Introduction: donor-derived modified immune cells (MIC) induced long-term specific immunosuppression against the allogeneic donor in preclinical models of transplantation. In a phase I clinical trial (TOL-1 Study), MIC treatment resulted in a cellular phenotype that was directly and indirectly suppressive to the recipient's immune system allowing for reduction of conventional immunosuppressive therapy. Here, we describe a protocol for a randomised controlled, multicentre phase-IIb clinical trial of individualised immunosuppression with intravenously administered donor MIC compared with standard-of-care (SoC) in living donor kidney transplantation (TOL-2 Study). Methods and analysis: sixty-three living donor kidney transplant recipients from six German transplant centres are randomised 2:1 to treatment with MIC (MIC group, N=42) or no treatment with MIC (control arm, N=21). MIC are manufactured from donor peripheral blood mononuclear cells under Good Manufacturing Practice conditions. The primary objective of this trial is to determine the efficacy of MIC treatment together with reduced conventional immunosuppressive therapy in terms of achieving an operational tolerance-like phenotype compared with SoC 12 months after MIC administration. Key secondary endpoints are the number of patient-relevant infections as well as a composite of biopsy-proven acute rejection, graft loss, graft dysfunction or death. Immunosuppressive therapy of MIC-treated patients is reduced during follow-up under an extended immunological monitoring including human leucocyte antigen-antibody testing, and determination of lymphocyte subsets, for example, regulatory B lymphocytes (Breg) and antidonor T cell response. A Data Safety Monitoring Board has been established to allow an independent assessment of safety and efficacy. In dieser Arbeit werden anhand von 30 Todspendern und 15 Lebendspendern die Unterschiede zwischen lebend- und todgespendeten Nieren durch die Betrachtung unterschiedlicher Zytokinausprägungen (IL6, IL8, TNF, IL2R) beschrieben. Hierzu werden die genannten Zytokine sowohl bei den Spendern als auch bei den gespendeten Nieren betrachtet. Weiter wird die Wirkung einer Nachperfusion auf die explantierten Nieren untersucht. Es konnten signifikante Unterschiede zwischen den einzelnen Gruppen nachgewiesen werden. Durch die Methode der Nachperfusion konnte die Zytokinbelastung der gespendeten Nieren gesenkt werden., Forty-five consecutive organ donors were studied in two groups. 15 living & 30 cadaveric renal transplants were investigated. Cytokin levels (IL6, IL8, TNF, IL2-R) were measured. A highly significant difference between the systemic cytokin levels of cadaveric and living donors could be dmonstrated. Also a highly significant difference could be seen by looking at a back-table perfusion of the transplants.

To explore whether blockade of AT1 and AT2 receptors had an effect on apoptosis in the graft and the development and progression of chronic allograft nephropathy (CAN), we orthotopically transplanted kidneys of Fisher (F344) rats into Lewis rats. Animals were treated with the AT1 and AT2 receptor antagonist and appropriate vehicles either from day –7 before transplantation to week 24 after transplantation, from week 12 to week 24 after transplantation, or from day –7 to day +5. 24-hour protein excretion and number of apoptotic cells were significantly lower in AT1 antagonist treated grafts as compared to AT2 antagonist ones and controls. Apoptosis affected primarily tubular cells. mRNA levels of bax, caspase-1, and caspase 3 were lower and bcl-2 increased in AT1 antagonist treated animals as compared to AT2 antagonist treated and vehicle treated animals. However, no differences were observed between experimental groups if treatment was started at later time points. AT1 receptor mediated signals reduce the number of apoptotic cells during CAN with beneficial effects on protein excretion as well as the grade of CAN. Chronic allograft nephropathy (CAN) is one essential cause of chronic renal graft failure. But at present there is no effective treatment to prevent it. Everolimus is a new immunosuppressive agent with anti-proliferative and apoptosis-enhancing effects. We detected whether everolimus could reduce the progression of CAN and how it affected the apoptosis in CAN. Our results indicated that everolimus could ameliorate the progression of advanced CAN, and the intra-graft number of apoptotic cells were signigicantly higher in everolimus treated animals. Therefore, everolimus might ameliorate CAN due to it apoptosis-enhancing effects., Die Chronische Allotransplantatnephropathie (CAN) ist eine wesentliche Ursache des chronischen Transplantatversagens. Derzeit steht allerdings keine medikamentöse Therapie zur Verfügung. Everolimus ist ein neueres Immunsuppressivum, das proliferationshemmende und apoptoseinduzierende Eigenschaften hat. Wir untersuchten, ob Everolimus die Progression einer experimentellen CAN vermindert und wie dabei die Apoptose beeinflusst wird. In unseren Untersuchungen verminderte Everolimus die Progression einer fortgeschrittenen CAN, wobei die mit Everolimus behandelten Tiere eine höhere Anzahl apoptotischer Zellen im Transplantat hatten.
