Purpose This study aims to investigate the clinical and molecular differences between diffuse large B-cell lymphoma (DLBCL) patients with MYD88L265P and MYD88other. Methods DLBCL patients with MYD88 variations were collected from the Cancer Hospital, Chinese Academy of Medical Sciences & Peking Union Medical College (CHCAMS), and Suzhou Municipal Hospital from February 6th, 2007 to May 20th, 2022. Clinicopathological parameters and treatment outcomes between MYD88L265P and MYD88other were investigated. Results A total of 132 patients with MYD88 variations from a cohort of 475 DLBCL patients were included, among which, 78 were MYD88L265P, while 54 were MYD88other. MYD88L265P was more common in non-GCB subtype than MYD88other (83% vs. 60%, P = 0.004). Besides, MYD88L265P was significantly related to higher proportion of testicle/ central nervous system involvement (31% vs. 6%, P P P = 0.018), compared with MYD88other. Compared with MYD88L265P, MYD88other were more likely to have higher percentage of advanced stage (60% vs. 42%, P = 0.044), extranodal site ≥ 2 (45% vs. 28%, P = 0.044), elevated LDH (55% vs. 35%, P = 0.033), positive CD10 expression (36% vs. 16%, P = 0.009), BCL-6 translocation (20% vs. 8%, P = 0.033), and NOTCH pathway gene alteration (24% vs. 13%, P = 0.040). In non-GCB DLBCL subtype, patients with MYD88other were significantly associated with worse progression free survival (PFS) than those with MYD88L265P when treated initially with R-CHOP/R-CHOP-like regimen (P = 0.010). Conclusion The findings of this study indicate that DLBCL patients with MYD88L265P and MYD88other are likely to be two subgroups with different clinical and molecular characteristics. The survival of patients with MYD88other is not superior than those with MYD88L265P, even poorer when focusing on the non-GCB subtype.

Objective: Limited data about the prognostic significance of BCL2 mutations and BCL2 copy number variations in diffuse large B-cell lymphoma (DLBCL) are available. This study aimed to comprehensively describe BCL2 genetic alterations in DLBCL patients, and examine correlation of BCL2, TP53 and other genetic alterations with outcomes in patients treated with R-CHOP. Methods: Probe capture-based high-resolution sequencing was performed on 191 patients diagnosed with de novo DLBCL. MYC, BCL2, and BCL6 protein expressions were detected by immunohistochemistry. Results: The presence of BCL2 alterations significantly correlated with poor progression-free survival (PFS) (5-year PFS: 13.7% vs. 40.8%; P = 0.003) and overall survival (OS) (5-year OS: 34.0% vs. 70.9%; P = 0.036). Importantly, patients who harbored BCL2 gain/amplifications (BCL2GA/AMP) also had a remarkably inferior 5-year PFS (11.1% vs. 38.3%; P < 0.001) and OS (22.1% vs. 69.6%; P = 0.009). In contrast, neither BCL2 mutations nor BCL2 translocations were significantly prognostic for survival. Multivariable analyses showed that the presence of BCL2 alterations, especially BCL2GA/AMP, TP53 mutations, and International Prognostic Index (IPI) were significantly associated with inferior PFS and OS. Novel prognostic models for OS were constructed based on 3 risk factors, including BCL2 alterations (Model 1) or BCL2GA/AMP (Model 2), TP53 mutations, and IPI, to stratify patients into 4 risk groups with different survival outcomes. Conclusions: This study showed that DLBCL patients treated with R-CHOP, BCL2 alterations, especially BCL2GA/AMP and TP53 mutations were significantly associated with inferior outcomes, which were independent of the IPI. The novel prognostic models we proposed predicted outcomes for DLBCL patients treated with R-CHOP, but further validation of the prognostic models is still warranted.

This paper presents an image fusion network based on a special residual network and attention mechanism. Compared with the traditional fusion network, the image fusion network has the advantages of an end-to-end network and integrates the feature extraction advantages of the attention mechanism residual network. It overcomes the shortcomings of the traditional network that need complex design rules and manual operation. In this method, hierarchical feature fusion is used to achieve effective fusion. A combined loss function is designed to optimize training results and improve image fusion quality. This paper uses many qualitative and quantitative experimental analyses on different data sets. The results show that, compared with the comparison algorithm, the method in this paper has a stronger retention ability of infrared and visible light information and better indexes. 72% of eleven indexes compared with some images in the public TNO data set are optimal or sub-optimal, and 80% are optimal or suboptimal in the RoadScene data set, which is much higher than other algorithms. The overall fusion effect is more in line with human visual perception.

Harmful algal blooms (HABs) are major ecological and environmental problems in China's coastal waters and seriously threaten the stability of the marine ecosystem and human health. Gymnodinium catenatum is a toxic red tide dinoflagellate. It can produce paralytic shellfish toxins (PSP), which cause serious hazards to marine organisms, public health and safety. In this paper, a test strip based on colloidal gold immunochromatography (GICG) was developed for the rapid detection of Gymnodinium catenatum. The experimental results showed that the test trip has good specificity and sensitivity. It not only detects the different components of Gymnodinium catenatum but also may detect algal toxins. The lowest density of Gymnodinium catenatum that can be detected by this test strip is approximately 120 cells/mL. Cross-reaction indicated that the test strip had a high specificity for Gymnodinium catenatum. This test strip provides a rapid method for on-site detection of Gymnodinium catenatum and a reference method for the monitoring of other harmful algae to serve as an early warning of upcoming red tides. It also provides a new way to prepare more detection methods for toxic algal toxins.

Background: The clinical course of relapsed or refractory (r/r) diffuse large B-cell lymphoma (DLBCL) is variable, with limited efficacy data of second-line treatment in a post-rituximab real-world context. Hence, we explored the predictors and constructed a nomogram for risk stratification in this population. Patients and methods: Among 296 r/r DLBCL patients pretreated with R-CHOP (rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone) at the Cancer Hospital, Chinese Academy of Medical Sciences & Peking Union Medical College between 2006 and 2017, 231 were included for nomogram construction. After randomization, we constructed the prognostic nomogram in the primary cohort (n=161) based on a multivariate analysis and confirmed it in the validation cohort (n=70). Additionally, we explored predictive factors for second-line therapy using a ordinal regression analysis. Results: Four independent prognostic factors including rituximab in the second-line setting, initial Eastern Cooperative Oncology Group (ECOG) performance status (PS), response to front-line treatment, and invasion on progression/recurrence were used to construct the nomogram. The nomogram had a C index of 0.70 with AUC values of 0.73 and 0.71 for the primary and validation cohorts, respectively. Subsequently, three risk groups (low, intermediate, and high) were determined with median overall survival (OS) of 38.0, 25.0, and 7.0 months, respectively. Additionally, we conducted a multivariate ordinal regression analysis and identified prior response to front-line treatment (odds ratio=4.50, 95% CI: 1.84-11.27, p=0.001) and bulky disease at diagnosis (odds ratio=0.36, 95% CI: 0.182-0.702, p=0.003) were two independent factors for second-line treatment efficacy. Conclusions: The established predictors for treatment efficacy and the novel nomogram for survival in r/r DLBCL patients could potentially be applied for risk stratification and treatment guidance in the post-rituximab era.

Background: To investigate the prognostic role of red blood cell distribution width (RDW) and platelet/lymphocyte ratio (PLR) in early-stage classical Hodgkin lymphoma (cHL). Materials & methods: Data from 402 patients with newly diagnosed early-stage cHL were retrospectively collected. The impact of factors on complete response (CR) rate and freedom from progression (FFP) was analyzed. Results: High PLR was associated with lower CR, but high RDW was not. The univariate analysis showed that RDW and PLR were predictive of FFP. On multivariate analysis, high PLR was an independent risk factor for inferior FFP. Subgroup analysis and a prognostic model for FFP based on PLR validated the prognostic role of PLR. Conclusion: PLR was a robust prognostic factor for newly diagnosed early-stage cHL.

Graph convolutional networks have significantly improved 3D human pose estimation by representing the human skeleton as an undirected graph. However, this representation fails to reflect the articulated characteristic of human skeletons as the hierarchical orders among the joints are not explicitly presented. In this paper, we propose to represent the human skeleton as a directed graph with the joints as nodes and bones as edges that are directed from parent joints to child joints. By so doing, the directions of edges can explicitly reflect the hierarchical relationships among the nodes. Based on this representation, we further propose a spatial-temporal conditional directed graph convolution to leverage varying non-local dependence for different poses by conditioning the graph topology on input poses. Altogether, we form a U-shaped network, named U-shaped Conditional Directed Graph Convolutional Network, for 3D human pose estimation from monocular videos. To evaluate the effectiveness of our method, we conducted extensive experiments on two challenging large-scale benchmarks: Human3.6M and MPI-INF-3DHP. Both quantitative and qualitative results show that our method achieves top performance. Also, ablation studies show that directed graphs can better exploit the hierarchy of articulated human skeletons than undirected graphs, and the conditional connections can yield adaptive graph topologies for different poses.

Purpose This study aimed to develop a prognostic nomogram in diffuse large B-cell lymphoma (DLBCL) and compare it with traditional prognostic systems. Materials and methods We included 1,070 consecutive and nonselected patients with DLBCL in the National Cancer Center/National Clinical Research Center for Cancer/Cancer Hospital, Chinese Academy of Medical Sciences, between 2006 and 2012. A nomogram based on the Cox proportional hazards model was developed. Results The entire group were divided into the primary (n = 748) and validation (n = 322) cohorts. The 5-year overall survival (OS) rate was 64.1% for the entire group. Based on a multivariate analysis of the primary cohort, seven independent prognostic factors including age, Ann Arbor stage, Eastern Cooperative Oncology Group performance status score, lactate dehydrogenase, β2-microglobulin, CD5 expression, and Ki-67 index were identified and entered the nomogram. The calibration curve showed the optimal agreement between nomogram prediction and actual observation. In addition, the concordance index (C-index) of the nomogram for OS prediction was 0.77 (95% confidence interval [CI], 0.73–0.81) in the primary cohort and 0.76 (95% CI, 0.70–0.81) in the validation, superior to that of the international prognostic index (IPI), revised IPI (R-IPI), and National Comprehensive Cancer Network (NCCN)-IPI (range, 0.69–0.74, p Conclusions The proposed nomogram provided an accurate estimate of survival of patients with DLBCL, especially for those receiving R-CHOP or R-CHOP-like regimens, allowing clinicians to optimized treatment plan based on individualized risk prediction. Implications for Practice A diffuse large B-cell lymphoma (DLBCL)-specific prognostic nomogram was developed based on Chinese patients with DLBCL. As a tertiary hospital, National Cancer Center/Cancer Hospital, Chinese Academy of Medical Sciences is the number 1 ranked cancer center in China, with more than 800,000 outpatients in 2018. Patients included in this study were nonselected and came from 29 different provinces, municipalities, and autonomous regions in China. Thus, the data is believed to be representative to an extent.

Despite the great success of GANs in images translation with different conditioned inputs such as semantic segmentation and edge maps, generating high-fidelity realistic images with reference styles remains a grand challenge in conditional image-to-image translation. This paper presents a general image translation framework that incorporates optimal transport for feature alignment between conditional inputs and style exemplars in image translation. The introduction of optimal transport mitigates the constraint of many-to-one feature matching significantly while building up accurate semantic correspondences between conditional inputs and exemplars. We design a novel unbalanced optimal transport to address the transport between features with deviational distributions which exists widely between conditional inputs and exemplars. In addition, we design a semantic-activation normalization scheme that injects style features of exemplars into the image translation process successfully. Extensive experiments over multiple image translation tasks show that our method achieves superior image translation qualitatively and quantitatively as compared with the state-of-the-art., Comment: Accepted to CVPR 2021

Inferring the scene illumination from a single image is an essential yet challenging task in computer vision and computer graphics. Existing works estimate lighting by regressing representative illumination parameters or generating illumination maps directly. However, these methods often suffer from poor accuracy and generalization. This paper presents Geometric Mover's Light (GMLight), a lighting estimation framework that employs a regression network and a generative projector for effective illumination estimation. We parameterize illumination scenes in terms of the geometric light distribution, light intensity, ambient term, and auxiliary depth, which can be estimated by a regression network. Inspired by the earth mover's distance, we design a novel geometric mover's loss to guide the accurate regression of light distribution parameters. With the estimated light parameters, the generative projector synthesizes panoramic illumination maps with realistic appearance and high-frequency details. Extensive experiments show that GMLight achieves accurate illumination estimation and superior fidelity in relighting for 3D object insertion. Dealing with the inconsistency between a foreground object and a background image is a challenging task in high-fidelity image composition. State-of-the-art methods strive to harmonize the composed image by adapting the style of foreground objects to be compatible with the background image, whereas the potential shadow of foreground objects within the composed image which is critical to the composition realism is largely neglected. In this paper, we propose an Adversarial Image Composition Net (AIC-Net) that achieves realistic image composition by considering potential shadows that the foreground object projects in the composed image. A novel branched generation mechanism is proposed, which disentangles the generation of shadows and the transfer of foreground styles for optimal accomplishment of the two tasks simultaneously. A differentiable spatial transformation module is designed which bridges the local harmonization and the global harmonization to achieve their joint optimization effectively. Extensive experiments on pedestrian and car composition tasks show that the proposed AIC-Net achieves superior composition performance qualitatively and quantitatively.

T cell lymphoblastic lymphoma (T-LBL) has an aggressive clinical behavior. To date, powerful and consistent prognostic factors have not been established for T-LBL. In this study, we first evaluated the association of event-free survival (EFS) at 24 months (EFS24) with overall survival (OS) in T-LBL patients. Besides, we sought to identify clinical factors of prognostic importance in this rare entity. Between January 2006 and December 2017, ninety-one patients with newly diagnosed T-LBL were retrospectively analyzed. EFS was defined as the time from diagnosis to relapse or progression, unplanned retreatment, death from any cause, or to the last follow-up. In total, 91 patients with a median age of 24 years were enrolled. At a median follow-up of 40.4 months (range, 1.4 to 163.3 months), the 5-year OS and EFS was 47.9% and 43.2%, respectively. Of all patients, 45 (49.5%) achieved EFS24 and 46 (50.5%) did not. Illumination estimation from a single image is critical in 3D rendering and it has been investigated extensively in the computer vision and computer graphic research community. On the other hand, existing works estimate illumination by either regressing light parameters or generating illumination maps that are often hard to optimize or tend to produce inaccurate predictions. We propose Earth Mover Light (EMLight), an illumination estimation framework that leverages a regression network and a neural projector for accurate illumination estimation. We decompose the illumination map into spherical light distribution, light intensity and the ambient term, and define the illumination estimation as a parameter regression task for the three illumination components. Motivated by the Earth Mover distance, we design a novel spherical mover's loss that guides to regress light distribution parameters accurately by taking advantage of the subtleties of spherical distribution. Under the guidance of the predicted spherical distribution, light intensity and ambient term, the neural projector synthesizes panoramic illumination maps with realistic light frequency. Extensive experiments show that EMLight achieves accurate illumination estimation and the generated relighting in 3D object embedding exhibits superior plausibility and fidelity as compared with state-of-the-art methods., Comment: Accepted to AAAI 2021

Extranodal natural killer (NK)/T cell lymphoma, nasal type (ENKTL) is an aggressive non-Hodgkin lymphoma and the majority of ENKTL cases are diagnosed at the localized stage. Radiotherapy in combination with chemotherapy has been used for localized ENKTL, but the optimal combination treatment modality and the best first-line chemotherapy regimen have not been defined. In this retrospective study, 44 patients with newly diagnosed, stages I/II ENKTL were enrolled and received intensity-modulated radiation therapy (IMRT, 50-56 Gy) followed by GDP (gemcitabine, dexamethasone, and cisplatin) chemotherapy. The median number of chemotherapy cycles per patient was 4 (range, 2-6 cycles). At the end of treatment, the overall response rate was 95% (42/44), including 39 patients (89%) who attained complete response. Two patients developed systemic progression after IMRT. With a median follow-up of 37.5 months, the 3-year overall survival (OS) rate and progression-free survival (PFS) rate were 85% (95% CI, 74 to 96%) and 77% (95% CI, 64 to 91%), respectively. Locoregional and systemic failure rates for this treatment were 9% (4/44) and 14% (6/44), respectively. The most common grades 3 to 4 adverse events included leukopenia (37%), neutropenia (34%), and mucositis (25%). No treatment-related deaths were observed. This study suggested high efficacy and low toxicity of IMRT followed by GDP regimen chemotherapy for newly diagnosed stage I/II ENKTL patients. These results require further investigation in prospective trials.

The univariate analysis indicated that Ann Arbor Stage ( p = 0.008), Eastern Cooperative Oncology Group score (ECOG) ( p = 0.009), International Prognostic Index (IPI) ( p = 0.023), βLRI ( p = 0.003), LLRI ( p = 0.04), neutrophil-lymphocyte ratio index ( p = 0.025) and monocyte/granulocyte to lymphocyte ratio ( p = 0.030) were significantly associated with overall survival (OS) in ENKTL patients. However, multivariate analysis demonstrated that only Ann Arbor Stage ( p = 0.028), βLRI ( p < 0.001) and LLRI ( p = 0.006) were only correlated independently with OS. Furthermore, βLRI and LLRI based new prognostic model showed improved discrimination for stage IE/IIE upper aerodigestive tract in ENKTL patients than IPI and Korean Prognostic Index. Overall, our study concluded that new βLRI-based prognosis model is useful to stratify ENKTL patients and higher βLRI and LLRI can act as independent prognostic predictor candidates in early stage ENKTL.

The recent person re-identification research has achieved great success by learning from a large number of labeled person images. On the other hand, the learned models often experience significant performance drops when applied to images collected in a different environment. Unsupervised domain adaptation (UDA) has been investigated to mitigate this constraint, but most existing systems adapt images at pixel level only and ignore obvious discrepancies at spatial level. This paper presents an innovative UDA-based person re-identification network that is capable of adapting images at both spatial and pixel levels simultaneously. A novel disentangled cycle-consistency loss is designed which guides the learning of spatial-level and pixel-level adaptation in a collaborative manner. In addition, a novel multi-modal mechanism is incorporated which is capable of generating images of different geometry views and augmenting training images effectively. Background The expression and significance of CD276 in non-small cell lung cancer (NSCLC) was explored. Method The BioGPS database was used to analyze the expression level of CD276 in normal tissues. Studies on the expression of CD276 in NSCLC patients using the Oncomine database. The prognostic roles of CD276 in NSCLC was studied using the Kaplan-Meier plotter database. Result The BioGPS database showed CD276 expression in all the human normal tissues. Compared with normal lung tissue, CD276 gene highly expressed in NSCLC tissue at mRNA level (P Conclusion According to the information mined from the tumor gene database, CD276 mRNA was found highly expressed in NSCLC tissue and the expression of CD276 has a significant impact on survival of NSCLC patients, which provides an important theoretical basis for further study of the role of CD276 in the occurrence and development of NSCLC.

Objective To explore the clinicobiologic features and outcomes of diffuse large B-cell lymphoma (DLBCL) patients in China according to the primary site. Methods A total of 1,085 patients diagnosed with DLBCL in National Cancer Center/Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College during a 6-year period were enrolled. Their clinical characteristics and outcomes were analyzed according to the primary site. Results In the 1,085 patients, 679 (62.6%) cases were nodal DLBCL (N-DLBCL) and 406 cases (37.4%) were extranodal DLBCL (EN-DLBCL). The most common sites of N-DLBCL were lymphonodus (64.8%), Waldeyer's ring (19.7%), mediastinum (12.8%) and spleen (2.7%), while in EN-DLBCL, stomach (22.4%), intestine (16.0%), nose and sinuses (8.9%), testis (8.4%), skin (7.9%), thyroid (6.9%), central nervous system (CNS) (6.4%), breast (5.7%), bone (3.4%), and salivary gland (2.7%) were most common. N-DLBCL patients tend to present B symptoms, bulky disease, and elevated LDH more often, while age >60 years, extranodal sites >1, Ann Arbor stage I or II, bone marrow involvement, and Ki-67 index >90% were usually seen in EN-DLBCL. The 5-year overall survival (OS) rate and progression-free survival (PFS) rate for all patients were 62.5% and 54.2%. The 5-year OS rate for patients with N-DLBCL and EN-DLBCL were 65.5% and 56.9% (P=0.008), and the 5-year PFS were 57.0% and 49.0% (P=0.020). Waldeyer's ring originated DLBCL possessed the highest 5-year OS rate (83.6%) and PFS rate (76.9%) in N-DLBCL. The top five EN-DLBCL subtypes with favorable prognosis were stomach, breast, nose and sinuses, lung, salivary gland, with 5-year OS rate: 70.3%, 69.6%, 69.4%, 66.7% and 63.6%, respectively. While CNS, testis, oral cavity and kidney originated EN-DLBCL faced miserable prognosis, with 5-year OS rate of 26.9%, 38.2%, and 42.9%. Conclusions In our study, primary sites were associated with clinical characteristics and outcomes. Compared with EN-DLBCL, N-DLBCL had better prognosis.

Video contents have become a critical tool for promoting products in E-commerce. However, the lack of automatic promotional video generation solutions makes large-scale video-based promotion campaigns infeasible. The first step of automatically producing promotional videos is to generate visual storylines, which is to select the building block footage and place them in an appropriate order. This task is related to the subjective viewing experience. It is hitherto performed by human experts and thus, hard to scale. To address this problem, we propose WundtBackpack, an algorithmic approach to generate storylines based on available visual materials, which can be video clips or images. It consists of two main parts, 1) the Learnable Wundt Curve to evaluate the perceived persuasiveness based on the stimulus intensity of a sequence of visual materials, which only requires a small volume of data to train; and 2) a clustering-based backpacking algorithm to generate persuasive sequences of visual materials while considering video length constraints. In this way, the proposed approach provides a dynamic structure to empower artificial intelligence (AI) to organize video footage in order to construct a sequence of visual stimuli with persuasive power. The prognostic value of pretreatment serum beta-2 microglobulin (B2MG) level in advanced Hodgkin lymphoma (HL) patients treated in the modern era has not been well established. We conducted a retrospective study involving 202 advanced classical HL (cHL) patients treated from 1998.5 to 2015.7 to evaluate the impact of serum B2MG level on prognosis. Multivariate analysis showed that serum B2MG level ≥ 2.5 mg/L was an independent predictor for freedom from progression (FFP) (P = 0.001), lymphoma-specific survival (P = 0.030) and overall survival (P = 0.034). The 5-year FFP of patients with serum B2MG level ≥ 2.5 mg/L was 66.8%, compared with 89.7% in patients with B2MG level < 2.5 mg/L (P < 0.001). The traditionally used International Prognostic Score (IPS) remained prognostic for FFP (P = 0.013) but the predictive range narrowed, with 5-year FFP ranging from 90.9% to 62.3%. The 5-year FFP of the 44 patients with both IPS ≥ 3 and serum B2MG ≥ 2.5 mg/L was 50.7%, which was significantly worse than that of the 87 patients with only one of the two factors (81.9%, P < 0.001) or the 71 patients with both B2MG < 2.5 mg/L and IPS < 3 (91.1%, P < 0.001). The difference of FFP between the latter two groups was smaller but also significant (P = 0.038). In summary, our data suggest pretreatment serum B2MG level ≥ 2.5 mg/L was an independent unfavorable prognostic factor in advanced cHL patients treated in the modern era. It improves IPS in predicting the outcomes as the combination of IPS and B2MG indentified a wider prognostic range than IPS alone with a sizable number of patients in different risk groups.

Background To evaluate the antitumor activity and toxicity of icotinib as initial treatment in lung adenocarcinoma patients with brain metastases. Methods Twenty-one patients with histologically or pathologically documented brain metastatic lung cancer were administered icotinib as initial treatment from 2011 to 2015 at the Cancer Institute and Hospital, Chinese Academy of Medical Sciences. Chemotherapy response was assessed by Response Evaluation Criteria in Solid Tumors and toxicity was evaluated according to National Cancer Institute-Common Toxicity Criteria. Icotinib was administered three times per day at a dose of 125mg. Results The median overall and progression-free survival rates were 15.2 (1.2–31.5 months, 95% confidence interval [CI] 6.6–23.7 months) and 8.9 months (0.6–30.5 months, 95% CI 3.4–14.3 months), respectively. The overall response and disease control rates were 61.9% and 90.5%, respectively. Icotinib was well tolerated, and no grade 3/4 adverse events were observed. The most common grade 1/2 adverse events included acneiform eruptions (38.1%), diarrhea (19.0%), and stomatitis (9.5%). Conclusion Icotinib is effective and well tolerated as initial treatment in lung adenocarcinoma patients with brain metastases.

Background: High-dose chemotherapy followed by autologous stem cell transplantation (ASCT) is a promising approach for lymphomas. This study aimed to evaluate the effect of ifosfamide, cisplatin or carboplatin, and etoposide (ICE)-based regimen as a mobilization regimen on relapsed, refractory, or high-risk aggressive lymphoma. Methods: From June 2001 to May 2013, patients with lymphomas who mobilized by ICE-based regimen for ASCT were analyzed in this retrospective study. The results of the autologous peripheral blood stem cells collection, toxicity, engraftment after ICE-based mobilization regimen were analyzed in this study. Furthermore, risk factors for overall survival (OS) and progression free survival (PFS) were evaluated by univariate analysis. Results: The stem cells were mobilized using ICE-based regimen plus rituximab or ICE-based regimen alone in 12 patients and 54 patients, respectively. The results of stem cell mobilization were excellent. Ninety-seven percentages of the patients had the stem cell collection of at least 2.0 × 10 6 CD34 + cells/kg and 68% had at least 5 × 10 6 CD34 + cells/kg. Fifty-eight percentage of the patients experienced Grade 4 neutropenia, 20% developed febrile neutropenia, and only 12% had Grade 4 thrombocytopenia. At a median follow-up of 63.8 months, the 5-year PFS and OS were 64.4% and 75.3%, respectively. Conclusion: ICE is a powerful regimen for stem cell mobilization in patients with lymphomas.

Standard therapeutic options for patients with relapsed or refractory peripheral T cell lymphoma—not otherwise specified (PTCL—NOS) remain unclear. There are few large cohort studies specifically focused on gemcitabine-based chemotherapy for PTCL—NOS. We retrospectively reviewed patients with relapsed or refractory PTCL—NOS who received salvage GDP (gemcitabine, dexamethasone, and cisplatin) chemotherapy at the Cancer Hospital, Chinese Academy of Medical Sciences (CAMS) and Peking Union Medical College (PUMC), Beijing, China, from May 2008 to August 2014. Twenty-five patients were enrolled and analyzed. The median number of cycles of GDP chemotherapy per patient was four (range, 2–8 cycles). Overall response rate was 64.0% (16/25) with five achieved complete remission or complete remission unconfirmed. After a median follow-up of 9 months, median overall survival (OS) and progression-free survival after relapse or progression (second-PFS) were 9.3 and 5.4 months. One-year PFS rate and 1-year OS rate were 27.4% and 43.9%, respectively. To analyze the clinicopathological characteristics, treatment protocls and prognostic factors in patients with primary intestinal diffuse large B-cell lymphoma (DLBCL).The clinical data of 54 patients with DLBCL who were treated in Cancer Hospital of Chinese Academy of Medical Sciences in the period from August 2003 to November 2012 were retrospectively analyzed, and the relevant prognostic factors of DLBCL were analyzed.Of the 54 patients, there were 43 males and 11 females (male∶female ratio was 3.91∶1), with a median age of onset of 49 (7-76) years. Most patients (64.8%) were in Lugano stage Ⅰ-Ⅱ. Of all the patients, 50 were followed up for a median of 49 (1-118) months. The 1-, 3- and 5-year overall survival (OS) rates were 82.0%, 69.9% and 60.7%, respectively; the 1-, 3- and 5-year progression-free survival (PFS) rates were 68.0%, 58.0% and 53.4%, respectively. Univariate analysis showed that the factors affecting prognosis of DLBCL patients included Lugano stage, B symptom, International Prognostic Index (IPI) score, lactate dehydrogenase level, β2-microglobulin level, tumor size, and treatment protocols (all P0.05). The 5-year OS rate was 67.1% in the patients treated with surgery plus chemoradiotherapy, and 40.0% in those treated with surgery or chemotherapy alone (P0.05). In the patients treated with chemotherapy combined with rituximab, the 5-year OS rate was higher than in those treated with chemotherapy alone (71.2% vs 47.6%, P0.05). Multivariate analysis indicated that tumor size (RR=7.686, P=0.022) and lactate dehydrogenase level (RR=10.131, P=0.017) were independent prognostic risk factors affecting OS.Primary intestinal DLBCL is a highly heterogeneous malignancy. Surgery combined with chemoradiotherapy and rituximab may help improve the overall prognosis of DLBCL patients.

Optimal chemotherapy regimen for peripheral T-cell lymphomas (PTCL) has not been fully defined. This study aimed to evaluate the optimal chemotherapy regimen in the first-line treatment for PTCL patients.Between 2003 and 2014, 93 consecutive patients with PTCL were enrolled in this study. Of 93 patients, 42 patients received CHOPE, 40 patients with CHOP, and 11 patients with GDP regimen.Response could be evaluated in 88 of 93 patients at the end of primary treatment. The CR rate for patients received CHOP (n = 38), CHOPE (n = 39), and GDP (n = 11) were 28.9, 51.3, and 45.5%, respectively, (P = 0.132) with an ORR of 65.8, 76.9, and 90.9%, respectively, (P = 0.210). The median follow-up time was 17.1 (1.4-108.3) months. Median progression-free survival (PFS) in CHOP (n = 40), CHOPE (n = 42), and GDP (n = 11) groups were 6.0, 15.3, and 9.7 months (P = 0.094) with 1-year PFS of 35.0, 54.8, and 45.5%, respectively, (P = 0.078). One-year OS for patients received CHOP (n = 40), CHOPE (n = 42), and GDP (n = 11) were 65.0, 83.3, and 100%, respectively, (P = 0.013) (CHOP vs CHOPE, P = 0.030; CHOP vs GDP, P = 0.024; CHOPE vs GDP, P = 0.174).CHOPE has a trend to improve CR rate, 1-year PFS and OS compared with CHOP alone. GDP shows promising efficacy which worth further exploration in large cohort studies. Clinical experience presented in this study may serve as reference for future large cohort studies.

This study was conducted to evaluate the effectiveness and tolerance of GDP (gemcitabine, dexamethasone, and cisplatin) regimen in patients with newly diagnosed stage IV and relapsed/refractory extranodal natural killer/T-cell lymphoma, nasal type (ENKTL). The study enrolled 41 ENKTL patients who received GDP regimen at the Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College between January 2008 and January 2015. The disease status was newly diagnosed stage IV in 15 patients and relapsed/refractory in 26 patients. The median number of cycles of chemotherapy per patient was 6 (range, 2–8 cycles). The overall response rate and complete-remission rate were 83.0% (34/41) and 41.5% (17/41), respectively. After a median follow-up of 16.2 months, 1-year progression-free survival rate and 1-year overall survival rate for the whole cohort were 54.5% and 72.7%. Grade 3 to 4 adverse events included neutropenia (34.1%), thrombocytopenia (19.5%), and anemia (14.6%). Our study has suggested high efficacy and low toxicity profile of GDP regimen in patients with newly diagnosed stage IV and relapsed/refractory ENKTL.

To further explore the role of rituximab when added to the CHOP-like regimen in the treatment of immunohistochemically defined non-germinal center B-cell subtype (non-GCB) diffuse large B-cell lymphoma (DLBCL), 159 newly diagnosed DLBCL patients were studied retrospectively based on the immunohistochemical evaluation of CD10, Bcl-6, MUM-1, and Bcl-2. Altogether, 110 patients underwent the CHOP-like regimen, and rituximab was added for the other 49 patients. Cox regression analysis showed that compared with the CHOP-like regimen, the rituximab-based regimen (R-CHOP regimen) significantly decreased the risk of disease relapse and progression in CD10-negative patients (P = 0.001), Bcl-6-negative patients (P = 0.01), and MUM-1-positive patients (P = 0.003). The risk of disease relapse in patients with non-GCB subtype (P = 0.002) also decreased. In contrast, patients with the opposite immunohistochemical marker expression profile and GCB subtype did not benefit from treatment with the R-CHOP regimen. In addition, non-GCB subtype patients had a significantly higher expression rate of Bcl-2 than GCB subtype patients (P = 0.042). Although univariate analysis found that both Bcl-2-positive and -negative patients had significantly higher event-free survival rates with the R-CHOP regimen, only Bcl-2 positivity (P = 0.004) maintained significance in the Cox regression analysis. We conclude that the addition of rituximab can significantly improve the prognosis of patients with non-GCB subtype DLBCL, which is closely related to the expression of CD10, Bcl-6, MUM-1, and Bcl-2.

Background The clinical course of diffuse large B-cell lymphoma (DLBCL) is variable. Although the prognostic markers and models have been developed and adapted in DLBCL, prognostic indices in relapse and refractory (r/r) DLBCL remain unclear. In order to better guide clinical practice, this study aimed to develop a prognostic nomogram in r/r DLBCL. Patients and methods Among 1858 DLBCL patients treated in National Cancer Center/National Clinical Research Center for Cancer/Cancer Hospital in China, between 2006 and 2016, 682 were r/r DLBCL, of which 573 patients were treated with enough cycles of first-line anthracycline-based chemotherapy. The clinical data were reviewed and evaluated. Progression free survival (PFS) was defined as the time from first treatment administration to disease progression/relapse. Relapse was defined as progression after initial complete or partial remission, and refractory disease was defined as achieving less than a partial response in the first-line setting. Cell of origin (COO) was assessed at diagnosis via Hans algorithm. The concordance index (C-index) and a calibration curve were used to determine and compare their predictive and discriminatory capacity. Results Overall, the median age at diagnosis was 54 (range 10-91) years old. Among all the patients, 325 (56.7%) were male and 248 (43.3%) were female. There were 255 (45.1%) patients categorized as stage I-II and 310 (54.9%) categorized as stage III-IV at diagnosis. Of all the patients, 145 (27.0%) were germinal center B-cell-like (GCB) and 393 (73.0%) were non-GCB. According to the Eastern Cooperative Oncology Group (ECOG) performance status (PS) score, the majority of patients had a good performance, with 457 (79.8%) of cases presenting with an ECOG PS score of 0-1; 89 (15.5%) presenting with ECOG PS of 2; 27(4.7%) scored ≥3. In the entire group, 563 (98.3%) patients received CHOP or CHOP-like regimen and 301 (52.5%) received rituximab treatment in the first-line setting. There were 200 patients (34.9%) had refractory disease; 203 (35.4%) had relapse in the first year from treatment commence; 91 (15.9%) had relapse in the second year; and 79 (13.8%) had relapse after the first 2 years. The median overall survival (OS) from diagnosis were 12, 17, 46 and 66 months, respectively in the above four groups, and OS in the entire group was shown in Figure 1. A total of 437 patients with complete clinical information were further divided into the primary (n=300) and validation (n=137) cohorts. Based on a multivariate analysis of the primary cohort, four independent prognostic factors including lactate dehydrogenase (LDH) level at diagnosis; response to the first-line treatment (CR with not CR); progression free survival and recurrence location (original with new with both) were identified and entered the nomogram (Figure 2). The calibration curve showed the optimal agreement between nomogram prediction and actual observation. In addition, the C-index of the nomogram for OS prediction was 0.743. The area under the receiver operating characteristic (ROC) curve (AUC) was 0.75 in the internal validation and 0.84 in the external validation. Four risk groups depending the nomogram were further developed and OS were presented in Figure 3. Conclusions Independent predictors of survival in r/r DLBCL have been identified and 4 indicators were used to create a nomogram. This proposed nomogram help with assess individual risks and decide on second-line treatment in clinical practice. Disclosures No relevant conflicts of interest to declare.

To evaluate the Epstein Barr virus (EBV) positive rate in untreated Hodgkin's lymphoma (HL) and investigate the prognostic significance of EBV status.A total of 207 previously untreated patients with histologically confirmed Hodgkin's lymphoma were enrolled in the study. The EBV infection status was confirmed through examining EBV-RNA (EBER) or EBV latent membrane protein-1. The correlation of clinical features and EBV infection status was analyzed, also the prognostic significance of EBV infection.A total of 66 cases were confirmed to be EBV positive Hodgkin's lymphoma (EBV⁺ HL) in all 207 cases, accounting for 31.9%. EBV⁺ HL group had more male patients (46/66, 69.7%) and mixed cellularity subtype (34/66, 51.5%) than EBV negative Hodgkin's lymphoma (EBV⁻ HL) group. The EBV positive rate had two peaks in age, respectively, in the age of 0-15 yeaR-old and60 yeaR-old. During a median follow-up period of 35 months, EBV⁻ HL was significant better than EBV⁺ HL in overall survival (P=0.001), but no significant difference was found in progression-free survival (P=0.763).EBV positive is a poor prognostic factor for overall survival time in HL.

The role of rituximab in combination with CHOP regimen in patients with stage I diffuse large B-cell lymphoma (DLBCL) remains to be defined. We aimed to compare CHOP plus rituximab (R-CHOP) with CHOP alone and determine the value of radiotherapy in these patients.Between 2003 and 2009, 140 untreated patients with stage I DLBCL were retrospectively analyzed in this study.Seventy-eight patients were treated in R-CHOP group and 62 in CHOP group. Ninety-one patients received additional radiotherapy at the end of chemotherapy. The different treatment groups were well-balanced with respect to baseline characteristics. Complete response (CR) rate was 77% both in R-CHOP and CHOP groups (P=0.945). After a median follow-up period of 56 months, patients received R-CHOP regimen had similar 5-year progression-free survival (PFS) (76% vs. 85%; log-rank P=0.215) and 5-year overall survival (OS) (90% vs. 96%; log-rank P=0.175) compared with those with CHOP alone. Patients with radiotherapy had significantly increased 5-year PFS compared with those who had chemotherapy alone (86% vs. 71%; log-rank P=0.005). At multivariate analysis, patients who had CR (P=0.008) and received radiotherapy (P=0.003) were significantly associated with superior PFS.CHOP alone could be as effective as R-CHOP regimen and additional radiotherapy would be necessary for stage I or stage I non-bulky DLBCL patients.

Background The regimen of cyclophosphamide, doxorubicin, vincristine, and prednisolone (CHOP) is an efficient treatment of non-Hodgkin’s lymphoma (NHL). This study aimed to assess the efficacy and toxicity of dose-adjusted CHOP alone or in combination with rituximab (R-CHOP) by examining the stem cell mobilization in NHL patients. Factors affecting the collection of CD34+ cells were also explored. Methods Our retrospective study included 39 patients eligible for autologous stem cell transplantation: 14 patients who expressed CD20 and were financially eligible received R-CHOP for autologous peripheral blood stem cell (APBSC) mobilization; the remaining 25 patients received CHOP. Results The median CD34+ cell yield was 7.01 × 106 cells/kg body weight (range 1.49–28.39 × 106 cells/kg body weight), with only two patients failing to meet the target CD34+ cell harvest of ≥2.0 × 106 cells/kg body weight. The median number of apheresis procedures per patient was 1 (range 1–3). The APBSC mobilization yield of the CHOP group appeared to be higher than that of the R-CHOP group (P = 0.005), whereas the success rate was similar between groups. R-CHOP elevated the complete response (CR) rate in B cell lymphoma patients as compared with CHOP (P = 0.01). No significant differences in toxicity or engraftment were observed between the two groups. Conclusion The present study demonstrated that dose-adjusted CHOP chemotherapy effectively mobilized APBSCs in NHL patients and that the addition of rituximab to dose-adjusted CHOP chemotherapy elevated the CR rate for patients with B-cell lymphoma.

To evaluate the clinicopathological features and outcomes of lymphoma in pregnancy.A total of 21 patients with lymphoma in pregnancy were diagnosed and treated at our hospital between January 1999 and January 2012. The clinicopathological data were analyzed retrospectively.There were 11 cases of Hodgkin's lymphoma (HL) and 10 cases of nodular sclerosis classical Hodgkin's lymphoma (NSCHL). And, among 10 cases of non-Hodgkin's lymphomas (NHL), there were diffuse large B cell lymphoma (n = 6),B-cell lymphoma, non-classifiable with features intermediate between diffuse large B-cell lymphoma and Burkitt lymphoma (n = 1), small lymphocytic lymphoma (n = 1), anaplastic large cell lymphoma with anaplastic lymphoma kinase (ALK) positive (n = 1) and T-cell lymphoblastic lymphoma (n = 1). The median age was 26(22-35) years.Superficial lymphadenopathy was more common in HL than in NHL (10/11 vs 3/10, P = 0.008). At diagnosis, bulky disease and extranodal involvement were more prevalent in NHL than in HL (8/10 vs 2/11, P = 0.009; 7/10 vs 2/11, P = 0.030). All patients received chemotherapy and those with early stages also had combined radiotherapy. Ten patients with HL and 6 patients with NHL achieved complete remission. During a median follow-up of 90 months for HL, the 5-year progression-free survival (PFS) and overall survival (OS) rates were 87.5% and 100% respectively. And during a median follow-up of 31 months for NHL, the 2-year PFS and OS rates were 66.7% and 77.8% respectively. The values of PFS and OS of NHL were inferior to those of HL (P = 0.073 and P = 0.066 respectively).One case of HL and 1 case of NHL received chemotherapy in the second trimester. The patients and their children experienced good outcomes.NSCHL is the most prevalent subtype of HL during pregnancy. B cell lymphoma and aggressive subtypes are most common for NHL during pregnancy. The outcomes of NHL are inferior to those of HL during pregnancy.

We developed a new Boson chemiluminescence immunoassay (CIA) and evaluated its application with cross-sectional analyses. Our results indicated that the Boson CIA demonstrated strong discriminatory power in diagnosing syphilis and that it can be used as a first-line screening test for syphilis serodiagnosis using the European Centre for Disease Prevention and Control algorithm or as a confirmatory test when combined with a patient's clinical history.

The study evaluated the effectiveness of autologous hematopoietic stem cell transplantation (AHSCT) in the treatment of lymphoblastic lymphoma (LL).We retrospectively analyzed the data from 41 patients with chemotherapy-sensitive LL who underwent hematopoietic stem cell transplantation (HSCT) from December 1989 to December 2009 in a single institution.HSCT was conducted as first-line consolidation therapy and salvage therapy in 36 and 5 patients, respectively. The median follow-up was 97.1 months (range, 24.6-173.1 months). The 5-year overall survival (OS) and event-free survival (EFS) rate were 64% and 47% for the initially treated patients, respectively, and were both 20% for the relapsed ones. Bone marrow (BM) involvement and chemotherapy cycles prior to transplantation were identified as significant prognostic factors for EFS in multivariate analysis.These results confirm that AHSCT is a reasonable option for chemotherapy-sensitive LL patients in first complete remission (CR1).

Anaplastic large-cell lymphoma (ALCL) is characterized by frequently presenting adverse factors at diagnosis. Many groups believed aggressive treatment strategies such as autologous stem cell transplantation brought survival benefit for ALCL patients. However, few compared these approaches with conventional chemotherapy to validate their superiority. Here, we report a study comparing the efficacy of peripheral blood stem cell transplantation (PBSCT) and conventional chemotherapy on ALCL. A total of 64 patients with primary systemic ALCL were studied retrospectively. The median follow-up period was 51 months (range, 1-167 months). For 48 patients undergoing conventional chemotherapy only, the 4-year event-free survival (EFS) and overall survival (OS) rates were 70.7% and 88.3%, respectively. Altogether, 16 patients underwent PBSCT, including 11 at first remission (CR1/PR1), 3 at second remission, and 2 with disease progression during first-line chemotherapy. The 4-year EFS and OS rates for patients underwent PBSCT at first remission were 81.8% and 90.9%, respectively. Compared with conventional chemotherapy, PBSCT did not show superiority either in EFS (P = 0.240) or in OS (P = 0.580) when applied at first remission. Univariate analysis showed that patients with B symptoms (P = 0.001), stage III/IV disease (P = 0.008), bulky disease (P = 0.075), negative anaplastic lymphoma kinase (ALK) expression (P = 0.059), and age ≤ 60 years (P = 0.054) had lower EFS. Furthermore, PBSCT significantly improved EFS in patients with B symptoms (100% vs. 50.8%, P = 0.027) or bulky disease (100% vs. 52.8%, P = 0.045) when applied as an up-front strategy. Based on these results, we conclude that, for patients with specific adverse factors such as B symptoms and bulky disease, PBSCT was superior to conventional chemotherapy in terms of EFS.

Background and objective The efficacy of standard chemotherapy regimen on aggressive non-Hodgkin's lymphoma (NHL) of certain pathologic types is unsatisfied. This study was to evaluate the safety and efficacy of modified Hyper-CVAD regimen on Chinese patients with aggressive NHL. Methods Clinical records of 31 NHL patients who received modified Hyper-CVAD regimen in Cancer Hospital of Chinese Academy of Medical Sciences from June 2004 to June 2008 were analyzed in terms of toxicity and response. Results The 31 patients totally received 91 cycles of regimen A and 41 cycles of regimen B with a median of 4 cycles (ranged 1-7 cycles). The major toxicity was myelosuppresion: the occurrence rates of neutropenia of grades III-IV, thrombocytopenia and febrile neutropenia were 49.5%, 3.3% and 12.1% during treatment of regimen A, and were 80.5%, 82.9% and 46.3% during treatment of regimen B. No treatment-related death was observed. The responses were assessable in 26 patients. The total response rate was 80.8%, and 12 patients achieved complete response (46.2%). Conclusion Modified Hyper-CVAD regimen is a promising regimen for the patients with intermediate and high grade NHL.

Sinomenine transdermal patch was prepared and its properties were studied. The patches were produced by salivation method. The releasing rate in vitro of the patch was determined by HPLC. Peel test was used to evaluate the adhesion. Acute skin irritation test was performed in comparison with formalin (0.8%) by using mouse model. The Sinomenine TDDS Patch was prepared. The releasing rate in vitro followed the Higuchi equation (r0.99), the releasing amount was beyond 90% in 24h. The peel adhesion to steel (N/25 mm) is 10 or above. The skin irritation tests showed negligible erythema and edema. The Sinomenine transdermal patch was prepared successfully and it may be beneficial for topical use.

To investigate the correlation between sensitivity to Fas ligand (FasL) and expression level of decoy receptor 3 (DcR3) on tumor cell surface, Fas/DcR3 mRNA expression was detected by RT-PCR. Anti-DcR3 mAb was used to detect expression level of DcR3 on surface of tumor cells by flow cytometry. Caspase-8, caspase-9, caspase-3, Bcl-2 expressions were analyzed by Western blot, respectively. Sensitivity to apoptosis induced by FasL was determined by Annexin V apoptosis kit. The expressions of DcR3 on the surface of tumor cells from high to low were approximately 35.3% in BGC823 cells, and 21.6% in MCF-7 cells, respectively. The apoptotic rates induced by FasL from low to high were 15.6% in BGC823 cells, and 58.2% in MCF-7 cells, respectively. There was a significant correlation between the expression levels of DcR3 with FasL-inducing apoptosis.

Background Extranodal natural killer/T-cell lymphoma, nasal type (ENKL) is a rare and distinct subtype of non-hodgkin lymphoma (NHL). The frequency was higher in Asia than in western countries and it has become the most common subtype of peripheral T-cell lymphomas in China. The majority of ENKL patients present with early stage. Optimal treatment modalities and prognostic factors for localized ENKL have not been fully defined. This study aimed to evaluate the optimal treatment strategy and prognostic factors for localized ENKL patients. Methods Between 2003 and 2013, three hundred and five patients with stage IE/IIE ENKL were comprehensively analyzed in this study. A total of 180 patients received combined chemoradiotherapy, with 111 patients received radiotherapy alone and 14 patients recieved chemotherapy alone. Chemotherapy regimens include GDP (gemcitabine, cisplatin, and dexamethasone), CHOP (epirubicin, cyclophosphamide, vincristine, and prednisolone) and other regimens. A total dose of 50 Gy to the primary tumor was considered as radical dose for ENKL, and additional 5 to 10 Gy was administered as a boost to the residual disease. Results The complete response (CR) rate for patients received chemoradiotherapy (n=175) was significantly higher than that for patients received radiotherapy alone (n=102) (89.1 % vs.77.5 %, P = 0.009) or chemotherapy alone (n=14) (89.1 % vs.21.4 %, P< 0.001). The median follow up time for all 305 patients was 38.7 (1.1 to 393) months. For 228 stage IE paranasal extension or IIE patients, 3-year overall survival (OS) in combined chemoradiotherapy (n=154), radiotherapy alone (n=60) and chemotherapy alone (n=14) groups were 85.7%, 73.3% and 57.1% respectively (chemoradiotherapy vs. radiotherapy, P=0.003; chemoradiotherapy vs. chemotherapy, P Conclusions Combined chemoradiotherapy is the most optimal therapy strategy for stage IE paranasal extension or IIE ENKL patients. GDP or combined with pegaspargase regimen shows promising efficacy, significant superior to the traditional CHOP regimen. The sequence of chemotherapy and radiotherapy for patients received novel chemotherapy regimens still needs further assessment in phase 3 clinical trials. We identified 3 risk groups based on 3 prognostic factors (stage II, LDH elevated and paranasal extension) with different survival outcomes and this novel prognostic model may better predict prognosis than previous International Prognostic Index (IPI) and Korean Prognostic Index (KPI) score for ENKL patients with limited stage. Background: PTCL-NOS usually lacks B-cell associated antigens. However, rare cases of CD20 positive peripheral T cell lymphpoma have been reported[1-4], with the majority of cases being classified as PTCL-NOS[4]. So far, the vast majority of the published reports on CD20+ PTCL are from US or European countries with limited data on Chinese patients. Furthermore, the clinical and prognostic profile of this rare disease entity remains largely unknown. Aim: To explore the clinical features and prognostic impact of aberrant CD20 expression in Chinese patients with PTCL-NOS. Methods: We retrospectively reviewed patients diagnosed with PTCL-NOS at our institution from August 2002 to March 2015. Patients with aberrant expression of CD20 confirmed by immunohistochemical analyses were included in the study. We examined patients' pathological and clinical features, treatment response and survival. Results: Twelve patients with CD20+ PTCL-NOS were identified. CD20 was strongly and uniformly expressed in 6 cases and dimly expressed or present on a subgroup of neoplastic cells in the remaining cases. In 6 patients, the gene rearrangement studies for immunoglobulin and T-cell receptors were performed to determine the precise lineage for neoplastic clones. The Ki-67 proliferation index ranged from 10~85% in these cases. Among the patients, there were 10 men and 2 women. Patients' median age was 54 yr (range:16~69 yr). The majority(83%) of patients had a ECOG-PS score of 0~1. Eight (67%) patients presented with advanced stage (Ann Arbor stage 3~4) disease and the remaining 3 patients presented with localized (stage 1~2) disease. Nodal disease was present in all the patients, whereas 6 patients had extra-nodal involvement including the parotid glands, tongue, oropharynx, subcutaneous tissue, spleen, lung and breast. No patient presented with bone marrow involvement. The IPI score was 0~1 in 5 cases, 2 in one case, and 3~4 in 6 cases. All patients were initially treated with CHOP-like combination chemotherapy with a response rate of 75% (CR in 5 patients, PR in 4 patients, SD in 1 patient and PD in 2 patients). Three patients with stage 1~2 disease received consolidation radiotherapy, and no patient received autologous or allogeneic stem cell transplantation. Among CR patients, three remained disease-free at 4.5, 11.3 and 57 months, respectively. The remaining 9 patients developed disease recurrence or progression, among whom seven underwent gemcitabine-based salvage chemotherapy. No patient received rituximab in first-line or subsequent lines of treatment. At last follow-up, 6 patients have died from disease progression. The median PFS and OS for all the patients were 10.5 months (range:0.7~57 months, Figure 1) and 41.9 months (range:3.4~57 months, Figure 2), respectively. Conclusions: CD20-expressing PTCL-NOS is rare, and appears to be clinically heterogeneous with a subset of patients associated with high IPI score and more aggressive clinical course. Despite the expression of B-cell antigens, the overall treatment outcome with anthracycline-based chemotherapy remains poor, and prognosis is similar to that of CD20 negative PTCL-NOS. The aberrant expression of CD20 may warrant further exploration of the role of rituximab in these patients. The clinical and prognostic implications of CD20 positivity in PTCL-NOS patients needs to be further elucidated in large-scale case series. References: [1] J Clin Oncol. 2011;29(32):e789-91.
