1. High flow nasal cannula combined with non-invasive ventilation versus high flow nasal cannula alone in patients with acute hypoxemic respiratory failure due to pneumonia: a randomized controlled trial

Abstract

Patients with hypoxemic respiratory failure due to community acquired pneumonia are actually treated with a wide range of oxygen devices from nasal cannula to mechanical ventilation. In this monocentric, open label, randomized controlled trial we aimed to compare the efficacy of combined High Flow Nasal Cannula (HFNC) and Non-Invasive Ventilation (NIV) versus HFNC alone in acute Hypoxemic Respiratory Failure (hARF) in patients affected by Community Acquired Pneumonia (CAP). We enrolled 49 patients affected by CAP with hypoxemic respiratory failure (PO2/FiO2 < 300). The patients were randomized into two groups: one has been treated with HFNC alone (group A) while the other received NIV alternated to HFNC every 3 hours (group B). The primary outcome was PO2/FiO2 change from baseline to 21 hours. Secondary outcomes included variation of pH and pCO2, need to continue HFNC or NIV/HFNC after 45 hours, orotracheal intuba-tion, mortality rate, and device comfort. No statistically significant differences between the two arms were shown in PO2/FiO2 change at 21 hours since baseline, in pCO2 and pH variation, mortality at hospital and at follow-up. Further research is needed to better understand the role of combined HFNC and NIV in hypoxemic respiratory failure in patients with CAP. r.

Abstract

Data capturing multiple axes of tree size and shape, such as a tree's stem diameter, height and crown size, underpin a wide range of ecological research—from developing and testing theory on forest structure and dynamics, to estimating forest carbon stocks and their uncertainties, and integrating remote sensing imagery into forest monitoring programmes. However, these data can be surprisingly hard to come by, particularly for certain regions of the world and for specific taxonomic groups, posing a real barrier to progress in these fields. To overcome this challenge, we developed the Tallo database, a collection of 498,838 georeferenced and taxonomically standardized records of individual trees for which stem diameter, height and/or crown radius have been measured. These data were collected at 61,856 globally distributed sites, spanning all major forested and non-forested biomes. The majority of trees in the database are identified to species (88%), and collectively Tallo includes data for 5163 species distributed across 1453 genera and 187 plant families. The database is publicly archived under a CC-BY 4.0 licence and can be access from: https://doi.org/10.5281/zenodo.6637599. To demonstrate its value, here we present three case studies that highlight how the Tallo database can be used to address a range of theoretical and applied questions in ecology—from testing the predictions of metabolic scaling theory, to exploring the limits of tree allometric plasticity along environmental gradients and modelling global variation in maximum attainable tree height. In doing so, we provide a key resource for field ecologists, remote sensing researchers and the modelling community working together to better understand the role that trees play in regulating the terrestrial carbon cycle. © 2022 The Authors. Abstract

Aim Tree crowns determine light interception, carbon and water exchange. Thus, understanding the factors causing tree crown allometry to vary at the tree and stand level matters greatly for the development of future vegetation modelling and for the calibration of remote sensing products. Nevertheless, we know little about large-scale variation and determinants in tropical tree crown allometry. In this study, we explored the continental variation in scaling exponents of site-specific crown allometry and assessed their relationships with environmental and stand-level variables in the tropics. Location Global tropics. Time period Early 21st century. Major taxa studied Woody plants. Methods Using a dataset of 87,737 trees distributed among 245 forest and savanna sites across the tropics, we fitted site-specific allometric relationships between crown dimensions (crown depth, diameter and volume) and stem diameter using power-law models. Stand-level and environmental drivers of crown allometric relationships were assessed at pantropical and continental scales. Results The scaling exponents of allometric relationships between stem diameter and crown dimensions were higher in savannas than in forests. We identified that continental crown models were better than pantropical crown models and that continental differences in crown allometric relationships were driven by both stand-level (wood density) and environmental (precipitation, cation exchange capacity and soil texture) variables for both tropical biomes. For a given diameter, forest trees from Asia and savanna trees from Australia had smaller crown dimensions than trees in Africa and America, with crown volumes for some Asian forest trees being smaller than those of trees in African forests. Main conclusions Our results provide new insight into geographical variability, with large continental differences in tropical tree crown allometry that were driven by stand-level and environmental variables. They have implications for the assessment of ecosystem function and for the monitoring of woody biomass by remote sensing techniques in the global tropics.

Abstract

The purpose of this paper is to present a project in order to verify -without the need of knowing the distance CERN-Gran Sasso- the discovery made by the OPERA Collaboration concerning the speed of the CERN neutrinos. The project consists of two parts. A simple one and a less simple one. Both have the great advantage of being totally independent of the knowledge of the distance, ≃ 732 km, between the two Labs, CERN and LNGS, where the neutrinos are produced and detected, respectively. The "simple" version of this project is based on the high-energy horizontal cosmic muons, which traverse LVD and OPERA detectors, thus allowing to cross-calibrate the timing systems of both experiments in a way which is totally independent of the TOF measurements of CNGS. This component of the project is being studied in collaboration with the OPERA group, as the time stabilities of both experiments are needed. In fact it is since a long time that the two groups are engaged with this problem. In this paper we will present and discuss the "less simple" part which allows to establish, at the highest possible level of accuracy, if (v > c) effects really exist.

9. Synthesis and Conformational analysis of fructose-derived scaffolds: molecular diversity from a single molecole

Abstract

Bi- and tricyclic compounds were synthesized starting from fructose. The different hydroxyl groups present in fructose were exploited in the formation of a number of conformationally constrained sugar-based scaffolds, including azido acids. Introduction of an azido group and carboxy terminus into different bicyclic iodo ethers, allowed the synthesis of different conformationally constrained azido acids. Conformational analysis of compounds 10, 11, 17, and 20 by NMR experiments assisted by molecular mechanics, allowed the determination of the distances between the relevant functional groups, that is the azido and carboxy functionalities.

The objective of this study was to test the value of the concept of the net commercial value of standing timber in predicting the impact of logging activities on forest-cover modifications. A study area was selected in the East province of Cameroon which contains major primary forests and which contributes strongly to national timber production. A Geographic information System containing ecological and economic variables was used in combination with remote sensing data to define the net commercial value of standing timber in the East province. Taking account of the potential commercial value of standing timber improves our understanding of the spatial determinants of logging activities and of the resulting forest-cover modifications. The occurrence of logging-induced forest-cover modifications increases with the value of forest rent in one of the study sites, half of the very high rent areas have already been logged. In that site, therefore, it is mostly the low rent or marginal forest areas that remain unlogged. This was not the case, however, throughout the study area as shown by the observations at another site. (C) 2001 Academic Press.

Abstract

Raw and blanched oregano (Origanum vulgare L. ssp. prismaticum Gaudin) were air-dried at 35° and 50°C in two drier pilot plants; raw oregano was also dried at room temperature in a shady place. Oregano leaves were analysed for volatile phenolic compounds by GC/MS-SIM, for chlorophyll by HPLC and for colour by reflected-light colorimetry. The highest contents of thymol and carvacrol were found in unblanched oregano dried at 35°C, followed by oregano dried at room temperature and by both samples dried at 35° and 50°C after blanching. The highest chlorophyll content was identified in oregano dried at room temperature, followed by unblanched oregano dried at 50°C and blanched oregano dried at 35°C. All unblanched, air-dried oregano leaves had colour attributes similar to the raw ones, while blanched-dried leaves were greener. Abstract

Acetatobis(triphenylphosphine)dicarbonylrhenium (I), (PPh3)2(CO)2Re(O2CCH3), has been prepared in a novel way by treating (PPh3)2(CO)2Re(NHCOR) (R = C6H5, p-MeC6H4) with triethylamine and water in the presence of air. Oxidation of the ethyl group of the tertiary amine is presumably involved in the formation of the acetate ligand. Three-dimensional single-crystal X-ray diffraction analysis shows that the complex is octahedral with the phosphines in transpositions and the acetate ion acting as a chelating ligand. The complex crystallizes in theP21/c space group with cell dimensions a= 17.63(2), b= 9.72(1), c= 20.95(2)Å, β = 104°38'(6'), Z= 4. The mean values of bond lengths observed are Re-P 2.415, Re-O 2.21 and Re-(CO) 1.85Å. The same acetate derivative and a series of car☐ylato complexes (PPh3)2(CO)2Re(O2CR') have been obtained from reactions of Re(CO)2(PPh3)3H with car☐ylic acids R'COOH (R' = H, CH3, CH2Cl, CH2CH3, C6H5). When trifluoroacetic acid is used, a product of formula (PPh3)2(CO)3Re(OC(O)CF3) is isolated. The action of neutral ligands L on some of these products gave rise to derivatives of formula (PPh3)2(CO)2(L)Re(OC(O)R') (L = CO, R' = H, CH3, C6H5; L = pMeC6H4NC, R' = CH3), having monodentate car☐ylato moieties.

Abstract

Patients with hypoxemic respiratory failure due to community acquired pneumonia are actually treated with a wide range of oxygen devices from nasal cannula to mechanical ventilation. In this monocentric, open label, randomized controlled trial we aimed to compare the efficacy of combined High Flow Nasal Cannula (HFNC) and Non-Invasive Ventilation (NIV) versus HFNC alone in acute Hypoxemic Respiratory Failure (hARF) in patients affected by Community Acquired Pneumonia (CAP). We enrolled 49 patients affected by CAP with hypoxemic respiratory failure (PO2/FiO2 < 300). The patients were randomized into two groups: one has been treated with HFNC alone (group A) while the other received NIV alternated to HFNC every 3 hours (group B). The primary outcome was PO2/FiO2 change from baseline to 21 hours. Secondary outcomes included variation of pH and pCO2, need to continue HFNC or NIV/HFNC after 45 hours, orotracheal intuba-tion, mortality rate, and device comfort. No statistically significant differences between the two arms were shown in PO2/FiO2 change at 21 hours since baseline, in pCO2 and pH variation, mortality at hospital and at follow-up. Further research is needed to better understand the role of combined HFNC and NIV in hypoxemic respiratory failure in patients with CAP. r.

Abstract

Conformational analysis of DNA–basic polypeptide complexes, based on stereo-chemical criteria and energy calculations and experimental results, is reported. Three types of polypeptide conformations were selected: the distorted β conformation similar to that proposed by Feughelmann et al.; a structure characterized by a repetition of a right-handed and left-handed α helix-type conformation; and an intermediate structure. A model of the complex between DNA and basic polypeptides was proposed, where the polypeptide chain fits the narrow groove of the B form of DNA. Van der Waals, hydrogen bond, electrostatic, and hydrophobic forces cooperate to stabilize the association complex. This structure also seems to be suitable to represent the molecular model of nucleoprotamines. In the case of nucleohistones, both grooves of DNA are involved in the interaction with the proteins. These have the nonbasic-rich portion in the α-helical conformation, whereas the part where a greater proportion of basic amino acids occurs presents a structure similar to nucleoprotamines and complexes between DNA and basic homopolypeptides. The distribution of basic residues of the F2A1(IV) histone on DNA is markedly disproportionate for the two opposite cylindrical sectors of the double helix. This suggests a new mechanism of supercoiling in nucleo-histones.

Abstract

Objective: To analyze the risk and protective factors to the development of children under three years of age during the COVID-19 pandemic., Method: Cross-sectional, quantitative study carried out in three early childhood education centers in the city of São Paulo, Brazil, in October 2020. The data were collected with an online questionnaire. Risk and protection factors were measured with the Primeira Infância Para Adultos Saudáveis (Early Childhood For Healthy Adults) instrument and the children's development status was measured using the Caregiver Reported Early Development Instruments - CREDI., Results: The study included 108 parents and guardians of children up to three years of age. Living with grandparents and participating in cash transfer programs were protective factors for child development. The family being headed by a woman posed a significant risk factor for child development., Conclusion: Intersectoral actions to support families headed by women and access to cash transfer programs are essential for promoting equity opportunities for the development in early childhood.

Abstract

Hepatic fibrolamellar carcinoma (FLC) is an uncommon tumour that differs from hepatocellular carcinoma (HCC) in demographics, condition of the affected liver, tumour markers, and prognosis. FLC characteristically manifests as a large hepatic mass in adolescents or young adults with female predominance (mean age 23 years). Cirrhosis, elevated alpha-fetoprotein levels, and risk factors for HCC such as viral hepatitis are typically absent. FLC is usually associated with serum tumour markers such as vitamin B12 binding protein, and neurotensin. FLC is characterized pathologically by cords of tumour cells surrounded by abundant collagenous fibrous tissue arranged in a parallel or lamellar distribution. FLC usually appears on radiologic images as a lobulated heterogeneous mass with a central scar in an otherwise normal liver. The clinical presentation of patients with FLC is variable. These patients commonly have pain, and palpable right upper quadrant abdominal mass. An uncommon presenting sign is gynaecomastia in men. Use of percutaneous biopsy (FNAB) is beneficial if there is diagnostic uncertainty about the radiologic diagnosis (US, CT MRI). Although FLC is frequently recurrent, patients have a better prognosis than those with HCC, and aggressive surgical liver resection with extended lymphadenectomy or liver transplantation may be indicated. The presence of advanced-stage disease, direct invasion of adjacent organs, lymphadenopathy, or limited metastasis does not preclude attempts at curative resection. In inoperable cases, the patient may benefit from chemotherapy, permitting in up to 50% of these cases a curative resection. The case is reported of a 18-year-old man with bilateral gynecomastia secondary to an unknown hepatic fibrolamellar carcinoma producing oestrogens. Serum alpha-fetoprotein was negative; des-gamma-carboxy prothrombin (DCP) level was elevated. CT scan and MRI showed a solid hepatic tumour (theta 10 cm) without evidence of extrahepatic spreading. By a needle biopsy a fibrolamellar carcinoma was diagnosed. On March 1995 a right hemihepatectomy was performed. The postoperative course was uneventful and the patient recovered. Specimen's histologic examination confirmed the preoperative diagnosis. Intracellular (hepatocytes) oestrogens were found, but oestrogen and androgen receptors were negative. After surgery DCP and oestradiol levels rapidly decreased and gynaecomastia disappeared. A follow-up program was established. On April 2000 a probable recurrence within the caudate lobe was discovered by a liver CT scan without evidence of extrahepatic spreading. Tumour markers, FNAB, and bone scintigraphy were negative. On July 2000 the patient underwent second look laparotomy. Only a coeliac lymphadenopathy was found and a lymphadenectomy performed. Specimen's histologic examination showed a metastatic lymph nodal disease (FLC). The postoperative course was uneventful and the patient recovered. He is currently alive without evidence of recurrence 5 years after the second operation.
