Abstract There is a worrying scarcity of drug options for patients with severe COVID-19. Glycine possesses anti-inflammatory, cytoprotective, endothelium-protective, and platelet-antiaggregant properties, so its use in these patients seems promising. In this open label, controlled clinical trial, inpatients with severe COVID-19 requiring mechanical ventilation randomly received usual care (control group) or usual care plus 0.5 g/kg/day glycine by the enteral route (experimental group). Major outcomes included mortality, time to weaning from mechanical ventilation, total time on mechanical ventilation, and time from study recruitment to death. Secondary outcomes included laboratory tests and serum cytokines. Patients from experimental (n = 33) and control groups (n = 23) did not differ in basal characteristics. There were no differences in mortality (glycine group, 63.6% vs control group, 52.2%, p = 0.60) nor in any other major outcome. Abstract Pulmonary mechanics has been traditionally viewed as determined by lung size and physical factors such as frictional forces and tissue viscoelastic properties, but few information exists regarding potential influences of cytokines and hormones on lung function. Concentrations of 28 cytokines and hormones were measured in saliva from clinically healthy scholar children, purposely selected to include a wide range of body mass index (BMI). Lung function was assessed by impulse oscillometry, spirometry, and diffusing capacity for carbon monoxide, and expressed as z‐score or percent predicted. Ninety‐six scholar children (55.2% female) were enrolled. Bivariate analysis showed that almost all lung function variables correlated with one or more cytokine or hormone, mainly in boys, but only some of them remained statistically significant in the multiple regression analyses. Thus, after adjusting by height, age, and BMI, salivary concentrations of granulocyte‐macrophage colony‐stimulating factor (GM‐CSF) in boys were associated with zR5‐R20 and reactance parameters (zX20, zFres, and zAX), while glucagon inversely correlated with resistances (zR5 and zR20). Thus, in physiological conditions, part of the mechanics of breathing might be influenced by some cytokines and hormones, including glucagon and GM‐CSF. This endogenous influence is a novel concept that warrants in‐depth characterization.

Hypoxia and hypoxia-inducible factors (HIFs) are essential in regulating several cellular processes, such as survival, differentiation, and the cell cycle; this adaptation is orchestrated in a complex way. In this review, we focused on the impact of hypoxia in the physiopathology of idiopathic pulmonary fibrosis (IPF) related to lung development, regeneration, and repair. There is robust evidence that the responses of HIF-1α and -2α differ; HIF-1α participates mainly in the acute phase of the response to hypoxia, and HIF-2α in the chronic phase. The analysis of their structure and of different studies showed a high specificity according to the tissue and the process involved. We propose that hypoxia-inducible transcription factor 2a (HIF-2α) is part of the persistent aberrant regeneration associated with developing IPF.

Background: Infection by SARS-CoV-2 has been associated with multiple symptoms; however, still, little is known about persistent symptoms and their probable association with the risk of developing pulmonary fibrosis in patients post-COVID-19. Methods: A longitudinal prospective study on health workers infected by SARS-CoV-2 was conducted. In this work, signs and symptoms were recorded of 149 health workers with a positive PCR test for SARS-CoV-2 at the beginning of the diagnosis, during the active infection, and during post-COVID-19 follow-up. The McNemar chi-square test was used to compare the proportions and percentages of symptoms between the baseline and each follow-up period. Results: The signs and symptoms after follow-up were cardiorespiratory, neurological, and inflammatory. Gastrointestinal symptoms were unusual at the disease onset, but unexpectedly, their frequency was higher in the post-infection stage. The multivariate analysis showed that pneumonia (HR 2.4, IC95%: 1.5–3.8, p < 0.001) and positive PCR tests still after four weeks (HR 5.3, IC95%: 2.3-12.3, p < 0.001) were factors associated with the diagnosis of post-COVID-19 pulmonary fibrosis in this study group. Conclusions: Our results showed that pneumonia and virus infection persistence were risk factors for developing pulmonary fibrosis post-COVID-19, after months of initial infection.

The cholinergic system is present in both bacteria and mammals and regulates inflammation during bacterial respiratory infections through neuronal and non-neuronal production of acetylcholine (ACh) and its receptors. However, the presence of this system during the immunopathogenesis of pulmonary tuberculosis (TB) in vivo and in its causative agent Mycobacterium tuberculosis (Mtb) has not been studied. Therefore, we used an experimental model of progressive pulmonary TB in BALB/c mice to quantify pulmonary ACh using high-performance liquid chromatography during the course of the disease. In addition, we performed immunohistochemistry in lung tissue to determine the cellular expression of cholinergic system components, and then administered nicotinic receptor (nAChR) antagonists to validate their effect on lung bacterial burden, inflammation, and pro-inflammatory cytokines. Finally, we subjected Mtb cultures to colorimetric analysis to reveal the production of ACh and the effect of ACh and nAChR antagonists on Mtb growth. Our results show high concentrations of ACh and expression of its synthesizing enzyme choline acetyltransferase (ChAT) during early infection in lung epithelial cells and macrophages. During late progressive TB, lung ACh upregulation was even higher and coincided with ChAT and α7 nAChR subunit expression in immune cells. Moreover, the administration of nAChR antagonists increased pro-inflammatory cytokines, reduced bacillary loads and synergized with antibiotic therapy in multidrug resistant TB. Finally, in vitro studies revealed that the bacteria is capable of producing nanomolar concentrations of ACh in liquid culture. In addition, the administration of ACh and nicotinic antagonists to Mtb cultures induced or inhibited bacterial proliferation, respectively. These results suggest that Mtb possesses a cholinergic system and upregulates the lung non-neuronal cholinergic system, particularly during late progressive TB. The upregulation of the cholinergic system during infection could aid both bacterial growth and immunomodulation within the lung to favor disease progression. Furthermore, the therapeutic efficacy of modulating this system suggests that it could be a target for treating the disease.

Abstract Background Patients with cystic fibrosis (CF) have airway inflammation that contributes to symptoms and to pulmonary function derangement. Current drugs used to diminish airway inflammation improve the clinical and spirometric status of patients with CF, but their use is limited due to their undesired side effects, for example, glucose intolerance, growth retardation, and cataracts with corticosteroids, gastrointestinal toxicity with ibuprofen, and macrolide resistance with azythromycin. Glycine is known to decrease activation of inflammatory cells, including alveolar macrophages and neutrophils, and is relatively inexpensive, palatable, and virtually devoid of untoward effects. These features make glycine a good candidate for antiinflammatory treatment of CF. Thus, we aimed to explore whether glycine can exert a beneficial effect in a population of patients with CF. Methods This was a randomized, double blinded, cross-over pilot clinical trial. Subjects with CF received, in random order, oral glycine (0.5 g/kg/day, dissolved in any liquid) and placebo (glass sugar), each during 8 weeks with an intermediate 2-week wash-out period. Results Thirteen subjects aged 6–23 years, 8 females, completed the two arms of the study. As compared with placebo, after glycine intake patients had better symptom questionnaire scores (p = 0.02), mainly regarding sputum features and dyspnea. While spirometric variables tended to decline during placebo intake, they remained stable or even increased during glycine treatment (p = 0.04 to p = 0.003). In this context, FEV1 declined 8.6% after placebo and increased 9.7% at the end of the glycine period. Pulse oximetry improved after glycine intake (p = 0.04 vs. placebo). TNF-α in serum and IL-6 and G-CSF in sputum tended to decline at the end of the glycine period (p = 0.061, p = 0.068 and p = 0.04, respectively, vs placebo). Glycine was remarkably well tolerated. Conclusions The clinical, spirometric and inflammatory status of subjects with CF improved after just 8 weeks of glycine intake, suggesting that this amino acid might constitute a novel therapeutic tool for these patients. Con el propósito de disponer de información sobre los riesgos para infección por TB, la Dirección General de Epidemiología del Ministerio de Salud, desarrolló una vigilancia centinela en establecimientos de salud de la Provincia constitucional del Callao, dicha vigilancia incluyó el diagnóstico de infección tuberculosa latente (ITL) mediante la aplicación del método IGRA. El objetivo del presente estudio fue estimar la prevalencia de ITL en trabajadores de salud de un área con alta carga de enfermedad de tuberculosis. La prevalencia de ITL en trabajadores de salud fue 56%. En trabajadores con más de 10 años de servicio la prevalencia se incrementó a 63% y en trabajadores con más de 35 años de servicio se encontraron prevalencias entre 58 y 60%. Existe una alta prevalencia de ITL en trabajadores de salud de establecimientos del primer nivel de atención, identificándose al mayor tiempo de servicio, como uno de los principales factores de riesgo.

Introducción. La resección hepática sigue siendo la única opción de tratamiento con intención curativa y con posibilidad de supervivencia a largo plazo. El objetivo del estudio fue evaluar los resultados de la resección hepática en un grupo de pacientes. Métodos. Se llevó a cabo un estudio descriptivo retrospectivo de una serie consecutiva de 97 hepatectomías por metástasis de cáncer colorrectal, en el período comprendido entre el 15 de junio de 2005 y el 30 diciembre de 2015, en el Hospital Pablo Tobón Uribe de Medellín. Se hizo análisis bivariado de los factores asociados a la supervivencia. Resultados. De 370 hepatectomías, se practicaron 97 resecciones hepáticas en 93 pacientes por metástasis de cáncer colorrectal. El promedio de edad fue de 58 años, y el 50,5 % fueron resecciones hepáticas mayores. La mediana del volumen de sangrado intraoperatorio fue de 200 ml (rango intercuartílico, RIQ: 100 a 400 ml) y la mediana del tiempo quirúrgico fue de 150 minutos (RIQ: 120 a 180 minutos). La estancia hospitalaria tuvo una mediana de 3 días (RIQ: 2 a 4 días). Solo se trasladó a la unidad de cuidados intensivos el 25,9 % de los pacientes; se presentaron complicaciones en 21,5 % de los pacientes. La mortalidad perioperatoria a 90 días fue de 2,06 %. Se cumplió con el protocolo quirúrgico de recuperación rápida (Fast Track Surgery) en 54,8 % de los pacientes. La supervivencia global a cinco años fue de 34,5 % y, a 100 meses, de 23 %. Debido a que no se encontró significancia estadística con ninguna de las variables en el análisis bivariado para supervivencia, no se hizo un análisis multivariado. Conclusiones. La resección hepática en pacientes con metástasis de cáncer colorrectal se puede practicar en nuestro medio de manera segura, con morbilidad, mortalidad y resultado oncológico comparables a los de otras series del mundo.

Objetivo. Evaluar las indicaciones, resultados y supervivencia de los pacientes sometidos a duodenopancreatectomía cefálica en el Hospital Pablo Tobón Uribe de Medellín. Pacientes y métodos. Se incluyeron todos los pacientes sometidos a duodenopancreatectomía cefálica (operación de Whipple), desde abril de 2004 hasta abril de 2011. La información se obtuvo de la base de datos prospectiva de la Unidad de Cirugía Hepato-Biliar y Pancreática del Hospital Pablo Tobón Uribe. Resultados. Se operaron 68 pacientes, con una edad promedio de 59 años (rango, 16-82) y 50 % eran hombres. La principal indicación de la cirugía fue neoplasia maligna (97 %), con la siguiente distribución: adenocarcinoma pancreático, 25 pacientes (36,7 %); carcinoma ampular, 23 (33,8 %); colangiocarcinoma distal, 8 (11,7 %) cáncer de duodeno, 4 (5,8 %), y 8 (11,7 %) tuvieron otras indicaciones. El sangrado operatorio en promedio fue de 455 ml (rango, 200 a 2.000 ml); 5,8 % de los pacientes sangraron 1.000 ml o más; en 35,2 % de los casos se requirió transfusión de glóbulos rojos, con un rango entre 1 y 8 unidades; 5,8 % de los pacientes requirió cinco o más unidades de glóbulos rojos. El rango de estancia en la unidad de cuidados intensivos fue 1 a 20 días, con un promedio de 2 días; 45,5 % de los pacientes estuvo en la unidad de cuidados intensivos un solo día. Se presentaron complicaciones en el 33,8 % de los pacientes, y las más comunes fueron atonía gástrica, infección distante al sitio operatorio, infección del sitio operatorio y sangrado posoperatorio. La mortalidad posoperatoria a 30 días fue de 14,7 % en toda la serie; al hacer un análisis del último grupo de 25 pacientes, la cifra bajó a 8 %, con una supervivencia actuarial de toda la serie de pacientes de 54,4 %. Conclusión. En nuestro hospital, la duodenopancreatectomía cefálica sigue teniendo alta mortalidad en comparación con algunos centros de referencia en el mundo, aunque es evidente que ha mejorado en los últimos años; las cifras de morbilidad están acordes a las reportadas en diferentes series mundiales de excelencia.Introduction: Objective: To evaluate the indications, results and survival of patients undergoing pancreatoduodenectomy at Hospital Pablo Tobon Uribe of Medellin. Materials and methods: We collected all patients that underwent pancreatoduodenectomy (Whipple operation), from April 2004 to April 2011. The information was obtained from a prospective database from the Hepato-Biliary and Pancreatic Unit of Hospital Pablo Tobón Uribe. Results: 68 patients were operated in total, with an average age of 59 years (16-82), 50% were men; the main indication for surgery was malignant disease (97%), with the following distribution: 25 patients with pancreatic adenocarcinoma ( 36.7%), 23 patients with ampullary carcinoma (33.8%), 8 with distal cholangiocarcinoma (11.7%), 4 patients with duodenal cancer (5.8%) and 11.7% with other indications. Average bleeding at surgery was 455 ml (200-2000ml), 5.8% of patients bled 1000 ml or more, 35.2% of cases required transfusion of red blood cells, with a range of 1-8 units, 5.8% of patients required 5 or more units of red cells. The range of ICU stay was 1-20 days, with an average of 2 days, 45.5% of these patients just stayed one day. Complications occurred in 33.8% of patients, the most common was gastric atony, distant infection and postoperative bleeding. Postoperative mortality at 30 days was 14.7% in the whole series, with an analysis of the last group of 25 patients, the number dropped to 8%, with an actuarial survival of the entire series of 54.4%. Conclusions: In our hospital, pancreatoduodenectomy mortality is still high compared to some reference centers in the world, but clearly has improved in recent years, morbidity results are consistent with those reported in centers of excellence around the world.

Introducción. La pancreaticoduodenectomía es una cirugía compleja con alta morbilidad y una mortalidad que ha venido disminuyendo en las últimas décadas. El objetivo del estudio fue determinar los factores asociados a la mortalidad posoperatoria temprana de los pacientes sometidos a cirugía de Whipple. Materiales y métodos. De la base de datos prospectiva de la Unidad de Cirugía Hepatobiliar y Pancreática, se incluyeron todos los pacientes en quienes se practicó la cirugía de Whipple en el Hospital Pablo Tobón Uribe de Medellín, en el periodo comprendido entre junio de 2004 y junio de 2013. Se hizo el análisis bivariado y multivariado de los factores asociados a la mortalidad posoperatoria temprana, definida como la muerte durante los primeros 30 días del periodo posoperatorio. Resultados. Se practicaron 132 cirugías de Whipple. Como factores asociados a mortalidad posoperatoria a 30 días, con significancia estadística en el análisis bivariado, se encontraron: sangrado operatorio (p=0,014), pH intraoperatorio (p=0,006), amilasa en el drenaje en el primer día posoperatorio (p=0,012) y cuarto día posperatorio (p=0,023), y proteína C reactiva al quinto día posoperatorio (p=0,036). En el análisis multivariado se encontró el sangrado mayor de 300 ml, bicarbonato de menos de 22,2 mEq/L en el día 1 posoperatorio, pH intraoperatorio menor de 7,34 y amilasa en el drenaje en el primer día posoperatorio menor de 2.500 U/ml, como los factores asociados a mortalidad temprana, aunque sin lograr significancia estadística. Discusión. El seguimiento y análisis riguroso de la evolución de los pacientes sometidos a pancreaticoduo- denectomía, permite establecer que en nuestro medio se pueden conseguir resultados comparables a los de otras series en centros especializados del mundo e identificar factores asociados a la mortalidad posoperatoria para detectar aspectos en los cuales la calidad de la atención médica puede ser mejorada.

BACKGROUND: We describe the temporal variation in viral agents detected in influenza like illness (ILI) patients before and after the appearance of the ongoing pandemic influenza A (H1N1) (pH1N1) in Peru between 4-January and 13-July 2009. METHODS: At the health centers, one oropharyngeal swab was obtained for viral isolation. From epidemiological week (EW) 1 to 18, at the US Naval Medical Research Center Detachment (NMRCD) in Lima, the specimens were inoculated into four cell lines for virus isolation. In addition, from EW 19 to 28, the specimens were also analyzed by real time-polymerase-chain-reaction (rRT-PCR). RESULTS: We enrolled 2,872 patients: 1,422 cases before the appearance of the pH1N1 virus, and 1,450 during the pandemic. Non-pH1N1 influenza A virus was the predominant viral strain circulating in Peru through (EW) 18, representing 57.8% of the confirmed cases; however, this predominance shifted to pH1N1 (51.5%) from EW 19-28. During this study period, most of pH1N1 cases were diagnosed in the capital city (Lima) followed by other cities including Cusco and Trujillo. In contrast, novel influenza cases were essentially absent in the tropical rain forest (jungle) cities during our study period. The city of Iquitos (Jungle) had the highest number of influenza B cases and only one pH1N1 case. CONCLUSIONS: The viral distribution in Peru changed upon the introduction of the pH1N1 virus compared to previous months. Although influenza A viruses continue to be the predominant viral pathogen, the pH1N1 virus predominated over the other influenza A viruses.
