Congenital hypothyroidism (CH) is a relatively frequent congenital endocrine disorder, caused by defective production of thyroid hormones (THs) at birth. Because THs are essential for the development of normal neuronal networks, CH is also a common preventable cause of irreversible intellectual disability (ID) in children. Prolonged hypothyroidism, particularly during the THs-dependent processes of brain development in the first years of life, due to delays in diagnosis, inadequate timing and dosing of levothyroxine (l-thyroxine or l-T4), the non-compliance of families, incorrect follow-up and the interference of foods, drugs and medications affecting the absorption of l-T4, may be responsible for more severe ID. In this review we evaluate the main factors influencing levels of THs and the absorption of l-T4 in order to provide a practical guide, based on the existing literature, to allow optimal follow-up for these patients.

Preliminary evidence supports the notion that COVID-19 patients may have an increased susceptibility to develop venous thromboembolism (VTE). However, the magnitude of this association still needs to be defined. Furthermore, clinical predictors of thrombogenesis, and the relationship with the inflammatory status are currently unknown. On this basis, we conducted a retrospective, observational study on 259 consecutive COVID-19 patients admitted to an academic tertiary referral hospital in Northern Italy between March 19th and April 6th, 2020. Records of COVID-19 patients with a definite VTE event were reviewed for demographic information, co-morbidities, risk factors for VTE, laboratory tests, and anticoagulation treatment. Twenty-five cases among 259 COVID-19 patients developed VTE (9.6%), all of them having a Padua score > 4, although being under standard anticoagulation prophylaxis since hospital admission. In the VTE subcohort, we found a significant positive correlation between platelet count (PLT) and either C reactive protein (CRP) (p p = 0.0013), while a significant inverse correlation was observed between PLT and mean platelet volume (p p p = 0.002 and p = 0.005, respectively). No significant difference was found in d-dimer levels between VTE and non VTE patients, while significantly higher levels of LDH (p = 0.04) and IL-6 (p = 0.04) were observed in VTE patients in comparison to non-VTE patients. In conclusion, our findings showed a quite high prevalence of VTE in COVID-19 patients. Raised inflammatory indexes and increased serum levels of pro-inflammatory cytokines should raise the clinical suspicion of VTE.

Preliminary evidence supports the notion that COVID-19 patients may have an increased susceptibility to develop venous thromboembolism (VTE). However, the magnitude of this association still needs to be defined. Furthermore, clinical predictors of thrombogenesis, and the relationship with the inflammatory status are currently unknown. On this basis, we conducted a retrospective, observational study on 259 consecutive COVID-19 patients admitted to an academic tertiary referral hospital in Northern Italy between March 19th and April 6th, 2020. Records of COVID-19 patients with a definite VTE event were reviewed for demographic information, co-morbidities, risk factors for VTE, laboratory tests, and anticoagulation treatment. Twenty-five cases among 259 COVID-19 patients developed VTE (9.6%), all of them having a Padua score 4, although being under standard anticoagulation prophylaxis since hospital admission. In the VTE subcohort, we found a significant positive correlation between platelet count (PLT) and either C reactive protein (CRP) (p 0.0001) or lactate dehydrogenase (LDH) (p = 0.0013), while a significant inverse correlation was observed between PLT and mean platelet volume (p 0.0001). Platelet-to-lymphocyte ratio significantly correlated with CRP (p 0.0001). The majority of VTE patients was male and younger compared to non-VTE patients (p = 0.002 and p = 0.005, respectively). No significant difference was found in D-dimer levels between VTE and non VTE patients, while significantly higher levels of LDH (p = 0.04) and IL-6 (p = 0.04) were observed in VTE patients in comparison to non-VTE patients. In conclusion, our findings showed a quite high prevalence of VTE in COVID-19 patients. Raised inflammatory indexes and increased serum levels of pro-inflammatory cytokines should raise the clinical suspicion of VTE.

Background The long pentraxin PTX3 is a key component of the humoral arm of innate immunity related to sepsis severity and mortality. We evaluated the clinical and prognostic significance of circulating PTX3 in the largest cohort ever reported of patients with severe sepsis or septic shock. Design Plasma PTX3 was measured on days 1, 2 and 7 after randomization of 958 patients to albumin or crystalloids for fluid resuscitation in the multicenter Albumin Italian Outcome Sepsis (ALBIOS) trial. We tested the association of PTX3 and its changes over time with clinical severity, prevalent and incident organ dysfunctions, 90-day mortality, and treatment. 26. Hacia una teoría del prólogo: a propósito de Miguel de Unamuno

En este artículo partimos de la demanda que el filósofo danés Søren Kierkegaard formulara en su libro Prólogos (1844) acerca de la necesidad de elaborar una teoría del prólogo, algo que más de un siglo después llevó adelante Gérard Genette en su estudio sobre los llamados «paratextos» y que tituló precisamente Umbrales, haciendo referencia a los aledaños del texto. Luego de realizar un rápido recorrido a través de varios textos, entre ellos Cinco prefacios a cinco libros no escritos (1872) de Friedrich Nietzsche, «Fuera–de–libro (Prefacios)» (1972) de Jaques Derrida y Prólogos con un prólogo de prólogos (1975) de Jorge Luis Borges, nos detendremos especialmente en el caso paradigmático de Miguel de Unamuno y en su filiación con Macedonio Fernández. Las conclusiones apuntan a subrayar la novedad que este «género» adquiere en las formulaciones de estos escritores: el gesto autorreferencial de teorizar desde el prólogo sobre el prólogo en un ejercicio de deconstrucción y resignificación del mismo. El lugar privilegiado que ambos asignan al problema teórico de lo marginal versus lo central, de lo abierto versus lo cerrado, de lo concluso versus lo inconcluso responde a una común concepción estética que apuesta por el fragmento, el esbozo o el proyecto provisional deconstruyendo la mímesis realista del XIX. In this paper we start from the request that the danish philosopher Søren Kierkegaard formulated in his book Prólogos (1844) to develop a theory of the prologue, some- thing that more than a century later carried on Gerard Genette in his study on the so-called «paratextos» and that titled precisely Umbrales, making reference to the surroundings of the text. After making a quick tour through several texts, including Cinco prefacios a cinco libros no escritos (1872) by Friedrich Nietzsche, «Fuera–de–libro (Prefaci- os)» (1972) by Jacques Derrida and Prólogos con un prólogo de prólogos (1975) by Jorge Luis Borges, we stop especially in the paradigmatic case of Miguel de Unamuno and his affiliation with Macedonio Fernández. The conclusions aim to emphasize the novelty that this «gender» acquires in the formulations of these writers: the self-referential gesture of theorizing from the prologue on the prologue in an simultaneous exercise of deconstruction and resignification. The privileged place that both assign to the theoretical problem of the marginal versus the central, the open versus the closed, the conclusive versus the unfinished responds to a common aesthetic conception that bets on the fragment, the outline or the provisional project decon- structing the realistic mimesis of the XIX century.

Resumen El melanoma es un cáncer de origen melanocítico extremadamente raro, de pronóstico grave con una pobre sobrevida a los cinco años. El objetivo del presente trabajo es comunicar un caso clínico de melanoma de la cavidad bucal correspondiente a una mujer de 75 años de edad, en la cuál se estudiaron los niveles séricos de las enzimas lisosomales, Fosfatasa Ácida (FA), Hexosaminidasa (Hex), Beta Galactosidasa (b-Gal) y el Antígeno Carcinoembrionario (CEA). El tratamiento efectuado fue quirúrgico completándose el mismo con quimio e inmunoterapia, falleciendo la paciente a los doce meses del diagnóstico. El dosaje de las enzimas lisosomales séricas al momento del diagnóstico mostraban un valor superior al valor máximo normal, por lo que investigaciones posteriores podrían validar el dosaje de estas enzimas como método complementario para el diagnóstico de esta patología. La identificación de lesiones precursoras del melanoma maligno y la determinación de marcadores tumorales séricos junto a la aplicación de terapias locales y sistémicas mas efectivas podrían contribuir a mejorar la sobrevida de estos pacientes SUMMARY The melanoma is a melanocytic start cancer extremely strange, of serious forecast with a poor survive at five year. The aim of the present work is communicate a clinical case of mouth cavity melanoma belonging to a 75-year-old woman, in which were studied lysosomal enzymes levels, Acid Fosfatase (FA), Hexosaminidase (Hex), B-Galactosidasa (B-Gal) and the Carcinoembrionic Antigen (CEA). The treatment carried out was surgical and being completed with quimio and immunotherapy, dying twelve months latter the patient. The seric lysosomal enzymes at the moment of diagnostic showed a value over the normal one, for what this study suggested may be used as complementary method of diagnosis to this pathology. The precursor injuries identification of malignant melanoma and seric determination of tumour scoreboards with the application of local and systemic therapies more effective might help to provide better survive to these patients

La poesía de José Hierro, Ángel González y Guillermo Carnero es abordada en su proceso autoconciente de escritura, cuando problematiza la naturaleza del lenguaje y sus vinculaciones con la referencia, la representación y el poder. Esta lectura da cuenta de la sistematicidad, riqueza y diversidad de una teoría poética en la escritura española del siglo XX. El presente libro aporta una nueva mirada a los estudios realizados sobre el tema, al cuestionar la linealidad usual de las periodizaciones y entender la secuencia literaria del ´40 al ´80 en términos de serie y cada segmento de la misma como una trama de diversos espesores, en la que no hay cabida para programas estéticamente cerrados. La denominación de "coartada metapoética" designa una táctica conciente o inconciente de escritura, desde una mirada no restrictiva, que va descubriendo temas, que no sólo incluyen el debate acerca de la autonomía o el compromiso del arte, sino también y principalmente arrojan luz sobre aspectos menos revisados, pero no menos importantes en la actualidad, cuando las circunstancias político culturales de España no obligan ya a las urgencias de una toma de posición. The poetry of José Hierro, Angel González and Guillermo Carnero is approached in its self-conscious process of writing when it problematizes the nature of language and its links with reference, representation and power. This reading gives an account of the systematic, richness and diversity of a poetry theory in the twentieth century Spanish writing. This book intends to offer a new point of view at the studies on the subject, to question the usual linearity of the periodisation and understand the literary sistem of '40 to '80 in terms of "serie" and each segment of it as a web of different thicknesses in which there is no room for closed aesthetic programs. The term " metapoetic alibi " means a conscious or unconscious tactic of writing, from a not restrictive viewpoint which includes not only the debate about autonomy or commitment to art, but also focuses on topics less reviewed, but no less important today. La siguiente investigación analiza las figuraciones de la niñez en la obra de dos poetas españoles contemporáneos: Fernando Beltrán (1956) y Luis García Montero (1958). El investigador y crítico José María Pozuelo Yvancos (2010) propone que al concepto de figuración es fundamental vincularlo con “la idea de dibujo imaginativo, fantasía de algo o bien su representación, algo que, sin serlo, o sin ser de una manera determinada, lo suplanta o figura, esto es, representa imaginariamente como tal” (23). De modo que, tomando el planteo antedicho, vinculamos la figuración con un dibujo imaginativo en el cruce con la infancia y la poesía. El recorrido implica entonces relacionar la infancia visible y la infancia imaginaria, es decir, analizamos la infancia que se presenta como tópico en los poemas, así como también nos centramos en el relevamiento de las figuraciones de la niñez o dibujos imaginativos de la infancia en donde ésta ya no responde a una edad cronológica, sino que se presenta como una irrupción (Fumis, 2016, 2019) (Muzzoppapa, 2017), como un reflejo alejado o una otredad significativa (Premat, 2016) o como ensoñaciones de infancia, siguiendo los planteos de Bachelard (1997) quien piensa que éstas se convierten en el germen de una obra poética y se relacionan con un tiempo elegíaco, un tiempo íntimo que perdura en el adulto. The following research analyzes the figurations of childhood in the work of two contemporary Spanish poets: Fernando Beltrán (1956) and Luis García Montero (1958). The researcher and critic José María Pozuelo Yvancos (2010) proposes that it is essential to link the concept of figuration with “the idea of imaginative drawing, fantasy of something or its representation, something that without being, or without being in a certain way, it supplants or figures, that is, it represents imaginatively as such” (23). So, taking the aforementioned approach, we link the figuration with an imaginative drawing at the crossroads with childhood and poetry. The path implies relating the visible childhood and the imaginary childhood, that is, we analyze the childhood that is presented as a topic in the poems, as well as we focus on the survey of the figurations of childhood or imaginative drawings of childhood where it no longer responds to a chronological age, but is presented as an irruption (Fumis, 2016, 2019) (Muzzoppapa, 2017), as a distant reflection or a significant otherness (Premat, 2016) or as childhood daydreams, following the proposals of Bachelard (1997) who thinks that these become the germ of a poetic work and are related to an elegiac time, an intimate time that lasts in the adult. Congenital hypothyroidism (CH) is a relatively frequent congenital endocrine disorder, caused by defective production of thyroid hormones (THs) at birth. Because THs are essential for the development of normal neuronal networks, CH is also a common preventable cause of irreversible intellectual disability (ID) in children. Prolonged hypothyroidism, particularly during the THs-dependent processes of brain development in the first years of life, due to delays in diagnosis, inadequate timing and dosing of levothyroxine (l-thyroxine or l-T
4 ), the non-compliance of families, incorrect follow-up and the interference of foods, drugs and medications affecting the absorption of l-T4 , may be responsible for more severe ID.
