Purpose of the study. To determine the sensitivity and specificity of central lymph nodes (CLN) ultrasound in the detection of papillary thyroid cancer metastases (PTCM), and the possibility of detecting metastases with ultrasound, depending on their size. Materials and methods. A retrospective mono­centric study of PTCM ultrasound detecting capabilities was carried out using a linear sensor with a frequency of 3—12 MHz.The presence of metastases in the CLN was assessed in 239 patients with a diagnosis of papillary thyroid cancer (PTC) by ultrasound before surgery (extrafascial thyroidectomy, central eck dis­section, lymphadenectomy), with further histopatho­logical examination. We determined the sensitivity and specificity of ultrasound in detecting PTCM in the central lymphatic collector.The predictive value of a positive and egative ultrasound results were calculated. The probability of detecting PTCM is presented depending on their size. Results and discussion. Calculations of the operational characteristics of CLN ultrasound showed (95 % CI — confidence interval): the sensitivity of the method is 0.49 (0.43; 0.55), the specificity of the method is 0.96 (0.92; 0.98). Predictive value of a positive ultrasound result: 0.92 (0.88; 0.95). Predictive value of a egative ultrasound result: 0.64 (0.58; 0.70).The results indicate a low sensitivity and high specificity of ultrasound in detecting metastases in the CLN. In addition, if the size of the PTCMare ≥ 1.2 cm, the probability of its detection is 95 %. 76.2 % of observations fall within the range of PTCM sizes up to 0.6 cm, the probability of their detection on ultrasound is 0.47. Conclusions. The sensitivity of ultrasound in detecting PTCM in the CLN is low, this is due to the fact that 76.5 % of metastases fall within the range of sizes up to 0.6 cm, the probability of detection of which does ot exceed 0.47.The specificity of CLN ultrasound is high— if a PTCM is detected, it will be verified with 95 % probability.If the size of the metastasis in the CLNis ≥ 1.2 cm, the probability of its detection on ultrasound is 95 %.If PTCM is detected on ultrasound, this, as a rule, will be confirmed histologically. Negative ultrasound result is often erroneous.Since the probability ofdetecting most metastases in CLN on ultrasound is 0.47, it is ecessary to apply an additional method of their verification, amely: using of the latest advanced ultrasound imaging technologies. Prophylactic central lymph dissection of the eck with intraoperative histological examination should be performed in all patients with suspected or proven papillary thyroid cancer., Цель работы — определить чувствительность и специфичность ультразвукового исследования (УЗИ) центральных лимфоузлов шеи (ЦЛУШ) относительно выявления метастазов папиллярного рака щитовидной железы (МПРЩЖ) на дооперационном этапе и возможности визуализации этих метастазов в зависимости от их размера. Материалы и методы. Проведено ретро-проспективное моноцентровое исследование возможностей УЗИ относительно выявления МПРЩЖ с использованием линейного датчика частотой 3—12 МГц. Наличие метастазов в ЦЛУШ оценили у 239 пациентов с диагнозом «рак щитовидной железы» по данным УЗИ до операции (экстрафасциальная тиреоидэктомия, центральная диссекция шеи, лимфаденэктомия) и результатам патогистологического исследования после операции. Определили чувствительность и специфичность УЗИ относительно выявления МПРЩЖ в центральный лимфатический коллектор. Рассчитали прогностическую ценность положительного и отрицательного результата УЗИ. Определили вероятность обнаружения МПРЩЖ в зависимости от их размера. Результаты и обсуждение. Расчеты опера­ционных характеристик УЗИ метастазов в ЦЛУШ показали, что чувствительность метода — 0,49 (95 % доверительный интервал (ДИ) — 0,43; 0,55), специфичность метода — 0,96 (95 % ДИ — 0,92; 0,98), прогностическая ценность положительного результата УЗИ — 0,92 (95 % ДИ — 0,88; 0,95), прогностическая ценность отрицательного результата УЗИ — 0,64 (95 % ДИ — 0,58; 0,70). Полученные результаты свидетельствуют о низкой чувствительности и высокой специфичности УЗИ относительно выявления метастазов в ЦЛУШ. Выводы. Отримані результати свідчать про низьку чутливість і високу специфічність УЗД щодо виявлення метастазів у ЦЛВШ. Висновки. Чутливість УЗД щодо виявлення метастазів у ЦЛВШ є низькою, що зумовлено тим, що 76,5 % метастазів потрапили в інтервал розмірів

The purpose of this work was: 1. To test the efficacy of two molecular methods: Restriction Fragment Length Polymorphism Polymerase Chain Reaction (RFLP PCR) for detection of mutation in codon 12 of oncosuppressor K-ras gene and Reverse Transcription (RT) PCR for Cytokeratine20 (CK20) expression as markers for molecular detection of occult metastases (MDM) in patients with adenocarcinoma of the pancreas. 2. To analyze factors of influence and pattern of MDM appearance in distant lymph nodes in patients after extended pancreatoduodenal resection (R0 patients). 3. To analyze the influence of distant nodal MDM on clinical prognosis. Methods: The study was carried out on 28 patients. Altogether there were 531 lymph nodes histologically investigated in this study. Of these 305 lymph nodes were distant regional lymph nodes hemisected and tested for the K-ras and CK20 markers. Results: 1. Histological examination showed tumor invasion in 11,0 % distant lymph nodes. Application of molecular methods (K-ras and CK20 together) increased this index up to 25,9 %. The method of combined K-ras and CK20 detection has sensitivity 96,7 %. 2. The distant nodal MDM in patients with pancreatic cancer is a phenomenon independent from the morphological parameters of primary tumor and pN status. The most frequent site of MDM appearance in regional lymphatic system was the para-aortal lymph nodal station. 3. Among patients with MDM who were diagnosed on the basis of a concurrence of both positive K-ras and CK20 markers in the same lymph node (concordant markers), there were 4 patients with MDM in para-aortic lymph nodes (LN16). In this group there were no 2-year survivors observed and the median survival and median PFS were both equal to 13 (range, 1-18) months. This group of patients was compared to the group of 10 patients with no signs of MDM in para-aortic lymph nodes and the difference in the survival rate was considerable. The 2-year actual survival and 2-year progression free survival (PFS) in the latter group respectively was 50 % and 60 % while median survival and median PFS, respectively, were 32 (range, 1-40) and 26 ( range, 1-37) months. The data concerning clinical relevance of MDM was obtained from 14 patients. The question of MDM influence on clinical prognosis requires further investigation in larger populations of patients.
