BackgroundThe World Health Organization (WHO) and the Food and Agriculture Organization (FAO) recommend the consumption of less than 2,000 mg of sodium/day to reduce blood pressure and the risk of conditions such as cardiovascular disease and coronary heart disease. The sodium intake among Vietnamese was reported to be 7,200 mg/d or more. Free L-glutamate enhances flavor when it is added to food and improves the taste of sodium-reduced foods.ObjectiveThis study aims to investigate whether the intake of free L-glutamate-rich seasonings contributes to maintaining a low sodium intake in a cross-over study.MethodsFrom a total of 145 subjects, 42 participants were screened for participation in the cross-over design study. Subjects were randomly allocated to the Low free L-glutamate group (Low free L-Gl) and the Normal free L-glutamate group (Normal free L-Gl). Both received a direct educational guideline to reduce sodium intake. The Low free L-Gl group started with a restriction in the variety of free L-glutamate-rich seasonings, and the Normal free L-Gl group had no restriction in the variety of seasonings. Blood pressure was measured at week 0 (baseline), week 2, week 4, and week 6, while body weight, height, urine sodium and potassium excretion, chromogranin-A (CgA pmol/mg protein) from saliva, and free L-glutamate from food were measured at week 0, week 3, and week 6.ResultsIn Low free L-Gl, the amount of free L-glutamate in food decreased significantly from baseline to week 6 (p 0.05). In this purpose, the phenomenon is controlled through the specific constructive form that facilitates the conversion of solar energy, both in thermal energy as well as in electrical energy and because the system involves thermo-mechanical actuators. 5. Epicardial connection(s) inside the pulmonary vein antrum isolation lines contribute to subsequent atrial fibrillation: a case report.

- Abstract
Aims Recent reports have demonstrated that ∼10% of patients with atrial fibrillation (AF) have epicardial connections (ECs) inside the pulmonary vein (PV) antrum isolation (AI) (PVAI) lines, which could be a possible mechanism of failure to achieve a complete PVAI, thus contributing to AF recurrence. Case summary We present an 80-year-old female case with consistently continuing AF associated with ECs. Epicardial connections involving the PVs were detected in the left and right superior PVs after the completion of the PVAI. The AF steadily terminated after the ablation of those ECs. She has remained well without any AF or symptoms for 2 years post-ablation. Discussion Those ECs might have played a possible important role in initiating and maintaining the AF. The mechanism(s) of the ECs may be a cornerstone of the failure to achieve a complete PVAI contributing to AF recurrence. Ablation of the EC(s) in addition to the PVAI may be better able to achieve the completion of the PVAI. Thus, physicians should be aware of the possibility of the presence of EC(s) when performing ablation of AF, even though complete PVAI lines have been achieved. [ABSTRACT FROM AUTHOR]

6. The Potential of an Inexpensive Plant-Based Medium for Halal and Vegetarian Starter Culture Preparation.

The restrictions for halal and vegetarian fermented products apply not only to the food ingredients, but also to the inoculum media. The utilization of a medium for lactic acid bacteria (LAB) leads to some issues from animal-derived proteins sources that may be doubtful for halal and/or vegetarian use. This study aimed to develop a plant-based medium for culturing and maintaining LAB. The result demonstrated that 10 g/L soybean powder in sweet potato extract was suitable for cultivating Lactiplantibacillus plantarum TISTR 2075 with no significant difference (p < 0.05) from MRS (de Man, Rogosa and Sharpe) in the cell number (9.12 log CFU/mL) and specific growth rate (0.04). The feasibility of a plant-based medium to grow and maintain the LAB strains from different origins was evaluated. Compared to MRS, Lpb. plantarum TISTR 2075, Lpb. plantarum MW3, and Lacticaseibacillus casei TISTR 1463 could grow almost as well in a plant-based medium. This medium was also suitable for maintaining the viability of LAB during storage, especially when subjected to slant agar stock culture. It is practical and costs at least 10 times less than MRS. Thus, this study created a low-cost plant-based medium that could be used in laboratories, especially for applications in halal and vegetarian food products. [ABSTRACT FROM AUTHOR]

Abstract Background Chiropractic maintenance care (MC) has been found to be effective for patients classified as dysfunctional by the West Haven-Yale Multidimensional Pain Inventory (MPI). Although displaying good psychometric properties, the instrument was not designed to be used in clinical practice to screen patients for stratified care pathways. The aim was to develop a brief clinical instrument with the intent of identifying dysfunctional patients with acceptable diagnostic accuracy. Methods Data from 249 patients with a complete MPI dataset from a randomized clinical trial that investigated the effect and cost-effectiveness of MC with a 12-month follow-up was used in this cross-sectional analysis. A brief screening instrument was developed to identify dysfunctional patients, with a summary measure. Different cut-offs were considered with regards to diagnostic accuracy using the original instrument’s classification of dysfunctional patients as a reference. Very good diagnostic accuracy was defined as an area under the curve (AUC) metric between 0.8 and 0.9. The instrument was then externally validated in 3 other existing datasets to assess model transportability across populations and medical settings. Results Using an explorative approach, the MAINTAIN instrument with 10 questions (0–6 Likert responses) capturing 5 dimensions (pain severity, interference, life control, affective distress, and support) was developed, generating an algorithm-based score ranging from − 12 to 48. Reporting a MAINTAIN score of 18 or higher, 146 out of the 249 patients were classified as dysfunctional with 95.8% sensitivity and 64.3% specificity. At a score of 22 or higher, 109/249 were classified as dysfunctional with 81.1% sensitivity and 79.2% specificity. AUC was estimated to 0.87 (95% CI 0.83, 0.92) and Youden’s index was highest (0.70) at a score of 20. The diagnostic accuracy was similar and high across populations with minor differences in optimal thresholds for identifying dysfunctional individuals. Conclusion The MAINTAIN instrument has very good diagnostic accuracy with regards to identifying dysfunctional patients and may be used as a decision aid in clinical practice. By using 2 thresholds, patients can be categorized into “low probability (− 12 to 17)”, “moderate probability (18 to 21)”, and “high probability (22 to 48)” of having a good outcome from maintenance care for low back pain. The ex situ conservation of plant genetic resources in seed genebank plays an important role for food security in the future. The National Genebank of Bulgaria is located on the territory of the Institute of Plant Genetic Resources “K. Malkov” - Sadovo. It was built in 1984 and carry out a scientific programme for long-term preservation of germplasm with seeds under controlled conditions in accordance with the standards, developed by FAO. The total number of registered accessions in the Genebank is above 60 000, where 44572 are preserved in the base collection. These accessions are representatives of 32 families, 150 genera, 600 plant species. Total 10 193 of the accessions originated from Bulgaria. The accessions received abroad originated from more than 80 countries. Germplasm material consists of: local population, old cultivars, advanced varieties, breeding lines. The genetic stocks are available for research and for plant breeding, further utilisation by international and regional co-operation, free exchange of plant genetic resources according to CBD and conditions contained in the standard material transfer agreement. The death penalty in Indonesia is still maintained to combat crime within the Indonesian criminal law reform. Although many states have it removed, Indonesia would have the sole discretion to keep it. Despite the opposition, the death penalty application still has a juridical and sociological basis, so it is still legal to be maintained. Indonesia itself has overshadowed the death penalty with a form of legality according to international law so that the position of Indonesia that still maintain the death penalty cannot be blamed. Moreover, sociologically, Indonesian people still accept those who commit an offence who may have profound implications that could lead to the death penalty. This issue is what became the basis for lawmakers in Indonesia to keep it. This research uses the doctrinal method toexamine various regulations regarding capital punishment and non-doctrinal to understand the community's situation related to the existence of capital punishment in Indonesia. The death penalty is a more effective deterrent and therefore prevents crime better. With the death penalty, others were about to commit a similar crime is expected not to commit the crime. The death penalty is more effectively immobilizing offenders. Perpetrators, in principle, still manage to have the desire to commit the crime again after release. The death penalty for perpetrators of crimes is not a violation of human rights, but rather to respect human rights itself, namely for victims of crime. The setting and the application of the death penalty in Indonesia until now is still needed. They are considering that there are still many crimes that undermine humanity's values or the crimes that harm the State and crackdown on corruption in society.

Abstract Introduction Ixekizumab, a high-affinity monoclonal antibody that selectively targets interleukin-17A, is approved for treatment of moderate-to-severe plaque psoriasis. Our objective was to evaluate the long-term efficacy and safety of ixekizumab in moderate-to-severe plaque psoriasis through 5 years. Methods Data were integrated from the UNCOVER-1 and UNCOVER-2, randomized, double-blinded, phase-3 trials. Patients who continuously received the labeled ixekizumab dose, were static Physician’s Global Assessment (sPGA) (0,1) responders at Week 12 and completed 60 weeks of treatment could enter the long-term extension (LTE) period. Patients could escalate to every-2-week dosing per investigator opinion. Efficacy and health outcomes included proportion of patients achieving Psoriasis Area and Severity Index (PASI) 75/90/100, sPGA (0,1) and (0), absolute PASI ≤ 5/ ≤ 3/ ≤ 2/ ≤ 1 and Dermatology Life Quality Index (DLQI) (0,1). Results exclude patients who escalated to every-2-week dosing. A modified non-responder imputation method was used to account for missing data. Supplemental analyses include patients who escalated to every-2-week dosing and observed and multiple imputation results. Exposure-adjusted safety outcomes are also reported. Results Of 206 patients who entered the LTE periods, 172 completed treatment. At Week 60, PASI 75/90/100 responses were 94.7%, 85.0% and 62.1%, respectively, and at year 5 were 90.3%, 71.3% and 46.3%, respectively. Similarly, meaningful responses were achieved for the other efficacy and health measures. Among patients with PASI 100 through 5 years, 92% achieved DLQI (0,1), indicating no impact of skin disease on quality of life. During the LTE period, exposure-adjusted incidence rates were 31.4 per 100 patient-years for treatment-emergent adverse events and 6.8 per 100 patient-years for serious adverse events. No deaths were reported. No new or unexpected safety findings were noted. Conclusions The results demonstrate 80 mg ixekizumab maintains long-term efficacy and a safety profile consistent with previous data in patients with moderate-to-severe plaque psoriasis through 5 years of treatment. Supplemental analyses include patients who escalated to every-2-week dosing and observed and multiple imputation results. Exposure-adjusted safety outcomes are also reported. Results: Of 206 patients who entered the LTE periods, 172 completed treatment. At Week 60, PASI 75/90/100 responses were 94.7%, 85.0% and 62.1%, respectively, and at year 5 were 90.3%, 71.3% and 46.3%, respectively. Similarly, meaningful responses were achieved for the other efficacy and health measures. Among patients with PASI 100 through 5 years, 92% achieved DLQI (0,1), indicating no impact of skin disease on quality of life. During the LTE period, exposure-adjusted incidence rates were 31.4 per 100 patient-years for treatment-emergent adverse events and 6.8 per 100 patient-years for serious adverse events. No deaths were reported. No new or unexpected safety findings were noted. Conclusions: The results demonstrate 80 mg ixekizumab maintains long-term efficacy and a safety profile consistent with previous data in patients with moderate-to-severe plaque psoriasis through 5 years of treatment. Compressed medical air1 must meet certain criteria defined by European standard EN 120212, it shouldn't have any flavours or smell (odour). During filling bottle of breathing apparatus cylinders, it should be taken into account that vehicle exhaust gases, vapours of hazardous liquids, smoke and other harmful substances are not in the vicinity of the suction pipe cause they may get inside the cylinder of breathing apparatus and with their presence disrupt the microclimate (strictly defined parameters of medical air) and directly endanger the user. This isn't the case with cylinders which are the subject of this research. However, with physical-chemical and microbiological results samples of CMA, it can be said with responsibility that the user of the bottle / cylinder for the isulation devices did not comply the above when charging them with the CMA nor did it replace the filters of the high-pressure compressor in accordance with the manufacturer's instructions. This directly threatened his own health, but also the health of other users. Irresponsibility in maintain compressors of high-pressure, brought to creation of gram-positive bacterias and mildews as well as strain of pathogenic bacterias in CMA inside of the bottle/cylinder. Bakgrund: Hjärtsvikt är en kronisk folksjukdom som globalt drabbar cirka 26 miljoner årligen med stigande incidens. Sjukdomens symptom påverkar patientens förmåga till att upprätthålla sin egenvård som resulterar i försämrad livskvalitet. Egenvård är en stor och viktig del av patientens behandling och vardag. Sjuksköterskans uppgift är att informera och stötta för att främja patientens egenvård. Därför är det relevant att sjuksköterskan har kunskap om patientens upplevelse av främjande egenvård. Syfte: Syftet var att beskriva hjärtsviktspatienters upplevelser av vad som främjar egenvård. Metod: En litteraturöversikt med kvalitativ design och induktiv ansats genomfördes. Tolv vetenskapliga artiklar analyserades med Fribergs fem steg. Resultat: Fem subteman formade två teman. Att ha socialt nätverk och att hitta motivation formade stöd och motivation. Att känna engagemang till behandling, att ha tillräckligt med information och att ha tillgång till hjälpmedel formade sjukdomsinsikt. Slutsats: Resultatet visade att det fanns främjande upplevelser av egenvård hos patienter. Stöd och motivation var centrala komponenter för att underhålla egenvård. För att främja egenvård framkom det också att sjukdomsinsikt hade en betydande roll. Engagemang till behandling, information och symtomövervakning med diverse hjälpmedel bidrog till ökad sjukdomsinsikt och goda upplevelser av egenvård., Background: Heart failure is a chronic disease that globally affecting approximately 26 million yearly with increasing incidence. The symptoms of the disease affects the patient's ability to maintain their self-care, resulting in a reduced life quality. Self-care is a large and important part of the patient’s treatment and everyday life. The nurse’s job is to inform and support the patient to promote self-care. Therefore, it is relevant that the nurse has knowledge about the patient’s experience of self-care. Aim: The aim was to describe heart failure patients' experiences of what promotes self-care. Method: A literature review with qualitative design and inductive approach was conducted. Twelve scientific articles were analyzed with Friberg’s five-step. Results: Five subthemes formed two themes. To have social network and to find motivation, formed support and motivation. To feel committed to treatment, to have enough information and to have access to devices, formed disease awareness. Conclusion: The results showed that there were promoting experiences of self-care in patients. Support and motivation were central components for maintaining self-care. To promote self-care, it were also found that disease awareness played an important role. Commitment to treatment, information and monitoring of symptoms with different devices contributed to increased disease awareness and good experiences of self-care.

Unlike most other organism groups, plant species can possess dormant life‐history stages such as soil seed banks, which may help plant communities to resist or at least postpone the detrimental impact of global changes. This study investigates the potential for soil seed banks to achieve this.\ud \ud \ud \ud Location\ud \ud Europe.\ud \ud \ud \ud Time period\ud \ud 1978–2014.\ud \ud \ud \ud Major taxa studied\ud \ud Flowering plants.\ud \ud \ud \ud Methods\ud \ud Using a space‐for‐time/warming approach, we study plant species richness and composition in the herb layer and the soil seed bank in 2,796 community plots from 54 datasets in managed grasslands, forests and intermediate, successional habitats across a climate gradient.\ud \ud \ud \ud Results\ud \ud Soil seed banks held more species than the herb layer, being compositionally similar across habitats. Species richness was lower in forests and successional habitats compared to grasslands, with annual temperature range more important than mean annual temperature for determining richness. Climate and land‐use effects were generally less pronounced when plant community richness included seed bank species richness, while there was no clear effect of land use and climate on compositional similarity between the seed bank and the herb layer.\ud \ud \ud \ud Main conclusions\ud \ud High seed bank diversity and compositional similarity between the herb layer and seed bank plant communities may provide a potentially important functional buffer against the impact of ongoing environmental changes on plant communities. This capacity could, however, be threatened by climate warming. Dormant life‐history stages can therefore be important sources of diversity in changing environments, potentially underpinning already observed time‐lags in plant community responses to global change. However, as soil seed banks themselves appear, albeit less, vulnerable to the same changes, their potential to buffer change can only be temporary, and major community shifts may still be expected.

Introduction Ixekizumab, a high-affinity monoclonal antibody that selectively targets interleukin-17A, is approved for treatment of moderate-to-severe plaque psoriasis. Our objective was to evaluate the long-term efficacy and safety of ixekizumab in moderate-to-severe plaque psoriasis through 5 years. Methods Data were integrated from the UNCOVER-1 and UNCOVER-2, randomized, double-blinded, phase-3 trials. Patients who continuously received the labeled ixekizumab dose, were static Physician’s Global Assessment (sPGA) (0,1) responders at Week 12 and completed 60 weeks of treatment could enter the long-term extension (LTE) period. Patients could escalate to every-2-week dosing per investigator opinion. Efficacy and health outcomes included proportion of patients achieving Psoriasis Area and Severity Index (PASI) 75/90/100, sPGA (0,1) and (0), absolute PASI ≤ 5/ ≤ 3/ ≤ 2/ ≤ 1 and Dermatology Life Quality Index (DLQI) (0,1). Results exclude patients who escalated to every-2-week dosing. A modified non-responder imputation method was used to account for missing data. Polyploidy, resulting from the duplication of an organism’s or a cell’s entire genome, greatly affects genes and genomes, cells and tissues, organisms and even entire ecosystems. Despite the wide-reaching importance of polyploidy, communication across disciplinary boundaries to identify common themes at different scales has been almost non-existent. However, a critical need remains to understand commonalities that derive from shared polyploid cellular processes across organismal diversity, levels of biological organization, and fields of inquiry – from biodiversity and biocomplexity to medicine and agriculture. Here, we review the current understanding of polyploidy at the organismal and sub-organismal levels, identify shared research themes and elements, and propose new directions to integrate research on polyploidy toward confronting interdisciplinary grand challenges of the 21(st) century.

Bu çalışmada Türkiye'de turizm yoğun olarak bulunduğu Nevşehir ilinde faaliyet gösteren otel ve restoran işletmelerinde ki üst düzey çalışanlara anket uygulanmıştır. Katılımcılara, anket formunda performansı etkileyen yenilik, süreklilik ve esneklik ile ilgili sorular yöneltilmiştir. Cevapları analiz etmek için parametrik testlerden "t" testi ve ANOVA testi kullanılmıştır. Analiz sonuçlarına göre konaklama ve yiyecek-içecek işletmelerinde ki yöneticilerin demografik özelliklerine göre işletme performansı algılarında farklılık olduğu belirlenmiştir. Today, performance analysis of companies operating under intense competition conditions is very important. Performance analyzes are usually carried out periodically, and in some cases performance analysis can be performed depending on performance perception. The main purpose of working in this context is to measure performance perceptions of hotel and restaurant businesses. The study, which operates in Turkey's Cappadocia region as a busy tourist hotel which is built on and senior employees in the restaurant business. Participants were directed to expressing perceptions of innovation, continuity, and flexibility that affect performance. The "t" test and the ANOVA test were used to analyze the answers. According to the results of the analysis, it was determined that there is a difference in the perception of the management performance according to the demographic characteristics of the managers operating in the accommodation and catering businesses.

Current treatment of lupus nephritis consists of both induction and maintenance therapy, with the latter being designed to consolidate remissions and prevent relapses. Long-term maintenance treatment with intravenous cyclophosphamide was effective but associated with considerable toxicity. A small but well-designed controlled trial found that for post-induction maintenance therapy, both oral mycophenolate mofetil (MMF) and oral azathioprine were superior in efficacy and had reduced toxicity than a regimen of continued every third month intravenous cyclophosphamide. Although these oral agents were rapidly accepted and utilized as maintenance medications, their usage was based on scant evidence and there were no comparisons between the two. Recently, two relatively large, randomized, well-controlled, multicenter trials dealing with maintenance therapy for severe lupus nephritis have been completed. The Aspreva Lupus Management Study (ALMS) maintenance and MAINTAIN nephritis trials provide important information regarding the comparative efficacy and safety of MMF and azathioprine as maintenance therapies, as well as information on the effect of dosage and duration of treatment with these agents. En av de dimensionerande faktorerna vid underhåll av asfaltsvägar är spårdjup, spårdjupen får inte överstiga den specifika underhållsstandarden som finns eftersom om det sker ökar olycksrisken och en försämrad framkomlighet kan uppstå. Ett av syftena med detta examensarbete är att undersöka om det går att förutse var och varför asfaltsbeläggningar inte uppfyller underhållsstandarden som finns. Metoden som används grundar sig i en analys av mätdata som inhämtats genom laserskanning av vägytan med hjälp av en mätbil. Datan presenteras i form av grafer och figurer i Trafikverkets öppna webbportal PMSV3, graferna och figurerna har analyserats för att sedan utgöra resultatet och slutsatserna i arbetet. Resultatet som framgick stämmer som helhet överens med grundinställningen innan arbetet påbörjades, att det går att se en indikation efter några år vart beläggningen kommer att överstiga maximalt spårdjup enligt underhållsstandarden. One of the dimension factors for maintenance of asphalt roads is rut depth, the groove depth must not exceed the specific maintenance standard that exists because, if this happens, the accident risk increases One of the purposes of this thesis is to investigate whether it is possible to predict where and why asphalt pavements do not meet the maintenance standard that exists. The result which appeared to be in agreement with the basic attitude before the work was started, that it is possible to see an indication after a few years where the occupancy will exceed the maintenance standard But to predict if and where the asphalt coating will deteriorate first and most is more difficult at an early stage The work's conclusions have contributed to several ideas for further work being proposed, which makes the work to be considered successful
