2. Highly-multiplexed microwave SQUID readout using the SLAC Microresonator Radio Frequency (SMuRF) Electronics for Future CMB and Sub-millimeter Surveys

Abstract

The next generation of cryogenic CMB and submillimeter cameras under development require densely instrumented sensor arrays to meet their science goals. The readout of large numbers ($\sim$10,000--100,000 per camera) of sub-Kelvin sensors, for instance as proposed for the CMB-S4 experiment, will require substantial improvements in cold and warm readout techniques. To reduce the readout cost per sensor and integration complexity, efforts are presently focused on achieving higher multiplexing density while maintaining readout noise subdominant to intrinsic detector noise. Highly-multiplexed cold readout technologies in active development include Microwave Kinetic Inductance Sensors (MKIDs) and microwave rf-SQUIDs. Both exploit the high quality factors of superconducting microwave resonators to densely channelize sub-Kelvin sensors into the bandwidth of a microwave transmission line. We present advancements in the development of a new warm readout system for microwave SQUID multiplexing, the SLAC Superconducting Microresonator RF electronics, or SMuRF. The SMuRF system is unique in its ability to track each tone, minimizing the total RF power required to read out each resonator, thereby significantly reducing the linearity requirements on the cold and warm readout. Here, we present measurements of the readout noise and linearity of the first full SMuRF system, including a demonstration of closed-loop tone tracking on a 528 channel cryogenic microwave SQUID multiplexer. SMuRF is being explored as a potential readout solution for several future CMB projects including Simons Observatory, BICEP Array, CCAT-prime, Ali-CPT, and CMB-S4. Abstract

The Simons Observatory (SO) is a new cosmic microwave background experiment being built on Cerro Toco in Chile, due to begin observations in the early 2020s. We describe the scientific goals of the experiment, motivate the design, and forecast its performance. SO will measure the temperature and polarization anisotropy of the cosmic microwave background in six frequency bands: 27, 39, 93, 145, 225 and 280 GHz. The initial configuration of SO will have three small-aperture 0.5-m telescopes (SATs) and one large-aperture 6-m telescope (LAT), with a total of 60,000 cryogenic bolometers. Our key science goals are to characterize the primordial perturbations, measure the number of relativistic species and the mass of neutrinos, test for deviations from a cosmological constant, improve our understanding of galaxy evolution, and constrain the duration of reionization. The SATs will target the largest angular scales observable from Chile, mapping ~10% of the sky to a white noise level of 2 $\mu$K-arcmin in combined 93 and 145 GHz bands, to measure the primordial tensor-to-scalar ratio, $r$, at a target level of $\sigma(r)=0.003$. The LAT will map ~40% of the sky at arcminute angular resolution to an expected white noise level of 6 $\mu$K-arcmin in combined 93 and 145 GHz bands, overlapping with the majority of the LSST sky region and partially with DESI. With up to an order of magnitude lower polarization noise than maps from the Planck satellite, the high-resolution sky maps will constrain cosmological parameters derived from the damping tail, gravitational lensing of the microwave background, the primordial bispectrum, and the thermal and kinematic Sunyaev-Zel'dovich effects, and will aid in delensing the large-angle polarization signal to measure the tensor-to-scalar ratio. Abstract

Rationale & objectiveCardiovascular disease (CVD) is common and overall graft survival is suboptimal among kidney transplant recipients. Although albuminuria is a known risk factor for adverse outcomes among persons with native chronic kidney disease, the relationship of albuminuria with cardiovascular and kidney outcomes in transplant recipients is uncertain.Study designPost hoc longitudinal cohort analysis of the Folic Acid for Vascular Outcomes Reduction in Transplantation (FAVORIT) Trial.Setting & participantsStable kidney transplant recipients with elevated homocysteine levels from 30 sites in the United States, Canada, and Brazil.PredictorUrine albumin-creatinine ratio (ACR) at randomization.OutcomesAllograft failure, CVD, and all-cause death.Analytical approachMultivariable Cox models adjusted for age; sex; race; randomized treatment allocation; country; systolic and diastolic blood pressure; history of CVD, diabetes, and hypertension; smoking; cholesterol; body mass index; estimated glomerular filtration rate (eGFR); donor type; transplant vintage; medications; and immunosuppression.ResultsAmong 3,511 participants with complete data, median ACR was 24 (Q1-Q3, 9-98) mg/g, mean eGFR was 49±18 (standard deviation) mL/min/1.73m2, mean age was 52±9 years, and median graft vintage was 4.1 (Q1-Q3, 1.7-7.4) years. There were 1,017 (29%) with ACR < 10mg/g, 912 (26%) with ACR of 10 to 29mg/g, 1,134 (32%) with ACR of 30 to 299mg/g, and 448 (13%) with ACR ≥ 300mg/g. During approximately 4 years, 282 allograft failure events, 497 CVD events, and 407 deaths occurred. Event rates were higher at both lower eGFRs and higher ACR. ACR of 30 to 299 and ≥300mg/g relative to ACR < 10mg/g were independently associated with graft failure (HRs of 3.40 [95% CI, 2.19-5.30] and 9.96 [95% CI, 6.35-15.62], respectively), CVD events (HRs of 1.25 [95% CI, 0.96-1.61] and 1.55 [95% CI, 1.13-2.11], respectively), and all-cause death (HRs of 1.65 [95% CI, 1.23-2.21] and 2.07 [95% CI, 1.46-2.94], respectively).LimitationsNo data for rejection; single ACR assessment.ConclusionsIn a large population of stable kidney transplant recipients, elevated baseline ACR is independently associated with allograft failure, CVD, and death. Future studies are needed to evaluate whether reducing albuminuria improves these outcomes.

11. Assessment of the effectiveness of phentolamine in reducing the duration of dental local anaesthesia

Abstract

Effective local anaesthesia is required to ensure pain-free dental procedures, but existing anaesthetics produce persistent soft tissue anaesthesia well beyond the average time for the painful treatment. This may result in lip and tongue biting in young children and the handicapped. The study reports the clinical attempts to usefully reduce this level of unwanted soft tissue anaesthesia by injecting into the anaesthetised tissues a potent vasodilating agent, phentolamine. Lignocaine and adrenaline is the standard dental local anaesthetic combination, with the adrenaline included to prolong the duration and depth of anaesthesia in the highly vascular perioral tissues. The added catecholamine prevents the anaesthetic from being rapidly absorbed from the injection site by producing intense vasoconstriction. An alternative dental local anaesthetic is the combination of prilocaine and felypressin. This also produces excessively long periods of soft tissue anaesthesia despite the vasoconstriction not being mediated through ?-adrenoceptors. Investigation of the clinical effects of reversing residual soft tissue anaesthesia produced by prilocaine and felypressin were thought worthwhile to enable the clinical safety of a potentially inappropriate drug combination to be assessed. It also produced comparative data for interpreting the catecholamine reponses to the routine lignocaine and adrenaline anaesthetics. Phentolamine is a short-acting and potent a-adrenoceptor blocking agent. The investigations confirm that a 2mg/ml phentolamine reversal solution, when injected after a 30 minute delay into the perioral tissues in the vicinity of the local anaesthetic solution, quickly reverses the vasoconstrictor effects of adrenaline in vivo. Maxillary infiltration soft tissue anaesthesia was reduced from 138.1 to 16.9 minutes (95% CI) and pulp anaesthesia from 40.3 to 4.8 minutes (95% CI). Inferior dental nerve block reversals were reduced from 160.3 to 17.3 minutes (95% CI) and pulp anaesthesia from 59.5 to 11.1 minutes (95% CI). Multiple injections of 6ml 2% lignocaine + 1:80,000 adrenaline were reversed with 3ml 2mg/ml phentolamine, and venous plasma local anaesthetic concentrations assayed using High Performance Liquid Chromatography (HPLC). No statistically significant increase in plasma lignocaine concentration (95% CI) was observed. Venous plasma lignocaine and catecholamine concentrations were measured after multiple perioral injections of 5.4ml 2% lignocaine with three differing adrenaline concentrations; 1:50,000, 1:80,000 and 1:100,000. No statistically significant difference in plasma lignocaine concentrations was noted between the groups. Plasma adrenaline concentrations were statistically raised in the 1:50,000 group (95% CI), but the noradrenaline concentrations showed no significant variation. The perioral injection of 3 ml 2mg/ml phentolamine reversal agent after the 30 minute delay produced increases in plasma lignocaine concentrations, with the weakest 1:100,000 adrenaline anaesthetic the most statistically significant (95% CI). The intense local vasodilatation was demonstrated to be effective against the nonadrenaline alternative anaesthetic prilocaine and felypressin, with a statistically significant rise in the venous plasma prilocaine concentration (95% CI). Plasma local anaesthetic concentrations failed to reach accepted levels of toxicity, and no adverse cardiovascular events were recorded. Venous plasma adrenaline concentrations were measured before and after dental surgical extractions under lignocaine and adrenaline or prilocaine and felypressin local anaesthesia. Venous plasma adrenaline concentrations were significantly raised in only half the cases, but the prilocaine and felypressin group showed a marked reduction in data scatter. Additional experiments attempting to reverse the soft tissue anaesthesia following injection of another dental local anaesthetic combination, mepivacaine and adrenaline, are presented as appendices. Plasma mepivacaine concentrations were not statistically raised compared with placebo. Venous plasma catecholamine concentrations were measured following perioral injection of 6ml 2% mepivacaine + 1:100,000 adrenaline, and after a 30 minute delay, with 3ml 2mg/ml phentolamine or placebo. No statistical difference was noted between the groups for adrenaline or noradrenaline concentrations, but a trend for the noradrenaline concentration to rise over the 2 hour test period was observed. No adverse local reactions or systemic cardiovascular changes were observed in any of the studies, implying that the prolonged soft tissue effects of dental local anaesthesia may be safely and rapidly reversed by injection of small increments of phentolamine into the adjacent perioral tissues. Such a facility will be useful in clinical dental practice where needlessly prolonged anaesthesia may have significant consequences to patients, such as lip biting in the handicapped.

16. Concentrations of organic contaminants in industrial and municipal bioresources recycled in\ud agriculture in the UK

Abstract

Many types of bioresource materials are recycled in agriculture for soil improvement and as bedding materials for livestock and have potential for transfer into plant and animal foods. Representative types of industrial and municipal bioresources were selected to assess the extent of\ud organic chemical contamination, including: (i) land applied materials: treated sewage sludge biosolids), meat and bone meal ash (MBMA), poultry litter ash (PLA), paper sludge ash (PSA) and compost-like-output (CLO), and (ii) bedding materials: recycled waste wood (RWW), dried paper\ud sludge (DPS), paper sludge ash (PSA) and shredded cardboard.\ud The materials generally contained lower concentrations of polychlorinated dibenzo-pdioxins/dibenzofurans (PCDD/Fs) and dioxin-like polychlorinated biphenyls (PCBs) relative to earlier reports, indicating the decline in environmental emissions of these established contaminants. However, concentrations of polycyclic aromatic hydrocarbons (PAHs) remain elevated in biosolids samples from urban catchments. Polybrominated dibenzo-p-dioxins/dibenzofurans (PBDD/Fs) were present in larger amounts in biosolids and CLO compared to their chlorinated counterparts and hence are of potentially greater significance in contemporary materials. The presence of non-ortho-polychlorinated biphenyls (PCBs) in DPS was probably due to non-legacy sources of PCBs in paper production. Emerging flame retardant compounds, including: decabromodiphenylethane (DBDPE)and organophosphate flame retardants (OPFRs), were detected in several of the materials. The profile of perfluoroalkyl substances (PFAS) depended on the type of waste category;\ud perfluoroundecanoic acid (PFUnDA) was the most significant PFAS for DPS, whereas perfluorooctane sulfonate (PFOS) was dominant in biosolids and CLO. The concentrations of\ud polychlorinated alkanes (PCAs) and di-2-ethylhexyl phthalate (DEHP) were generally much larger than the other contaminants measured, indicating that there are major anthropogenic sources of these potentially hazardous chemicals entering the environment. The study results suggest that continued vigilance is required to control emissions and sources of these contaminants to support the\ud beneficial use of secondary bioresource materials.

Abstract

Caregivers for patients undergoing solid organ transplantation play an essential role in the process of transplantation. However, little is known about stress and coping among these caregivers. Six hundred and twenty-one primary caregivers of potential candidates for lung (n = 317), liver (n = 147), heart (n = 115), and/or kidney (n = 42) transplantation completed a psychometric test battery at the time of the candidate’s initial pre-transplant psychosocial evaluation. Caregivers were generally well adjusted, with only 17% exhibiting clinical symptoms of depression (Beck Depression Inventory-II score > 13) and 13% reporting clinical levels of anxiety (State Trait Anxiety Inventory score >48). Greater caregiver burden and negative coping styles were associated with higher levels of depression. Greater objective burden and avoidant coping were associated with higher levels of anxiety. Caregivers evidenced a high degree of socially desirable (i.e., defensive) responding, which may reflect a deliberate effort to minimize fears or worries so as to not jeopardize patients’ listing status.

Abstract

BACKGROUND: Small-scale experimental investigations were carried out on the anaerobic digestion of potato processing wastewater and its co-digestion with pig slurry and/or abattoir wastewater. A simple and rapid procedure was used to determine the suitability of these wastes for digestion.RESULTS: During the initial 5-day acclimation phase, the seed (digested brewery waste) was replaced by the test waste before allowing the tests to incubate without further addition, where methanogenesis was measured. Although potato processing wastewater has low pH, with high fat content treatment via anaerobic digestion was still feasible in spite of low methane production. Co-digestion with pig slurry and abattoir wastewater was therefore investigated to enhance the process. Pig slurry improved the process, which, when co-digested with potato processing wastewater in equal ratio achieved 72% volatile solids removal, 35 mL average daily biogas production and 32% maximum methane content in 22 days (following the acclimation period). Co-digestion with abattoir wastewater did not improve the digestion process due to poor buffering and low pH value.CONCLUSION: Anaerobic co-digestion may be a feasible treatment option for industrial bio-wastes and livestock wastes produced in Cyprus and indeed in similar other countries of comparable market size and activities. Treatment with intravenous immunoglobulin (IVIG) combined with plasmapheresis (PP) has been used recently in many centers. We report the incidence, clinical characteristics, and outcome of patients with AHR treated with IVIG and PP. All patients (n = 519) at our institution who underwent kidney transplantation between January 1999 and August 2003 were retrospectively analyzed and classified according to biopsy results into three groups: AHR, ACR, and no rejection. AHR was diagnosed in 23 patients (4.5%) and ACR in 75 patients (15%). Mean follow-up was 844 ± 23 days. Female sex, black race, and high panel-reactive antibody were risk factors for AHR. Most AHR patients (22 of 23) were treated with IVIG and PP. Two-year graft survival was numerically worse in patients with AHR versus ACR (78% vs. 85%, p = 0.5) but the difference was not statistically significant. Graft survival after AHR treated with IVIG and PP is much better than it has been historically. IVIG in combination with PP is an effective treatment for AHR. Graft survival in this setting is similar to graft survival in patients with ACR.
