This paper analyzes the impact of population aging on health care expenditures in Korea. Examination of the age-expenditure profile reveals that health care resources are allocated more for the older cohort of population over time, suggesting significant growth of health care expenditures due to population aging. We contend, however, that population aging is considered as a parameter rather than an independent variable to explain rising health care expenditures. This paper shows that population aging is not found to be a significant determinant of health care expenditures according to the econometric analysis using OECD health data and time-series data for Korea. Using the components decomposition method, which measures the contribution of each component of health care expenditure, we estimate that population aging contributes only less than 10 percent.

This paper evaluates the current state of work-related musculoskeletal disorders of Korean farmers. Korean farmers are exposed to a multitude of ergonomic risk factors, and have high rates of musculoskeletal disorders such as backpain and knee osteoarthritis, while hip osteoarthritis is relatively low. As of 2010, the average age of Korean farmers is 62.3, and the average period of their work in agriculture is 33.3 years. Therefore, it is difficult to distinguish the influence of natural aging and farm work on the development of musculoskeletal disorders. It is necessary to conduct research on a correlation between specific diagnosis such as rotator cuff syndrome, and farm work in general because farmers complain of a great deal of pain. Abstract: This paper describes analyses and application of OPDE (OECD Piping Data Failure Exchange) database and NPFD (Nuclear Piping Failure Database) to some safety related issues for Korea nuclear power plants. Safety issues considered in this study include the piping rupture frequency assessment, the screening criteria of the leak before break application, and the augmented in-service inspection and the risk-informed ISI (RI-ISI). As a result of this study: (1) Piping rupture frequencies of the very small LOCA, feedwater line break events, and flood events were evaluated and used as initiating event frequencies for the PSA of Korean PWR plants; (2) Based on the review of OPDE DB and LBB screening criteria, LBB application to the pressurizer surge line, safety injection system, and shutdown cooling system were approved under the conditions that detailed thermal stress analyses should be performed to the systems. (3) For RI-ISI application, major degradation mechanisms in the reactor coolant system were analyzed based on the analysis of the OPDE and NPFD and augmented inspections were enforced to the pipe segments susceptible to degradation mechanism such as thermal stratification/fatigue. Background Korea is one of the fastest aging countries and is expected to become a super-aged society within 12 years. The Korean Urban Rural Elderly (KURE) study was developed to evaluate the epidemiological characteristics and establish the prevention and management of major disorders of the elderly in Korea. Methods/Design The KURE study is a community-based prospective cohort study on health, aging, and common geriatric disorders of Korean elderly persons aged at least 65 years. To construct a cohort reflecting both urban and rural areas, we selected 2 representative communities in the country. To establish multidisciplinary approaches to geriatric health, this study was performed by researchers in the divisions of geriatrics, preventive medicine, endocrinology, and sociology. The baseline examinations began in 2012; the study will follow more than 4,000 elderly Koreans over 10 years. The first and second follow-up health examinations will be performed every 4 years. Every 2 years after each health examination, inter-assessment interview will be conducted to improve participant retention. Discussion The KURE study will provide longitudinal epidemiologic data on health, aging, and common geriatric disorders of the elderly in Korea. This is a comprehensive, multidisciplinary study of the elderly with respect to biological, physical, socio-economic, and environmental factors. Background: Frailty is related to adverse outcomes in the elderly. However, current status and clinical significance of frailty have not been evaluated for the Korean elderly population. We aimed to investigate the usefulness of established frailty criteria for community-dwelling Korean elderly. We also tried to develop and validate a new frailty index based on a multidimensional model. Methods: We studied 693 participants of the Korean Longitudinal Study on Health and Aging (KLoSHA). We developed a new frailty index (KLoSHA Frailty Index, KFI) and compared predictability of it with the established frailty indexes from the Cardiovascular Health Study (CHS) and Study of Osteoporotic Fracture (SOF). Mortality, hospitalization, and functional decline were evaluated. Results: The prevalence of frailty was 9.2% (SOF index), 13.2% (CHS index), and 15.6% (KFI). The criteria from CHS and KFI correlated with each other, but SOF did not correlate with KFI. During the follow-up period (5.6±0.9 years), 97 participants (14.0%) died. Frailty defined by KFI predicted mortality better than CHS index (c-index: 0.713 and 0.596, respectively; p<0.001, better for KFI). In contrast, frailty by SOF index was not related to mortality. The KFI showed better predictability for following functional decline than CHS index (area under the receiver-operating characteristic curve was 0.937 for KFI and 0.704 for CHS index, p = 0.001). However, the SOF index could not predict subsequent functional decline. Frailty by the KFI (OR = 2.13, 95% CI 1.04–4.35) and CHS index (OR = 2.24, 95% CI 1.05–4.76) were associated with hospitalization. In contrast, frailty by the SOF index was not correlated with hospitalization (OR = 1.43, 95% CI 0.68–3.01). Conclusion: Prevalence of frailty was higher in Korea compared to previous studies in other countries. A novel frailty index (KFI), which includes domains of comprehensive geriatric assessment, is a valid criterion for the evaluation and prediction of frailty in the Korean elderly population. Background: Prospective investigation of obesity and renal function decline in Asia is sparse. We examined the associations of body mass index (BMI) and waist circumference (WC) with renal function decline in a prospective study of Korean population. Methods: A total of 454 participants who had baseline estimated glomerular filtration rate (eGFR) levels of more than 60 mL/min/1.73 m2 in Hallym Aging Study (HAS) were included and followed for 6 years. Renal function decline was defined as follows: (1) an eGFR decline ≥3 mL/min/1.73 m2/year (n = 82 cases); (2) an eGFR decrease of 20% or greater (n = 87 cases) at follow-up; (3) an eGFR decrease of 20% greater at follow-up or eGFR decline ≥3 mL/min/1.73 m2/year (n = 91 cases); and (4) an eGFR <60 mL/min/1.73 m2 at follow-up (n = 54 cases). eGFR was determined based on the Modification of Diet in Renal Disease (MDRD) Study equation. Multivariate logistic regression model was used to determine the association between obesity and renal function decline. Results: We found that central obesity was associated with faster renal function decline. Comparing WC of >95 cm in men or >90 cm in women with ≤90 cm in men or ≤85 cm in women, ORs (95% CIs) ranged from 2.31 (1.14–4.69) to 2.78 (1.19–6.50) for the 4 definitions of renal function decline (all p-values for trend <0.05). Waist-to-hip ratio (WHR) and waist-to-height ratio (WHtR) also was associated with renal function decline. There was no significant association of BMI with renal function decline. Conclusions: Central obesity, but not BMI, is associated with faster renal function decline in Korean population. The age-related increase in body fat and decrease in muscle mass are associated with increased morbidity in elderly populations. Pulmonary function also decreases with age, but no study has investigated whether regional body composition is associated with pulmonary function in an older population. The Korean Longitudinal Study on Health and Aging is a community-based cohort study of people aged >65 years selected by random stratified sampling. Anthropometrics, biochemical factors, and lung function by spirometry were evaluated in 439 men (mean age of 75.9 ± 8.6 years) and 561 women (mean age of 76.0 ± 8.8 years). Dual-energy X-ray absorptiometry (DXA) was performed to assess the whole and regional body composition. Computed tomography (CT) was also used to measure fat or muscle distribution at the abdominal and mid-thigh levels. Although pulmonary function and muscle mass were inversely related to age, fat mass was not. After adjusting for age, height, BMI, smoking and exercise status, and high sensitivity C-reactive protein (hsCRP), fat mass in trunk or central area was inversely associated with lung function in both sexes (P < 0.01). Men with more muscle in trunk and mid-thigh level had better lung function (P < 0.01). The results of this community-based study show that regional body composition is significantly associated with lung function. Augmentation of muscle in the trunk and low extremity in men, and reduction of fat in the trunk and upper body in men and women may be helpful in maintaining lung function in the elderly population. Background: Musculoskeletal diseases are becoming increasingly important due to population aging. However, studies on the economic burden of musculoskeletal disease in Korea are scarce. Therefore, we conducted a population-based study to measure the economic burden of musculoskeletal disease in Korea using nationally representative data. Methods: This study used a variety of data sources such as national health insurance statistics, the Korea Health Panel study and cause of death reports generated by the Korea National Statistical Office to estimate the economic burden of musculoskeletal disease. The total cost of musculoskeletal disease was estimated as the sum of direct medical care costs, direct non-medical care costs, and indirect costs. Direct medical care costs are composed of the costs paid by the insurer and patients, over the counter drugs costs, and other costs such as medical equipment costs. Direct non-medical costs are composed of transportation and caregiver costs. Indirect costs are the sum of the costs associated with premature death and the costs due to productivity loss. Age, sex, and disease specific costs were estimated. Results: Among the musculoskeletal diseases, the highest costs are associated with other dorsopathies, followed by disc disorder and arthrosis. The direct medical and direct non-medical costs of all musculoskeletal diseases were $4.18 billion and $338 million in 2008, respectively. Among the indirect costs, those due to productivity loss were $2.28 billion and costs due to premature death were $79 million. The proportions of the total costs incurred by male and female patients were 33.8% and 66.2%, respectively, and the cost due to the female adult aged 20-64 years old was highest. The total economic cost of musculoskeletal disease was $6.89 billion, which represents 0.7% of the Korean gross domestic product. Conclusions: The economic burden of musculoskeletal disease in Korea is substantial. As the Korean population continues to age, the economic burden of musculoskeletal disease will continue to increase.
