Bakgrund: Ohälsosamma levnadsvanor såsom fysisk inaktivitet och ohälsosamma kostvanor leder till ökad risk för sjukdom vilket kan innebära en stor belastning för individen, hälso- och sjukvården samt för samhället. Hälso- och sjukvården ska vägleda individen till att främja en hälsosam livsstil, dock ställer det krav på att individen själv utför förändringsarbetet. Det finns idag ett brett utbud av hälsoapplikationer med olika hälsofrämjande inriktningar som används som verktyg för att stödja/vägleda individen till en förbättrad hälsa. Idag finns inga riktlinjer för hälso-och sjukvårdspersonal gällande rekommendationer till patienter avseende adekvata hälsoapplikationer. Syfte: Syftet var att undersöka om personer med hjälp av en mobilapplikation avsedd för en specifik hälsofrämjande åtgärd, tex. viktnedgång, erhåller ett annat resultat jämfört med personer som genomgår samma livsstilsåtgärd men som ej har mobilapplikationen som stöd. Ett yttligare syfte var att undersöka mobilapplikationernas huvudsakliga funktion. Metod: För att besvara litteraturöversiktens syfte genomfördes databassökningar där studier av RCT design inkluderades, totalt inkluderades 18 studier. Samtliga studier kvalitetsgranskades med hjälp av ett vedertaget granskningsprotokoll. Resultat: Mobilapplikationernas effekt på livsstilsförändringar varierade, en del studier påvisade förbättrade kliniska resultat medan andra studier inte påvisade någon effekt, detta oavsett vilken livsstilsförändring som undersöktes. Majoriteten av studierna behandlade livsstilsförändringar avseende fysisk aktivitet, kosthållning samt diabetes. Mobilapplikationernas huvudsakliga funktion var självmonitorering, feedback och målsättning samt information/kunskap. Slutsats: Mobilapplikationers effektivitet tycks bero på personens individuella förutsättningar och kontext vilket kan vara avgörande för resultatet. Mobilapplikationer som verktyg i en livsstilsförändring har potential att i framtiden kunna vara effektiva, dock krävs mer forskning. Background: Unhealthy living habits such as physical inactivity and unhealthy eating habits lead to an increased risk of disease, which can pose a major burden for individuals, health care and society. Healthcare providers should assist the individual to promote a healthy lifestyle, but it requires that the individual himself performs this change. Today there ́s a wide range of health promotion applications used as tools to support/guide the individual to an improved health. At present there are no gudielines for healthcare professionals regarding recommendations to the patient regarding adequate health-applications. Objective: The purpose was to investigate whether persons using a mobile application intended for a specific health promotion measure, eg. weight loss, would have different results compared to people undergoing the same lifestyle change but who do not have the mobile application as support. A further purpose was to investigate the main function of mobile applications. Method: Database searches were conducted, studies of RCT design were included, in total, 18 studies were included. All studies were evaluated regarding the studies quality using a approved audit protocol. Results: The effects of the mobile applications on lifestyle changes varied, some studies showed improved clinical outcomes, while other studies did not show any effect, regardless of the lifestyle change investigated. The majority of the studies involved lifestyle changes concerning physical activity, diet and diabetes. The main features of the apps were self- monitoring, feedback and goalsetting, as well as information / knowledge domain. Conclusion: The effectiveness of mobile applications seams to depend on the individuals conditions and context, which may be crucial to the outcome. Applications as tools in a lifestyle change have the potential to be effective in the future, but further reaserch is required.

Syftet med studien är att beskriva och analysera lärares subjektiva upplevelser och erfarenheter av det sociala arbete som de utför. Följande frågeställningar har formulerats; Upplever lärarna att de utför socialt arbete i förhållande till sin yrkesroll? Vilka delar av sitt arbete ser de som socialt arbete? Hur upplever lärarna sitt handlingsutrymme för att kunna utföra socialt arbete i sin yrkesroll? Vad är lärares uppfattning kring huruvida socialt arbete ska ingå i yrkesrollen eller inte? Studien bygger på en kvalitativ ansats med semistrukturerade kvalitativa intervjuer som metod. Det empiriska materialet analyserades med hjälp av organisationsteorins organisationskultur samt handlingsutrymme. 7 grundskollärare från olika kommunala skolor i Skåne har intervjuats om sina subjektiva upplevelser av det utförda sociala arbetet. Resultatet av studien visar att för samtliga intervjupersoner utgör det sociala arbetet en central del av det vardagliga praktiska arbetet. De beskriver hur det praktiska kunskapsutlärandet inte är genomförbart utan att inkludera socialt arbete. Lärarens berättelser om det sociala arbetet i yrkesrollen handlar om att bygga relationer, att lära eleverna att samspela med andra samt hur de arbetar förebyggande. The purpose of this study is to describe and analyze teacher’s subjective experience of the social work they conduct. The following questions have been formulated: Do the teachers experience that they conduct social work in relation to their professional role? What parts of their work do they recognize as social work? How do teachers experience their room of maneuver to be able to conduct social work in their professional role? What are the teacher’s opinions of whether social work should be included in the professional role or not? This study is based on a qualitative approach with semi-structured qualitative interviews as a method. The empirical data were analyzed with the help of organizational theory, organizational culture, and the room of maneuver. 7 primary school teachers from various municipal schools in Skåne (Scania) were interviewed about their subjective experiences of the performed social work. The result of the study shows that for all the interviewees, social work is a central part of everyday practical work. They describe how the practical teaching of knowledge is not feasible without including social work. The teachers’ accounts of the social work within their professional role depict to build relations, to teach the students how to interact with others and how they work preventively.

Syftet med denna studie är att utifrån en fenomenologisk ansats öka förståelsen för hur säljare av komplexa IT-lösningar upplever flytande arbetstider. Detta gör vi genom vår frågeställning: Hur upplever säljare av komplexa IT-lösningar att vara tillgängliga? För att få en djupare insikt i ämnet har vi genomfört semistrukturerade intervjuer med åtta respondenter på sju olika företag inom IT-branschen. Den teoretiska utgångspunkt vi använt oss av i denna studie är Bourdieus teori om habitus och Goffmans teori om roller. Resultatet från insamlingen av vårt empiriska material visar att det hos IT-säljare verkar råda ett gemensamt habitus utifrån hur de beskriver sig själva, vilka behov de får tillgodosedda genom de krav yrket ställer på dem samt hur de balanserar sina roller mellan arbete och familjeliv. De roller IT-säljaren spelar i arbetslivet kontra familjelivet verkar vara integrerade med varandra och utifrån vårt empiriska resultat drar vi slutsatsen att fenomenet tillgänglighet fyller en meningsskapande funktion som genom socialiseringsprocessen inom säljteamet också förstärker IT-säljarens habitus. På så sätt blir tillgänglighet en del i deras sätt att leva vilket gör att de inte upplever denna på ett negativt sätt till skillnad från hur individer med ett annat habitus kan tänkas uppleva tillgänglighet. Vår studie har således bidragit till forskningsfältet på det sätt att den visat att säljare av komplexa IT-lösningar inte upplever den stress gällande tillgänglighet som tidigare forskning generellt visat.

Intestinal ischemia-reperfusion (IR)-induced damage requires complement receptor 2 (CR2) for generation of the appropriate natural Ab repertoire. Pathogenic Abs recognize neoantigens on the ischemic tissue, activate complement, and induce intestinal damage. Because C3 cleavage products act as ligands for CR2, we hypothesized that CR2(hi) marginal zone B cells (MZBs) require C3 for generation of the pathogenic Abs. To explore the ability of splenic CR2(+) B cells to generate the damaging Ab repertoire, we adoptively transferred either MZBs or follicular B cells (FOBs) from C57BL/6 or Cr2(-/-) mice into Rag-1(-/-) mice. Adoptive transfer of wild type CR2(hi) MZBs but not CR2(lo) FOBs induced significant damage, C3 deposition, and inflammation in response to IR. In contrast, similarly treated Rag-1(-/-) mice reconstituted with either Cr2(-/-) MZB/B1 B cells (B1Bs) or FOBs lacked significant intestinal damage and displayed limited complement activation. To determine whether C3 cleavage products are critical in CR2-dependent Ab production, we evaluated the ability of the natural Ab repertoire of C3(-/-) mice to induce damage in response to IR. Infusion of C3(-/-) serum into Cr2(-/-) mice restored IR-induced tissue damage. Furthermore, Rag-1(-/-) mice sustained significant damage after infusion of Abs from C3(-/-) but not Cr2(-/-) mice. Finally, adoptive transfer of MZBs from C3(-/-) mice into Rag-1(-/-) mice resulted in significant tissue damage and inflammation. These data indicate that CR2 expression on MZBs is sufficient to induce the appropriate Abs required for IR-induced tissue damage and that C3 is not critical for generation of the pathogenic Abs.

Reperfusion of ischemic tissue induces significant tissue damage in multiple conditions, including myocardial infarctions, stroke and transplantation. Although not as common, the mortality rate of mesenteric ischemia/reperfusion (IR) remains over 70%. Although complement and naturally occurring antibodies are known to mediate significant damage during IR, the target antigens are intracellular molecules. We investigated the role of the serum protein, β2-glycoprotein I as an initiating antigen for antibody recognition and β2-GPI peptides as a therapeutic for mesenteric IR. The time course of β2-GPI binding to the tissue indicated binding and complement activation within 15 min post-reperfusion. Treatment of wildtype mice with peptides corresponding to the lipid binding domain V of β2-GPI blocked intestinal injury and inflammation, including cellular influx and cytokine and eicosanoid production. The optimal therapeutic peptide (peptide 296) contained the lysine rich region of domain V. In addition, damage and most inflammation was also blocked by peptide 305 which overlaps with peptide 296 but does not contain the lysine rich, phospholipid binding region. Importantly, peptide 296 retained efficacy after replacement of Cys residues with Ser. In addition, infusion of wildtype serum containing reduced levels of anti-β2-GPI antibodies into Rag-1−/− mice prevented IR-induced intestinal damage and inflammation. Together these data suggest that the serum protein,β2-GPI initiates the IR-induced intestinal damage and inflammatory response and as such is a critical therapeutic target for IR-induced damage and inflammation. Syftet med föreliggande litteraturstudie var att beskriva inom vilka områden motiverande samtal kan användas av personal inom hälso- och sjukvård i behandlingen av personer med övervikt eller fetma. Vidare var syftet att beskriva vilka förmågor personalen som använder motiverande samtal bör inneha och vilken inverkan motiverande samtal har för personer med övervikt eller fetma som avser genomgå livsstilsförändringar. Litteratur söktes i databaserna Medline via PubMed samt Cinahl, och samtliga 12 studier som utgör resultatet var från åren 2005-2010. Huvudresultatet i studien visade att hälso- och sjukvårdspersonal använder motiverande samtal till att implementera förändringar gällande kost, fysisk aktivitet, motivation samt tilltro till dessa i arbetet med personer med övervikt och fetma. Motiverande samtal kunde stärka dessa förändringar och det visade sig även vara ett bra komplement till beteendeinterventioner. Förmågor hos hälso- och sjukvårdspersonal, såsom empati, hade betydelse för hur personers livsstilsförändringar implementerades. Konklusionen var att personer med övervikt och fetma kan genomgå livsstilsförändringar, såsom förbättrad kosthållning samt ökad fysisk aktivitet, som främjar hälsa och leder till viktnedgång med hjälp av motiverande samtal. The aim of this study was to describe the areas in which motivational interviewing can be used by health care personnel in the treatment of persons with overweight or obesity. Furthermore, the aim was to describe what abilities the personnel using motivational interviewing should obtain and what impact motivational interviewing has for persons with overweight or obesity about to undergo lifestyle changes. Literature was searched in Medline through PubMed and Cinahl, and all 12 studies which represent the result were from year 2005-2010. The main result of the study showed that health care personnel are using motivational interviewing to implement changes in terms of diet, physical activity, motivation and belief in these in the work with persons with overweight and obesity. Motivational interviewing could strengthen these changes and is also proved to be a good supplement to behavioral interventions. Health care personnel abilities, such as empathy, had a meaning for how persons lifestyle changes were implemented. The conclusion was that persons with overweight and obesity could undergo lifestyle changes, such as improved diet and increased physical activity, which promote health and lead to weight loss through motivational interviewing.
