The robots automatically retarget their optical fibers every 10–20 minutes, each to a precision of several microns, with a reconfiguration time of fewer than 2 minutes. Over the next 5 yr, they will enable the newly constructed Dark Energy Spectroscopic Instrument (DESI) to measure the spectra of 35 million galaxies and quasars. DESI will produce the largest 3D map of the universe to date and measure the expansion history of the cosmos. In addition to the 5020 robotic positioners and optical fibers, DESI's Focal Plane System includes six guide cameras, four wave front cameras, 123 fiducial point sources, and a metrology camera mounted at the primary mirror. The system also includes associated structural, thermal, and electrical systems. In all, it contains over 675,000 individual parts. We discuss the design, construction, quality control, and integration of all these components. We include a summary of the key requirements, the review and acceptance process, on-sky validations of requirements, and lessons learned for future multiobject, fiber-fed spectrographs. © 2022. Objectives Burnout and occupational stress are frequently experienced by healthcare professionals (HCPs). Mindfulness-based stress reduction (MBSR) has been found to improve the psychological health outcomes of HCPs. To date, systematic reviews and meta-analyses have primarily focused upon empirical investigations into the reduction of stress amongst HCPs using MBSR and are limited to empirical studies published before December 2019. This systematic review aimed to update the current evidence base and broaden our understanding of the effectiveness of MBSR on improving the psychological functioning of HCPs. Methods Three electronic databases (Medline, Psych Info and Web of Science) were searched without time frame restrictions. Quantitative studies included randomised controlled trials, clinical controlled trials, pre-post designs and studies with up to a 12-month follow-up period. All studies included in the review employed a MBSR programme, standardised measures of psychological functioning and qualified HCPs as participants. Results Using PRISMA guidelines thirty studies were included in the review. The reviewed literature suggested that MBSR was effective in reducing HCPs experiences of anxiety, depression and stress. MBSR was also found to be effective in increasing HCP levels of mindfulness and self-compassion. However, MBSR did not appear as effective in reducing burnout or improving resilience amongst HCPs. Abbreviated MBSR programmes were found to be as effective as the traditional 8-week MBSR programmes. Conclusions MBSR is an effective intervention which can help improve the psychological functioning of HCPs. Recommendations include improving the overall quality of the studies by employing more robust controlled designs with randomisation, increased sample sizes with heterogeneous samples, and making active comparisons between interventions used.

ObjectivesTo explore reasons for the lack of uptake of ‘Better Care Better Value (BCBV)’ prescribing indicators for renin–angiotensin–aldosterone system (RAAS) inhibitors and identify learning lessons to inform the implementation of future prescribing policies.DesignIn-depth, semistructured interviews to explore: general practitioners’ (GPs) experiences in prescribing RAAS, perceptions of the BCBV policy and potential barriers to policy implementation and suggestions for improving future policy implementation. Interviews were audio recorded, transcribed verbatim and analysed thematically, then mapped onto behavioural change frameworks (the Capability, Opportunity, Motivation and Behaviour) model and Behaviour Change Wheel (BCW)).SettingPrimary care setting in EnglandParticipantsInterviews were conducted with 16 GPs recruited from a purposive sample of 91 GP practices in three English counties.ResultsFour factors/barriers, related mainly to GPs’ psychological capability and reflective motivation, emerged as the possible barriers for the BCBV’s lack of uptake, including: lack of the policy awareness, negative attitude to the policy, lack of incentives and GPs’ reluctance to switch patients from angiotensin receptor blockers (ARBs) to ACE inhibitors (ACEIs). The participating GPs proposed interventions to improve future BCBV implementation and they were related to six intervention/policy functions of the BCW, addressing the four identified barriers: education/communication (increase GPs’ awareness) and environmental restructuring/regulations (provide GPs with reminding alerts); incentivisation/fiscal (provide GPs with financial incentives); enablement/guidelines-regulations (provide GPs with benchmarking against peers) and enablement/regulations and education/guidelines (facilitate switching from ARBs to ACEIs).ConclusionsThe main reason underpinning the low uptake of the BCBV indicator appears to be lack of a proactive implementation strategy. This case study demonstrated that passively disseminating policy without an effective implementation strategy results in low uptake. Furthermore, multifaceted implementation strategies are necessary to influence complex clinical decision making in a time-limited environment, such as prescribing behaviours. These findings suggest that effective policy implementation requires the application of a systematic comprehensive behaviours change framework.

The Dark Energy Spectroscopic Instrument (DESI) is a new instrument currently under construction for the Mayall 4-m telescope at Kitt Peak National Observatory. It will consist of a wide-field optical corrector with a 3.2 degree diameter field of view, a focal plane with 5,000 robotically controlled fiber positioners and 10 fiber-fed broad-band spectrographs. The DESI Instrument Control System (ICS) coordinates fiber positioner operations, interfaces to the Mayall telescope control system, monitors operating conditions, reads out the 30 spectrograph CCDs and provides observer support and data quality monitoring. In this article, we summarize the ICS design, review the current status of the project and present results from a multi-stage test plan that was developed to ensure the system is fully operational by the time the instrument arrives at the observatory in 2019.

We describe the first public data release of the Dark Energy Survey, DES DR1, consisting of reduced single epoch images, coadded images, coadded source catalogs, and associated products and services assembled over the first three years of DES science operations. DES DR1 is based on optical/near-infrared imaging from 345 distinct nights (August 2013 to February 2016) by the Dark Energy Camera mounted on the 4-m Blanco telescope at Cerro Tololo Inter-American Observatory in Chile. We release data from the DES wide-area survey covering ~5,000 sq. deg. of the southern Galactic cap in five broad photometric bands, grizY. DES DR1 has a median delivered point-spread function of g = 1.12, r = 0.96, i = 0.88, z = 0.84, and Y = 0.90 arcsec FWHM, a photometric precision of < 1% in all bands, and an astrometric precision of 151 mas. The median coadded catalog depth for a 1.95" diameter aperture at S/N = 10 is g = 24.33, r = 24.08, i = 23.44, z = 22.69, and Y = 21.44 mag. DES DR1 includes nearly 400M distinct astronomical objects detected in ~10,000 coadd tiles of size 0.534 sq. deg. produced from ~39,000 individual exposures. Benchmark galaxy and stellar samples contain ~310M and ~ 80M objects, respectively, following a basic object quality selection. These data are accessible through a range of interfaces, including query web clients, image cutout servers, jupyter notebooks, and an interactive coadd image visualization tool. The Dark Energy Spectroscopic Instrument (DESI) is under construction to measure the expansion history of the universe using the baryon acoustic oscillations technique. The spectra of 35 million galaxies and quasars over 14,000 square degrees will be measured during a 5-year survey. A new prime focus corrector for the Mayall telescope at Kitt Peak National Observatory will deliver light to 5,000 individually targeted fiber-fed robotic positioners. The fibers in turn feed ten broadband multi-object spectrographs. We describe the ProtoDESI experiment, that was installed and commissioned on the 4-m Mayall telescope from August 14 to September 30, 2016. ProtoDESI was an on-sky technology demonstration with the goal to reduce technical risks associated with aligning optical fibers with targets using robotic fiber positioners and maintaining the stability required to operate DESI. The ProtoDESI prime focus instrument, consisting of three fiber positioners, illuminated fiducials, and a guide camera, was installed behind the existing Mosaic corrector on the Mayall telescope. A Fiber View Camera was mounted in the Cassegrain cage of the telescope and provided feedback metrology for positioning the fibers. ProtoDESI also provided a platform for early integration of hardware with the DESI Instrument Control System that controls the subsystems, provides communication with the Telescope Control System, and collects instrument telemetry data. Lacking a spectrograph, ProtoDESI monitored the output of the fibers using a Fiber Photometry Camera mounted on the prime focus instrument. The Dark Energy Survey (DES) is a next generation optical survey aimed at measuring the expansion history of the universe using four probes: weak gravitational lensing, galaxy cluster counts, baryon acoustic oscillations, and Type Ia supernovae. To perform the survey, the DES Collaboration is building the Dark Energy Camera (DECam), a 3 square degree, 570 Megapixel CCD camera which will be mounted at the Blanco 4-meter telescope at the Cerro Tololo Inter- American Observatory. DES will survey 5000 square degrees of the southern galactic cap in 5 filters (g, r, i, z, Y). DECam will be comprised of 74 250 micron thick fully depleted CCDs: 62 2k x 4k CCDs for imaging and 12 2k x 2k CCDs for guiding and focus. Construction of DECam is nearing completion. In order to verify that the camera meets technical specifications for DES and to reduce the time required to commission the instrument, we have constructed a full sized telescope simulator and performed full system testing and integration prior to shipping. To complete this comprehensive test phase we have simulated a DES observing run in which we have collected 4 nights worth of data. During the Cold War, the United Nations' role in defending international norms was constrained by U S- Soviet rivalry. On the few occasions when economic sanctions were invoked, they were usually hortatory and relatively weak. With the end ofthe Cold War, UN sanctions initially became more frequent, more likely to be mandated by the security council, and more likely to be broad and painful. After a flurry of comprehensive sanctions, however, concerns were raised about the impact on vulnerable populations within target countries, as well as on neighbouring countries and other trading partners. UN sanctions since the mid-1990s have become both less frequent and narrower, often targeting individuals, and have had more limited economic, and perhaps political, impact.What has been the impact of these shifts on the effectiveness of UN economic sanctions? How is effectiveness measured and how have those assessments changed? The starting point for measuring the effectiveness of sanctions must be the nature ofthe goal sought, but the analysis must also take into account the fact that there are usually secondary objectives and that policymakers' true goals may be hidden behind the public rhetoric. Finally, the potential benefits of sanctions must also be weighed against the political, economic, and social costs of using them, as well as the relative costs and benefits of the other policy tools that are available.1The United Nations was created after World War II with the fundamental goal of promoting the collective security of its members, but its reach was limited to lesser disputes from the beginning because of US-Soviet differences.2 Over the years, and long before the Cold War ended, the United Nations also sought to uphold norms related to human rights within countries. As noted by eminent sanctions scholar Margaret Doxey:[N]ew rationales for sanctions have been provided by the development of new international norms in the second half of the twentieth century which go beyond respect for territorial integrity and political independence to encompass self-determination in colonial situations, racial equality, and a broad spectrum of human rights, including the right to democratic systems of government. If 'just sanctions' - like 'just war' - are the international community's justified response to norm-violation, a wide range of external and internal policies may how be judged to merit such a response.3Thus, one measure of UN sanctions that distinguishes them from other sanctions is the responsibility to ensure that they are "just." While not entirely absent during the Cold War, concerns about the impact on individuals were raised to a new level by the humanitarian consequences of comprehensive sanctions, first against Iraq following the first Gulf War and then against Haiti in response to the overthrow of President Jean-Bertrand Aristide. Thus, most UN sanctions now have more than one explicit goal-to stem conflict or uphold international norms, and to do so without harming innocent civilians. But the response - targeted travel or financial sanctions against individuals - has raised other human rights issues that are just beginning to be addressed.4TRADITIONAL APPROACHES TO ASSESSING SANCTIONS EFFECTIVENESSThe effectiveness of any policy tool can only be measured against the goals it is intended to achieve, as well as the costs involved in achieving them. While each case has one or more stated goals specific to that case, such as ending or mitigating conflict, promoting human rights or democracy, or discouraging the spread of weapons of mass destruction, operational objectives fall into three broad and overlapping categories - to signal disapproval, to deny or contain, or to coerce. Different types of sanctions are likely to be relatively more appropriate for some categories than others:* to signal disapproval, punish, or deter: hortatory sanctions that members are exhorted but not required to impose; and travel, financial, or property sanctions targeted at individuals. …

This review examines evidence on the prevalence of non-adherence to medication in adults with rheumatoid arthritis, the reasons for non-adherence, and the effectiveness of interventions to improve adherence. Medications for rheumatoid arthritis (NSAIDs, corticosteroids, disease-modifiers, and biologic agents) are not curative, but can reduce symptoms and slow progression of the disease. The Dark Energy Camera is a new imager with a 2.2-degree diameter field of view mounted at the prime focus of the Victor M. Blanco 4-meter telescope on Cerro Tololo near La Serena, Chile. The camera was designed and constructed by the Dark Energy Survey Collaboration, and meets or exceeds the stringent requirements designed for the wide-field and supernova surveys for which the collaboration uses it. The camera consists of a five element optical corrector, seven filters, a shutter with a 60 cm aperture, and a CCD focal plane of 250 micron thick fully-depleted CCDs cooled inside a vacuum Dewar. The 570 Mpixel focal plane comprises 62 2kx4k CCDs for imaging and 12 2kx2k CCDs for guiding and focus. The CCDs have 15 microns x15 microns pixels with a plate scale of 0.263 arc sec per pixel. A hexapod system provides state-of-the-art focus and alignment capability. The camera is read out in 20 seconds with 6-9 electrons readout noise. In this essay, Kim Elliott examines the patterns of support for agriculture across countries and commodities in the industrialized world. She then summarizes the approach to reducing trade-distorting support that came out of the Uruguay Round, and concludes with a discussion of the implications for reviving and successfully concluding the Doha Round.

For over two years the U.S. has pushed to open markets globally, in our hemisphere and with sub-regions or individual countries. As WTO members ponder the future the U.S. will not wait: we will move towards free trade with can-do countries.

The Miami Summit launching the Free Trade Area of the Americas process recognized that free markets and free societies work best when they work together. The core labor —standards freedom of association and the right to organize and bargain collectively; freedom from forced labor; the abolition of child labor; and freedom from discrimination— are fundamental rights and, in an environment that promotes democracy and market–oriented economies, as the FTAA is intended to do, there is no trade– off between these principles and development; indeed, they become mutually reinforcing. A review of International Labor Organization information on implementation of core labor standards in the hemisphere suggests that there is substantial room for progress, including in export sectors. Recent developments in the treatment of labor issues in US trade agreements also suggests that labor standards cannot be ignored if an FTAA agreement is to gain approval in the US Congress.

Objective To undertake a process evaluation of pharmacists’ recommendations arising in the context of a complex IT-enabled pharmacist-delivered randomised controlled trial (PINCER trial) to reduce the risk of hazardous medicines management in general practices. Methods PINCER pharmacists manually recorded patients’ demographics, details of interventions recommended, actions undertaken by practice staff and time taken to manage individual cases of hazardous medicines management. Data were coded, double-entered into SPSS version 15 and then summarised using percentages for categorical data (with 95% confidence interval (CI)) and, as appropriate, means (± standard deviation) or medians (interquartile range) for continuous data. Key findings Pharmacists spent a median of 20 min (interquartile range 10, 30) reviewing medical records, recommending interventions and completing actions in each case of hazardous medicines management. Pharmacists judged 72% (95% CI 70, 74; 1463/2026) of cases of hazardous medicines management to be clinically relevant. Pharmacists recommended 2105 interventions in 74% (95% CI 73, 76; 1516/2038) of cases and 1685 actions were taken in 61% (95% CI 59, 63; 1246/2038) of cases; 66% (95% CI 64, 68; 1383/2105) of interventions recommended by pharmacists were completed and 5% (95% CI 4, 6; 104/2105) of recommendations were accepted by general practitioners (GPs), but not completed at the end of the pharmacists’ placement; the remaining recommendations were rejected or considered not relevant by GPs. Conclusions The outcome measures were used to target pharmacist activity in general practice towards patients at risk from hazardous medicines management. Recommendations from trained PINCER pharmacists were found to be broadly acceptable to GPs and led to ameliorative action in the majority of cases. It seems likely that the approach used by the PINCER pharmacists could be employed by other practice pharmacists following appropriate training.

Economic Instruments of Security Policy: Influencing Choices of Leaders. By Gary M. Shiffman Basingstoke, UK: Palgrave MacMillan, 2006. 256 pp., $75.00 cloth (ISBN: 1-4039-4953-0), $29.95 paper (ISBN: 1-4039-4964-6). Despite the slim size of Economic Instruments of Security Policy , Gary Shiffman promises readers that he will undertake a “thorough examination” of (1) basic economic behavior at the level of the individual; (2) decision making under different political systems; and (3) the utility of a range of economic tools in promoting national security. Shiffman's basic theses are that individuals, not states, make decisions, that dictators behave differently than democrats, and that economic tools can be used to promote national security by influencing those individuals. Each of these elements is discussed separately. Unfortunately, Shiffman's efforts to connect them in a coherent and systematic way are largely unsuccessful. Economic Instruments of Security Policy reads like a series of loosely connected university lectures, which Shiffman says was in fact the genesis for the project. As one would expect under these circumstances, Shiffman draws selectively on the existing literature in various areas and breaks little new ground. As a result, undergraduate college students interested in international economic policy are the book's most likely audience, not the policymakers that Shiffman seeks to reach. The effort to stitch together and amplify a series of lectures on international trade and finance also results in a confusing structure. Shiffman begins Economic Instruments of Security Policy with brief chapters discussing the economic behavior of the individual and various types of autocracies. He then jumps to a case study of Cuba that combines an application of his model for predicting the behavior of dictators with a truncated summary of the literature on economic sanctions. The paper addresses the issue of the potential costs of a duty-free-quota-free given to the Least-Developed-Countries (LDCs) and the issue of the eligibility of other poor countries to this kind of preferential program. Using the MIRAGE Computable General Equilibrium model, it assesses the impact of full liberalization given by high-income-countries members of OECD and the emerging markets, and estimate outcomes in terms of LDCs’ trade performance and giving-countries’ level of production. This study also allows evaluating if some of the poorest countries will suffer from preference erosion, taking into account their initial preferential schemes, under different scenarios which differ from giving-countries and receiving-countries implied in partial (97% of tariff lines) or full liberalization (100% of tariff lines). The main goal of this paper is to highlight the role that rich and emerging countries could play to help LDCs to improve their trade performance and to assess in which modalities the costs of such policy will be lowest for the developing world and in line with a political feasibility for giving-countries. Our conclusions underscore that because of rich-country tariff peaks and LDCs exports concentration on a few sectors, only full product coverage offers significant benefits. Despite good access in its main markets, sub-Saharan Africa can still gain from 100 percent DFQF, particularly Malawi from elimination of remaining agricultural restrictions in the US markets, and others if the major emerging markets also adopt 100 percent DFQF market access. Extending preferential access to other Low-Income-Countries has little impact on LDCs preferences, but if the program is extended to Low-Middle-Income-Countries it will produce preference erosion, especially in the EU market where LDCs already have 100 percent (nominal) access. Finally, this study shows that rich countries which provide full market access have little impact on Welfare and production.

Despite six decades of trade liberalization, trade policies in rich countries still discriminate against the exports of the world’s poorest countries. Preferential market access programs were designed to spur larger and more diversified exports from developing countries, but product exclusions and burdensome rules undermined their usefulness, especially for the poorer countries. Most rich countries have made reforms since the UN Millennium Declaration in 2000 called for duty-free, quota-free market access for the least developed countries. After the World Trade Organization ministerial communique called upon developing countries “in a position to do so” to also provide such access, key countries have moved toward that goal. But much remains to be done to achieve the goal of meaningful market access for the poorest countries, including reformed rules of origin that facilitate rather than inhibit trade.

While the precise contribution of biofuels to surging food prices is difficult to know, policies promoting production of the current generation of biofuels are not achieving their stated objectives of increased energy independence or reduced greenhouse gas emissions. Reaching the congressionally mandated goal of blending 15 billion gallons of renewable fuels in gasoline by 2015 would consume roughly 40 percent of the corn crop (based on recent production levels) while replacing just 7 percent of current gasoline consumption. Moreover, while it has long been known that the net energy and greenhouse gas emission benefits of corn-based ethanol are relatively small because its production is energy-intensive, recent scientific studies suggest that the current generation of biofuels, including biodiesel made from palm oil, soybeans, and rapeseed, as well as corn-based ethanol, actually add to greenhouse gas emissions relative to petroleum-based fuels when land use changes are taken into account. That is, greenhouse gases are released when forests are cut down or grasslands cleared to plant biofuels, or food is planted on new acreage to replace crops diverted to fuel elsewhere. In sum, the food crisis adds urgency to the need to change these policies but does not change the basic fact that there is little justification for the current set of policies.

After a half century of multilateral bargaining to reduce trade barriers, agriculture stands out for the degree of protection and government support that it still enjoys in most rich countries. This makes agricultural protection a natural focus of the current Doha Round of trade negotiations: in addition to offering the juiciest targets for liberalization, this round is supposed to address the needs of developing countries, where the vast majority of the world's farmers, most of them poor, reside. But is there any reason to think trade negotiations are more likely now than in the past to encourage substantial reform of rich countries' farm policies? This paper looks at the evolution of and current approaches to agricultural policies in rich countries to see if there are lessons from the past that might improve chances for reform this time around.

With the continuing expansion of global economic integration, labor standards in developing countries have become a hot button issue. One result has been a proliferation of efforts to use the market to put pressure directly on multinational corporations to improve wages and working conditions in their overseas operations and to insist that their suppliers do so as well. This paper analyzes the dynamics of these efforts in terms of a 'market for standards' in which consumers, stimulated by human rights activists, demand that corporations improve working conditions in supplier factories. The paper presents evidence that such a consumer demand exists and analyzes the incentives corporations face to respond to it. It examines the nature of the critical intermediary role played by activists in stimulating consumer demands and assesses the outcomes in the major anti-sweatshop campaigns of the 1990s. The paper also addresses the limitations of such consumer-based campaigns and the concern expressed by some that these activist campaigns may do more harm than good, by deterring investment in and trade with poor countries. It concludes with an overall assessment of when 'doing good' actually does good.

Relatively little controversy surrounds three of the four core labor standards - forced labor, discrimination, and child labor. But the right to associate and organize freely and to bargain collectively is more controversial. And the use of trade sanctions to enforce labor standards is most divisive of all. In the context of trade negotiations, attention to labor issues can lower adjustment costs, slow a race to the bottom from the bottom, among developing countries themselves, and increase political support for trade agreements in developed countries. Elliott suggests using a parallel track to negotiate labor issues and link progress in those negotiations more closely to the trade negotiations. She concludes that nothing is to be gained by workers and labor activists keeping sanctions to enforce standards in trade agreements as the focus of their demands. Instead, they should ratchet up the pressure on governments to adopt concrete plans of action for raising labor standards and to finance implementation of those plans.

The recently-adopted OECD convention outlawing bribery of foreign public officials is welcome evidence of how much progress has been made in the battle against corruption. The financial crisis in East Asia is an indication of how much remains to be done. Corruption is by no means a new issue but it has only recently emerged as a global issue. With the end of the Cold War, the pace and breadth of the trends toward democratization and international economic integration accelerated and expanded globally. Yet corruption could slow or even reverse these trends, potentially threatening economic development and political stability in some countries. As the global implications of corruption have grown, so has the impetus for international action to combat it. In addition to efforts in the OECD, the Organization of American States, the World Trade Organization, and the United Nations General Assembly, the World Bank and the International Monetary Fund have both begun to emphasize corruption as an impediment to economic development. This book includes a chapter by the Chairman of the OECD Working Group on Bribery discussing the evolution of the OECD convention and what is needed to make it effective. Other chapters address the causes and consequences of corruption, including the impact on investment and growth and the role of multinational corporations in discouraging bribery. The final chapter summarizes and also discusses some of the other anticorruption initiatives that either have been or should be adopted by governments, multilateral development banks, and other international organizations.
