Purpose: The study aimed to evaluate the clinical and tissue response to a hollow polymethylmethacrylate orbital implant with a multiperforated posterior surface in an animal model after evisceration. Methods: Sixteen New Zealand rabbits had their right eye eviscerated. All animals received a hollow polymethylmethacrylate implant 12 mm in diameter that is multiperforated in its posterior hemisphere. The animals were divided into four groups, and each one had the eye exenterated at 7, 30, 90, and 180 days post-evisceration. Clinical signs were assessed daily for 14 days post-evisceration and then every 7 days until 180 days. Inflammatory pattern, collagen structure, and degree of neovascularization generated with implant placement were analyzed with hematoxylin-eosin, picrosirius red, and immunohistochemistry staining. Results: There were no signs of infection, conjunctival or scleral thinning, or implant exposure or extrusion in any animal during the study. On day 7, the new tissue migrated into the implant and formed a fibrovascular network through the posterior channels. Inflammatory response reduced over time, and no multinucleated giant cells were found at any time. Conclusion: Hollow polymethylmethacrylate orbital implants with a multiperforated posterior surface enable rapid integration with orbital tissues by fibrovascular ingrowth. We believe that this orbital implant model can be used in research on humans. RESUMO Objetivo: Avaliar a resposta tecidual e clínica a um implante orbitário de polimetilmetacrilato, oco e multiperfurado em sua porção posterior em modelo animal após evisceração. Métodos: Dezesseis coelhos da raça Nova Zelândia foram submetidos à evisceração do globo ocular direito. Todos receberam implante oco de polimetilmetacrilato de 12 mm de diâmetro, multiperfurado em sua semiesfera posterior. O estudo foi dividido em avaliação clínica e histopatológica. A avaliação clínica foi diária até 14 dias pós-evisceração e, a cada sete dias, até completar 180 dias. Os animais foram divididos em grupos de quatro animais e cada um foi submetido à exenteração com 07, 30, 90 e 180 dias e depois à eutanásia. A análise histopatológica teve por fim caracterizar o padrão inflamatório, a estrutura do colágeno e o grau de neovascularização. Para isso, além da tradicional coloração pela hematoxilina-eosina, utilizou-se o corante Picrosirius Red (PSR) e imuno-histoquímica com o marcador CD 34. Resultados: Não houve sinais de infecção, afinamento conjuntival ou escleral, exposição ou extrusão do implante em nenhum animal durante o estudo. Já no sétimo dia, o tecido neoformado migrou para dentro do implante formando uma rede fibrovascular através dos canais posteriores. A resposta inflamatória diminuiu ao longo do tempo avaliado e não foram encontradas células gigantes multinucleadas. Conclusão: O implante analisado permite a sua integração aos tecidos orbitários pelo crescimento fibrovascular em seu interior. Os autores acreditam que este modelo de implante orbital pode fazer parte de testes com humanos.

Plasmablastic lymphoma (PBL) is a rare B-cell lymphoma occurring mainly in HIV patients. The tumor frequently involves extranodal sites such as the oral cavity, nasal cavity, gastrointestinal tract, skin, and lungs. The neoplastic cells are characterized by a plasmablastic appearance and typical immunophenotype that indicates plasma cell differentiation. Herein, we report a case of intraocular involvement by plasmablastic lymphoma in a HIV patient with a long history of cytomegalovirus retinitis and loss of vision. After an evisceration performed to control the blind, painful eye a diagnosis of PBL was made by examining the eye contents. Two months later, a biopsy of cervical lymph node revealed nodal lymphoma of the same type. To the best of our knowledge, this is the second case of intraocular PBL reported in English literature.

Posterior scleral tuberculoma formation is an extremely rare condition. The few reports on scleral involvement in tuberculosis refer to cases of anterior scleritis. In the present manuscript we describe a patient who had rheumatoid arthritis and developed a large posterior scleral tuberculoma. The lesion provoked retinal detachment and visual loss and was diagnosed only after enucleation due to a misdiagnosis of choroidal melanoma. A formação de granuloma tuberculoso na esclera posterior é um evento extremamente raro. Os poucos relatos de acometimento escleral na tuberculose referem-se a casos de esclerite anterior. No presente trabalho é descrito um caso de granuloma escleral posterior em um paciente portador de artrite reumatóide. A lesão provocou descolamento da retina e perda visual e só foi diagnosticada após enucleação por suspeita de melanoma de coróide.

A 68-year-old man presented with a history of a right optic glioma. Eighteen months ago he underwent a lateral orbitotomy at another institution for removal of an optic nerve mass. At that time histology revealed that the tumor was an optic nerve glioma with a pilocytic pattern. No further treatment was instituted and one year after surgery he noticed that his right eye was proptotic again. Magnetic resonance imaging of the orbit showed that his right orbit was almost completely filled with a mass which extended through the optic canal to the chiasma. The tumor was excised by a combined neurosurgical and orbital approach. Histology proved that the neoplasm was a low grade pilocytic astrocytoma of the optic nerve. Paciente masculino de 68 anos com história de orbitotomia lateral para exérese de tumor no nervo óptico 18 meses antes em outro serviço. O exame histológico demonstrou glioma do nervo óptico com padrão pilocítico. Um ano após, observou-se novo episódio de proptose no olho direito. Ressonância nuclear magnética das órbitas mostrou massa preenchendo quase toda a cavidade orbitária direita com extensão pelo canal óptico até o quiasma. Foi realizada exenteração da órbita direita com excisão da porção posterior do tumor via transcraniana pela neurocirurgia e reconstrução orbitária. A histologia confirmou astrocitoma pilocítico de baixo grau do nervo óptico.

INTRODUÇÃO: Embora mutações no BRCA1 sejam raras nos carcinomas mamários não-familiares, é comum haver perda de expressão da proteína codificada por esse gene. A importância prognóstica desse achado não está bem estabelecida. OBJETIVOS: analisar a expressão imuno-histoquímica do BRCA1 em carcinomas mamários esporádicos e comparar os resultados obtidos com dados clínicos e histopatológicos de importância prognóstica. MATERIAL E MÉTODO: 102 carcinomas ductais sem outras especificações foram submetidos ao método imuno-histoquímico com os anticorpos BRCA1, RE, RP, p53, c-erbB-2 e Ki67. Os dados clínicos foram obtidos dos prontuários médicos. RESULTADOS: No epitélio normal, o BRCA1 apresentou apenas marcação nuclear, enquanto os carcinomas apresentaram tanto marcação nuclear quanto a citoplasmática. Os carcinomas com baixa expressão imuno-histoquímica do BRCA1 foram pouco diferenciados, com alta taxa proliferativa, negativos para receptores hormonais e positivos para c-erbB-2. Não houve correlação da expressão do BRCA1 com idade, estado menstrual, tamanho tumoral, acometimento de linfonodos, estadiamento patológico e expressão do p53. As sobrevidas média e livre de doença das pacientes cujos tumores expressaram menos e mais de 25% de marcação para o BRCA1 nas células neoplásicas foram de 4,2 e 3,9; e 7,8 e 6,3 anos, respectivamente. BACKGROUND: Although mutations in BRCA are rare in non familiar breast carcinomas, it is common the lost of expression of the protein codified by this gene. The prognostic importance of this finding is not well established. OBJECTIVES: to analyze the immunohistochemistry expression of BRCA1 in sporadic breast carcinomas and to compare the results with clinical and histopathologic data of prognostic significance. MATERIAL AND METHOD: 102 NOS ductal carcinomas were submitted to the immunohistochemistry method with the antibodies BRCA1, ER, PR, p53, c-erbB-2 and Ki67. The clinical data were retrieved from medical files. RESULTS: In normal epithelium, BRCA1 showed only nuclear staining, while the carcinomas showed either nuclear or cytoplasmatic staining. Carcinomas with reduced expression of BRCA1 were poorly differentiated, with high proliferative rate, negative for hormonal receptors, and positive for c-erbB-2. There was not correlation between BRCA1 expression with age, menstrual status, tumoral size, lymph nodes status, pathologic stage, and p53 expression. The mean survival and disease-free survival of patients which tumors expressed less and more of 25% of neoplastic cells stained for BRCA1 were 4.2 and 3.9; and 7.8 and 6.3 years respectively. CONCLUSIONS: BRCA1 may be considered a useful marker in identification of sporadic carcinomas of poor prognosis that is in high risk for precocious recurrence.

Os autores relatam 2 casos de pacientes com melanoma em cavidade anoftálmica secundária a eviscerações ocorridas há 30 e 60 anos. Em ambos os casos a análise histopatológica mostrou que o tumor estava aderido a remanescentes esclerais. A implicação desses casos foi discutida no contexto das indicações de evisceração e enucleação. We report 2 cases of malignant melanoma in anophthalmic sockets of patients who had undergone eviscerations 30 and 60 years ago. The histopathologic analysis showed that the tumors were adherent to scleral remnants. The implications of these cases were discussed in the context of the indications of evisceration and enucleation.

Em crianças, as inflamações orbitárias idiopáticas são extremamente raras e de difícil diagnóstico. O presente trabalho tem por objetivo descrever um caso de inflamação orbitária idiopática aguda em criança de um ano e um mês de idade, que evoluiu com importante seqüela oculomotora. Além do estudo de caso, discute-se o diagnóstico diferencial entre a celulite orbitária e as inflamações orbitárias idiopáticas e faz-se uma revisão da literatura sobre a ocorrência das inflamações orbitárias idiopáticas na infância. In children, idiopathic orbital inflammations are extremely rare and difficult to diagnose. The present study describes one case of idiopathic orbital inflammation in a one-year-old child that led to important oculomotor impairment. The differential diagnosis between idiopathic orbital inflammation and orbital cellulitis is discussed and the literature on the idiopathic orbital inflammation in childhood is reviewed.

OBJETIVO: Relatar o caso de paciente de 17 anos de idade com granuloma eosinofílico (GE) na parede superior da órbita com regressão espontânea após biópsia. MÉTODOS: Relato de caso. RESULTADOS: Remissão espontânea após biópsia incisional, via palpebral, de lesão de teto orbitário, sem evidências clínicas e tomográficas de recorrência com seguimento de 22 meses. A análise histopatológica da biópsia confirmou granuloma de aspecto proliferativo apresentando macrófagos, eosinófilos e células de Langerhans sem características neoplásicas. CONCLUSÃO: A remissão espontânea do caso descrito levanta a hipótese de que seguimento cuidadoso de pacientes com GE orbitário, confirmado por biópsia incisional, com exames de controle rotineiros, pode ser uma maneira menos invasiva e eficaz de abordagem. PURPOSE: To report a case of a 17-year-old patient who presented with spontaneuos remission of an eosinophilic granuloma (EG) of the superior orbital wall after biopsy. METHODS: Case report. RESULTS: An orbital roof lesion was biopsied through lid crease approach. The histopathological study showed a granuloma with a proliferative aspect with macrophages, eosinophils and Langerhans' cells, without neoplastic features. After biopsy there was spontaneous remission demonstrated by computerized tomography scans of the orbit on a 22-month follow-up period. CONCLUSION: This case highlights that spontaneous remission of unifocal eosinophilic granuloma should be considered in the management of these lesions.
