In the literature, this label and broader identity are typically discussed in technical (amount and frequency of use) or social terms (environment and social network changes).Objective: The current paper seeks to simplify the discussion by focusing on personal labels without complex technical or social considerations. Alumni of an inpatient addiction treatment facility were asked how they refer to themselves regarding their sobriety status post-discharge.Methods: Forty-nine patients were contacted 3 months post-discharge from a residential inpatient addiction treatment (men = 67%; Mage = 47.75 years). The patients completed a post-discharge assessment that was conducted by a trained research assistant over a 20-minute video call. The current study focused on a “sobriety label” measure in which patients indicated what they want to be called. Patients also explained why they chose their answer in an open-ended question.Results: Most patients identified as in recovery (n = 29; 59.18%) followed by a sober person (n = 7; 14.29%) and four other responses. No alum selected the in remission option, which is notably a common way to refer to patients who no longer use substances.Conclusion: The current study adds a critical patient/alumni perspective to the existing body of literature and serves as a call to action for researchers to add a similar “sobriety label” measure to future assessments, studies, and batteries in effort to bring consistency to the labels, definitions, and identities that are published. This methodology of understanding how this population identifies will create uniformity in future literature and decrease the stigma surrounding addiction.Plain Language Summary: There is a history of inconsistent use of labels, definitions, and identities in the addiction treatment field. Few past studies have directly asked patients how they self-label, and it is important to ask those who use substances or who have reduced their use what they preferred to be called. This study asked a simple question to alumni of an inpatient treatment facility what they want to be called. We then asked them to explain why they chose that answer. Most alumni identified as “in recovery” or “a sober person”. This simple tool can be utilized by other facilities and also highlights that many research studies are referring to individuals by terms they do not prefer (eg, “in remission”).Keywords: sobriety, identity, label, addiction, discharge, recovery, sober person, inpatient

This is especially true in the addiction field, but there is little cited evidence for optimal methods to improve EBP in inpatient addiction facilities with minimal resources.Objective: The current paper focuses on evaluating the state of our facility’s group CBT manual and clinical training on the manual in a “realistic” (ie, non-RCT, non-grant-funded) inpatient addiction treatment setting.Methods: Five full-time clinicians volunteered to take part in the study (woman = 60%; Mage = 36.20 years). The study involved a mix of semi-structured interviews and surveys designed to measure seven outcomes (barriers, feasibility, useability, appropriateness, acceptability, burden, trialability).Results: Three themes emerged from the data that impacted the group CBT manual: training, timing, and functionality. Addressing these themes allowed for a new, optimal manual and training procedure to be put into place.Conclusion: The current study highlights that under-resourced inpatient addiction facilities can still methodically utilize implementation approaches to study their EBP, namely CBT. Such an approach will ensure that the highest quality care is being delivered to patients and actively addresses known training barriers that prevent proper EBP delivery.Plain Language Summary: Quality training is a barrier when implementing evidenced-based practice to treat substance use disorders.CBT is a manualized evidence-based practice that is performed at inpatient addiction facilities.An evaluation of the CBT manual was made by an inpatient addiction facility to identify barriers of implementation of EBPs.Identified barriers included training, timing, and functionality.Keywords: cognitive behavioral therapy, addiction, evidence-based practice, inpatient, substance use disorders

It elaborates on the design considerations for the beam as a whole and for individual elements. The most important design details of individual components are described. Beam monitoring systems and procedures, including the tuning and alignment of the beam and NuMI long-term performance, are also discussed.

Abstract

A sample of 1.53$\times$10$^{9}$ cosmic-ray-induced single muon events has been recorded at 225 meters-water-equivalent using the MINOS Near Detector. The underground muon rate is observed to be highly correlated with the effective atmospheric temperature. The coefficient $\alpha_{T}$, relating the change in the muon rate to the change in the vertical effective temperature, is determined to be 0.428$\pm$0.003(stat.)$\pm$0.059(syst.). An alternative description is provided by the weighted effective temperature, introduced to account for the differences in the temperature profile and muon flux as a function of zenith angle. Using the latter estimation of temperature, the coefficient is determined to be 0.352$\pm$0.003(stat.)$\pm$0.046(syst.)., Comment: 9 pages 10 figures

Abstract

The atmospheric neutrino flavour ratio measured using a 1.52 kton-year exposure of Soudan 2 is found to be 0.72 +- 0.19 +0.05 -0.07 relative to the expected value from a Monte Carlo calculation. Abstract

UNLABELLED: Objectives and importance of the study: Most older Aboriginal peoples live in urban locations. Many of these people were displaced by the policies and practices that produced the Stolen Generations. As a result, access to 'Country' and cultural landscapes that are minimally impacted by urbanisation can be limited for older Aboriginal peoples, restricting the health and wellbeing benefits these environments promote. STUDY TYPE: Qualitative study. METHODS: Our study worked collaboratively with Aboriginal traditional cultural knowledge holders to observe and analyse how participation in a 'cultural camp' on a Yuwaalaraay sacred site in New South Wales (NSW), Australia, impacted wellbeing and connection to place among older Aboriginal people who were survivors or descendants of the Stolen Generations. RESULTS: Eight participants (three women; five men) attended the cultural camp and took part in the yarning circle. Thematic analysis of a yarning circle uncovered memories of traumatic experiences of institutionalisation, including abuse and loss of Country, community, and culture. Experiences of the cultural camp generated a sense of reconnection, cultural pride, wellbeing and place attachment. The sensory experience of Country emphasised a sense of belonging and healing. CONCLUSIONS: Our findings reflect the importance of sensory-led experiences on Country for older urban Aboriginal peoples and reinforce previous evidence on the 'therapeutic' aspects of culture and natural landscapes minimally impacted by colonisation. Policies and resources supporting grassroots initiatives such as Aboriginal cultural camps are needed to ensure accessibility for older Aboriginal peoples living in urban places.

Abstract

This study described the distribution of healthy body composition among Aboriginal adolescents in Australia aged 10-24 years and examined associations with health behaviours and self-rated health. Data were cross-sectional from the 'Next Generation: Youth Well-being study' baseline (N = 1294). We used robust Poisson regression to quantify associations of self-reported health behaviours (physical activity, screen time, sleep, consumption of vegetables, fruit, soft drinks and fast food, and tobacco smoking and alcohol) and self-rated health to healthy body mass index (BMI) and waist/height ratio (WHtR). Overall, 48% of participants had healthy BMI and 64% healthy WHtR, with healthy body composition more common among younger adolescents. Higher physical activity was associated with healthy body composition (5-7 days last week vs none; adjusted prevalence ratio (aPR) healthy BMI 1.31 [95% CI 1.05-1.64], and healthy WHtR 1.30 [1.10-1.54]), as was recommended sleep duration (vs not; aPR healthy BMI 1.56 [1.19-2.05], and healthy WHtR 1.37 [1.13-1.67]). There was a trend for higher proportion of healthy body composition with more frequent fast food consumption. Healthy body composition was also associated with higher self-rated health ('very good/excellent' vs 'poor/fair'; aPR healthy BMI 1.87 [1.45-2.42], and healthy WHtR 1.71 [1.40-2.10]). Culturally appropriate community health interventions with a focus on physical activity and sleep may hold promise for improving body composition among Aboriginal adolescents.

Abstract

Background Niemann-Pick disease type C (NPC) is a rare autosomal recessive neurodegenerative lysosomal disease characterized by multiple symptoms such as progressive cerebellar ataxia and cognitive decline. The modified amino acid N-acetyl-leucine has been associated with positive symptomatic and neuroprotective, disease-modifying effects in various studies, including animal models of NPC, observational clinical case studies, and a multinational, rater-blinded phase IIb clinical trial. Here, we describe the development of a study protocol (Sponsor Code “IB1001-301”) for the chronic treatment of symptoms in adult and pediatric patients with NPC. Methods This multinational double-blind randomized placebo-controlled crossover phase III study will enroll patients with a genetically confirmed diagnosis of NPC patients aged 4 years and older across 16 trial sites. Patients are assessed during a baseline period and then randomized (1:1) to one of two treatment sequences: IB1001 followed by placebo or vice versa. Each sequence consists of a 12-week treatment period. The primary efficacy endpoint is based on the Scale for the Assessment and Rating of Ataxia, and secondary outcomes include cerebellar functional rating scales, clinical global impression, and quality of life assessments. Discussion Pre-clinical as well as observational and phase IIb clinical trials have previously demonstrated that IB1001 rapidly improved symptoms, functioning, and quality of life for pediatric and adult NPC patients and is safe and well tolerated. In this placebo-controlled cross-over trial, the risk/benefit profile of IB1001 for NPC will be evaluated. It will also give information about the applicability of IB1001 as a therapeutic paradigm for other rare and common neurological disorders. Trial registrations The trial (IB1001-301) has been registered at www.clinicaltrials.gov (NCT05163288) and www.clinicaltrialsregister.eu (EudraCT: 2021–005356-10). Registered on 20 December 2021.

Abstract

Objective There is a lack of standardisation in the terminology used to describe gout. The aim of this project was to develop a consensus statement describing the recommended nomenclature for disease states of gout. Methods A content analysis of gout-related articles from rheumatology and general internal medicine journals published over a 5-year period identified potential disease states and the labels commonly assigned to them. Based on these findings, experts in gout were invited to participate in a Delphi exercise and face-to-face consensus meeting to reach agreement on disease state labels and definitions. Results The content analysis identified 13 unique disease states and a total of 63 unique labels. The Delphi exercise (n=76 respondents) and face-to-face meeting (n=35 attendees) established consensus agreement for eight disease state labels and definitions. The agreed labels were as follows: asymptomatic hyperuricaemia', asymptomatic monosodium urate crystal deposition', asymptomatic hyperuricaemia with monosodium urate crystal deposition', gout', tophaceous gout', erosive gout', first gout flare' and recurrent gout flares'. There was consensus agreement that the label gout' should be restricted to current or prior clinically evident disease caused by monosodium urate crystal deposition (gout flare, chronic gouty arthritis or subcutaneous tophus). Conclusion Consensus agreement has been established for the labels and definitions of eight gout disease states, including gout' itself. The Gout, Hyperuricaemia and Crystal-Associated Disease Network recommends the use of these labels when describing disease states of gout in research and clinical practice.

31. Re-examining the gap: A critical realist analysis of eye health inequity among Aboriginal and Torres Strait Islander Australians

Abstract

The prevalence of diabetes among Aboriginal and Torres Strait Islander (hereafter ‘Aboriginal’) Australians is three times greater than non-Aboriginal Australians, contributing to a greater risk of blindness from treatable and preventable ocular conditions, most prominently cataract and diabetic retinopathy. In rural and remote Aboriginal communities, blindness prevalence is higher, and ocular treatment coverage and uptake are lower. In collaboration with Aboriginal Community Based Researchers (CBRs), this study explored complex contingent factors that shape access to eye health services for rural and remote Aboriginal Australians living with diabetes. Interviews (n = 126) and focus groups (n = 12) were conducted with patients, primary care clinicians, and CBRs, in four rural communities in the Northern Territory and New South Wales. Factors internal and external to health systems were examined, drawing on Bourdieu's concepts of habitus, and doxa to understand agency and decision-making among patients and clinicians. The study used the ontology of critical realism, categorising contingent factors as underlying structures (generative mechanisms), and applying a decolonising approach that centred causal relationships and tensions between dominant (Western biomedical neoliberalism) and non-dominant (Aboriginal) value systems. Three forms of marginalisation; linguistic, economic, and cultural, were identified as the generative mechanisms that inhibit equitable eye health outcomes. Marginality is linked to structural factors that position Aboriginal culture as a barrier, and is reinforced through biomedical health systems, and the agents who operate in and influence them. In order to address eye health inequity, a shift in how Aboriginal cultural sovereignty is understood within health systems is needed, to position it as a strength that can facilitate eye care accessibility, and to support enhanced cultural responsivity among clinicians and service providers.

We have searched for nucleon decay into five two-body final states using a 4.4 kiloton-year fiducial exposure of the Soudan 2 iron tracking calorimeter. For proton decay into the fully visible final states μ+η0 and e+η0, we observe zero and one event, respectively, that satisfy our search criteria for nucleon decay. The lifetime lower limits (τ/B) thus implied are 89×1030 years and 81×1030 years at 90% confidence level. For neutron decay into ν̄η0, we obtain the lifetime lower limit 71×1030 years. Limits are also reported for neutron decay into ν̄π0, and for proton decay into ν̄π+. ©2000 The American Physical Society.

Abstract

A shadow of the Moon, with a statistical significance of 5 σ, has been observed in the underground muon flux at a depth of 2090 mwe using the Soudan 2 detector. The angular resolution of the detector is well described by a Gaussian with a sigma ≤0.3°. The position of the shadow confirms that the alignment of the detector is known to better than 0.15° and has remained stable during ten years of data taking. ©2000 The American Physical Society.

A deficit of atmospheric νμ events, consistent with the hypothesis of neutrino oscillations, is observed in the 5.90 kiloton-year fiducial exposure of the Soudan 2 detector. An unbinned maximum likelihood analysis of the neutrino LIE distribution has been carried out using the Feldman-Cousins prescription. The probability of the no oscillation hypothesis is 5.8×10-4. The 90% confidence allowed region in the sin 22θ,Δm2 plane is presented. The region includes the 90% confidence allowed region of the Super-K experiment. © 2003 The American Physical Society.

We have searched for the proton decay mode p → ν K+ using the one-kiloton Soudan 2 high resolution calorimeter. Contained events obtained from a 3.56 kiloton-year fiducial exposure through June 1997 are examined for occurrence of a visible K+ track which decays at rest into μ+ ν or π+ π0. We found one candidate event consistent with background, yielding a limit, τ/B(p → ν K+) > 4.3 × 1031 years at 90% CL with no background subtraction. © 1998 Published by Elsevier Science B.V. A search for nucleon decay into two-body final states containing K0 mesons has been conducted using the 963 metric ton Soudan 2 iron tracking calorimeter. The topologies, ionizations, and kinematics of contained events recorded in a 5.52 kiloton-year total exposure (4.41 kton-year fiducial volume exposure) are examined for compatibility with nucleon decays in an iron medium. For proton decay into the fully visible final states μ+KS0 and e+KS0, zero and one event candidates are observed respectively. The lifetime lower limits (τ/B) thus implied are 1.5×1032 yr and 1.2×1032 yr, respectively. Lifetime lower limits are also reported for proton decay into l+K10, and for neutron decay into vKS0. ©2000 The American Physical Society.

Abstract

Tissue heterogeneity is a serious limiting factor for sound cell-specific molecular studies including genomic and proteomic analyses. Although tissue microdissection technologies (e.g. laser capture microdissection) have advanced tremendously over the last decades several factors such as their generally high cost and inability to microdissect fresh or live tissues limit their widespread use. Therefore, there is a need for a low-cost and easy-to-use microdissection device. Here, we developed a low-cost vacuum-assisted capillary-based cell and tissue acquisition system (CTAS) and demonstrated its use for microdissection of brain tissues samples for several downstream applications including isolation of high quality RNA from microdissected brain tissue samples, their use for proteomics studies and electron microscopy as well as microdissection of native living brain tissues for primary cell culturing. Unlike LCM, CTAS is capable of microdissecting fresh frozen and live tissues, works in a thicker tissue sections ranging from 10 mm to 300 mm and can collect individual cells, cell clusters and subanatomical regions. CTAS has been established as a straightforward and robust microdissection tool, allowing rapid, precise and efficient procurement of specific tissue and cell types at low cost. Developed microdissection protocol avoids extensive heating, chemical treatment, laser beam exposure, and other potentially harmful physical treatment of the tissue samples, thus preserving the primary functions of the dissected cells and the macromolecules within for subsequent downstream applications.

Abstract

The magnetized MINOS Near Detector, at a depth of 225 mwe, is used to measure the atmospheric muon charge ratio. The ratio of observed positive to negative atmospheric muon rates, using 301 days of data, is measured to be 1.266 +/- 0.001(stat)(-0.014)(+0.015)(syst). This measurement is consistent with previous results from other shallow underground detectors and is 0.108 +/- 0.019(stat + syst) lower than the measurement at the functionally identical MINOS Far Detector at a depth of 2070 mwe. This increase in charge ratio as a function of depth is consistent with an increase in the fraction of muons arising from kaon decay for increasing muon surface energies.

Abstract

The alarming prevalence of diabetes among the adult Australian Indigenous population (37% >55 years) (ABS, 2013) has subsequently raised the risk and prevalence of diabetic visual impairment (Taylor, 2009). Issues around prevention and access to care continue to support disparities in health equity between the Indigenous and non-Indigenous population in Australia. Using a qualitative, action research design, we aim to understand how human experiences and the social determinants which shape them, impact equity in eye health, for Indigenous Australians living with diabetes in remote communities. Results of the study were reported back to ACCHS with an aim to strengthen primary eye care systems and improve quality and access to eye care for Indigenous Australians. Semi-structured interviews (n=82) and focus groups (n=8) were conducted by Indigenous Community Based Researchers (CBRs) (TF, WF, KB, GB, FH, RS, BD, BJ) with Indigenous adults (>40 years) living with diabetes in 4 remote communities in Australia. Interviews were audio recorded and transcribed verbatim. Data was analysed thematically using grounded theory and Nvivo 10. Analysis occurred in tandem with data collection to determine theoretical saturation. The principal investigator (AY) led analysis sessions with CBRs, fostering cultural appropriateness, knowledge exchange and capacity building. Overall participants had poor knowledge of diabetic eye diseases including risk of permanent vision loss, prevention and treatment. Participants understood diabetic eye disease through physical symptoms such as blurriness; and linked this to poor diet, use of medication or the need for spectacles. Fear and distrust of mainstream services was a notable barrier to care. Participants engaged in routine diabetes management had better knowledge of risks, prevention and treatment. Abstract

The temperature of the upper atmosphere affects the height of primary cosmic ray interactions and the production of high-energy cosmic ray muons which can be detected deep underground. The MINOS far detector at Soudan, MN, has collected over 67 X 10(6) cosmic ray induced muons. The underground muon rate measured over a period of five years exhibits a 4% peak-to-peak seasonal variation which is highly correlated with the temperature in the upper atmosphere. The coefficient, alpha(T), relating changes in the muon rate to changes in atmospheric temperature was found to be alpha(T) 0: 873 +/- 0: 009(stat) +/- 0.010(syst). Pions and kaons in the primary hadronic interactions of cosmic rays in the atmosphere contribute differently to alpha(T) due to the different masses and lifetimes. This allows the measured value of alpha(T) to be interpreted as a measurement of the K/pi ratio for E-p greater than or similar to 7 TeV of 0.12(-0.05)(+0.07), consistent with the expectation from collider experiments.

Abstract

We report the results of a search for nu(mu) disappearance by the Main Injector Neutrino Oscillation Search [D. G. Michael (MINOS), Phys. Rev. Lett. 97, 191801 (2006).]. The experiment uses two detectors separated by 734 km to observe a beam of neutrinos created by the Neutrinos at the Main Injector facility at Fermi National Accelerator Laboratory. The data were collected in the first 282 days of beam operations and correspond to an exposure of 1.27x10(20) protons on target. Based on measurements in the Near Detector, in the absence of neutrino oscillations we expected 336 +/- 14 nu(mu) charged-current interactions at the Far Detector but observed 215. This deficit of events corresponds to a significance of 5.2 standard deviations. The deficit is energy dependent and is consistent with two-flavor neutrino oscillations according to |Delta m(2)|=2.74(-0.26)(+0.44) x 10(-3) eV(2)/c(4) and sin(2)2 theta > 0.87 at 68% confidence level.

Abstract

We found 140 neutrino-induced muons in 854.24 live days in the MINOS far detector, which has an acceptance for neutrino-induced muons of 6.91x10(6) cm(2) sr. We looked for evidence of neutrino disappearance in this data set by computing the ratio of the number of low momentum muons to the sum of the number of high momentum and unknown momentum muons for both data and Monte Carlo expectation in the absence of neutrino oscillations. The ratio of data and Monte Carlo ratios, R, is R=0.65(-0.12)(+0.15)(stat)+/- 0.09(syst), a result that is consistent with an oscillation signal. A fit to the data for the oscillation parameters sin(2)2 theta(23) and Delta m(23)(2) excludes the null oscillation hypothesis at the 94% confidence level. We separated the muons into mu(-) and mu(+) in both the data and Monte Carlo events and found the ratio of the total number of mu(-) to mu(+) in both samples. The ratio of those ratios, R boolean AND(CPT), is a test of CPT conservation. The result R boolean AND(CPT)=0.72(-0.18)(+0.24)(stat)(-0.04)(+0.08)(syst) is consistent with CPT conservation.
