Abstract

Elongation of Very Long chain fatty acids-like 4 (ELOVL4) is a fatty acid elongase responsible for the biosynthesis of very long chain (VLC; ≥ C26) fatty acids in the retina, brain, skin, Meibomian gland, and testes. Heterozygous inheritance of mutant ELOVL4 causes juvenile macular degeneration in autosomal dominant Stargardt-like macular dystrophy (STGD3). Retinal photoreceptors are enriched with VLC polyunsaturated fatty acids (VLC-PUFAs), which have been shown by our group and others to be necessary for the survival of rod photoreceptors. Our group performed a series of studies using mice conditionally depleted of retinal Elovl4 (KO) aimed at understanding the role of VLC-PUFAs in long-term retinal health and function, focusing on the role of these fatty acids in mediating synaptic function between the photoreceptors and the rest of the neural retina. The absence of VLC-PUFA from the retina of KO mice resulted in a marked decrease in retinal b-wave responses of the electroretinogram as well as a decrease in the amplitude of the oscillatory potentials mediated by the neural retina. Although there were no measureable differences between KO and wild type (WT) mice in either pre-synaptic rod calcium channel function or post-synaptic bipolar cell glutamate receptor responses, ultrastructural analysis revealed a marked decrease in the diameter of synaptic vesicles in rod terminals. Recent quantification suggests that this decrease in synaptic vesicle size due to the absence of VLC-PUFAs in KO mice, and the consequent decrease in glutamate content, could account for the decrease in b-wave response amplitudes that were previously measured in these animals.

Abstract

This report begins with a general appraisal of school desegregation in the Southern region and concludes with a discussion by four black high school students who explain in specific terms what their first year of desegregated schooling has meant to them. Interposed are four community studies and an assessment of the continuing segregated academy movement, including court action and other crucial issues arising from the experiences of the selected communities. While many problems remain, the six individual contributors to the study agree that the important and hopeful factor was that the students were there and were involved in determining their own destiny. Abstract

Point-of-use, powdered activated carbon (PAC) filters reduce the bacterial content of groundwater supplies only when the tap is left open for sometime. PAC filters decrease the heterotrophic-plate-count (HPC) and the quantity of coliforms in water, though they can be detected in some samples. Water kept stagnant overnight or longer shows an increase in the level of HPC bacteria despite the use of filters. The tap should be left open for a few minutes before using to take the complete advantage of the PAC treatment.

The accountability inherent in the social interaction between a patient and a health care provider affects patients’ motivation to adhere to treatment. Nevertheless, accountability is a concept not found in adherence models, and is rarely employed in typical medical practice, where patients may be prescribed a treatment and not seen again until a return appointment 8–12 weeks later. The purpose of this paper is to describe the concept of accountability and to incorporate accountability into an existing adherence model framework. Based on the Self-Determination Theory, accountability can be considered in a spectrum from a paternalistic use of duress to comply with instructions (controlled accountability) to patients’ autonomous internal desire to please a respected health care provider (autonomous accountability), the latter expected to best enhance long-term adherence behavior. Existing adherence models were reviewed with a panel of experts, and an accountability construct was incorporated into a modified version of Bandura’s Social Cognitive Theory. Defining accountability and incorporating it into an adherence model will facilitate the development of measures of accountability as well as the testing and refinement of adherence interventions that make use of this critical determinant of human behavior. Abstract

Cell-mediated immune responses are deficient in individuals infected with the human immunodeficiency virus (HIV), and the deficiency is progressive. Two clinical markers of the progression of HIV disease, antibody to the p24 core protein of HIV and neopterin concentrations in the blood, were studied in terms of their relationship to the outcome of HIV disease. Using data from a large population-based survey, 238 HIV-seropositive homosexual men were studied. Antibody levels to p24 core protein were determined, levels of CD4 cells were measured, and the concentration of neopterin in the blood was determined. Both antibody to p24 and the concentration of neopterin in the blood were found to be predictive of the development of AIDS as long as 54 months prior to its development. Specifically, low antibody levels and high levels of neopterin were independently and strongly related to poor outcome. This is consistent with the hypothesis that the intensity of the initial immune response to HIV (as measured by antibody levels) and the degree of immune response over time (measured by concentration of neopterin in the blood) are both important factors in determining disease outcome, independent of each other. About 60 percent of those with low antibody levels and high levels of neopterin developed AIDS over the 54 months, as compared with only 34 percent of patients who had either low antibody levels or high concentrations of neopterin in the blood. Fewer than 10 percent of those with high antibody levels and low neopterin concentrations developed AIDS during the period. It is suggested therefore that antibody and neopterin levels are independent measures that predict the outcome of HIV infection, but that they affect outcome in an interactive way.
