Abstract

BackgroundThis first mixed-methods UK trial examined the feasibility and acceptability of a future definitive randomised controlled trial (RCT) to evaluate whether Family Focussed Treatment for Adolescents with Bipolar Disorder (FFT-A) UK version can improve family functioning and well-being as part of the management of Paediatric Bipolar Disorder (PBD).MethodThe trial used a randomised, parallel group, non-blinded design where participants received FFT-A UK (16 sessions over 6 months) immediately or after 12 months (delayed arm). Measures of family functioning, well-being and quality of life of the young person and the main carer (most commonly a parent) were completed at baseline, 6 and 12-months in both arms. Primary outcome measures included rates of eligibility, consent and retention along with estimates of variability in the measures and assessment of the intervention delivery. Qualitative interviews allowed assessment of participants' views about FFT-A and the trial processes.ResultsTwenty-seven of 36 young persons with PBD and their families consented; of these, 14 families were randomised to the immediate and 13 to the delayed arm. Two families from the immediate arm withdrew consent and discontinued participation. Quantitative measures were completed by 22 families (88%) at 6-months and 21 families (84%) at 12-months. Qualitative interviews were conducted with 30 participants (9 young people, 15 parents and 6 other family members). Nine families attended 3 post-trial focus groups.ConclusionIt was feasible to recruit and retain to this trial. The results highlighted that trial design and measures were acceptable to participants. A benefit in family relationships was reported by participants which they attributed to the intervention in qualitative interviews. Families recommended that future modifications include definitive trial(s) recruiting participants in the age range 15-25 years as it felt this was the age range with maximum need. 2. Monitoring contaminants of emerging concern in aquatic systems through the lens of citizen science

Abstract

Global urbanization trends have led to the widespread increasing occurrence of contaminants of emerging concern (CECs) such as pharmaceuticals, personal care products, pesticides, and micro- and nano-plastics in aquatic systems. Even at low concentrations, these contaminants pose a threat to aquatic ecosystems. To better understand the effects of CECs on aquatic ecosystems, it is important to measure concentrations of these contaminants present in these systems. Currently, there is an imbalance in CEC monitoring, with more attention to some categories of CECs, and a lack of data about environmental concentrations of other types of CECs. Citizen science is a potential tool for improving CEC monitoring and to establish their environmental concentrations. However, incorporating citizen participation in the monitoring of CECs poses some challenges and questions. In this literature review, we explore the landscape of citizen science and community science projects which monitor different groups of CECs in freshwater and marine ecosystems. We also identify the benefits and drawbacks of using citizen science to monitor CECs to provide recommendations for sampling and analytical methods. Our results highlight an existing disparity in frequency of monitoring different groups of CECs with implementing citizen science. Specifically, volunteer participation in microplastic monitoring programs is higher than volunteer participation in pharmaceutical, pesticide, and personal care product programs. These differences, however, do not necessarily imply that fewer sampling and analytical methods are available. Finally, our proposed roadmap provides guidance on which methods can be used to improve monitoring of all groups of CECs through citizen science.

Abstract

The advent of immune-checkpoint inhibitors (ICI) in modern oncology has significantly improved survival in several cancer settings. A subgroup of women with breast cancer (BC) has immunogenic infiltration of lymphocytes with expression of programmed death-ligand 1 (PD-L1). These patients may potentially benefit from ICI targeting the programmed death 1 (PD-1)/PD-L1 signaling axis. The use of tumor-infiltrating lymphocytes (TILs) as predictive and prognostic biomarkers has been under intense examination. Emerging data suggest that TILs are associated with response to both cytotoxic treatments and immunotherapy, particularly for patients with triple-negative BC. In this review from The International Immuno-Oncology Biomarker Working Group, we discuss (a) the biological understanding of TILs, (b) their analytical and clinical validity and efforts toward the clinical utility in BC, and (c) the current status of PD-L1 and TIL testing across different continents, including experiences from low-to-middle-income countries, incorporating also the view of a patient advocate. 10. Screening tools for common mental disorders in older adults in South Asia: a systematic scoping review

Abstract

Objectives:Common mental disorders (CMDs), particularly depression, are major contributors to the global mental health burden. South Asia, while diverse, has cultural, social, and economic challenges, which are common across the region, not least an aging population. This creates an imperative to better understand how CMD affects older people in this context, which relies on valid and culturally appropriate screening and research tools. This review aims to scope the availability of CMD screening tools for older people in South Asia. As a secondary aim, this review will summarize the use of these tools in epidemiology, and the extent to which they have been validated or adapted for this population.Design:A scoping review was performed, following PRISMA guidelines. The search strategy was developed iteratively in Medline and translated to Embase, PsychInfo, Scopus, and Web of Science. Data were extracted from papers in which a tool was used to identify CMD in a South Asian older population (50+), including validation, adaptation, and use in epidemiology. Validation studies meeting the criteria were critically appraised using the Quality Assessment of Diagnostic Accuracy Studies – version 2 (QUADAS-2) tool.Results:Of the 4694 papers identified, 176 met the selection criteria at full-text screening as relevant examples of diagnostic or screening tool use. There were 15 tool validation studies, which were critically appraised. Of these, 10 were appropriate to evaluate as diagnostic tests. All of these tools assessed for depression. Geriatric Depression Scale (GDS)-based tools were predominant with variable diagnostic accuracy across different settings. Methodological issues were substantial based on the QUADAS-2 criteria. In the epidemiological studies identified (n = 160), depression alone was assessed for 82% of the studies. Tools lacking cultural validation were commonly used (43%).Conclusions:This review identifies a number of current research gaps including a need for culturally relevant validation studies, and attention to other CMDs such as anxiety.

Abstract

Background: TNBC, an aggressive subtype of breast carcinoma, has limited treatment options. Improved progression free and overall survival has been observed in patients with unresectable locally advanced and metastatic TNBC (NCT02425891) with atezolizumab and nab-paclitaxel in PD-L1-positive [>=1% on immune cells using VENTANA SP142 PD-L1 IHC]. Although this predictive marker is pivotal for treatment decision making, there are limited data on inter- and intra-pathologist (P) agreement for SP142 PD-L1 scoring. We investigated reproducibility of SP142 IHC in TNBC amongst Ps trained in SP142 scoring and the effect of training on accuracy of assessment by Ps untrained in PD-L1. Method(s): Previously untreated, resected primary invasive TNBCs were collected from 4 centers. SP142 PD-L1 IHC was performed on tissue microarrays. 60 cases, selected for PD-L1 expression close to 1% cut-off, were dichotomized as negative (<1%) or positive (>=1%). Intra- and inter-observer reproducibility assessment involved 10 Ps, conducted over 2 days (D). Five Ps were previously trained in SP142 IHC (subgroup - SG1) and 5 were SP142 assessment naive (SG2). SG2 received 1 hour of training in SP142 IHC prior to scoring on D2; pre- and post-training scores were compared. Intra-observer reproducibility was evaluated by comparing the scores obtained by SG1 on D1 and D2. Inter-observer reproducibility compared scores between SG1 on D1. Result(s): High intra-observer agreement was demonstrated between SG1; overall percent agreement (OPA) 95.0% (95% CI 91.9- 97.0), average positive agreement (APA) 95.2% (95% CI 92.2 - 97.0), average negative agreement (ANA) 94.9% (95% CI 91.7 - 96.9), Cohen's kappa co-efficient (K) = 0.90. Inter-observer agreement was also high; OPA 93.3% (95% CI 91.1 - 95.1), APA 93.7% (95% CI 91.5 - 95.3), ANA 93.0% (95% CI 90.6 - 94.8), K = 0.87. 21. Randomized controlled trial of teaching practice nurses to carry out structured assessments of patients receiving depot antipsychotic injections

Abstract

BACKGROUND: A third of patients with schizophrenia are out of contact with secondary services. Many of these patients receive maintenance medication as depot antipsychotics from practice nurses, most of whom have negligible training in mental health. AIM: To examine the impact of a structured assessment on the process of care and clinical status of schizophrenia patients by practice nurses who received a one-day training course. METHOD: All identified patients were randomly allocated to structured assessments and outcome, measured by the number of assessments and the changes in care recorded in primary care notes. A comprehensive assessment of clinical and social functioning and level of unmet need in intervention and control patients was carried out after one year by an independent researcher. RESULTS: A high rate of consultation and clinical need in this patient group was demonstrated. Practice nurses were more diligent in carrying out assessments than general practitioners (GPs), but there was no impact on treatment patterns or clinical outcome. CONCLUSIONS: Structured assessments by practice nurses are feasible with this patient group, but training, targeted at both nurses and GPs, is needed if this intervention is to translate into health gain.

Abstract

BAG-1 (bcl-2-associated athanogene) enhances oestrogen receptor (ER) function and may influence outcome and response to endocrine therapy in breast cancer. We determined relationships between BAG-1 expression, molecular phenotype, response to tamoxifen therapy and outcome in a cohort of breast cancer patients and its influence on tamoxifen sensitivity in MCF-7 breast cancer cells in vitro. Publically available gene expression data sets were analysed to identify relationships between BAG-1 mRNA expression and patient outcome. BAG-1 protein expression was assessed using immunohistochemistry in 292 patients with invasive ductal carcinoma and correlated with clinicopathological variables, therapeutic response and disease outcome. BAG-1-overexpressing MCF-7 cells were treated with antioestrogens to assess its effects on cell proliferation. Gene expression data demonstrated a consistent association between high BAG-1 mRNA and improved survival. In ER+ cancer (n=189), a high nuclear BAG-1 expression independently predicted improved outcome for local recurrence (P=0.0464), distant metastases (P=0.0435), death from breast cancer (P=0.009, hazards ratio 0.29, 95% CI: 0.114-0.735) and improved outcome in tamoxifen-treated patients (n=107; P=0.0191). BAG-1 overexpression in MCF-7 cells augmented antioestrogen-induced growth arrest. A high BAG-1 expression predicts improved patient outcome in ER+ breast carcinoma. This may reflect both a better definition of the hormone-responsive phenotype and a concurrent increased sensitivity to tamoxifen. [ABSTRACT FROM AUTHOR]

Abstract

The aqueous solution chemistry of the trimethyllead(IV) species and the trimethyllead(IV) complexes of six carboxylic acids of pKavalues ranging from 2.75 to 4.95 has been investigated by proton magnetic resonance spectroscopy. Equilibrium constants for the reaction of (CH3)3Pb+with hydroxide ion to form (CH3)3PbOH and ((CH3)3Pb)2OH+, and the formation constants of the carboxylic acid complexes were determined from the pHdependence of the chemical shift of the methyl protons of trimethyllead. The formation constants of the complexes increase as the pKaof the ligand increases. The lead-207-proton coupling constant was found to be insensitive to complexation.

46. Thoracic suspension: quantitative effects upon seating pressure and posture

Abstract

Seating pressure in 10 subjects using a thoracic suspension orthosis was studied quantitatively. Anterior, posterior, and ischial pressures were measured bilaterally both unsuspended and during a 90 to 120 minute interval while suspended.Significant decreases in seating pressure were noted following suspension, with a mean decrease of 59.8%. Significant changes in lateral support were noted with suspension, as was a reduction in both the absolute pressure and relative distribution of pressure to the ischial areas. Relative redistribution of pressure in the anterior-posterior direction was related to leg support condition. All subjects who presented with pressure sores healed subsequent to thoracic suspension. Those with spinal curvature showed a 10 to 20 degree correction immediately following suspension. This correction generally was not maintained, however, in a temporal analysis. The data indicates that thoracic suspension can effectively reduce seating pressure and alter lateral posture, while leg support condition has a greater effect upon anterior-posterior pressure distribution. The use of a seat cushion to prevent subject swinging while suspended did not adversely affect the overall suspended pressures in this study, although the subjects without seat cushions showed lower overall suspended pressures.
