6. Coding for Parallel Channels: Gallager Bounds for Binary Linear Codes with Applications to Repeat-Accumulate Codes and Variations

Abstract

This paper is focused on the performance analysis of binary linear block codes (or ensembles) whose transmission takes place over independent and memoryless parallel channels. New upper bounds on the maximum-likelihood (ML) decoding error probability are derived. These bounds are applied to various ensembles of turbo-like codes, focusing especially on repeat-accumulate codes and their recent variations which possess low encoding and decoding complexity and exhibit remarkable performance under iterative decoding. The framework of the second version of the Duman and Salehi (DS2) bounds is generalized to the case of parallel channels, along with the derivation of their optimized tilting measures. The connection between the generalized DS2 and the 1961 Gallager bounds, addressed by Divsalar and by Sason and Shamai for a single channel, is explored in the case of an arbitrary number of independent parallel channels. The generalization of the DS2 bound for parallel channels enables to re-derive specific bounds which were originally derived by Liu et al. as special cases of the Gallager bound. In the asymptotic case where we let the block length tend to infinity, the new bounds are used to obtain improved inner bounds on the attainable channel regions under ML decoding. The tightness of the new bounds for independent parallel channels is exemplified for structured ensembles of turbo-like codes. The improved bounds with their optimized tilting measures show, irrespectively of the block length of the codes, an improvement over the union bound and other previously reported bounds for independent parallel channels; this improvement is especially pronounced for moderate to large block lengths., Comment: Submitted to IEEE Trans. on Information Theory, June 2006 (57 pages, 9 figures)

Abstract

Background: Diastolic dysfunction (DD) might help to risk stratify patients with heart failure (HF) with reduced ejection fraction (HFrEF). Nonetheless, HF guidelines/risk scores don't consider DD. We aimed to show the independent prognostic value of DD for nonfatal HF/death in patients with HFrEF on top of natriuretic peptides (NP). Methods: We analyzed 1155 baseline echocardiograms of the MADIT-CRT study (LVEF≤30%, QRS ≥ 130 ms, NYHA class I/II), classifying DD according to 2016 ASE/EACVI classification. Results: Patients were 64 ± 11 years-old, 24% females, LVEF was 24 ± 5%, 58% had abnormal BNP (≥100 pg/ml). While 45% had impaired relaxation, 33% had pseudonormal filling, 12% restrictive pattern, 6% indeterminate diastolic function, 4% were not classifiable due to missing data. During a follow-up of 2.1 ± 1.0 years, there were 233 HF/death. Compared to patients without pseudonormal/restrictive filling and with normal NP (23%), patients with pseudonormal/restrictive filling, alone (15%) or combined to elevated NP (30%), were at higher risk of events (respectively padj = 0.025 and padj < 0.001), as opposed to those with abnormal NP alone (22%; padj = 0.55). Adding DD to conventional markers of risk and NP improved prediction (C-statistic 0.733 versus 0.708, p = 0.024). DD was the first parameter to be considered to risk stratify MADIT-CRT population, according to Classification-And-Regression-Tree analysis. Conclusions: Among HFrEF patients with mild symptoms, pseudonormal/restrictive filling, either alone or combined with elevated NP, was associated with high risk of events, as opposed to isolated elevation of NP. DD provided incremental risk prediction for death/HF beyond commonly used markers. These data might suggest to integrate DD into HF guidelines/risk scores.

Abstract

Background: Risk stratification in long QT syndrome (LQTS) patients is important for optimizing patient care and informing clinical decision making. We developed a risk prediction algorithm with prediction of 5-year absolute risk of the first life-threatening arrhythmic event [defined as aborted cardiac arrest, sudden cardiac death, or appropriate implantable cardioverter defibrillator (ICD) shock] in LQTS patients, accounting for individual risk factors and their changes over time. Methods: Rochester-based LQTS Registry included the phenotypic cohort consisting of 1,509 LQTS patients with a QTc ≥ 470 ms, and the genotypic cohort including 1,288 patients with single LQT1, LQT2, or LQT3 mutation. We developed two separate risk prediction models which included pre-specified time-dependent covariates of beta-blocker use, syncope (never, syncope while off beta blockers, and syncope while on beta blockers), and sex by age < and ≥13 years, baseline QTc, and genotype (for the genotypic cohort only). Follow-up started from enrollment in the registry and was censored at patients’ 50s birthday, date of death due to reasons other than sudden cardiac death, or last contact, whichever occurred first. The predictive models were externally validated in an independent cohort of 1,481 LQTS patients from Pavia, Italy. Results: In Rochester dataset, there were 77 endpoints in the phenotypic cohort during a median follow-up of 9.0 years, and 47 endpoints in the genotypic cohort during a median follow-up of 9.8 years. The time-dependent extension of Harrell’s generalized C-statistics for the phenotypic model and genotypic model were 0.784 (95% CI: 0.740–0.827) and 0.785 (95% CI: 0.721–0.849), respectively, in the Rochester cohort. The C-statistics obtained from external validation in the Pavia cohort were 0.700 (95% CI: 0.610–0.790) and 0.711 (95% CI: 0.631–0.792) for the two models, respectively. 12. The Addict and Ex-Addict in the World of Work: Job Finding Experiences and Performance in Employment. Final Report.

Abstract

The purpose of the study was to investigate three separate aspects of the process by which ex-addicts-in-treatment are prepared for, enter into, and function in the world of work. These aspects were the actual labor market conditions in two different urban settings--New York City and the greater Boston area; the specific institutional supports available to ex-addicts and addicts-in-treatment as those supports are perceived by both clients and providers of service; and an analysis, by employers, of the performance and behavior of ex-addicts and addicts-in-treatment once they have secured employment. Surveys were taken to define the practices of drug treatment programs. In addition, a questionnaire was developed to isolate the key employment related issues and practices that comprise clients' and providers of service' perceptions of the rehabilitative situation. Finally, through the development of a specific relationship with PACT (Provide Addicts Care Today), the job-related behavior of ex-addicts and addicts-in-treatment was assessed. Results indicated that (1) the two urban labor markets examined offer only limited options for meaningful, remunerative, and stable employment, (2) drug treatment programs, despite stated priorities involving preparation for employment, are neither structured, funded, nor staffed to adequately provide concrete employment related services, (3) clients rely more on their own rather limited resources in order to gain employment, and (4) ex-addicts who are hired and retained are rated by employers as above average in performance, compared to non-addicts. 13. Psychotherapy and the Psychotherapist: The Non-Professional as Clinician Change Agent.

Abstract

Current research in psychotherapy is indicted because: (1) it omits the question of social or political values; and (2) it fails to come to grips with the fact that social, political and economic institutions are a large part of the problems of those who need help the most. Poverty is defined in psychological terms as a pattern of hopelessness and helplessness, of feeling limited and expendable. In these terms, psychotherapy is viewed as excluding the old, the black, the poor and the ignorant. Given the absence of hard data supporting the effectiveness of psychotherapy with these individuals, a case is made for the use of non-prfessionals in mental health efforts which could serve this unserved population. In line with this, it is proposed that psychotherapy must couple individual remediation with attempts at institutional change. A residential youth center, formed by the author and a group of non-professionals in New Haven's ghettos, is offered as an example. Twenty youngsters, aged 16-21, previously adjudged to have almost insurmountable problems, were admitted. A control group was used. Results showed increased work attendance, increased incomes, and decreased arrests and time in jail. Also, the group came to feel less alienated and more trustful than the control group. (TL)

Abstract

The clinical course of patients with 2 relatively common long QT syndrome type 3 mutations has not been well described. In the present study, we investigated the mutational-specific risk in patients with deletional (DeltaKPQ) and missense (D1790G) mutations involving the SCN5A gene. The study population involved 50 patients with the DeltaKPQ mutation and 35 patients with the D1790G mutation. The cumulative probability of a first cardiac event (syncope, aborted cardiac arrest, or long QT syndrome-related sudden death) was evaluated using the Kaplan-Meier method. The Cox proportional hazards survivorship model was used to determine the independent contribution of clinical and genetic factors to the first occurrence of cardiac events from birth through 40 years of age. The Andersen-Gill proportional intensity regression model was used to analyze the factors associated with recurrent syncope. Patients with a DeltaKPQ mutation had a significantly greater probability of a first cardiac event from birth through 40 years of age (34%) than those with the D1790G mutation (20%; p <0.001). Multivariate analysis demonstrated an increased risk of cardiac events among DeltaKPQ carriers compared to D1790G carriers (hazard ratio 2.42, p <0.0001) after adjustment for gender and QTc duration. Patients with DeltaKPQ mutations also had an increased risk of recurrent syncope (hazard ratio 5.20, p <0.001). In conclusion, the clinical course of patients with long QT syndrome type with DeltaKPQ mutations was shown to be more virulent than those with D1790G mutations, and this effect was independent of QTc duration. The findings highlight the importance of knowing the specific mutation in risk stratification of patients with long QT syndrome type 3

Abstract

Objectives: This study was designed to evaluate the clinical and prognostic aspects of long QT syndrome (LQTS)-related cardiac events that occur in the first year of life (infancy). Background: The clinical implications for patients with long QT syndrome who experience cardiac events in infancy have not been studied previously. Methods: The study population of 3,323 patients with QT interval corrected for heart rate (QTc) ≥450 ms enrolled in the International LQTS Registry involved 20 patients with sudden cardiac death (SCD), 16 patients with aborted cardiac arrest (ACA), 34 patients with syncope, and 3,253 patients who were asymptomatic during the first year of life. Results: The risk factors for a cardiac event among 212 patients who had an electrocardiogram recorded in the first year of life included QTc ≥500 ms, heart rate ≤100 beats/min, and female sex. An ACA before age 1 year was associated with a hazard ratio of 23.4 (p < 0.01) for ACA or SCD during ages 1 to 10 years. During the 10-year follow-up after infancy, beta-blocker therapy was associated with a significant reduction in ACA/SCD only in those with a syncopal episode within 2 years before ACA/SCD but not for those who survived ACA in infancy. Conclusions: Patients with LQTS who experience ACA during the first year of life are at very high risk for subsequent ACA or death during their next 10 years of life, and beta-blockers might not be effective in preventing fatal or near-fatal cardiac events in this small but high-risk subset

49. Organizational commitment in the military: Application of a profile approach

Abstract

According to the 3-component model of commitment, the individual components of commitment-affective (AC), normative (NC), and continuance (CC)-combine to form profiles, and these profiles have different implications for behavior and wellbeing. We tested these propositions in a military context and also examined conditions (perceived organizational support, organizational justice, job satisfaction, and satisfaction with leadership) that might contribute to the development of commitment profiles. Latent profile analyses of data from 6,501 respondents to the 2010 Canadian Forces Retention Survey revealed 6 distinct profiles. Personnel with profiles reflecting strong AC and NC reported the most favorable work conditions, stay intentions, and wellbeing; uncommitted personnel and those with CC-dominant profiles reported the least favorable conditions, were most active in job search activities, and scored highest on anxiety and depression. The value of taking a profile approach, and the implications of managing work conditions to promote optimal profiles, are discussed.
