However, it remains unclear whether or not the BP itself or other parameters related to the BP are associated with WMLs in hypertensive patients with well-controlled BP. We investigated the relationships of the presence of WMLs with the central systolic BP (cSBP) and estimated glomerular filtration rate (eGFR) in treated hypertensive patients.Method: We studied 185 hypertensive patients with median duration of hypertension, 10.0 years, whose BP is controlled to SBP and diastolic BP (DBP) of 139 ± 17 and 79 ± 10 mmHg, respectively. We measured cSBP and brain magnetic resonance imaging (MRI) was examined within 2 weeks after last BP and biological measurements.Results: Patients with higher-grade WMLs, as assessed by the presence of Scheltens deep white-matter hyperintensity (SDWMH) in the frontal (grade 0–2 vs 3–6) and parietal areas (grade 0–2 vs 3–6) where small arteries are affected at earlier stage of hypertension, as well as that of Fazekas deep white-matter hyperintensity (FDWMH) (grade 2–3 vs 0–1) and Fazekas periventricular hyperintensity (FPVH) (grade 1–3 vs 0) were older, had higher serum creatinine levels, a longer duration of hypertension, and lower eGFR values. The grade of the WMLs was not associated with either the cSBP or the brachial SBP. Abstract

Vibrational dynamics of dark excited states in carotenoids have been investigated using tunable Raman pump pulses. The S1 state has same vibrational dynamics in light-harvesting complex (LH1) and solution. The S* state in LH1 has similar vibrational modes with the triplet state of carotenoid. However, the so-called S* state in solution does not have the modes and is concluded to be different from the S* state in LH1.

Abstract

The most recent results and the future physics program of the high precision electron-scattering experiment at MAMI are briefly outlined. The A1 high-resolution spectrometers facility allows for a unique quality of virtual photon experiments. High precision form factor measurements, few-baryon systems highresolution structure studies and the innovative way in the search of dark photons illustrate the interplay between such diverse fields as precision atomic physics, nuclear astrophysics and astroparticle physics, where hadron physics plays a central and connecting role.

Abstract

The isoscalar giant monopole resonance (ISGMR) strength distribution in $^{24}$Mg has been determined from background-free inelastic scattering of 386-MeV $\alpha$ particles at extreme forward angles, including 0$^{\circ}$. The ISGMR strength distribution has been observed for the first time to have a two-peak structure in a light-mass nucleus. Isoscalar giant resonances and low spin states in $^{32}$S have been measured with inelastic $\alpha$ scattering at extremely forward angles including zero degrees at E$_{\alpha}$ = 386 MeV. By applying the multipole decomposition analysis, various excited states are classified according to their spin and parities (J$^{\pi}$), and are discussed in relation to the super deformed and $^{28}$Si + $\alpha$ cluster bands.

Abstract

Proton decay from the 3$\hbar\omega$ isoscalar giant dipole resonance (ISGDR) in $^{58}$Ni has been measured using the ($\alpha,\alpha'p$) reaction at a bombarding energy of 386 MeV to investigate its decay properties. We have extracted the ISGDR strength under the coincidence condition between inelastically scattered $\alpha$ particles at forward angles and decay protons emitted at backward angles. Branching ratios for proton decay to low-lying states of $^{57}$Co have been determined, and the results compared to predictions of recent continuum-RPA calculations. The final-state spectra of protons decaying to the low-lying states in $^{57}$Co were analyzed for a more detailed understanding of the structure of the ISGDR. It is found that there are differences in the structure of the ISGDR as a function of excitation energy., Comment: Minor changes after review. Accepted for publication in Phys. Rev. Abstract

The 24Mg(3He,t)24Al reaction has been studied at E(3He)=420 MeV. An energy resolution of 35 keV was achieved. Gamow-Teller strengths to discrete levels in 24Al are extracted by using a recently developed empirical relationship for the proportionality between Gamow-Teller strengths and differential cross sections at zero momentum transfer. Except for small discrepancies for a few weak excitations, good agreement with previous 24Mg(p,n) data and nuclear-structure calculations using the USDA/B interactions in the sd shell-model space is found. The excitation energy of several levels in 24Al of significance for determination of the 23Mg(p,gamma)24Al thermonuclear reaction rate were measured. Results are consistent with two of the three previous (3He,t) measurements, performed at much lower beam energies. However, a new state at Ex(24Al)=2.605(10) MeV was found and is the third state above the proton separation energy., Comment: 6 pages, 4 figures

Abstract

The Gamow-Teller strength for the transition from the ground state of 13C to the T=1/2, J^pi=3/2- excited state at 3.51 MeV in 13N is extracted via the 13C(3He,t) reaction at 420 MeV. In contrast to results from earlier (p,n) studies on 13C, a good agreement with shell-model calculations and the empirical unit cross section systematics from other nuclei is found. The results are used to study the analog 13N(e-,v_e)13C reaction, which plays a role in the pre-explosion convective phase of type Ia supernovae. Although the differences between the results from the (3He,t) and (p,n) data significantly affect the deduced electron-capture rate and the net heat-deposition in the star due to this transition, the overall effect on the pre-explosive evolution is small., Comment: 11 pages, 4 figures

Abstract

The strength distributions of the giant monopole resonance (GMR) have been measured in the even-A Sn isotopes (A=112--124) with inelastic scattering of 400-MeV $\alpha$ particles in the angular range $0^\circ$--$8.5^\circ$. We find that the experimentally-observed GMR energies of the Sn isotopes are lower than the values predicted by theoretical calculations that reproduce the GMR energies in $^{208}$Pb and $^{90}$Zr very well. Abstract

Differential cross sections for transitions of known weak strength were measured with the (3He,t) reaction at 420 MeV on targets of 12C, 13C, 18O, 26Mg, 58Ni, 60Ni, 90Zr, 118Sn, 120Sn and 208Pb. Using this data, it is shown the proportionalities between strengths and cross sections for this probe follow simple trends as a function of mass number. These trends can be used to confidently determine Gamow-Teller strength distributions in nuclei for which the proportionality cannot be calibrated via beta-decay strengths. Although theoretical calculations in distorted-wave Born approximation overestimate the data, they allow one to understand the main experimental features and to predict deviations from the simple trends observed in some of the transitions., Comment: 4 pages, 2 figures

Abstract

Charge-exchange reactions are an important tool for determining weak-interaction rates. They provide stringent tests for nuclear structure models necessary for modeling astrophysical environments such as neutron stars and core-collapse supernovae. In anticipation of (t,3He) experiments at 115 MeV/nucleon on nuclei of relevance (A~40-120) in the late evolution of stars, it is shown via a study of the 26Mg(t,3He) reaction that this probe is an accurate tool for extracting Gamow-Teller transition strengths. To do so, the data are complemented by results from the 26Mg(3He,t) reaction at 140 MeV/nucleon which allows for a comparison of T=2 analog states excited via the mirror reactions. Extracted Gamow-Teller strengths from 26Mg(t,3He) and 26Mg(3He,t) are compared with those from 26Mg(d,2He) and 26Mg(p,n) studies, respectively. A good correspondence is found, indicating probe-independence of the strength extraction. Furthermore, we test shell-model calculations using the new USD-05B interaction in the sd-model space and show that it reproduces the experimental Gamow-Teller strength distributions well. A second goal of this work is to improve the understanding of the (t,3He) and (3He,t) reaction mechanisms at intermediate energies since detailed studies are scarce. The Distorted-Wave Born Approximation is employed, taking into account the composite structures of the 3He and triton particles. The reaction model provides the means to explain systematic uncertainties at the 10-20% level in the extraction of Gamow-Teller strengths as being due to interference between Gamow-Teller dL=0, dS=1 and dL=2, dS=1 amplitudes that both contribute to transitions from 0+ to 1+ states., Comment: 16 pages, 6 figures

Abstract

Background Large multicenter studies reporting on the association between the duration of broad-spectrum antimicrobial administration and the detection of multidrug-resistant (MDR) bacteria in the intensive care unit (ICU) are scarce. We evaluated the impact of broad-spectrum antimicrobial therapy for more than 72 h on the detection of MDR bacteria using the data from Japanese patients enrolled in the DIANA study. Methods We analyzed the data of ICU patients in the DIANA study (a multicenter international observational cohort study from Japan). Patients who received empirical antimicrobials were divided into a broad-spectrum antimicrobial group and a narrow-spectrum antimicrobial group, based on whether they received broad-spectrum antimicrobials for more or less than 72 h, respectively. Differences in patient characteristics, background of infectious diseases and empirical antimicrobial administration, and outcomes between the two groups were compared using the chi-square tests (Monte Carlo method) for categorical variables and the Mann–Whitney U-test for continuous variables. We also conducted a logistic regression analysis to investigate the factors associated with the detection of new MDR bacteria. Results A total of 254 patients from 31 Japanese ICUs were included in the analysis, of whom 159 (62.6%) were included in the broad-spectrum antimicrobial group and 95 (37.4%) were included in the narrow-spectrum antimicrobial group. The detection of new MDR bacteria was significantly higher in the broad-spectrum antimicrobial group (11.9% vs. 4.2%, p = 0.042). Logistic regression showed that broad-spectrum antimicrobial continuation for more than 72 h (OR [odds ratio] 3.09, p = 0.047) and cerebrovascular comorbidity on ICU admission (OR 2.91, p = 0.041) were associated with the detection of new MDR bacteria. Conclusions Among Japanese ICU patients treated with empirical antimicrobials, broad-spectrum antimicrobial usage for more than 72 h was associated with the increased detection of new MDR bacteria. Antimicrobial stewardship programs in ICUs should discourage the prolonged use of empirical broad-spectrum antimicrobial therapy. Abstract

Regulatory T cells (Tregs) are essential to homeostatic immune tolerance. Clinical trials of autologous Treg cell therapy have demonstrated promising preliminary results, but there remain challenges to overcome including the problem of recipient-dependent expansion variability. Enhancing the suppressive function of expanded Tregs to reduce the required Treg dose would present a potential solution to this problem. Cellular metabolism regulates immune cells through two interacting aspects: bioenergic and biosynthetic activities, and non-canonical activities of metabolic enzymes and intermediates. Early in vitro work has revealed distinct metabolic profiles between mouse conventional T cells (Tconvs) and Tregs. However, it remains unclear how the metabolism of human Tregs is regulated. A better understanding of the contrasting metabolic demands and regulation of function between human Tconvs and Tregs may uncover routes to target Treg-specific metabolic pathways in order to tune their activity as desired. In this work, the immunometabolism of human Treg is investigated and new strategies revealed to harness elements of their metabolism for therapeutic use. In Chapters 3 and 4, I show that Treg display the ‘Warburg effect’ after T cell receptor stimulation alongside CD28 co-stimulation via the mTOR signalling pathway. Subsequently, I show that only naïve and central memory Tregs preferentially utilise glucose; suggesting that human Treg metabolism may differ due to their anatomical location. In contrast, effector memory Tregs and Tconvs have distinct metabolic characteristics: whilst Tconvs negatively alter effector function through limiting glucose metabolism, Tregs positively regulate their suppressive function by limiting mitochondrial energy metabolism. Furthermore, this metabolic regulation allows Tregs to enhance their suppressive activity towards Treg-resistant Tconvs which are characterised by their low mitochondrial mass. In Chapters 5 and 6, I investigate the mechanism of this phenomenon, and show that Tregs generate immunosuppressive extracellular vesicles (exosomes) by mitochondrial ATP synthase inhibition. Metabolomics data suggests that epigenetic alteration in Tregs through a change in mitochondrial metabolism intermediates. The donor-independent function of these small extracellular vesicles suggests a new potential therapeutic strategy in which Treg-derived extracellular vesicles could be used ‘off-the-shelf’. The immunometabolic experiments presented in this thesis shine new light on the differential metabolic regulation of human Tregs and Tconvs, and reveal novel and potent mode of Treg immunosuppresive action that can be induced by modulating metabolism. By defining these pathways, new therapeutic strategies to immune regulation may be potentially revealed.

Abstract

Background During the COVID-19 pandemic, decreased volumes of stroke admissions and mechanical thrombectomy were reported. The study's objective was to examine whether subarachnoid haemorrhage (SAH) hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines. Methods We conducted a cross-sectional, retrospective, observational study across 6 continents, 37 countries and 140 comprehensive stroke centres. Patients with the diagnosis of SAH, aneurysmal SAH, ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases, 10th Revision, codes. The 3-month cumulative volume, monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before (1 year and immediately before) and during the pandemic, defined as 1 March-31 May 2020. The prior 1-year control period (1 March-31 May 2019) was obtained to account for seasonal variation. Abstract

Background: The COVID-19 pandemic led to profound changes in the organization of health care systems worldwide. Aims: We sought to measure the global impact of the COVID-19 pandemic on the volumes for mechanical thrombectomy, stroke, and intracranial hemorrhage hospitalizations over a three-month period at the height of the pandemic (1 March–31 May 2020) compared with two control three-month periods (immediately preceding and one year prior). Methods: Retrospective, observational, international study, across 6 continents, 40 countries, and 187 comprehensive stroke centers. The diagnoses were identified by their ICD-10 codes and/or classifications in stroke databases at participating centers. Results: The hospitalization volumes for any stroke, intracranial hemorrhage, and mechanical thrombectomy were 26,699, 4002, and 5191 in the three months immediately before versus 21,576, 3540, and 4533 during the first three pandemic months, representing declines of 19.2% (95%CI, −19.7 to −18.7), 11.5% (95%CI, −12.6 to −10.6), and 12.7% (95%CI, −13.6 to −11.8), respectively. The decreases were noted across centers with high, mid, and low COVID-19 hospitalization burden, and also across high, mid, and low volume stroke/mechanical thrombectomy centers. High-volume COVID-19 centers (−20.5%) had greater declines in mechanical thrombectomy volumes than mid- (−10.1%) and low-volume (−8.7%) centers (p < 0.0001). There was a 1.5% stroke rate across 54,366 COVID-19 hospitalizations. SARS-CoV-2 infection was noted in 3.9% (784/20,250) of all stroke admissions. Conclusion: The COVID-19 pandemic was associated with a global decline in the volume of overall stroke hospitalizations, mechanical thrombectomy procedures, and intracranial hemorrhage admission volumes. Despite geographic variations, these volume reductions were observed regardless of COVID-19 hospitalization burden and pre-pandemic stroke/mechanical thrombectomy volumes. © 2021 World Stroke Organization.

Abstract

OBJECTIVE: To measure the global impact of COVID-19 pandemic on volumes of IV thrombolysis (IVT), IVT transfers, and stroke hospitalizations over 4 months at the height of the pandemic (March 1 to June 30, 2020) compared with 2 control 4-month periods. METHODS: We conducted a cross-sectional, observational, retrospective study across 6 continents, 70 countries, and 457 stroke centers. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases. RESULTS: There were 91,373 stroke admissions in the 4 months immediately before compared to 80,894 admissions during the pandemic months, representing an 11.5% (95% confidence interval [CI] -11.7 to -11.3, p < 0.0001) decline. There were 13,334 IVT therapies in the 4 months preceding compared to 11,570 procedures during the pandemic, representing a 13.2% (95% CI -13.8 to -12.7, p < 0.0001) drop. Interfacility IVT transfers decreased from 1,337 to 1,178, or an 11.9% decrease (95% CI -13.7 to -10.3, p = 0.001). Recovery of stroke hospitalization volume (9.5%, 95% CI 9.2-9.8, p < 0.0001) was noted over the 2 later (May, June) vs the 2 earlier (March, April) pandemic months. There was a 1.48% stroke rate across 119,967 COVID-19 hospitalizations. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was noted in 3.3% (1,722/52,026) of all stroke admissions. CONCLUSIONS: The COVID-19 pandemic was associated with a global decline in the volume of stroke hospitalizations, IVT, and interfacility IVT transfers. Primary stroke centers and centers with higher COVID-19 inpatient volumes experienced steeper declines. Recovery of stroke hospitalization was noted in the later pandemic months. © 2021 American Academy of Neurology.
