1. Can we reduce the stigmatisation experience with psychosocial interventions? An investigation of the meeting centre support programme impact on people with cognitive impairments

Abstract

Introduction People living with dementia or mild cognitive impairment (MCI) experience stigmatisation and there are not many specific psychosocial interventions dedicated to help them coping with this issue, reducing its impact on their lives. Objectives This study aimed to a) investigate the stigmatisation level among people with dementia and MCI in Poland, Italy and the United Kingdom and b) assess the role of the Meeting Centre Support Programme (MCSP) in decreasing stigmatisation. Methods We investigated outcomes for 114 people with dementia and MCI living in Italy, Poland and the UK who participated 6 months in MCSP or usual care (UC) using a pre/post-test control group study design. Level of stigmatisation was assessed with the Stigma Impact Scale: neurological impairment (SIS). Results Stigmatisation level (SIS) among participants varied from 2 to 65 (median=33.5; Q1=27; Q3=41) with people from the UK experiencing a statistically significantly higher level of stigmatisation than people in Italy and Poland. In Italy, stigmatisation was lower (p=0.02) in the MCSP group following the intervention. In Poland, the social isolation level did not significantly change in MCSP, but increased (p=0.05) in UC. In the UK, the social rejection level raised (p=0.03) in MCSP. Overall, the combined data of the three countries did not show statistically significant differences in SIS between MCSP and UC. Conclusions Stigmatisation among people with dementia and MCI is complex and seems culturally dependent. There is a great opportunity in psychosocial interventions to reduce the burden of stigma among people with dementia which requires further investigation. Disclosure No significant relationships.

Abstract

El sector norte del partido de Azul (centro de la Provincia de Buenos Aires) corresponde predominantemente a un' paisaje de llanura aluvial ocupado por pastizales naturales sobre los que se desarrolla la cría vacuna como actividad productiva. Con el propósito de estudiar las relaciones vegetación-ambiente en dicho paisaje, se describieron la fisiografía y las comunidades vegetales a lo largo de una transección transversal a las geoformas dominantes. En base a un relevamiento topográfico, fotointerpretación y análisis de cartas topográficas y de suelos se identificaron seis ambientes fisiográficos. En cada uno de ellos se relevó la vegetación, utilizando el método fitosociológico, quedando definidos 19 grupos florísticos y 6 comunidades. La participación relativa de las comunidades dentro de cada ambiente estuvo condicionada por las geoformas predominantes y las características del microrrelieve.

Abstract

Abstract Background The MEETINGDEM study aims to implement and evaluate an innovative, inclusive, approach to supporting community dwelling people with mild to moderate dementia and their family carers, called the Meeting Centers Support Program (MCSP), in three countries in the European Union (EU): Italy, Poland and United Kingdom. Demonstrated benefits of this person-centered approach, developed in The Netherlands, include high user satisfaction, reduced behavioral and mood problems, delayed admission to residential care, lower levels of caregiving-related stress, higher carer competence, and improved collaboration between care and welfare organizations. Methods The project will be carried out over a 36 month period. Project partners in the three countries will utilize, and adapt, strategies and tools developed in the Netherlands. In Phase One (month 1-18) activities will focus on establishing an initiative group of relevant organizations and user representatives in each country, exploring pathways to care and potential facilitators and barriers to implementing the program, and developing country specific implementation plans and materials. In Phase Two (month 19‑36) training will be provided to organizations and staff, after which the meeting centers will be established and evaluated for impact on behavior, mood and quality of life of people with dementia and carers, cost-effectiveness, changes in service use, user satisfaction and implementation process. Discussion An overall evaluation will draw together findings from the three countries to develop recommendations for successful implementation of MCSP across the EU. If the Meeting Centers approach can be widely implemented, this could lead to major improvements in dementia care across Europe and beyond. The waste-to-energy (WTE) incinerator plant located in the Turin area (Italy) started to recover energy from the combustion of municipal solid waste in 2013. A health surveillance program was implemented to evaluate the potential health effects on the population living near the plant. Abstract

Objective: To assess the accuracy of noninvasive parameters, fecal calprotectin (FC), increased bowel wall thickening (BWT) at intestinal ultrasound (IUS) and blood inflammatory indexes (BII), alone or in combination, as diagnostic tools for inflammatory bowel disease (IBD) in pediatric patients. Methods: Retrospective data were collected on consecutive children (age 2–18 years) referred to our pediatric gastroenterology clinic, for recurrent abdominal pain and/or altered bowel habit from 2007 to 2013. Subjects who had diagnostic workup: laboratory tests (FC, BII, white blood cell (WBC), C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR)) and IUS as initial assessment were eligible. Subjects with known gastrointestinal (GI) diseases, or signs or symptoms highly suggestive for organic diseases necessitating prompt endoscopy (e.g., perianal disease or rectal bleeding), or who had recently performed endoscopy were excluded. The accuracy of noninvasive tests for detecting IBD was assessed using endoscopic and/or radiological investigations, performed in subsequent clinical follow up, as reference gold standard. Results: Seventy-seven patients (mean age 11.3, 44 males) were included, 23 (29.9%) with a final diagnosis of IBD. As single tests, FC gave the highest sensitivity (96%) but lower specificity (72%) and IUS highest specificity (96%) with lower sensitivity (70%). The combination of FC + IUS showed excellent accuracy for detecting children with IBD with positive predictive value: 100%; negative predictive value: 88.5%. The probability of IBD in children with normal FC, BII and IUS was 0.09%. Conclusions: FC and increased BWT at IUS are accurate to guide reassurance or proceeding with further invasive procedures for detecting IBD in children with mild GI symptoms.

Abstract

The LHC experiments are undergoing important upgrades for the High Luminosity LHC project. Experiments will be required to operate under three times higher instantaneous luminosity and higher levels of radiation when compared to LHC Run2. The integrated luminosity will increase by a factor ten. To cope with these conditions resistive Micromegas chambers were chosen by the ATLAS experiment as one of the technologies for the upgrade of the first forward muon stations. This paper describes the results of two prototype Micromegas detectors that were installed in the CERN Gamma Irradiation Facility with the aim to evaluate the detector performance under high-rate irradiation and to assess ageing effects over a 2.5 year long irradiation period and an accumulated charge in excess of 0.3 C/cm$^2$.

Abstract

Introduction &amp, Aim: Anti-tissue transglutaminase antibody (tTGA) titer is used during the follow-up of celiac patients to evaluate gluten-free diet (GFD) responsiveness. However, no clear data are available on this issue. The aim of this study was to evaluate tTGA significance during celiac disease (CD) monitoring. Methods: From January 2017 to January 2020, consecutive CD patients on a GFD with persistent positive tTGA were enrolled. Antibody titres were evaluated on a yearly basis from CD diagnosis to the last follow-up. Urinary gluten detection tests, duodenal histology and capsule enteroscopy (CE) were performed. A tTGA-positive cohort was compared with a control group composed of 212 treated CD patients with negative tTGA. Results: 65 patients (12% males, median age at enrollment and CD diagnosis, 37 (14–86) and 31 (1–76), respectively, median follow up 4 (1–26) years) presented with positive tTGA during follow-up. Overall, the tTGA titres were 3 (1–79) fold increased (ULN). Three different tTGA trends were recognized: (I) 36 (55%) patients with a progressive titresdecrease, (II) 16 (25%) patients with a fluctuating behavior, (III) 13 (20%) patients with a steady state or increased titres. tTGA+ patients did not present with different clinical and demographic parameters. Duodenal atrophy was present in 10% vs 36% of the tTGA positive vs negative group (p &lt, 0.005), respectively. Gluten detection results were positive in 3 (8%) cases, all in the III group. In tTGA+ patients, CE did not identify any CD-related complications. Conclusions: tTGA positivity during CD follow up did not present a relevant clinical significance without association with autoimmune comorbidities and mucosal damage.

Abstract

Alexithymic traits, which entail finding it difficult to recognize and describe one’s own emotions, are linked with poor trait emotional intelligence and difficulties in identifying and managing stressors. There is evidence that alexithymia may have detrimental consequences for well-being and health, beginning in adolescence. In this cross-sectional study, we investigated the prevalence and incidence of alexithymia in teenage girls, testing the statistical power of trait emotional intelligence (TEI) and student burnout to discriminate between high- and low-alexithymic subjects. A sample of 884 female high school students (mean age 16.2 years, age range: 14-19) attending three Italian academic-track high schools (social sciences and humanities curriculum) completed self-report measures of alexithymia, school burnout, and TEI. Main descriptive statistics and correlational analysis preceded the discriminant analysis. The mean alexithymia scores suggest a high prevalence of alexithymia in female adolescents; as expected, this trait was negatively correlated with TEI and positively associated with school burnout. Participants with high vs low alexithymia profiles were discriminated by a combination of TEI and burnout scores. High scores for the emotionality and self-control dimensions of TEI were strongly associated with membership of the low alexithymia group; high scores for the emotional exhaustion dimension of school burnout were indicative of membership of the high alexithymia group. These findings suggest crucial focuses for educational intervention: efforts to reduce the risk of emotional exhaustion and school burnout should especially concentrate on enhancing emotional awareness and self-control skills, both strongly associated with low levels of alexithymia.

Abstract

Students' school burnout has been extensively investigated in relation to interpersonal factors such as peer relations and social adjustment. However, few studies have examined the role of individual traits such as empathic skills. Our aim in this study was to test, within a single comprehensive model, how students' empathic skills affect their levels of school burnout, both directly and indirectly via satisfaction with school relationships. A sample of 998 high school students (aged 14 to 19 years) took part in this cross-sectional study. Participants completed quantitative self-report measures of school burnout, empathic skills (both cognitive and affective), and satisfaction with school relationships (peers and teachers). Using structural equation modeling, we tested a conceptual model in which emphatic skills were hierarchically associated with satisfaction about school relationships and school burnout, while also controlling for age. The structural equation model offered an excellent fit for the empirical data. Analysis of the total, direct, and indirect effects showed that empathic skills were associated with both satisfaction about school relationships and school burnout. Satisfaction with school relationships appeared to mediate the relationship between empathy and school burnout. Students' age was also found to have statistically significant effects. The negative effect of high school students' empathic skills on their risk of school burnout may be prevented or at least reduced by helping them to develop positive and satisfying relationships with both teachers and peers.

Abstract

The main aim of the current study was to investigate the role of trait emotional intelligence (TEI) in preventing students&rsquo, school burnout directly and indirectly via anxiety and academic resilience. The data were derived from a sample of 1235 high school students (962 females and 273 males), ranging in age between 13 and 17 years (mean = 15.46, stand deviation = 1.22). Structural equation modelling revealed a strong indirect effect of TEI on school burnout, mediated via anxiety and resilience. Overall, students with high TEI were less likely to experience school anxiety and more likely to exhibit resilience which, in turn, reduced school burnout risk. Findings are discussed with reference to the wider role of TEI in educational contexts and highlight the need and potential for scientifically driven interventions to enhance emotional adjustment at school and in life, more generally.

Abstract

Radio-chemotherapy (RCT) is the primary treatment of anal cancer (AC). However, the role and the optimal total dose of a radiation boost is still unclear. No randomized controlled trials nor systematic reviews have been performed to analyze the efficacy of brachytherapy (BRT) as boost in AC. Therefore, we performed this systematic review based on PRISMA methodology to establish the role of BRT boost in AC. A systematic search of the bibliographic databases: PubMed, Scopus, and Cochrane library from the earliest possible date through January 31, 2018 was performed. At least one of the following outcomes: local control (LC), loco-regional control (LRC), overall survival (OS), disease-free survival (DFS), or colostomy-free survival (CFS) had to be present for inclusion in this systematic review in patients receiving a BRT boost. Data about toxicity and sphincter function were also included. Ten articles fulfilled the inclusion criteria. All the studies had retrospective study design. All studies were classified to provide a level of evidence graded as 3 according to SIGN classification. Median 5-year LC/LRC, CFS, DFS, and OS were: 78.6% (range, 70.7-92.0%), 76.1% (range, 61.4-86.4%), 75.8% (range, 65.9-85.7%), and 69.4% (63.4-82.0%), respectively. The reported toxicities were acceptable. RCT is the treatment cornerstone in AC. High-level evidences from studies on BRT boost in AC are lacking. Further studies should investigate: efficacy of BRT boost in comparison to no boost and to external beam boost, patients who can benefit from this treatment intensification, and optimal radiation dose.

Abstract

Background/aim The primary endpoint of this phase I study was the maximum tolerated dose (MTD) of middle half body (MHB) accelerated radiotherapy (RT) in multiple bone metastatic (BM) prostate cancer (PCa) patients. Patients and methods Three step dose escalation [13 Gy (3.25 Gy/fraction), 14 Gy (3.5 Gy/fraction), and 15 Gy (3.75 Gy/fraction)] in three consecutive patient cohorts were planned. RT was delivered in two consecutive days and two daily fractions. Six patients were enrolled in the first two cohorts and 12 in the third cohort. Grade ≥3 toxicity was considered as a dose-limiting toxicity (DLT). Results Twenty-five patients (median age=71 years, median follow-up=7.4 months) were enrolled. Defined MTD dose was 15 Gy. Overall pain response rate was 76%: 9 patients (36%) showed complete and 10 patients (40%) reported partial response of pain. Conclusion MHB accelerated RT (total dose: 15 Gy) delivered in two consecutive days and two daily fractions is well tolerated.

METHODS: We evaluated data of 652 patients treated with BNSRP for clinically localized prostate cancer (PCa). Erectile dysfunction (ED) was classified according to the IIEF-EF domain score. Return to baseline EF was defined as patients who reached the same preoperative ED category during the 3-year follow-up. Cox regression analyses were fitted to predict return to baseline IIEF-EF and to predict OS defined according to the IIEF-OS in the overall population. Logistic regression analyses were performed to analyze OS in men who reached the back to baseline status. MAIN OUTCOME MEASURES: The outcome of the study was to evaluate back to baseline EF status and to correlate it with postoperative OS. RESULTS: Preoperative satisfaction was reported by 218 (33.4%) patients. Postoperative satisfaction was achieved by 103 patients. Overall, 383 patients were able to achieve the preoperative IIEF-EF score. However, only 26.9% reported being satisfied. Age and preoperative IIEF-EF score were significantly associated with baseline IIEF-EF recovery (all P ≤ .02). Patients who were able to return to baseline IIEF-EF were more likely to be satisfied (P < .001). Time elapsed between surgery and achievement of baseline IIEF-EF was significantly associated with OS (P < .001). Among patients who were able to achieve the baseline IIEF-EF score, a preoperative IIEF-EF of 22-25 and 26-30 was significantly associated with postoperative satisfaction (all P < .001). CONCLUSION: After BNSRP, reaching the baseline IIEF-EF score is not always sufficient to obtain patient satisfaction. Abstract

Aim To define safety and efficacy of a palliative, short-course accelerated radiation therapy for symptomatic locally advanced primary pelvic cancer. Materials and methods A phase II trial was planned based on the minimax Simon's two-stage design. A total of 18 Gy in 4.5 Gy/fraction administered twice a day was delivered (SHARON). Pain and quality of life were recorded according to the Visual Analogue self-assessment and the cancer linear analog scales (CLAS), respectively. Results Twenty-five patients were enrolled in the study. The most frequent baseline symptoms were pain (48%), bleeding (40%), bleeding/pain (8%), and intestinal sub-occlusion (4%). The overall palliative response rate was 96.0%, with a median palliative duration of 6 months. An improvement of quality-of-life indices (well-being, fatigue, and ability to perform daily activities) was noted in 64.0%, 36.0%, and 48.0% of patients, respectively. Conclusion The SHARON regimen was well tolerated and effective in the palliative treatment of patients with locally advanced pelvic cancer. Based on these results, a multicentric prospective phase III trial is ongoing to compare this regimen with traditional 2-week radiotherapy treatment.

Abstract

The purpose of the present study was to propose an optimized 2D technique (2D-conformal) for radiotherapy (RT) of pancreatic cancer (CaP). This technique is based on double simulation which resolves the problems of radiographic image distortion. Five patients with locally advanced CaP were identified and enrolled. Treatment planning was simulated in 3 different ways: Two dimensional-standard (2D-SRT), 2D-conformal (2D-CRT), and three dimensional-conformal (3D-CRT) techniques for 10 MV LINAC. Simulation for a cobalt machine was also performed using only the 2D techniques. 2D-SRT technique was planned with fields definition based on anatomical landmarks (bone and duodenum). 3D-CRT was planned with standard virtual simulation technique, and 3D dose evaluation and optimization. 2D-CRT technique was based on manual information transfer from a diagnostic CT-scan to simulation radiograms. To eliminate the X-ray image distortion, a double simulation was employed and the profile of the GTV was delineated on radiographs bearing the simulator isocenter into the target center. Abstract

Aim To determine the maximum tolerated dose (MTD) of a short-course accelerated radiotherapy and its feasibility for symptomatic palliation of advanced head and neck cancer or head and neck metastases from any primary site. Patients and methods A phase I trial in four dose-escalation steps was planned: total dose ranged between 14 and 20 Gy in a total of four fractions administered twice a day. The dose-limiting toxicity (DLT) was determined as grade 3 or more toxicity occurring during treatment. The MTD obtained was used to plan a phase II trial. Results A total of 48 patients were treated. In the phase I trial, the 20 Gy dose level was determined to be the MTD. In the phase II trial, the palliative response rate was 82.7%, with a median duration of palliation of 3 months. Conclusion Short-course accelerated radiotherapy was well tolerated and effective for palliation. These findings may help design future prospective randomized studies.

Abstract

This study investigated the utilitarian and emotion-related components of morality, both as a function of gender and age and in relation to hostile vs. pro-social behaviors in bullying. We presented 45 primary and 62 middle school students with different types of moral and control dilemmas to assess their acceptance of utilitarian behaviors that did or did not violate moral norms (with high or low emotional involvement) in relation to their propensity to assume hostile and/or pro-social roles. We found greater acceptance of utilitarian solutions to dilemmas in boys, older children and those inclined to take on hostile roles. An inverse association was found between utilitarian styles and pro-social roles, particularly when utilitarian thinking violated a moral norm with high emotional involvement. We discuss these results in terms of different functioning styles in typically developing children.

Abstract

Resistive strip Micromegas detectors have already proved to be suitable for precision tracking in dense particle rate environment up to few kHz/cm2. In order to achieve higher rate capability, with low occupancy, fine-segmented strips could be replaced by few mm2 pads. A first prototype, with readout and resistive pads connected by resistors embedded in an insulating layer, has been constructed at CERN and first performance studies are presented.

Abstract

In view of the use of Micromegas detectors for the upgrade of the ATLAS muon system, two detector quadruplets with an area of 0.3 m2 per plane serving as prototypes for future ATLAS chambers have been constructed. They are based on the resistive-strip technology and thus spark tolerant. The detectors were built in a modular way. The quadruplets consist of two double-sided readout panels and three support (or drift) panels equipped with the micromesh and the drift electrode. The panels are bolted together such that the detector can be opened and cleaned, if required. Two of the readout planes are equipped with readout strips inclined by 1.5 degree. In this talk, we present the results of detailed performance studies based on X-Ray and cosmic ray measurements as well as measurements with 855 MeV electrons at the MAMI accelerator. In particular, results on reconstruction efficiencies, track resolution and gain homogeneity is presented.

Abstract

A resistive-MicroMeGaS quadruplet built at CERN has been installed at the new CERN Gamma Irradiation Facility (GIF++) with the aim of carrying out a long-term ageing study. Two smaller resistive bulk-MicroMeGaS produced at the CERN PCB workshop have also been installed at GIF++ in order to provide a comparison of the ageing behavior with the MicroMeGaS quadruplet. We give an overview of the ongoing tests at GIF++ in terms of particle rate, integrated charge and spatial resolution of the MicroMeGaS detectors.

Abstract

Background There is a procyclical relationship between economic growth and occupational injury rates in the short term. Injury rates rise during periods of economic growth and fall during recessions. Aims To estimate injury rates for the manufacturing sector in Italy between 1994 and 2012 and their correlation with major macroeconomic factors and to identify a possible change in the trend of injury rates at the beginning of the 2008 economic crisis. Methods Total and annual serious injury rates were calculated for the national sector. We used a linear autoregressive model to assess the relationship between injury rates and unemployment rate/real GDP growth, and a joint-point regression analysis to analyse changes in injury rates over time. Results After adjusting for the spontaneous dynamic change in injury rates over time, both total and serious injury rates were negatively associated with unemployment rate, and significantly positively associated with real GDP growth. Manufacturing injury rates dropped after 2008. Conclusions Manufacturing injury rates are associated with major macroeconomic factors. Workplace injury rates declined between 1994 and 2012. This downward trend was further accelerated after 2008. The changes in workforce composition before and after 2008 partly explain the procyclical relationship between business conditions and injury rates.

Abstract

Introduction. To evaluate the clinical response rate after a postoperative 18F-FDG PET/CT guided external beam radiotherapy (EBRT) in Iodine-refractory differentiated thyroid cancer. Material and Methods. Patients with thyroid cancer locally recurrent after total thyroidectomy plus metabolic radiotherapy and treated with radical EBRT were included. Inclusion criteria were detectable thyroglobulin (Tg), negative postmetabolic radiotherapy whole body scintigraphy, and no surgical indications. The pretreatment 18F-FDG PET/CT resulted positive in all cases (loggia, lymph nodes, and lung). EBRT was delivered with IMRT-SIB technique. A 18F-FDG PET/CT revaluation and Tg dosage were performed 3 months after the treatment. Results. Sixteen consecutive patients were included in this analysis (median follow-up: 6–44 months). Post-EBRT 18F-FDG PET/CT showed CR in 43.7%, PR in 31.2%, SD in 25.0% patients, and PD due to lung metastases in 12.5%. Overall response rate was 75.0% (CI 95%: 41.4–93.3%). Tg levels decreased in 75.0% with a median Δ of 68.0%. Two-year PFS and OS rates were 80.0% and 93.0%, respectively. Acute G3 toxicity occurred in 18.7% and late G2 toxicity in 12.5%. Conclusions. 18F-FDG PET/CT was useful in target definition for radiotherapy planning, identifying positive areas not detected with 131I scintigraphy. IMRT based EBRT was feasible and our results encourage future prospective studies. This clinical trial is registered with ID: NCT03191643.

Abstract

We present a resistive Micromegas detector built with two different pillar shapes. The pillars are elongated and extend in the direction orthogonal to the readout strips. One region features pillars of 2 mm × 0.2 mm with 4.8 mm pitch while in the other region the pillars extend over the full width of the detector. The larger surface of the pillars allows for a better adhesion to the readout structure and a more uniform amplification gap. Results on the detector performance for the two regions are presented. No striking performance differences between the two regions were found.

Abstract

Radiation studies on several resistive bulk-Micromegas chambers produced at CERN will be viewed in this document. Two resistive bulk-Micromegas chambers have been installed at the CERN Gamma Irradiation Facility (GIF++) exposed to an intense gamma irradiation with the aim of evaluating the detector behaviour under high irradiation and carrying out a long-term ageing study. The chambers under study have an active area of 10 x 10 cm$^2$, a strip pitch of 400 $\mu m$, an amplification gap of 128 $\mu m$, and a drift gap of 5 mm. The results on the detector performance as a function of the photon flux up to 44 MHz/cm$^2$ will be shown as well as the ageing properties as function of the integrated charge and the current intensity and its stability with time. In addition, the results of the efficiency measurements before, during, and after the irradiation will also be presented as a function of the amplification voltage at which the chambers are operated.

Abstract

Background/aim A review of the literature is proposed as a contribution to current knowledge on technical, physical, and clinical issues about PET-guided planning and re-planning radiotherapy (RT) in head and neck cancer. Materials and methods PubMed and Scopus electronic databases were searched for articles including clinical trials. Search terms were "gross tumor volume (GTV) delineation", "head and neck cancer", "radiotherapy", "adaptive radiotherapy" in combination with "PET". Results A 18F-FDG-PET and CT-scan comparison in GTV definition for RT planning of head and neck cancer was shown in twenty-seven clinical trials with a total of 712 patients. Only two clinical trials focused on PET-guided adaptive radiotherapy (ART) with a total of 31 patients. Conclusion 18F-FDG-PET is able to achieve an accurate and precise definition of GTV boundaries during RT planning, especially in combination with CT-scan. ART strategies are proposed to evaluate tumor volume changes, plan boost irradiation on metabolically active residual neoplasm and protect organs at risk (OaRs).

Abstract

INTRODUCTION AND OBJECTIVES: Prostate cancer (PCa) patients with seminal vesicle invasion (SVI) are at increased risk of recurrence and mortality after radical prostatectomy (RP). We aimed at assessing the impact of adjuvant radiotherapy (aRT) on cancer-specific mortality (CSM) in patients with SVI after adjusting for the risk of dying from other causes METHODS: A total of 1,218 patients with localized PCa undergoing RP at a single center between 1990 and 2015 were identified. All patients included in the study had SVI at final pathology. Cumulative incidence CSM rates were generated for the overall population and after stratifying patients according to aRT status and were compared using the Gray test. Competing-risks regression analyses assessed the impact of aRT on CSM after adjusting for the risk of dying from other causes. Covariates consisted of age, positive surgical margins, pathologic Gleason score, adjuvant hormonal therapies, and nodal status RESULTS: Median age was 66.4 years. Overall, 10.5, 42.3, and 47.2% of the patients included had pathologic Gleason score 6, 7, and 8, respectively. Overall, 539 (44.3%) patients had positive surgical margins. Overall, 641 (52.6%) patients had node positive disease. Median follow-up was 84 months. Overall, 222 patients experienced clinical recurrence (CR). The 15-year CR-free survival rate was 55.5%. Among patients who experienced CR, 19.7, 20.7, 8.0, and 51.6% had involvement of the prostatic fossa, pelvic lymph nodes, retroperitoneal nodes, and systemic metastases. Overall, 98 and 110 patients died from PCa and other causes, respectively. Overall, the 15-year CSM rate was 18.8%. When patients were stratified according to aRT status, the 15-year CSM rates were significantly higher among those who did not receive aRT as compared to those treated with aRT (22.9 vs. 13.7%, respectively; P1⁄40.02). In competing-risks regression analyses, aRT (Hazard ratio [HR]: 0.51; P1⁄40.01), pathologic Gleason score 8-10 (HR: 2.36; P1⁄40.01); and pN1 (HR: 1.79; P1⁄40.02) represented independent predictors of CSM CONCLUSIONS: A substantial proportion of patients with SVI is at risk of experiencing systemic recurrence after surgery. Adjuvant RT significantly reduced the risk of CSM after accounting for the risk of dying from other causes. These results suggest that maximising local control would improve oncologic outcomes in these patients. Conversely, men with node positive disease and higher Gleason score are at higher risk of CSM and might benefit more from systemic approaches

Abstract

Motor impairment represents the main clinical feature of Parkinson's disease (PD). Cognitive deficits are also frequently observed in patients with PD, with a prominent involvement of executive functions and visuo-spatial abilities. We used event-related functional MRI (fMRI) and a paradigm based on visual attention and motor inhibition (Go/NoGO-task) to investigate brain activations in 13 patients with early PD in comparison with 11 healthy controls. The two groups did not report behavioural differences in task performance. During motor inhibition (NoGO-effect), PD patients compared to controls showed an increased activation in the prefrontal cortex and in the basal ganglia. They also showed a reduced and less coherent hemodynamic response in the occipital cortex. These results indicate that specific cortico-subcortical functional changes, involving not only the fronto-striatal network but also the temporal-occipital cortex, are already present in patients with early PD and no clinical evidence of cognitive impairment. We discuss our findings in terms of compensatory mechanisms (fronto-striatal changes) and preclinical signs of visuo-perceptual deficits and visual hallucinations.

Abstract

The Japanese people are one of the most important populations for studying the origin and diversification of East Asian populations. As an island population, the Japanese's path of migration is a long-standing controversy. Archeological evidence suggests that there were at least two waves of migration to the Japanese archipelago in prehistory: the Paleolithic and Neolithic Jomonese as well as the Aeneolithic Yayoiese. However, the contributions of these Jomonese and Yayoiese to the contemporary Japanese population remain unclear. In this article, we provide evidence from human genetics as a new approach to addressing this topic. At the beginning, we introduce the history of human migration to the Japanese archipelago, as well as materials and methods human geneticists use. Subsequently, we tested three distinct population expansion models using evidences from recent human genetic studies on the Japanese, East Asian, and Serbian populations. Finally, we conclude that the contemporary main island Japanese are the result of population admixture of Jomonese, Yayoiese, and Han Chinese, which consists with the Admixture model.

Abstract

We report on the construction and initial performance studies of two micromegas detector quadruplets with an area of 0.5 m$^2$. They serve as prototypes for the planned upgrade project of the ATLAS muon system. Their design is based on the resistive-strip technology and thus renders the detectors spark tolerant. Each quadruplet comprises four detection layers with 1024 readout strips and a strip pitch of 415 $\mu$m. In two out of the four layers the strips are inclined by $\pm$1.5$^{\circ}$ to allow for the measurement of a second coordinate. We present the detector concept and report on the experience gained during the detector construction. Abstract

This immunohistochemical study aims to investigate the Atrial natriuretic peptide (ANP)-presence and localization in human articular cartilage. Fragments of articular cartilage covering the femoral head were removed from patients submitted to surgical operation after femoral neck fracture without joint disease. The samples were immunostained with anti-ANP antibody. The results demonstrate that ANP is present in chondrocytes in all the three zones of the articular cartilage. Superficial chondrocytes show strong ANP-immunopositivty. The presence of ANP in the articular cartilage suggests that ANP may play a role in cartilage metabolism by regulating transport of molecules through the different zones of the articular cartilage and in maintenance of its homeostasis; probably ANP could be also involved in the regulation of the balance between synovial fluid and the other body fluids.

Abstract

METHODS: We performed a single-institution, single-surgeon review of 3,433 patients who underwent RARP from 2005 to 2014. Patients were identified who underwent RARP for high-risk disease as determined by D’Amico classification (Gleason 810, stage T2c, and or PSA 20). Perioperative data was evaluated from a prospectively maintained RARP database. Treatment was considered a failure with PSA levels 0.2. RESULTS: In total, 363 patients were identified that met highrisk classification. The median age was 63. The median BMI was 29.24. Follow-up ranged from 3 to 98 months. 1-year biochemical recurrence free survival (1-RFS) was 81.2%. 2year biochemical recurrence free survival (2-RFS) was 63.3%. Gleason score of 8-10 was the most common preoperative high-risk identifier (66%). Positive surgical margins occurred in 32.8% of patients. Lymph node dissection was performed on 351 patients (97%). The 12 patients that did not receive a lymph node dissection had surgery. In total, 19 patients (5.4%) were found to have positive lymph node involvement. CONCLUSIONS: We present our data for the treatment of highrisk prostate cancer with RARP. Consistent with recent, but limited data, RARP appears to be an effective initial treatment for high-risk prostate cancer. Further follow-up for these patients remains necessary to help better understand long-term outcomes following RARP for high-risk prostate cancer.

Abstract

Mrk 501 is one of the brightest blazars at TeV energies and has been extensively studied since its first VHE detection in 1996. Our goal is to characterize in detail the source gamma-ray emission, together with the radio-to-X-ray emission, during the non-flaring (low) activity, which is less often studied than the occasional flaring (high) activity. We organized a multiwavelength (MW) campaign on Mrk 501 between March and May 2008. This multi-instrument effort included the most sensitive VHE gamma-ray instruments in the northern hemisphere, namely the imaging atmospheric Cherenkov telescopes MAGIC and VERITAS, as well as Swift, RXTE, the F-GAMMA, GASP-WEBT, and other collaborations and instruments. Mrk 501 was found to be in a low state of activity during the campaign, with a VHE flux in the range of 10%-20% of the Crab nebula flux. Nevertheless, significant flux variations were detected with various instruments, with a trend of increasing variability with energy. The broadband spectral energy distribution during the two different emission states of the campaign can be adequately described within the homogeneous one-zone synchrotron self-Compton model, with the (slightly) higher state described by an increase in the electron number density. This agrees with previous studies of the broadband emission of this source during flaring and non-flaring states. We report for the first time a tentative X-ray-to-VHE correlation during a low VHE activity. Although marginally significant, this positive correlation between X-ray and VHE, which has been reported many times during flaring activity, suggests that the mechanisms that dominate the X-ray/VHE emission during non-flaring-activity are not substantially different from those that are responsible for the emission during flaring activity., Comment: Accepted for publication in A&A

Abstract

We performed a 4.5-month multi-instrument campaign (from radio to VHE gamma rays) on Mrk421 between January 2009 and June 2009, which included VLBA, F-GAMMA, GASP-WEBT, Swift, RXTE, Fermi-LAT, MAGIC, and Whipple, among other instruments and collaborations. Mrk421 was found in its typical (non-flaring) activity state, with a VHE flux of about half that of the Crab Nebula, yet the light curves show significant variability at all wavelengths, the highest variability being in the X-rays. We determined the power spectral densities (PSD) at most wavelengths and found that all PSDs can be described by power-laws without a break, and with indices consistent with pink/red-noise behavior. We observed a harder-when-brighter behavior in the X-ray spectra and measured a positive correlation between VHE and X-ray fluxes with zero time lag. Such characteristics have been reported many times during flaring activity, but here they are reported for the first time in the non-flaring state. We also observed an overall anti-correlation between optical/UV and X-rays extending over the duration of the campaign. The harder-when-brighter behavior in the X-ray spectra and the measured positive X-ray/VHE correlation during the 2009 multi-wavelength campaign suggests that the physical processes dominating the emission during non-flaring states have similarities with those occurring during flaring activity. In particular, this observation supports leptonic scenarios as being responsible for the emission of Mrk421 during non-flaring activity. Such a temporally extended X-ray/VHE correlation is not driven by any single flaring event, and hence is difficult to explain within the standard hadronic scenarios. The highest variability is observed in the X-ray band, which, within the one-zone synchrotron self-Compton scenario, indicates that the electron energy distribution is most variable at the highest energies., Comment: Accepted for publication in A&A, 18 pages, 14 figures (v2 has a small modification in the acknowledgments, and also corrects a typo in the field "author" in the metadata)

Abstract

IntroductionThe Meeting Centre Support Programme (MCSP) is a community-based approach to support people living with dementia and their families. It was developed in the Netherlands and has been implemented in other European Countries (Italy, Poland and the UK) within the JPND-MEETINGDEM project.AimsTo assess the relationship between background characteristics of people with dementia participating in MCSP, mood, quality of life (QoL) and experienced stigma, and to explore if and how the experienced stigma changed after 6 months of participation in MCSP.MethodsA pretest (M1) post-test (M7) control group design with matched groups regarding severity of dementia was applied. In each country, a minimum of 25 participants using MCSP were compared with people with dementia receiving ‘usual care’. Data were collected with the Stigma Impact Scale, Cornell Scale for Depression in Dementia, Global Deterioration Scale and two QoL scales (QoL-AD & DQoL). Differences in background characteristics were taken into account in the analyses.ResultsThe preliminary analysis on 116 participants at baseline shows that the level of stigma was low to moderate. People felt more socially rejected in the UK than in Poland and Italy. The level of perceived stigmatization appeared negatively correlated with QoL areas and positively correlated with negative mood. Changes after 6 months will be presented.ConclusionsIt is expected that after 6 months people living with dementia participating in MCSP will experience less stigma, as in contrast with usual care MCSP promotes social integration of people with dementia and person-centered support.Disclosure of interestThe authors have not supplied their declaration of competing interest.
