pseudomonas aeruginosa and two Gram-positive Staphylococcus aureus, Bacillus substilis bacteria and fungi Candida albicans at 100 µg/ml concentrations. © 2014 Callender et al Heart failure places a significant burden on patients and health systems in high-income countries. However, information about its burden in low- and middle-income countries (LMICs) is scant. We thus set out to review both published and unpublished information on the presentation, causes, management, and outcomes of heart failure in LMICs. Methods and Findings: Medline, Embase, Global Health Database, and World Health Organization regional databases were searched for studies from LMICs published between 1 January 1995 and 30 March 2014. Additional unpublished data were requested from investigators and international heart failure experts. We identified 42 studies that provided relevant information on acute hospital care (25 LMICs; 232,550 patients) and 11 studies on the management of chronic heart failure in primary care or outpatient settings (14 LMICs; 5,358 patients). The mean age of patients studied ranged from 42 y in Cameroon and Ghana to 75 y in Argentina, and mean age in studies largely correlated with the human development index of the country in which they were conducted (r= 0.71, p,0.001). Overall, ischaemic heart disease was the main reported cause of heart failure in all regions except Africa and the Americas, where hypertension was predominant. Taking both those managed acutely in hospital and those in nonacute outpatient or community settings together, 57% (95% confidence interval [CI]: 49%–64%) of patients were treated with angiotensin-converting enzyme inhibitors, 34% (95% CI: 28%–41%) with beta-blockers, and 32% (95% CI: 25%–39%) with mineralocorticoid receptor antagonists. Mean inpatient stay was 10 d, ranging from 3 d in India to 23 d in China. Acute heart failure accounted for 2.2% (range: 0.3%–7.7%) of total hospital admissions, and mean in-hospitalmortalitywas 8% (95% CI: 6%–10%). 44. Energy Harvesting Through Reverse Electrowetting

Abstract

Over the last decade electrical batteries have emerged as a critical bottleneck for portable electronics development. High-power mechanical energy harvesting can potentially provide a valuable alternative to the use of batteries, but, until now, a suitable mechanical-to-electrical energy conversion technology did not exist. Here we describe a novel mechanical-to-electrical energy conversion method based on the reverse electrowetting phenomenon. Reverse electrowetting has emerged as a highly potential method for high power energy harvesting with good environmental coupling. Device simulation using fluid flow model of COMSOL multiphysics has been presented in this thesis. Harvested energy from this kind of device is directly proportional to change in capacitance from wetting to non-wetting condition i.e. the device should have high capacitance per unit volume. It has been found that high K dielectric material offers high capacitance at low operating voltage. Further reduction in operating voltage can be accomplished by decreasing dielectric thickness. However it has been proposed to use 10nm dielectric thickness for ease in fabrication. Finally capacitance of 4.213nF at an operating voltage of 4.9V has been achieved by choosing 10nm of TiO2 ( ) as a dielectric material with the device of radius 100um. Instead of using single big droplet, use of multiple small droplets of same volume offer more capacitance and hence the harvested energy. Apart from this simulation, device fabrication has been done using two different methods, one is based on UV lithography and another is based on X-ray lithography. All the obstacles and problems encountered in this fabrication have been discussed thoroughly in this thesis.

Abstract

Dy doped Lanthanum Aluminate phosphors are prepared for different concentrations of dopant and the luminescent properties of the phosphor are studied. High temperature modified solid-state reaction method is used for the synthesis. XRD analysis confirms the formation of LaAlO:Dy phosphor. Average crystallite size calculated is 40.085 nm.Thermoluminescence study of UV induced LaAlO:Dy phosphor is discussed in this paper. Thermoluminescence glow peak is recorded for different UV exposure time and for different concentrations of dopant. Kinetic parameters such as shape factor, activation energy, order of kinetics are calculated by using computerized glow curve deconvolution technique. Activation energy varies from 0.63 to 0.72 eV. The TL glow curve intensity varies nearly linear with increasing UV dose. Photoluminescence (PL) emission spectra are recorded for different concentrations of dopant. Highest intensity of PL emission was obtained for 1.5 mol% of Dy doped LaAlO phosphor. The excitation spectra of Dy doped LaAlO phosphor monitored at 550 nm shows broad band around 260 nm and a sharp peak at 394 nm. Peaks with lower intensity are obtained at 320 nm, 363 nm and 384 nm. In the Pl emission spectra, peak at 554 nm (yellow greenish) has highest intensity. Peaks with less intensity are obtained at 466 nm (blue), 645 nm (orange red) and 740 nm (brownish red). Pl intensity varies linearly with increasing concentration of dopant. CIE chromaticity coordinates are calculated for LaAlO:Dy (1.5 mol%), x = 0.3586 and y = 0.5421 which corresponds to yellow-greenish region. Contains fulltext : 110004.pdf (publisher's version ) (Open Access), Introduction: Tobacco use is a leading cause of deaths and disabilities in India, killing about 1.2 lakh people in 2010. About 29% of adults use tobacco on a daily basis and an additional 5% use it occasionally. In Odisha, non-smoking forms are more prevalent than smoking forms. The habit has very high opportunity cost as it reduces the capacity to seek better nutrition, medical care and education. In line with the WHO Framework Convention on Tobacco Control (FCTC), the Cigarettes and Other Tobacco Products Act (COTPA) is a powerful Indian national law on tobacco control. The Government of Odisha has shown its commitment towards enforcement and compliance of COTPA provisions. In order to gauge the perceptions and practices related to tobacco control efforts and level of enforcement of COTPA in the State, this cross-sectional study was carried out in seven selected districts. Materials and methods:A semi-structured interview schedule was developed, translated into Odiya and field-tested for data collection. It mainly contained questions related to knowledge on provisions of section 4-7 of COTPA 2003, perception about smoking, chewing tobacco and practices with respect to compliance of selected provisions of the Act. 1414 samples were interviewed. Results: The highest percentage of respondents was from the government departments. 70% of the illiterates consumed tobacco as compared to 34% post graduates. 52.1% of the respondents were aware of Indian tobacco control laws, while 80.8% had knowledge about the provision of the law prohibiting smoking in public places. However, 36.6% of the respondents reported that they had 'very often' ' seen tobacco products being sold 'to a minor', while 31.2% had seen tobacco products being sold 'by a minor'. In addition, 24.8% had 'very often' seen tobacco products being sold within a radius of 100 yards of educational institutions.
