National audience; La nutrition foetale est maintenant reconnue comme un facteur de risque pour le développement du syndrome métabolique (SMet) et du diabète de type 2 à l’âge adulte. Cette étude cas-témoins nichée au sein de la cohorte Haguenau a pour objectif d’identifier des biomarqueurs précoces et/ou prédictifs du SMet, associés ou non à l’empreinte foetale par une approche protéomique ciblée. Elle utilise les données anthropométriques et sociales d’individus nés à la maternité d’Haguenau et identifiés avec ou non un petit poids de naissance ajusté pour l’âge gestationnel (petit poids=SGA, poids normal=AGA). Nous avons sélectionné des sujets SGA, sains à 20 ans (T1, n=56), ayant développé (SMet30, n=27) ou non (non SMet30, n=29) un SMet à 30 ans (T2). Une analyse protéomique ciblée a été réalisée sur 56 échantillons biologiques prélevés à T1 en utilisant des biopuces à protéines. Les résultats ont été analysés pour mettre en évidence des modulations de protéines et identifier des biomarqueurs prédictifs de l’apparition du SMet à T2. Une étape de validation a été effectuée (n=12) pour tester la robustesse du modèle prédictif obtenu. Les résultats ont révélé la variation significative de 47 protéines entre cas et témoins à T1, soit plus de 7 ans avant l’apparition du SMet, et avec un ratio de variation supérieur à 1,5 (n=45) ou inférieur à 0,65 (n=2). Elles sont en majorité impliquées dans la régulation de l’angiogenèse (n=7), de l’inflammation (n=5) ou encore du métabolisme (n=3). D’autres participent à la prolifération cellulaire ou sont impliquées dans le développement foetal (n=5).Un modèle de régression logistique a permis de retenir 4 protéines avec une capacité prédictive. Ce projet va contribuer à renforcer nos connaissances sur le SMet et l’empreinte foetale; il va permettre l’identification précoce des sujets à risque pour améliorer leur prise en charge avec une meilleure stratégie préventive.

The direction of the association between mental health and adiposity is poorly understood. Our objective was to empirically examine this link in a UK study. This is a prospective cohort study of 3 388 people (men) aged ? 18 years at study induction who participated in both the UK Health and Lifestyle Survey at baseline (HALS-1, 1984/1985) and the re-survey (HALS-2, 1991/1992). At both survey examinations, body mass index, waist circumference and self-reported common mental disorder (the 30-item General Health Questionnaire, GHQ) were measured. Logistic regression models were used to compute odds ratios (OR) and accompanying 95% confidence intervals (CI) for the associations between (1) baseline common mental disorder (QHQ score > 4) and subsequent general and abdominal obesity and (2) baseline general and abdominal obesity and re-survey common mental disorders. After controlling for a range of covariates, participants with common mental disorder at baseline experienced greater odds of subsequently becoming overweight (women, OR: 1.30, 1.03 – 1.64; men, 1.05, 0.81 – 1.38) and obese (women, 1.26, 0.82 – 1.94; men, OR: 2.10, 1.23 – 3.55) than those who were free of common mental disorder. Similarly, having baseline common mental health disorder was also related to a greater risk of developing moderate (1.57, 1.21 – 2.04) and severe (1.48, 1.09 – 2.01) abdominal obesity (women only). Baseline general or abdominal obesity was not associated with the risk of future common mental disorder. These findings of the present study suggest that the direction of association between common mental disorders and adiposity is from common mental disorder to increased future risk of adiposity as opposed to the converse

The overall objective of the project is to develop accurate and robust markers of the evolution of health status toward metabolic syndrome (MetS), and to determine to what extent nutrition is a major determinant, by using a multidisciplinary approach, putting together sociology, epidemiology, nutrition, statistics, and computer science. The project uses the French population-based cohort GAZEL, an on-going epidemiological study set up in 1989 (around 20,000 volunteers) among employees of the French national Gas and Electricity Company. The study consists in integrating demographic, socioeconomic, clinical, and biological data (from annual questionnaires, including food frequency questionnaires (FFQs)) to analyse food trajectories between 1998 and 2009. A case-control approach is used within a sub-cohort to characterize metabolic signatures and identify early discriminant factors predictive of MetS development. Metabolic syndrome (MetS), an important component of insulin resistance and cardiovascular (CV) risk, is defined by 3 or more of the following characteristics: abdominal obesity, hyperglycemia, hypertension, hypertriglyceridemia, and hypo-high-density lipoprotein cholesterolemia. Based on the previously published age- and sex-mediated DESIR (Data from an Epidemiological Study on the Insulin Resistance Syndrome) cohort and parallel central hemodynamic measurements, our goal was to evaluate the effects of MetS on brachial central pulse pressure (PP), PP amplification, aortic stiffness, and wave reflections. These data were then compared with those of patients with essential hypertension but without MetS for the same mean arterial pressure. Increased aortic stiffness, a major mechanical factor predicting CV risk, has been well identified as playing a role in MetS. Its age progression is proportional to the number of risk factors involved in MetS and is responsible for increased systolic blood pressure and decreased diastolic blood pressure with increasing age, the principal hallmarks of hypertension in the elderly. Beyond brachial pressure measurements, central hemodynamic parameters involve increased aortic stiffness, reduced wave reflections, and increased PP amplification, a parameter commonly associated with increased heart rate. With the exception of arterial stiffness, all these findings are opposite in direction to those observed in essential hypertension, in which MetS is absent. A divergent behavior of wave reflections and PP amplification, but not of arterial stiffness, is observed when hypertension is studied alone or when compared with MetS for the same mean arterial pressure. This pulsatile hemodynamic abnormality contributes independently to increase age- and sex-mediated CV risk, justifying new research regarding Framingham scores and drug treatment. © 2013 American College of Cardiology Foundation.

The increased severity of obstructive sleep apnoea syndrome (OSAS) is associated with a parallel increase in the incidence of cardiovascular events. Whether the increased severity of OSAS is in fact associated with impaired arterial properties has never been thoroughly studied. In patients with OSAS who carry a high burden of cardiovascular risk factors, we investigated whether the severity of OSAS is associated with deterioration in the arterial properties, independent of classical cardiovascular risk factors. In 74 consecutive patients with OSAS, we non-invasively assessed, by means of tonometry and high-resolution ultrasound: carotid intima-media thickness (IMT), carotid diameter and plaques, carotid-femoral pulse wave velocity (PWV), central augmentation index (AI) and central blood pressures. The respiratory disturbance index was an independent predictor of IMT and PWV but not of carotid plaques, carotid diameter, AI or central blood pressures. Several parameters of nocturnal hypoxaemia were independently correlated with carotid IMT and PWV. In conclusion, arterial stiffening and thickening are modulated by the severity of OSAS, independently from age and cardiovascular risk factors.
