Background: Within the European Union (EU), substantial efforts are being made to achieve economic and social cohesion, and the reduction of health inequalities between EU regions is integral to this process. This paper is the first to examine how self-reported conditions and non-communicable diseases (NCDs) vary spatially between and within countries. Methods: Using 2014 European Social Survey (ESS) data from 20 countries, this paper examines how regional inequalities in self-reported conditions and NCDs vary for men and women in 174 regions (levels 1 and 2 Nomenclature of Statistical Territorial Units, 'NUTS'). We document absolute and relative inequalities across Europe in the prevalence of eight conditions: general health, overweight/obesity, mental health, heart or circulation problems, high blood pressure, back, neck, muscular or joint pain, diabetes and cancer. Results: There is considerable inequality in self-reported conditions and NCDs between the regions of Europe, with rates highest in the regions of continental Europe, some Scandinavian regions and parts of the UK and lowest around regions bordering the Alps, in Ireland and France. However, for mental health and cancer, rates are highest in regions of Eastern European and lowest in some Nordic regions, Ireland and isolated regions in continental Europe. There are also widespread and consistent absolute and relative regional inequalities in all conditions within countries. These are largest in France, Germany and the UK, and smallest in Denmark, Sweden and Norway. There were higher inequalities amongst women. Conclusion: Using newly available harmonized morbidity data from across Europe, this paper shows that there are considerable regional inequalities within and between European countries in the distribution of self-reported conditions and NCDs. Background Frailty is a common, multi-factorial, age-related syndrome commonly observed in people with diabetes. Although older diabetics are prone to adverse healthcare outcomes and diabetes increases the risk of developing frailty, little is known about the effects of frailty on diabetes. This paper examines the association between diabetes, frailty, and mortality in Europeans aged ≥50 years. Methods Data were included from The Survey of Health, Ageing and Retirement in Europe (SHARE) waves one and six. A participant's first interview was taken as the baseline and subsequent waves were used for mortality follow-up (time and cause). Frailty and pre-frailty were measured using established cut-offs using the Physical Phenotype (SHARE-FI) and a 55-item Frailty Index (FI-55). Kaplan-Meier survival analysis was used to assess the relationship between frailty and mortality in people with diabetes and significance tested using log-rank test. Cox regression was used to adjust for potential confounders (age, sex, education, income, employment, alcohol use, smoking, hypertension, hypercholesterolaemia, myocardial infarction, stroke, metastatic cancer, chronic lung disease, polypharmacy, self-perceived health and depression). Results Data from 8,954 diabetics aged 50-99 years were included with 1,598 deaths (17.8%). According to the SHARE-FI, 1,971 (22.0%) were frail, 4,183 (46.7%) pre-frail and 2,800 (31.3%) robust. According to the Kaplan-Meier log-rank test survival varied significantly across frailty strata according to both indexes (p<0.001). At 10-year follow-up, adjusting for confounders SHARE-FI frailty and pre-frailty were significantly associated with mortality, adjusted Hazard Ratio (HR) 2.19, (95% CI:1.66-2.89), and 1.38 (95% CI:1.09-1.74), respectively. Results were similar using the FI-55, HR for frailty 1.66 (95% CI:1.09-2.54). Causes of mortality were significantly different according to frailty status (p<0.05). Conclusion Frailty and pre-frailty are independent risk factors for mortality in diabetics. The identification of frailty is important for the risk-stratification and management of middle aged and older patients with diabetes and should be included in the routine assessment of these high-risk individuals. Context/problem: Comparative analysis of relative and absolute mortality differentials between socioeconomic groups are now available. These show consistently increasing relative increases in mortality differentials but complex trends in absolute mortality differentials. Objective: This paper provides an analysis of relative and absolute trends in mortality by cause of death and socioeconomic group (SEG) from 1984 to 2008 among men and active women aged 30-64 years in Ireland and compares these results with recent European and US studies to give an overview of trends. Methods: This paper uses mortality data from the Irish Central Statistics Office from 1984 to 2008 to calculate standardized death rates by age, sex, socioeconomic status and cause of death showing trends in SEG inequalities in mortality in Ireland. These show which specific causes of death are driving all-cause mortality trends. Results: SEG differentials in all-cause mortality among men and women have been increasing since the 1980s. Some of this increase reflects larger falls in cardiovascular causes among advantaged groups, but the trend is largely accounted for by increasing inequalities in mortality in digestive, neoplasm and external causes of deaths. Conclusions: These findings are in line with international findings that show that socioeconomic differentials in digestive, neoplasm and external cause deaths are driving general socioeconomic differentials in all-cause mortality. External cause deaths may have been influenced by levels of economic activity, particularly in construction, during the economic boom among manual workers. Furthermore, deaths from digestive diseases during the 1990s and 2000s may well be the result of increases in liver disease associated with excessive alcohol consumption. [ABSTRACT FROM AUTHOR]

The Republic of Ireland was the first country in the eurozone to adopt a neoliberal infused ‘austerity budget’, and the ill-judged state bailing out of a number of reckless banks and associated financial institutions is costing billions of euros. In late 2010, as the crisis deepened, the International Monetary Fund and European Central Bank provided a ‘bailout’ package that resulted in further punitive public spending cuts and the eradication of national sovereignty. This paper maintains that the present crisis is more than a mere economic crisis, but can be viewed as a series of interlinked ‘conjunctures’. Tracing the connection between the historical and the contemporary, the first part of the discussion will explore six thematic components relating to the structuring of the Irish state. These are identified as: a ‘class rule state’; an ‘authoritarian state’; a ‘confining state’; a ‘censoring state’; a ‘discriminatory state’; and a ‘state within the state’. These six dimensions provide a contextual underpinning for the second part of the paper, which focuses on the crisis in child welfare and child protection services and dwells on the shambolic performance of the Health Service Executive (HSE), deficits in the ‘care’ system and the HSE failure to furnish reliable data on the deaths of children in contact with social workers. In Ireland there is a widespread belief that the country is moving towards a 'European model' of industrial relations. One claim is that the industrial relations system can be now described as social corporatist. Advocates of this view invariably point to the substance and the durability of the latest round of centralized wage agreements in support of this claim. This paper assesses the validity of the argument. Because discussions about social corporatism are so diffuse, the first part of the paper develops a framework that sheds light on what the concept means in economic and institutional terms. In the second part of the paper this framework is used to assess the functioning of the recent national pay deals. Internet usage is a salient developmental factor in adolescents' lives. Although relevant correlates of Internet use have been documented earlier, there is a lack of information on lower socioeconomic status groups. This is important, as these adolescents have increased risk of negative online experiences. The current survey aimed to explore Internet use and parental involvement amongst adolescents from areas of socio-economic disadvantage in 30 urban schools across five European countries. A total of 2594 students participated, of whom 90% were 14–16 years. Virtually all adolescents of socioeconomic disadvantage had Internet access, with 88.5% reporting spending more than two hours per day online, often on apps such as Instagram, Snapchat, and YouTube. Almost one-third of adolescents did not talk with their parents about their Internet use and almost two-thirds indicated that their parents were only a little or not interested in their Internet use. A consistent finding across countries was that girls more often talked with their parents about their Internet use and more often reported that their parents were interested in their Internet use than boys. The results suggest that parents have an important task in explicitly showing interest in their adolescents' Internet use, with special attention needed for boys. [ABSTRACT FROM AUTHOR]

Background: Profound clinical, conceptual and ideological differences between child and adult mental health service models contribute to transition-related discontinuity of care. Many of these may be related to psychiatry training. Methods: A systematic review on General Adult Psychiatry (GAP) and Child and Adult Psychiatry (CAP) training in Europe, with a particular focus on transition as a theme in GAP and CAP training. Results: Thirty-four full-papers, six abstracts and seven additional full text documents were identified. Important variations between countries were found across several domains including assessment of trainees, clinical and educational supervision, psychotherapy training and continuing medical education. Three models of training were identified: i) a generalist common training programme; ii) totally separate training programmes; iii) mixed types. Only two national training programs (UK and Ireland) were identified to have addressed transition as a topic, both involving CAP exclusively. Conclusion: Three models of training in GAP and CAP across Europe are identified, suggesting that the harmonization is not yet realised and a possible barrier to improving transitional care. Training in transition has only recently been considered. This paper presents the physical properties of recycled asphalt pavement (RAP) and its influence on the mechanical performance of a binder course asphalt pavement mix. A series of binder course mixes were designed containing varying percentages of RAP. A mix made from only virgin material was selected as the control mix for the investigation. The effect of introducing RAP into the binder course mix was evaluated through a series of laboratory tests including the Marshall test, the indirect tensile stiffness modulus test, the indirect tensile fatigue test and the water sensitivity test. A circular wheel track (CWT) was developed in order to study the dynamic effects of a rolling wheel traveling over an asphalt pavement. The CWT was commissioned within a temperature controlled room along with a customized data acquisition system. The system involves the testing of rectangular slabs and allows for the investigation of dynamic tensile strain. The laboratory tests have shown that the introduction of RAP to the binder course mix resulted in an improvement in all mechanical properties. In particular, it was found that the mix containing up to 30% RAP, displayed improved fatigue resistance relative to the control mix manufactured from virgin materials. [ABSTRACT FROM AUTHOR]

In this paper we seek to put Irish poverty rates in a comparative European context. We do so in a context whereby the Irish economic boom and EU enlargement have led to increasing reservations being expressed regarding rates deriving from the EU ‘at risk of poverty’ indicator. Our comparative analysis reports findings for both overall levels of poverty and variation by household reference person characteristics for this indicator and a consistent poverty measure for Ireland, the UK and five smaller European countries spanning a range of welfare regimes. Our findings demonstrate that the distinctiveness of Ireland’s situation lies not in the overall levels of poverty per se but in the very high penalties associated with being in a household where the household reference person is a lone parent or excluded from the labour market. Several attempts have been made to develop strategies for an effective control of workplace hazards. This paper will focus on the results of a European project called Solbase, which is a databank for solutions to occupational hazards and risks. The Safety Science Group of Delft University of Technology in collaboration with TNO Work and Organisation (formerly NIA-TNO) designed Solbase in a series of projects funded by the Dutch Ministry of Social Affairs and Employment and the European Commission. It consists of the design of and software for a databank with an intelligent navigation system allowing users two principal entry points, which correspond to two basic types of solutions. The first entry point is based on the production process, subdivided into the production principle and production function. This entry point provides the dissemination of solutions within and between branches of industry. The second entry point includes the hazard and its emission and transmission as an access point for more conventional occupational hygiene control measures. With the partners of the consortium, from Spain, Italy, Ireland, Germany, the UK and The Netherlands, 535 new and existing solutions throughout Europe and the world were gathered to test the software and the solutions during a field study. Despite the relatively small number of ‘test solutions’ used, 54% of the search actions in the field study resulted in a useful and suitable solution which the company could actually put into practice. The companies characterized the software as very user friendly. The reproducibility of the coding system for solutions, the classification tree, was satisfactory. Most coders chose the same keywords from the classification tree to describe a corresponding solution. Solbase is a good searching machine for workplace solutions. Especially, the classification of production processes is an inherent guarantee of an exchange of information across the borders of a specific company or branch of industry. [ABSTRACT FROM AUTHOR]

Background In Europe, more than 15 million people live with heart failure (HF). It imposes an enormous social, organizational and economic burden. As a reaction to impending impact on healthcare provision, different country-specific structures for HF-care have been established. The aim of this report is to provide an overview and compare the HF-care approaches of Germany, Ireland, the Netherlands and the UK, and to open the possibility of learning from each other's experience. Methods A mixed methods approach was implemented that included a literature analysis, interviews and questionnaires with HF-patients and caregivers, and expert interviews with representatives from healthcare, health service research and medical informatics. Results The models of HF-care in all countries analyzed are based on the European Society of Cardiology guidelines for diagnosis and treatment of HF. Even though the HF-models differed in design and implementation in practice, key challenges were similar: (i) unequal distribution of care between urban and rural areas, (ii) long waiting times, (iii) unequal access to and provision of healthcare services, (iv) information and communication gaps and (v) inadequate implementation and financing of digital applications. Conclusion Although promising approaches exist to structure and improve HF-care, across the four countries, implementation was reluctant to embrace novel methods. A lack of financial resources and insufficient digitalization making it difficult to adopt new concepts. Integration of HF-nurses seems to be an effective way of improving current models of HF-care. Digital solutions offer further opportunities to overcome communication and coordination gaps and to strengthen self-management skills. [ABSTRACT FROM AUTHOR]

The geographer's contribution to the study of religious phenomena is of special importance under circumstances in which scholars representing various disciplines tend to make generalizations based on a few, possibly nonrepresentative examples. The understanding of contemporary Western European pilgrimage is currently clouded by lack of generally available data on continental shrines and a tendency toward overreliance on Irish cases without adequate consideration of the highly atypical nature of the Irish pilgrimage tradition. This paper, grounded in data on 5,130 currently active Roman Catholic shrines in Western Europe, provides a cultural-historical explanation for the uniqueness of Irish pilgrimage and analyzes the major structural differences between Irish pilgrimage traditions and those found elsewhere in Europe. Aim: The aim of the study was to test the psychometric properties of the Facilitative Student–Patient Relationship (FSPR) Scale in clinical practicum in hospital settings within six European countries. Design: A multi‐country, cross‐sectional survey design was applied. Methods: A convenience sample of graduating nursing students (N = 1,796) completed the FSPR Scale. Psychometric testing was carried out through explorative factor analysis and confirmatory factor analysis. Internal consistency was assessed using Cronbach's alpha. Results: Both validity and reliability of the scale were confirmed. The explorative factor analysis yielded a two‐factor construct explaining 47.7% of the total variance, identifying two sub‐scales: caring relationship and learning relationship. Confirmatory factor analysis confirmed the two‐factor structure. The Cronbach alpha coefficients (0.8–0.9) indicated acceptable reliability of the scale. [ABSTRACT FROM AUTHOR]

Aim: Nurse leaders influence workplace culture; however, little is known about ethnic cultural influences on nurse leader development. This research aims to identify personal strategies promoting effective leadership by nurse leaders from European small island countries. Design: Descriptive qualitative study. Methods: In 2017, nineteen semi‐structured interviews with nurse leaders from England, Greece, Republic of Ireland and Malta explored leadership journeys, strategies employed to support their growth and development, and how cultural identity played a role. Transcripts were analysed using reflexive thematic analysis. Results: Four main themes and 12 subthemes captured the strategies and approaches of the nurse leaders: (1) Influences, (2) Communication, (3) Process and (4) Relationships. These findings reflect and validate the five transformational leadership practices of the Exemplary Leadership Model. While cultural island identity was discussed, there was a shared cultural identity within the role of "nurse leader" that spanned all islands. Septoria tritici blotch (STB; Zymoseptoria tritici) is a severe leaf disease on wheat in Northern Europe. Fungicide resistance in the populations of Z. tritici is increasingly challenging future control options. Twenty-five field trials were carried out in nine countries across Europe from 2019 to 2021 to investigate the efficacy of specific DMI and SDHI fungicides against STB. During the test period, two single DMIs (prothioconazole and mefentrifluconazole) and four different SDHIs (fluxapyroxad, bixafen, benzovindiflupyr and fluopyram) along with different co-formulations of DMIs and SDHIs applied at flag leaf emergence were tested. Across all countries, significant differences in azole performances against STB were seen; prothioconazole was outperformed in all countries by mefentrifluconazole. The effects also varied substantially between the SDHIs, with fluxapyroxad providing the best efficacy overall, while the performance of fluopyram was inferior to other SDHIs. In Ireland and the UK, the efficacy of SDHIs was significantly lower compared with results from continental Europe. This reduction in performances from both DMIs and SDHIs was reflected in yield responses and also linked to decreased sensitivity of Z. tritici isolates measured as EC50 values. A clear and significant gradient in EC50 values was seen across Europe. The lower sensitivity to SDHIs in Ireland and the UK was coincident with the prevalence of SDH-Calterations T79N, N86S, and sporadically of H152R. The isolates' sensitivity to SDHIs showed a clear cross-resistance between fluxapyroxad, bixafen, benzovindiflupyr and fluopyram, although the links with the latter were less apparent. Co-formulations of DMIs + SDHIs performed well in all trials conducted in 2021. Only minor differences were seen between fluxapyroxad + mefentrifluconazole and bixafen + fluopyram + prothioconazole; the combination of benzovindiflupyr + prothioconazole gave an inferior performance at some sites. Fenpicoxamid performed in line with the most effective co-formulations. This investigation shows a clear link between reduced field efficacy by solo SDHIs as a result of increasing problems with sensitivity shifting and the selection of several SDH-C mutations. Background: Oral health, coupled with rising awareness on the impact that limited dental care coverage has on oral health and general health and well-being, has received increased attention over the past few years. The purpose of the study was to compare the statutory coverage and access to dental care for adult services in 11 European countries using a vignette approach. Methods: We used three patient vignettes to highlight the differences of the dimensions of coverage and access to dental care (coverage, cost-sharing and accessibility). The three vignettes describe typical care pathways for patients with the most common oral health conditions (caries, periodontal disease, edentulism). The vignettes were completed by health services researchers knowledgeable on dental care, dentists, or teams consisting of a health systems expert working together with dental specialists. Results: Completed vignettes were received from 11 countries: Bulgaria, Estonia, France, Germany, Republic of Ireland (Ireland), Lithuania, the Netherlands, Poland, Portugal, Slovakia and Sweden. While emergency dental care, tooth extraction and restorative care for acute pain due to carious lesions are covered in most responding countries, root canal treatment, periodontal care and prosthetic restoration often require cost-sharing or are entirely excluded from the benefit basket. Regular dental visits are also limited to one visit per year in many countries. Beyond financial barriers due to out-of-pocket payments, patients may experience very different physical barriers to accessing dental care. The limited availability of contracted dentists (especially in rural areas) and the unequal distribution and lack of specialised dentists are major access barriers to public dental care. Conclusions: According to the results, statutory coverage of dental care varies across European countries, while access barriers are largely similar. Many dental services require substantial cost-sharing in most countries, leading to high out-of-pocket spending. Socioeconomic status is thus a main determinant for access to dental care, but other factors such as geography, age and comorbidities can also inhibit access and affect outcomes. Objectives To measure changes in prevalence and predictors of home smoking bans (HSBs) among smokers in four European countries after the implementation of national smoke-free legislation. Design Two waves of the International Tobacco Control Policy Evaluation Project Europe Surveys, which is a prospective panel study. Pre- and post-legislation data were used from Ireland, France, Germany and the Netherlands. Two pre-legislation waves from the UK were used as control. Participants 4634 respondents from the intervention countries and 1080 from the control country completed both baseline and follow-up and were included in the present analyses. Methods Multiple logistic regression models to identify predictors of having or of adopting a total HSB, and Generalised Estimating Equation models to compare patterns of change after implementation of smoke-free legislation to a control country without such legislation. Results Most smokers had at least partial smoking restrictions in their home, but the proportions varied significantly between countries. After implementation of national smoke-free legislation, the proportion of smokers with a total HSB increased significantly in all four countries. Among continuing smokers, the number of cigarettes smoked per day either remained stable or decreased significantly. Multiple logistic regression models indicated that having a young child in the household and supporting smoking bans in bars were important correlates of having a pre-legislation HSB. Prospective predictors of imposing a HSB between survey waves were planning to quit smoking, supporting a total smoking ban in bars and the birth of a child. Generalised Estimating Equation models indicated that the change in total HSB in the intervention countries was greater than that in the control country. Conclusions The findings suggest that smoke-free legislation does not lead to more smoking in smokers' homes. On the contrary, our findings demonstrate that smoke-free legislation may stimulate smokers to establish total smoking bans in their homes. [ABSTRACT FROM AUTHOR]

The author consider the transnational, or interconnected aspects, of Catholicism, Protestantism, and Christianity in Great Britain and Ireland with Europe for the period circa 1689 to 1800. The author considers how British civilization during the period considered "Christendom;" notions of Protestant unity among European Protestants; and how Irish and English Catholics found support among their European brethren to face anti-Catholic British laws. The article also considers ongoing hostilities between Europe and the Ottoman Empire throughout the period, as well as the impact of the French Revolution upon British and Irish religious thought.

Different aspects and techniques of time series analysis were used to investigate long-term data sets of atmospheric mercury in the Northern Hemisphere. Two perennial time series from different latitudes with different seasonal behaviour were chosen: first, Mace Head on the west coast of Ireland (53°20'N, 9°54'W), representing Northern Hemispherical background conditions in Europe with no indications for so-called atmospheric mercury depletion events (AMDEs); and second, Alert, Canada (82°28'N, 62°30'W), showing strong AMDEs during Arctic springtime. Possible trends were extracted and forecasts were performed by using seasonal decomposition procedures, autoregressive integrated moving average (ARIMA) methods and exponential smoothing (ES) techniques. The application of time series analysis to environmental data is shown in respect of atmospheric long-term data sets, and selected advantages are discussed. Both time series have not shown any statistically significant temporal trend in the gaseous elemental mercury (GEM) concentrations since 1995, representing low Northern Hemispherical background concentrations of 1.72±0.09 ng m-3 (Mace Head) and 1.55±0.18 ng m-3 (Alert), respectively. The annual forecasts for the GEM concentrations in 2001 at Alert by two different techniques were in good agreement with the measured concentrations for this year. [ABSTRACT FROM AUTHOR]

Achieving the potential that educational activities offer to prison regimes depends highly on adopting what the Council of Europe report Education in Prison (1990) described as "a wide concept of education." By offering a wide variety of "learning opportunities" to prisoners, we can attract a large number of them into education and also better address the needs of "the whole person." Of course, much depends also on the quality of what is provided, here the key factors seem to be using adult education methods and the best educational practices in the community for client groups similar to our prisoners. Background In responding to Covid-19, governments have tried to balance protecting health while minimizing gross domestic product (GDP) losses. We compare health-related net benefit (HRNB) and GDP losses associated with government responses of the UK, Ireland, Germany, Spain and Sweden from UK healthcare payer perspective. Methods We compared observed cases, hospitalizations and deaths under 'mitigation' to modelled events under 'no mitigation' to 20 July 2020. We thus calculated healthcare costs, quality adjusted life years (QALYs), and HRNB at £20,000/QALY saved by each country. On per population (i.e. per capita) basis, we compared HRNB with forecast reductions in 2020 GDP growth (overall or compared with Sweden as minimal mitigation country) and qualitatively and quantitatively described government responses. Results The UK saved 3.17 (0.32–3.65) million QALYs, £33 (8–38) billion healthcare costs and £1416 (220–1637) HRNB per capita at £20,000/QALY. Per capita, this is comparable to £1455 GDP loss using Sweden as comparator and offsets 46.1 (7.1–53.2)% of total £3075 GDP loss. Germany, Spain, and Sweden had greater HRNB per capita. These also offset a greater percentage of total GDP losses per capita. Ireland fared worst on both measures. Countries with more mask wearing, testing, and population susceptibility had better outcomes. Highest stringency responses did not appear to have best outcomes. Conclusions Our exploratory analysis indicates the benefit of government Covid-19 responses may outweigh their economic costs. The extent that HRNB offset economic losses appears to relate to population characteristics, testing levels, and mask wearing, rather than response stringency. [ABSTRACT FROM AUTHOR]

The article focuses on the fertility transition in Ireland during 1880-1911. The decline in Irish marital fertility was neither uniform nor universal. Between 1881 and 1911, there were increases in Counties Galway, Mayo, Roscommon and Donegal, while the decline in several other counties was small. Cohort parity analysis (CPA), a fertility measure devised by researcher Paul David and his research associates, provides a means of inferring the extent and timing of birth control within marriage from distributions of married women by number of children born. A key aspect of CPA is the extent to which married couples resort to contraception in order to "space" births. Ireland has played an important part in the development of CPA, because David and his colleagues believe that the rural Irish of 1911 provide a suitable model, not only for urban Ireland in 1911 and the United States around the turn of the century, but also for Western Europe, Canada, Australia, and New Zealand, from the mid-nineteenth through the early twentieth centuries.

In this paper we examine an example of a conservation conflict that is encountered in Ireland arising from the designation of Special Protection Areas (SPAs) for the Hen Harrier under the European Birds Directive (Directive, 2009/147/EC) and the consequent restrictions that are placed on forestry activities within these SPAs. We examine the causes of the Hen Harrier-forestry conflict; identify what stakeholders believe are the policy instruments and management strategies that may be useful in managing the conflict and finally identify plausible solutions that may be relevant to similar conflicts around multi-functional forests elsewhere in Europe and globally. Semi-structured interviews were conducted with key actors in this conflict. Drawing on Walker and Daniels' conflict triangle theory, a qualitative analysis of the transcripts of these interviews revealed that the conflict between Hen Harrier conservation and forestry in Ireland has a number of deep-rooted dimensions including those relating to the substance (e.g. restrictions on forest management activity in the SPAs), as well as procedural (e.g. lack of stakeholder engagement) and relationship dimensions (e.g. lack of trust). The polarisation of views in this conflict testifies to how entrenched stakeholders can become through lack of communication and trust. The policy instruments that stakeholders identified as having potential to address the conflict include the introduction of incentives/compensation scheme; changes to restrictions; more data and research on Hen Harrier bird surveys; implementation of landscape management models; and better communication and stakeholder engagement. The study highlights that conservation conflicts persist due to the multi-functional nature of forests and also due to repeated mistakes in terms of the lack of engagement with local stakeholders. Increasing the involvement of local actors has important substantive and instrumental benefits including improving the quality of decisions, as well as creating a greater chance of policies being better socially and politically acceptable. Background Tobacco-control policies have been suggested to reduce smoking among adolescents. However, there is limited evidence on the real-world costs of implementation in different settings. In this study, we aimed at estimating the costs of school smoking bans, school prevention programmes and non-school bans (smoking bans in non-educational public settings, bans on sales to minors and bans on point-of-sale advertising), implemented in Finland, Ireland, the Netherlands, Belgium, Germany, Italy and Portugal, for 2016. Methods We retrospectively collected costs related to the inspection, monitoring and sanctioning activities related to bans and educational activities related to smoking prevention programmes. We used an 'ingredients-based' approach, identifying each resource used, quantity and unit value for one full year, under the state perspective. Costs were measured at national, regional, local and school-level and were informed by data on how these activities were performed in reality. Results Purchasing power parities adjusted-costs varied between €0.02 and €0.74 (average €0.24) per person (pp) for bans implemented outside schools. Mean costs of school smoking bans ranged from €3.31 to €34.76 (average €20.60), and mean costs of school educational programmes from €0.75 to €4.65 (average €2.92). Conclusions It is feasible to estimate costs of health policies as implemented in different settings. Costs of the tobacco control policies evaluated here depend mainly on the number of person-hours allocated to their implementation, and on the scale of intervention. An updated review by Philippe Grandjean, published in Environmental Health on December 19, 2019, of developmental fluoride neurotoxicity concluded that recent epidemiological results support the notion that elevated fluoride intake during early development can result in IQ deficits that may be considerable. He noted that the recognition of neurotoxic risks is necessary when determining the safety of fluoridecontaminated drinking water and fluoride uses for preventive dentistry purposes. In contrast, another review by Guth et al., published online in Archives of Toxicology on May 8, 2020, concluded that fluoride was not a human developmental neurotoxicant at the current exposure levels in Europe where there is a mean fluoride intake in adults of 5-14 µg/kg bw/day in areas with a low drinking water fluoride level and of 30-40 µg/kg bw/day in areas with a high drinking water fluoride level. This editorial explores the reasons for the different conclusions being reached and finds that Guth et al. reached their conclusion by not considering the well-designed 2017 prospective cohort study by Basash et al. in sufficient detail to recognize that intrauterine exposure to fluoride is more important than exposure at 6-12 years in causing developmental fluoride neurotoxicity and by not considering the calculations made of the threshold for the drinking water fluoride level for the development of fluoride-induced neurotoxicity of approximately 0.2 mg/L, calculated by Hirzy et al. in 2016 and Grandjean in 2019. A drinking water fluoride level of 0.2 mg/L would result in a fluoride intake of 0.248 mg day or 3.5 µg/kg bw/day in a 70 kg adult with a water intake of 1.24 L/day which is less than the mean adult European intakes of 5-14 and 30-40 µg/kg bw/day in areas with low and high drinking water fluoride levels, respectively. The conclusions that can be drawn from the studies by Basash et al, Hirzy et al., and Grandjean, together with the prospective studies by Valdez Jiménez et al. and Green et al., are that intrauterine exposure to fluoride is more important than exposure during childhood at age 6-12 years in causing developmental fluoride neurotoxicity and that the threshold level for drinking water fluoride for the development of this effect is approximately 0.2 mg/L. A further 2019 study by Till et al. indicates that the developing brain may also be adversely affected by exposure to drinking water with approximately 0.6 mg/L of fluoride during infancy. Accordingly, exposure of pregnant women and infants to community water fluoridation, with water with 0.7 mg/L of fluoride, the level recommended in the USA for community water fluoridation, or with 0.6-0.8 mg/L the level recommended in the Republic of Ireland, or to the current exposure levels in Europe, where there is a mean fluoride intake in adults of 5-14 µg/kg bw/day in areas with a low drinking water fluoride level and of 30-40 µg/kg bw/day in areas with a high drinking water fluoride level, is unsafe, will lead to a fluoride intake that exceeds the threshold for developmental fluoride neurotoxicity of 0.2 mg/L or 3.5 µg/kg bw/day, and will result in the occurrence of developmental fluoride neurotoxicity. Study Question: Are progesterone vaginal pessaries 400 mg twice a day (bid) non-inferior to progesterone vaginal gel (90 mg) once a day (od) in the primary endpoint of clinical pregnancy rate after 38 days of luteal phase support in women undergoing in vitro fertilisation (IVF)?Summary Answer: Non-inferiority of progesterone vaginal pessaries 400 mg bid to progesterone 8% vaginal gel (90 mg od) was shown for clinical pregnancy rate after 38 days of luteal phase support.What Is Known Already: To maximise successful embryo transfer after IVF, additionally administered progesterone is used for proper endometrium transformation in the luteal phase. Vaginally administered progesterone results in adequate secretory transformation of the endometrium.Study Design, Size, Duration: This multicentre, multinational, open, randomised, two-parallel group, non-inferiority Phase 3 clinical trial was carried out at 17 study sites in five European countries (Belgium, Bulgaria, Czech Republic, Hungary and Serbia) between October 2013 and August 2014. An interactive web response system (IWRS) was implemented for treatment allocation at the sites. Power analysis, based on the assumptions of a non-inferiority margin of -9%, a significance level of α 2.5% (one-sided), power 90%, at a reference pregnancy rate for the progesterone vaginal gel group of 30%, as well as applying a dropout rate of 10%, yielded a total number of 766 patients to be randomised.Participants/materials, Setting, Methods: Women aged between 18 and 40 years with a clinical indication for IVF/intracytoplasmic sperm injection (ICSI) and embryo transfer were eligible to participate. The clinical pregnancy rate was assessed by fetal heart movement measured by transvaginal ultrasound at day 38 (D38) (primary endpoint) and D70. Also assessed were biochemical pregnancy rate (assessed by serum β-hCG ≥25 IU/L), clinical implantation rates at D38, patient evaluation of vaginal bleeding and discharge (assessed by diary) and adverse event (AE) incidence, severity and relationship to study medication.Main Results and the Role Of Chance: A total of 769 female patients were randomised to progesterone 400 mg vaginal pessaries bid (n = 385, 50.1%) or progesterone 90 mg vaginal gel od (n = 384, 49.9%). Patients receiving progesterone vaginal pessaries and progesterone vaginal gel were comparable in demographics, baseline characteristics and number of retrieved oocytes. In the full analysis set (FAS; n = 369 progesterone vaginal pessaries and n = 368 progesterone vaginal gel), clinical pregnancy rates on D38 were 38.3% for progesterone vaginal pessaries and 39.9% for progesterone vaginal gel. In the per protocol analysis set (PP; n = 357 progesterone vaginal pessaries and n = 356 progesterone vaginal gel), clinical pregnancy rates on D38 were 38.1% for progesterone vaginal pessaries and 40.4% for progesterone vaginal gel. For the differences in pregnancy rates between the progesterone vaginal pessaries group and the progesterone vaginal gel, the lower limit of the 97.5% CI was -8.6 and -9.5% for the FAS and PP datasets, respectively. The original prespecified non-inferiority margin of -9% was thus met in the FAS dataset but was marginally below this in the PP dataset. However, the pregnancy rate of the comparator was higher than the anticipated rate of 30%, and a predetermined logistic regression model including treatment group, country and age group effects without interaction terms showed non-inferiority of progesterone vaginal pessaries to progesterone vaginal gel for both the FAS and PP populations, in that the lower limits of the 95% CIs were above 0.7 for both analyses. As a result of this, the relevant authorities accepted to widen the acceptable non-inferiority margin to -10%, and as such both the FAS and PP populations succeeded in showing non-inferiority. Biochemical pregnancy and clinical implantation rates were comparable for both treatments. Both treatment groups showed similar high compliance throughout the study, and the safety profiles were also comparable between the groups. Since their re-establishment in the early decades of the nineteenth century the Jesuits have successfully maintained a position at the pinnacle of Catholic elite education. In this article I propose to discuss Irish education in the context of global trends in cosmopolitan and elite forms of education. All across Europe we find the Jesuits competing for regional elites and sub elites in this period, and the Irish Jesuits are part of this transnational pattern. I will then focus on the two most important nineteenth century foundations -Tullabeg (1818-86) and Clongowes (1814-)- as the exemplary «elite» Jesuit boarding schools in Ireland. I will then briefly discuss two less socially ascendant but nevertheless important day schools, Belvedere College (1841-) and Gonzaga (1899- ), both in Dublin. The educational product was intentionally politically muted, informed by a desire for Catholic advancement in all aspects of life, including imperial service, religious leadership, gaining a foothold in the prestigious professions, and -where possible- advocating for general Catholic advancement. As with Jesuit education elsewhere in this period it was an explicitly elitist project at the beginning of the nineteenth century, with greater market segmentation evident later in the century with the advent of the prestigious urban day-schools. The relationship between lay theories of gender and attitudes to abortion policy has received minimal empirical attention. An ongoing theoretical debate in the psychological essentialism literature queries whether biological attributions causally influence social attitudes or primarily function to justify existing attitudinal commitments. The current research used the context of a national referendum on abortion in Ireland to investigate whether endorsement of certain gender theories is contingent on their rhetorical construction as supporting particular attitudes to abortion. Two experimental studies were conducted online in the three weeks preceding the Irish abortion referendum. The studies tested whether participants would adapt their causal gender beliefs after reading that biological (Study 1; N = 348) or social (Study 2; N = 241) accounts of gender supported or conflicted with their intended vote in the referendum. Both studies showed the opposite effect: causal gender theories presented as conflicting with participants’ voting intentions subsequently showed elevated support, relative to theories that purportedly aligned with participants’ voting intentions. While results confirm that lay theories of gender are mutable, the direction of effects does not support the proposition that gender theories are selectively endorsed to support existing socio-political attitudes to abortion. Circulation in the mid-troposphere in moderate and high latitudes of the Northern Hemisphere can be characterised by the Vangenheim-Girs (VG) circulation macro-forms. The aim of the research was to analyse the VG macro-forms as a factor determining the general characteristics of the atmospheric circulation in mid-troposphere in the Euro-Atlantic region and low-troposphere airflow characteristics in the profile (zonal belt) crossing Central Europe from Ireland to Kazan in Russia (5°30'W–44°00'E). Alongside the VG macro-form calendar, ERA-INTERIM data were used. The utilised meteorological variables comprised 500hPa geopotential height, SLP and air temperature at 995 sigma level. The temporal scope of the research was 35 years (1981–2015) and the resolution was 24h (12.00 UTC). The circulation in the low-troposphere was characterised by the geostrophic wind vector characteristics directly resulting from SLP and air temperature fields. Subsequently, derived indices (e.g. wind direction stability) were used. The presented results indicate that the variability of anemological conditions at SLP in the area of 50–60°N over Europe is in direct connection with the mid-troposphere circulation features. The differences are statistically significant across nearly the entire research area. This includes the reversal of the dominant air flow direction in some areas. The greatest variability in geostrophic wind characteristics due to W, E and C VG macro-forms is revealed in the central and eastern part of the 50–60°N zone – between the southern Baltic Sea and the western border of Russia. Background Decreasing gender differences in mental health are found largely in countries in which the roles of men and women have improved in terms of opportunities for employment, education, child care and other indicators of increasing gender equality. In this study, we examine how European welfare regimes influence this association between mental health and the social roles that men and women occupy. Methods The EU-World Mental Health data are used, which covers the general population in 10 European countries (n = 37 289); Countries were grouped into four welfare regions: Liberal regime (Northern Ireland), Bismarckian regime (Belgium, Germany, the Netherlands and France), Southern regime (Spain, Italy, Portugal) and Central-Eastern regime (Romania and Bulgaria). The lifetime prevalence of mood, anxiety and alcohol disorders was determined by using the Composite International Diagnostic Interview 3.0. Overall prevalence rates along with odds ratios by means of bivariate logistic regression models are calculated to compare the presence of common mental disorders in women versus men per welfare regime. Results Overall prevalence of common mental disorders is highest in the Liberal regime and lowest in the Central/Eastern regime. The gender gap in mental disorders is largest in the Southern regime and smallest in the Liberal regime. Marital status and certain employment positions help to explain variation in mental disorders across and within welfare regimes. Conclusion Most prominent pathways linking gender to mental ill-health being are related to marital status and certain employment positions. Background: Previous studies examining physical and mental non-communicable diseases (NCDs) in Europe have so far largely either focused on limited numbers of countries or on fairly limited ranges of NCDs, with mental health in particular often being ignored. This article has three aims: (i) To provide a recent, comprehensive overview of a broad range of NCDs across a range of countries in all European regions; (ii) To give an overview of measures of physical and mental health in the new special rotating module in the European Social Survey (ESS); and (iii) To offer the first comprehensive comparison of estimates on physical and mental NCDs across European countries in this new promising data source. Methods: We use data from the 7th wave of the ESS. Results are presented separately for men and women. All results were age-standardized by weighting up or down the unstandardized (crude) prevalence rates for 5-year age groups in each country to a common standard. We present pooled estimates for the combined cross-national sample as well as country-specific results. Results: Overall, 74.1% of men and 79.7% of women reported at least one physical NCD. Across the 21 countries were observed that these percentages varied between 45.2% (for men in Hungary) and 91.6% (women in Finland). Serious depressive symptoms were reported by 10.2% of men and 18.8% of women, with percentages ranging between 6.2% (men in Ireland) and 30.9% (women in Portugal). Conclusions: A substantial share of Europeans experience the burden of NCDs, and the extent to which people report these conditions varies across countries. However, all physical and mental NCDs in this rotating module are reported by considerable percentages in each of the 21 country samples, which emphasizes that these conditions are not marginal phenomena but public health concerns. In recent years, temperate bamboo species have been introduced in Europe not only as an ornamental plant, but also as a new biomass crop. To measure adaptation stress of bamboo to the climate of Western Europe, chlorophyll fluorescence was measured on a diurnal and seasonal basis in Ballyboughal, Co. Dublin, Ireland. Measurements were attained on the leaves of each node of Phyllostachys humilis. The most frequently used parameter in chlorophyll fluorescence is the photosynthetic efficiency (Fv/Fm). A seasonal dip - as well as a larger variation - of Fv/Fm in spring compared to the rest of the year was observed. Over the year, the upper leaves of the plant perform better than the bottom leaves. These findings were linked to environmental factors such as light intensity, air temperature and precipitation, as increased light intensities, decreasing air temperatures and their interactions, also with precipitation levels have an effect on the photosynthetic efficiency (Fv/Fm) in these plants. Background: Little is known about the differential impact of comprehensive and partial smoke-free legislation on smoking cessation. This study aimed to examine the impact of comprehensive smoke-free workplace legislation in Ireland and England, and partial hospitality industry legislation in the Netherlands on quit attempts and quit success. Methods: Nationally representative samples of 2,219 adult smokers were interviewed in three countries as part of the International Tobacco Control (ITC) Europe Surveys. Quit attempts and quit success were compared between period 1 (in which smoke-free legislation was implemented in Ireland and the Netherlands) and period 2 (in which smoke-free legislation was implemented in England). Results: In Ireland, significantly more smokers attempted to quit smoking in period 1 (50.5%) than in period 2 (36.4%) (p < 0.001). Percentages of quit attempts and quit success did not change significantly between periods in the Netherlands. English smokers were significantly more often successful in their quit attempt in period 2 (47.3%) than in period 1 (26.4%) (p = 0.011). In the first period there were more quit attempts in Ireland than in England and fewer in the Netherlands than in Ireland. Fewer smokers quitted successfully in the second period in both Ireland and the Netherlands than in England. Conclusion: The comprehensive smoke-free legislation in Ireland and England may have had positive effects on quit attempts and quit success respectively. The partial smoke-free legislation in the Netherlands probably had no effect on quit attempts or quit success.
