In this paper, the following critical Kirchhoff-type elliptic equation involving a logarithmic-type perturbation$ -\Big(a+b\int_{\Omega}|\nabla u|^2\mathrm{d}x\Big)\Delta u = \lambda|u|^{q-2}u\ln |u|^2+\mu|u|^2u $is considered in a bounded domain in $ \mathbb{R}^{4} $. One of the main obstructions one encounters when looking for weak solutions to Kirchhoff problems in high dimensions is that the boundedness of the $ (PS) $ sequence is hard to obtain. By combining a result by Jeanjean [27] with the mountain pass lemma and Brézis–Lieb's lemma, it is proved that either the norm of the sequence of approximation solutions goes to infinity or the problem admits a nontrivial weak solution, under some appropriate assumptions on $ a $, $ b $, $ \lambda $, and $ \mu $.

Abstract Background There is an urgent need for therapeutic strategies for inpatients with severe or critical COVID-19. The evaluation of the clinical benefits of nirmatrelvir and ritonavir (Nmr/r) for these patients beyond five days of symptom onset is insufficient. Methods A new propensity score-matched cohort was constructed by using multicenter data from 6695 adult inpatients with COVID-19 from December 2022 to February 2023 in China after the epidemic control measures were lifted across the country. The severity of disease of the inpatients was based on the tenth trial edition of the Guidelines on the Diagnosis and Treatment of COVID-19 in China. The symptom onset of 1870 enrolled severe or critical inpatients was beyond five days, and they received either Nmr/r plus standard treatment or only standard care. The ratio of patients whose SOFA score improved more than 2 points, crucial respiratory endpoints, changes in inflammatory markers, safety on the seventh day following the initiation of Nmr/r treatment, and length of hospital stay were evaluated. Results In the Nmr/r group, on Day 7, the number of patients with an improvement in SOFA score ≥ 2 was much greater than that in the standard treatment group (P = 0.024) without a significant decrease in glomerular filtration rate (P = 0.815). Additionally, the rate of new intubation was lower (P = 0.004) and the no intubation days were higher (P = 0.003) in the first 7 days in the Nmr/r group. Other clinical benefits were limited. Conclusions Our study may provide new insight that inpatients with severe or critical COVID-19 beyond five days of symptom onset benefit from Nmr/r. Future studies, particularly randomized controlled trials, are necessary to verify the above findings.

Background: Invasive fungal infections (IFIs) are important infectious complications amongst critically ill children. The most common fungal infections are due to Candida species. Aspergillus, Zygomycetes and Fusarium are also emerging because of the empirical use of antifungal drugs. This updated review discusses the epidemiology of IFIs as well as antifungal drugs, dosing and potential adverse effects in critically ill children. Methods: A PubMed search was conducted with Clinical Queries using the key terms “antifungal”, “children”, “critical care” AND “paediatric intensive care unit” OR “PICU”. The search strategy included clinical trials, randomized controlled trials, meta-analyses, observational studies and reviews and was limited to the English literature in paediatrics. Results: Candida and Aspergillus spp. are the most prevalent fungi in paediatric IFIs, causing invasive candidiasis infections (ICIs) and invasive aspergillosis infections (IAIs), respectively. These IFIs are associated with high morbidity, mortality and healthcare costs. Candida albicans is the principal Candida spp. associated with paediatric ICIs. The risks and epidemiology for IFIs vary if considering previously healthy children treated in the paediatric intensive care unit or children with leukaemia, malignancy or a severe haematological disease. The mortality rate for IAIs in children is 2.5–3.5-fold higher than for ICIs. Four major classes of antifungals for critically ill children are azoles, polyenes, antifungal antimetabolites and echinocandins. Conclusions: Antifungal agents are highly efficacious. For successful treatment outcomes, it is crucial to determine the optimal dosage, monitor pharmacokinetics parameters and adverse effects, and individualized therapeutic monitoring. Despite potent antifungal medications, ICIs and IAIs continue to be serious infections with high mortality rates. Pre-emptive therapy has been used for IAIs. Most guidelines recommend voriconazole as initial therapy of invasive aspergillosis in most patients, with consideration of combination therapy with voriconazole plus an echinocandin in selected patients with severe disease. The challenge is to identify critically ill patients at high risks of ICIs for targeted prophylaxis. Intravenous/per os fluconazole is first-line pre-emptive treatment for Candida spp. whereas intravenous micafungin or intravenous liposomal amphotericin B is alternative pre-emptive treatment. A comprehensive analysis of long-term water level trends is essential for freshwater sustainability. Given that Odisha heavily relies on agriculture, the monitoring and management of groundwater and its fluctuations are imperative for ensuring future sustainability in the state. Here, we analyzed the trend in Groundwater using water level data for a 30-year period (1990–2020) for the entire Odisha region. Moreover, to determine the long term variability, critical zones of future groundwater variability and controlling parameters of the water level change, we used spatio-temporal water level data of 746 locations. Water level rise of coastal districts during post-monsoon (POM), corresponds to the intensity of rainfall received, thus rising, however other districts of Odisha, showing decline in water level during the same season is due to shortage of rainfall, increase in population at a sudden, and over pumping due to industrial activities. Similarly, during pre-monsoon (PRM), water level shows an increasing trend in hard rock terrain of Odisha implying rabi crop irrigation, high density drainage network and lesser population density. Feature selection techniques were used in this study to know the parameters controlling most to this water level fluctuation in the entire Odisha state. Precipitation followed by landuse & landcover, lithology and population density are controlling the most for the long term water level change. Drainage, elevation, lithology and slope are positively related to the water level change while others are negatively related. It is also inferred that the districts like Mayurbhanj, Sundargarh, Keonjhar, Kandhamal, Boudh, Dhenkanal, Gajapati, Koraput and Kalahandi contain most of the high critical zone concerning future availability of groundwater while most of the coastal regions are safe.Article Highlights: Water level trend analysis was performed taking 30 years of historical data. Hardrock regions are prominent zones of declining in water level than the alluvium parts. Alvarez's beautifully sculpted book captures Mike's multidimensional psychological life as he lives, observes, and narrates it within structures that medicalize people's mental health experiences. This treatise seeks to show the problem of the theological method per se, in that it has always been infected with secular philosophies in its form and structure. That is, the theological method, which should be purely scriptural, has often coined the method of philosophy and/or the sciences (other than theology), often ignoring the exclusivity of the theological method per se, namely, exegetical. or interpretive itself. Therefore, in this essay we present the criticisms of some Christian thinkers and theologians of the use and method in vogue, advocating for a methodology that clearly emanates from the Holy Scriptures. This study explores the experiences of three Networked Improvement Communities (NICs) within mathematics departments as they work to critically transform their introductory mathematics programs. Drawing on the framework of dominant and critical axes of equity, we identify three key tensions experienced by the NICs: identity neutrality versus identity centrality, power over versus power with, and students as novices versus students as experts. These tensions are framed as productive enactments of working to change systems from within, highlighting the challenges and opportunities inherent in navigating the liminal space of nepantla. We argue that engaging with these tensions is crucial for fostering critical transformations and offer recommendations for leveraging the roles of identity, power, and student partnership to promote more equitable and inclusive mathematics education environments.

Abstract The alarming state of global insulin access in low‐resource settings presents a major barrier to diabetes care. A comprehensive review of these challenges is lacking at the global level. To address this weakness, enhance affordability and build capacity for a more sustainable approach to scaling up access. This review analyzes the specific issue of inconsistent access to insulin in low‐ and middle‐income countries. Using this analysis, we mapped the scope and intensity of issues such as the unaffordability and unavailability of insulin. We also identified six innovative and integrative strategies for increasing and securing accessibility in the areas of policy making, marketing, clinical practice, health education, domestication, and multisectoral approaches.

ABSTRACT The onset of critical rare diseases (RDs) in children is rapid and dangerous, accompanied by a high mortality rate, which brings a heavy burden to both families and society. Multiple malformations, neuromuscular diseases, metabolic diseases, and heart diseases are the most common types of RDs in children of China, often manifesting with multiple organ dysfunction. At present, the diagnosis and treatment of critical RDs in children face challenges such as prolonged diagnosis time, a high misdiagnosis rate, limited treatment modalities, and a significant disease burden. However, with the progress in genetic testing technology, the establishment of multidisciplinary diagnosis and treatment platforms, and the implementation of relevant RD policies in China, children with critical RDs will received enhanced medical services, experience improved prognoses, and reintegrate into social life.

Introduction: The global healthcare burden of COVID-19 pandemic has been unprecedented with a high mortality. Metabolomics, a powerful technique, has been increasingly utilized to study the host response to infections and to understand the progression of multi-system disorders such as COVID-19. Analysis of the host metabolites in response to SARS-CoV-2 infection can provide a snapshot of the endogenous metabolic landscape of the host and its role in shaping the interaction with SARS-CoV-2. Disease severity and consequently the clinical outcomes may be associated with a metabolic imbalance related to amino acids, lipids, and energy-generating pathways. Hence, the host metabolome can help predict potential clinical risks and outcomes. Methods: In this prospective study, using a targeted metabolomics approach, we studied the metabolic signature in 154 COVID-19 patients (males=138, age range 48-69 yrs) and related it to disease severity and mortality. Blood plasma concentrations of metabolites were quantified through LC-MS using MxP Quant 500 kit, which has a coverage of 630 metabolites from 26 biochemical classes including distinct classes of lipids and small organic molecules. We then employed Kaplan-Meier survival analysis to investigate the correlation between various metabolic markers, disease severity and patient outcomes. Results: A comparison of survival outcomes between individuals with high levels of various metabolites (amino acids, tryptophan, kynurenine, serotonin, creatine, SDMA, ADMA, 1-MH and carnitine palmitoyltransferase 1 and 2 enzymes) and those with low levels revealed statistically significant differences in survival outcomes. We further used four key metabolic markers (tryptophan, kynurenine, asymmetric dimethylarginine, and 1-Methylhistidine) to develop a COVID-19 mortality risk model through the application of multiple machinelearning methods. Conclusions: Metabolomics analysis revealed distinct metabolic signatures among different severity groups, reflecting discernible alterations in amino acid levels and perturbations in tryptophan metabolism. Notably, critical patients exhibited higher levels of short chain acylcarnitines, concomitant with higher concentrations of SDMA, ADMA, and 1-MH in severe cases and non-survivors. Introduction: Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), which appeared in 2019, has been classified as critical and non-critical according to clinical signs and symptoms. Critical patients require mechanical ventilation and intensive care unit (ICU) admission, whereas non-critical patients require neither mechanical ventilation nor ICU admission. Several factors have been recently identified as effective factors, including blood cell count, enzymes, blood markers, and underlying diseases. By comparing blood markers, comorbidities, co-infections, and their relationship with mortality, we sought to determine differences between critical and non-critical groups. Method: We used Scopus, PubMed, and Web of Science databases for our systematic search. Inclusion criteria include any report describing the clinical course of COVID-19 patients and showing the association of the COVID-19 clinical courses with blood cells, blood markers, and bacterial co-infection changes. Twenty-one publications were eligible for full-text examination between 2019 to 2021. Result: The standard difference in WBC, lymphocyte, and platelet between the two clinical groups was 0.538, -0.670, and -0.421, respectively. Also, the standard difference between the two clinical groups of CRP, ALT, and AST was 0.482, 0.402, and 0.463, respectively. The odds ratios for hypertension and diabetes were significantly different between the two groups. The prevalence of co-infection also in the critical group is higher. Conclusion: In conclusion, our data suggest that critical patients suffer from a suppressed immune system, and the inflammation level, the risk of organ damage, and co-infections are significantly high in the critical group and suggests the use of bacteriostatic instead of bactericides to treat co-infections. Introduction: Subgroups of autoantibodies directed against voltage-gated potassium channel (Kv) complex components have been associated with immunotherapy-responsive clinical syndromes. The high prevalence and the role of autoantibodies directly binding Kv remain, however, controversial. Our objective was to determine Kv autoantibody binding requirements and to clarify their contribution to the observed immune response. Methods: Binding epitopes were studied in sera (n = 36) and cerebrospinal fluid (CSF) (n = 12) from a patient cohort positive for Kv1.2 but negative for 32 common neurological autoantigens and controls (sera n = 18 and CSF n = 5) by phospho and deep mutational scans. Autoantibody specificity and contribution to the observed immune response were resolved on recombinant cells, cerebellum slices, and nerve fibers. Results: 83% of the patients (30/36) within the studied cohort shared one out of the two major binding epitopes with Kv1.2-3 reactivity. Eleven percent (4/36) of the serum samples showed no binding. Fingerprinting resolved close to identical sequence requirements for both shared epitopes. Kv autoantibody response is directed against juxtaparanodal regions in peripheral nerves and the axon initial segment in central nervous system neurons and exclusively mediated by the shared epitopes. Discussion: Systematic mapping revealed two shared autoimmune responses, with one dominant Kv1.2-3 autoantibody epitope being unexpectedly prevalent. The conservation of the molecular binding requirements among these patients indicates a uniform autoantibody repertoire with monospecific reactivity. The enhanced sensitivity of the epitope-based (10/12) compared with that of the cellbased detection (7/12) highlights its use for detection. The determined immunodominant epitope is also the primary immune response visible in tissue, suggesting a diagnostic significance and a specific value for routine screening. [ABSTRACT FROM AUTHOR]

Abstract Background Children with COVID-19 infection had fewer severe symptoms, which made it challenging to publish clinical data for this age group. This study aimed to determine the clinical features, laboratory markers, and predictors of mortality in children hospitalized in Pediatric Critical Care Units with COVID-19 pneumonia. Methods This retrospective research included all children between the age of ≥ 1 month and 18 years with laboratory-confirmed COVID-19 pneumonia through reverse transcriptase polymerase chain reaction (RT-PCR) from nasopharyngeal swabs who were admitted to Pediatric Critical Care Units between January 2022 and December 2022. Results The study included 62 patients with confirmed COVID-19 infection. Out of the 62 patients, 43 (69.4%) survived and 19 (30.6%) succumbed. The reason behind this increase is that COVID-19 causes hypoxia that promotes the growth of fungus. To identify the association between mucormycosis and COVID-19, in critically ill or immunocompromised COVID-19 patients. The literature included in the review was researched from October 1, 2021, to November 1, 2022, by using the Google Scholar database as the search engine. Of the 20 articles included, there were 4 case reports, 2 case series, 10 narrative reviews, and 4 quantitative studies. Mucormycetes growth is caused by several factors, including hyperglycemia owing to previously existing diabetes or excessive use of steroids, increased ferritin levels owing to the inflammatory cascade initiated by COVID-19, and immunosuppression caused by the use of steroids or other immunosuppressive therapy. Reduced white-cell count and activity in COVID-19 leads to increased germination of fungal spores hence developing a catastrophic picture of rhinocerebral mucormycosis. Considering that the hematological patient is frequently treated with cortisone, immunosuppressed due to the underlying condition, but also through the administered therapy, the association with a possible diabetes makes this patient susceptible to developing rhinocerebral mucormycosis during COVID-19 infection. Despite being severe, the association between mucormycosis and COVID-19 is specific and treatable. Development of mucormycosis in hematological patients suffering from severe COVID-19 disease is dangerous, yet not compulsory and can be prevented. Using a common steroid-dose protocol with hyperbaric oxygen and necessary preventive measure reveals the disease as a superadded infection. This article proposes a reflection on the intersection between animals, literature, and poshumanism, with the aim of enriching the theoretical and critical debate on animal representation through the exploration of Colombian zooliterature. In this way, a conceptual framework based on three categories is introduced: "the allegorical, the fantastic, the animal being," so that this classification opens new paths for literary studies to expand and adapt to the changing perceptions of the literary aesthetic object in contemporary discourses. The purpose of this article is to construct a theoretical framework in order to conduct a knowledge/wisdoms dialogue on the theme of pedagogy with the native Mapuche people living in Chile. Part of the problems of such a task are comparative (between pedagogies) and the specific problem that concern this article is the relation of compatibility/incompatibility between these pedagogies. The article proposes to use the philosophy of Gilles Deleuze to theorize incompatibility as a positive although disruptive event centred on his notions of knowledge, learning and encounter, as well as taking into account his broader scope of understanding problems in general. To further develop and specialize these notions the article takes up the methodological reflections on comparison, translation and the equivocal by the anthropologist Viveiros De Castro. The healthcare industry must increase electronic device accuracy, reliability, and productivity. Artificial intelligence (AI) has been applied in various industries, but cybersecurity for cyber-physical systems (CPS) is still a recent topic. This work presents a method for intelligent threat recognition based on machine learning (ML) that enables run-time risk assessment for better situational awareness in CPS security monitoring. Several machine learning techniques, including Nave Bayes (65.4%), Support Vector Machine (64.1%), Decision Tree (89.6%), Random Forest (92.5%), and Ensemble crossover (EC) XG boost classifier (99.64), were used to classify the malicious activities on real-world testbeds. The outcomes demonstrate that the Ensemble crossover XG boost enabled the best classification accuracy. When used in industrial reference applications, the model creates a safe environment where the patient is only made aware of risks when categorization optimism exceeds a specific limit, minimizing security managers' pressure and efficiently assisting their choices. IntroductionAcute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), which appeared in 2019, has been classified as critical and non-critical according to clinical signs and symptoms. Critical patients require mechanical ventilation and intensive care unit (ICU) admission, whereas non-critical patients require neither mechanical ventilation nor ICU admission. Several factors have been recently identified as effective factors, including blood cell count, enzymes, blood markers, and underlying diseases. By comparing blood markers, comorbidities, co-infections, and their relationship with mortality, we sought to determine differences between critical and non-critical groups.MethodWe used Scopus, PubMed, and Web of Science databases for our systematic search. Inclusion criteria include any report describing the clinical course of COVID-19 patients and showing the association of the COVID-19 clinical courses with blood cells, blood markers, and bacterial co-infection changes. Twenty-one publications were eligible for full-text examination between 2019 to 2021.ResultThe standard difference in WBC, lymphocyte, and platelet between the two clinical groups was 0.538, -0.670, and -0.421, respectively. Also, the standard difference between the two clinical groups of CRP, ALT, and AST was 0.482, 0.402, and 0.463, respectively. The odds ratios for hypertension and diabetes were significantly different between the two groups. The prevalence of co-infection also in the critical group is higher.ConclusionIn conclusion, our data suggest that critical patients suffer from a suppressed immune system, and the inflammation level, the risk of organ damage, and co-infections are significantly high in the critical group and suggests the use of bacteriostatic instead of bactericides to treat co-infections.

Online learning is becoming more and more popular than traditional learning, and the need to investigate its influence within the framework of instruction and learning domains is — in today's emerging, cutting-edge technology world — an academic trend. The current study's problem intends to explore the impact of virtual versus traditional learning on the academic achievement of EFL students, a subject that has garnered substantial attention from English Language Teaching (ELT) researchers. This study aims to clarify if EFL students' academic progress in a listening skills classroom is influenced by traditional or online learning. The present research compiled information on how EFL students performed when English language teaching took place online as opposed to in a traditional classroom environment. This study, using an experimental research design with (N = 30) pairs of students (both male and female), was held at Najran University, KSA, in the academic year 2022–2023 b y using probability (random sampling). This study used pre-and post-tests to gather data from the subjects of the study, bifurcated into controlled and experimental groups employing the two modes of teaching, viz., online and traditional. The findings of the investigation proved that the experimental group achieved better performance compared to the control group in terms of results and scores. There are no significant differences based on gender. In addition, (N = 20) teachers teaching listening skills to EFL learners participated in semi-structured interviews. The qualitative analysis enlisted flexibility, accessibility, effective communication, collaboration, monitoring of student progress, and the use of a blackboard as constructive elements, followed by maintenance costs, wastage of available resources, long-term engagement, limited face-to-face interaction, and demotivation as critical perspectives. According to the findings of this research, the author recommends further studies with more variables.

Purpose – Policy makers, professional associations and scholars continue to advocate for the integration of enhanced clinical experiences for future teachers’ preparation. These recommendations reflect the growing recognition that few events in preservice teachers’ education are more significant than their experiences in the classrooms of veteran peers. Aware of the fact that the field of teacher education needs examples of effective clinical experiences, the authors examined the “critical, project-based” (CPB) model, employing Photovoice activities in a dropout prevention course in a secondary education partner school at the beginning of the COVID-19 pandemic. This paper aims to discuss the aforementioned objective. Design/methodology/approach – Aware that the field of teacher education needs examples of effective clinical experiences, the authors examined the CPB model, employing Photovoice activities in a dropout prevention course in a secondary education partner school at the beginning of the COVID-19 pandemic. In this article they detail a practitioner research examination that explores the experiences of 12 preservice middle/high school teachers, reporting on these individuals’ considerations of general pedagogies, writing instruction strategies and teaching personas. Findings – Results suggest that preservice teachers might best identify pedagogical practices that are consistent with their nascent teaching identities via experiences that occur in school-university partnerships in which future teachers are positioned as pedagogues. Originality/value – This manuscript explores the use of the “CPB” clinical experience model, identifying the impacts of this approach for preparing future teachers.

Abstract Objective This study aims to construct the risk prediction nomogram model of critical condition in patients with hypertension during pregnancy and to verify its evaluation effect. Methods A total of 531 patients with hypertension during pregnancy were randomly grouped into 427 model group and 104 validation group. The model group patients included 59 cases of critical group and 368 cases of non-critical group according to the occurrence of critical situation. Multivariate Logistic regression analysis was conducted to determine the risk factors of critical condition in patients with hypertension during pregnancy, and R software was used to construct the nomogram model. Moreover, the prediction efficiency of the model was evaluated. Results The proportions of patients aged over 30 years, with an educational background of junior high school or below, a family history of hypertension, anemia during pregnancy, and a lower erythrocyte count were significantly higher in the critical group compared to the non-critical group (P 30 years old, educational background of junior high school and below, family history of hypertension, anemia during pregnancy, and red blood cell count were independent risk factors for the occurrence of critical condition in patients with hypertension during pregnancy (P

The richness of the soil in beneficial and growth-stimulating microorganisms and their symbiotic interactions with plants is a factor to be discovered. This kind of relationships is still poorly studied in cannabis (Cannabis sativa L.). The endomycorrhizal status of cannabis was assessed in two cannabis varieties (Khardala and Critical) cultivated in the Taounate region located in northern Morocco. Soil samples collected from the rhizospheric soil of two cannabis parcels of khardala and critical were studied using the wet sieving method. Rhizospher soil of khardala presents a sporal density of 200 spores / 100 g of soil and a combinaison of 12 different AMF species divided morphologically into 6 genera predominated by Glomus and Funneliformis. In the other hand, 243 spores per 100 g of soil were noted in association with critical variety, the endomycorrhizal fungi community was represented by 15 species of 6 genera predominated by Glomus. Contrary to expectations, cannabis is found to be mycorrhized, this symbiotic association can be exploited in the soon future to develop profitability and cope with different types of stress.

As the emerging field of equity in STEM education continues to grow, debates surrounding how to best serve populations with historically restricted access and successful outcomes within the STEM fields is growing as well. In this article, a group of doctoral students who took a course titled Equity in STEM Education and discusses their takeaways regarding the literature discussed in the course. The topics covered in this article will center around Gutiérrez’s (2007) equity framework of power, identity, access, and achievement. Through the lens of the equity framework, the dimension of power will be used to present discourse on the sociopolitical turn in STEM education. Identity will be used to explore debates on race, poverty, gender, and sexuality within STEM equity and education. Access will be used to examine discussions on students’ ability to achieve STEM equity in rural settings and within specific domains such as computer science. Finally, achievement will be used to explore arguments on both sides of the achievement gap research. To conclude, we urge researchers, educators, and policymakers to listen and act upon the work of contemporary scholars in order to achieve an equitable STEM education system.

In the 21st century, computational thinking (CT) has emerged as a fundamental skill. Building on this momentum and recognizing the importance of exploring the use of computational thinking (CT) concepts and tools in teaching and learning, this study conducted a qualitative content analysis to investigate online resources for school and community outreach practices related to integrating CT and coding into mathematics education. The data set was selected from sample websites hosting a community of practice and interpreted through Kafai et al.’s (2020) framings of CT and a combination of three theories of learning and teaching (i.e., constructionism, social constructivism, and critical literacy). The study found that in mathematics, more attention is given to the cognitive approach of CT, which focuses on acquiring CT skills and concepts, rather than the situated approach that emphasizes participation during learning. Additionally, there is not enough emphasis on the critical framing of CT, which examines how learning reflects values and power structures. The study’s significance is grounded in enhancing the perspectives of researchers, educators, and policymakers by providing insights into the wide affordances of CT which meet and exceed the expectations of curriculum content and skills. In light of the recent attention paid to adding coding to the new mathematics curriculum, this study contributes to the literature, practice, and curriculum development on the integration of CT into school mathematics and serves as a basis for future research in the field.

Early warning signals (EWSs) represent a potentially universal tool for identifying whether a system is approaching a tipping point, and have been applied in fields including ecology, epidemiology, economics, and physics. This potential universality has led to the development of a suite of computational approaches aimed at improving the reliability of these methods. Classic methods based on univariate data have a long history of use, but recent theoretical advances have expanded EWSs to multivariate datasets, particularly relevant given advancements in remote sensing. More recently, novel machine learning approaches have been developed but have not been made accessible in the R (www.r‐project.org) environment. Here, we present EWSmethods – an R package that provides a unified syntax and interpretation of the most popular and cutting edge EWSs methods applicable to both univariate and multivariate time series. EWSmethods provides two primary functions for univariate and multivariate systems respectively, with two forms of calculation available for each: classical rolling window time series analysis, and the more robust expanding window. It also provides an interface to the Python machine learning model EWSNet which predicts the probability of a sudden tipping point or a smooth transition, the first of its form available to R users. This note details the rationale for this open‐source package and delivers an introduction to its functionality for assessing resilience. Context: Oral health literacy (OHL) has emerged as an important dimension of oral health care with the potential to have multiple applications in a health-care setting. Most of the tools that measure were developed out of India. Aim: Since OHL is a contextually sensitive construct, this study developed and validated a tool to measure OHL among adults in India. Material and Methods: A questionnaire covering all three domains of OHL was framed in two languages using the "dual development approach." After content validation, the 20-item questionnaire was subjected to test-retest, pretest, and pilot test. It was then administered to 200 adults in a dental college and five private clinics. Socio-demographics, Decayed, Missing, Filled Permanent Teeth (DMFT) and Oral Hygiene Index– Simplified (OHI-S) were recorded to establish the construct and predictor validity of the tool. Results: The results showed that the overall reliability of the questionnaire was high, with Cronbach's alpha of 0.81. The mean OHL score was 15.4, with a minimum score of 2 and a maximum of 20. A statistically significant correlation was found between socioeconomic status and OHL as an indicator of convergent validity (r = 0.62). The discriminant validity of the questionnaire was assessed by comparing OHL levels with education levels using the Kruskal–Wallis test, and statistically significant results were derived. Significant associations (P < 0.001) between DMFT and OHL; and OHI-S and OHL were found with an adjusted odds ratio of 3.01 and 4.78, respectively, establishing its predictive validity. The surname Fitzpatrick is readily identified as Irish. Until recently, the traditional Fitzpatrick surname narrative was of a medieval super-progenitor named Giolla Phádraig. His offspring, the eponymous Mac Giolla Phádraig, it was said, somehow came to dwell in every Irish province; yet this is an Irish surname myth that works to erase the history of ancient 'Fitzpatrick' clans. This article demonstrates how deconstructing the surname Fitzpatrick, through working the hyphen of gene-eaology, is a practice of decolonisation. Via genetic data and archival records, dominant clan identities are disrupted, while connections with lost clans are re/membered. Critical analysis dismantles the dominant narrative imposed by colonial strategies and reconnects people with kinship groups and forgotten forebears. Abstract Background The concept of critical value is not evident in surgical pathology, and there is no established protocol for determining, reporting, and documenting these results. Materials and methods A questionnaire was designed regarding critical value in surgical pathology, and all pathologists and some clinicians from five laboratories were asked to participate through an invitation link. The most important items were selected, and all pathologists were instructed to follow a standard operating procedure to deal with critical results for a year. Results A total of 43 pathologists and 44 non-pathologists participated in the study. Some critical or unexpected items were selected. Most participants agreed that the optimal time to announce critical reports is within 24 h of establishing the final diagnosis, and a phone call was the most dependable communication option. In addition, the most qualified recipients were the attending physicians. Therefore, a written policy was implemented for a year. One hundred seventy-seven critical or unexpected cases (0.5%) were detected. Mucormycosis and cytomegalovirus (CMV) were the most frequent critical cases. Conclusion There are no set criteria for critical items or the reporting process in surgical pathology. It is possible to establish more uniform norms for reporting these cases by boosting pertinent research efforts and recruiting more pathologists and physicians. Additionally, it is advised that each medical facility compile its own unique critical or unexpected diagnosis list.

Abstract Introduction Critical H1N1 pneumonia patients usually have one of the symptoms such as respiratory failure, septic shock, multiple organ dysfunction, or other need for intensive care management, which are associated with high risk of mortality. It is essential to differentiate the severity of H1N1 pneumonia and take corresponding target treatments. Objectives We aim to investigate the differences in clinical characteristics and chest computed tomography (CT) findings between severe and critical patients with H1N1 pneumonia. Methods A total of 27 patients diagnosed with H1N1 pneumonia from October 2018 to March 2019 were retrospectively analyzed, and the differences in clinical manifestations, laboratory tests, and chest CT findings between the severe group (15 patients) and the critical group (12 patients) were compared. Results Frequency of dyspnea at rest was higher in critical group than that in severe group (P = 0.019). The neutrophil percentage was higher (P = 0.014) and the lymphocyte percentage was lower (P = 0.025) in critical compared with severe group. Bilateral lung involvement was the predominant pattern in both severe and critical patients, whereas the number of involved lobes in critical patients was more than that in severe patients (P = 0.024). Peripheral distribution was the predominant pattern in severe patients (40%), whereas more diffuse involvement of the lungs was observed in critical patients (83.30%). Ground‐glass opacities and consolidation were the main CT findings in both groups, and prevalence of consolidation was higher in critical relative to severe group (83.30%). Conclusion Compared with severe patients, those with critical H1N1 pneumonia were more likely to present with dyspnea at rest and decreased lymphocyte percentage. Chest CT showed that diffuse bilateral involvement and higher prevalence of consolidation are associated with critical outcomes.

The area under the curve (AUC) of the nomogram in the modeling group for predicting the risk of critical situations was 0.926 (95% CI = 0.887 ~ 0.964), indicating excellent discrimination. The calibration curve closely resembled the ideal curve, demonstrating good agreement between the predicted and actual values. The AUC of the validation group's nomogram to predict the risk of critical situation was 0.942 (95% CI = 0.872 ~ 0.998), with good discrimination. The calibration curve was close to the ideal curve, and the actual value was in good agreement with the predicted value. Conclusion: The nomograph model can predict the risk of critical condition in patients with hypertension during pregnancy and screen high-risk population. [ABSTRACT FROM AUTHOR]

Palaeo weathering during the Cretaceous–Eocene interval is most favorable for bauxitization, i.e., transport and deposition in traps on the karstified surfaces of the Mediterranean karst bauxite belt, including the Parnassos–Ghiona bauxite deposit. Resources of lithium (Li), a critical metal of strategic significance in karst-type bauxite deposits, have attracted significant attention in recent years. Due to the discovery of the Li enrichment in certain karstic bauxite deposits in Europe and particularly in China, this review study is focused on the unexplored Li content in the Parnassos–Ghiona (Greece) bauxite deposit, aiming to improve the understanding of the major controlling factors for their origin and enrichment of critical metals. The presence of thin (up to 50 cm) pyrite-bearing coal seams and carbonaceous facies on top of the Parnassos–Ghiona bauxite deposit, at the transition between B3 (the youngest) bauxite horizon and the occurrence of multicolor bauxite ores reflecting a multistage evolution and changes in the mineralogy and geochemistry, is a common feature with other bauxite deposits in Europe and elsewhere. The organic matter, such as microorganisms in coal layers and carbonaceous layers (derived from plants and algae growing in paleo-mires after a regression of the sea), contains Li, which is derived from seawater, as suggested by a positive correlation with B (a seawater component as well). The available geological, mineralogical, and geochemical data highlight the particular significance of coal layers and carbonaceous layers overlying bauxite bodies as a driving force for redox reactions and as a source of Li in the exploration of karst-type bauxite deposits. This essay follows a somewhat unconventional approach to writing about Indian dance in the diaspora. I say "unconventional" because it unfolds as a kind of self-reflexive narration of my own journey as a "doubly diasporic" Indian dancer, born in Singapore but having made my career in North America. In essence, I map my own unconventional paths to understanding Indian dance in the diaspora, outside the tired and troublesome idea of "dance as heritage". The aim of this critical meditation on my own work is to offer up new possibilities for moving Indian dance into progressive conceptual spaces that direct it out of the discursive field of cultural nationalism that frames the idea of "heritage". Abstract Critical congenital heart diseases (CCHD) are important causes of mortality and morbidity in the newborn period. Even after diagnosis, their management could be seriously compromised by the unplanned delivery in hospitals with limited expertise and resources. The newborn may spend days or week before transport putting a significant burden on the neonatal team to manage such challenging diseases. In this review, the management principles of each individual pathology are discussed in the setting before transport to cardiac centers. Understanding these principles will help the treating teams evaluate and manage those complex conditions. The review avoids the advanced discussion on the management of CCHDs not applicable to the pre-transport setup. It highlights the critical elements in the maintenance of normal hemodynamics specific to each pathology and their variation.

Background Fixed and dilated pupils (FDPs) have become synonymous with devastating neurological damage and brainstem injury commonly associated with mass effect and herniation. Infrequently, changes in pupillary light response have been described with seizures; however, the loss of pupillary response with documented Neurological Pupil index has not been well established in patients with seizures. Case Report We present a case report describing a middle-aged female patient with focal status epilepticus with intermittent FDPs. An abnormal pupillary response occurred with right hemispheric lateralized periodic discharges and resolution with anti-seizure medication escalation. To our knowledge, this is the first description of objective documentation of reversible FDP as a possible clinical correlate of lateralized periodic discharges. Conclusion The use of handheld automated pupillometry in conjunction with electroencephalogram (EEG) has provided the therapeutic direction. Further research is warranted to fully describe the mechanistic underpinnings of these observations.

Low albumin level is a risk factor for increasing mortality among patients in the intensive care unit (ICU). This study investigated the effect of albumin infusion on critical COVID-19 patients with hypoalbuminemia.Methods: A total of 114 COVID-19 ICU patients with hypoalbuminemia were recruited from Wuhan Leishenshan Hospital and Zhongnan Hospital of Wuhan University. Clinical features and laboratory variables were collected through electronic medical records. The cohorts were divided into two groups: albumin infusion and non-albumin infusion. Propensity-matched analysis was used to compare patients who received albumin to controls. Statistical analyses were used to investigate the survival time and inflammation-related blood biomarkers between groups.Results: Lactate dehydrogenase, interleukin (IL)-6, IL-2 receptor, and IL-8 levels were significantly downregulated in the albumin infusion group. Significant upregulations of lymphocyte counts and IL-10 were found in the albumin infusion group. There was a negative association between albumin level and D-dimer or procalcitonin levels after treatment. The albumin infusion group had a significantly longer survival time and shorter hospitalization time than control patients. Notably, a 1g increase in albumin level reduced the risk of death by approximately 7.3% after adjusting for age and sex. Patients with increased albumin levels after treatment had better prognoses than those without.Conclusion: Albumin administration can regulate COVID-19-related biomarkers and reduce the risk of death in critical patients with hypoalbuminemia. Clinicians should pay more attention to these risk factors. Targeted clinical interventions should be implemented to minimize the negative impacts of hypoalbuminemia and improve disease outcomes.Keywords: albumin, COVID-19, critical, prognosis, hypoalbuminemia

Ana sorunsalı çözümlemek üzere teorik bir çerçeve olarak nörobilim araştırmaları açısından konuya yaklaşılmıştır. Söz konusu teorik çerçeve, nörobilim araştırmalarındaki organik sinir ağlarının yapısını, yapay zeka çalışmalarında yapay sinir ağlarını geliştirmekte kullanan, bağlantılar ve etkileşimler üzerine sosyal göndermeleri de bulunan bir bakış açısıdır. Bu bakış açısı aynı zamanda makalenin, yaşamın bağlantısal bütünlüğü olarak tanımlanan fikirsel temelini oluşturmaktadır. Bu fikirsel temeli açıklayabilmek üzere kritik ve klinik kavramları ile araştırma amacında yer bulan teşhis ve tedavi süreçlerinin yeniden biçimlenişi, birbirine ilişkilendirmek suretiyle yöntemsel bir işleyiş geliştirilmiştir. Sağlıkta dönüşümün yeni bilimsel ve toplumsal koşullarıyla kavranabilmesi, dijitalleşen toplumu tıp açısından çözümleyebilmek için önem arz etmektedir. Bağlantısallık temelli bir toplumsallık içinde gerçekleşen bu dönüşümün, sağlık alanında dijitalleşme açısından insanın ve teknolojinin bir araya geldiği yeni bir toplumsallığın ipuçlarını sunan bir bileşim olduğu tespit edilmiştir. ABSTRACT: This article examines the new social perspective that the digital health transformation issue, which is emerging in the context of artificial intelligence research and applications, is produced through this current interaction of human societies and intelligent machines. By examining this problematic, it is aimed to understand what the current formation of diagnosis and treatment processes means in terms of social change. In order to analyze the main problematic, the subject has been approached in terms of neuroscience research as a theoretical framework. The theoretical framework in question is a perspective that uses the structure of organic neural networks in neuroscience research to develop artificial neural networks in artificial intelligence studies and has social references on connections and interactions. This point of view also constitutes the intellectual basis of the article, which is defined as the interconnected whole of life. A methodical operation has been developed by associating the critical and clinical concepts to clarify this intellectual basis, with the remodeling of the diagnosis and treatment processes that took place in the research aim. Understanding the transformation in health with its new scientific and social conditions is important in order to analyze the digitalized society in terms of medicine. It has been determined that this transformation, which takes place in an interconnectivity-based sociality, is a combination that offers clues of a new sociality in which human and technology come together in terms of digitalization in the field of health.
