2. Complication of mandibular third molars extraction: a case report

Objective: the objective of this paper is to show through a clinical case that an adequate clinical and radiographic examination should be performed before third molar removal to avoid complications especially the displaced tooth.Methods: the authors present a case report of a submandibular displacement of a mandibular third molar during extraction.Results: after standard radiological examination and a computed tomography (CT-scan) surgery for retrieving the displaced tooth was performed under general anesthesia without difficulty.Conclusion: localization with CT-scan and proper surgical methods are the keys to retrieving the displaced tooth but we must to remind dentists on ways to prevent and manage this complication.

Introduction : le keratokyste odontogene (KKO) est une des tumeurs odontogenes les plus frequentes. Plusieurs travaux se sont interesses a l’etude de cette lesion depuis sa premiere description afin de pouvoir comprendre son comportement biologique.Materiel et methode : il s’agit d’une etude retrospective immunohistochimique des marqueurs PCNA, Ki67 et P53 chez 40 patients operes pour un KKO.Resultats : ces marqueurs sont retrouves dans les couches suprabasales de l’epithelium de la paroi kystique. Il n’y a pas de difference statistiquement significative dans l’expression de ces marqueurs entre les differentes formes cliniques (sporadique, recurrent et syndromique), et il n’existe pas de correlation entre ces marqueurs et la survenue d’une recidive.Conclusion : PCNA, Ki67 et P53 ne sont pas des marqueurs pour l’histopronostic des KKO.

4. Hémangiome de la fosse infra-temporale : à propos d’un cas

L’hemangiome caverneux touche la region cervico-faciale dans 20 % des cas. Neanmoins, la localisation infra-temporale reste rare. Les auteurs discutent les difficultes diagnostiques et therapeutiques a travers un cas d’hemangiome caverneux de la fosse infra-temporale gauche, d’evolution progressive, chez un homme de 31 ans. Le diagnostic de malformation vasculaire a ete evoque sur l’IRM. Le traitement a consiste, apres abord de la fosse infra-temporale gauche, a realiser l’exerese tumorale complete apres ligature vasculaire. L’histologie a confirme le diagnostic. L’abord de la fosse infra- temporale reste laborieux malgre la diversite des voies d’abord proposees dans la litterature.

5. Métastases mandibulaires : présentation de 8 cas

Objectif : Les metastases mandibulaires peuvent constituer la premiere manifestation de la dissemination d’un cancer primitif, posant alors un probleme diagnostique. Les auteurs se proposent d’etudier les aspects anatomocliniques, radiologiques et therapeutiques de ces metastases.Materiel et methodes : Il s’agit d’une etude retrospective concernant 8 cas de metastases mandibulaires colliges entre janvier 1993 et decembre 2006, dans le service de Chirurgie maxillo-faciale de l’Hopital Sahloul, Sousse, Tunisie.Resultats : Huit patients ont ete inclus dans l’etude (5 hommes et 3 femmes) dont l’âge varie de 12 a 92 ans. L’âge moyen est de 42 ans. L’atteinte mandibulaire a ete revelatrice de la tumeur primitive dans 3 cas. Dans les autres cas, c’est le bilan d’extension de la tumeur primitive qui a mis en evidence l’atteinte mandibulaire. Le cancer primitif etait un carcinome canalaire du sein, un seminome, un neuroblastome, un osteosarcome fibulaire, un carcinome bronchique a petites cellules, un sarcome d’Ewing femoral et deux synovialosarcomes. Seulement deux patients, un cas de synovialosarcome et un cas d’osteosarcome chez des sujets jeunes, ont pu etre operes. Les autres cas ont recu un traitement palliatif.Conclusion : Les tumeurs metastatiques de la mandibule sont rares. Leur aspect radiologique varie selon le type histologique de la tumeur primitive. Malgre les progres therapeutiques, le pronostic des tumeurs avec metastases reste reserve.

Frontal pneumosinus dilatans is a rare entity that is documented only by a few reports. It is an abnormal expansion of the aerated frontal sinus, with sinus walls of normal thickness. The expansion of the bone may be generalized or focal. It is not only an aesthetic problem but may also have functional consequences. We herein present the case of a 23-year-old woman who had been referred to the maxillofacial and plastic unit in Sousse (Tunisia) complaining of a prominence on her right supraorbital region and forehead. The cause was unknown and there were no functional problems. Computed tomography showed a large pneumatized frontal sinus and confirmed the resulting bony prominence. The patient was taken to the operating room, where the frontal and supraorbital regions were exposed through a bicoronal incision. The anterior wall of the right frontal sinus was removed, and was divided into segments that were then fixed in the desired position using a titanium mesh plate. Excellent results were obtained, and the patient had no complaints and was satisfied with her appearance. She remained well two years postoperatively. Pneumosinus dilatans is an entity every plastic surgeon should be aware of because its treatment falls within the realm of craniofacial surgery. The etiology remains unclear, and surgical management is directed toward surgical exploration of the sinus to ensure recontouring of the anterior table of the frontal sinus to correct any cosmetic deformity. Les osteomes sont des neoplasies benignes, caracterises par une proliferation de tissu osseux spongieux et/ou compact. Ils peuvent etre solitaires ou multiples. Les osteomes solitaires peripheriques interessent rarement les maxillaires, excepte le sinus. Seulement neuf cas d’osteome du proces alveolaire maxillaire ont ete rapportes dans la litterature. Ils se manifestent comme une tumefaction asymptomatique, ferme, sessile ou pediculee. L’imagerie oriente le diagnostic en montrant une image radio-opaque bien circonscrite. Nous rapportons le cas d’un osteome volumineux pedicule du proces alveolaire maxillaire.

Plusieurs tumeurs congenitales de la cavite buccale qui se developpent, entre les deux processus palatins, avant la sixieme semaine de gestation empechent l’elevation et l’horizontalisation de ceux-ci ainsi que leur fusion avec la cloison nasale. Il en resulte une fente palatine. Ces tumeurs, associees a la fente palatine, peuvent occasionner des troubles respiratoires et/ou des difficultes lors de l’alimentation. Deux cas sont presentes : l’un concerne un teratome du rhinopharynx, l’autre un hamartome de la langue; tous deux etaient associes a une fente palatine. Dans les deux cas, l’evolution a ete favorable apres exerese complete de la tumeur et fermeture de la fente.

Lymphoepithelial carcinoma of the salivary glands is a rare neoplasm that is characterized by a non-neoplastic lymphocytic infiltration associated with an epithelial proliferation. It involves mainly the parotid gland. Racial and geographical factors contribute to the pathogenesis of this tumor. We report a case of a 70-year old woman from a non-endemic area who presented with several months history of swelling in the parotid region. Magnetic resonance imaging showed a parotid mass suggestive of a pleomorphic adenoma. The diagnosis of lymphoepithelial carcinoma of the parotid gland was performed on the surgical specimen. A primitive nasopharyngeal carcinoma was ruled out by random biopsies of the nasopharynx mucosa. The Epstein-Barr virus (EBV) was absent in neoplastic cells. We insist that, even in non-endemic areas and when clinical and radiological characteristics are not suggestive of malignancy, intra-operative frozen section analysis should be used in order to ensure the appropriate treatment.
