Smoothness is a hallmark of skilled, coordinated movement, however, mathematically quantifying movement smoothness is nuanced. Several smoothness metrics exist, each having its own limitations and may be specific to a particular motion such as upper limb reaching. To date, there is no consensus on which smoothness metric is the most appropriate for assessing cycling motion in children with cerebral palsy (CP). We evaluated the ability of four preexisting metrics, dimensionless jerk, spectral arc length measure, roughness index, and cross-correlation; and two new metrics, arc length and root mean square error, to quantify the smoothness of cycling in a preexisting dataset from children with CP (mean age 13.7 ± 2.6 years). First, to measure the repeatability of each measure in distinguishing between different levels of un-smoothness, we applied each metric to a set of simulated crank motion signals with a known number of aberrant revolutions using subjects’ actual crank angle data. Second, we used discriminant function analysis to statistically compare the strength of the six metrics, relative to each other, to discriminate between a smooth cycling motion obtained from a dataset of typically developed children (TD), the control group (mean age 14.9 ± 1.4 years), and a less smooth, halted cycling motion obtained from children with CP. Our results show that (1) ArcL showed the highest repeatability in accurately quantifying an unsmooth motion when the same cycling revolutions were presented in a different order, and (2) ArcL and DJ had the highest discriminatory ability to differentiate between an unsmooth and smooth cycling motion. Combining the results from the repeatability and discriminatory analysis, ArcL was the most repeatable and sensitive metric in identifying unsmooth, halted cycling motion from smooth motion. ArcL can hence be used as a metric in future studies to quantify changes in the smoothness of cycling motion pre- vs. post-interventions. Further, this metric may serve as a tool to track motor recovery not just in individuals with CP but in other patient populations with similar neurological deficits that may present with halted, unsmooth cycling motion.

Medical training simulators have the potential to provide remote and automated assessment of skill vital for medical training. Consequently, there is a need to develop “smart” training devices with robust metrics that can quantify clinical skills for effective training and self-assessment. Recently, metrics that quantify motion smoothness such as log dimensionless jerk (LDLJ) and spectral arc length (SPARC) are increasingly being applied in medical simulators. However, two key questions remain about the efficacy of such metrics: how do these metrics relate to clinical skill, and how to best compute these metrics from sensor data and relate them with similar metrics? This study addresses these questions in the context of hemodialysis cannulation by enrolling 52 clinicians who performed cannulation in a simulated arteriovenous (AV) fistula. For clinical skill, results demonstrate that the objective outcome metric flash ratio (FR), developed to measure the quality of task completion, outperformed traditional skill indicator metrics (years of experience and global rating sheet scores). For computing motion smoothness metrics for skill assessment, we observed that the lowest amount of smoothing could result in unreliable metrics. Furthermore, the relative efficacy of motion smoothness metrics when compared with other process metrics in correlating with skill was similar for FR, the most accurate measure of skill. These results provide guidance for the computation and use of motion-based metrics for clinical skill assessment, including utilizing objective outcome metrics as ideal measures for quantifying skill.

Recently, activities of daily living (ADL) received strong interest in research on aging and neurorehabilitation. Such tasks have complex signal structures and kinematic parameters need to be adapted. In the present study we examined four different approaches to quantify movement smoothness in ADL. We tested the appropriateness of these approaches, namely the number of velocity peaks per meter (NoP), the spectral arc length (SAL), the speed metric (SM) and the log dimensionless jerk (LDJ), by comparing movement signals from eight healthy elderly (67.1a ± 7.1a) with eight healthy young (26.9a ± 2.1a) participants performing an activity of daily living (making a cup of tea). All approaches were able to identify group differences in smoothness (Cohen's d NoP = 2.53, SAL = 1.95, SM = 1.69, LDJ = 4.19), three revealed high to very high sensitivity (z-scores: NoP = 1.96 ± 0.55, SAL = 1.60 ± 0.64, SM = 3.41 ± 3.03, LDJ = 5.28 ± 1.52), three showed low within-group variance (NoP = 0.72, SAL = 0.60, SM = 0.11, LDJ = 0.71), two showed strong correlations between the first and the second half of the task execution (intra-trial R2s: NoP = 0.22 n.s., SAL = 0.33, SM = 0.36, LDJ = 0.91), and one was independent of other kinematic parameters (SM), while three showed strong models of multiple linear regression (R2s: NoP = 0.61, SAL = 0.48, LDJ = 0.70). Based on our results we make suggestion toward use examined smoothness measures. In total the log dimensionless jerk proved to be the most appropriate in ADL, as long as trial durations are controlled.
