Objective: This study aims to estimate the incidence of Vestibular neuritis (VN) in three different districts in Italy, its epidemiological features, and the prevalence of comorbidities associated with it., Methods: An observational prospective study of 198 patients referred to ENT departments in Siena, Grosseto, and Cuneo was carried out over a 2-year period. Each patient underwent a complete otoneurologic examination in the first 48 h from the onset of symptoms and a brain MRI in the early stages of the disease. The follow-up lasted for 1 year., Results: The total VN incidence rate of the three municipalities was 48.497 (95% CI: 48.395-48.598) and its standardized value was 53.564 (95% CI: 53.463-53.666). The total VN incidence rate for the whole sample (municipality and district of the three centers) was 18.218 (95% CI: 18.164-18.272), and its standardized value was 20.185 (95% CI: 20.129-20.241). A significant difference was highlighted between patients living in the city compared to those living in the surrounding area ( p < 0.000), this may be due to the ease of reaching the otoneurological referral center., Conclusion: The total incidence rate for the three municipalities was 48.497. Objectives: The aim of the present study was to investigate current treatment practices and self-reported effectiveness in Ménière's disease., Materials and Methods: Members of two Italian Ménière's disease support (n=170) with ≥6-month history of Ménière's disease were administered an online survey about recent treatments. Vertigo episode count, work absenteeism, and limitations in family life, social life, work, or travel as included in the Social Life and Work Impact of Dizziness Questionnaire before and after recent treatments were queried., Results: Twenty-four different treatments were reported for Ménière's disease, with dietary modifications (55%), diuretics (47%), and betahistine (41%) being the most common. The majority (71%) received multiple simultaneous treatments. Prior to the most recent treatments, 78%-89% of respondents indicated limitations in family or social life, work, or traveling. After their most recent treatment, respondents reported improvements in mean vertigo episode counts (5.7±7.6 vs. 2.6±4.6, p<0.001), days off work per month (10.1±9.2 vs. 4.2±6.7, p<0.001), and proportions indicating limitations in any functional measure assessed (p<0.05). These findings were consistent regardless of treatment approach (p<0.05 for all). Intratympanic gentamicin provided the greatest reductions in vertigo count, functional limitations, and work absenteeism (p<0.01 for all), as well as the fewest respondents reporting post-treatment functional limitations (16%-37%)., Conclusion: Despite many treatment approaches targeting different proposed pathophysiology for Ménière's disease in this cross-sectional survey, all treatments are reported as effective by patients. These findings support a prominent placebo effect in Ménière's disease and highlight challenges in studying treatment outcomes; there is a critical need to better understand Ménière's disease.

High-mobility group box 1 (HMGB1) is a nuclear non-histone protein, playing a critical role as a mediator between innate and acquired immunity; when released extracellularly, it coordinates the cellular stress response (under necrosis, bacterial lipopolysaccharide stimulation) and acts as an inflammatory marker and cytokine. The aim of the study was to demonstrate whether HMGB1 is over-expressed in chronic middle-ear pathologies and whether the entity of expression and the localization are correlated with the degree of the inflammatory reaction, thus suggesting that HMGB1 may play a crucial role in chronic inflammatory disorders of the middle ear, as already demonstrated in other airway diseases. We analyzed 30 samples of middle-ear mucosa in patients affected by chronic suppurative otitis media with ear drum perforation with/without cholesteatoma and otosclerosis as control. The distribution of HMGB1 was evaluated as nuclear, cytoplasmic, and/or extracellular staining. The inflammatory cells observed in the biopsies were mostly lymphocytes and plasmacells. A statistically significant difference in inflammation score between otosclerosis and chronic otitis samples ( P < 0.01; Anova test) and between otosclerosis and cholesteatoma samples ( P < 0.05; Anova test) was observed; the HMGB1 positivity was in accordance with the density of the inflammatory infiltrate. HMGB1 is over-expressed in chronic middle-ear pathologies and may play a role in the progression of the inflammatory process from recurrent acute otitis media to chronic suppurative otitis media.

Vitamin D status has been linked to immune system and autoimmune disorders; in fact, low levels of vitamin D are common in many autoimmune disorders. The aims of our study were to assess the prevalence of vitamin D insufficiency and the possible correlation with clinical parameters in systemic sclerosis (SSc). We recruited 40 patients (38 female and two male) with scleroderma and 40 healthy controls matched for age and gender. Demographic and clinical parameters were recorded and the 25-hydroxivitamin D3 serum levels were measured. Serum 25-hydroxivitamin D3 levels were significantly lower in patients with systemic sclerosis than in the control group. The prevalence of 25-hydroxivitamin D3 insufficiency was 50% in the patients and 22.5% in the control group. A statistically significant association was observed between the insufficiency of 25-hydroxivitamin D3 and skin involvement ( p = 0.02) and echocardiography systolic pulmonary artery pressure >35 mmHg ( p = 0.02). Our data show that the systemic sclerosis group has significantly lower serum 25-hydroxivitamin D3 concentrations compared to the control group; skin involvement and pulmonary hypertension are associated with vitamin D3 insufficiency., Competing Interests: The authors declare no conflict of interest.

Obstructive sleep apnoea syndrome (OSAS) is a disorder that leads to metabolic abnormalities and increased cardiovascular risk. The aim of this study was to identify early laboratory markers of cardiovascular disease through analysis of oxidative stress in normal subjects and patients with OSAS. A prospective study was designed to compare outcomes of oxidative stress laboratory tests in 20 adult patients with OSAS and a control group of 20 normal subjects. Laboratory techniques for detecting and quantifying free radical damage must be targeted to assess the pro-oxidant component and the antioxidant in order to obtain an overall picture of oxidative balance. No statistical differences in age, sex distribution, or BMI were found between the two groups (p>0.05). There were significant differences in the apnoea/hypopnoea index (AHI) between OSAS patients and the control group (p<0.05). Statistically significant differences in isoprostane, advanced oxidation protein products (AOPP) and non-protein bound iron (NPBI) levels were found between the study and control groups. No significant difference in the levels of thiol biomarkers was found between the two groups. The main finding of the present study was increased production of oxidative stress biomarkers in OSAS patients. The major difference between thiols and other oxidative stress biomarkers is that thiols are antioxidants, while the others are expressions of oxidative damage. The findings of the present study indicate that biomarkers of oxidative stress in OSAS may be used as a marker of upper airway obstructive episodes due to mechanical trauma, as well as a marker of hypoxaemia causing local oropharyngeal inflammation.

Which cognitive processes are accessible to conscious report? To study the limits of conscious reportability, we designed a novel method of quantified introspection, in which subjects were asked, after each trial of a standard cognitive task, to estimate the time spent completing the task. We then applied classical mental-chronometry techniques, such as the additive-factors method, to analyze these introspective estimates of response time. We demonstrate that introspective response time can be a sensitive measure, tightly correlated with objective response time in a single-task context. In a psychological-refractory-period task, however, the objective processing delay resulting from interference by a second concurrent task is totally absent from introspective estimates. These results suggest that introspective estimates of time spent on a task tightly correlate with the period of availability of central processing resources.

Purpose: To evaluate the correlation between computerised visual field testing and optic disc morphometric parameters (rim area, rim/disc area ratio, cup/disc area ratio and optic disc surface smoothness (ODSS)) in the follow-up of a group of patients affected by primary open angle glaucoma (POAG)., Methods: Reliable automated perimetry (Humphrey 640 VFA, central 30-2 threshold program) was performed at T0 (the enrollment time), T1 (after 6 +/- 1 months; range 5-7 months), T2 (12 +/- 1 months), T3 (18 +/- 1 months), and T4 (the end of the follow-up period: 24 +/- 2 months) in one eye randomly chosen from each of 30 POAG patients. Computerised optic disc analysis (IMAGEnet X Rev-3-51b, Topcon Europe, The Netherlands) was performed at T0 and T4. To evaluate the correlation between morphometric parameters and computerised visual field testing, the stability or worsening of visual field test was evaluated by means of 'Mean Deviation linear regression analysis' (STATPAC 2 software); that of morphometric parameters was studied using their coefficients of variation. A rank of '0' was assigned to stability and a rank of '1' to worsening. The Spearman rank correlation coefficient was used to evaluate statistically the correlations between visual field analysis and morphometric parameters. Furthermore kappa statistic was used to evaluate the agreement of morphometric parameter changes with visual field progression analysis., Results: According to the MD slope regression analysis, in 18 patients the visual fields were stable while in 12 they were worsening during the follow-up period. In 86.65% of patients (n = 26) the morphometric parameter ODSS and visual field analysis were concordant (p < 0.0001). In 80% of patients (n = 24) the cup/disc area ratio and visual field analysis were concordant (p < 0.001). The other morphometric parameters (rim area, rim/dics area ratio) were less correlated with visual field analysis than ODSS (p < 0.05). The agreement of visual field analysis with all the morphometric parameters was good (kappa = 0.690, 95% confidence interval (CI) of kappa = 0.589-0.790). The agreement of visual field analysis with ODSS and cup/disc area (kappa = 0.688; 95% CI = 0.511-0.864) was better than the agreement of visual field analysis with rim area and rim/disc area ratio (kappa = 0.438; 95% CI = 0.260-0.655)., Conclusion: Analysis of the progression of visual field damage and optic nerve head morphometric parameters should both be taken into account in glaucoma follow-up. Among the morphometric parameters evaluated by means of Topcon IMAGEnet, ODSS and (to a lesser extent) the cup/disc ratio should have the greatest weight.
