Abstract: Stellar-mass and supermassive black holes abound in the Universe, whereas intermediate-mass black holes (IMBHs) of ~10^2-10^5 solar masses in between are largely missing observationally, with few cases found only. Here we report the real-time discovery of a long-duration X-ray transient, EP240222a, accompanied by an optical flare with prominent H and He emission lines revealed by prompt follow-up observations. Its observed properties evidence an IMBH located unambiguously in the halo of a nearby galaxy and flaring by tidally disrupting a star -- the only confirmed off-nucleus IMBH-tidal disruption event so far. This work demonstrates the potential of sensitive time-domain X-ray surveys, complemented by timely multi-wavelength follow-ups, in probing IMBHs, their environments, demographics, origins and connections to stellar-mass and supermassive black holes., Comment: 64 pages, 15 figures, submitted

Abstract: The Water Cherenkov Detector Array (WCDA) is one of the components of Large High Altitude Air Shower Observatory (LHAASO) and can monitor any sources over two-thirds of the sky for up to 7 hours per day with >98\% duty cycle. In this work, we report the detection of two outbursts of the Fanaroff-Riley I radio galaxy NGC 1275 that were detected by LHAASO-WCDA between November 2022 and January 2023 with statistical significance of 5.2~$\sigma$ and 8.3~$\sigma$. The observed spectral energy distribution in the range from 500 GeV to 3 TeV is fitted by a power-law with a best-fit spectral index of $\alpha=-3.37\pm0.52$ and $-3.35\pm0.29$, respectively. The outburst flux above 0.5~TeV was ($4.55\pm 4.21)\times~10^{-11}~\rm cm^{-2}~s^{-1}$ and ($3.45\pm 1.78)\times~10^{-11}~\rm cm^{-2}~s^{-1}$, corresponding to 60\%, 45\% of Crab Nebula flux. Variation analysis reveals the variability time-scale of days at the TeV energy band. A simple test by one-zone synchrotron self-Compton model reproduces the data in the gamma-ray band well., Comment: 11 pages, 8 figures, 3 tables

Abstract: We report the detection of an extended very-high-energy (VHE) gamma-ray source coincident with the location of middle-aged (62.4~\rm kyr) pulsar PSR J0248+6021, by using the LHAASO-WCDA data of live 796 days and LHAASO-KM2A data of live 1216 days. A significant excess of \gray induced showers is observed both by WCDA in energy bands of 1-25~\rm TeV and KM2A in energy bands of $>$ 25~\rm TeV with 7.3 $\sigma$ and 13.5 $\sigma$, respectively. The best-fit position derived through WCDA data is R.A. = 42.06$^\circ \pm$ 0.12$^\circ$ and Dec. = 60.24$^\circ \pm $ 0.13$^\circ$ with an extension of 0.69$^\circ\pm$0.15$^\circ$ and that of the KM2A data is R.A.= 42.29$^\circ \pm $ 0.13$^\circ$ and Dec. = 60.38$^\circ \pm$ 0.07$^\circ$ with an extension of 0.37$^\circ\pm$0.07$^\circ$. No clear extended multiwavelength counterpart of this LHAASO source has been found from the radio band to the GeV band. The most plausible explanation of the VHE \gray emission is the inverse Compton process of highly relativistic electrons and positrons injected by the pulsar. Abstract: Massive stars end their life as core-collapse supernovae, amongst which some extremes are Type Ic broad-lined supernovae associated with long-duration gamma-ray bursts (LGRBs) having powerful relativistic jets. Their less-extreme brethren make unsuccessful jets that are choked inside the stars, appearing as X-ray flashes or low-luminosity GRBs. On the other hand, there exists a population of extragalactic fast X-ray transients (EFXTs) with timescales ranging from seconds to thousands of seconds, whose origins remain obscure. Known sources that contribute to the observed EFXT population include the softer analogs of LGRBs, shock breakouts of supernovae, or unsuccessful jets. Here, we report the discovery of the bright X-ray transient EP240414a detected by the Einstein Probe (EP), which is associated with the Type Ic supernova SN 2024gsa at a redshift of 0.401. The X-ray emission evolution is characterised by a very soft energy spectrum peaking at < 1.3 keV, which makes it distinct from known LGRBs, X-ray flashes, or low-luminosity GRBs. Follow-up observations at optical and radio bands revealed the existence of a weak relativistic jet that interacts with an extended shell surrounding the progenitor star. Located on the outskirts of a massive galaxy, this event reveals a new population of explosions of Wolf-Rayet stars characterised by a less powerful engine that drives a successful but weak jet, possibly owing to a progenitor star with a smaller core angular momentum than in traditional LGRB progenitors., Comment: 43 pages, 9 figures, 4 tables, submitted. Abstract: In this work we try to search for signals generated by ultra-heavy dark matter at the Large High Altitude Air Shower Observatory (LHAASO) data. We look for possible gamma-ray by dark matter annihilation or decay from 16 dwarf spheroidal galaxies in the field of view of LHAASO. Dwarf spheroidal galaxies are among the most promising targets for indirect detection of dark matter which have low fluxes of astrophysical $\gamma$-ray background while large amount of dark matter. By analyzing more than 700 days observational data at LHAASO, no significant dark matter signal from 1 TeV to 1 EeV is detected. Accordingly we derive the most stringent constraints on the ultra-heavy dark matter annihilation cross-section up to EeV. Abstract: The KM2A is the largest sub-array of the Large High Altitude Air Shower Observatory (LHAASO). It consists of 5216 electromagnetic particle detectors (EDs) and 1188 muon detectors (MDs). The data recorded by the EDs and MDs are used to reconstruct primary information of cosmic ray and gamma-ray showers. This information is used for physical analysis in gamma-ray astronomy and cosmic ray physics. To ensure the reliability of the LHAASO-KM2A data, a three-level quality control system has been established. It is used to monitor the status of detector units, stability of reconstructed parameters and the performance of the array based on observations of the Crab Nebula and Moon shadow. This paper will introduce the control system and its application on the LHAASO-KM2A data collected from August 2021 to July 2023. During this period, the pointing and angular resolution of the array were stable. From the observations of the Moon shadow and Crab Nebula, the results achieved using the two methods are consistent with each other. According to the observation of the Crab Nebula at energies from 25 TeV to 100 TeV, the time averaged pointing errors are estimated to be $-0.003^{\circ} \pm 0.005^{\circ}$ and $0.001^{\circ} \pm 0.006^{\circ}$ in the R.A. and Dec directions, respectively., Comment: 15 pages, 9 figures

Abstract: The first source catalog of Large High Altitude Air Shower Observatory reported the detection of a very-high-energy gamma ray source, 1LHAASO J1219+2915. In this paper a further detailed study of the spectral and temporal behavior of this point-like source have been carried. The best-fit position of the TeV source ($\rm{RA}=185.05^{\circ}\pm0.04^{\circ}$, $\rm{Dec}=29.25^{\circ}\pm0.03^{\circ}$) is compatible with NGC 4278 within $\sim0.03$ degree. Variation analysis shows an indication of the variability at a few months level in the TeV band, which is consistent with low frequency observations. Based on these observations, we report the detection of TeV $\gamma$-ray emissions from this low-luminosity AGN NGC 4278. The observations by LHAASO-WCDA during active period has a significance level of 8.8\,$\sigma$ with best-fit photon spectral index $\varGamma=2.56\pm0.14$ and a flux $f_{1-10\,\rm{TeV}}=(7.0\pm1.1_{\rm{sta}}\pm0.35_{\rm{syst}})\times10^{-13}\,\rm{photons\,cm^{-2}\,s^{-1}}$, or approximately $5\%$ of the Crab Nebula. The discovery of VHE from NGC 4278 indicates that the compact, weak radio jet can efficiently accelerate particles and emit TeV photons., Comment: 11 pages, 5 figures

Abstract: Long gamma-ray bursts (GRBs) are believed to originate from core collapse of massive stars. High-redshift GRBs can probe the star formation and reionization history of the early universe, but their detection remains rare. Here we report the detection of a GRB triggered in the 0.5--4 keV band by the Wide-field X-ray Telescope (WXT) on board the Einstein Probe (EP) mission, designated as EP240315a, whose bright peak was also detected by the Swift Burst Alert Telescope and Konus-Wind through off-line analyses. At a redshift of $z=4.859$, EP240315a showed a much longer and more complicated light curve in the soft X-ray band than in gamma-rays. Benefiting from a large field-of-view ($\sim$3600 deg$^2$) and a high sensitivity, EP-WXT captured the earlier engine activation and extended late engine activity through a continuous detection. With a peak X-ray flux at the faint end of previously known high-$z$ GRBs, the detection of EP240315a demonstrates the great potential for EP to study the early universe via GRBs., Comment: 41 pages, 8 figures, 7 tables

Abstract: KM2A is one of the main sub-arrays of LHAASO, working on gamma ray astronomy and cosmic ray physics at energies above 10 TeV. Detector simulation is the important foundation for estimating detector performance and data analysis. It is a big challenge to simulate the KM2A detector in the framework of Geant4 due to the need to track numerous photons from a large number of detector units (>6000) with large altitude difference (30 m) and huge coverage (1.3 km^2). In this paper, the design of the KM2A simulation code G4KM2A based on Geant4 is introduced. The process of G4KM2A is optimized mainly in memory consumption to avoid memory overffow. Some simpliffcations are used to signiffcantly speed up the execution of G4KM2A. The running time is reduced by at least 30 times compared to full detector simulation. The particle distributions and the core/angle resolution comparison between simulation and experimental data of the full KM2A array are also presented, which show good agreement.

10. Development of low-radon ultra-pure water for the Jiangmen Underground Neutrino Observatory

Abstract: The Jiangmen Underground Neutrino Observatory(JUNO) is a state-of-the-art liquid scintillator-based neutrino physics experiment under construction in South China. To reduce the background from external radioactivities, a water Cherenkov detector composed of 35~kton ultra-pure water and 2,400 20-inch photomultiplier tubes is developed. Even after specialized treatment, ultra-pure water still contains trace levels of radioactive elements that can contribute to the detector background. Among which $^{222}$Rn is particularly significant. To address this, an online radon removal system based on the JUNO prototype has been developed. By integrating micro-bubble generators to enhance degasser's radon removal efficiency, the radon concentration in water can be reduced to 1~mBq/m$^{3}$ level, meeting the stringent requirements of JUNO. Additionally, a highly sensitive online radon concentration measurement system capable of detecting concentrations $\sim$1~mBq/m$^3$ has been developed to monitor the radon concentration in water. Abstract: We present the measurements of all-particle energy spectrum and mean logarithmic mass of cosmic rays in the energy range of 0.3-30 PeV using data collected from LHAASO-KM2A between September 2021 and December 2022, which is based on a nearly composition-independent energy reconstruction method, achieving unprecedented accuracy. Our analysis reveals the position of the knee at $3.67 \pm 0.05 \pm 0.15$ PeV. Below the knee, the spectral index is found to be -$2.7413 \pm 0.0004 \pm 0.0050$, while above the knee, it is -$3.128 \pm 0.005 \pm 0.027$, with the sharpness of the transition measured with a statistical error of 2%. The mean logarithmic mass of cosmic rays is almost heavier than helium in the whole measured energy range. It decreases from 1.7 at 0.3 PeV to 1.3 at 3 PeV, representing a 24% decline following a power law with an index of -$0.1200 \pm 0.0003 \pm 0.0341$. This is equivalent to an increase in abundance of light components. Above the knee, the mean logarithmic mass exhibits a power law trend towards heavier components, which is reversal to the behavior observed in the all-particle energy spectrum. Additionally, the knee position and the change in power-law index are approximately the same. These findings suggest that the knee observed in the all-particle spectrum corresponds to the knee of the light component, rather than the medium-heavy components., Comment: 8 pages, 3 figures

Abstract: The Jiangmen Underground Neutrino Observatory (JUNO), a 20~kton multi-purpose low background Liquid Scintillator (LS) detector, was proposed primarily to determine the neutrino mass ordering. To suppress the radioactivity from the surrounding rocks and tag cosmic muons, the JUNO central detector is submerged in a Water Cherenkov Detector (WCD). In addition to being used in the WCD, ultrapure water is used in LS filling, for which the $^{226}$Ra concentration in water needs to be less than 50~$\mu$Bq/m$^3$. To precisely measure the $^{226}$Ra concentration in water, a 6.0~$\mu$Bq/m$^3$ $^{226}$Ra concentration in water measurement system has been developed. This study aims to investigate how depressive and insomnia symptoms evolved among Chinese college students throughout the pandemic, including the beginning and end of the dynamic zero-COVID policy period.Patients and Methods: We conducted a 2-years longitudinal survey involving 1102 college students, collecting data at three key time points. Depressive symptoms were assessed using the Patient Health Questionnaire-9, and insomnia symptoms were measured with the Youth Self-rating Insomnia Scale-8. Three contemporaneous symptom networks and two cross-lagged panel networks were constructed.Results: In the current sample, the prevalence of clinically significant depressive symptoms was 6.1%, 8.9%, and 7.7% during the first, second, and third waves, respectively. The prevalence of clinically significant insomnia symptoms was 8.1%, 13.0%, and 14.1%. Over time, the severity of depressive and insomnia symptoms and network density increased, persisting at least one year after the pandemic and control measures ended. “Difficulty initiating sleep” bridged the two disorders, while “anhedonia” played a pivotal role in triggering and sustaining other symptoms.Conclusion: This study underscores the lasting impact of the evolving zero-COVID policy on depressive and insomnia symptoms among college students, elucidating the underlying interaction mechanisms. Dimensionality reduction cluster analysis processed the single-cell data, while weighted gene co-expression network analysis (WGCNA) and differential expression gene analysis were applied to bulk RNA-seq data. Prognostic models were developed using Cox proportional hazards (COX) and least absolute shrinkage and selection operator (LASSO) regression, with validation through survival analysis, decision curve analysis, and principal component analysis (PCA). Tumor mutation data were analyzed using the “maftools” package, and the immune microenvironment was assessed with TIMER2 data.Results: We developed a senescence-related (SENR) six-gene prognostic signature for PC, which stratifies patients by risk, with high-risk groups showing poorer prognoses. This model also offers predictive insights into tumor mutations and immune microenvironment characteristics. Caveolin-1 (CAV1) emerged as a significant prognostic biomarker, with functional validation showing its role in promoting cancer cell proliferation and migration, highlighting its potential as a therapeutic target.Conclusion: This study provides a novel senescence-related prognostic tool for PC, enhancing patient stratification for prognosis and immunotherapy, and identifies CAV1 as a key gene with clinical significance for targeted interventions.Keywords: pancreatic cancer, cellular senescence, prognosis, biomarker, immunotherapy

Abstract: For decades, supernova remnants (SNRs) have been considered the prime sources of Galactic Cosmic rays (CRs). But whether SNRs can accelerate CR protons to PeV energies and thus dominate CR flux up to the knee is currently under intensive theoretical and phenomenological debate. The direct test of the ability of SNRs to operate as CR PeVatrons can be provided by ultrahigh-energy (UHE; $E_\gamma \geq 100$~TeV) $\gamma$-rays. In this context, the historical SNR Cassiopeia A (Cas A) is considered one of the most promising target for UHE observations. This paper presents the observation of Cas A and its vicinity by the LHAASO KM2A detector. The exceptional sensitivity of LHAASO KM2A in the UHE band, combined with the young age of Cas A, enabled us to derive stringent model-independent limits on the energy budget of UHE protons and nuclei accelerated by Cas A at any epoch after the explosion. Abstract: The highest energy gamma-rays from gamma-ray bursts (GRBs) have important implications for their radiation mechanism. Here we report for the first time the detection of gamma-rays up to 13 TeV from the brightest GRB 221009A by the Large High Altitude Air-shower Observatory (LHAASO). The LHAASO-KM2A detector registered more than 140 gamma-rays with energies above 3 TeV during 230$-$900s after the trigger. The intrinsic energy spectrum of gamma-rays can be described by a power-law after correcting for extragalactic background light (EBL) absorption. Such a hard spectrum challenges the synchrotron self-Compton (SSC) scenario of relativistic electrons for the afterglow emission above several TeV. Observations of gamma-rays up to 13 TeV from a source with a measured redshift of z=0.151 hints more transparency in intergalactic space than previously expected. 17. Super-Eddington Accretion as a Possible Scenario to Form GW190425

Abstract: On 2019 April 25, the LIGO/Virgo Scientific Collaboration detected a compact binary coalescence, GW190425. Under the assumption of the binary neutron star (BNS), the total mass of $3.4^{+0.3}_{-0.1}\, M_\odot$ lies five standard deviations away from the known Galactic population mean. In the standard common envelope scenario, the immediate progenitor of GW190425 is a close binary system composed of an NS and a He-rich star. With the detailed binary evolutionary modeling, we find that in order to reproduce GW190425-like events, super-Eddington accretion (e.g., $1,000\,\dot{M}_{\rm Edd}$) from a He-rich star onto the first-born NS with a typical mass of 1.33 $M_\odot$ via stable Case BB mass transfer (MT) is necessarily required. Furthermore, the immediate progenitors should potentially have an initial mass of $M_{\rm ZamsHe}$ in a range of $3.0-3.5$ $M_\odot$ and an initial orbital period of $P_{\rm init}$ from 0.08 days to 0.12 days, respectively. The corresponding mass accreted onto NSs via stable Case BB MT phase varies from $0.70\, M_\odot$ to $0.77\, M_\odot$. 18. Revisiting the Properties of GW190814 and Its Formation History

Abstract: GW190814 was reported during LIGO's and Virgo's third observing run with the most asymmetric component masses (a $\sim 23$ $M_{\odot}$ black hole and a $\sim2.6$ $M_{\odot}$ compact object). Under the assumption that this event is a binary black hole (BBH) merger formed through the isolated binary evolution channel, we reanalyze the publicly released data of GW190814 with the modified astrophysical priors on the effective spin $\chi_{\rm eff}$, and further explore its formation history using detailed binary modeling. We show that GW190814 is likely to have been formed through the classical common envelope channel. Our findings show that the properties inferred using the modified astrophysical priors are consistent with those inferred by the uniform priors. With the newly-inferred properties of GW190814, we perform detailed binary evolution of the immediate progenitor of the BBH (namely a close binary system composed of a BH and a helium star) in a large parameter space, taking into account mass-loss, internal differential rotation, supernova kicks, and tidal interactions between the helium star and the BH companion. Our findings show that GW190814-like events could be formed in limited initial conditions just after the common envelope phase: a $\sim 23$ $M_{\odot}$ BH and a helium star of $M_{\rm ZamsHe}$ $\sim$ 8.5 $M_{\odot}$ at solar metallicity ($\sim$ 7.5 $M_{\odot}$ at 10\% solar metallicity) with an initial orbital period at around 1.0 day. Additionally, the inferred low spin of the secondary indicates that the required metallicity for reproducing GW190814-like events should not be too low (e.g., Z $\gtrsim$ 0.1 $Z_{\odot}$)., Comment: 8 pages, 8 figures, 1 table, final version published in MNRANS

Abstract: We present the first catalog of very-high energy and ultra-high energy gamma-ray sources detected by the Large High Altitude Air Shower Observatory (LHAASO). The catalog was compiled using 508 days of data collected by the Water Cherenkov Detector Array (WCDA) from March 2021 to September 2022 and 933 days of data recorded by the Kilometer Squared Array (KM2A) from January 2020 to September 2022. This catalog represents the main result from the most sensitive large coverage gamma-ray survey of the sky above 1 TeV, covering declination from $-$20$^{\circ}$ to 80$^{\circ}$. In total, the catalog contains 90 sources with an extended size smaller than $2^\circ$ and a significance of detection at $> 5\sigma$. Based on our source association criteria, 32 new TeV sources are proposed in this study. Among the 90 sources, 43 sources are detected with ultra-high energy ($E > 100$ TeV) emission at $> 4\sigma$ significance level. Abstract: The diffuse Galactic $\gamma$-ray emission, mainly produced via interactions between cosmic rays and the interstellar medium and/or radiation field, is a very important probe of the distribution, propagation, and interaction of cosmic rays in the Milky Way. In this work we report the measurements of diffuse $\gamma$-rays from the Galactic plane between 10 TeV and 1 PeV energies, with the square kilometer array of the Large High Altitude Air Shower Observatory (LHAASO). Diffuse emissions from the inner ($15^{\circ}10$~TeV). The energy spectrum in the inner Galaxy regions can be described by a power-law function with an index of $-2.99\pm0.04$, which is different from the curved spectrum as expected from hadronic interactions between locally measured cosmic rays and the line-of-sight integrated gas content. Furthermore, the measured flux is higher by a factor of $\sim3$ than the prediction. A similar spectrum with an index of $-2.99\pm0.07$ is found in the outer Galaxy region, and the absolute flux for $10\lesssim E\lesssim60$ TeV is again higher than the prediction for hadronic cosmic ray interactions. The latitude distributions of the diffuse emission are consistent with the gas distribution, while the longitude distributions show clear deviation from the gas distribution. Selenium nanoparticles (SeNPs) possess high antioxidant properties but are prone to inactivation. In this study, human serum albumin/SeNPs (HSA-SeNPs) were synthesized for enhanced stability.Methods: HSA-SeNPs were prepared by self-assembling denatured human serum albumin and inorganic selenite. The cytotoxicity of HSA-SeNPs was assessed using the MTT method. Cell survival and proliferation rates were tested to observe the protective effect of HSA-SeNPs on human skin keratinocytes against photoaging. Simultaneously, ICR mice were used for animal experiments. H&E and Masson trichromatic staining were employed to observe morphological changes in skin structure and collagen fiber disorders after UVB irradiation. Quantitative RT-PCR was utilized to measure changes in mRNA expression levels of factors related to collagen metabolism, inflammation, oxidative stress regulation, and senescence markers.Results: The HSA-SeNPs group exhibited significantly higher survival and proliferation rates of UVB-irradiated keratinocytes than the control group. Following UVB irradiation, the back skin of ICR mice displayed severe sunburn with disrupted collagen fibers. However, HSA-SeNPs demonstrated superior efficacy in alleviating these symptoms compared to SeNPs alone. In a UVB-irradiated mice model, mRNA expression of collagen type I and III was dysregulated while MMP1, inflammatory factors, and p21 mRNA expression were upregulated; concurrently Nrf2 and Gpx1 mRNA expression were downregulated. In contrast, HSA-SeNPs maintained the mRNA expression of those factors to be stable In addition, the level of SOD decreased, and MDA elevated significantly in the skin after UVB irradiation, but no significant differences in SOD and MDA levels between the HSA-SeNPs group with UVB irradiation and the UVB-free untreated group.Discussion: HSA-SeNPs have more anti-photoaging effects on the skin than SeNPs, including the protective effects on skin cell proliferation, cell survival, and structure under photoaging conditions. Various potential etiological factors, such as genetics, autoimmunity, infection, and exposure to infectious urine, have been suggested. However, the accurate etiology of LS in the male urethra remains unclear.Patients and Methods: In this study, we conducted single-cell RNA sequencing to identify the transcriptional profiles of three patients with LS USD and three patients with non-LS USD. Immunofluorescence was used to confirm the single-cell sequence results.Results: Our study revealed distinct subsets of vein endothelial cells (ECs), smooth muscle cells (SMCs), and fibroblasts (FBs) with high proportions in LS USD, contributing to the tissue microenvironment primarily involved in proinflammatory and immune responses. In particular, FBs displayed a unique subset, Fib7, which is exclusively present in LS USD, and exhibited high expression levels of SAA1 and SAA2. The accumulation of macrophages, along with the dysregulated ratios of M1/M2-like phenotype macrophages, may be engaged in the pathogenesis of LS USD. Through cell–cell communication analysis, we identified significant interactions involving CXCL8/ACKR1 and CCR7/CCL19 in LS USD. Remarkably, Fib7 exhibited exclusive communication with IL-1B macrophages through the SAA1/FPR2 receptor–ligand pair.Conclusion: Our study provides a profound understanding of the tissue microenvironment in LS USD, which may be valuable for understanding the pathogenesis of LS USD. Methodologies and devices for measuring the diffusion coefficient and solubility of radon in materials are developed and described. The radon diffusion coefficient of the LS was measured for the first time and in addition the solubility coefficient was also obtained. Abstract: The Jiangmen Underground Neutrino Observatory (JUNO), a 20 kton multipurpose underground liquid scintillator detector, was proposed for the determination of the neutrino mass hierarchy as primary physics goal. The central detector will be submerged in a water Cherenkov detector to lower the background from the environment and cosmic muons. Radon is one of the primary background sources. Nitrogen will be used in several sub-systems, and a highly sensitive radon detector has to be developed to measure its radon concentration. A system has been developed based on $^{222}$Rn enrichment of activated carbon and $^{222}$Rn detection based on the electrostatic collection. This study aimed to determine the connection between pancreatic fat and atherosclerosis in overweight and obese people without HT.Patients and methods: We included 128 patients who were non-hypertensive and overweight or obese in a study between December 2019 and November 2022. Medical history was collected, and all participants underwent a physical examination and blood tests. Pancreatic fat content was measured by magnetic resonance imaging (MRI) and was grouped into quartiles based on pancreatic fat fraction (PFF). The upper three quartiles (PFF≥ 10.33%) were defined as non-alcoholic fatty pancreas disease (NAFPD) and the first quartile (PFF< 10.33%) as non-NAFPD. High baPWV (H-baPWV) and low baPWV (L-baPWV) were classified according to the median baPWV (1159 cm/s). The effect of NAFPD on baPWV was examined using binary logistic regression. The study population consisted of 96 NAFPD and 32 non-NAFPD cases.Results: Participants with NAFPD had significantly higher levels of baPWV than people without. The rates of NAFPD and the PFF values varied significantly in the L-baPWV and H-baPWV groups. Logistic regression analysis suggested that the presence of NAFPD was independently correlated with increased baPWV after adjusting for age, smoking, body mass index, blood pressure, lipid profiles, and glycemic index.Conclusion: NAFPD is an independent risk factor for increased baPWV in individuals with overweight and obesity but no HT, suggesting that the presence of NAFPD may be a warning signal of early atherosclerosis.Keywords: obesity, non-alcoholic fatty pancreas disease, overweight, arterial stiffness, brachial-ankle pulse wave velocity

The purpose of this study was to investigate the influencing factors on clinical pregnancy after IUI.Methods: This study retrospectively analyzed 1464 cycles of IUI performed at the Meizhou People’s Hospital between March 2014 and June 2023. The χ2 test and logistic regression analysis was applied to assess the associations between the some factors (maternal age, paternal age, cycle type (natural cycle or ovulation induction cycle), hormone level on the day of endometrial transformation (estradiol (E2), luteinizing hormone (LH), and progesterone (P)), endometrial thickness on the day of endometrial transformation, and forward motile sperm concentration after treatment) and pregnancy failure.Results: Among the 1464 IUI cycles in this study, 268 cycles of assisted reproduction resulted in clinical pregnancy, with a clinical pregnancy rate of 18.3%. During the cycles with clinical pregnancy, there were 25 (12.9%) preterm births and 169 (87.1%) full-term births. The E2 level on the day of endometrial transformation in clinical pregnancy group was higher than that in the pregnancy failure group (658.79± 656.02 vs 561.21± 558.83 pg/mL)(P=0.025). The clinical pregnancy group had a higher percentage of endometrial thickness between 8 and 13mm on the day of endometrial transformation than the pregnancy failure group (83.2% vs 75.0%)(P=0.002). The results of regressions analysis showed that low E2 level on the day of endometrial transformation (< 238.3 pg/mL vs ≥ 238.3 pg/mL: OR 1.493, 95% CI: 1.086– 2.052, P=0.014), and endometrial thickness < 8mm on the day of endometrial transformation (< 8mm vs 8– 13mm: OR 1.886, 95% CI: 1.284– 2.771, P=0.001) may increase risk of pregnancy failure performed IUI.Conclusion: Low estradiol level, and endometrial thickness on the day of endometrial transformation may increase risk of pregnancy failure performed intrauterine insemination.Keywords: intrauterine insemination, pregnancy failure, estradiol, endometrial thickness

Furthermore, many known situations are risk factors of S. constellatus infection, but S. constellatus pyopneumothorax associated with Hashimoto’s thyroiditis has not been reported.Case Presentation: We present the case of a 74-year-old male with multiple encapsulated pyopneumothorax caused by S. constellatus. Given his respiratory failure, we provided two-stage percutaneous right empyema radiography for catheter drainage in the radiology interventional department instead of surgery. Moreover, an occult Hashimoto’s thyroiditis was discovered in the patient, which was possibly associated with S. constellatus pyopneumothorax. Levothyroxine was administered to improve his situation.Conclusion: To our knowledge, it is the first case described in this context. We provided an alternative treatment for S. constellatus encapsulated pyopneumothorax in patient who might not tolerate surgery. There are currently few case reports, diagnoses and treatments for these syndromes at home and abroad.Case Description: This study reports a case of a boy with “growth and language development delay” as the predominant clinical manifestation. FHS was clinically diagnosed based on his growth hormone (GH) deficiency, significant bone age delay, left testicular hydrocele, and the whole exon gene in peripheral blood, which indicated heterozygous mutation of SRCAP gene. Following the treatment with recombinant human GH (rhGH), the child exhibited height increase benefits, and his articulation improved after language therapy.Conclusion: Genetic testing facilitates early detection, diagnosis, and treatment of the FHS. Chi-square test, Mann–Whitney U-test, and two independent t-test were used to identify the intergenerational differences in sociodemographic characteristics, received family support, anxiety, depression, and HRQoL. Eight generalized linear models were developed to examine the associations in both generations.Results: Parents and grandparents were most in need of rehabilitation and financial support, but both populations received the least amount of financial support. 33.6% and 36.1% of family caregivers had the risk of anxiety and depression and found no significant intergenerational differences. As for HRQoL, intergenerational differences were found in the physical component score, but not found in the mental component score. Among parents, childcare support of respite care and household tasks assistance was negatively associated with their depression (p< 0.05), professional support of appropriate surgery for children (p< 0.05) and psychological support from professional psychologists (p< 0.01) were negatively associated with their anxiety and depression, psychological support from relatives and friends was negatively associated with their depression (p< 0.05) whereas was positively associated with their mental HRQoL (p< 0.01). As for grandparents, financial support for daily living expenses was negatively associated with depression (p< 0.05), and psychological support from families was negatively associated with depression (p< 0.05) whereas was positively associated with mental HRQoL (p< 0.05). However, no relationship was found between family support and anxiety among grandparents. Notably, no association was found between family support and physical HRQoL among both populations.Conclusion: Both parents and grandparents had high risks of anxiety, depression and low levels of mental HRQoL. Furthermore, multivariate linear regression analyses showed that baseline age (B =− 0.020, P = 0.020), SE (B = 0.0517, P < 0.001), and total PSQI (B=0.026, P = 0.001) were associated with axial elongation. Advanced logistic regression analyses demonstrated that shorter average near-work distance (P = 0.034), higher SE value (P = 0.023), and poorer sleep quality (P = 0.003) were associated with fast axial elongation.Conclusion: Sleep quality is one of the key factors associated with axial elongation in children with myopia after using ortho-k for one year. Gold nanoparticles (AuNPs) have been suggested as potential radio-sensitizers, but it is unclear if they can enhance radiation-induced ICD. This study aimed to investigate the potential of AuNPs to improve the effectiveness of radiation-induced ICD.Methods: G422 cells were treated with a combination of AuNPs and RT to induce cell death. Various assays were conducted to assess cell death, surface expression of CRT, and release of HMGB1 and ATP. In vitro co-culture experiments with bone marrow-derived dendritic cells (BMDCs) were performed to analyze the immunogenicity of dying cancer cells. Flow cytometry was used to measure the maturation rate of BMDCs. An in vivo mouse tumor prophylactic vaccination model was employed to assess immunogenicity.Results: The study findings presented here confirm that the combination of radiotherapy (RT) with AuNPs can induce a stronger ICD effect on glioblastoma cells compared to using RT alone. Specifically, treatment with AuNPs combined with RT resulted in the emission of crucial damage-associated molecular patterns (DAMPs) such as CRT, HMGB1 (479.41± 165.34pg/mL vs 216.04± 178.16 pg/mL, &ast;P< 0.05) and ATP (The release of ATP in the AuNPs + RT group was 1.2 times higher than in the RT group, &ast;P< 0.05). The proportion of BMDC maturation rate was higher in the group treated with AuNPs and RT compared to the group treated with RT alone. (32.53± 0.52% vs 25.03± 0.28%,&ast;&ast;&ast;P < 0.001). One of the features of pancreatic cancer (PC) metabolic reprogramming is the crosstalk between PC and peripheral tissues (skeletal muscle and adipose tissues), emphasized by insulin resistance (IR). Our previous study reported that mice pancreatic cancer-derived exosomes could induce skeletal muscle cells (C2C12) IR, and exosomal microRNAs (miRNAs) may exert an important effect. However, the underlying mechanism remains to be further elucidated.Methods: qPCR was used to determine the expression of let-7b-5p in normal pancreatic islet cells and PC cells. Exosomes were purified from PC cell culture medium by ultracentrifugation. The role let-7b-5p on IR-mediated by PC cells-derived exosomes was asses by Oil Red O staining using miRNA inhibitor. Western blot assay was performed to examine the expression of IR-related genes and the activation of signaling pathways. A Luciferase experiment was applied to confirm how let-7b-5p regulated the expression of RNF20. IP/WB analysis further determined whether RNF20 promoted STAT3 ubiquitination. Rescue experiment using RNF20 overexpression plasmid was performed to confirm the role of RNF20 on IR-mediated using PC cell-derived exosomes in C2C12 myotube cells.Results: miRNA-let-7b-5p was identified as the key exosomal miRNA, which could promote the IR in C2C12 myotube cells supported the lipid accumulation, the activation of STAT3/FOXO1 axis, and the decreased expression of IRS-1 and GLUT4. RNF20, an E3 ubiquitin ligase, was confirmed as the target gene of let-7b-5p and was found to improve IR by downregulating STAT3 protein expression via ubiquitination-mediated protein degradation. The ectopic expression of RNF20 could effectively attenuate the IR mediated by the pancreatic cancer-derived exosomes in C2C12 myotube cells.Conclusion: Our data suggest that exosomal miRNA-let-7b-5p may promote IR in C2C12 myotube cells by targeting RNF20 to activate the STAT3/FOXO1 axis.Graphical Abstract: Keywords: insulin resistance, exosome, let-7b-5p, RNF20, STAT3, FOXO1

A total of 37 youths with T1DM aged 10– 45 years were enrolled. The intervention comprised 11 structured online DSMES course sessions; these were video-based and delivered by a diabetes specialist, nurses and a dietitian. The primary outcome was a change in (glycated hemoglobin) HbA1c. The secondary outcomes were changes in hypoglycaemia frequency, time in target range (TIR) among patients using a continuous glucose monitoring (CGM) system and self-management behaviour; the latter was measured using a T1DM self-management scale for Chinese adults (SMOD-CA) and the Chinese version of the diabetic behaviour rating scale in adolescents with T1DM (DBRS).Results: Twenty-three (85.2%) participants attended ≥ 8 of the online sessions. There was a significant reduction in HbA1c (from 6.92% to 6.47%, P = 0.002), hypoglycaemic episodes (from 6.0 to 4.0 during the preceding month, P = 0.026) and a significant increase in TIR (from 74.0% to 80.5%, P = 0.027) and an increase in the SMOD-CA score (from 79.6 to 84.6, P = 0.026) in young adults. No significant change in glucose control, hypoglycaemic events or DBRS score were found among children and adolescents. The score of the 12-item version of the Barrett–Lennard Relationship Inventory (B-L RI:mini) indicated that more than half of the participants experienced congruence, positive regard, and an empathic understanding in this programme.Conclusion: The online structured DSMES programme was effective in improving the glycaemic control and self-management behaviour of young adults with T1DM; however, integrating offline visits or appointments with online consultations may be necessary for youth patients.Keywords: children and adolescents, youth, type 1 diabetes, structured diabetes self-management education and support, online

Although CFCS is considered rare, approximately 300 cases have been documented in the literature. In this report, we discuss a patient diagnosed with CFCS without the typical heart malformations but with craniofacial features, skin abnormalities, intellectual disability, and short stature. Genetic testing revealed the presence of three potentially harmful variants: one in the MAP2K1 gene and two in the ATP2B3 and CDC42BPB genes, the significance of which is currently not yet found. Our findings in this case report suggest that the clinical symptoms of CFCS may be atypical, thereby expanding our understanding of the symptom spectrum of the disease. Simultaneously, the link between the clinical symptoms of the patient and the two unknown pathogenic variants has not been established. By one-to-two propensity score matching for age and sex, the control group of those not re-testing positive during the same period served as matched control.Results: A total of 223 patients were included in our study, 94 in re-positive group and 129 in non-re-positive group. The result shows that the rates of nasal douche treatment in the non-re-positive group were considerably higher than that of the re-positive group. And the Ct value of nasal douche group increased faster than that of non-nasal douche group after the Ct value reaching ≥ 35. Further analysis revealed that the higher the Ct value at the time of readmission, the shorter the time of average Ct values to reach ≥ 35.Conclusion: These findings suggest that nasal douche is beneficial to shorten the time of virus nucleic acid turning negative, thereby reducing the incidence of re-positive. 37. Fast neural Poincar\'e maps for toroidal magnetic fields

Abstract: Poincar\'e maps for toroidal magnetic fields are routinely employed to study gross confinement properties in devices built to contain hot plasmas. In most practical applications, evaluating a Poincar\'e map requires numerical integration of a magnetic field line, a process that can be slow and that cannot be easily accelerated using parallel computations. We show that a novel neural network architecture, the H\'enonNet, is capable of accurately learning realistic Poincar\'e maps from observations of a conventional field-line-following algorithm. After training, such learned Poincar\'e maps evaluate much faster than the field-line integration method. Moreover, the H\'enonNet architecture exactly reproduces the primary physics constraint imposed on field-line Poincar\'e maps: flux preservation. This structure-preserving property is the consequence of each layer in a H\'enonNet being a symplectic map. We demonstrate empirically that a H\'enonNet can learn to mock the confinement properties of a large magnetic island by using coiled hyperbolic invariant manifolds to produce a sticky chaotic region at the desired island location. This suggests a novel approach to designing magnetic fields with good confinement properties that may be more flexible than ensuring confinement using KAM tori., Comment: 28 pages, 26 figures

Abstract: To maximize the light yield of the liquid scintillator (LS) for the Jiangmen Underground Neutrino Observatory (JUNO), a 20 t LS sample was produced in a pilot plant at Daya Bay. The optical properties of the new LS in various compositions were studied by replacing the gadolinium-loaded LS in one antineutrino detector. The concentrations of the fluor, PPO, and the wavelength shifter, bis-MSB, were increased in 12 steps from 0.5 g/L and <0.01 mg/L to 4 g/L and 13 mg/L, respectively. The numbers of total detected photoelectrons suggest that, with the optically purified solvent, the bis-MSB concentration does not need to be more than 4 mg/L. To bridge the one order of magnitude in the detector size difference between Daya Bay and JUNO, the Daya Bay data were used to tune the parameters of a newly developed optical model. Then, the model and tuned parameters were used in the JUNO simulation. Abstract: We report the design, fabrication and testing of Lumped Element Kinetic Inductance Detectors (LEKID) showing performance in line with the requirements of the next generation space telescopes operating in the spectral range from 80 to 600 GHz. This range is of particular interest for Cosmic Microwave Background (CMB) studies. For this purpose we have designed and fabricated 100-pixel arrays covering five distinct bands. These wafers have been measured via multiplexing, where a full array is read out using a single pair of lines. We adopted a custom cold black-body installed in front of the detectors and regulated at temperatures between 1 K and 20 K. We will describe in the present paper the main design considerations, the fabrication processes, the testing and the data analysis.

Abstract: Subseasonal-to-seasonal (S2S) prediction, especially the prediction of extreme hydroclimate events such as droughts and floods, is not only scientifically challenging, but also has substantial societal impacts. Motivated by preliminary studies, the Global Energy and Water Exchanges (GEWEX)/Global Atmospheric System Study (GASS) has launched a new initiative called "Impact of Initialized Land Surface Temperature and Snowpack on Subseasonal to Seasonal Prediction"(LS4P) as the first international grass-roots effort to introduce spring land surface temperature (LST)/subsurface temperature (SUBT) anomalies over high mountain areas as a crucial factor that can lead to significant improvement in precipitation prediction through the remote effects of land-atmosphere interactions. LS4P focuses on process understanding and predictability, and hence it is different from, and complements, other international projects that focus on the operational S2S prediction. More than 40 groups worldwide have participated in this effort, including 21 Earth system models, 9 regional climate models, and 7 data groups. This paper provides an overview of the history and objectives of LS4P, provides the first-phase experimental protocol (LS4P-I) which focuses on the remote effect of the Tibetan Plateau, discusses the LST/SUBT initialization, and presents the preliminary results. Multi-model ensemble experiments and analyses of observational data have revealed that the hydroclimatic effect of the spring LST on the Tibetan Plateau is not limited to the Yangtze River basin but may have a significant large-scale impact on summer precipitation beyond East Asia and its S2S prediction. Preliminary studies and analysis have also shown that LS4P models are unable to preserve the initialized LST anomalies in producing the observed anomalies largely for two main reasons: (i) inadequacies in the land models arising from total soil depths which are too shallow and the use of simplified parameterizations, which both tend to limit the soil memory; (ii) reanalysis data, which are used for initial conditions, have large discrepancies from the observed mean state and anomalies of LST over the Tibetan Plateau. Innovative approaches have been developed to largely overcome these problems.

The chemotherapy and targeted medication are the mainstay for the recurrent ovarian cancer treatment. About 70% of the advanced-stage cases will relapse. Ascites-derived organoid is a pre-clinical model for the precise prediction of the therapeutic effectiveness for the ovarian cancer: it can be used to assess the drug sensitivity, to guide individualized precise treatment, and to improve advanced stage as well as recurrent ovarian cancer patient’ survival and prognosis. Until now, there has been no report concerning the establishment of the organoid out of the patient’s ascites and the concurrent usage of drug sensitivity test to guide the individualized precise treatment for the ovarian cancer. Here, we report a case of recurrent ovarian cancer of a 59-year-old female patient whose CA125 at its peak increased to 4523.4 U/mL. Then, patient’s own ovarian cancer organoid was constructed from the ascites by the abdominocentesis; concurrently, medication sensitivity test was performed on the organoid to guide individualized precise treatment. After the treatment, CA125 decreased to 33.7 U/mL, and the patient’s condition relieved effectively. A retrospective cross-sectional analysis was performed to understand the characteristics of critically ill patients and compare the proportion of hospitalizations, deaths, and emergency ambulance calls (EACs).Results: The number of patients presenting to the ED was significantly decreased, with a 37.75% reduction in 2020 (6196) compared to the same period in 2019 (9954). A rise in patient ED presentations was observed in the same period in 2021 (10,503). This decline was mostly in the 15– 45 age group. In 2019, 2020, and 2021, critically ill patients treated by the ED totaled 568 (5.706%), 339 (5.495%), and 590 (5.617%), respectively. Compared to the same period in 2019 and 2021, the proportion of critically ill patients with respiratory system involvement, severe trauma, and poisoning decreased most significantly in 2020 (P< 0.05). In contrast, the rates of EACs, hospitalizations, and deaths increased significantly (P< 0.05).Conclusion: The number of ED visits to hospitals was decreased during the 2020 lockdown, while the rates of EACs, hospitalizations, and deaths increased significantly though there were no documented COVID-19 cases. In patients clinically suspected with PCD, a variety of examination methods are available to assist in diagnosis, such as high-speed video microscopic imaging to analyze ciliary movement patterns, transmission electron microscopy to observe ciliary ultrastructure, genetic testing, and detection of nitric oxide content in nasal expiratory air.Case Description: We present a case summary of the clinical data and treatment process of a child with PCD and short stature induced by Novel exon 1 of CCNO mutation (NM-021147.5) at c.323del, and the proband father and mother were heterozygous mutators, who was diagnosed and treated in the Pediatric Healthcare Department of our hospital. We treated the child with recombinant human growth hormone to increase the height, and the patient was also advised to improve nutrition, prevent and control infections, and encouraged sputum expectoration. We also recommended regular follow-up visits to the outpatient department, and to seek other symptomatic and supportive treatments as necessary.Conclusion: The height and nutritional status of the child improved after treatment. 44. Evaluation of the interactive stratospheric ozone (O3v2) module in the E3SM version 1 Earth system model

Abstract: Stratospheric ozone affects climate directly as the predominant heat source in the stratosphere and indirectly through chemical reactions controlling other greenhouse gases. The U.S. Department of Energy’s Energy Exascale Earth System Model version 1 (E3SMv1) implemented a new ozone chemistry module that improves the simulation of the sharp tropopause gradients, replacing a version based partly on long-term average climatologies that poorly represented heating rates in the lowermost stratosphere. The new O3v2 module extends seamlessly into the troposphere and preserves the naturally sharp cross-tropopause gradient, with 20 %–40 % less ozone in this region. Additionally, O3v2 enables the diagnosis of stratosphere–troposphere exchange flux of ozone, a key budget term lacking in E3SMv1. Here, we evaluate key features in ozone abundance and other closely related quantities in atmosphere-only E3SMv1 simulations driven by observed sea surface temperatures (SSTs, years 1990–2014), comparing them with satellite observations of ozone and also with the University of California, Irvine chemistry transport model (UCI CTM) using the same stratospheric chemistry scheme but driven by European Centre forecast fields for the same period. In terms of stratospheric column ozone, O3v2 shows reduced mean bias and improved northern midlatitude variability, but it is not quite as good as the UCI CTM. As expected, SST-forced E3SMv1 simulations cannot synchronize with observed quasi-biennial oscillations (QBOs), but they do show the typical QBO pattern seen in column ozone. This new O3v2 E3SMv1 model mostly retains the same climate state and climate sensitivity as the previous version, and we recommend its use for other climate models that still use ozone climatologies.

Abstract: In this paper, we introduce a notion of generalized potential games that is inspired by a newly developed theory on generalized gradient flows. More precisely, a game is called generalized potential if the simultaneous gradient of the loss functions is a nonlinear function of the gradient of a potential function. Applications include a class of games arising from chemical reaction networks with detailed balance condition. For this class of games, we prove an explicit exponential convergence to equilibrium for evolution of a single reversible reaction. 46. AC Oscillation of a Spin Soliton Driven by a Constant Force

Abstract: The phenomena of AC oscillation generated by a DC drive, such as the famous Josephson AC effect in superconductors and Bloch oscillation in solid physics, are of great interest in physics. Here we report another example of such counter-intuitive phenomenon that a spin soliton in a two-component Bose-Einstein condensate is driven by a constant force: The initially static spin soliton first moves in a direction opposite to the force and then changes direction, showing an extraordinary AC oscillation in a long term. In sharp contrast to the Josephson AC effect and Bloch oscillation, we find that the nonlinear interactions play important roles and the spin soliton can exhibit a periodic transition between negative and positive inertial mass even in the absence of periodic potentials. We then develop an explicit quasiparticle model that can account for this extraordinary oscillation satisfactorily. 47. Design and Bolometer Characterization of the SPT-3G First-year Focal Plane

Abstract: During the austral summer of 2016-17, the third-generation camera, SPT-3G, was installed on the South Pole Telescope, increasing the detector count in the focal plane by an order of magnitude relative to the previous generation. Designed to map the polarization of the cosmic microwave background, SPT-3G contains ten 6-in-hexagonal modules of detectors, each with 269 trichroic and dual-polarization pixels, read out using 68x frequency-domain multiplexing. In China, the most common species is Sporothrix globosa which is difficult to treat with most antifungal drugs. Atypical clinical forms of sporotrichosis can be a hinder to clinicians for an early diagnosis and treatment. We report a case of fixed cutaneous sporotrichosis of the face caused by S. globosa in a healthy adult that was initially misdiagnosed as rosacea due to its unusual clinical features. Rifampin (RIF) is a first-line antimicrobial agent with potent bactericidal action. Nonetheless, the blood-brain barrier (BBB) limits the therapeutic effects on CNS-TB. Exosomes, however, can facilitate drug movements across the BBB. In addition, exosomes show high biocompatibility and drug-loading capacity. They can also be modified to increase drug delivery efficacy. In this study, we loaded RIF into exosomes and modified the exosomes with a brain-targeting peptide to improve BBB permeability of RIF; we named these exosomes ANG-Exo-RIF.Methods: Exosomes were isolated from the culture medium of BMSCs by differential ultracentrifugation and loaded RIF by electroporation and modified ANG by chemical reaction. To characterize ANG-Exo-RIF, Western blot (WB), nanoparticle tracking analysis (NTA) and transmission electron microscopy (TEM) were performed. Bend.3 cells were incubated with DiI labeled ANG-Exo-RIF and then fluorescent microscopy and flow cytometry were used to evaluate the targeting ability of ANG-Exo-RIF in vitro. Fluorescence imaging and frozen section were used to evaluate the targeting ability of ANG-Exo-RIF in vivo. MIC and MBC were determined through microplate alamar blue assay (MABA).Results: A novel exosome-based nanoparticle was developed. Compared with untargeted exosomes, the targeted exosomes exhibited high targeting capacity and permeability in vitro and in vivo. The MIC and MBC of ANG-Exo-RIF were 0.25 μg/mL, which were sufficient to meet the clinical needs.Conclusion: In summary, excellent targeting ability, high antitubercular activity and biocompatibility endow ANG-Exo-RIF with potential for use in future translation-aimed research and provide hope for an effective CNS-TB treatment.Graphical Abstract: Keywords: exosome, central nervous system tuberculosis, blood-brain barrier, rifampin

The purpose of this study was to explore the possible correlation of VFA with DPN in such a population.Methods: A total of 2498 hospitalized patients with type 2 diabetes mellitus (T2DM) undergone VFA measurement, and were divided into DPN group (n=900) and non-DPN group (n=1594). The association of VFA with the presence of DPN was evaluated by correlation and multiple logistic regression analyses, generalized additive model with a smooth curve fitting, and receiver operating characteristic (ROC) curve analysis.Results: The VFA was significantly lower in the DPN group than in the non-DPN group (P < 0.001). VFA was significantly and positively associated with sural nerve conduction velocity (SNCV) and amplitude potential (SNAP) and negatively associated with the presence of DPN (all P< 0.001); there was no significant difference in the curve fitting (P = 0.344). Multivariate logistic regression analysis showed that the risk of presence of DPN decreased progressively across the VFA quartiles (P for trend < 0.001) and was significantly lower in patients in the highest VFA quartile than in those in the lowest quartile (OR: 0.382, 95% CI 0.151– 0.968, P< 0.001) after multivariate adjustment. The ROC analysis revealed that the best cut-off value of VFA for predicting the presence of DPN was 50.5cm2 (sensitivity 84.40%; specificity 34.00%).Conclusion: These results suggest that lower VFA level may be associated with increased risk of the presence of DPN in T2DM patients.Keywords: visceral fat area, diabetic peripheral neuropathy, sural nerve conduction velocity, sural nerve amplitude potential, Chinese population
