1. Overcoming Therapeutic Inertia in Type 2 Diabetes: Exploring Machine Learning-Based Scenario Simulation for Improving Short-Term Glycemic Control

Abstract: Background: International guidelines for diabetes care emphasize the urgency of promptly achieving and sustaining adequate glycemic control to reduce the occurrence of micro/macrovascular complications in patients with type 2 diabetes mellitus (T2DM). However, data from the Italian Association of Medical Diabetologists (AMD) Annals reveal that only 47% of T2DM patients reach appropriate glycemic targets, with approximately 30% relying on insulin therapy, either solely or in combination. This artificial intelligence analysis seeks to assess the potential impact of timely insulin initiation in all eligible patients via a “what-if” scenario simulation, leveraging real-world data. Methods: This retrospective cohort study utilized the AMD Annals database, comprising 1,186,247 T2DM patients from 2005 to 2019. Employing the Logic Learning Machine (LLM), we simulated timely insulin use for all eligible patients, estimating its effect on glycemic control after 12 months within a cohort of 85,239 patients. Of these, 20,015 were employed for the machine learning phase and 65,224 for simulation. Results: Within the simulated scenario, the introduction of appropriate insulin therapy led to a noteworthy projected 17% increase in patients meeting the metabolic target after 12 months from therapy initiation within the cohort of 65,224 individuals. The LLM’s projection envisages 32,851 potential patients achieving the target (hemoglobin glycated < 7.5%) after 12 months, compared to 21,453 patients observed in real-world cases. The receiver operating characteristic (ROC) curve analysis for this model demonstrated modest performance, with an area under the curve (AUC) value of 70.4%. Conclusions: This study reaffirms the significance of combatting therapeutic inertia in managing T2DM patients. Early insulinization, when clinically appropriate, markedly enhances patients’ metabolic goals at the 12-month follow-up.

Abstract: Summary: Background: A delay in reaching HbA1c targets in patients with newly-diagnosed type 2 diabetes (T2D) is associated with an increased long-term risk of developing cardiovascular diseases (CVD), a phenomenon referred to as legacy effect. Whether an early introduction of glucose-lowering drugs with proven benefit on CVD can attenuate this phenomenon is unknown. Methods: Using data derived from a large Italian clinical registry, i.e. the AMD Annals, we identified 251,339 subjects with newly-diagnosed T2D and without CVD at baseline. Through Cox regressions adjusted for multiple risk factors, we examined the association between having a mean HbA1c between 7.1 and 8% or >8%, compared with ≤7%, for various periods of early exposure (0–1, 0–2, 0–3 years) and the development of later (mean subsequent follow-up 4.6 ± 2.9 years) CVD, evaluated as a composite of myocardial infarction, stroke, coronary or peripheral revascularization, and coronary or peripheral bypass. We performed this analysis in the overall cohort and then splitting the population in two groups of patients: those that introduced sodium-glucose transport protein 2 inhibitors (SGLT-2i) during the exposure phase and those not treated with these drugs. Findings: Considering the whole cohort, subjects with both a mean HbA1c between 7.1 and 8% and >8%, compared with patients attaining a mean HbA1c ≤ 7%, showed an increased risk of developing the outcome in all the three early exposure periods assessed, with the highest risk observed in patients with mean HbA1c > 8% in the 3 years exposure period (hazard ratio [HR]1.33; 95% confidence interval [CI] 1.063–1.365). The introduction of SGLT-2i during the exposure periods of 0–1 and 0–2 years eliminated the association between poor glycemic control and the outcome (p for interaction 0.006 and 0.003, respectively, vs. patients with the same degree of glycemic control but not treated with these drugs). Interpretation: Among patients with newly diagnosed T2D and free of CVD at baseline, a poor glycemic control in the first three years after diagnosis is associated with an increased subsequent risk of CVD. This association is no longer evident when SGLT-2i are introduced in the first two years, suggesting that these drugs attenuate the phenomenon of legacy effect. An early treatment with these drugs might thus promote a long-lasting benefit in patients not attaining proper glycemic control after T2D diagnosis. Abstract: Presentamos el caso de una paciente de 70 años, con el único antecedente médico de interés de anafilaxia por Anisakis, que presenta un fracaso renal agudo con biopsia compatible con nefritis tubulointersticial de etiología alérgica tras la ingesta de pescado. A pesar de que no se ha publicado ningún caso previo de nefritis por hipersensibilidad a Anisakis, la clínica, evolución y biopsia fueron compatibles con esta sospecha, sin poderse encontrar otra posible etiología del fracaso renal agudo.

4. Gender-Disparities in Adults with Type 1 Diabetes: More Than a Quality of Care Issue. A Cross-Sectional Observational Study from the AMD Annals Initiative.

Abstract: We evaluated gender-differences in quality of type 1 diabetes (T1DM) care. Starting from electronic medical records of 300 centers, 5 process indicators, 3 favorable and 6 unfavorable intermediate outcomes, 6 treatment intensity/appropriateness measures and an overall quality score were measured. The likelihood of women vs. men (reference class) to be monitored, to reach outcomes, or to be treated has been investigated through multilevel logistic regression analyses; results are expressed as Odd Ratios (ORs) and 95% confidence intervals (95%CIs). The inter-center variability in the achievement of the unfavorable outcomes was also investigated. Overall, 28,802 subjects were analyzed (45.5% women). Women and men had similar age (44.5±16.0 vs. 45.0±17.0 years) and diabetes duration (18.3±13.0 vs. 18.8±13.0 years). No between-gender differences were found in process indicators. As for intermediate outcomes, women showed 33% higher likelihood of having HbA1c ≥8.0% (OR = 1.33; 95%CI: 1.25-1.43), 29% lower risk of blood pressure ≥140/90 mmHg (OR = 0.71; 95%CI: 0.65-0.77) and 27% lower risk of micro/macroalbuminuria (OR = 0.73; 95%CI: 0.65-0.81) than men, while BMI, LDL-c and GFR did not significantly differ; treatment intensity/appropriateness was not systematically different between genders; overall quality score was similar in men and women. Consistently across centers a larger proportion of women than men had HbA1c ≥8.0%, while a smaller proportion had BP ≥140/90 mmHg. No gender-disparities were found in process measures and improvements are required in both genders. The systematic worse metabolic control in women and worse blood pressure in men suggest that pathophysiologic differences rather than the care provided might explain these differences.

Abstract: AIMS: Beta-cell dysfunction is an early event in the natural history of type 2 diabetes. However, its progression is variable and potentially influenced by several clinical factors. We report the baseline data of the BetaDecline study, an Italian prospective multicenter study on clinical predictors of beta-cell dysfunction in type 2 diabetes. MATERIALS AND METHODS: Clinical, lifestyle, and laboratory data, including circulating levels of inflammatory markers and non-esterified fatty acids, were collected in 507 type 2 diabetic outpatients on stable treatment with oral hypoglycemic drugs or diet for more than 1 year. Beta-cell dysfunction was evaluated by calculating the proinsulin/insulin ratio (P/I). RESULTS: At baseline, the subjects in the upper PI/I ratio quartile were more likely to be men and receiving secretagogue drugs; they also showed a borderline longer diabetes duration (P = 0.06) and higher serum levels of glycated hemoglobin (HbA1c), fasting blood glucose, and triglycerides. 9. Absence of Long Range Magnetic Order in the La1.4Sr0.8Ca0.8Mn2O7 Bilayered Manganite

Abstract: In this work we studied, by means of high-resolution neutron diffraction as a function of temperature, the La1.4Sr0.8Ca0.8Mn2O7 bilayered manganite for two different annealing treatments. Out data allowed us to shown, for the first time, the absence of long-range magnetic order in this optimally doped bilayered manganite where the A-site of the structure is doped with equal proportions of different isovalent cations (Ca and Sr). The system, however, presents defined IM transitions which suggest that the transport properties are not linked to the evolution of long-range order and that two dimensional spin ordering in the layers of the perovskite blocks may be sufficient to 'assist' the hole hopping. Possible reason for the suppression of magnetic order induced by the Ca doping is a size effect coupled to the cation size mismatch between the Sr and Ca ions., Comment: 24 pages, 7 figures

Abstract: In this paper we report the results of the synthesis and structural, transport and magnetic characterization of pure La0.5Sr1.5MnO4 (LSMO) and 5% doped samples, i.e. La0.5Sr1.5Mn0.95B0.05O4, where B = Ru, Co and Ni. It is shown that even a light doping is successful in suppressing the charge and orbital ordering found for the pure LSMO. In general, doping favours the carrier motion and, from a magnetic point of view, the set-up a spin-glass state. Abstract: In this work we report a structural, electrical and magnetic characterization of the La1-xNaxMn1-yRuyO3+d (LNMRO) system with x = 0.05, 0.15 and y = 0, 0.05, 0.15, also comprising an investigation of the role of the oxygen content on the related redox properties. The experimental investigation has been realized with the aid of X-ray powder diffraction, electron microprobe analysis, thermogravimetry, electrical resistivity and magnetization measurements, and electron paramagnetic resonance. We demonstrate that the effect of ruthenium doping on the studied LNMRO compounds is not only directly related to the Ru/Mn substitution and to the Ru oxidation state but also indirectly connected to the oxygen content in the sample. Our data show that ruthenium addition can improve electrical and magnetic properties of non-optimally (low) cation doped manganites, causing an increase of the TC value and the insurgence of MR effect, as observed for the x = 0.05 and y = 0.05 sample., Comment: 28 Pages, 7 Figures

Abstract: In this paper we report studies on the correlation between the presence and distribution of cation vacancies in doped manganites (La,M)MnO3+delta (where M = Na, Ca) and their magnetic properties. Results indicate that cation vacancies are distributed differently for the different crystal structures and dopant ion type. In particular it is shown that knowledge of the total vacancies concentration alone is not enough to fully characterize the physical properties of manganites and that their distribution between the A and B sites of the perovskite structure plays a crucial role which should be taken into account in future studies., Comment: 27 pages, 5 figure. Abstract: Purpose Stroke is a leading cause of long-term disability with high mortality rate in the first year after the event. In Campania, mechanical thrombectomy treatment significantly increases in the last 3 years, as well as hospitals delivering acute stroke treatments. The aim of this study is to demonstrate how a full opening of our stroke network improves stroke management and stroked patients’ survival in Campania. Material and methods In Federico II University Hospital of Naples acting as a HUB center of 7 peripheral SPOKE hospitals in regional territory, 68 patients with acute ischemic stroke were evaluated with NIHSS and m-RS clinical scores and neuroradiological ASPECT scores, from January 1 to December 31, 2021. At hospital discharge, NIHSS score and three months after m-RS score were re-assessed to evaluate the therapeutic effects. Results Forty-two of 68 patients (63%) admitted to our hub center had ischemic acute stroke at CT evaluation; 29 patients had ASPECT score > 7 (69%), and 6 a score Discussion The inter-company agreement between Federico II University and several peripheral hospitals allows an absolute and relative increase in endovascular mechanical thrombectomy and intravenous thrombolysis procedures, with a relative prevalence of mechanical thrombectomy. A regional implementation of the stroke multi-disciplinary care system is hardly needed to ensure the optimum treatment for the largest number of patients, improving patient’s outcome.

Abstract: Telemedicine is advocated as a fundamental tool in modern clinical management. However, data on the effects of telemedicine vs face-to-face consultation on clinical outcomes in type 2 diabetes (T2DM) are still uncertain. This paper describes the use of telemedicine during the 2020 COVID-19 emergency and compares volume activity and quality indicators of diabetes care between face-to-face vs telemedicine counseling in the large cohort of T2DM patients from the AMD Annals Initiative.Demographic and clinical characteristics, including laboratory parameters, rate of the screening of long-term complications, current therapies and the Q-score, a validated score that measures the overall quality of care, were compared between 364,898 patients attending face-to-face consultation and 46,424 on telemedicine, during the COVID-19 pandemic.Patients on telemedicine showed lower HbA1c levels (7.1 ± 1.2 % vs 7.3 ± 1.3 %, p 0.0001), and they were less frequently treated with metformin, GLP1-RAs and SGLT2i and more frequently with DPP4i. The telemedicine group showed reduced monitoring of the various parameters considered as process indicators, especially, eye and foot examination. The proportion of patients with a good quality of care (Q score 25) was higher among those receiving face-to-face consultation. Moreover, in the telemedicine group, all major clinical outcomes remained stable when further compared to those collected in the year 2019, when the same patients underwent a regular face-to-face consultation, suggesting that the care provided through telemedicine did not negatively affect the most important parameters.During the COVID-19 pandemic, telemedicine provided an acceptable quality of diabetes care, comparable to that of patients attending face-to-face consultation, although a less frequent screening of complications seems to have occurred in subjects consulted by telemedicine.

Abstract: One of the main neutron sources for the astrophysical s process is the reaction ^{13}C(α,n)^{16}O, taking place in thermally pulsing asymptotic giant branch stars at temperatures around 90 MK. To model the nucleosynthesis during this process the reaction cross section needs to be known in the 150-230 keV energy window (Gamow peak). At these sub-Coulomb energies, cross section direct measurements are severely affected by the low event rate, making us rely on input from indirect methods and extrapolations from higher-energy direct data. This leads to an uncertainty in the cross section at the relevant energies too high to reliably constrain the nuclear physics input to s-process calculations. We present the results from a new deep-underground measurement of ^{13}C(α,n)^{16}O, covering the energy range 230-300 keV, with drastically reduced uncertainties over previous measurements and for the first time providing data directly inside the s-process Gamow peak. Selected stellar models have been computed to estimate the impact of our revised reaction rate. For stars of nearly solar composition, we find sizeable variations of some isotopes, whose production is influenced by the activation of close-by branching points that are sensitive to the neutron density, in particular, the two radioactive nuclei ^{60}Fe and ^{205}Pb, as well as ^{152}Gd.

Abstract: Background: Shell hydrogen burning during the asymptotic giant branch (AGB) phase through the oxygen isotopes has been indicated as a key process that is needed to understand the observed O 18 / O 16 relative abundance in presolar grains and in stellar atmospheres. This ratio is strongly influenced by the relative strengths of the reactions O 18 ( p , α ) N 15 and O 18 ( p , γ ) F 19 in low-mass AGB stars. While the former channel has been the focus of a large number of measurements, the ( p , γ ) reaction path has only recently received some attention and its stellar reaction rate over a wide temperature range rests on only one measurement. Purpose: Our aim is the direct measurement of states in F 19 as populated through the reaction O 18 ( p , γ ) F 19 to better determine their influence on the astrophysical reaction rate, and more generally to improve the understanding of the nuclear structure of F 19 .Method: Branchings and resonance strengths were measured in the proton energy range E p lab = 150 – 400 keV , using a high-purity germanium detector inside a massive lead shield. The measurement took place in the ultra-low-background environment of the Laboratory for Underground Nuclear Astrophysics (LUNA) experiment at the Gran Sasso National Laboratory, leading to a highly increased sensitivity. Results: The uncertainty of the γ branchings and strengths was improved for all four resonances in the studied energy range; many new transitions were observed in the case of the 334 keV resonance, and individual γ decays of the 215 keV resonance were measured for the first time. In addition a number of transitions to intermediate states that decay through α emission were identified. The strengths of the observed resonances are generally in agreement with literature values. Conclusions: Our measurements substantially confirm previous determinations of the relevant resonance strengths. Therefore the O 18 ( p , γ ) F 19 reaction rate does not change with respect to the reaction rate reported in the compilations commonly adopted in the extant computations of red-giant branch and AGB stellar models. Nevertheless, our measurements definitely exclude a nonstandard scenario for the fluorine nucleosynthesis and a nuclear physics solution for the O 18 depletion observed in Group 2 oxygen-rich stardust grains.

Abstract: The presence of non-hexagonal rings in the honeycomb carbon arrangement of graphene produces rippled graphene layers with valuable chemical and physical properties. In principle, a bottom-up approach to introducing distortion from planarity of a graphene sheet can be achieved by careful insertion of curved polyaromatic hydrocarbons during the growth of the lattice. Corannulene, the archetype of such non-planar polyaromatic hydrocarbons, can act as an ideal wrinkling motif in 2D carbon nanostructures. Herein we report an electrochemical bottom-up method to obtain egg-box shaped nanographene structures through a polycondensation of corannulene that produces a new conducting layered material. Characterization of this new polymeric material by electrochemistry, spectroscopy, electron microscopy (SEM and TEM), scanning probe microscopy, and laser desorption-ionization time of flight mass spectrometry provides strong evidence that the anodic polymerization of corannulene, combined with electrochemically induced oxidative cyclodehydrogenations (Scholl reactions), leads to polycorannulene with a wavy graphene-like structure., A bottom-up synthesis of wavy graphene structures obtained through an anodic polymerization process, combined with an electrochemically triggered oxidative cyclodehydrogenation, of the bowl-shaped polyaromatic hydrocarbon corannulene.

A pesar de que no se ha publicado ningún caso previo de nefritis por hipersensibilidad a Anisakis, la clínica, evolución y biopsia fueron compatibles con esta sospecha, sin poderse encontrar otra posible etiología del fracaso renal agudo.

Methods In the displacement group, a sinus surgical fracture results in the distal displacement of the anterior wall by means of an electromagnetic device. In the filling group, a sinus lifting with lateral access and grafting with particulate bone was performed. Bone volume beneath the maxillary sinus was investigated with Computerized Tomography after data superimposition. Clinical and radiological outcomes over 3 years had been evaluated. Results Forty-three dental implants were enrolled. The surgery of both groups significantly increased the bone volume in the VOIs (p-value≤0.0017) respectively for displacement group from 1.17±0.34cc to 1.53±0.39cc with a final bone volume gain of +0.36±0.17cc and for filling group from 1.24±0.41cc to 1.94±0.68cc with a bone augmentation of 0.71±0.31cc. Two implants early failed in the filling group, attesting the 3-year survival rate to 92.6%(CI95%: 82.7%-100%). Marginal bone loss at the distal aspect was 1.66±0.72mm and 1.25±0.78mm for displacement and filling group, respectively (p-value=0.0497). Conclusions The study showed an effective bone gain around dental implants at 3-year of survey both for sinus augmented by backward displacement of the anterior wall (+34%) and by sinus lifting with a lateral window approach (+57%).

Abstract: Direct measurements of reaction cross-sections at astrophysical energies often require the use of solid targets able to withstand high ion beam currents for extended periods of time. Thus, monitoring target thickness, isotopic composition, and target stoichiometry during data taking is critical to account for possible target modifications and to reduce uncertainties in the final cross-section results. A common technique used for these purposes is the Nuclear Resonant Reaction Analysis (NRRA), which however requires that a narrow resonance be available inside the dynamic range of the accelerator used. In cases when this is not possible, as for example the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ reaction recently studied at low energies at the Laboratory for Underground Nuclear Astrophysics (LUNA) in Italy, alternative approaches must be found. Here, we present a new application of the shape analysis of primary $$\gamma $$ rays emitted by the $$^{13}\hbox {C}(\hbox {p},\gamma )^{14}\hbox {N}$$ radiative capture reaction. This approach was used to monitor $$^{13}\hbox {C}$$ target degradation in situ during the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ data taking campaign. The results obtained are in agreement with evaluations subsequently performed at Atomki (Hungary) using the NRRA method. A preliminary application for the extraction of the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ reaction cross-section at one beam energy is also reported.

Abstract: Among the reactions involved in the production and destruction of deuterium during Big Bang Nucleosynthesis, the deuterium-burning D(p,$$\gamma $$)$$^3$$He reaction has the largest uncertainty and limits the precision of theoretical estimates of primordial deuterium abundance. Here we report the results of a careful commissioning of the experimental setup used to measure the cross-section of the D(p,$$\gamma $$)$$^3$$He reaction at the Laboratory for Underground Nuclear Astrophysics of the Gran Sasso Laboratory (Italy). The commissioning was aimed at minimising all sources of systematic uncertainty in the measured cross sections. The overall systematic error achieved ($$< 3\%$$) will enable improved predictions of BBN deuterium abundance.

Abstract: Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m2 and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen. 34. Revisión actualizada del carcinoma de célula pequeña de pulmón. ¿Por qué difiere tanto con otros tumores?

Abstract: INTRODUCCIÓN. El carcinoma de célula pequeña o microcítico es un tumor neuroendocrino muy agresivo, siendo el tumor con mayor mortalidad. El tratamiento de este tumor apenas ha variado, presentando muy buena respuesta quimioterápica al inicio, pero haciéndose resistente al poco tiempo. Al diagnóstico, la mayor parte de los pacientes tiene la enfermedad diseminada. En otros tumores pulmonares los avances en el campo de la biología molecular han dado lugar a efectivas terapias como la inmunoterapia, que están empezando a ser aplicadas a este tumor en diversos estudios. OBJETIVO. El objetivo principal es obtener una revisión actualizada de este tumor, incluyendo los nuevos campos como la inmunoterapia y una revisión de la biología molecular, buscando nuevas posibles terapias y diferenciando la evolución respecto al carcinoma de célula no pequeña en los últimos avances. MATERIAL Y MÉTODOS. Se realizaron 3 búsquedas principales; la primera en diciembre del 2017 en cuatro bases de datos diferentes con un resultado total de 730 artículos. La segunda se realizó para acotar datos y añadir información, en febrero del 2018 con un total de 66 resultados. En abril de 2018 se realizó la última búsqueda para buscar novedades respecto al tema en cuestión y se obtuvieron un total de 241 artículos. Finalmente, se seleccionaron 33, 6 y 14 artículos de las respectivas búsquedas. RESULTADOS. Diversos ensayos están aún en progresión para analizar si realmente la inmunoterapia es una vía factible en este tumor. El tratamiento actual sigue siendo la quimioterapia. La biopsia líquida ofrece una alternativa a la convencional, siendo las células tumorales circulantes una buena vía para ampliar la información que tenemos de este tumor mediante el estudio de la biología molecular de estas células. CONCLUSIONES. A pesar del gran avance en la información que otorga la biología molecular, no se puede decir que actualmente este avance se aprecie en los campos de la terapéutica. A diferencia con otros tumores, en el CPCP la biología molecular y la inmunoterapia, de momento, no han avanzado lo suficiente como para obtener resultados respecto a la mortalidad, lo cual no implica que en un futuro no se puedan obtener nuevas dianas terapéuticas desde estos campos de investigación.

L'elaborazione successiva dei dati è stata affidata a un gruppo multidisciplinare, che li ha messi a confronto con le ricerche già svolte sulla stessa area territoriale e/o sugli stessi settori lavorativi. Il volume tenta quindi di offrire un'analisi allargata a tutte le scienze sociali dei risultati dell'indagine conoscitiva.

Abstract: Background Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage ≥3 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage ≥3 CKD in a large cohort of patients affected by T1DM. Methods A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage ≥3 CKD (eGFR 30% from baseline was evaluated. Results The mean estimated GFR was 98 ± 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions Albuminuria and eGFR reduction represent independent risk factors for incident stage ≥3 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening. 37. Migration and the Environment: Some Reflections on Current Legal Issues and Possible Ways Forward

Abstract: The link between migration and the environment is not new, environmental conditions have always influenced human mobility. However, the nature, the dynamics and the scale of environment related migration have dramatically changed in recent years, and the complex nexus between migration and the environment has drawn increasing attention in the contemporary international context. This relationship is not easy to outline for several reasons, including the range of environmental phe- nomena that may cause migration flows (natural disaster as well as gradual process of environmental deterioration), the difficulty of isolating environmental factors from other political, social and eco- nomic drivers of migration, the complexity of differentiating between forced and voluntary migra- tions in cases of environment-induced flows. In addition, from a different perspective, migratory flows may contribute to environmental degradation in the areas of destination and transit, giving rise to phenomena such as unmanaged urbanization or temporary camps or shelter that produce further pressure on the environment. The complexity of the link between migration and the environment challenges the current legal par- adigm in which migration is largely framed and calls into question the adequacy of the existing pro- tection frameworks. In several international fora, it is increasingly recognized that there are certain groups of people who move for environmental reasons and are in need of assistance and who currently fall outside of the scope of international protection. The goal of the volume on Migration and the Environment: Some Reflections on Current Legal Issues and Possible Ways Forward is to discuss the extent to which people whose movements are induced by environmental factors are protected under the existing international legal framework, to investigate the main legal issues and the normative gaps and to analyze the solutions being discussed in the international arena.

38. INHIBIDORES DEL PUNTO DE CONTROL INMUNOLÓGICO PD-1/PD-L1

Abstract: El cáncer de pulmón constituye la principal causa de muerte por cáncer en todo el mundo. La resección quirúrgica con intención curativa es el tratamiento de elección hoy en día, sin embargo, la mayoría de casos se diagnostican en estadíos avanzados, por lo que el pronóstico es sombrío. Dentro del tratamiento no quirúrgico del cáncer de pulmón, que se basa fundamentalmente en quimioterapia y radioterapia, las técnicas de inmunoterapia están ganando protagonismo en los últimos años. Los inhibidores del complejo PD-1/PD-L1, son unos fármacos recientes, tipo anticuerpos monoclonales humanizados, con resultados prometedores, cuyo mecanismo de acción se basa en inhibir dicho complejo, en especial el receptor PD-1, que es utilizado por las células tumorales para escapar del ataque del sistema inmune y así evitar ser eliminadas. Para que estos fármacos sean eficaces frente al tumor, las células tumorales deben expresar la proteína PD-1 en su membrana en un porcentaje suficiente, ya que si esto no sucede así los fármacos carecen de diana y el tumor escapa a los controles del organismo por otras vías, aún desconocidas. El papel del anatomopatólogo es fundamental para determinar si las células tumorales de la muestra expresan o no dicha proteína y en qué porcentaje (Score de expresión del receptor: alta y baja expresión), constituyendo así el pilar fundamental para el inicio del tratamiento con este tipo de fármacos, que han demostrado un aumento de la supervivencia global, de la tasa de respuesta objetiva y de la supervivencia libre de progresión, en comparación con las terapias convencionales hasta la fecha.

Abstract: The ^{17}O(p,α)^{14}N reaction plays a key role in various astrophysical scenarios, from asymptotic giant branch stars to classical novae. It affects the synthesis of rare isotopes such as ^{17}O and ^{18}F, which can provide constraints on astrophysical models. A new direct determination of the E_{R}=64.5 keV resonance strength performed at the Laboratory for Underground Nuclear Astrophysics (LUNA) accelerator has led to the most accurate value to date ωγ=10.0±1.4_{stat}±0.7_{syst} neV, thanks to a significant background reduction underground and generally improved experimental conditions. The (bare) proton partial width of the corresponding state at E_{x}=5672 keV in ^{18}F is Γ_{p}=35±5_{stat}±3_{syst} neV. This width is about a factor of 2 higher than previously estimated, thus leading to a factor of 2 increase in the ^{17}O(p, α)^{14}N reaction rate at astrophysical temperatures relevant to shell hydrogen burning in red giant and asymptotic giant branch stars. The new rate implies lower ^{17}O/^{16}O ratios, with important implications on the interpretation of astrophysical observables from these stars.

Abstract: The charge-transfer and spectral properties of two octithiophenes, namely 4',4''',3'(v),3(v)'-tetra(octylsulfanyl)-2,2':5',2'':5'',2''':5''',2'(v):5'(v),2(v):5(v),2(v)':5(v)',2(v)''-octithiophene and 4,3'',4(v),4(v)''-tetrabromo-4',4''',3'(v),3(v)'-tetra[(R)-2-methylbutylsulfanyl]-2,2':5',2'':5'',2''':5''',2'(v):5'(v),2(v):5(v),2(v)':5(v)',2(v)''-octithiophene, OT1 and OT2, respectively, are characterized by cyclic voltammetry and spectroelectrochemistry under ultradry conditions. The analysis of the voltammetric results shows the formation of up to the dication for both OT1 and OT2 and up to the tetraanion (OT1) and trianion (OT2) anions. The optical properties of the OT1 (2+, 1+, neutral, 1-, 2-) species were probed by in situ UV-vis-NIR spectroelectrochemistry. The calculated standard potentials at the B3LYP/cc-pVTZ level of the theory allowed the rationalization of the experimental electrochemical results. The UV-vis-NIR spectra were successfully compared with the theoretical electronic transitions and oscillator strength data obtained by time-dependent B3LYP/6-31G* calculations. Theoretical redox potentials and optical transitions properties are calculated including "the solvent effect" within the CPCM model. The consistency obtained between experimental and theoretical results indicates the existence of the hypothesized high-spin/high-charge p- and n-doped electronic states for the OT1 and OT2 octithiophenes here studied.

Abstract: The synthesis and structural, transport and magnetic characterization of pure [La.sub.0.5][Sr.sub.1.5]MnO4 and B-site lightly doped samples are described. It is shown that even a light doping is successful in suppressing the charge and orbital order found in pure [La.sub.0.5][Sr.sub.1.5]MnO4 and no long-range magnetic order was detected in any of the doped samples.

Abstract: The structural, electrical and magnetic characterization of the La(sub 1-x)Na(sub x)Mn(sub 1-y)Ru(sub x)O(sub 3+delta) (LNMRO) system with x = 0.05 and 0.15 and y = 0.005, and 0.15, is reported along with an investigation of the role of the oxygen content on the related redox properties. The analysis shows that ruthenium addition improves electrical and magnetic properties of nonoptimally cation-doped manganites, causing an increase of the Tc value and the insurgence of mangetoresistance effect.

Abstract: Although the desirability of developing synthetic molecular machine systems that can function on surfaces is widely recognized, to date the only well-characterized examples of electrochemically switchable rotaxane-based molecular shuttles which can do so are based on the tetracationic viologen macrocycle pioneered by Stoddart. Here, we report on a [2]rotaxane which features succinamide and naphthalene diimide hydrogen-bonding stations for a benzylic amide macrocycle that can shuttle and switch its net position both in solution and in a monolayer. Three oxidation states of the naphthalene diimide unit can be accessed electrochemically in solution, each one with a different binding affinity for the macrocycle and, hence, corresponding to a different distribution of the rings between the two stations in the molecular shuttle. Cyclic voltammetry experiments show the switching to be both reversible and cyclable and allow quantification of the translational isomer ratios (thermodynamics) and shuttling dynamics (kinetics) for their interconversion in each state. Overall, the binding affinity of the naphthalene diimide station can be changed by 6 orders of magnitude over the three states. Unlike previous electrochemically active amide-based molecular shuttles, the reduction potential of the naphthalene diimide unit is sufficiently positive (−0.68 V) for the process to be compatible with operation in self-assembled monolayers on gold. Incorporating pyridine units into the macrocycle allowed attachment of the shuttles to an acid-terminated self-assembled monolayer of alkane thiols on gold. The molecular shuttle monolayers were characterized by X-ray photoelectron spectroscopy and their electrochemical behavior probed by electrochemical impedance spectroscopy and double-potential step chronoamperometry, which demonstrated that the redox-switched shuttling was maintained in this environment, occurring on the millisecond time scale.

Abstract: Los melanomas de mucosa sinonasal (SNMM) son entidades agresivas e infrecuentes, originadas en melanocitos presentes en el epitelio y estroma de la mucosa respiratoria. El diagnóstico es complicado debido a su morfología, distintos patrones arquitecturales/citológicos y ocasional ausencia de pigmento melánico. La mutación KIT, identificada en melanomas en 2004, esta descrita en aproximadamente 15-30% de los melanomas acrales y mucosos. Su estudio, plantea aprovechar el potencial terapéutico de fármacos inhibidores de la tirosinquinasa. Se revisaron los casos de SNMM, en los servicios de anatomía patológica de los Hospitales Clínico Universitario “Lozano Blesa” de Zaragoza y Complejo Hospitalario de Navarra en Pamplona, entre enero 1990 y junio 2016. Se estudiaron histológico e Inmunohistoquímicamente, incluyendo la técnica CKIT/CD117. Se recopilaron seis casos, cuatro epitelioides, un fusocelular y un plasmocitoide, todos ellos diagnosticados en adultos/ancianos, con características acordes a las descritas en la literatura. La expresión del CKIT (CD117) fue positiva en todos los casos. El tratamiento de elección fue la cirugía en todos los pacientes, algunos con posterior radioterapia. La sobrevida no fue mayor a tres años. Se plantea la utilidad de la tinción inmunohistoquímica como posible complemento al panel diagnóstico, el estudio mediante FISH para confirmar la presencia de la mutación y considerar una probable connotación terapéutica con inhibidores de la tirosinquinasa.

Abstract: Purpose: Proper evaluation of polyphenols intake at the population level is a necessary step in order to establish possible associations with health outcomes. Available data are limited, and so far no study has been performed in people with diabetes. The aim of this work was to document the intake of polyphenols and their major food sources in a cohort of people with type 2 diabetes and in socio-demographic subgroups. Methods: We studied 2573 men and women aged 50–75 years. Among others, anthropometry was measured by standard protocol and dietary habits were investigated by food frequency questionnaire (EPIC). The intake of polyphenols was evaluated using US Department of Agriculture and Phenol-Explorer databases. Results: The mean total polyphenol intake was 683.3 ± 5.8 mg/day. Non-alcoholic beverages represented the main food source of dietary polyphenols and provided 35.5% of total polyphenol intake, followed by fruits (23.0%), alcoholic beverages (14.0%), vegetables (12.4%), cereal products and tubers (4.6%), legumes (3.7%) and oils (2.1%); chocolate, cakes and nuts are negligible sources of polyphenols in this cohort. The two most important polyphenol classes contributing to the total intake were flavonoids (47.5%) and phenolic acids (47.4%). Polyphenol intake increased with age and education level and decreased with BMI; furthermore, in the northern regions of Italy, the polyphenol intake was slightly, but significantly higher than in the central or southern regions. Conclusions: The study documents for the first time the intake of polyphenols and their main food sources in people with diabetes using validated and complete databases of the polyphenol content of food. Compared with published data, collected in people without diabetes, these results suggest a lower intake and a different pattern of intake in people with diabetes. © 2016 Springer-Verlag Berlin Heidelberg

Abstract: The ^{22}Ne(p,γ)^{23}Na reaction takes part in the neon-sodium cycle of hydrogen burning. This cycle affects the synthesis of the elements between ^{20}Ne and ^{27}Al in asymptotic giant branch stars and novae. The ^{22}Ne(p,γ)^{23}Na reaction rate is very uncertain because of a large number of unobserved resonances lying in the Gamow window. At proton energies below 400 keV, only upper limits exist in the literature for the resonance strengths. Previous reaction rate evaluations differ by large factors. In the present work, the first direct observations of the ^{22}Ne(p,γ)^{23}Na resonances at 156.2, 189.5, and 259.7 keV are reported. Their resonance strengths are derived with 2%-7% uncertainty. In addition, upper limits for three other resonances are greatly reduced. Data are taken using a windowless ^{22}Ne gas target and high-purity germanium detectors at the Laboratory for Underground Nuclear Astrophysics in the Gran Sasso laboratory of the National Institute for Nuclear Physics, Italy, taking advantage of the ultralow background observed deep underground. The new reaction rate is a factor of 20 higher than the recent evaluation at a temperature of 0.1 GK, relevant to nucleosynthesis in asymptotic giant branch stars.

Abstract: EPR and optical absorption measurements have been performed on nearly stoichiometric lithium niobate single crystals with iron in traces and in 0.1% molar nominal amount. Thermal treatments in reducing atmosphere allowed to obtain samples with different Fe2+/Fe3+ ratios. The Fe2+/Fe3+ dynamic has been observed by irradiating the samples with UV and visible light in the temperature range 4-300 K: the photochromic effect resulted to be more evident in the strongly reduced crystals. A simple model to explain the photochromic behavior in nearly stoichiometric LiNbO3:Fe is proposed.

Abstract: Sono stati studiati frammenti di vetro colorati provenienti da vetrate, risalenti al XV secolo, del Monastero della Certosa di Pavia. Combinando due tecniche analitiche, microanalisi con sonda elettronica e risonanza paramagnetica, abbiamo determinato la composizione chimica dei campioni e gli stati di ossidazione degli ioni di elementi di transizione, presenti come cromofori, giungendo a chiarirne il ruolo nella colorazione. Importanti contributi sono stati derivati da osservazioni in microscopia elettronica.
