Acute total occlusion of the unprotected left main coronary artery (LMCA) is a dramatic event. There are limited data regarding this population. We aimed to describe the clinical presentation and outcomes of patients and to determine predictors of in-hospital mortality.

Purpose: Physical inactivity is considered an important risk factor for osteoporosis, however, some athletes performing extremely high training volumes can also develop bone mass loss. Moreover, the effect of total body mass or body surface area on bone mineral density remains controversial. Therefore, the aim of this study was to compare the absolute bone mineral density and bone mineral density adjusted to body surface area between amateur triathletes and nonactive women.Methods: Forty-two healthy women (23 amateur triathletes and 19 nonactive individuals) were evaluated for body composition using a dual-energy X-ray absorptiometry system.Results: Compared to nonactive women, amateur triathletes exhibited lower body mass index (p < 0.001), lower bone mineral density (p < 0.001), and body surface area (p < 0.001). However, bone mineral density adjusted by body surface area in the triathletes was higher than in the nonactive women (p = 0.03).Conclusion: These findings showed that amateur triathles presented lower absolute bone mineral density, but higher bone mineral density adjusted to body surface area. Future studies are recommended to identify if the higher bone mineral density adjusted to body surface area are associated with a lower bone fragility.

Atrial fibrillation (AF), the most common arrhythmia in the adult population worldwide, represents a significant burden in terms of cardiovascular mortality and morbidity and has repercussions on health economics. Oral anticoagulation (OAC) is key to stroke prevention in AF and, in recent years, results from landmark clinical trials of non-vitamin K oral anticoagulants (NOAC) have triggered a paradigm shift in thrombocardiology. Despite these advances, there is still a significant residual vascular risk associated with silent AF, bleeding, premature sudden death and heart failure.

Os anticoagulantes orais demonstraram ser altamente eficazes na prevenção do acidente vascular cerebral (AVC) associado a fibrilhação auricular (FA). A ocorrência de AVC apesar da hipocoagulação oral não é expectável, existindo escassa informação sobre os mecanismos responsáveis pela sua ocorrência. O objetivo deste estudo foi avaliar possíveis mecanismos para a ocorrência de AVC isquémico, como a má adesão à terapêutica e a não adequação da dosagem dos anticoagulantes em doentes cronicamente hipocoagulados por FA.

Background Cerebral edema is frequent in patients with acute ischemic stroke (AIS) who undergo reperfusion therapy and is associated with high mortality. The impact of collateral pial circulation (CPC) status on the development of edema has not yet been determined.Methods We studied consecutive patients with AIS and documented M1–middle cerebral artery (MCA) and/or distal internal carotid artery (ICA) occlusion who underwent reperfusion treatment. Edema was graded on the 24-hour non-contrast computed tomography (NCCT) scan. CPC was evaluated at the acute phase (≤6 hours) by transcranial color-coded Doppler, angiography and/or CT angiography. We performed an ordinal regression model for the effect of CPC on cerebral edema, adjusting for age, baseline National Institutes of Health Stroke Scale, Alberta Stroke Program Early Computed Tomography Score (ASPECTS) on admission, NCCT, parenchymal hemorrhagic transformation at 24 hours and complete recanalization at six hours.Results Among the 108 patients included, 49.1% were male and mean age was 74.2 ± 11.6 years. Multivariable analysis showed a significant association between cerebral edema and CPC status (OR 0.22, 95% CI 0.08–0.59, p= 0.003), initial ASPECTS (OR 0.72, 95% CI 0.57–0.92, p= 0.007) and parenchymal hemorrhagic transformation (OR 23.67, 95% CI 4.56–122.8, p< 0.001).Conclusions Poor CPC is independently associated with greater cerebral edema 24 hours after AIS in patients who undergo reperfusion treatment.

Sudden cardiac death is a major public health challenge, which can be caused by genetic or acquired structural or electrophysiological abnormalities. These abnormalities include hereditary channelopathies: long QT, short QT and Brugada syndromes. These syndromes are a notable concern, particularly in young people, due to their high propensity for severe ventricular arrhythmias and sudden cardiac death.

Brugada syndrome is an inherited cardiac condition with the potential for development of life-threatening arrhythmias in relatively young individuals without significant structural cardiac abnormalities. The condition is characterized by a distinct coved-type ST segment elevation in the right precordial leads (V1-V3). This hallmark pattern (type 1) is often dynamic and sometimes concealed, and may be unmasked in certain conditions or under the effect of certain agents, which include variation of sympathovagal balance, hormones, metabolic factors and drugs. These factors may not only modulate electrocardiographic morphology and induce the characteristic type 1 pattern, but also predispose to ventricular arrhythmias. The risk of malignant arrhythmias in acute events with induced type 1 pattern may be imminent, particularly if the patient in fact has Brugada syndrome. The physician should be aware of the modulating factors that may underlie a Brugada pattern, and be able to recognize, identify and promptly correct them. The mechanisms responsible for the type 1 pattern and possible associated ventricular arrhythmias induced by these modulating factors have attracted growing attention and interest. Furthermore, not all induced Brugada ECG patterns are observed in patients with Brugada syndrome, existing the possibility for acquired Brugada patterns/syndrome and Brugada phenocopies. This paper reviews the modulating factors associated with induced type 1 pattern as possible causes of arrhythmogenesis, particularly in Brugada syndrome patients, describes some of the probable underlying mechanisms, and discusses the concepts of acquired Brugada syndrome and Brugada phenocopies.

O condrossarcoma é uma neoplasia maligna formadora de cartilagem. O tratamento é difícil, devido à resistência tanto à quimioterapia como à radiação; a excisão local ampla é o único tratamento. O presente estudo relata o caso de um homem ativo de 43 anos diagnosticado com condrossarcoma de células claras do fêmur esquerdo proximal recorrente, previamente reconstruído com prótese total de quadril, estendia‐se à abóbada do acetábulo, que sustenta peso. O estudo de estadiamento de câncer não revelou sinais de disseminação tumoral a distância. Considerando o excelente estado funcional do paciente, os autores fizeram uma ressecção pélvica periacetabular do tipo Enneking‐Dunham com ressecção em bloco, com a prótese total do quadril, incluiu 22cm do fêmur e uma porção do aparelho abdutor do quadril. A reconstrução acetabular foi feita com uma prótese de pedestal não cimentada fixada no ilíaco e a reconstrução no fêmur usou uma prótese modular cimentada para o fêmur proximal com revestimento em prata. Hoje, após um seguimento de dez anos, o paciente anda sem muletas, pratica ciclismo recreativo sem assistência e está assintomático e livre de doença tumoral. Não foram observados sinais de afrouxamento relevante, instabilidade, infecção, ossificação heterotópica ou quaisquer outras complicações. As reconstruções pélvicas são cirurgias difíceis e arriscadas; entretanto, o surgimento de implantes mais funcionais, como a prótese de pedestal, e sua correta aplicação e indicação podem permitir resultados clínicos e funcionais promissores, com baixa taxa de complicações.

Syncope is a common but concerning event in young athletes. Although mostly due to benign reflex causes, syncope may be arrhythmic and precede sudden cardiac death. Efforts must therefore be made to distinguish post-exertional syncope from syncope during exercise, which can be an ominous sign of a possible underlying heart disease, such as hypertrophic cardiomyopathy. Prevention requires cooperation between physician and athlete, in order to identify individuals at risk and to protect them from sudden death. Solving this diagnostic dilemma may lead to recommendations for athletes to be cleared to play or disqualified from competitive sports, and presents challenging and controversial decisions to the health care provider that can prove difficult to implement. Although exercise contributes to physical and psychological well-being, there are insufficient data to indicate whether an athlete with hypertrophic cardiomyopathy diagnosed after a syncopal episode can safely resume competitive physical activity. The purpose of this study was to review the literature on syncope in young athletes and its relationship to individuals with hypertrophic cardiomyopathy, in order to enable accurate assessment of prognosis and the possibility of resuming competitive sports.

Young athletes are considered the healthiest group in society. Although rare, there are still reports of sudden death or cardiac arrest on the playing fields. Clinical evaluation is of paramount importance for the identification of possible pathological states that confer increased risk of these events. Interpretation of the electrocardiogram of young athletes can help identify changes associated with heart disease that might preclude the participation in sports. In this context, it is essential to recognize the electrocardiographic patterns that represent the structural and electrical remodeling resulting from continued adaptation to exercise, and which thus do not increase the risk of adverse events during exercise. The European Society of Cardiology (ESC) and the American Heart Association (AHA) have issued consensus documents summarizing which electrocardiographic abnormalities should be considered ‘physiological’, resulting from adaptation to exercise (‘athlete's heart’), and which should be considered pathological and thus require further study. However, the two societies have different approaches with respect to the electrocardiographic screening of athletes. This paper provides a brief review of current evidence regarding the electrocardiographic findings considered normal and abnormal in athletes, and presents the arguments of the ESC and AHA for electrocardiographic screening in this population.

Wristband devices used in the compression of the radial puncture site add cost to the procedure and have not been adequately compared with conventional compressive dressings. This study evaluated the effectiveness and safety of both forms of hemostasis in patients undergoing coronary angiography and/or percutaneous coronary intervention in daily practice.

Avaliamos a efetividade e a segurança de ambas as formas de hemostasia em pacientes submetidos à cinecoronariografia e/ou intervenção coronária percutânea na prática diária.

Orthostatic intolerance (OI) syndromes are a confusing topic and determining a specific diagnosis to achieve optimal treatment can be troublesome. We sought to assess biomarker, hemodynamic and autonomic variables in OI patients (autonomic dysfunction [AD], postural orthostatic tachycardia syndrome [POTS] and neurally mediated syncope [NMS]) and healthy controls during supine and head-up tilt position in order to achieve a better diagnosis.

An accurate and unified finite element description is presented, in both stiffness and flexibility formats, of the response of planar elastic beams subject to terminal loads. The stability and bowing functions are interrelated, their descriptions accounting for the effects of axial deformability and a measure of the shear deformation effects. A perturbation technique is used to solve the nonlinear governing equations. The solutions so derived are cast in four distinct forms: 1 Deformation; 2 incremental; 3 perturbation; and 4 asymptotic forms of structural analysis. A brief comparison between related formulations presented in the literature is included.
