We will use micro-computed tomography to scan 31 sets of the adult lower cervical vertebrae (155 vertebrae) to observe the morphological characteristics and direction of trabeculae in the lower cervical vertebrae by outlining and reconstructing the regions of interest and to calculate the variation laws of the microstructure in the regions of interest to reveal their structural characteristics and weak areas. As a result, the images showed that the trabeculae in the lower cervical pedicle near the medial and lateral cortices were relatively dense, and their bone plates were lamellar. There were cavities between the superior and inferior articular processes where the ossification centers had not been absorbed after ossified. The lamellar trabeculae in the vertebral plates near the cortical bones were only 1–2 layers, extended and transformed into rod-shaped trabeculae in a radial shape toward the medullary space. The lamellar trabeculae of the vertebral plate extend over the spinous process near the cortical bone. The statistical results of the trabeculae's morphological parameters showed significant differences in bone volume fraction values among the four parts (P < 0.05). There were substantial differences in BS/BV, except for no differences between the pedicle and the vertebral plate (P < 0.05). There was a significant difference in trabecular pattern factor values between the articular process, the spinous process, and the vertebral plate (P < 0.05) and a significant difference between the pedicle, the spinous process, and the vertebral plate (P < 0.05). There were no significant differences in trabecular bone thickness and trabecular space values among the four parts (P < 0.05). The anatomical microstructural perspective confirms that the optimal choice is internal fixation via the pedicle. If using pedicle screws, the nail tract needs to be placed into the spinous process to increase its holding power and resistance to extraction. [ABSTRACT FROM AUTHOR]

Background. Spinal injury in children usually occurs in the cervical spine region. Anterior fixation of the lower cervical spine has been applied in treating pediatric cervical spine injury and disease due to its stable and firm mechanical properties. This study performed finite element analysis and comparison of four different anterior cervical internal fixation systems for children to explore more standard methods of anterior cervical internal fixation in children and seek more effective and safe treatment for children's cervical spine diseases. Methods. A finite element model of 6-year-old children with lower cervical spine C4/5 discectomy was established, and the self-designed lower cervical spine anterior locking internal fixation system ACBLP and the children's anterior cervical internal fixation system ACOP, ACVLP, and ACSLP plate screws were fixed and loaded on the model. 27.42 N·m torque load was applied to each internal fixation model under six working conditions of anteflexion, backward flexion, left flexion, right flexion, left rotation, and right rotation, to simulate the movement of the cervical spine. The activity and stress distribution cloud diagram of each finite element model was obtained to explore the optimal method of anterior cervical fixation in children. Results. In the four internal fixation models of ACOP, ACVLP, ACSLP, and ACBLP, the mobility of the C4/5 segment showed a decreasing relationship, and the mobility of adjacent segments increased significantly. In the Mises stress cloud diagram of the cervical spine of the four models, the vertebral body and accessories of the ACBLP model born the least stress, followed by ACSLP. The steel plate and screws in the ACVLP internal fixation model were the most stressed. The stress of the internal fixation system (plate/screw) in all models increased in the order of ACBLP, ACSLP, ACVLP, and ACOP. Conclusions. ACBLP internal fixation system had obvious advantages in anterior internal fixation of the lower cervical spine in children, C4/5 had the smallest degree of movement, relative displacement was minimal, and the stress on the centrum and pedicle was the least, while the stress on the plate screw was relatively the smallest. Purpose: Using micro-CT and finite element analysis to establish regional variation microarchitectures and correlation with mechanical properties of cervical articular facet trabecular bone to predict cervical spine security and material properties. Methods: A total of 144 cervical articular processes (each articular was separate to four region of interest (ROI), superior-anterior (SA), superior-posterior (SP), inferior-anterior (IA), and inferior-posterior (IP) regions) specimens with a volume of 5 × 5 × 5 mm3 were scanned by micro-CT, and allowable stress and other mechanical properties parameters in each region were calculated after mechanical testing, then the effectiveness was verified of finite element models by ABAQUS software. Results: Maximum and minimum values of C2–C7 articular processes and regions are C5 and C7 level, SA and SP regions for bone volume fraction (BV/TV) and trabecular thickness (Tb.Th), whose variation tendency is similar to the Young's modulus, allowable stress, BMD, maximum force and strain. Between Young's modulus and all microstructure parameters, especially between BV/TV, bone mineral density (BMD) and Tb.Th, had higher linear regression coefficients R2 = 0.5676, 0.6382, 0.3535, respectively. BMD and yield strength, BV/TV, and allowable stress also had better regression coefficients, R2 = 0.5227, 0.5259, 0.5426, respectively. Conclusions: The contribution of the microstructure and mechanical properties of the C2–C7 cervical spine to the movement of the cervical spine is different and has a good correlation and the effectiveness of the finite element model is also verified that we can correctly calculate the microstructure and mechanical properties of the cervical articular process to evaluate the stability and injury risk of cervical vertebrae by the established model.
