1. Tenodermodesis para el tratamiento del dedo en martillo tendinoso de presentación tardía en niños y adolescentes

Abstract

Introducción: El diagnóstico del dedo en martillo tendinoso puede pasar desapercibido inicialmente en niños y adolescentes, esto limita las posibilidades del tratamiento conservador. El objetivo fue evaluar los resultados del tratamiento quirúrgico con la técnica de tenodermodesis en lesiones de presentación tardía. Materiales y Métodos: Se evaluó retrospectivamente a 9 pacientes (8 niños) con una edad promedio de 8.6 ± 6 años (rango 1-15). Los días promedio de evolución de la lesión eran 27±11.4 (rango 15-45). El mecanismo de lesión fue una herida cortante (4 casos) y un traumatismo indirecto (5 casos). El tratamiento consistió en tenodermodesis e inmovilización transitoria con clavija transarticular. El seguimiento promedio fue de 61 ± 34.7 meses (rango 12-106). Se evaluaron la movilidad activa y pasiva de la articulación interfalángica distal, la presencia de dolor o deformidad, la limitación de actividades de la vida diaria y la necesidad de tratamientos adicionales. Se clasificaron los resultados con los criterios de evaluación de Crawford. Resultados: En 8 pacientes, el resultado fue excelente y, en uno, regular según Crawford. Un paciente poco colaborador requirió una segunda intervención por re-rotura. En dos casos, hubo una complicación (granuloma) y requirió resección. Ningún paciente refirió dolor al final del seguimiento, ni limitaciones para las actividades de la vida diaria. Ocho presentaron extensión activa completa y uno, una deformidad residual de 20°. Conclusión: La tenodermodesis permite la reconstrucción anatómica del mecanismo extensor en niños y adolescentes. Los resultados clínicos de este estudio son alentadores en lesiones no diagnosticadas en forma temprana. Nivel de Evidencia: IV

Abstract

Recent frontiers in material development are represented by a class of so-called auxetic metamaterials that, thanks to their structure rather than composition, are characterized by a negative Poisson's ratio. In the present paper a two-dimensional auxetic plate, made by structural straight elements forming a lattice periodic structure with re-entrant cells, is considered. A thorough discussion on the linear and geometrically nonlinear deformability of the auxetic plate is presented. The key geometric parameters governing the deformability of the plate are identified, and some analytical expressions for calculating the Poisson's ratio, as a function of the applied strain, are given. Numerical (finite element) analyses and experimental tests on 3D printed specimens are carried out to verify the theoretical findings. For the latter ones, full field strain maps are obtained by means of a suitable interpolation of the sampled displacement field measured by digital image techniques.

OBJETIVO: Estimar os fatores de risco associados ao óbito até 28 dias após o nascimento de recém-nascidos internados entre os anos de 2005 a 2007 em uma Unidade de Terapia Intensiva Neonatal de hospital privado no Município de Taubaté, usando a análise de regressão de Cox. MÉTODOS: Estudo epidemiológico do tipo longitudinal. Os dados foram obtidos de prontuários de todos os recém-nascidos internados na Unidade de Terapia Intensiva Neonatal, no período de janeiro de 2005 a dezembro de 2007. Foram obtidas variáveis maternas e relativas ao recém-nascido, sendo o desfecho primário a mortalidade neonatal. Foi feita a análise univariada, utilizando teste do qui-quadrado, risco relativo e gráfico de Kaplan-Meier para cada variável separadamente. A análise multivariada foi realizada por meio da regressão de Cox para verificar a associação de fatores maternos e neonatais ao desfecho primário. RESULTADOS: Foram estudados 185 neonatos, dos quais 10,5% foram a óbito antes do 28º dia de vida. As variáveis que se associaram de modo significante ao óbito no modelo de Cox foram: peso OBJETIVO: Estimar los factores de riesgo asociados al óbito hasta 28 días después del nacimiento de recién-nacidos inter-nados entre los años de 2005 y 2007 en una Unidad de Terapia Intensiva de un hospital privado en el Municipio de Taubaté (São Paulo, Brasil), usando el análisis de regresión de Cox. MÉTODOS: Estudio epidemiológico de tipo longitudinal. Los datos fueron obtenidos de prontuarios de todos los recién-nacidos internados en las UTI neonatal privadas en el periodo de enero/2005 a diciembre/2007. Se obtuvieron variables tanto maternas como relativas al recién-nacido y categorizadas, siendo el desfecho primario la mortalidad neonatal. Se realizó el análisis univariado utilizando la prueba de chi cuadrado, el riesgo relativo y la gráfica de Kaplan-Meier para cada variable en separado. El análisis multivariado fue realizado por medio de la regresión de Cox para verificar la asociación de factores maternos y neonatales al desfecho primario. Abstract

Far-infrared emission spectroscopy has beendemonstrated to be a valuable method for remotesensing of trace species in the stratosphere, with theability to simultaneously detect a number of keychemical species. SAFIRE-A is a new far-infraredFourier Transform (FT) spectrometer which has beenspecifically designed to operate on board of a highaltitude aircraft in the lower stratosphere and uppertroposphere regions where relatively few remotesensing measurements have been made. Using newtechnology, the sensitivity of the FT spectrometermethod has been substantially improved for the longwavelength region. Results are reported formeasurements of O3, HNO3 and N2O at 17and 19 km using a detection window near 23 cm-1.Geographical and altitude variability of the volumemixing ratio of these constituents and their relativecorrelation are discussed. Ozone measurements agreewell with in situ measurements, except in regions ofstrong stirring and mixing associated with deformationof the northern vortex edge. Whilst SAFIREmeasurements of trace gases do not capture all of thelocal variability seen by rapid in-situ techniques,they can indicate horizontal variability close to, butnot intercepted by, the aircraft's flight path. Apossible detection of ClO at the low background levelsexpected outside the polar vortex is also reported.

Abstract

The design of the fine grained 300 t liquid argon calorimeter BARS is described. The BARS electronics include about 30 K channels of low noise amplifiers and ADCs. The DAQ system makes it possible to select channels with signals above the chosen threshold. 48 scintillation hodoscopes placed inside the liquid argon are used to form the first level trigger. The total number of scintillation counters in liquid argon is 384. Sums of ionization signals are used to produce the second level trigger. Results of the first use of liquid argon calorimetry for the measurements of tagged neutrino interactions, cosmic-ray muon spectra and composition of extensive atmospheric showers are discussed.

Abstract

A total of 60 samples of dry sausages were analyzed (50 of "salami" and 10 of "chorizo" "candelario" type) obtained at random in markets authorized for their commercialization, for the purpose of evidencing the presence of bacteria of the genus Listeria (Listeria monocytogenes and Listeria spp.). The results obtained in salami were the following: 10 (20%) isolates of Listeria spp., were characterized: 1 (2%) strain as L. monocytogenes type 1, 7 (14%) strains as L. innocua, 2 (4%) strains as L. welshimeri. In chorizo candelario type 6 (60%) isolates of Listeria spp., were characterized: 2 (33%) strains as L. monocytogenes type 1 and 4 (66%) strains as L. innocua. The total percentages of isolations were: 26.6% of Listeria spp., 5% of L. monocytogenes type 1, 18.3% of L. innocua and 3.3% of L. welshimeri. In conclusion, we consider that methodologies of control must be developed and implemented in order to guarantee the inocuity of these products.

Abstract

Reaction of Cp2M(PMe3)(2) complexes (M = Ti, Zr; Cp = eta(5)-C5H5) with the N-(p-tolyl)-diphenylketenimine Ph'N=C=CPh2 (Ph' = p-MeC6H4) in a 1:1 molar ratio affords the ketenimine-containing metallocene derivatives Cp2M(eta(2)-(C,N)-Ph'N=C=CPh2)(PMe3) (M = Ti (1); Zr (2)). The ketenimine ligand reacts in the same way with the ''Cp*M-2'' species (Cp* = eta(5)-C5Me5) generated from the reduction of the corresponding Cp*2MCl2 complexes with (LiBu)-Bu-t (1:2 molar ratio) to give the related complexes Cp*M-2(eta(2)-(C,N)-Ph'N=C=CPh2) (M = Ti (3); Zr (4)). The molecular structure of 3 shows a titanium atom bonded to two eta(5)-cyclopentadienyl rings and a eta(2)-(C,N)-bonded ketenimine ligand. Reaction of ''Cp*Ti-2'' with the ketenimine ligand in a 1:2 molar ratio gives 1,1,5,5-tetraphenyl-3-(p-tolyl)-2-(p-toluidino)-3-aza-1,4-pentadiene, which probably results from the coupling, followed by hydrolysis, of two ketenimine molecules coordinated to one titanocene moiety. Protonation of 3 with Et3NHCl or H2O (1:1 molar ratio) affords the intermediate species Cp*Ti-2(X)(eta(2)-(C,N)-Ph'N=C(H)=CPh2) (X = Cl (5); OH (6)), which on hydrolysis evolves to give the enamine Ph'N(H)-CH=CPh2 as the final product. Finally, 3 reacts reversibly with H-2 to give the hydride enamidate complex Cp*Ti-2(H)(eta(1)-Ph'N-CH=CPh2) (7). The structures of the different compounds have been determined by IR and NMR spectroscopic methods.

Abstract

We have constructed and operated charge preamplifiers for silicon strip detectors with a dynamic range extending from fractions of minimum ionising particle (MIP) up to 16 124 MIPs. These silicon detectors combined with time-of-flight counters and cesium iodide scintillator form a segment of the VENUS detector that has been exposed to a calcium beam of 0.5 GeV/u at the GSI accelerator. The aim of the instrument is the identification of all nuclides of the periodic table of the elements. Measurements of electronic noise, cross-talk among channels and energy deposit resolutions in various experimental conditions for silicon detectors are given. The measured light output of the CsI(Tl) crystal induced by calcium is compared with that extrapolated from lower-energy data of various nuclide species determined in other experiments. The charge resolution for calcium ions, determined by the d E d χ detectors and TOF counters of time resolution of 55 ± 7 ps, amounts to 0.42 charge units (rms). Improvements in ion discrimination with respect to the present detector configuration are considered.

31. A complete solution to neutrino mixing

Abstract

Deviations from expectations have been claimed for solar, atmospheric and high energy prompt neutrinos from charm decay. This information, supplemented only by the existing very good upper limits for oscillations of the $\nu_{\mu}$ at accelerator energies, is used as input to a phenomenological three-flavour analysis of neutrino mixing. The solution found is unique and completely determines the mass eigenstates as well as the mixing matrix relating mass and flavour eigenstates. Assuming the mass eigenstates to follow the hierarchy $m_{1} \ll m_{2} \ll m_{3}$, their values are found to be $m_{1} \ll 10^{-2}$ eV, $m_{2} = (0.18 \pm 0.06)$ eV, $m_{3} = (19.4 \pm 0.7)$ eV. These masses are in agreement with the leptonic quadratic hierarchy of the see-saw model and large enough to render energy-independent any oscillation-induced phenomenon in solar neutrino physics observable on Earth. This possibility is not excluded by the present knowledge of solar neutrino physics. The mixing angles are determined to be $\theta_{12} = 0.55 \pm 0.08$, $\theta_{13} = 0.38 \pm 0.06$, $\theta_{23} < 0.03$. Small values of $\theta_{23}$ are typical of any solution in which $m_{3}$ lies in the cosmological interesting region. The solution found is not in serious contradiction with any of the present limits to the existence of neutrino oscillations. Abstract

We have searched for neutralinos produced via the reactions e+e− → χχ′ and e+e−→ χ′χ′, where the next-to lightest neutralino, χ′, decays into the lightest neutralino, χ, and either a photon or a fermion pair. Based on 1.8 × 106 hadronic Z decays collected with the L3 detector at LEP, no signal has been observed. We present upper limits of a few times 10−5 on the branching ratios Z → χχ′ and Z → χ′χ′. In the framework of the Minimial Supersymmetric Standard Model, we exclude a lightest χ with mχ less than 18 GeV, if either tan β > 2 or the gluino mass m≈g > 100 GeV., Facultad de Ciencias Exactas

Abstract

To avoid the need to preclot porous polyester (Dacron) vascular prostheses, we have proposed the use of a protein coating that will promote the growth and adhesion of endothelial cells. This study assessed the relative advantages of coating woven, knitted, and velour polyester fabrics with albumin, collagen, and a albumin-collagen mixture after preservation in saline or drying by a commercial dehydration process. Preclotted fabrics were used as controls. The cytocompatibility of these biopolymers was measured by an organotypic culture technique which relies on the migration of chick embryonic endothelial cells. After 7 d of culture the cytocompatibility was quantified by counting the cells in the area of migration and the morphology of the endothelial cells was observed by scanning electron microscopy. In general, the knitted and velour fabrics showed superior compatibility than the woven one. The results confirmed that collagen, either alone or combined with albumin, provides in most cases a more cytocompatible surface than albumin alone. A cell morphology most closely resembling that of natural arterial endothelial cells was observed on the albumin-collagen substrate.

Abstract

The article describes a case of anesthetic management of thoracoscopy for ligation of the thoracic duct and pericardiectomy in a canine with chylothorax. The patient, an 8-year-old male dog, underwent a series of anesthetic and surgical procedures to treat his condition. General anesthesia was used, and different techniques were performed to maintain adequate oxygenation and ventilation during the procedure. The article highlights the importance of minimally invasive surgery in the treatment of certain surgical pathologies in veterinary medicine. [Extracted from the article]

Abstract

Five-coordinate complexes [PtX2(olefin)(2,9-Me2-phen)] (1; 2,9-Me2-phen = 2,9-dimethyl-1,10-phenanthroline; olefin = ethene denoted by a, propene by b, 1-butene by c; × = Cl denoted by x, Br by y) undergo photoactivated reactions with Cl2 and Br2 to give the (β-haloalkyl)platinum(IV) complexes [Pt(CH2CHRX)(2,9-Me2-phen)X3] (2). Bromination of the chloro species 1ax leads to the formation of the PtIV species 2axy containing the bromide, the bromoalkyl, and the phenanthroline ligands in the equatorial plane and two chloride ions in axial positions. The iodo complexes 1(a–c)z are not oxidized by iodine even under UV irradiation but react readily with Cl2 or Br2 to give 2(a–c)x and 2(a–c)y. The structure of 2ay, the first structurally characterized (β-haloalkyl)platinum complex, has been determined by X-ray diffraction methods. The stereochemistry of the (β-haloalkyl)platinum(IV) complexes is in accord with a simultaneous addition of two halogen atoms to the coordinated olefin and to the metal center.

Abstract

The effect of introducing a methyl substituent onto the central nitrogen atom of the ligand dien(dien = diethylenetriamine or 3-azapentane-1,5-diamine) on the ease of formation of mixed-valence chain complexes of platinum has been investigated. The chain complex prepared by partial oxidation of [Pt(4Me-dien)I]I, viz. [Pt(4Me-dien) I][Pt(4Me-dien)I3]I2(4)(4Me-dien = 3-methyl-3-azapentane-1,5-diamine), breaks down on recrystallisation to yield separate crystals of [Pt(4Me-dien)I]X [X = I3–(1) or I–(2)] and [Pt(4Me-dien)I3]I(3). X-Ray structural investigation of (1)–(3) and of similar discrete complexes of platinum reported previously indicate that dien co-ordinates in the planar (pseudo-mer) conformation in four-co-ordinate platinum(II) but in the bent (fac) conformation in six-co-ordinate platinum(IV) complexes, However, in mixed-valence chain complexes, dien is forced to co-ordinate to the platinum(IV) centre in the mer conformation. In the case of 4Me-dien the mer conformation around platinum(IV) is severely hindered; hence unless this species is trapped quickly between two platinum(II) entities it isomerises from mer to fac, thereby preventing the assembly of the mixed-valence chain complex. Complex (4) has a very intense Raman spectrum at resonance with the intervalence band, the principal observed progression being v1ν1[ν1(116.5 cm–1) is the symmetric stretching mode of the chain, νsym(I–PtIV–I)]. The wavenumber of ν1 found for this complex is lower than that of the analogous species with unsubstituted dien in accord with a lengthening of the PtIV–I linkage on N-methyl substitution of the ligand. Abstract

Objectives: today's consumers are increasingly interested in the relationship between food and health, recognizing food as a means to meet nutritional needs and prevent diseases. A diet rich in fish is beneficial to health, potentially protecting against cancer, cardiovascular, autoimmune, neurodegenerative, and metabolic diseases. During pregnancy, adequate nutrition benefits both the mother and the unborn child. This study compares pregnant women from a decade ago with those recently enrolled to evaluate differences in eating styles, specifically the consumption of fresh and frozen fish., Methods: we compared 114 pregnant women from 2013 with 168 women from 2023, using the same questionnaire to evaluate their eating habits during pregnancy, focusing on fresh and frozen fish consumption. Variables for statistical analyses included age, education, profession, family size, pre-pregnancy BMI, differential BMI, and frequency of fish consumption., Results: the comparison showed an increase in fish consumption, both fresh and frozen, among pregnant women in 2023 compared to 2013, indicating greater awareness of the health benefits of fish. Changes in dietary habits were influenced by profession, education level, and family size. Women in the 2023 cohort experienced smaller weight gain during pregnancy, suggesting potential health benefits. These shifts likely result from improved nutrition education and access to healthy foods, highlighting the importance of public health efforts to enhance maternal and fetal health., Conclusions: significant changes in the dietary habits of pregnant women over a decade were observed, with increased fish consumption in 2023 compared to 2013. These findings emphasize the role of nutrition education and improved access to healthy foods in promoting maternal and fetal health.

Abstract

The Coma Recovery Scale-revised (CRS-r) is the gold standard for the behavioral assessment of patients with Disorders of Consciousness (DoCs). However, the misdiagnosis rate is around 40%. For this reason, recent guidelines suggested enhancing the assessment with neurophysiological measures: among these, surface electromyography (sEMG) represents a convenient bedside solution. This work presents the use of the STRIVEfc system, a wearable device that allows CRS-r administration while recording four sEMG signals. STRIVEfc was employed in 40 sessions on 33 DoCs patients and the sEMG was analyzed to look for voluntary and consistent over threshold (OT) muscular activities. Their duration, amplitude, and number were retained and compared between patients in Unresponsive Wakefulness Syndrome (UWS) and Minimally Conscious State (MCS), revealing more numerous and significantly longer OTs in the latter group. Lastly, the EMG information was exploited to enrich the behavioral assessment by building the instrumented CRS-r score (ICRS-r). In 9/16 UWS sessions, the ICRS-r score suggested a higher-level functioning, not translated into a behavioral response, compatible with MCS diagnosis. Overall, the use of STRIVEfc allows to reveal hidden muscular patterns not detectable by the clinician, thus improving the characterization of DoCs patient's functional capabilities and supporting the diagnostic process., (© 2024. 45. Association between Fish Consumption during Pregnancy and Maternal and Neonatal Outcomes: A Statistical Study in Southern Italy.

Abstract

Background: This research aimed to evaluate the association between the monthly consumption of fish (differentiated by type) and both gestational and neonatal outcomes. Methods: Women who were admitted for delivery in the last 6 months of 2023 were prospectively included and divided according to type of fish consumed (based on DHA and mercury content) and frequency of consumption. Neonatal outcomes included weight, length, head circumference, and 1st and 5th minute Apgar scores. Maternal outcomes were threats of abortion, preterm birth, gestational diabetes and hypertension, cesarean section, and differential body mass index (BMI). Results: Small-size oily fish with high DHA and low mercury content (type B fish) consumption was positively associated with neonatal weight and head circumference, and less weight gain in pregnancy. It was also significantly associated with lower incidences of gestational diabetes and hypertension, and cesarean section. Correlation between differential BMI and monthly consumption of fish resulted in statistical significance, especially in type B fish consumers. Conclusions: The consumption of type B fish was significantly associated with increased neonatal weight and head circumference and better maternal outcomes.

Abstract

The cellular mechanisms underlying axonal morphogenesis are essential to the formation of functional neuronal networks. We previously identified the autism-linked kinase NUAK1 as a central regulator of axon branching through the control of mitochondria trafficking. However, (1) the relationship between mitochondrial position, function and axon branching and (2) the downstream effectors whereby NUAK1 regulates axon branching remain unknown. Here, we report that mitochondria recruitment to synaptic boutons supports collateral branches stabilization rather than formation in mouse cortical neurons. NUAK1 deficiency significantly impairs mitochondrial metabolism and axonal ATP concentration, and upregulation of mitochondrial function is sufficient to rescue axonal branching in NUAK1 null neurons in vitro and in vivo. Finally, we found that NUAK1 regulates axon branching through the mitochondria-targeted microprotein BRAWNIN. Our results demonstrate that NUAK1 exerts a dual function during axon branching through its ability to control mitochondrial distribution and metabolic activity., (© 2024. The Author(s).)

47. Willingness to Pay a Higher Price for Pork Obtained Using Animal-Friendly Raising Techniques: A Consumers' Opinion Survey.

Abstract

In Italy, the consumption of pork meat is increasing, despite consumers' attitudes being addressed toward a greater sensitivity about animal welfare and its link with safe food. Considering the relatively high animal welfare standards and the divergence in public interest in farm animal welfare and ethical issues, the objective of this study-in continuation of our previous paper relating to consumer behavior and preferences in welfare-friendly pork breeding-was to investigate habits of pork consumers regarding pig welfare, principally evaluating their willingness to pay (WTP) a higher price for pork obtained using raising techniques with an approach based on animal welfare. An ad hoc questionnaire-based survey was submitted to consumers (n = 404) in Messina province, Italy. Results suggest that 47% of consumers were willing to pay an additional price for pork from farms that apply specific animal welfare standards. Positive correlations were between WTP and farming techniques ( p = 0.001), organic farming methods ( p = 0.001), and farms in which animal welfare is taken care of and guaranteed ( p < 0.001). These findings suggest that consumers intend to pay a higher price for pork, like other animal products obtained using animal-friendly raising techniques. The sensitivity to the animal welfare of a single human being may influence consumers' attitudes toward pork consumption.

Abstract

This study investigates consumer behavior and interest in "welfare-friendly" forms of pork production, considering the growing presence of animal welfare-focused breeding practices. The aim is to outline the typical profile of pork consumers and identify the key attributes influencing their purchasing decisions. A survey was conducted on a sample of 286 individuals after excluding those who only consumed beef and/or poultry or identified as vegetarians/vegans. Regression coefficients (b), 95% Confidence Intervals (95% C.I.), and p -values were reported for univariate and multivariate models. Statistical significance was determined at p < 0.05 (indicated in bold). The findings indicate that younger participants show greater sensitivity towards consuming meat raised using welfare-friendly methods, raising considerations about the age composition of the sample. The research's originality lies in evaluating consumer interest in pork raised with animal welfare-respecting techniques. The use of appropriate statistical tools, such as multivariate and multilayer models, allows effective solutions for multidimensional hypothesis testing problems in non-parametric permutation inference.

49. Mitochondrial Bcl-xL promotes brain synaptogenesis by controlling non-lethal caspase activation.

Abstract

Non-lethal caspase activation (NLCA) has been linked to neurodevelopmental processes. However, how neurons control NLCA remains elusive. Here, we focused on Bcl-xL, a Bcl-2 homolog regulating caspase activation through the mitochondria. We generated a mouse model, referred to as ER-xL, in which Bcl-xL is absent in the mitochondria, yet present in the endoplasmic reticulum. Unlike bclx knockout mice that died at E13.5, ER-xL mice survived embryonic development but died post-partum because of altered feeding behavior. Enhanced caspase-3 activity was observed in the brain and the spinal cord white matter, but not the gray matter. No increase in cell death was observed in ER-xL cortical neurons, suggesting that the observed caspase-3 activation was apoptosis-independent. ER-xL neurons displayed increased caspase-3 activity in the neurites, resulting in impaired axon arborescence and synaptogenesis. 50. Non-Functional Jaw Muscular Activity in Patients with Disorders of Consciousness Revealed by A Long-Lasting Polygraphy.

Abstract

The presence of involuntary, non-functional jaw muscle activity (NFJMA) has not yet been assessed in patients with disorders of consciousness (DOC), although the presence of bruxism and other forms of movement disorders involving facial muscles is probably more frequent than believed. In this work, we evaluated twenty-two prolonged or chronic DOC patients with a long-lasting polygraphic recording to verify NFJMA occurrence and assess its neurophysiological patterns in this group of patients. A total of 5 out of 22 patients showed the presence of significant NFJMA with electromyographic patterns similar to what can be observed in non-DOC patients with bruxism, thus suggesting a disinhibition of masticatory motor nuclei from the cortical control. On the other hand, in two DOC patients, electromyographic patterns advised for the presence of myorhythmia, thus suggesting a brainstem/diencephalic involvement. Functional, non-invasive tools such as long-lasting polygraphic recordings should be extended to a larger sample of patients, since they are increasingly important in revealing disorders potentially severe and impacting the quality of life of DOC patients.
