1. Changes in Students' Achievement Motivation in the Context of the COVID-19 Pandemic: A Function of Extraversion/Introversion?

Abstract

Students' mental health has been an increased concern since the outbreak of the COVID-19 pandemic. However, academic outcomes have received very little attention. In this study, changes in students' achievement motivation are investigated using an expectancy-value framework. Participants (n = 90) were high school students (grades 9 and 10) who reported on their expectancy and value perceptions in regard to learning before and during the pandemic (i.e., January and November 2020). Changes over time and as a function of extraversion/introversion were analyzed using repeated measures multivariate analyses of variance (MANOVAs). Most perceptions were found to be stable with the exception of interest in learning, which increased as a function of extraversion. Results are discussed in light of relevant pre-pandemic evidence.

5. The Impact of Teacher-Student Relationships and Achievement Motivation on Students' Intentions to Dropout According to Socio-Economic Status

Abstract

The main goal was to test if teacher-student relationships and achievement motivation are predicting dropout intention equally for low and high socio-economic status students. A questionnaire measuring teacher-student relationships and achievement motivation was administered to 2,360 French Canadian secondary students between 12 and 15 years old during the spring of 2005. A hierarchical multiple regressions model with interactions predicted their dropout intention. The moderator variable was SES (socio-economic status). Results showed that most predictors of dropout intention acted similarly for both SES. However, strong competence beliefs in mathematics predicted low dropout intention for students from high SES. Knowing that low SES students dropout more than others, our homogeneous predictors do not explain entirely the dropout phenomenon. (Contains 1 figure and 5 tables.)

8. Real-life comparison of posaconazole versus fluconazole for primary antifungal prophylaxis during remission-induction chemotherapy for acute leukemia

Abstract

Background: Patients undergoing remission-induction intensive chemotherapy for acute leukemia are at high risk for life-threatening invasive fungal infections (IFIs). Primary antifungal prophylaxis with posaconazole has been shown to reduce the incidence of IFI compared to fluconazole, but real-life data are limited and the effect on mortality remains unclear. Methods: This retrospective cohort study compared fluconazole and posaconazole as primary prophylaxis in real-life practice over a 10-year period, in a Canadian hospital. Results: A total of 299 episodes were included (fluconazole, n = 98; posaconazole, n = 201), of which 68% were first inductions. The underlying hematologic malignancy was acute myeloid leukemia or myelodysplastic syndrome in 88% of episodes and acute lymphoblastic leukemia in 9%. Overall, 20 cases of IFI occurred (aspergillosis, n = 17; candidiasis, n = 3) and 14 were considered as breakthrough IFI. IFI incidence was significantly lower in the posaconazole group (3.5% versus 13.2%; p = 0.001). Empirical or targeted antifungal therapy was also reduced in the posaconazole cohort. Mortality was similar in both groups. Conclusions: In a real-life setting in Canada, primary posaconazole prophylaxis reduces the incidence of IFI during remission-induction chemotherapy, compared to fluconazole.

Abstract

Optimizing research on the developmental origins of health and disease (DOHaD) involves implementing initiatives maximizing the use of the available cohort study data; achieving sufficient statistical power to support subgroup analysis; and using participant data presenting adequate follow-up and exposure heterogeneity. It also involves being able to undertake comparison, cross-validation, or replication across data sets. To answer these requirements, cohort study data need to be findable, accessible, interoperable, and reusable (FAIR), and more particularly, it often needs to be harmonized. Harmonization is required to achieve or improve comparability of the putatively equivalent measures collected by different studies on different individuals. Although the characteristics of the research initiatives generating and using harmonized data vary extensively, all are confronted by similar issues. Having to collate, understand, process, host, and co-analyze data from individual cohort studies is particularly challenging. The scientific success and timely management of projects can be facilitated by an ensemble of factors. The current document provides an overview of the 'life course' of research projects requiring harmonization of existing data and highlights key elements to be considered from the inception to the end of the project.

17. Maternal stress during pregnancy and gestational duration:A cohort study from the Danish National Birth Cohort

Abstract

Background: Preterm birth is one of the most important contributors to neonatal mortality and morbidity. Experiencing stress during pregnancy may increase the risk of adverse birth outcomes, including preterm birth. This association has been observed in previous studies, but differences in measures used limit comparability. Objective: The objective of the study was to investigate the association between two measures of maternal stress during pregnancy, life stress and emotional distress, and gestation duration. Methods: Women recruited in the Danish National Birth Cohort from 1996 to 2002, who provided information on their stress level during pregnancy and expecting a singleton baby, were included in the study. We assessed the associations between the level of life stress and emotional distress in quartiles, both collected at 31 weeks of pregnancy on average, and the rate of giving birth using Cox regression within intervals of the gestational period. Results: A total of 80,991 pregnancies were included. Women reporting moderate or high levels of life stress vs no stress had a higher rate of giving birth earlier within all intervals of gestational age (e.g. high level: 27–33 weeks: hazard ratio (HR) 1.38, 95% confidence interval (CI) 1.04, 1.84; 34–36 weeks: 1.10, 95% CI 0.97, 1.25; 37–38 weeks: 1.21, 95% CI 1.15, 1.28). These associations between life stress and preterm birth were mainly driven by pregnancy worries. For emotional distress, a high level of distress was associated with shorter length of gestation in the preterm (27–33 weeks: 1.38, 95% CI 1.02, 1.86; 34–36 weeks: 1.05, 95% CI 0.91, 1.19) and early term (1.11, 95% CI 1.04, 1.17) intervals. Conclusions: Emotional distress and life stress were shown to be associated with gestational age at birth, with pregnancy-related stress being the single stressor driving the association. Objectives: Existing individual-level human data cover large populations on many dimensions such as lifestyle, demography, laboratory measures, clinical parameters, etc. Recent years have seen large investments in data catalogues to FAIRify data descriptions to capitalise on this great promise, i.e. make catalogue contents more Findable, Accessible, Interoperable and Reusable. However, their valuable diversity also created heterogeneity, which poses challenges to optimally exploit their richness. Methods: In this opinion review, we analyse catalogues for human subject research ranging from cohort studies to surveillance, administrative and healthcare records. Results: We observe that while these catalogues are heterogeneous, have various scopes, and use different terminologies, still the underlying concepts seem potentially harmonizable. We propose a unified framework to enable catalogue data sharing, with catalogues of multi-center cohorts nested as a special case in catalogues of real-world data sources. Moreover, we list recommendations to create an integrated community of metadata catalogues and an open catalogue ecosystem to sustain these efforts and maximise impact. Conclusions: We propose to embrace the autonomy of motivated catalogue teams and invest in their collaboration via minimal standardisation efforts such as clear data licensing, persistent identifiers for linking same records between catalogues, minimal metadata ‘common data elements’ using shared ontologies, symmetric architectures for data sharing (push/pull) with clear provenance tracks to process updates and acknowledge original contributors. Abstract

Malnutrition continues to be one of the world's most critical health and human development challenges, threatening countries' Universal Health Coverage (UHC) goals and the achievement of the Sustainable Development Goals (SDGs). Given the complex, multifactorial, and interlinked determinants of nutritional status and well-being, multisectoral nutrition programming has been widely promoted as the most effective way to address the direct and indirect determinants of malnutrition and to improve nutrition outcomes. Robust governance systems are essential for implementing multisectoral nutrition interventions and creating cost-effective and sustainable programs. The objectives of this report are to (i) document and synthesize implementation experiences, challenges, and opportunities from seven countries supported by the World Bank and Global Financing Facility (GFF) in operationalizing large-scale multisectoral nutrition projects that emphasize and strengthen governance (Cambodia, the Democratic Republic of Congo, Guatemala, Indonesia, Malawi, Nigeria, and Rwanda); and (ii) facilitate cross-country learning. Given that the seven countries used as examples in this report are still implementing their multisectoral programs, the report focuses on documenting progress and lessons learned on implementation modalities and innovations, rather than highlighting impact at this stage. The report uses a multisectoral governance framework, adapted from Gillespie, Van Den Bold, and Hodge (2019), to synthesize the implementation experiences across the World Bank/GFF–financed multisectoral nutrition projects. The report provides eight lessons learned, organized under three broad categories: (1) Advocacy, leadership, and institutional support for multisectoral nutrition; (2) Management capacity and financing; and (3) Results measurement, monitoring, and accountability.

Abstract

VIALE-C compared the safety and efficacy of venetoclax or placebo plus low-dose cytarabine (+LDAC) in patients with untreated AML ineligible for intensive chemotherapy. Overall, 211 patients were enrolled (n = 143, venetoclax; n = 68, placebo). At the primary analysis, the study did not meet its primary endpoint of a statistically significant improvement in overall survival (OS), however, similar to 60% of patients had been on study for = 3 adverse events were similar between study arms and overall safety profiles were comparable to the primary analysis. These data support venetoclax +LDAC as a frontline treatment option for patients with AML ineligible for intensive chemotherapy.

A study was conducted on African people to analyze the stromal cell derived factor 1 (SDF1)-3'A single nucleotide polymorphism (SNP) and performed mutation screening for polymorphic markers in the chemokine receptor 4 (CXCR4) gene to determine existence of significant associations with susceptibility to HIV-1 infection. Comprehensive mutation analysis of the CXCR4 gene confirmed a high degree of genetic conservation within the coding region of African ancient populations.

Abstract

T cells responsive to minor histocompatibility (H) antigens are extremely effective in curing leukemia but it remains unknown whether they can eradicate solid tumors. We report that injection of CD8[sup.+] T cells primed against the immunodominant H7[sup.a] minor H antigen can cure established melanomas in mice. Tumor rejection was initiated by preferential extravasation at the tumor site of interferon (IFN)-[gamma]-producing H7[sup.a]-specific T cells. Intratumoral release of IFN-[gamma] had two crucial effects: inhibition of tumor angiogenesis and upregulation of major histocompatibility complex (MHC) class I expression on tumor cells. Despite ubiquitous expression of H7[sup.a], dissemination of a few H7[sup.a]-specific T cells in extralymphoid organs caused neither graft-versus-host disease (GVHD) nor vitiligo because host nonhematopoietic cells were protected by their low expression of MHC class I. Abstract

Senegal is among the few countries in Sub-Saharan Africa that have succeeded in improving their population’s nutritional status in recent decades. The prevalence of key nutrition indicators, such as the proportion of children stunted, wasted, and underweight, is lower than that in most other countries in the region, and even among the poorest segments of the population. Progress against malnutrition achieved over time notwithstanding, reversals in global funding for nutrition in Senegal neglect the still-too-high prevalence of child stunting, wasting, and micronutrient deficiencies, as well as the persistent prevalence of under- and overweight and anemia among women. Nutrition in early childhood has been deemed by the global community to be a key determinant of both labor productivity and economic growth. For Senegal, which has arguably entered its nutrition transition and which is embarking on a new, multi-sectoral approach to tackle the double and triple burdens of malnutrition, there is no better time to ramp up nutrition investment. This investment case outlines future directions in nutrition for Senegal on the basis of the plan stratégique multisectoriel de la nutrition (PSMN) and synthesizes a series of eight reports prepared for the purpose of understanding the country’s progress to date, while highlighting remaining gaps in funding and implementation for nutrition. The first section presents the developmental and economic rationales for investing in nutrition, including a review of Senegal’s unique political climate, which lends itself to the development of new, unconventional policies. The second section draws from a situation analysis, a political economy study, and an institutional performance assessment to describe ongoing barriers to good nutrition in Senegal, while the third section presents solutions on how to overcome them, in alignment with the PSMN. The fourth section presents the financial requirements for these activities, which are based on a calculation of sector-specific financial needs costed during the development of the PSMN, secured funding identified in the financing analysis study, and the costs of scaling up to 90 percent coverage as calculated in the economic analysis report.
