11. Effectiveness of Chinese herbal medicine combined with conventional medicine on acute exacerbation of chronic obstructive pulmonary disease: a systematic review and Meta-analysis

Abstract

OBJECTIVE: To systematically evaluate the efficacy and safety of Chinese herbal medicine (CHM) combined with conventional Western Medicine (CWM) on acute exacerbation of chronic obstructive pulmonary disease (AECOPD) based on high-quality randomized placebo-controlled trials. METHODS: We searched PubMed, Embase, Cochrane Library, China National Knowledge Infrastructure Database, Chinese Biomedical Literature Database, China Science and Technology Journal Database, and Wanfang databases for randomized placebo-controlled trials of CHM treatment for AECOPD from inception to June 4, 2021. The Cochrane Collaboration’s tool and the Grading of Recommendations, Assessment, Development and Evaluation were used to assess the risk of bias and the evidence quality of the included studies. Revman 5.3 software was used for Meta-analysis. RESULTS: A total of 9 trials involving 1591 patients were included. The Meta-analysis showed that based on CWM treatment, CHM group had significant advantages over the placebo group in ameliorating clinical total effective rate [RR = 1.29, 95% CI (1.07, 1.56), P = 0.007, low quality] and TCM symptom scores [MD = -2.99, 95% CI (-4.46, -1.53), P < 0.0001, moderate quality], improving arterial blood gas results [PaO(2): MD = 4.51, 95% CI (1.97, 7.04), P = 0.0005, moderate quality; PaCO(2): MD = -2.87, 95% CI (-4.28, -1.46), P < 0.0001, moderate quality], reducing CAT scores [MD = -2.08, 95% CI (-2.85, -1.31), P < 0.000 01, moderate quality]，length of hospitalization [MD = -1.87, 95% CI (-3.33, -0.42), P = 0.01, moderate quality], and acute exacerbation rate [RR = 0.60, 95% CI (0.43, 0.83), P = 0.002, moderate quality]. No serious CHM-related adverse events were reported. CONCLUSIONS: The current evidence indicates that CHM is an effective and well-tolerated adjunct therapy for AECOPD patients receiving CWM. However, considering the high heterogeneity, this conclusion requires confirmation.

20. Design of Broadband 9×9 Butler Matrix and its Extension for Multibeam Application

Abstract

In this brief, a broadband $9\times9$ Butler Matrix (BM) and its extension for multibeam antenna array (MAA) application are presented. Based on the configuration of $3\times3$ beamforming network (BFN) comprised of three couplers and two 90° phase shifters, the topology of $9\times9$ BM is built, which can provide in-phase signals at output ports. And a broadband $9\times9$ BM is implemented to verify the design by adopting broadband components. The results indicate that the designed broadband $9\times9$ BM achieves equal power division and 9 uniformly distributed progressive phases (PPs) between output signals in a relative bandwidth of 40% when the 9 input ports are excited, respectively. And the fabricated $9\times9$ BM is applied in MAA for producing 9 beams including a boresight beam. In addition, according to the design method of $9\times9$ BM, the topology of the extended $27\times27$ BM is built and presented, which validates that an extended BM with higher order can be realized by the design method, and the extended BM can also provide in-phase signals at output ports.

Randomized controlled trials were included if they observed the efficacy of early PR in AECOPD patients. Study selection, data extraction, risk of bias and quality of evidence were assessed by two researchers independently. Assessment of the risk of bias and eidence quality were evaluated by the Cochrane Collaboration’s tool and Grading of Recommendations, Assessment, Development and Evaluation system, respectively.Results: Fourteen trials (829 participants) were identified. Significant improvement was found in the 6-minute walk distance (6MWD; mean difference (MD): 69.64; 95% CI: 40.26 to 99.01; Z = 4.65, P < 0.0001, low quality). In the subgroup analysis, the exercise-training group showed marked improvement (MD: 96.14; 95% CI: 20.24 to 172.04; Z = 2.48, P = 0.001). The Saint George’s Respiratory Questionnaire (SGRQ) total score was low (MD: − 12.77; 95% CI: − 16.03 to − 9.50; Z = 7.67, P < 0.0001, moderate quality). Significant effects were not found for the duration of hospital stay, quadriceps muscle strength or five times sit to stand test. Only one serious adverse event was reported in experimental group, which was not associated with early PR.Conclusion: PR initiated < 3 days of hospitalization may increase exercise capacity and improve quality of life, but the results should be interpreted prudently and dialectically, and the role of early PR in AECOPD needs further exploration.Keywords: chronic obstructive pulmonary disease, acute exacerbation, pulmonary rehabilitation, meta-analysis

33. Design of Modified Butler Matrix Based on N-Way Directional Coupler and Its Multibeam Application

Abstract

In this article, a new approach to designing a modified $N^{2} \times N^{2}$ Butler matrix (BM) based on an $N$ -way directional coupler (NWDC) is presented. First, by introducing phase shifters and ingeniously arranging the input and output ports, the topologies of $9 \times 9$ BM based on a three-way directional coupler (3WDC) and $16 \times 16$ BM based on a four-way directional coupler (4WDC) are built. Next, according to the design method of $9 \times 9$ BM and $16 \times 16$ BM, the topology design of modified $N^{2} \times N^{2}$ BM based on NWDC is developed. Considering whether there are progressive phase differences (PPDs) of 0° at output ports, the two cases in which $N$ is odd and even are discussed. Finally, in order to verify the design method, a prototype of microstrip $9 \times 9$ BM working at 5.5 GHz is designed, fabricated, and measured. The results show that the designed microstrip $9 \times 9$ BM achieves equal power division and nine different PPDs among the output signals when the nine input ports are excited, respectively, which meets design expectations. And it is used as a beamforming network (BFN) for a multibeam antenna array (MAA) to generate nine radiation beams. And then, the design method of modified $N^{2} \times N^{2}$ BM is validated by the implementation of microstrip $9 \times 9$ BM. And the design method of the modified $N^{2} \times N^{2}$ BM provides a novel approach for BFN technology when massive input and output ports are required.

Abstract

Abstract Background Hypofractionated stereotactic body radiation therapy (SBRT) has been advanced as monotherapy for low-risk prostate cancer. We examined the dose distributions and early clinical outcomes using this modality for the treatment of intermediate-risk prostate cancer. Methods Forty-one sequential hormone-naïve intermediate-risk prostate cancer patients received 35–36.25 Gy of CyberKnife-delivered SBRT in 5 fractions. Radiation dose distributions were analyzed for coverage of potential microscopic ECE by measuring the distance from the prostatic capsule to the 33 Gy isodose line. PSA levels, toxicities, and quality of life (QOL) measures were assessed at baseline and follow-up. Results All patients completed treatment with a mean coverage by the 33 Gy isodose line extending >5 mm beyond the prostatic capsule in all directions except posteriorly. Clinical responses were documented by a mean PSA decrease from 7.67 ng/mL pretreatment to 0.64 ng/mL at the median follow-up of 21 months. Forty patients remain free from biochemical progression. No Grade 3 or 4 toxicities were observed. Mean EPIC urinary irritation/obstruction and bowel QOL scores exhibited a transient decline post-treatment with a subsequent return to baseline. No significant change in sexual QOL was observed. Conclusions In this intermediate-risk patient population, an adequate radiation dose was delivered to areas of expected microscopic ECE in the majority of patients. Although prospective studies are needed to confirm long-term tumor control and toxicity, the short-term PSA response, biochemical relapse-free survival rate, and QOL in this interim analysis are comparable to results reported for prostate brachytherapy or external beam radiotherapy. Trial registration The Georgetown Institutional Review Board has approved this retrospective study (IRB 2009–510).
