Retrospective case-control study. A total of 42 pseudophacic eyes of 39 patients (7 with full thickness macular hole and 35 with epiretinal membrane) with coexisting dry AMD underwent pars plana vitrectomy (PPV) with internal limiting membrane (ILM) and epiretinal membrane peel. Preoperative and postoperative data including best corrected visual acuity (BCVA), AMD grade according to AREDS, central retinal thickness (CRT), development of choroidal neovascularization (CNV), and central retinal atrophy have been evaluated. Twenty-eight fellow eyes with dry AMD of the included 39 patients served as a control group.Results: A significant improvement in the visual acuity could be observed after surgery (initial BCVA 0.47± 0.31 logarithm of the minimal angle of resolution (logMAR) vs 0.33± 0.29logMAR 9 months postoperatively; p=0.006). CRT decreased significantly after surgery (p< 0.001). In the surgery group, there were 4 eyes (9.5%) with CNV and 1 eye (2.5%) with new central retinal atrophy development after surgery. All these eyes had preoperative AREDS 3 (4 eyes) or AREDS 4 (1 eye) AMD category. In the control group, there was 1 eye (4%) with CNV and 4 eyes (14%) with new central retinal atrophy development during the follow-up of 9 months. These eyes had initially AREDS 2 (1 eye), AREDS 3 (3 eyes) or AREDS 4 (1 eye) AMD category.Conclusion: Eyes with dry AMD of AREDS 3 and AREDS 4 with coexisting VMI abnormalities improve significantly after PPV with membrane peel. While there is a higher risk of CNV development after surgery (9.5%) in these eyes, the vitrectomy does not seem to accelerate central retinal atrophy progression compared to the fellow eyes course.Keywords: macular membrane peel, choroidal neovascularization, pars plana vitrectomy, age-related macular degeneration, ILM, epiretinal membrane peeling

Scaffolds have been used to foster self-regulated learning, but scaffolds often are standardized and do not do not adapt to the individual learning process. Learning analytics and machine learning offer an approach to better understand SRL-processes during learning. Yet, current approaches lack validity or require extensive analysis after the learning process. The FLORA project aims to investigate how to advance support given to students by i) improving unobtrusive data collection and machine learning techniques to gain better measurement and understanding of SRL-processes and ii) using these new insights to facilitate student’s SRL by providing personalized scaffolds. We will reach this goal by investigating and improving trace data in exploratory studies (exploratory study 1 and study 2) and using the insight gained from these studies to develop and test personalized scaffolds based on individual learning processes in laboratory (experimental study 3 and study 4) and a subsequent field study (field study 5). At the moment study 2 is ongoing. The setup consists of a learning environment presented on a computer with a screen-based eye-tracker. Other data sources are log files and audio of students’ think aloud. The analysis will focus on detecting sequences that are indicative of micro-level self-regulated learning processes and aligning them between the different data sources.

Abstract

Background On festive days worldwide eyes are severely injured by fireworks. The data on the number and severity are to date not registered in Germany. Objective How frequent are firework-induced injuries in Germany, who are the affected, how serious are the injuries, which forms of treatment are necessary and how frequent are accompanying injuries? Method A German nationwide online-based survey was carried out in all inpatient eye departments and data over the last 3 consecutive years were descriptively analyzed. Results From New Years Eve 2016/2017, when 41 eye departments sent in data, the participation could be increased to 51 eye departments in 2018/2019. More than one third (33-39%) of all 1356 patients over 3 years were minors, 60% were younger than 25 years old and roughly 60% of patients were injured as bystanders or in an unclear situation. In total 25% of all eye injuries were considered severe and required inpatient treatment. Accompanying injuries of the other eye, the face and hands were more frequent in minors than in adults. Eyeball ruptures were reported in 10 minors and 38 adults over the 3 years. Conclusion Particularly minors and bystanders need better protection.

22. Mycophenolate sodium significantly reduces the relapse rate in patients with non-infectious intermediate uveitis

Abstract

OA gold Purpose : To evaluate whether a mycophenolate sodium-based treatment regimen is able to reduce the probability of a relapse compared to corticosteroid therapy alone in patients with non-infectious intermediate uveitis. Methods : Investigator-initiated, phase 3 open, prospective, controlled, randomized national multicenter trial (protocol MYCUV-IIT02). To be eligible, patients had to suffer from a documented at least 6 months history of unilateral or bilateral intermediate uveitis, either idiopathic or due to non-infectious systemic disease, that was active at study entry. All patients received initial treatment with oral prednisolone 1 mg/kg, followed by a slow tapering over 3 months. For maintenance therapy, subjects were randomized in a 1:1 ratio to either mycophenolic sodium 1440 mg/day (Myfortic®, Novartis Pharma) plus prednisolone 5 mg/day (treatment arm) or prednisolone 5mg/day alone (control arm). Maximum treatment duration was 15 months. The primary endpoint was defined as the time from study entry to the first relapse defined as a deterioration of best corrected visual acuity ≥ 3 lines and/or an at least 2-step increase of vitreous haze and/or an at least 2-step increase of anterior chamber cells and/or a new onset or worsening of preexisting cystoid macular edema, proven by optical coherence tomography, and/or a new onset or worsening of retinal vasculitis, proven by fluorescein angiography. Results : Fourty-four subjects at 8 sites were included. Of these, 41 patients (16 males, 25 females; median age 41.5 years, 18.6 – 82.7 years) were eligible for further analysis. Twenty-two patients (53.7 %) were randomized to treatment arm, 19 patients (46.3%) to control arm. In a first analysis using the Kaplan-Meier method, the proportion of relapse-free patients was estimated as 58.2 % in the treatment arm and 26.3 % in the control arm at 9 months after study entry, and as 52.9 % in the treatment arm and 19.7 % in the control arm at 15 months after study entry. The difference between treatment arm and control arm regarding relapse-free survival was statistically significant (log-rank test p = 0.011). Conclusions : During an observation period of 15 months, a combination of mycophenolate sodium plus low-dose corticosteroids resulted in a significant reduction of relapse rate compared to treatment with low-dose corticosteroids alone in patients with non-infectious intermediate uveitis.

Abstract

Background and purpose. In September 2011 the cornea section of the German Ophthalmological Society (DOG) established the first German Acanthamoeba keratitis registry. The data of this multicenter survey are being collected, compiled and evaluated at the Department of Ophthalmology at the Saarland University. The aim of this article is to present an intermediate report. Patients and methods. Data from 172 eyes with Acanthamoeba keratitis were collected during the last 10 years. For this interim report we actually evaluated 121 eyes (60.2 % female patients, average age 41.3 years) and collected the following data: date of onset of symptoms, date and method of diagnosis, initial diagnosis, anamnestic data, clinical symptoms and signs at diagnosis and during follow-up, conservative and surgical therapy. Criteria for inclusion in the Acanthamoeba registry was the established diagnosis of an Acanthamoeba keratitis with at least one of the methods described in this article. Results. Acanthamoeba keratitis could be histologically proven in 55.3 % of the cases, via PCR in 25.6 %, with confocal microscopy in 20.4 % and using in vitro cultivation in 15.5 %. Clinical symptoms and signs in Acanthamoeba keratitis were pain in 67.0 %, ring infiltrates in 53.4 %, pseudodendritiform epitheliopathy in 11.7 % and keratoneuritis in 5.8 %. In 47.6 % of the cases the initial diagnosis was herpes simplex virus keratitis followed by bacterial keratitis in 25.2 % and fungal keratitis in 3.9 %. Acanthamoeba keratitis was the correct initial diagnosis in only 23.2 % of cases. The average time period between first symptoms and diagnosis was 2.8 +/- 4.0 months (range 0-23 months). A triple therapy with BroleneA (R) LavaseptA (R) and antibiotic eye drops at least 5 x/day was used in 54.5 % of eyes (n = 66). Abstract

Purpose: Proliferative vitreoretinopathy (PVR) is the most important reason for blindness following retinal detachment. Presently, vitreous tamponades such as gas or silicone oil cannot contact the lower part of the retina. A heavier-than-water tamponade displaces the inflammatory and PVR-stimulating environment from the inferior area of the retina. The Heavy Silicone Oil versus Standard Silicone Oil Study (HSO Study) is designed to answer the question of whether a heavier-than-water tamponade improves the prognosis of eyes with PVR of the lower retina. Methods: The HSO Study is a multicentre, randomized, prospective controlled clinical trial comparing two endotamponades within a two-arm parallel group design. Patients with inferiorly and posteriorly located PVR are randomized to either heavy silicone oil or standard silicone oil as a tamponading agent. Three hundred and fifty consecutive patients are recruited per group. After intraoperative re-attachment, patients are randomized to either standard silicone oil (1000 cSt or 5000 cSt) or Densiron® as a tamponading agent. The main endpoint criteria are complete retinal attachment at 12 months and change of visual acuity (VA) 12 months postoperatively compared with the preoperative VA. Secondary endpoints include complete retinal attachment before endotamponade removal, quality of life analysis and the number of retina affecting re-operation within 1 year of follow-up. Results: The design and early recruitment phase of the study are described. Conclusions: The results of this study will uncover whether or not heavy silicone oil improves the prognosis of eyes with PVR. © 2007 Acta Ophthalmol Scand.

Hintergrund: RGD-Peptide hemmen kompetitiv die Adhäsionsmoleküle von Linsenepithelzellen (LEC). Ziel unserer Untersuchungen war es herauszufinden, ob diese Peptide in der Lage sind, auch nach Kurzzeitinkubation eine suffiziente Inhibition der Adhäsion bzw. eine Ablösung adhärenter Zellen und damit eine ausreichende Prävention des Nachstars zu bewirken. Außerdem wurde überprüft, ob das von uns verwendete RGD-Peptid eine Toxizität für die Hornhaut aufweist. Material und Methoden: Kulturen boviner und humaner LEC, boviner Hornhautendothelzellen, humane und bovine Linsenkapselexzidate und humane explantierte Hornhäute wurden verwendet. Wir untersuchten die Inhibition der Adhäsion und die Ablösung konfluenter LEC-Layer mittels des zyklischen RGD-Peptids cRGDDFV (Inkubationszeiten von 1 Stunde bzw. 5-7 Tagen und Konzentrationen von 10-4 M, 10-3 M und 2x10-3 M wurden angewandt). Ergebnisse: Wir fanden nach nur einstündiger Inkubation in der Kulturschale eine Adhäsionsinhibition von 48% für bovine LEC und von 100% für humane LEC. Die Differenz zwischen Kontrollpeptid und cRGDDFV war statistisch signifikant (p, Purpose: RGD-peptides competitively inihibit adhesion molecules of the lens epithelial cells (LEC). The purpose of our study was to investigate whether this peptide could be able to inhibit adhesion sufficiently after short term incubation resp. to detach adherent cells and so to prevent posterior capsule opacification (PCO). Methods: Cultures of bovine and human LEC, bovine cornea endothelial cells, humane and bovine fragments of the lens capsule and explanted humane corneas were used. The inhibition of adhesion and the detachment of confluent LEC-layer by the cyclic RGD-peptide cRGDDFV were studied (incubation time was 1 hour resp. 5-7 days and concentrations of 10-4, 10-3 M and 2x10-3 M were used).
