OBJECTIVE: Endothelium-related events in patients with coronavirus disease 2019 are linked to a poor prognosis. Lipoprotein(a) plays a role in vascular endothelial cell dysfunction. This research aims to investigate whether baseline serum lipoprotein(a) levels could be a predictor for intensive care unit admission and related clinical parameters in coronavirus disease 2019 patients. MATERIAL AND METHODS: The research covers 126 patients who were hospitalized in intensive care unit or the non-intensive care unit in our hospital. This prospective cohort study was conducted from January 2021 to June 2021. The patients who were positive for severe acute respiratory syndrome coronavirus 2 according to real-time polymerase chain reaction test results were included in the study. Two groups were created according to the status of intensive care unit admission. Lipoprotein(a) was studied from blood samples taken at the time of hospital admission. RESULTS: According to the results of the first clinical evaluation, 46 patients were admitted to the intensive care unit and 80 patients were admitted to non-intensive care unit in the hospital. Patients with intensive care unit admission had significantly higher serum lipoprotein(a) levels than patients without intensive care unit admission (40.9 ng/mL and 17.4 ng/mL, P < .001, respectively). The regression analysis revealed that serum lipoprotein(a) levels were independently related to intensive care unit admission (odds ratio 1.242, 95% CI 1.109-1.391, P < .001). In receiver operating characteristic curve analysis, lipoprotein(a) level =31.42 ng/mL had 82.6% sensitivity and 72.5% specificity in predicting intensive care unit admission. The risk of intensive care unit admission was seen to be 12.522-fold higher in cases with lipoprotein(a) level =31.42. CONCLUSION: Lipoprotein(a) could be used as a useful biomarker for the triage of coronavirus disease 2019 patients. Baseline serum lipoprotein(a) levels may serve as a useful prognostic biomarker in patients hospitalized for coronavirus disease 2019. [ABSTRACT FROM AUTHOR]

5. Koroner Stent Uygulanan ST Elevasyonsuz Miyokard İnfarktüsünde Pentraksin-3’ün Uzun Vadeli Prognostik Önemi

- Abstract
Amaç: ST elevasyonu olmayan miyokard infarktüsü (NSTEMI) ve stabil anjina pektoris (SAP) tanısıyla perkütan koroner girişim (PKG) yapılmış hastalarda ardışık ölçülen pentraksin-3 (PTX3) düzeylerinin Gensini risk skoru ve uzun dönem takiplerde kardiyovasküler mortalite ile ilişkisini araştırmayı amaçladık. Gereç ve Yöntem: Çalışmamız retrospektif olarak planlanmış olup PKG yapılan NSTEMI ve SAP tanılı hastaların uzun dönem kardiyovasküler mortalite sonuçları değerlendirildi. Çalışmamız, çalışma ve kontrol grubu olmak üzere iki gruptan oluşmaktadır. PKG yapılan NSTEMI tanılı 18 hasta çalışma grubuna, SAP tanılı 37 hasta ise kontrol gurubuna dahil edildi. Tüm hastalardan başvuru anı, 8. ve 24. saatte PTX3 ölçümleri için kan örnekleri alındı. Gensini skorları PKG öncesi hesaplandı. Bulgular: Sekizinci saatte bakılan PTX3 düzeyleri NSTEMI hastalarında SAP tanılı hastalara göre sayısal ve istatistiksel olarak anlamlı saptandı [13,37 (5,47-27,75) ve 5 (3,83-12,42), p=0,006]. Başvuru anında bakılan PTX3 değerleri Gensini skoru ile ilişkili saptandı (r=0,299, p=0,026). Sekizinci saatte bakılan PTX3 değerleri uzun dönem kardiyovasküler mortalite için bağımsız öngördürücü olarak saptandı (Hazard oranı: 1,294, %95 güven aralığı: 1,024-1,653, p=0,039). Sonuç: PTX3, NSTEMI hastalarında uzun dönemde kardiyovasküler mortalite için yüksek riskli bireylerin belirlenmesinde yardımcı olabilir.

Objectives: Coronary tortuosity (CT) is a common coronary angiography result and the clinical significance of CT is not clearly understood. In this study, the relationship between the presence of CT with CHA2DS2-VASc score and atrial fibrillation (AF) was analyzed. Materials and Methods: Our study included 511 patients who underwent coronary angiography with evidence of coronary artery ischemia. All patients were assessed for the presence and severity of CT. Three or more bends that caused at least 45° changes in the main body of the coronary artery in both systole and diastole were defined as CT. The study patients were divided into groups according to the presence and severity of CT. Results: CHA2DS2-VASc score was higher in the significant (SCT) group (p=0.001). Increased CHA2DS2-VASc score, regardless of age and female gender, was statistically SCT only for the presence of LAD SCT [p=0.003, adjusted odds ratio (OR): 1.95, 95% confidence interval (CI): 1.69-2.20]. Left anterior descending (LAD) SCT (p=0.014, OR: 3.11, 95% CI: 1.25-7.69) was reported to be a possible predictor of AF. Conclusion: Patients who have LAD SCT observed on coronary angiography should be considered for periodic verification with electrocardiography in terms of AF development. [ABSTRACT FROM AUTHOR]

Amaç: Kararlı anjina ve ST yükselmesiz akut koroner sendrom (AKS) tanısıyla perkütan koroner girişim (PKG) yapılmış 39 hastada ardışık ölçülen “pentraxin-3” (PTX3) düzeyleri ve bu düzeylerin GRACE risk skoru ile ilişkisi incelendi. Çalışma planı: Çalışmaya perkütan koroner stent takılan ST yükselmesiz AKS tanılı 17, kararlı anjina tanılı 22 olgu alındı. Tüm hastalarda girişim öncesi, girişim sonrası 8. ve 24. saatte PTX3 ölçümü için kan örnekleri alındı. Bulgular: Kararlı anjinalı grupta diabetes mellitus ve hipertansiyonlu olgu sayısı anlamlı olarak daha yüksek iken, AKS grubunda lökosit sayısı anlamlı olarak daha yüksek saptandı. Sekizinci saat ölçülen PTX3 düzeyleri anlamlı olarak AKS grubunda daha yüksekti (p=0.003). AKS’li olgularda 24. saat PTX3 düzeyleri ile GRACE başvuru hastane içi ve 6 ay ölüm-kardiyak ölüm-ME ve GRACE taburculuk kardiyak ölüm-ME risk oranları arasında güçlü korelasyon saptandı (sırasıyla, p=0.02, R: 0.558; p=0.03, R=0,512; p=0.01, R=0,571; p=0.01, R=0.564; p=0.004, R=0.653). Sonuç: Serum PTX3 düzeyleri koroner stent takılan AKS’li olgularda erken risk sınıflaması açısından önemli bilgiler verebilir., Objectives: We analyzed pentraxin 3 (PTX3) levels and he relation of PTX3 levels with GRACE risk scores in 39 patients with non-ST elevation acute coronary syndrome ACS) and stabile angina after stenting. Study design: Seventeen patients with ACS and 22 pa- ients with stabile angina who underwent coronary stenting were included in the study. PTX3 levels were measured se- ially at admission, at the 8th hour and at the 24th hour after stenting. Results: While diabetes and hypertension were more fre- quent in the stabile angina group, leukocyte counts were significantly higher in the ACS group. PTX3 levels mea- sured at the 8th hour were significantly higher in the ACS group compared to the stabile angina group (p=0.003). Strong correlations were observed between 24th hour PTX3 levels and GRACE scores calculated for risk of death and death/MI at admission (in-hospital/to 6 months), and for isk of death/MI at discharge to 6 months (R=0.571, p=0.01, R=0.564, p=0.01; R=0.558, p=0.02, R=0.512, p=0.03; R=0.653, p=0.004, respectively). Conclusion: the serum PTX3 levels may provide impor- ant information for the early risk stratification of patients with ACS who underwent coronary stenting.

Bu yazıda 36 yaşında "non-Hodgkin" lenfoma tanısı ile beşinci kür kemoterapisini (sitarabin, etoposide ve cisplatin) alırken akut miyokard infarktüsü gelişen bir olgu sunulmuştur. Koroner arter hastalığı risk faktörü taşımayan, koronerleri anjiyografide normal bulunan ve apikal diskinezi saptanan bu premature miyokard infarktüsü olgusu dolayısı ile kemoterapi rejimi ile miyokardiyal iskemi arasındaki patofizolojik ilişki tartışılarak genç hastalarda da böyle bir komplikasyona karşı bilgili olmanın önemi vurgulanmıştır., A 36-year-old man with non-Hodgkin's lymphoma disease was presented with acute myocardial infarction during the fifth cycle of chemotherapy with cytarabine, etoposide and cisplatin. He had no significant risk factors for coronary heart disease and coronary angiogram revealed normal coronaries meanwhile ventriculography showed an apical akinesia. the physiopathological mechanisms of causal relationship between the chemotherapy regimen and myocardial ischemia are discussed and the importance of the awareness of such a complication is emphasized.

Background: Aortic valve stenosis (AVS) is the most common cause of left ventricular outflow obstruction, and its prevalence among elderly patients causes a major public health burden. Recently, platelet-to-lymphocyte ratio (PLR) has been recognized as a novel prognostic biomarker that offers information about both aggregation and inflammation pathways. Since PLR indicates inflammation, we hypothesized that PLR may be associated with the severity of AVS due to chronic inflammation pathways that cause stiffness and calcification of the aortic valve. Materials and Methods: We retrospectively enrolled 117 patients with severe degenerative AVS, who underwent aortic valve replacement and 117 control patients in our clinic. PLR was defined as the absolute platelet count divided by the absolute lymphocyte count. Severe AVS was defined as calcification and sclerosis of the valve with a mean pressure gradient of >40 mmHg. Results: PLR was 197.03 ± 49.61 in the AVS group and 144.9 ± 40.35 in the control group, which indicated a statistically significant difference (P < 0.001). A receiver operating characteristic (ROC) curve analysis demonstrated that PLR values over 188 predicted the severity of aortic stenosis with a sensitivity of 87% and a specificity of 70% (95% confidence interval = 0.734-0.882; P < 0.001; area under ROC curve: 0.808). Conclusion: We suggest that the level of PLR elevation is related to the severity of degenerative AVS, and PLR should be used to monitor patients' inflammatory responses and the efficacy of treatment, which will lead us to more closely monitor this high-risk population to detect severe degenerative AVS at an early stage. Objective: Impairment in left ventricular (LV) function due to excessive ventricular extrasystoles (VESs) occurs during long-time follow-up. Speckle tracking echocardiography (STE) has been shown to be superior to conventional methods for evaluating cardiac functions. We aimed to use STE for early detection of LV dysfunction in patients with apparently normal hearts who have frequent VESs. Methods: Fifty-five patients with frequent VESs were identified according to the Lown classification (Grade 2; unifocal more than 30 times in 1 h). Subjects aged 22-60 years with frequent VESs that had been detected for at least 1 year were included in the study according to the inclusion criteria. Forty-five subjects with similar demographic characteristics, but without VESs, were included as the control group. All participants were evaluated by STE. Results: Fifty-five patients with frequent VESs (mean age 47 years, range 22-60 years; 42.2% male) and 45 control subjects (mean age 46 years, range 22-60 years; 37.8% male) were enrolled in the study. Global LV longitudinal strain (GLS) was decreased in patients with frequent VESs (-18.41±3.37 and -21.82±2.43; p<0.001). In addition, global LV circumferential strain was decreased in patients with frequent VESs (-16.83±6.06, -20.51±6.02; p<0.001). The frequency and exposure time of VESs were negatively correlated with GLS (r=-0.398, p<0.001; r=-0.191, p=0.001, respectively). Conclusion: STE revealed that LV functions were decreased in patients with VESs. This deterioration increased with the frequency and exposure time of VESs. Impairment of LV function due to excessive VESs occurs during long-time follow-up. STE may be used for early detection of LV dysfunction. [ABSTRACT FROM AUTHOR]

Objective: Hepcidin is an acute-phase reactant produced in the liver displaying intrinsic antimicrobial activity. There are few studies about hepcidin considered to be acute and chronic inflammatory marker in acute coronary syndromes patients. We investigated in our study whether the level of hepcidin has increased in the acute phase of non-ST elevation myocardial infarction patients (NSTEMI) known as acute inflammatory aggravation of chronic atherosclerotic process. Methods: Seventy patients with NSTEMI and twenty healthy people were recruited as controls in this observational cross-sectional study. Serum hepcidin levels were determined by ELISA, and troponin levels were measured by standard laboratory methods. Levels of hepcidin and troponin were measured at admission and 6 hours later. Mean values of continuous variables were compared between groups using the Student t-test or Mann-Whitney U test, according to whether normally distributed or not, as tested by the Kolmogorov-Smirnov test. Serum troponin and hepcidin levels measured at admission and after 6th hours were compared using paired t-test. Results: Hepcidin level was similar between NSTEMI and controls at admission (24.55±32.13, 23.67±33.62 ng/mL, p>0.05, respectively). Also, serum hepcidin levels did not change significantly from baseline in blood samples taken after 6 hour from admission in NSTEMI patients (24.55±32.13 ng/mL, 29.75±31.48 ng/mL, p=0.62, respectively). However, serum troponin levels were increased significantly compared to baseline (0.29±3.56, 2.92±7.2 ng/mL, p<0.01). Conclusion: Our findings suggest that hepcidin could not be use as a marker of myocardial necrosis in acute phase such as troponin in patients with NSTEMI. 21. New fully automated software for assessment of brachial artery flow- mediated dilation with advantages of continuous measurement.

- Abstract
Objective: Flow-mediated dilation (FMD) is used to evaluate endothelial functions. Computer-assisted analysis utilizing edge detection permits continuous measurements along the vessel wall. We have developed a new fully automated software program to allow accurate and reproducible measurement.Methods: FMD has been measured and analyzed in 18 coronary artery disease (CAD) patients and 17 controls both by manually and by the software developed (computer supported) methods. The agreement between methods was assessed by Bland-Altman analysis.Results: The mean age, body mass index and cardiovascular risk factors were higher in CAD group. Automated FMD% measurement for the control subjects was 18.3±8.5 and 6.8±6.5 for the CAD group (p=0.0001). The intraobserver and interobserver correlation for automated measurement was high (r=0.974, r=0.981, r=0.937, r=0.918, respectively). Manual FMD% at 60th second was correlated with automated FMD % (r=0.471, p=0.004).Conclusions: The new fully automated software© can be used to precise measurement of FMD with low intra- and interobserver variability than manual assessment. Background: The aim of our study was to determine the coronary angiographic distribution and frequency of atherosclerotic lesions in the coronary arteries in patients with stable angina and metabolic syndrome (MS) as well as to assess the association of these lesions with TIMI frame count and combinations of MS criteria. Methods: Two hundred sixty-one patients aged 61.8±1.4 years were included in the study. All subjects were divided into three large groups: group 1 - patients with significant coronary artery stenosis and with MS (n=148), group 2 - patients with significant coronary artery stenosis and without MS (n=75), and group 3 - patients without significant coronary artery stenosis and without MS (n=38). At the same time, the patients in group 1 were again divided into 3 subgroups based on the number of MS criteria. Arterial blood pressure (BP), waist circumference (WC), and levels of fasting glucose (FG), triglycerides (TG) and high-density lipoproteins (HDL) were measured in all patients. All subjects underwent coronary angiography and TIMI frame count was calculated. Results: According to the results in patients with stable angina with 5 criteria of MS, the TIMI frame count was significantly greater than in patients with stable angina with 3 and 4 criteria of MS. While in the group of patients with stable angina with 3 criteria of MS, the most frequent combination found was BP +WC + FG (28.3±6.2%), in patients with stable angina with 4 MS criteria, it was BP + WC + FG + HDL combination (48.0±7.1%). Conclusions: The TIMI frame count method is simple, inexpensive and broadly applicable and plays role in the understanding of the physiology and pathophysiology of coronary artery blood flow in patients with stable angina and MS. For this reason, the calculation of TIMI frame count in patients with stable angina and MS has an important place in selecting the treatment options and determining the prognosis of the disease. Günümüzde kalp yetersizliği (KY) klinik sendromu olan hastaların yaklaşık yarısını korunmuş ejeksiyon fraksiyonlu kalp yetersizliği (KEF-KY) oluşturmaktadır. Renin anjiotensin aldosteron sistemi (RAAS) blokerlerinin düşük ejeksiyon fraksiyonlu hastalarda prognoz ve sağ kalımı düzeltiği kanıtlanmasına rağmen KEF-KY hastalarında şu ana kadar RAAS blokerleride dahil hiçbir tedavinin hastalarda mortalite ve morbiditeyi azaltığı gösterilememiştir. Biz bu çalışmada RAAS blokajının sol ventrikül üzerindeki yararlı etkilerinin, KEF-KY hastalarında diyastolik disfonksiyon parametreleri, sol atriyal (SA) fonksiyonlar ve serum apelin seviyeleri üzerindeki değişiklikler ve birbirleri ile olan ilişkilerini değerlendirmeyi amaçladık.Gereç-YöntemÇalışmamıza 34 RAAS blokeri kullanmayan hipertansif ve 19 RAAS blokeri kullanan toplam 53 KEF-KY hastası dâhil edildi. Olguların ayrıntılı anamnez ve fizik muayeneleri yapıldıktan sonra transtorasik ekokardiyografi incelemesi ve serum apelin seviyeleri için kan örnekleri alındı. RAAS blokeri kullanmayan olgulara RAAS blokeri başlandı. RAAS blokeri başlanılan olgular ortalama 3 ay sonra kontrole çağrılarak ikinci kez transtorasik ekokardiyografi ve kan örnekleri tekrarlandı. Hastaların transtorasik ekokardiyografi görüntülerinden mitral giriş yolu doppler akımları ve mitral anular doku doppler kayıtları alındı ve kaydedildi. Ayrıca 4 ve 2 boşluk görüntülerden SA volüm indeksleri ve SA global longitudinal zirve ve pompa strainleri kaydedilerek hesaplandı.BulgularHer iki grup arasında demografik veriler açısından anlamlı fark izlenmedi. Önceden RAAS blokeri kullanan hastaların RAAS blokaj süresi ise 25,26±17,3 aydı. RAAS blokeri başlanan grup 2,14±0,97 ay takip edildi. Takip süresince E dalga hızında anlamlı artış (0,602±0,123 vs 0,662±0,166 p:0,041) A dalga hızında anlamlı azalma (0,819±0,193 vs 0,766±0,179 p:0,030) E/A oranında anlamlı artma (0,756±0,169 vs 0,888±0,279 p:0,007) izlendi. SA volüm ve volüm indekslerinde, SA global longitudinal zirve ve pompa strainlerinde ve serum apelin seviyelerinde tedavi süresince anlamlı değişiklik izlenmedi. Ancak apelin seviyelerinin tedavi ile değişimi ile A dalga hızının değişimi arasında anlamlı pozitif korelasyon saptandı (r=0,563 p:0,004). Önceden RAAS blokeri kullanan ve RAAS blokeri başlananların kontrolleri arasında serum apelin seviyelerinde anlamlı fark izlendi (1074±163 vs 491±19 P: 0,003). SonuçKEF-KY hastalarında renin anjiotensin aldosteron sistem blokajı ile kısa süreli tedavinin SA volüm indeksleri, SA global strain ölçümleri ve apelin seviyeleri üzerinde anlamlı bir etkisinin olmadığını ancak diyastolik fonksiyon parametrelerinde olumlu etkisinin olduğunu tespit ettik. Tedavi ile serum apelin seviyelerindeki değişim ve A dalga hızındaki değişim arasındaki pozitif korelasyon, apelinin SA sistolik fonksiyonların değerlendirilmesinde kullanılabileceğini düşündürmektedir. Ayrıca önceden RAAS blokeri kullananlarda apelin seviyelerinin anlamlı yüksek çıkması, uzun süreli tedavinin RAAS blokerlerinin ventriküler yeniden şekillenme üzerine olumlu etkilerini yansıtıyor olabilir. It is now well known that approximately half of the patients with the clinical syndrome of heart failure have preserved ejection fraction (HF-PEF). Although blockers of renin-angiotensin aldosterone system (RAAS) have been proven to correct prognosis and survival in patients with reduced ejection fraction, no treatment option, including RAAS blockade, has been demonstrated to decrease the rate of morbidity and mortality in patients with HF-PEF. In this study, we aimed to evaluate the beneficial effects of RAAS blockade on left ventricular function in patients with HF-PEF, and also to evaluate diastolic dysfunction parameters, left atrial functions and changes in apelin levels, and the relationships between each other. Material and Methods A total of 53 HF-PEF patients including 34 hypertensive patients never received a RAAS-blocker and 19 RAAS-blocker users were enrolled into the study. Following anamnesis and physical examination, transthoracic echocardiography was performed, and blood samples for serum apelin levels were taken in all patients. RAAS blockers were prescribed in all non-RAAS-blocker-user patients. Those patients were re-called for a second transthoracic echocardiography and blood sampling for control apelin levels after 3 months of the treatment. Mitral inflow Doppler patterns and mitral annular tissue Doppler profiles were obtained from the transthoracic echocardiography images of the patients. Additionally, left atrial volume indices and global longitudinal peak and pump strains were calculated from the 4- and 2-chamber images of the left atrium.ResultsDemographic data were similar in the groups. The mean duration of RAAS blockade in the patients with a history of prior usage of RAAS blockers was 25.26 ± 17.3 months. It was 2.14 ± 0.97 months in the patients with recent RAAS-blocker treatment. It was observed during follow-up that E-wave velocity significantly increased (0.602 ± 0.123 vs 0.662 ± 0.166 p = 0.041), A wave velocity significantly decreased (0.819 ± 0.193 vs 0.766 ± 0.179 p = 0.030), and E / A ratio significantly increased (0.756 ± 0.169 vs 0.888 ± 0.279 p = 0.007). Left atrial volumes and volume indices, left atrial global peak and pump longitudinal strains, and apelin levels were similar throughout the treatment. But the change of apelin levels was positively correlated with the change in A wave velocity (R = 0.563 p = 0.004). There was a significant difference in apelin levels between the patients with prior RAAS-blocker treatment and the patients with recent RAAS-blocker treatment (1074 vs 491 p: 0.003).DiscussionWe found that the short-term treatment with RAAS blockers provided beneficial effects on the variables representing diastolic function but not on the left atrial volume indices, left atrial global strain measurements and apelin levels. The correlation between the change of apelin levels and the change in A wave velocity may suggests that apelin may be used to evaluate left atrial systolic functions. Additionally, the high level of apelin in the patients with prior RAAS-blocker treatment may reflect the beneficial effects of the long-term treatment with RAAS blockers on ventricular remodeling. 97

6.ÖZETAMAÇKalp hızı değiskenliği, kardiyak otonomik aktiviteyi ölçen önemli bir belirteçtir ve kalp yetersizliğinde dahil olmak üzere bir çok klinik durumda prognoz hakkında bilgi veren bağımsız bir öngördürücüdür. Sağ ventrikül fonksiyonlarının mortalite ve morbiditenin en önemli belirleyicilerinden biri olduğu bilinmektedir. Daha önce yapılan çalısmalarda sol ventrikül sistolik fonksiyonu düsük olan hastalarda sağ ventrikül fonksiyon bozukluğu mortalitenin bağımsız bir belirleyicisi olduğu gösterilmistir. Litaretürde sol ventrikül sistolik disfonksiyonu olan bireylerde, ekokardiyografik olarak sağ ventrikül fonksiyonlarını gösteren parametreler ile kalp hızıdeğiskenliği arasındaki iliskiyi gösteren çalısma göze çarpmamaktadır. Çalısmamızda bu amaçla normal sağlıklı bireyler ile sol ventrikül sistolik disfonksiyonu olan bireylerde ekokardiyografik olarak bakılacak sağ ventrikül fonksiyonlarının kalp hızı değiskenliği üzerine olan etkileri arastırılacaktır.MATERYEL-METODÇalısmaya, Çanakkale 18 Mart Üniversitesi Tıp Fakültesi Kardiyoloji Kliniğinde takipte olan, Kalp yetersizliği tanısı konmus, ESC kılavuzuna göre optimal medikal tedavi altındaki, NHYA Sınıf IIIII sol ventrikül sistolik disfonksiyonlu (LVEF < % 40), 20-75 yas aralığında 60 hasta alındı (42 erkek, 18 kadın). Kontrol grubuna ise kalp yetmezliği olmayan 30 sağlıklı olgu alındı (16 erkek,14 kadın). Bu iki grup sağ ventrikül ekokardiyografik parametreleri ve kalp hızı değiskenliği yönünden karsılastırıldı.BULGULARTAPSE değeri düsük olan hastaların kalp hızı değisikliği ile kontrol grubu arasında anlamlı fark saptanmadı,SONUÇDiyabeti bulunan kalp yetmezliği olgularının sağ kalp fonksiyonları ile kalp hızı değisikliği paremetreleri arasında pozitif korelasyon saptanmıstır. Right ventricular function is known to be one of the most important determinants of mortality and morbidity. In the previous studies in patients with low left ventricular systolic function, right ventricular dysfunction was shown to be an independent predictor of mortality. There is no study in world literature showing the relationship between heart rate variability and echocardiographically observed right ventricular function in patients with left systolic dysfunction. Objective: The prognostic value of the Global Registry of Acute Coronary Events (GRACE) risk score (GRS) and the Thrombolysis In Myocardial Infarction (TIMI) risk index (TRI) has been reported in coronary artery disease (CAD) patients. We aimed to evaluate the relationship between the GRS, TRI, and severity of CAD evaluated by SYNTAX score (SS) in patients with acute coronary syndrome (ACS)., Methods: Patients with ACS who were admitted to the coronary care unit of our institution were retrospectively evaluated in this study. A total of 287 patients with ACS [154 non-ST elevated ACS (NSTE-ACS), 133 ST elevated myocardial infarction (STEMI)] were included in the study. The GRS and TRI were calculated on admission using specified variables. The severity of CAD was evaluated using the SS. The patients were divided into low (GRS<109)-, intermediate (GRS 109-140)-, and high (GRS>140)-risk groups and group 1 (TRI<17), group 2 (TRI 17-26), and group 3 (TRI>26) according to GRS and TRI scores. A Pearson correlation analysis was used for the relation between GRS, TRI, and SS., Results: Patients with a history of coronary artery bypass surgery, those who had missing data for calculating the GRS and TRI, and those whose systolic blood pressure (SBP) was more than 180 mm Hg or whose diastolic blood pressure (DBP) was more than 110 mm Hg were excluded from the study. There were significant differences in mean age (p<0.001), heart rate (p<0.001), SS (p<0.001), TRI (p<0.001), rate of NSTE-ACS (p<0.001), and STEMI (p<0.001) in all patients between the risk groups. There was a positive significant correlation between the GRS and the SS (r=0.427, p<0.001), but there were no significant correlation between the TRI and SS (r=0.121, p=0.135). The area under the ROC curve value for GRS was 0.65 (95% CI: 0.56-0.74, p=0.001) in the prediction of severity of CAD., Conclusion: The GRS is more associated with SS than TRI in predicting the severity of CAD in patients with ACS.
