Endoscopic resection (ER) and laparoscopic surgery are the main methods for treatment of early gastric cancer (EGC). Radical gastrectomy(RG) and lymph node dissection are required for the patients who don’t meet the indications of ER, but it may cause postoperative stomach dysfunction and reduce quality of life (QOL). Pylorus-preserving gastrectomy (PPG) is one of the function-preserving gastrectomy for EGC. The short-term complications of PPG are equal to that of RG and it is also beneficial to long-term nutritional status and QOL. The 5-year survival rate of PPG is similar to that of distal subtotal gastrectomy (DSG) according to a retrospective study of the matched cohort. But delayed gastric emptying (DGE) is a common problem in the short and long term after PPG. The limited lymph node dissection of PPG makes its application not widely promoted. Sentinel node navigation surgery (SNNS) may provide more personalized and minimally invasive surgery, in order to preserve the gastric function to the maximum extent.

Objective: To analyse the incidence, diagnosis and treatment effect of multiple synchronous primary co-lorectal carcinoma(SCRC) to both explore reasonable alternative treatment types and improve the recognition of SCRC. Methods: The clinical data of 39 cases with SCRC admitted to Department of Surgery, Ruijin Hospital from December 2012 to June 2018 were retrospectively. Diagnosis and surgical types with postoperative survival was analysed. Results: There were 39 cases with SCRC, 27 cases male and 12 cases female, accounting for 2.84% of the cases with colorectal cancer in same period. The age (59.46±1.80) years in this study was lower than that of cases with single colorectal cancer in that period (P0.05) and in hospital stay (P>0.05) among different surgery. The most number of lymph nodes dissected was the cases using segmental resection and total or subtotal resection (P=0.004) and the cases with total or subtotal resection had the most number of defecation per day(P

It is still unclear about neoadjuvant therapy in patients with locally advanced colon cancer (LACC). This review summarized neoadjuvant chemotherapy, neoadjuvant chemotherapy combined with targeted therapy and combined radio-chemotherapy for the patients with LACC to increase the survival of patients focusing on the efficacy and safety of neoadjuvant therapy.

Objective To analyze the expression of CXCL5 (C-X-C motif ligand 5) and programme death ligand 1 (PD-L1) in colorectal cancer tissues and explore two chemokines associated with clinical characteristics and prognosis. Methods The specimens of tumor tissues and adjacent normal tissues were collected from 78 patients with colorectal cancer who had laparoscopic resection from 2010 to 2011. The clinical data were gotten and follow-up were done for prognosis. Tissue microarrays were made from all specimens and immunohistochemical staining was performed. Expression of CXCL5 and PD-L1 was evaluated using immune risk scoring and analyzed with the association of clinical pathological parameters and prognosis of patients. Results Expression of CXCL5 and PD-L1 was associated with tumor size and TNM stage, and was higher in colorectal cancer tissues than in adjacent normal tissues. Cox univariate regression analysis showed that TNM stage, high expression of both CXCL5 and PD-L1 were the prognostic risk factors for overall survival of colorectal patients. It was shown with multivariate regression analysis that TNM stage and high expression of PD-L1 were the independent prognostic risk factors for overall survival rate. Patients with higher expression of both CXCL5 and PD-L1 had the worst prognosis. Conclusions CXCL5 combined with PD-L1 could stratify the risk of patients with colorectal cancer and predict the prognosis of patients.

Objective To investigate pathologic effects combined with postoperative results on right colon cancer with laparoscopic complete mesocolic excision(CME) via dorsal-medial hybrid approach compared with traditional medial approach. Methods A total of 68 patients with right colon cancer undergoing laparoscopic CME in our hospital between Ja-nuary and June 2017 were studied retrospectively. There were two groups including 37 cases in dorsal-medial hybrid group and 31 cases in traditional medial group. CME quality evaluated with the method proposed by West et al along with safety, short-term results and long-term prognosis were compared between two groups. Results CME completion rate was 86.5% in dorsal-medial hybrid group and 80.6% in traditional medial group without statistical difference(P=0.53). Laparoscopic dissection time in dorsal-medial group was significantly shorter than that in traditional medial group[(62.70±12.22) min vs.(70.39±11.98) min, P=0.01]. There was no statistical difference in operation time, blood loss, postoperative flatus time, postoperative complications and long-term prognosis of the patients between two groups. Conclusions Laparoscopic CME via dorsal-medial hybrid approach for right colon cancer could have less laparoscopic dissection time, and two approaches were similar in CME quality, safety and prognosis.

Background: Although neoadjuvant therapy has brought numerous benefits to patients, not all patients can benefit from it. Chemokines play a crucial role in the tumor microenvironment and are closely associated with the prognosis and treatment of colorectal cancer. Therefore, constructing a prognostic model based on chemokines will help risk stratification and providing a reference for the personalized treatment., Methods: Employing LASSO-Cox predictive modeling, a chemokine-based prognostic model was formulated, harnessing the data from TCGA and GEO databases. Then, our exploration focused on the correlation between the chemokine signature and elements such as the immune landscape, somatic mutations, copy number variations, and drug sensitivity. CXCL10+M1 macrophages identified via scRNA-seq. Monocle2 showed cell pseudotime trajectories, CellChat characterized intercellular communication. CytoTRACE analyzed neoadjuvant therapy stemness, SCENIC detected cell type-specific regulation. Lastly, validation was performed through multiplex immunofluorescence experiments., Results: A model based on 15 chemokines was constructed and validated. High-risk scores correlated with poorer prognosis and advanced TNM and clinical stages. Individuals presenting elevated risk scores demonstrated an increased propensity towards the development of chemotherapy resistance. Subsequent scRNA-seq data analysis indicated that patients with higher presence of CXCL10+ M1 macrophages in tumor tissues are more likely to benefit from neoadjuvant therapy., Conclusion: We developed a chemokine-based prognostic model by integrating both single-cell and bulk RNA-seq data. Furthermore, we revealed epithelial cell heterogeneity in neoadjuvant outcomes and identified CXCL10+ M1 macrophages as potential therapy response predictors.
